In this study we aimed to identify the molecular genetic cause of a progressive multisystem disease with prominent lipodystrophy.In total, 5 affected individuals were investigated using exome sequencing. Dermal fibroblasts were characterized using RNA sequencing, proteomics, immunoblotting, immunostaining, and electron microscopy. Subcellular localization and rescue studies were performed.We identified a lipodystrophy phenotype with a typical facial appearance, corneal clouding, achalasia, progressive hearing loss, and variable severity. Although 3 individuals showed stunted growth, intellectual disability, and died within the first decade of life (A1, A2, and A3), 2 are adults with normal intellectual development (A4 and A5). All individuals harbored an identical homozygous nonsense variant affecting the retention and splicing complex component BUD13. The nucleotide substitution caused alternative splicing of BUD13 leading to a stable truncated protein whose expression positively correlated with disease expression and life expectancy. In dermal fibroblasts, we found elevated intron retention, a global reduction of spliceosomal proteins, and nuclei with multiple invaginations, which were more pronounced in A1, A2, and A3. Overexpression of both BUD13 isoforms normalized the nuclear morphology.Our results define a hitherto unknown syndrome and show that the alternative splice product converts a loss-of-function into a hypomorphic allele, thereby probably determining the severity of the disease and the survival of affected individuals.

Die Familiensoziologie war uber lange Zeit mit den jeweils vorherrschenden theoretischen Positionen in der allgemeinen Soziologie verbunden. Zunachst standen funktionalistische und rollentheoretische Ansatze sowie ihr Zusammenwirken im Vordergrund. Die Weiterentwicklung in Richtung des Makroansatzes hin zur Systemtheorie ging allerdings an der Familiensoziologie fast spurlos voruber. Hingegen konnte sich der Symbolische Interaktionismus zumindest in Grenzen und in Kombination mit der qualitativen Sozialforschung in Deutschland behaupten, wahrend die ihrem Anspruch nach explanativen Handlungstheorien das Forschungsfeld in der Familiensoziologie heute eindeutig dominieren. Die Okonomische Theorie hat fast alle Teilbereiche der Familiensoziologie durchdrungen und spezifische Weiterentwicklungen in Verhandlungs- bzw. Bargainingtheorien gefunden. Die Austauschtheorie ist mittlerweile in die allgemeinere Rational-Choice-Theorie integriert, die ihrerseits Spezifikationen beispielsweise im Framing- und Commitment-Ansatz gefunden hat. Die Familiensoziologie ist somit fest in ein Paradigma eingebunden. In der Kuhnschen Sprache wird hier Normalwissenschaft betrieben, es werden erfolgreich ‚Ratsel‘ gelost.

Ist die Familie immer noch die grundlegende Institution der Gesellschaft oder – wie sinkende Geburtenzahlen, alternative Lebensmodelle und ähnlichem mehr zeigen – in einer fundamentalen Krise? Die Antwort auf diese Frage hängt stark von der eingenommenen Perspektive ab: Familie ist immer noch fester Bestandteil der allermeisten Lebensläufe und eine wichtige Institution, in einer punktuellen Perspektive gewinnen jedoch auch alternative Arrangements an Bedeutung und Legitimität.

Automotive radar sensors output a lot of unwanted clutter or ghost detections, whose position and velocity do not correspond to any real object in the sensor's field of view. This poses a substantial challenge for environment perception methods like object detection or tracking. Especially problematic are clutter detections that occur in groups or at similar locations in multiple consecutive measurements. In this paper, a new algorithm for identifying such erroneous detections is presented. It is mainly based on the modeling of specific commonly occurring wave propagation paths that lead to clutter. In particular, the three effects explicitly covered are reflections at the underbody of a car or truck, signals traveling back and forth between the vehicle on which the sensor is mounted and another object, and multipath propagation via specular reflection. The latter often occurs near guardrails, concrete walls or similar reflective surfaces. Each of these effects is described both theoretically and regarding a method for identifying the corresponding clutter detections. Identification is done by analyzing detections generated from a single sensor measurement only. The final algorithm is evaluated on recordings of real extra-urban traffic. For labeling, a semi-automatic process is employed. The results are promising, both in terms of performance and regarding the very low execution time. Human induced pluripotent stem cells (hiPSCs) hold great potential for modeling human diseases and the development of innovative therapeutic approaches. Here, we report on a novel, simplified differentiation method for forming functional osteoclasts from hiPSCs. The three-step protocol starts with embryoid body formation, followed by hematopoietic specification, and finally osteoclast differentiation. We observed continuous production of monocyte-like cells over a period of up to 9 weeks, generating sufficient material for several osteoclast differentiations. The analysis of stage-specific gene and surface marker expression proved mesodermal priming, the presence of monocyte-like cells, and of terminally differentiated multinucleated osteoclasts, able to form resorption pits and trenches on bone and dentine in vitro. In comparison to peripheral blood mononuclear cell (PBMC)-derived osteoclasts hiPSC-derived osteoclasts were larger and contained a higher number of nuclei. Detailed functional studies on the resorption behavior of hiPSC-osteoclasts indicated a trend towards forming more trenches than pits and an increase in pseudoresorption. We used hiPSCs from an autosomal recessive osteopetrosis (ARO) patient (BIHi002-A, ARO hiPSCs) with compound heterozygous missense mutations p.(G292E) and p.(R403Q) in CLCN7, coding for the Cl-/H+-exchanger ClC-7, for functional investigations. The patient's leading clinical feature was a brain malformation due to defective neuronal migration. Mutant ClC-7 displayed residual expression and retained lysosomal co-localization with OSTM1, the gene coding for the osteopetrosis-associated transmembrane protein 1, but only ClC-7 harboring the mutation p.(R403Q) gave strongly reduced ion currents. An increased autophagic flux in spite of unchanged lysosomal pH was evident in undifferentiated ARO hiPSCs. ARO hiPSC-derived osteoclasts showed an increased size compared to hiPSCs of healthy donors. They were not able to resorb bone, underlining a loss-of-function effect of the mutations. In summary, we developed a highly reproducible, straightforward hiPSC-osteoclast differentiation protocol. We demonstrated that osteoclasts differentiated from ARO hiPSCs can be used as a disease model for ARO and potentially also other osteoclast-related diseases. © 2021 The Authors. Objective: The objective of this study is to evaluate prostate-specific membrane antigen positron emission tomography-computed tomography (PSMA PET/CT)-based primary staging in exclusively D′Amico intermediate-risk prostate cancer (PCa) patients. Patients and Methods: We relied on the Braunschweig institutional database and retrospectively identified D′Amico intermediate-risk PCa patients who were administered to 68Ga-PSMA PET/CT-based primary staging prior to consecutive radical prostatectomy and extended lymph node dissection. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) for the detection of lymph node metastases were analyzed per-patient (n = 39), per-pelvic side (n = 78), and per-anatomic-region (external iliac artery and vein left/right vs. obturator fossa left/right vs. internal iliac artery left/right) (n = 203), respectively. Results: Sensitivity, specificity, PPV, and NPV per-patient were 20.0, 94.1, 33.3, and 88.9%, respectively. Sensitivity, specificity, PPV, and NPV per-pelvic-side were 16.7, 97.2, 33.3, and 93.3%, respectively. Sensitivity, specificity, PPV, and NPV per-anatomic-region were 16.7, 99.0, 33.3, and 97.5%, respectively. Conclusions: We recorded high rates of specificity and NPV for 68Ga-PSMA PET/CT-based primary staging in D′Amico intermediate-risk PCa patients. Conversely, the sensitivity and PPV were lower than anticipated. Larger and favorably prospective trials are needed to verify our results and to unravel possible bias from such smaller studies.

Die Konstitution der Lebens- und Wohnverhaltnisse beschrieb Friedrich Engels bereits in seinem Werk „Zur Lage der arbeitenden Klasse in England“. Davon ausgehend gibt das Kapitel eine aktuelle Verortung zur Relevanz der Thematik und deren historischen Hintergrunde. Dazu gehoren Einflussfaktoren wie die Bestimmung der Wohnqualitat sowie die Entwicklung und Bedeutung von Nachbarschaften fur die subjektive Lebensqualitat. Zudem wird der Frage nachgegangen, inwiefern eine interdisziplinare Herangehensweise in Form einer Kooperation aus Architektur und Soziologie fruchtbare Sozialraumanalysen gewahrleisten kann.

Der allgegenwartige demographische Wandel verscharft die Problematik des steigenden Pflegebedarfs und legt damit auch einen Fokus auf die Wohnsituation der alteren Bevolkerung. Um der Frage nachzugehen, welche Wohnformen als besonders auf das Alter ausgelegt beschrieben werden konnen, wird zunachst auf die aktuell vorliegende Wohnsituation alterer Menschen eingegangen. Auch welche Wunsche an das Wohnumfeld geausert und welche Wohnformen fur das Alter praferiert werden, wird mit Hilfe unterschiedlicher Erhebungen untersucht.

Zielsetzung des diesem Band zugrundeliegenden Lehrforschungsprojekt war es, mit Heavy Metal eine bestimmte Musikszene und mit Festivals eine bestimmte Art des Musikerlebens genauer zu betrachten und zu analysieren. Dabei sollten jedoch nicht musiktheoretische Uberlegungen – wie komplex ist dieser Musikstil? – oder betriebswirtschaftliche Aspekte – Event-Management ist ein beliebter Studiengang – im Mittelpunkt stehen, sondern immer eine soziologische Perspektive (vgl. dazu den einleitenden Text von Nico Richter in diesem Band). Es geht um die Beschreibung einer bestimmten Szene und Kultur und damit um ein in modernen Gesellschaften wahrscheinlich immer wichtiger werdendes Phanomen. Eher individualisierte und diversifizierte Gesellschaften haben eine Fulle von Lebensoptionen und bedingen sogenannte Bastelbiographien.

Die steigende Nachfrage nach Wohnraum, knappe Neubauflachen und auch die Attraktivitat der vorhandenen Architektur fuhren dazu, dass immer haufiger Funktionsgebaude zu Wohnraum umgewandelt werden. Als regionales Beispiel fur solche Gentrifizierungsentwicklungen kann das Bobinet-Areal in Trier-West angesehen werden. Das vorliegende Kapitel setzt sich zunachst mit der Transformation des ehemaligen Fabrikgelandes zum attraktiven Wohnquartier auseinander. Im weiteren Verlauf werden an Hand einer 2018 durchgefuhrten Befragung die Sozialstruktur der Bewohnenden, die im Viertel vorliegenden Wohnformen und die Wohnzufriedenheit sowie nachbarschaftlichen Interaktionen zwischen den Bewohnenden analysiert und auf Besonderheiten hin untersucht.

Loss-of-function mutations in the human coagulation factor 9 (F9) gene lead to hemophilia B. Here, we dissected the consequences and the pathomechanism of a non-coding mutation (c.2545A>G) in the F9 3’ untranslated region. Using wild type and mutant factor IX (FIX) minigenes we revealed that the mutation leads to reduced F9 mRNA and FIX protein levels and to lower coagulation activity of cell culture supernatants. The phenotype could not be compensated by increased transcription. The pathomechanism comprises the de novo creation of a binding site for the spliceosomal component U1snRNP, which is able to suppress the nearby F9 poly(A) site. This second, splicing-independent function of U1snRNP was discovered previously and blockade of U1snRNP restored mutant F9 mRNA expression. In addition, we explored the vice versa approach and masked the mutation by antisense oligonucleotides resulting in significantly increased F9 mRNA expression and coagulation activity. This treatment may transform the moderate/severe hemophilia B into a mild or subclinical form in the patients. This antisense based strategy is applicable to other mutations in untranslated regions creating deleterious binding sites for cellular proteins., Author summary The elucidation of the pathomechanisms of non-coding variants yields important insights into diseases as well as cellular processes causing the defect. Although these variants may account for the majority of phenotypic variation, only a minority of them can be explained mechanistically. The human coagulation factor 9 3’ UTR variant described here converts a non-essential sequence motif into a U1snRNP-binding site with deleterious effects on RNA 3’ end processing at the nearby poly(A) site. Poly(A) site suppression by U1snRNP was described before and it normally protects cellular mRNAs from premature termination. However, if misled by creation of a U1 site close the authentic poly(A) site as in the F9 3’ UTR, this nuclear surveillance mechanism results in the opposite. Since recognition by U1snRNP depends on sequence complementarity we were able to use antisense oligonucleotides to mask the mutant site and partially restored F9 mRNA levels. This antisense based strategy may be applicable to other variants in untranslated regions, which create deleterious binding sites for cellular proteins.

At present, there are only a few reliable findings on the ever-increasing number of doctors employed in outpatient care. Therefore, some results of a nationwide quantitative survey of persons in this occupational group will be presented and discussed here.The study is a standardized quantitative survey of physicians employed in the outpatient sector. The target population is represented by a disproportionately stratified sample from the databases of the Association of Statutory Health Insurance Physicians of the federal states. A total of 10,580 doctors were contacted and the response rate was 21.8%. The sample design not only allows descriptive analyses for small subpopulations to be carried out but also regional disparities to be taken into account.It can be shown that mainly young female doctors take up employment in the outpatient sector. 59% of doctors work part-time, the average weekly working time is 28.9 h. A medical office employs about 4 doctors and on average (median) about 30 people are treated per day.An important reason for the steady growth of this occupational group may be the flexibility of working hours and the possibility of part-time work, which in turn has a positive effect on the compatibility of family and career. Finally, yet importantly, this could be the reason why this occupational group seems to consist predominantly of young female doctors. Often, however, this kind of employment also represents a transitional model towards a private practice or - for former practice owners - into retirement. In general, however, being employed in outpatient care seems to be the desired "normal working model" for many doctors.Über die aktuell immer größer werdende Gruppe angestellter Ärztinnen und Ärzte im ambulanten Sektor liegen derzeit nur wenige gesicherte Befunde vor. Daher sollen hier einige Ergebnisse einer bundesweiten quantitativen Befragung von Personen dieser Berufsgruppe dargestellt und diskutiert werden.Bei der Studie handelt es sich um eine standardisierte quantitative Befragung von angestellten Ärztinnen und Ärzten im ambulanten Sektor. Da die angestrebte Grundgesamtheit eine disproportional geschichtete Stichprobe aus den Datenbanken der Kassenärztlichen Vereinigungen der Bundesländer darstellte und insgesamt 10 580 Ärztinnen und Ärzte angeschrieben wurden, beträgt die Responsequote 21,8%. Das Stichprobendesign erlaubt es nicht nur deskriptive Analysen für kleine Subpopulationen durchzuführen, sondern auch regionale Disparitäten in den Blick zu nehmen.Es lässt sich zeigen, dass es sich v. a. um junge Ärztinnen handelt, die eine Anstellung im ambulanten Sektor aufnehmen. 59% der angestellten Ärztinnen und Ärzte arbeiten in Teilzeit, die durchschnittliche Wochenarbeitszeit beträgt 28,9 Stunden. Eine durchschnittliche Praxis beschäftigt insgesamt 4 Ärzte oder Ärztinnen und es werden im Mittel (Median) circa 30 Personen am Tag behandelt.Ein wichtiger Grund für das stetige Wachstum dieser Berufsgruppe ist womöglich die Flexibilität der Arbeitszeitgestaltung und die Möglichkeit zur Teilzeitarbeit, welche sich wiederum positiv auf die Vereinbarkeit von Familie und Beruf auswirken. Nicht zuletzt scheint diese Berufsgruppe auch deshalb überwiegend aus jungen Ärztinnen zu bestehen. Oftmals stellt die Angestelltentätigkeit aber auch ein Übergangsmodell hin zu einer Niederlassung oder – für ehemalige Praxisinhaber – in den Ruhestand dar. Generell scheint die Angestelltentätigkeit in der Ambulanten Versorgung allerdings für viele das angestrebte „Normalarbeitsmodell“ zu sein.
