Comparisons between Viking-Age Scandinavia and the cultures of Oceania have long antecedents, stretching back at least to the late nineteenth century, with a significant milestone in the first-ever synthesis of Polynesian archaeology – Peter Buck’s Vikings of the Sunrise published in 1938. This brief contribution offers some critical commentary on a recent example, Mads Ravn’s paper in the 2018 volume of this journal, setting it in disciplinary context and also against Hawaiian work on this topic that has been undertaken by the authors since 2013. We consider the very real potential in this kind of comparative research, with some discussion of possible ways forward, and a note on pitfalls that must be avoided. Long sequences of continuous historical data, with a focus on internal social processes in addition to external influences, are at the centre of our approach. Above all, we stress the need for an emphasis on emic perspectives, not only in relation to native Hawaiians and other Pasifika, but also – as far as possible – in the study of the Scandinavian Iron Age.

Li-Fraumeni syndrome is a cancer predisposition syndrome caused by pathogenic TP53 germline variants and associated with a high lifelong cancer risk. We analysed the German LFS registry that contains data on 304 individuals. Cancer phenotypes were correlated with variants grouped according to their ability to transactivate target genes in a yeast assay using a traditional (non-functional, partially-functional) and a novel (clusters A, B, C) classification of variants into different groups. Partially-functional and cluster B or C variants were enriched in patients not meeting clinical testing criteria. Time to first malignancy was longer in carriers of partially-functional variants (Hazard Ratio [HR] = 0.38; 95% CI, 0.22 to 0.66). Variants grouped within clusters B (HR = 0.45; 95% CI, 0.28 to 0.71) or C (HR = 0.34; 95% CI, 0.19 to 0.62) were associated with later cancer onset than NULL variants. Children's high and increasing levels of screen time are of growing concern to parents, health professionals, and researchers. With the growing availability and use of devices such as smartphones and tablets, it is important to understand the impact of children's screen use on development. Prospective longitudinal data from 6,281 children (48.3% female) in the Growing Up in New Zealand study were used to examine relations between the extent of screen exposure in early childhood (2-4.5 years) and later language development, early educational skills, and peer social functioning at ages 4.5 and 8 years. Higher levels of screen exposure were associated with lower levels of vocabulary, communication, writing, numeracy, and letter fluency and higher levels of peer problems. These associations were reduced after controlling for confounding family social background factors but remained significant. Results indicate that more than 1.5 hr of daily direct screen time at age 2 was associated with below average language and educational ability and above average levels of peer relationship problems at age 4.5. Exposure to more than 2.5 hr of daily direct screen time was associated with higher than average peer relationship problems at age 8. Findings indicate that high levels of screen exposure during early childhood are negatively associated with children's later language, educational, and social development. Such information is critical to help inform policy guidelines, health care, and parenting practices regarding the availability and children's use of screens in early childhood. Studies have shown that the human microbiome influences the response to systemic immunotherapy. However, only scarce data exist on the impact of the urinary microbiome on the response rates of bladder cancer (BC) to local Bacillus Calmette-Guérin instillation therapy. We launched the prospective SILENT-EMPIRE study in 2022 to address this question. We report the results of the pilot study of SILENT-EMPIRE, which aimed to compare the microbiome between fresh frozen (FF) and formalin-fixed paraffin-embedded (FFPE) samples in the cancerous tissue and adjacent healthy tissue of BC patients. Our results show that alpha diversity is increased in FF samples compared to FFPE (coverage index p = 0.041, core abundance index p = 0.008). No significant differences concerning alpha diversity could be detected between cancerous and non-cancerous tissue in the same BC patients. This study demonstrates that microbiome analysis from both FF and FFPE samples is feasible. Implementing this finding could aid in the translation of research findings into clinical practice.

Our current global Food System is facing extraordinary challenges in both size and severity, including a rise in unsustainable consumption behaviours, continued environmental degradation, growing food insecurity, and widening social inequalities. A Food System transformation is now both critically important and overwhelmingly complex, requiring nothing less than a complete overhaul of the entire value chain. Everyone is needed: Small Medium Enterprises (SMEs) with technological solutions, Non-Governmental Organisations (NGOs) with social innovations, researchers with novel methodologies, governments with food policy advancements, professionals with varying expertise, and last but not least, empowered and informed citizens with the ability and resources for better decision-making. Living Labs offer a holistic, place-based approach needed to facilitate multi-actor inputs on various levels, specifically Food System Living Labs (FSLLs) like the ones established as part of the FoodSHIFT 2030 Project. Nine front-runner Food System Living Labs were operationalised alongside a novel framework merging high-level interdisciplinary initiatives with a diverse set of innovative approaches towards more Sustainable Food Systems (SFS). The FoodSHIFT Approach concept was praised by external evaluators for its ground-breaking framework, and the nearly completed project has been listed as a best practice. Background: In KwaZulu-Natal (KZN), South Africa, COVID-19 vaccinations commenced in May 2021. This study investigated the extent and reasons for COVID-19 vaccine (C19V) wastage in KZN and strategies undertaken to mitigate loss., Methods: This two-phase multicenter study was conducted at private and public healthcare facilities from May 2021 to July 2022., Results: KZN reported 2% Pfizer and 1% Janssen C19V wasted, mainly due to expiry. C19V waste-minimization strategies reported by 100% public and private sector vaccination leads included cold chain monitoring, designated trained staff and the use of stock-management systems., Conclusions: The WHO's risk-mitigation factors should be implemented continuously to minimize vaccine wastage., (© The Author(s) 2024. The Scottish verdict system includes three verdicts: 'guilty', 'not guilty' and 'not proven'. Politicians propose that the three-verdict system is partially to blame for the low conviction rate of rape, whereas research suggests that rape myths may be having a larger impact. To test the effects of varying verdict systems (guilty, not guilty and not proven; guilty and not guilty; a series of proven and not proven verdicts) and rape myths on juror verdicts. A total of 180 participants answered questions regarding their acceptance of rape myths using the Acceptance of Modern Myth and Sexual Aggression (AMMSA) scale. They then watched a staged rape trial filmed in a real courtroom and reached a verdict. Participants also provided longer-form answers on which thematical analysis was conducted. The main findings are as follows: (1) The special verdict system leads to a higher conviction rate than the other systems when rape myth acceptance is controlled for. (2) The higher the rape myth acceptance, the more favourably the accused was perceived and the less favourably the complainer was perceived.

We report a case of transperineostomal bipolar resection of the prostate (TPR-P) for lower urinary tract symptoms (LUTS). To our knowledge, this is the first description in the scientific literature. A 67-year-old man with a medical history of multiple penile debridements and formation of a perineostomy due to an episode of severe Fournier's gangrene in 2015, was admitted to the emergency room with acute urinary retention. Consecutively, a suprapubic catheter was inserted. Attempts of catheterization failed due to bulbar stenosis and an obstructive prostatic urethra. After the resolution by dilatation of the bulbar stenosis, post-voiding residual volume persisted at up to 150 ml. The intra- and postoperative course after TPR-P was uneventful. No adverse events occurred. The assessment after six weeks revealed an International Prostate Symptom Score (IPSS) improvement of nearly 50% for the symptoms and >60% for overall satisfaction (preoperative: IPSS: S=24, L=6; postoperative IPSS: S=13, L=2). The average post-voiding residual volume decreased from 150 ml preoperatively to 15 ml (range 0-30 ml) postoperatively. Due to the missing full length of the urethra, the augmented range of motion seemed almost too loose for classic resection techniques in our hands. Therefore, we believe that in such cases it might be advantageous to use enucleation techniques. Metal-organic frameworks (MOFs) have emerged as promising nanocarriers for cancer treatment due to their unique properties. Featuring high porosity, extensive surface area, chemical stability, and good biocompatibility, MOFs are ideal for efficient drug delivery, targeted therapy, and controlled release. They can be designed to target specific cellular organelles to disrupt metabolic processes in cancer cells. Additionally, functionalization with enzymes mimics their catalytic activity, enhancing photodynamic therapy and overcoming apoptosis resistance in cancer cells. The controllable and regular structure of MOFs, along with their tumor microenvironment responsiveness, make them promising nanocarriers for anticancer drugs. These carriers can effectively deliver a wide range of drugs with improved bioavailability, controlled release rate, and targeted delivery efficiency compared to alternatives. In this article, we review both experimental and computational studies focusing on the interaction between MOFs and drug, explicating the release mechanisms and stability in physiological conditions. Notably, we explore the relationship between MOF structure and its ability to damage cancer cells, elucidating why MOFs are excellent candidates for bio-applicability. Viral Haemorrhagic Fever Outbreak presents a significant public health threat, requiring a timely, robust, and well-coordinated response. This paper aims to describe the roles of the Tanzania Field Epidemiology and Laboratory Training Program (TFELTP) graduates and residents in responding to Tanzania's first Marburg Viral Disease (MVD) outbreak. We performed a secondary data analysis using a range of documents, such as rosters of deployed responders and the TFELTP graduate and resident database, to count and describe them. Additionally, we conducted an exploratory textual analysis of field deployment reports and outbreak situational reports to delineate the roles played by the residents and graduates within each response pillar. A total of 70 TFELTP graduates and residents from different regions were involved in supporting the response efforts. TFELTP graduates and residents actively participated in several interventions, including contact tracing and follow up, sensitising clinicians on surveillance tools such as standard case definitions, alert management, supporting the National and Kagera Regional Public Health Emergency Operations Centres, active case search, risk communication, and community engagement, coordination of logistics, passenger screening at points of entry, and conducting Infection Prevention and Control (IPC) assessments and orientations in 144 Health Facilities. The successes achieved and lessons learned from the MVD response lay a foundation for sustained investment in skilled workforce development. While parasites are likely to connect to multiple host plants in nature, parasitism dynamics under multiple association conditions remain unclear and are difficult to separate from competitive effects. In this study, a five-compartment split root-box was constructed to allow a single facultative root hemiparasite, Phtheirospermum japonicum, to connect to zero, one or two Medicago sativa hosts while maintaining constant plant number and independently controlling nutrient supply. In the first experiment, we found that P. japonicum derived equal, additive benefits from attachment to a second host irrespective of parasite N status. In the second experiment, parasites were grown at four N levels in either parasitic or control conditions. Attachment caused a constant, absolute increase in parasite mass at all N levels, while host damage increased at higher parasite N levels despite an apparent decrease in host to parasite N transfer. Introduction: Social Stories (SS) is a socially-valid intervention for autistic children and young people (CYP) which is used widely by professionals and parents. Research suggests that whilst parents are in an ideal position to deliver interventions for their autistic CYP, a lack of procedural integrity can result in a great deal of variability in parent-mediated intervention outcomes., Methods: This exploratory study investigated the extent to which SS can be effectively developed and delivered, through digital mediation, by parents with little to no researcher input ( n  = 17, sample 1) and the factors that impact effectiveness. Furthermore, the study also investigated the extent to which digitally-mediated SS can support autistic CYP to develop and deliver their own stories, thereby utilising the intervention as a means for self-support and self-management ( n  = 5, sample 2)., Results: The outcomes of the study indicate that digital mediation can effectively support parent-led SS intervention. Findings also indicate that receptive/expressive language skills of autistic CYP, their level of systemizing, as well as the practice of consulting with the autistic CYP whilst identifying goals and developing stories, are individual and procedural characteristics which positively influence the effectiveness of the parent-led intervention. The transperitoneal approach (TP) and the retroperitoneal approach (RP) are two common methods for performing nephrectomy or partial nephrectomy. However, both approaches face difficulties, such as trocar placement and limited working space (RP). TP is impaired in the case of dorsal tumors and dissection of the renal artery can be challenging due to the anatomic localization dorsally to the renal vein. A hybrid approach that combines both methods has been previously reported in a case series, but not evaluated systematically. This study proposes a modified hybrid approach, which we call the transabdominal lumbar approach (TALA), involving late robotic docking after elaborating the retroperitoneum using conventional laparoscopy. The study compares the last 20 consecutive patients who underwent RP and the last 20 patients who underwent TALA at our institution. The investigated variables include operative time and amount of blood loss, hospitalization duration, postoperative analgesia requirement, and postoperative complications. The study found no significant difference in operative time, blood loss, ischemia time, or hospital stay between the two groups. The TALA group had fewer complications regarding Clavien-Dindo category 3, but one complication of category 4. In Conclusion, TALA is a safe and promising approach that combines the advantages of RP and TP.

Social Stories™ is one of the most popular interventions for autistic children and has been researched extensively. However, effectiveness data has been gathered mainly through single-participant designs which generate outcomes which can lack generalizability and social validity. Stories Online For Autism (SOFA) is a digital application which supports the development and delivery of Social Stories in a real-world setting and has the potential to contribute toward furthering (1) Social Stories research and (2) research on digital applications for autism by gathering large data sets from multiple participants. Three data sets ( N  = 856) were gathered through the SOFA app and were analyzed to investigate three key variables: What predicted closeness-to-goal of the Social Stories (as rated by an adult/parent/guardian, n  = 568); the child's comprehension of the Social Stories (assessed by story comprehension questions, n  = 127); and the child's rating of the enjoyability of the Social Stories ( n  = 161). A merged data set then investigated correlations between these three key variables. Age range (≤15), gender, autism diagnosis, and the child's level of language understanding were the potential predictors for these three key variables. Regression analysis indicated that parental closeness-to-goal ratings for their children were highest for children who were younger and more verbal. Regression analysis also indicated that older children scored higher in comprehension assessment, and autistic children rated the Social Stories as more enjoyable. Closeness-to-goal, comprehension scores and enjoyment ratings did not significantly correlate with each other. This is the largest study of Social Stories effectiveness, which was enabled through the collection of data through a digital app from multiple participants. The results indicate that digital social stories are particularly effective for younger verbal children. While this was the case for all children, it was particularly true for autistic children and female (and gender-diverse) children. For the first time, the gathering of large digital data sets has highlighted that while digital Social Stories can be effective for autistic males, they can be more effective for autistic females and gender-diverse autistic individuals. This research work aims to explore the potential usage of post-consumer waste expanded polystyrene (EPS) for the fabrication of self-standing electrodes by incorporating reduced graphene oxide (rGO) into it via a facile cost-effective mechanical mixing process. The π-π interaction between the expanded polystyrene and rGO is evidenced from FT-IR and Raman analysis. The elevated thermal stability of the EPS/rGO composite from thermogravimetric analysis (TGA) further confirms the interconnection between the rGO and EPS. This π-π stacking interaction between the rGO and the polystyrene molecules present in the polymer matrix enable the composite material to be interconnected throughout which is beneficial for the charge transport process. The symmetric coin cell supercapacitor fabricated using the EPS/rGO composite electrode can be operated with a high operating voltage of 1.6 V in aqueous KOH and Na 2 SO 4 electrolytes. The devices fabricated with KOH and Na 2 SO 4 electrolytes deliver an areal capacitance of 11.9 mF cm -2 and 10 mF cm -2 at the discharge current density of 0.1 mA cm -2 . Further, the devices fabricated with the KOH and Na 2 SO 4 electrolytes demonstrated remarkable rate capability of 87.1% and 99.5% after 10 000 continuous charge discharge cycles. This study investigated associations between change in the food environment and change in measured body mass index (BMI) and waist circumference (WC) in the Christchurch Health and Development Study (CHDS) birth cohort. Our findings suggest that cohort members who experienced the greatest proportional change towards better access to fast food outlets had the slightly larger increases in BMI and WC. Contrastingly, cohort members who experienced the greatest proportional change towards shorter distance and better access to supermarkets had slightly smaller increases in BMI and WC. Objective: Long-term studies following disasters are rare. It is important to quantify long-term effects of disasters to determine impacts on populations over time. We therefore aim to report the long-term associations between exposure to the Canterbury earthquakes and common mental disorders, taking into account potential confounding factors., Methods: The Christchurch Health and Development Study is a 40-year longitudinal study of a birth cohort of New Zealand children (635 males and 630 females). The Christchurch Health and Development Study includes 884 participants with data on earthquake exposure and mental health outcomes at ages 34 and 40 years. Rates of Diagnostic and Statistical Manual of Mental Disorders (4th ed.) disorders were measured categorically and using an expanded definition that included sub-syndromal symptoms. The current impact of the earthquakes is reported using 12-month prevalence data 7 years after the earthquakes. The cumulative impact of the earthquakes over the 7 years since onset is also reported., Results: There was a linear trend towards increasing rates of disorder with increasing exposure to the earthquakes. After adjusting for covariates, the 12-month prevalence of anxiety disorder symptoms was significantly increased ( p  = 0.003). The earthquakes were also associated with cumulative increases in symptoms of post-traumatic stress disorder ( p  < 0.001), anxiety disorder ( p  = 0.016), nicotine dependence ( p  = 0.012), and the total number of disorders ( p  = 0.039)., Conclusion: The Canterbury earthquakes were associated with persistent increases in Anxiety Disorder symptoms 7 years after their onset. The earthquakes were also associated with cumulative increases in symptoms of common psychiatric disorders. The magnitude of these effects is small, may no longer be clinically significant and has decreased over time.

Africa is set to experience a three-fold increase in vaccine demand by 2040, yet the continent possesses few domestic capabilities for vaccine production. This lack of production capacity, heavy reliance on foreign aid, disruptions of hard-won immunization progress due to the effects of the COVID-19 pandemic, and fluctuating vaccine market dynamics threaten to hinder ongoing efforts to increase vaccination rates on the continent. In order meet the vaccine demands of a rapidly growing population, and to be able to provide novel vaccines to its population in the future, the African continent must develop a sustainable vaccine manufacturing infrastructure. The African Union, in partnership with the Africa Centres for Disease Control and Prevention, recently set forth its Program for African Vaccine Manufacturing Framework for Action, which sets the goal of Africa producing 60 % of its vaccine needs by 2040. To meet these goals, African governments and their multinational, philanthropic, and private sector partners must work to secure low-cost financing and provide a favourable regulatory environment for nascent African vaccine manufacturers. Social Stories™ (SS) is one of the most popular and researched interventions for autistic children. To date, research that focuses on outcomes has been prioritized over the investigation of the psychological mechanisms that inform the intervention. In this article we consider theoretical accounts proposed thus far which could underpin SS. We argue that mechanisms that are based on social deficit theories lack validity, and propose a rule-based theoretical account to inform a strengths-based approach toward conceptualizing the mechanisms that underpin SS. We apply this account to the 'double-empathy problem' to propose that SS can be adapted to involve all parties in the development and delivery of SS support by adopting a rule-based perspective. High resolution poverty mapping supports evidence-based policy and research, yet about half of all countries lack the survey data needed to generate useful poverty maps. To overcome this challenge, new non-traditional data sources and deep learning techniques are increasingly used to create small-area estimates of poverty in low- and middle-income countries (LMICs). Convolutional Neural Networks (CNN) trained on satellite imagery are emerging as one of the most popular and effective approaches. However, the spatial resolution of poverty estimates has remained relatively coarse, particularly in rural areas. To address this problem, we use a transfer learning approach to train three CNN models and use them in an ensemble to predict chronic poverty at 1 km2 scale in rural Sindh, Pakistan. The models are trained with spatially noisy georeferenced household survey containing poverty scores for 1.67 million anonymized households in Sindh Province and publicly available inputs, including daytime and nighttime satellite imagery and accessibility data. Results from both hold-out and k-fold validation exercises show that the ensemble provides the most reliable spatial predictions in both arid and non-arid regions, outperforming previous studies in key accuracy metrics. A third validation exercise, which involved ground-truthing of predictions from the ensemble model with original survey data of 7000 households further confirms the relative accuracy of the ensemble model predictions. Background: This study examined differences in ADHD symptoms and diagnosis between preterm and term-born adults (≥18 years), and tested if ADHD is related to gestational age, birth weight, multiple births, or neonatal complications in preterm borns., Methods: (1) A systematic review compared ADHD symptom self-reports and diagnosis between preterm and term-born adults published in PubMed, Web of Science, and PROQUEST until April 2021; (2) a one-stage Individual Participant Data(IPD) meta-analysis (n = 1385 preterm, n = 1633 term; born 1978-1995) examined differences in self-reported ADHD symptoms[age 18-36 years]; and (3) a population-based register-linkage study of all live births in Finland (01/01/1987-31/12/1998; n = 37538 preterm, n = 691,616 term) examined ADHD diagnosis risk in adulthood (≥18 years) until 31/12/2016., Results: Systematic review results were conflicting. In the IPD meta-analysis, ADHD symptoms levels were similar across groups (mean z-score difference 0.00;95% confidence interval [95% CI] -0.07, 0.07). Whereas in the register-linkage study, adults born preterm had a higher relative risk (RR) for ADHD diagnosis compared to term controls (RR = 1.26, 95% CI 1.12, 1.41, p < 0.001). Among preterms, as gestation length (RR = 0.93, 95% CI 0.89, 0.97, p < 0.001) and SD birth weight z-score (RR = 0.88, 95% CI 0.80, 0.97, p < 0.001) increased, ADHD risk decreased., Conclusions: While preterm adults may not report higher levels of ADHD symptoms, their risk of ADHD diagnosis in adulthood is higher., Impact: Preterm-born adults do not self-report higher levels of ADHD symptoms, yet are more likely to receive an ADHD diagnosis in adulthood compared to term-borns. Previous evidence has consisted of limited sample sizes of adults and used different methods with inconsistent findings. This study assessed adult self-reported symptoms across 8 harmonized cohorts and contrasted the findings with diagnosed ADHD in a population-based register-linkage study. Background: Individuals born very low birthweight (VLBW) are at increased risk of impaired cardiovascular and respiratory function in adulthood. To identify markers to predict future risk for VLBW individuals, we analyzed DNA methylation at birth and at 28 years in the New Zealand (NZ) VLBW cohort (all infants born < 1500 g in NZ in 1986) compared with age-matched, normal birthweight controls. Associations between neonatal methylation and cardiac structure and function (echocardiography), vascular function and respiratory outcomes at age 28 years were documented., Results: Genomic DNA from archived newborn heel-prick blood (n = 109 VLBW, 51 controls) and from peripheral blood at ~ 28 years (n = 215 VLBW, 96 controls) was analyzed on Illumina Infinium MethylationEPIC 850 K arrays. Following quality assurance and normalization, methylation levels were compared between VLBW cases and controls at both ages by linear regression, with genome-wide significance set to p < 0.05 adjusted for false discovery rate (FDR, Benjamini-Hochberg). In neonates, methylation at over 16,400 CpG methylation sites differed between VLBW cases and controls and the canonical pathway most enriched for these CpGs was Cardiac Hypertrophy Signaling (p = 3.44E -11 ). The top 20 CpGs that differed most between VLBW cases and controls featured clusters in ARID3A, SPATA33, and PLCH1 and these 3 genes, along with MCF2L, TRBJ2-1 and SRC, led the list of 15,000 differentially methylated regions (DMRs) reaching FDR-adj significance. Fifteen of the 20 top CpGs in the neonate EWAS showed associations between methylation at birth and adult cardiovascular traits (particularly LnRHI). In 28-year-old adults, twelve CpGs differed between VLBW cases and controls at FDR-adjusted significance, including hypermethylation in EBF4 (four CpGs), CFI and UNC119B and hypomethylation at three CpGs in HIF3A and one in KCNQ1. DNA methylation GrimAge scores at 28 years were significantly greater in VLBW cases versus controls and weakly associated with cardiovascular traits. Four CpGs were identified where methylation differed between VLBW cases and controls in both neonates and adults, three reversing directions with age (two CpGs in EBF4, one in SNAI1 were hypomethylated in neonates, hypermethylated in adults). Of these, cg16426670 in EBF4 at birth showed associations with several cardiovascular traits in adults., Conclusions: These findings suggest that methylation patterns in VLBW neonates may be informative about future adult cardiovascular and respiratory outcomes and have value in guiding early preventative care to improve adult health., (© 2023. The early and accurate detection of colorectal cancer (CRC) significantly affects its prognosis and clinical management. However, current standard diagnostic procedures for CRC often lack sensitivity and specificity since most rely on visual examination. Hence, there is a need to develop more accurate methods for its diagnosis. Support vector machine (SVM) and feedforward neural network (FNN) models were designed using the Fourier transform infrared (FT-IR) spectral data of several colorectal tissues that were unanimously identified as either benign or malignant by different unrelated pathologists. The set of samples in which the pathologists had discordant readings were then analyzed using the AI models described above. Between the SVM and NN models, the NN model was able to outperform the SVM model based on their prediction confidence scores. Using the spectral data of the concordant samples as training set, the FNN was able to predict the histologically diagnosed malignant tissues ( n = 118) at 59.9-99.9% confidence (average = 93.5%). Of the 118 samples, 84 (71.18%) were classified with an above average confidence score, 34 (28.81%) classified below the average confidence score, and none was misclassified. Moreover, it was able to correctly identify the histologically confirmed benign samples ( n = 83) at 51.5-99.7% confidence (average = 91.64%). Of the 83 samples, 60 (72.29%) were classified with an above average confidence score, 22 (26.51%) classified below the average confidence score, and only 1 sample (1.20%) was misclassified. The study provides additional proof of the ability of attenuated total reflection (ATR) FT-IR enhanced by AI tools to predict the likelihood of CRC without dependence on morphological changes in tissues.

The prevalence of polycystic ovary syndrome (PCOS) among trans men has been reported as higher than among the cisgender population, which varies between 14.4% and 58%. In this cross-sectional study, we aimed to evaluate the association of oligo-anovulation and/or features of hyperandrogenism with the scores on the Utrecht Gender Dysphoria Scale (UGDS), the Body Image Scale (BIS), and the Short Form-36 Health Survey (SF-36) in treatment-naive trans men with PCOS seeking help for gender transition. The study sample consisted of trans men who were diagnosed with gender dysphoria (GD) between December 2019 and November 2020. To assess body dissatisfaction and psychological functioning, the UGDS, BIS, and SF-36 were administered to all transgender individuals as part of the routine assessment procedure. A total of 49 treatment-naive trans men were included in our study; 38.8% ( n =19) of the participants were diagnosed with PCOS using the Rotterdam 2003 criteria. The scores of UGDS and BIS were significantly lower in the PCOS group compared with the non-PCOS group ( p <0.001 and p =0.003, respectively). Among the eight parameters within SF-36, general health, emotional role, and vitality scores were significantly higher in the PCOS group ( p =0.031, p =0.015, and p =0.006, respectively). Our study is the first study that demonstrates significantly lower GD, higher body image satisfaction and quality of life in trans men with PCOS compared with those without PCOS. The activity concentrations of 238U, 232Th and 40K in the soil of areas surrounding the Kayelekera uranium mine were assessed. This study aims to provide a comprehensive profile of soil radioactivity distribution in the area surrounding the uranium mine to determine radiological hazards associated with mining and processing activities. Soil samples were analysed using gamma-ray spectrometry with a high-purity germanium detector. Mean specific activities of 238U, 232Th and 40K were 58.3 ± 3.7, 40.3 ± 0.3 and 590.9 ± 63.9 Bq kg-1, respectively. Results from this study have indicated changes in radioactivity levels of naturally occurring radioactive materials in the area surrounding the understudied mine, with certain areas close to the mine site showing elevated levels of 238U, highlighting the need for systematic and periodic monitoring., (© The Author(s) 2022. Bruineberg and colleagues highlight work using Markov blankets to demarcate the bounds of the mind. This echoes earlier attempts to demarcate the bounds of the mind from a dynamical systems perspective. Advocates of mechanistic explanation have challenged the dynamical approach to independently motivate the application of the formalism, a challenge that Markov blanket theorists must also meet.

In this paper, we describe the development and evaluation of a novel tissue-holding device (THD) for use during robotic-assisted laparoscopic partial nephrectomy. The THD is a vacuum-based apparatus made of either 3D-printed polyethylene or stainless steel. The proximal end connects to suction tubing routed outside the body, while the distal end is conically shaped and designed to firmly interface with the tumor. Device feasibility studies were performed on six porcine kidneys, two porcine livers, and two embalmed human cadavers. A Likert-scale rating was used to assess device setup, suction, and tissue handling. Additional tests were performed using the daVinci Xi® robotic system. Finally, the holding force of the THD was assessed using different standard vacuum systems and pressure settings. In porcine tissue, the device setup, tissue suction, and handling were rated as "good". THD insertion and removal was uncomplicated. In a simulated transabdominal approach on fixed human cadavers, the device setup, suction, and tissue handling were also rated as "good". No macroscopic tissue compromise or device deterioration was noted. The handling and holding abilities using the daVinci Xi® robotic system were also rated "good". The device was able to successfully hold over 300 g of tissue at a suction pressure of -600 mmHg. The preliminary evaluation of the THD demonstrated satisfactory results.

Background: Executive function difficulties are common among children born very preterm and/or very low birthweight (<1500 g; VLBW), but little is known about whether they persist into adulthood., Objectives: Examine the nature and pattern of self-reported executive functioning at 23 and 28 years of age using data from a national cohort study of adults born VLBW and a comparison group of same-age full-term (FT) born adults. Also examined were associations between executive function difficulties and socio-economic outcomes., Methods: All infants born VLBW in New Zealand during 1986 were prospectively included in an audit of retinopathy of prematurity (n = 413), with 250 (77% of survivors) followed to median age 28 years. A comparison group of FT adults was also recruited at age 23 and followed to 28 years (n = 100). Across both adult assessments, executive functioning was assessed using the Behaviour Rating Inventory of Executive Function-Adult Version (BRIEF-A) and analysed with semi-parametric models to examine the effects of age and group on executive function., Results: At 23 and 28 years, VLBW adults had increased risk of executive function impairment compared with FT adults in behaviour regulation (relative risk [CI] 2.37, 95% confidence interval (CI)1.27, 4.45), meta-cognition (RR 6.03, 95% CI 2.18, 16.78) and global functioning (RR 3.20, 95% CI 1.40, 7.28). Impaired global executive functioning was associated with lower socio-economic status (regression estimate [b] = -0.43, 95% CI -0.59, -0.27) and a reduced likelihood of home ownership by age 28 years (RR 0.98, 95% CI 0.96, 1.00), even after controlling for sex, ethnicity and parental socio-economic backgrounds for both groups., Conclusion(s): VLBW-born adults continue to experience more executive function difficulties in their everyday life relative to term controls at age 28 years. Background: Antenatal corticosteroids (ACS) given to mothers with anticipated very preterm delivery are widely used and improve infant outcomes. Follow-up studies of the first trials of ACS have shown no adverse effects, but recently there have been concerns about possible longer-term harms., Objectives: We aimed to assess the relationship of ACS therapy to a range of physical health and welfare measures in a cohort of very low birthweight (VLBW; <1500 g) young adults., Methods: Population-based cohort follow-up study. All VLBW infants born in New Zealand in 1986 were included in a prospective audit of retinopathy of prematurity. Perinatal data collection included information on ACS. At 26-30 years, 250 of 323 (77%) survivors participated, 58% having received ACS, with 229 assessed in one centre, including cardiovascular, metabolic, respiratory and neurocognitive measures. Differences in outcome between those receiving/not receiving ACS were summarised by the mean difference for continuous outcomes supplemented by Cohen's d as a standardised measure of effect size (ES), and risk ratios (RRI) for dichotomous outcomes, adjusted for relevant covariates using generalised linear regression methods., Results: There were no or minimal adverse effects of receipt of ACS versus no receipt across a range of health and welfare outcomes, both for the full cohort (adjusted ES range d = 0.01-0.23; adjusted RR range 0.78-2.03) and for individuals with gestation <28 weeks (extremely preterm; EP), except for a small increase in rates of major depression. In EP adults, receipt of ACS was associated with a higher incidence of hypertension, but might have a small benefit for IQ., Conclusions: In this population-based VLBW cohort, we detected minimal adverse outcomes associated with exposure to ACS by the third decade of life, a similar result to the 30-year follow-up of participants in the first ACS trial. Background: The ability to read and interpret a prescription is a crucial and rate-determining step for the provision of correct medicine(s), appropriate instructions to patients and for good practice of pharmacy in general. However, both in public and private healthcare settings, pharmacists continue to receive numerous illegible handwritten prescriptions. This is time consuming, and it poses serious legal and health consequences if prescriptions are read incorrectly, resulting in wrong medicines being dispensed., Objectives: This study investigated the impact of using a WhatsApp Group to request assistance to read and interpret illegible prescriptions by pharmacists in South Africa., Method: Communication records that span between June 2018 and October 2020 were included in this study. Two registered and practising pharmacists, who were not members of WhatsApp Group, verified the correctness of the responses to enquiries., Key Findings: The WhatsApp Group had 189 participants during the period of this study, and 136 enquiries were made. Only 10 enquiries did not receive responses, while 62, 32 and 32 enquiries received one, two and ≥3 responses, respectively. None of the responses to 29 images, or prescriptions that had received three or more responses, were deemed incorrect. The medians for the period of the first and subsequent responses were two minutes (with interquartile range of 1-4 min) and six minutes (with interquartile range of 3-14 min), respectively; and the difference (i.e., four minutes) was statistically significant (P <0.0001)., Conclusion: The findings of this study suggest that social media platforms such as WhatsApp Groups can be used effectively as tools for aiding in the interpretation of illegible prescriptions., (© The Author(s) 2022. Background: Drug-induced hypersensitivity syndrome (DIHS), also called drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome, is a potentially fatal drug-induced hypersensitivity reaction that is characterized by a cutaneous eruption, multiorgan involvement, viral reactivation, and hematologic abnormalities., Case Presentation: We present a case of lamotrigine-associated DIHS/DRESS complicated by an unusually prolonged course requiring oral corticosteroids and narrow-band ultraviolet B treatment and with development of extensive alopecia areata and autoimmune thyroiditis., Conclusions: DIHS/DRESS is a severe cutaneous adverse reaction that may require prolonged treatment until symptoms resolve. Oral corticosteroids are the mainstay of treatment, but long-term use is associated with significant adverse effects. Alternative therapies, such as cyclosporine, look promising, but further studies are needed to determine safety profile and efficacy. Metabolic state can alter olfactory sensitivity, but it is unknown whether the activity of the olfactory bulb (OB) may fine tune metabolic homeostasis. Our objective was to use CRISPR gene editing in male and female mice to enhance the excitability of mitral/tufted projection neurons (M/TCs) of the OB to test for improved metabolic health. Ex vivo slice recordings of MCs in CRISPR mice confirmed increased excitability due the targeted loss of Kv1.3 channels, which resulted in a less negative resting membrane potential (RMP), enhanced action potential (AP) firing, and insensitivity to the selective channel blocker margatoxin (MgTx). CRISPR mice exhibited enhanced odor discrimination using a habituation/dishabituation paradigm. CRISPR mice were challenged for 25 weeks with a moderately high-fat (MHF) diet, and compared with littermate controls, male mice were resistance to diet-induced obesity (DIO). Female mice did not exhibit DIO. CRISPR male mice gained less body weight, accumulated less white adipose tissue, cleared a glucose challenge more quickly, and had less serum leptin and liver triglycerides. CRISPR male mice consumed equivalent calories as control littermates, and had unaltered energy expenditure (EE) and locomotor activity, but used more fats for metabolic substrate over that of carbohydrates. Counter to CRISPR-engineered mice, by using chemogenetics to decrease M/TC excitability in male mice, activation of inhibitory designer receptors exclusively activated by designer drugs (DREADDs) caused a decrease in odor discrimination, and resulted in a metabolic profile that was obesogenic, mice had reduced EE and oxygen consumption (VO 2 ). We conclude that the activity of M/TC projection neurons canonically carries olfactory information and simultaneously can regulate whole-body metabolism. SIGNIFICANCE STATEMENT The olfactory system drives food choice, and olfactory sensitivity is strongly correlated to hunger and fullness. Olfactory function thereby influences nutritional balance and obesity outcomes. Obesity has become a health and financial crisis in America, shortening life expectancy and increasing the severity of associated illnesses. It is expected that 51% of Americans will be obese by the year 2030. Using CRISPR gene editing and chemogenetic approaches, we discovered that changing the excitability of output neurons in the olfactory bulb (OB) affects metabolism and body weight stabilization in mice. Seizures likely result from aberrant network activity and synchronization. Changes in brain network connectivity may underlie seizure onset. We used a novel method of rapid network model estimation from intracranial electroencephalography (iEEG) data to characterize pre-ictal changes in network structure prior to seizure onset. We analyzed iEEG data from 20 patients from the iEEG.org database. Using 10 s epochs sliding by 1 s intervals, a multiple input, single output (MISO) state space model was estimated for each output channel and time point with all other channels as inputs, generating sequential directed network graphs of channel connectivity. These networks were assessed using degree and betweenness centrality. Both degree and betweenness increased at seizure onset zone (SOZ) channels 37.0 ± 2.8 s before seizure onset. Degree rose in all channels 8.2 ± 2.2 s prior to seizure onset, with increasing connections between the SOZ and surrounding channels. Interictal networks showed low and stable connectivity. A novel MISO model-based network estimation method identified changes in brain network structure just prior to seizure onset. Increased connectivity was initially isolated within the SOZ and spread to non-SOZ channels before electrographic seizure onset. Such models could help confirm localization of SOZ regions., (© 2022. Objective: Knowledge surrounding the link between childhood adversity and reproductive outcomes at midlife is limited. The present study examined the relationship between childhood maltreatment (childhood sexual abuse [CSA], childhood physical punishment [CPP]), and menopause status at age 40., Methods: Data were gathered from female members of the Christchurch Health and Development Study, a longitudinal birth cohort of 1,265 individuals (630 females) born in Christchurch, New Zealand in 1977. Menopause status was defined by categorizing the female cohort at age 40 as either: 1) premenopausal, or 2) peri/ postmenopausal. Retrospective reports of CSA (<16 y) and CPP (<16 y) were obtained at ages 18 and 21 years., Results: The analysis sample comprised n = 468 women with data recorded on both their menopause status at age 40 and history of maltreatment (<16 y), of whom 22% (n = 104) were classified as peri/postmenopausal. A statistically significant association was found between and severity of CSA and menopause status, but not between CPP and menopause status. The association with CSA was robust to control for both childhood confounding factors (<16 y) and intervening adult factors (18-40 y) associated with the menopause transition. In the fully adjusted model, women who had experienced severe CSA involving attempted/completed sexual penetration had twice the rate of entering peri/postmenopause compared with those who reported no CSA (39.0% vs 18.8%)., Conclusions: Severity of CSA exposure was associated with earlier menopausal transition in this female cohort.
