4. Outcomes from 306 consecutive robotic ventral hernia repairs in the community setting.

Abstract

Robotic assisted laparoscopy is increasingly popular for primary ventral and incisional hernia repair. A variety of robotic techniques have been described. More data is needed to evaluate the indications and benefits of these approaches. This is a retrospective analysis of patients who underwent robotic hernia repair over a 2-year period in the community setting. Techniques included: intraperitoneal onlay mesh repair with fascial defect closure (IPOM +), ventral transabdominal preperitoneal repair (vTAPP), and retrorectus (RR) repairs with or without transverse abdominis release (TAR). Follow up was obtained at minimum 6 months postoperatively. Outcomes data included complications, readmissions, reoperations, and hernia recurrences. This study included 306 consecutive robotic ventral hernia repairs: 63 IPOM + , 199 vTAPP, and 44 RR of which 25 (57%) required TAR. Average console times were similar between IPOM + and vTAPP groups (63 vs. 62 min, p = 0.71) and longer for RR repairs without vs. with TAR (107.8 vs. 184.4 min, p < 0.001). There were few intraoperative (0.6%) and immediate postoperative (0.3%) complications. Same day discharge was feasible in most patients (95%). Follow up was completed in 81.0% of patients with an average follow up time of 13.3 months (range 6–27 months). Rates of complications (1.3%), readmissions (1.0%), and reoperations (1.0%) were recorded. There were no hernia recurrences. Robotic ventral hernia repair is a safe and effective strategy for treatment of most abdominal wall hernias with low complication and recurrence rates in medium term follow up. Abstract

Vertebrates and tunicates are sister groups that share a common fusogenic factor, Myomaker (Mymk), that drives myoblast fusion and muscle multinucleation. Yet they are divergent in when and where they express Mymk. In vertebrates, all developing skeletal muscles express Mymk and are obligately multinucleated. In tunicates, Mymk is expressed only in post-metamorphic multinucleated muscles, but is absent from mononucleated larval muscles. In this study, we demonstrate that cis-regulatory sequence differences in the promoter region of Mymk underlie the different spatiotemporal patterns of its transcriptional activation in tunicates and vertebrates. Although in vertebrates myogenic regulatory factors (MRFs) such as MyoD1 alone are required and sufficient for Mymk transcription in all skeletal muscles, we show that transcription of Mymk in post-metamorphic muscles of the tunicate Ciona requires the combinatorial activity of MRF, MyoD and Early B-cell Factor (Ebf). This macroevolutionary difference appears to be encoded in cis, likely due to the presence of a putative Ebf-binding site adjacent to predicted MRF binding sites in the Ciona Mymk promoter. We further discuss how Mymk and myoblast fusion might have been regulated in the last common ancestor of tunicates and vertebrates, for which we propose two models. [ABSTRACT FROM AUTHOR]

Abstract

Tunicates are the sister group to the vertebrates, yet most species have a life cycle split between swimming larva and sedentary adult phases. During metamorphosis, larval neurons are replaced by adult-specific ones. The regulatory mechanisms underlying this replacement remain largely unknown. Using tissue-specific CRISPR/Cas9-mediated mutagenesis in the tunicate Ciona, we show that orthologs of conserved hindbrain and branchiomeric neuron regulatory factors Pax2/5/8 and Phox2 are required to specify the 'neck', a cellular compartment set aside in the larva to give rise to cranial motor neuron-like neurons post-metamorphosis. Using bulk and single-cell RNA-sequencing analyses, we characterize the transcriptome of the neck downstream of Pax2/5/8. We present evidence that neck-derived adult ciliomotor neurons begin to differentiate in the larva and persist through metamorphosis, contrary to the assumption that the adult nervous system is formed after settlement and the death of larval neurons during metamorphosis. Finally, we show that FGF signaling during the larval phase alters the patterning of the neck and its derivatives. Suppression of FGF converts neck cells into larval neurons that fail to survive metamorphosis, whereas prolonged FGF signaling promotes an adult neural stem cell-like fate. [ABSTRACT FROM AUTHOR]

Abstract

Ligand-protected gold nanoclusters (AuNCs) feature a dense but finite electronic structure that can be rationalized using qualitative descriptions such as the well-known superatomic model and predicted using quantum chemical calculations. However, the lack of well-resolved experimental probes of a AuNC electronic structure has made the task of evaluating the accuracy of electronic structure descriptions challenging. We compare electronic absorption spectra computed using time-dependent density functional theory to recently collected high resolution experimental spectra of A u 9 ( P P h 3 ) 8 3 + and A u 8 ( P P h 3 ) 7 2 + AuNCs with strikingly similar features. After applying a simple scaling correction, the computed spectrum of A u 8 ( P P h 3 ) 7 2 + yields a suitable match, allowing us to assign low-energy metal–metal transitions in the experimental spectrum. No similar match is obtained after following the same procedure for two previously reported isomers for A u 9 ( P P h 3 ) 8 3 + , suggesting either a deficiency in the calculations or the presence of an additional isomer. Instead, we propose assignments for A u 9 ( P P h 3 ) 8 3 + based off of similarities A u 8 ( P P h 3 ) 7 2 + . We further model these clusters using a simple particle-in-a-box analysis for an asymmetrical ellipsoidal superatomic core, which allows us to reproduce the same transitions and extract an effective core size and shape that agrees well with that expected from crystal structures. This suggests that the superatomic model, which is typically employed to explain the qualitative features of nanocluster electronic structures, remains valid even for small AuNCs with highly aspherical cores. Abstract

The role of water in the formation of particles from atmospheric trace gases is not well understood, in large part due to difficulties in detecting its presence under atmospheric conditions and the variety of possible structures that must be screened computationally. Here, we use infrared spectroscopy and variable-temperature ion trap mass spectrometry to investigate the structural motifs adopted by water bound to ammonium bisulfate clusters and their temperature dependence. For clusters featuring only acid–base linkages, water adopts a bridging arrangement spanning an adjacent ammonium and bisulfate. For larger clusters, water can also insert into a bisulfate–bisulfate hydrogen bond, yielding hydration isomers with very similar binding energies. The population of these isomers shows a complex temperature evolution, as an apparent third isomer appears with a temperature dependence that is difficult to explain using simple thermodynamic arguments. Abstract

Atmospheric new particle formation is the process by which atmospheric trace gases, typically acids and bases, cluster and grow into potentially climatically relevant particles. Here, we evaluate the structures and structural motifs present in small cationic ammonium and aminium bisulfate clusters that have been studied both experimentally and computationally as seeds for new particles. For several previously studied clusters, multiple different minimum-energy structures have been predicted. Vibrational spectra of mass-selected clusters and quantum chemical calculations allow us to assign the minimum-energy structure for the smallest cationic cluster of two ammonium ions and one bisulfate ion to a CS-symmetry structure that is persistent under amine substitution. We derive phenomenological vibrational frequency scaling factors for key bisulfate vibrations to aid in the comparison of experimental and computed spectra of larger clusters. Finally, we identify a previously unassigned spectral marker for intermolecular bisulfate–bisulfate hydrogen bonds and show that it is present in a class of structures that are all lower in energy than any previously reported structure. Tracking this marker suggests that this motif is prominent in larger clusters as well as ∼180 nm ammonium bisulfate particles. 45. Outcome Assessment of Mentorship Program

Abstract

This paper reports results of a survey that studied the outcomes of a mentorship program. Students enrolled in a core course for the MA and certificate in gerontology programs were required to select either a long-term care service, a program in aging, or an agency providing services for senior adults, where they could spend a day interacting with a mentor. A wide range of sites were available and students could select one site for 8 hours or two sites for 4 hours each. The researchers were interested in assessing both the opinions of the students who participated and of the mentors themselves about this educational experience. A survey that had a series of open-ended and close-ended questions was administered to all graduate students who participated as well as to the mentors at their various sites. This paper discusses the implications of this program as a model for other gerontology programs. It delineates mentor and student suggestions about the strengths and weaknesses of the current set-up of the mentorship program. Suggestions are made for further study of this pedagogical tool.

47. Rewards and Costs of Faculty Involvement in Intergenerational Service-Learning

Abstract

Service-learning (S-L) has been regarded as a relatively well-established and effective teaching pedagogy. Students who participate in S-L are more likely to learn more efficiently, more effectively, and remember more of what they have learned than their counterparts. Current studies have been done on the experiences of students in service-learning and implementing S-L components. Other research has focused on the barriers to conducting effective S-L. However, no qualitative studies have examined a faculty component of S-L. Also, there have been few studies or discussions delineating faculty perceptions or experiences in S-L. An e-mail survey supplemented with personal interviews was utilized in an effort to understand experiences of participating in S-L. Of specific interest were faculty perceptions of the costs and rewards in employing S-L in gerontology-related curriculums.

50. Sustaining Intergenerational Service-Learning in Gerontology Education

Abstract

Service-learning is a pedagogical innovation in higher education that offers experiential learning to students while providing benefits to communities. Recognizing the importance of this innovation, the Corporation for National and Community Service has promoted the development of intergenerational service-learning components in gerontology classes through grants to the Association for Gerontology in Higher Education in partnership with Generations Together at the University of Pittsburgh. The funding resulted in 66 service-learning projects in institutions of higher education nationwide over six years. This study assessed what has happened since the funding for these projects ended. A survey of former faculty grant recipients found that the majority were still utilizing service-learning components in their classes. This was despite the perception of all of the respondents that the time and effort involved far exceeds that of traditional classes. Respondents identified a number of barriers to the implementation and continuation of service-learning, and made recommendations that encourage intergenerational service-learning at institutions of higher education.
