Introduction: Health services rely upon carers to provide care for loved ones with cancer, yet many carers often feel ill‐prepared for this role. Despite a multitude of programmes to support carer mental health, programmes that help carers feel better equipped to support a person with cancer are lacking. This study aimed to address this need by adapting an evidence‐based intervention to be suitable for carers of people with cancer. Methods: This study used an exploratory, qualitative design consisting of experienced‐based co‐design and an in‐depth stakeholder engagement strategy. An existing evidence‐based programme to promote resilience in the context of providing care was adapted for relevance to carers for people with cancer via two co‐design workshops with carers and healthcare professionals (n = 8). The resulting prototype programme was refined based on stakeholder consultations with staff and consumer members of cancer and carer support organisations across Australia (n = 16). Transcripts of the workshops, meetings and written feedback from carers were thematically analysed. Results: Major programme developments were guided by three themes that emerged from the co‐design workshops: 'creating value for carers', 'multiple contributors to carer distress' and 'the need for flexible implementation'. Analysis of the stakeholder consultation data showed that the themes of 'diversity in carer journeys' and 'creating impact for carers' were key to further tailoring the programme for applicability to practice. An adapted programme called 'iCanSupport' resulted from the process, with key adaptations being more relevant case study scenarios for carers and greater flexibility in accessing and engaging with the intervention to accommodate a range of carer circumstances. Conclusion: Programmes to build skills for becoming a carer for someone with cancer are absent, yet they are desired by carers. Using co‐design provided a user‐centric approach to adapt an existing evidence‐based programme. Programme evaluation is required to determine the effectiveness of the co‐designed approach in improving carer preparedness among a range of cohorts. Background: Rates of burnout are currently at record high levels, and GPs experience higher burnout than many other specialties. Organisational interventions may reduce burnout, but few studies have investigated these in primary care. Aim: The current study investigated whether breaks, both with and without social interactions, were associated with burnout and patient safety perceptions in GPs. Design: A within-subjects, interval contingent, quantitative daily diary design. Setting: UK GP practices. Method: Participants completed questionnaires at baseline measuring demographic variables, burnout and patient safety perceptions. They then completed a questionnaire in the evening each day for a week which captured whether they had taken a break that day, whether it involved a positive social interaction, burnout (comprising subscales of disengagement and exhaustion), positive and negative affect and patient safety perceptions. The data were analysed using hierarchical linear modelling to assess same-day and next-day associations. Results: We included 241 responses from 58 GPs for analysis. Taking at least one break (involving any or no social interactions) was associated with lower disengagement that day and lower exhaustion the next day. Taking at least one break involving a positive interaction was associated with 1) lower disengagement, exhaustion, overall burnout and negative affect on the same day, as well as higher positive affect and improved perceptions of patient safety, and 2) lower exhaustion and improved patient safety perceptions on the next day. Conclusion: Organizing daily team or practice breaks where staff can socialise may help to reduce burnout and improve perceptions of patient safety. [ABSTRACT FROM AUTHOR]

Objectives This study aimed to investigate perceptions of what it means to "age well" and to explore similarities and differences between a Western and non-Western culture (Britain and Java). Methods Qualitative interviews explored how Javanese and British older adults defined aging well, establishing the similarities and differences between cultures. Javanese (n  = 14) and British (n  = 15) adults aged 61–80 (mean age = 68) participated. The data were analyzed using reflexive thematic analysis and organized with NVivo. Results Four themes were identified that captured what it means to age well across cultures: (a) good health is a springboard for aging well ; (b) holding a positive outlook is a decisive factor in aging well ; (c) "having enough" and "feeling safe" provide peace of mind ; and (d) spirituality and religiosity provide tranquility. Although both cultures mentioned similar factors, there were variations in the interpretations and emphasis within themes. For example, Javanese participants emphasized the importance of the social environment whereas British participants highlighted the physical environment. Discussion Differences between cultures are important for understanding how best to support people as they age. For example, in Java, aging well may be best supported by providing a vibrant social environment. For people in Britain, having a safe and secure physical environment may be more important. This paper examines the use and implications of dreams in the Bible, both Old Testament and New, from a transpersonal perspective. An initial examination of the biblical references to dreams, dreamers, and dreaming, generates four broad themes: 1) reference to a message received, or a report of the actual content of the dream; 2) reference to a person (i.e. the authority derived from receiving a dream or the ability to interpret); 3) the word 'dream' as a metaphor for something that is fleeting and unreliable; and 4) dreams as a tool of false prophets. It should be noted that a clear distinction between the first two themes is sometimes difficult to make: the actual text may be a reference to a dream, but the context carries implications about the person. In these instances an interpretive decision has been made, tending to include these references under the second theme.
