Abstract

Summary: Biomarkers for cytopenias following CAR T‐cell treatment in relapsed/refractory (RR) multiple myeloma (MM) are not completely defined. We prospectively analysed 275 sequential peripheral blood (PB) samples from 58 RRMM patients treated with BCMA‐targeted CAR T cells, and then divided them into three groups: (i) baseline (before leukapheresis), (ii) ≤day+30, and (iii) >day+30 after CAR T‐cell therapy. We evaluated laboratory data and performed flow cytometry to determine the (CAR) T‐cell subsets. Baseline hyperferritinaemia was a risk factor for long‐lasting grade ≥3 anaemia (r = 0.47, p < 0.001) and thrombocytopenia (r = 0.44, p = 0.002) after CAR T‐cell therapy. Low baseline haemoglobin (Hb) and PLT were associated with long‐lasting grade ≥3 anaemia (r = −0.56, p < 0.001) and thrombocytopenia (r = −0.44, p = 0.002) respectively. We observed dynamics of CAR‐negative T‐cell subsets following CAR T‐cell infusion. In the late phase after CAR T‐cell therapy (>day+30), CD4Tn frequency correlated with anaemia (r = 0.41, p = 0.0014) and lymphocytopenia was related to frequencies of CD8+ T cells (r = 0.72, p < 0.001) and CD8Teff (r = 0.64, p < 0.001). CD4Tcm frequency was correlated with leucocytopenia (r = −0.49, p < 0.001). In summary, preexisting cytopenias and hyperferritinaemia indicated long duration of grade ≥3 post‐CAR T‐cell cytopenias. Prolonged cytopenia may be related to immune remodelling with a shift in the CAR‐negative T‐cell subsets following CAR T‐cell therapy. [ABSTRACT FROM AUTHOR]

Abstract

Background: BCMAxCD3 bispecific antibodies (BsAb) have changed the treatment landscape in relapsed refractory multiple myeloma (RRMM). However, despite high response rates, the majority of patients continue to relapse. Infectious complications, mainly viral, represent another challenge with BsAb, seen in up to 90% of patients with half of them requiring hospitalization. Both issues, loss of response as well as infectious complications, can be linked to impaired T-cell function potentially emerging during BsAb therapy. Most BsAbs have been developed as weekly treatments and one hypothesis is that dose dense treatment could overstimulate T cells finally leading to exhaustion and dysfunctionality. Thus, we compared T cell profiles and their functionality from patients with weekly dosing to patients who switched to a monthly schedule due to toxicity or patient request.

Abstract

Publisher Summary This chapter focuses on the use of additives to design and improve the performance of multilayer flexible packages. The two principal approaches of modifying the properties of a polymeric substrate are structural modification of the polymer and incorporation of additives. Incorporation of additives has two general possibilities for multilayer films. First, the additives can be incorporated into the organic substrate after synthesis and second, the additives can be incorporated into the layers of a multilayer film. Additives can be incorporated at various points during the life time of a polymer. The specific additive and its most appropriate physical form have an impact on the most appropriate addition point during a substrate's life cycle. It is important to match the additive's physical form that will be used in handling and dosing to the industrial plant requirements. While manufacturing multilayer films, it should be highlighted that not every additive needs to be added to every layer of the construct. Some additives, such as UV additives, need only to be added to the skin layer. In the case when additives are added to selected layers only, migration effects must be considered as migration into neighboring layers will decrease the additive concentration in the initial layer. The term “antioxidant” is used to describe all stabilizer classes that are utilized to protect organic substrates from thermal and thermooxidative degradation during storage, processing or conversion, and service life in the absence of ultraviolet light and maintain its initial properties.

Abstract

Distal radius fractures are common in children, yet complications are rare. A rarely described complication, acute volar compartmental syndrome, occurred in a 15-year-old boy. An accurate physical examination and awareness of the syndrome are essential for diagnosis. Compartmental pressures can be obtained easily and afford a rapid means of corroboration. Once the diagnosis is established, adequate decompression of all involved compartments, including carpal tunnel release, is essential. The literature is unclear regarding the etiology of this complication. There is nothing structurally intrinsic to the distal radius that should lead to a compartmental syndrome. Both the amount of soft tissue damage at the time of fracture and the mode of immobilization (excessive elevation, constricting splint, etc.) are the ultimate determinants of a successful (or unsuccessful) outcome.
