Die psychodynamischen Diagnoseinstrumente OPD (für Erwachsene) und OPD-KJ (für Kinder und Jugendliche) sind in der aktuell vorliegenden, zweiten Version mittlerweile im Rahmen der Richtlinien-Psychotherapie gut etabliert. Insbesondere in der psychiatrischen und psychotherapeutischen Behandlung von Kindern und Jugendlichen stellt der Einbezug des familiären Bezugssystems aber eine besondere Herausforderung dar, und oft fehlt es den psychodynamisch ausgebildeten Fachkräften an einem entsprechenden diagnostischen Modell. Diese Lücke soll durch die »OPD für Beziehungssysteme« geschlossen werden. Aktuell befindet sich dieses neue Instrument in Entwicklung. Die Achsen »Konflikt« und »Struktur« sind bereits ausgearbeitet und werden an dieser Stelle vorgestellt.

This study examined patients' personality traits as operationalized by the five-factor model in relation to early alliance and reduction of interpersonal distress through an intensive group treatment program for personality dysfunction. A sample of 79 consecutively admitted psychiatric outpatients with personality dysfunction who attended an 18-week intensive group treatment program completed the NEO Five-Factor Inventory at pretreatment, the Inventory of Interpersonal Problems at pre- and posttreatment, and the Edmonton Therapeutic Alliance Scale, a measure of the therapeutic alliance with the program therapist, at Session 5. Results indicated that patients who were relatively extraverted tended to rate the alliance with their program therapist higher and subsequently reported more improvement of interpersonal distress. The presence of a personality disorder did not moderate this mediation. Patients' extraversion likely promotes a bonding with the therapist and facilitates the interpersonal group work necessary for improvement. Assessing patients' level of extraversion before starting intensive group treatment might indicate which intervention strategies could be useful with that patient within the program frame.

We comment on the papers by Schönbrodt et al. (2022) and Gärtner et al. (2022) on responsible research assessment from the perspective of clinical psychology and psychotherapy research.

Zusammenfassung. Die Erfassung von Kindheitserfahrungen stellt einen wichtigen Baustein für Verständnis und Behandlung psychischer Belastungen im Erwachsenenalter dar. Dabei kommt der Erfassung protektiver Kindheitserfahrungen neben etablierten aversiven Erlebnissen eine wachsende Bedeutung zu. Ziel der vorliegenden Studie ist die Erstellung einer validen, reliablen und zeitökonomischen Kurzform des Fragebogens zur Erfassung aversiver und protektiver Kindheitserfahrungen (APK). Mithilfe des Ant-Colony Optimization (ACO) Algorithmus wurden an einer klinisch-psychotherapeutischen Stichprobe ( n = 701) aus den 59 Items der Langversion ein Set von 18 Items identifiziert und an einer nicht-klinischen Stichprobe ( n = 643) validiert. Die Kurzversion (APK-18) zeigte gute Reliabilität und Validität und unterschied anhand latenter Mittelwertunterschiede zuverlässig zwischen klinischen und nicht-klinischen Teilnehmenden. Einschränkungen bezüglich der Reliabilität ergaben sich bei 2 der insgesamt 8 Sub-Skalen, „Sonstige traumatische Ereignisse“ und „Trennungserfahrungen“. Mit der Kurzversion APK–18 liegt ein ökonomisches und valides Messinstrument vor, welches in klinische Routineerhebungen integriert werden kann.

Background: Compared to adult psychotherapy training, there is little knowledge about the use of standardized actor-patients (SPs) for the development of professional competencies for younger patient populations. Objective: We assessed the effectiveness of two university-based training programs for diagnostic as well as intervention skills in child and adolescent psychotherapy. Methods: Psychology students took part in two courses on the development of diagnostic competencies ( N = 59) and on intervention skills ( N = 51). We applied active and experience-based learning strategies, including exercises on self-awareness, mindfulness, peer role-play, and the use of SPs. Competence gain as well as a general evaluation of the seminars were assessed by self-report. Results: For both seminars, self-reported competence increased significantly with a large effect. The retrospective evaluation of learning success and acceptance ratings of the didactic formats were high. Conclusions: Experience-based trainings that include SPs are a feasible option for child and adolescent psychotherapy. The findings are discussed from a conceptual as well as a practical perspective.

OBJECTIVE: To determine the treatment effect of psychodynamic therapy for adolescents in comparison to normative developmental progression in two groups without treatment: healthy adolescents and adolescents with juvenile diabetes. RESEARCH DESIGN AND METHODS: In a 3-wave longitudinal study, n = 531 adolescents (n = 303 treated adolescents, n = 119 healthy, n = 109 with diabetes) and their parents filled out psychopathology questionnaires (Youth Self-Report [YSR] and Child Behaviour Checklist [CBCL]). Growth curve modeling (GCM) was used to examine within-person change in psychopathology while controlling for stable between-person differences. RESULTS: GCM analyses revealed significant within-person reductions in patients' YSR and CBCL (d = 1.02-1.99) at the end of treatment. When accounted for the control groups' developmental progression (d = .14-.94), patients' within-person change remained significantly higher (d = .48-.82). In all three groups, parents rated the severity of psychopathology significantly lower, and within-person change significantly higher than the adolescents him/herself. CONCLUSIONS: Psychodynamic therapy led to a significant symptom reduction in treated adolescents and was superior to development-related symptom changes occurring in the two control groups. Hence, clinically relevant symptoms in adolescents do not "grow out", but require psychotherapeutic treatment. Differences between adolescents and their parents in the evaluation of symptom severity and change require attention in psychotherapy treatment and research.

Zusammenfassung Hintergrund Durch die Einführung des neuen „Psychotherapiestudiums“ zum Herbst 2020 und die Wünsche bisheriger Ausbildungsteilnehmer an psychotherapeutischen Ausbildungsinstituten ist die Notwendigkeit gegeben, erfahrungsbasierte und kompetenzorientierte Lehre in das zukünftige Studium und die nachfolgende Weiterbildung werdender Psychotherapeuten zu integrieren. Material und Methode Orientiert am Kern-Zyklus, einem bedeutenden Modell zur Curriculumsentwicklung, werden die Entwicklung, Konzeptionalisierung und Implementierung eines longitudinalen Curriculums zur Vermittlung psychodynamischer Interventionskompetenzen (DYNAMIK-Curriculum) in die derzeitige Ausbildung zum psychologischen Psychotherapeuten vorgestellt. Ergebnisse Es wurde ein longitudinales Curriculum mit 8 Modulen zur Vermittlung allgemeiner und spezifischer psychodynamischer Interventionskompetenzen mithilfe von Peer-Rollenspielen und standardisierten Patienten entwickelt. Dieses stellt seit Mitte 2019 einen integralen Bestandteil der Ausbildung zum tiefenpsychologisch fundierten Psychotherapeuten am Heidelberger Institut für Psychotherapie (HIP) dar. Schlussfolgerung Das DYNAMIK-Curriculum ist der erste wichtige Schritt in Richtung der longitudinalen erfahrungsorientieren, praxisnahen Kompetenzorientierung in der derzeitigen Ausbildung und dem zukünftigen Studium mit nachfolgender Weiterbildung von psychologischen Psychotherapeuten. Es resultiert die Notwendigkeit zur Wirksamkeitsüberprüfung solcher Curricula und zur praktischen Prüfung von Interventionskompetenzen.

ZusammenfassungIn Coaching und Supervision ist die Arbeitsbeziehung zentraler Wirkfaktor. Zwischenmenschliches Geschehen ist aber auch eine Bühne für Störfaktoren, die es zu handhaben gilt. In diesem Beitrag wird das aus der Psychotherapieforschung stammende Konzept der Brüche der Arbeitsbeziehung und ihrer Reparatur vorgestellt. Hierzu gehen wir auf das „Rupture Resolution Rating System“ (3RS) ein, das Spannungsphänomene systematisiert, die Qualität einzelner Brüche veranschaulicht und für die Prozessforschung zugänglich macht. Anschließend wird das Phänomen der Brüche auf den Kontext von Coaching und Supervision übertragen und mit theoretischen und praktischen Implikationen für die Beratung abgerundet.

As part of a larger research project on understanding change in helping professions, this paper investigates into therapists’ requesting examples and their interactional and sequential contri-bution to clients’ change. Requesting examples by therapists in psychodiagnostic interviews explicitly or implicitly criticize the patient’s prior turn as insufficient, i.e. as unclear, vague, or as too general. Such a request opens a retro-sequence (Schegloff 2007) and in the following provides for a description that both helps clarify the semantic vagueness and evinces psychic or relational aspects of the topic at hand. While the patient’s insufficient presentation is initi-ated by a prior request of the therapist, the patient’s example presentation is regularly fol-lowed by the therapist’s summarizing comments or by further requests focusing on the pa-tient’s problem. Requesting examples thus are a particular case of requests that follow ‘ex-pandable responses’ as described by Muntigl & Zabala (2008); they follow the same sequential organization, yet, given that they make examples conditionally relevant, they are more specif-ic. With the help of this sequential organization both participants co-construct elements of common knowledge. Such an ‘interplay of understanding’ (Voutilainen & Peräkylä 2014) al-lows the therapist to pursue the overall aim of therapy, i.e. to increase the patients’ awareness of their distorted perceptions, and thus to pave the way for change. The data comprises of 16 videotaped first interviews following the manual of the Operationalized Psychodynamic Di-agnostics (OPD Task Force 2009). It was collected in cooperation with the Clinic for General Internal Medicine and Psychosomatic at the University Clinic of Heidelberg.

Different measures of attachment are usually weakly correlated. In a subsample of an RCT comparing short-term cognitive-behavioural therapy (CBT) and psychodynamic therapy (PDT), we examined the association between attachment and outcome using two attachment measures. The sample comprises 148 patients with social anxiety disorder who were treated in the SOPHO-NET trial. Pretreatment attachment was assessed using the Adult Attachment Prototype Rating (AAPR) and the Bielefeld Questionnaire of Client Expectations (BQCE). Regression models were used to predict the therapeutic alliance (HAQ) at session 8, the Liebowitz Social Anxiety Scale (LSAS) at the end of therapy and a 6-month follow-up. Attachment groups (secure, avoidant, and ambivalent) classified with the AAPR and the BQCE were not significantly correlated (Cohen's κ = 0.08). Only the BQCE was associated with the HAQ indicating avoidantly attached patients showing lower HAQ scores than securely attached (Cohen's d = 0.722). Regarding the AAPR, we found an interaction effect of treatment and attachment related to the post-treatment LSAS scores. Post hoc tests revealed that securely attached patients in CBT had lower scores than securely attached in PDT (d = 0.922) and, on a trend level, avoidantly attached in CBT (d = 0.782). We conclude that attachment-outcome associations are affected by the applied measure. The identified differential effects suggest that psychotherapists should adapt the interventions on the attachment of their patients.

We tested the hypothesis that structural integration, as assessed in the Operationalized Psychodynamic Diagnosis (OPD) system, and emotional intelligence (EI), as studied in personality psychology, might be closely related constructs at a general level, as both might assess general personality functioning. In three studies (n 1 = 166, n 2 = 204, n 3 = 349), we used a self-report measure of OPD structural integration and measures of Trait and Ability EI. Structural integration and Trait EI display very high correlation at general factor level (r = .77 - .82) and almost perfect latent correlation (r = .85 - .90). This correlation cannot be explained away by the general positivity of self-views or socially desirable responding. There is also substantial latent correlation between structural integration and Ability EI (r = .20 - .65). Results replicate over different samples from different countries and extend to the DSM-5 self-report personality functioning scale.

Level of personality functioning and attachment style as predictors of the successful referral to outpatient psychotherapyObjectives: Outpatient psychotherapy is a key element in the effective treatment of mental health problems. First results suggest that interpersonal problems lead to difficulties in receiving outpatient psychotherapeutic treatment. The relationship between these difficulties, attachment style, and the level of personality functioning is still unclear. Methods: We invited 1011 patients of a psychosomatic-psychotherapeutic university outpatient clinic to participate in the study. The clinical diagnoses according to ICD-10, as well as symptoms of depression (PHQ-D), and quality of life (SF-36) were recorded. Hypothesized predictors for the successful referral to outpatient therapy were patient age, availability of local outpatient treatment, number of ICD-10 diagnoses, the motivation for psychotherapy (FPTM), fear of stigmatization (Stig-9), level of personality functioning (OPD-SQ), and attachment style (ECR-RD). Results: We were able to catamnestically reassess n = 300 patients (67.3 % of patients initially referred to outpatient therapy). A smaller number of clinical diagnoses, greater availability of psychotherapeutic care and higher therapy motivation, as well as a lower level of personality functioning predicted the successful referral to outpatient psychotherapy, while the combination of impaired personality functioning and avoidant attachment style was a negative predictor. Conclusions: Contrary to expectations, patients with a lower level of personality functioning are more successful in receiving outpatient psychotherapy. However, patients with a combination of impaired personality functioning and a high degree of attachment avoidance run the risk of not asserting their need for treatment.

Zusammenfassung Hintergrund Menschen mit der Diagnose einer posttraumatischen Belastungsstörung (PTBS) sind oftmals mit sehr langen Wartezeiten zwischen Diagnose und Beginn einer leitliniengerechten traumafokussierten Behandlung konfrontiert. Um dieser Versorgungslücke zu begegnen, wurde eine Gruppenbehandlung mit 9 Sitzungen konzipiert, die den Fokus auf Stabilisierung und Selbstregulation legt, um entweder eine Traumabearbeitung in einer nachfolgenden Einzeltherapie zu erleichtern oder aber für Menschen mit komplexer PTBS eine zusätzliche Hilfe bei der Behandlung entsprechender Symptome zu ermöglichen. Methodik In einer Machbarkeitsstudie mit 5 Patientinnen mit der Diagnose einer PTBS nach DSM-IV wurden die Stimmung vor und nach jeder Sitzung sowie allgemeine und traumaspezifische Symptome vor und nach der Behandlung erfasst. Nach Behandlungsende und in einer Zweimonatskatamnese wurden Daten zu Verlauf und Akzeptanz erhoben. Ergebnisse Die Stimmung der Patientinnen hob sich im Verlauf der jeweiligen Sitzungen. Ebenso nahmen die Häufigkeit des selbstberichteten Übens und Anwendens der Übungen zu. Die Methode wurde insgesamt als sehr positiv eingeschätzt. Deskriptiv nahmen die Fähigkeit zur Selbstberuhigung ebenso wie strukturelle Fähigkeiten der Persönlichkeitsfunktion zu sowie Traumasymptomatik, Depressivität, Ängstlichkeit und interpersonelle Probleme ab. Diskussion Die Daten geben erste Hinweise auf die Machbarkeit und Akzeptanz einer Gruppenintervention zu Stabilisierung und Selbstregulation bei Frauen mit Traumafolgestörungen. Die Gruppenintervention hat das Potenzial, in der ambulanten Versorgung die Wartezeit auf eine traumafokussierte Einzeltherapie sinnvoll zu nutzen.

In Deutschland stellt die psychosomatische Rehabilitation neben der akutmedizinischen Versorgung eine feste Saule im Gesundheitswesen dar. Im Sinne einer gestaffelten Versorgung und begrenzter stationarer Behandlungszeiten sollen die Behandlungseffekte in ambulanten Nachsorgeangeboten in Gruppenformaten stabilisiert werden. Ungeachtet der weiten Verbreitung sind diese Nachsorgeangebote wenig beforscht. In einer naturalistischen Studie mit einem Pra-post-Design ohne Kontrollgruppe wurden insgesamt 261 Patienten in 23 psychosomatischen Nachsorgegruppen untersucht. Neben soziodemografischen Daten wurde die symptomatische Belastung zu insgesamt 3 Messzeitpunkten mithilfe des Klinisch-Psychologischen Diagnosesystem 38 (KPD-38) untersucht. Weiterhin wurde die Stichprobe hinsichtlich der Patienten, die die Masnahme komplett durchlaufen haben, und der Patienten, die die Masnahme vorzeitig beendeten, verglichen. In der Gesamtgruppe nahm die Symptomatik im Laufe der Gruppenbehandlung insgesamt signifikant und mit kleinen bis mittleren Effekten ab. Patienten mit regularem Abschluss der Masnahme profitierten am meisten im Gegensatz zu denen, die diese vorzeitig beendeten. Die Ergebnisse der Studie sind fur die Anwendung der ambulanten gruppenpsychotherapeutischen Nachsorge vielversprechend, jedoch sind weitere Studien zu Wirksamkeit und Pradiktoren der Veranderung notig.

The alliance is dyadic in its nature with both the patient and the therapist contributing. Relatively little is known about the effects of congruence between patient and therapist perception of alliance on treatment outcome. The current study investigated how patient and therapist agreement and disagreement about the alliance predict symptom severity over the course of long-term psychotherapy. We investigated N = 361 patients nested within N = 102 therapists longitudinally every 5th session across long-term treatment. Multilevel polynomial regression with response surface analysis was used to predict symptom severity five sessions later from congruence of the alliance ratings. Throughout treatment, patient and therapist agreement about stronger alliances significantly predicted lower subsequent patient-reported symptom distress. Patient and therapist disagreement was a marginally significant predictor of subsequent symptom distress. There was no significant difference in the effects of alliance agreement and disagreement on symptoms across time in long-term treatment. Findings support the importance of alliance agreement and disagreement as predictors of subsequent patient symptom severity. (PsycInfo Database Record (c) 2020 APA, all rights reserved).

Stress is associated with mental health problems and physical diseases. Results from basic research indicate that stress-induced dysregulations of bodily stress response systems might mediate these processes. In consequence, many programs to prevent or manage stress have been developed and evaluated using endpoints of these bodily stress systems. Over the last few years, emerging evidence from human and animal studies has indicated that stress also leads to changes in epigenetic signatures. The functional consequences of these changes, among them altered gene expression, have been shown to modulate the psychobiological stress response. Some recent studies now show that psychosocial interventions, such as stress prevention and stress management, can alter epigenetic processes. In this review, we will present an outline on (1) relevant epigenetic mechanisms, (2) the current literature on the association of stress and epigenetics, and (3) mechanisms through which psychological interventions might alter stress-related epigenetic markers.

Level of personality functioning (OPD-2) and the symptom severity of posttraumatic stress disorder - a cohort study Objectives: To investigate the affirmed correlation between the level of personality functioning (OPD-2, Axis IV "Structure") and the symptom severity of posttraumatic stress disorder based on clinical routine data was the main goal of this study. Method: Cross-sectional data was acquired between 2013 and 2016 using random samples of standardized questionnaires in the trauma outpatient clinic of the Department of Psychosomatics and Psychotherapy, University Hospital in Munster, Germany. The German version of the PDS (Ehlers et al. 1996) measured the symptoms and the severity of PTSD, level of personality functioning was assessed by the OPD-SFK (Ehrenthal et al. 2015) Results: Level of personality functioning in OPD-SFK and the severity of posttraumatic symptoms are significantly correlated to each other. The less the functioning of the psychic structure the more the symptom severity shows up. The results were confirmed with different sets of predictors in our statistical models. Conclusions: The level of the personality functioning (OPD-2) is related to the severity of posttraumatic symptoms. Whether a low-level personality functioning is a disposal for PTSD or more likely a result of a traumatic life event, should be discussed.

Die anstehenden Veranderungen der Psychotherapieausbildung fordern insbesondere von psychodynamischen Lehr- und Ausbildungsinstitutionen ein Umdenken und eine konsequente Erweiterung ihrer bisherigen Herangehensweise an Lehrformate. In Erganzung zu Literaturstudium, Selbsterfahrung und Supervision ist die Integration innovativer didaktischer Konzepte zur Verknupfung von klinischer Praxis mit Forschung, Konzeptentwicklung und Handlungskompetenz gefragt. Im Beitrag werden entsprechende Konzepte und Erfahrungen mit Beispielen aus der Praxis skizziert und diskutiert.

Zusammenfassung. Theoretischer Hintergrund. Trotz der gesundheitspolitischen Relevanz ist die Datenlage zu psychischen Erkrankungen und Belastungen von Studierenden in Deutschland im internationalen Vergleich eher schmal. Bestehende Untersuchungen mit größeren Stichproben leiden entweder an methodischen Problemen in der Erfassung von psychischen Störungen oder sind wenig kontrolliert. Fragestellungen: Um einen Überblick über das Ausmaß psychischer Symptome von Studierenden zu bekommen, wurden in einem Screening-Ansatz die Studierenden der Universität zu Köln im Rahmen einer Online-Befragung befragt. Zusätzlich zu den störungsbezogenen Ergebnissen wurden Daten zur Inanspruchnahme von Hilfeleistungen und studienrelevante Informationen und dem Vorliegen des Verdachts einer Persönlichkeitsstörung erfasst. Methodik. 44 299 Studierende der Universität zu Köln wurden zu einer Online-Befragung zum Thema Studieren mit psychischen Belastungen und Erkrankungen eingeladen. Zum Erfassen psychischer Beschwerden und Erkrankungen wurden der Patient-Health-Questionnaire (PHQ-D) und ein Screeningfragebogen für Persönlichkeitsstörungen (Persönlichkeitsstörungs-Screening Kurzform [PSS-K]), eingesetzt. Ergebnisse. Die Rücklaufquote der Befragung lag bei 11.18 % ( N = 4 952; bereinigte Rücklaufquote: N = 4 894; 11,04 %). Bei 53.6 % der Studierenden ( N = 2 657) zeigte sich mindestens ein syndromales Störungsbild (PHQ-D). Der Verdacht auf eine Störung der Persönlichkeitsentwicklung stellt sich bei 14.5 % der Respondentinnen und Respondenten ( N = 708). Schlussfolgerung. Die Ergebnisse bestätigen, dass psychische Belastungen unter Studierenden weit verbreitet sind. Neben behandlungsbedürftigen psychischen Störungen findet sich ein nicht geringer Prozentsatz subklinischer Zustände. Präventions- und Interventionsansätze sollten auf unterschiedliche Schweregrade der Belastung abgestimmt sein. Weiterhin stellen psychische Beschwerden einen Risikofaktor für Studienabbruchgedanken dar.

Hintergrund: Psychodynamische Psychotherapien spielen eine wichtige Rolle im deutschen Gesundheitssystem. Der Nachweis ihrer Lehrbarkeit bekommt vor dem Hintergrund des neuen Psychotherapiestudiums eine neue Bedeutung. Dies bezieht sich insbesondere auf erfahrungsbasierte di­daktische Modelle und auf das spezifische Üben von Interventionen. Hier ist bisher unklar, welche Aspekte kompetenz­orientierter psychodynamischer Lehre mit dem subjektiven Lernerfolg zusammenhängen. Methode: In einer Stichprobe mit 47 Kandidatinnen und Kandidaten eines Ausbildungsinstitutes wurden psychodynamische Interventionen mit ­Videofeedback, Peer-Rollenspielen und Simulationspatientinnen und -patienten unterrichtet. Der subjektive Kompetenzgewinn von Prä zu Post wurde in Bezug auf die Interventionen Klarifizieren, Konfrontieren und Deuten mithilfe von t-Tests abgebildet. Zudem wurden in einer multiplen linearen Regression verschiedene Prädiktoren für den subjektiven Lernerfolg getestet. Ergebnisse: Die subjektive Kompetenz nahm für die Interventionen Klarifizieren und Konfrontieren mit großen Effekten, für Deuten mit einem kleinen bis ­mittleren Effekt zu. Der Lernerfolg war korreliert mit einem antizipierten Transfer in den therapeutischen Alltag, einem positiven Erleben der Arbeit mit den Simulationspatientinnen und -patienten sowie mit hilfreich erlebtem Feedback durch die Dozentinnen und Dozenten. Schlussfolgerungen: Psychodynamische Interventionen sind mit erfahrungsbasierten und kompetenzorientierten Unterrichtsformaten lehrbar. Dabei ist wichtig, dass praxisorientierte Formate eine Lernumgebung schaffen, in der die Arbeit mit Simulationspatientinnen und -patienten und das Feedback als konstruktiv und hilfreich erlebt werden. Zukünftige Untersu­chungen sollten den Transfer in den Alltag sowie longitudinale Curricula in den Fokus nehmen.

Chronischer oder traumatischer Stress kann psychische Störungen und körperliche Erkrankungen begünstigen oder sogar verursachen. Ergebnisse aus der Grundlagenforschung zeigen, dass Stress zu einer Dysregulation wichtiger körperlicher Stresssysteme führen kann, welche diese Prozesse vermutlich vermitteln. Hierauf aufbauend wurden Interventionen zur Prävention oder zum Umgang mit Stress entwickelt, deren Wirkung mittels verschiedener Indikatoren dieser Stresssysteme evaluiert werden kann. In den letzten Jahren zeigten Ergebnisse aus Tier- und Humanstudien, dass sich Stress auch auf epigenetische Signaturen auswirken kann. Die funktionellen Konsequenzen dieser Veränderungen, darunter eine veränderte Expression der betroffenen Gene, wurden zudem als Modulatoren der psycho­biologischen Stressantwort beschrieben. Eine Reihe erster Pionierstudien zeigt nun, dass stressassoziierte epigenetische Prozesse durch psychologische Interventionen, darunter Stressprävention und Stressbewältigung, verhindert oder sogar umgekehrt werden können. In der vorliegenden Übersichtsarbeit werden (1) relevante epigenetische Mechanismen beschrieben, (2) eine Übersicht zum Zusammenhang von Epigenetik und Stress dargestellt und (3) erläutert, wie epigenetische Prozesse durch psychologische Interventionen beeinflusst werden könnten.

Das gesprochene Wort ist in der Richtlinienpsychotherapie das zentrale Vehikel therapeutischen Handelns – ganz besonders in Situationen, in denen kein Standardschema das Vorgehen bestimmt, wenn es z. B. um Verstehen, Aushandeln, Mustererkennen oder das Bearbeiten von Brüchen der therapeutischen Arbeitsbeziehung geht. Am Beispiel psychodynamischer Interventionen soll gezeigt werden, wie praxisorientierte Kompetenzentwicklung mithilfe innovativer Lehr- und Lernformate erleichtert werden kann.

Although the relationship between insecure attachment patterns and eating disorder (ED) psychopathology has repeatedly been demonstrated, the underlying mechanisms of this association are not fully understood. Therefore, the current study aimed to examine personality functioning, defined as an impairment in self and interpersonal functioning, as a mediator between attachment insecurity and ED psychopathology.In a representative population-based sample (N = 2508; age range 14-92 years) ED symptomatology, personality functioning, and attachment insecurity (anxiety and avoidance) were assessed. Besides descriptive uni-/bivariate analysis, path analysis was used to test a mediation model while controlling for the effects of age, gender, mental distress, and BMI.ED symptomatology was associated with lower levels of personality functioning (r = 0.22) and higher levels of attachment anxiety (r = 0.14) but did not correlate with attachment avoidance (r = 0.02). Path analysis revealed that personality functioning fully mediated the effect of attachment anxiety on ED symptomatology: The indirect effect via personality functioning (β = 0.04, p .001) accounted for 77% of the total effect. Fit indices were excellent. Sensitivity analyses revealed that the main results were mainly applicable to women and the middle age group.The present findings contribute to the growing body of research using dimensional conceptualizations of personality functioning, suggesting that it provides an informative, overarching framework for understanding and treating ED psychopathology. Findings indicate that underlying individual differences, e.g., with respect to insecure attachment configurations, have relevant implications for symptom manifestations. Potential clinical implications and avenues for future research are discussed.

Objective: The association between alliance and therapy outcome is one of the most investigated factors in psychotherapy research. However, even studies using advanced methods estimate effects over a specific time period (interval) between measurement occasions. Thus, it remains unknown how the magnitude and direction of effects depend on the considered time interval, resulting in limited comparability across studies. The current study examines the influence of time on the within-person relationship between alliance and symptom severity. Method: Alliance (WAI-SR) and symptom severity (SCL-K11) were assessed every fifth session in N = 650 patients receiving up to 100 weekly sessions (mode = 55; M = 41.03; SD = 27.23) of individual psychotherapy in a German outpatient clinic. Bivariate continuous-time (CT) structural equation modeling (SEM) was used to estimate within-person cross- and auto-effects. Results: Analysis revealed significant reciprocal within-person cross-effects with stronger relative effects of SCL-K11 on WAI-SR (a21) than vice versa (a12). CT analysis showed that both cross-lagged effects increased for longer time intervals with strongest effects for time intervals of about 40 sessions (a21 = -.47; a12 = -.19). Conclusions: Alliance and symptom severity showed a reciprocal relationship. Expanding current evidence, our analysis showed how the magnitude of these effects depends on the considered time interval. Applying CT-SEM on longitudinal data of the alliance outcome association complements current cross-lagged panel analysis and allows to compare results of studies which are based on different time intervals between measurement occasions. Is the influence of religiousness on fearful, depressive and somatic symptoms and psychic traumatization overestimated? A representative cross-sectional studyObjectives: The aim of this study is to prove if religious faith, spirituality and religious praxis are joined with lower depression, anxiety and physical pain-level and if subjects with traumatic experiences report more spirituality. Methods: In this consecutive study, 2508 adults of a representative sample in Germany were interviewed about religious faith and spirituality in relation to depression, anxiety, physical disorders as well as traumatic experiences. Results: Unlike our hypotheses people who are charged with mental (PHQ-4; Lowe et al. 2010) or physical disorders (GBB_8; Kliem et al. 2017) report more spirituality and more private religious/spiritual praxis than people without mental or physical problems. As expected people with traumatic experiences in their childhood (CTS; Grabe et al. 2012) describe significantly more spirituality than people without these experiences. Conclusions: Other than expected people with more mental or physical disorders report more spirituality and more private religious/spiritual praxis. It is to discuss if spirituality is less a protective factor for mental or physical disorders than disorders activate to look for spirituality and private religious/spiritual praxis.

Objective We aimed to investigate change in self-esteem through intensive group treatment for personality dysfunction, by exploring: (a) the relationship between patients' experience of therapeutic alliance and improvement in self-esteem during treatment, including patients' quality of object-relations (QOR) as a possible moderator; and (b) the association between improvement in self-esteem during treatment, and depressive symptoms 9 months later. Method Eighty patients with personality dysfunction, consecutively enrolled in a group-oriented treatment program, were assessed at pretreatment, posttreatment, and 9 months follow-up. Results Especially for patients with lower QOR, alliance predicted self-esteem change during treatment. In addition, change in self-esteem during treatment predicted follow-up depression severity, even when controlling for within-treatment symptom change. Conclusions Patients with impoverished inner relational representations may benefit more from a secure alliance in terms of improving their self-esteem. Change in self-esteem may also be important in preventing relapse of depressive symptoms in people with personality dysfunction.

Nicht zuletzt vor dem Hintergrund des Entwurfs einer Reform der Psychotherapieausbildung steht die Lehrbarkeit psychodynamischer Kompetenzen im besonderen Fokus der Aufmerksamkeit. Spezifische didaktische Ansatze konnen helfen, psychoanalytisch begrundete Interventionsstrategien erfahrbar und damit auf eine Weise zuganglich zu machen, die die klassische Trias von Literaturstudium, Selbsterfahrung und Supervision erganzt. In der vorliegenden Arbeit werden zunachst Hintergrunde fur die Bedeutsamkeit erfahrungsbasierten Lernens beschrieben, bevor exemplarisch zwei universitare Seminarformate dargestellt und mit Daten zum subjektiven Kompetenzzuwachs der teilnehmenden Studierenden unterlegt werden. Zum Abschluss werden Voraussetzungen, Moglichkeiten und Grenzen dieser Form der erfahrungsbasierten Lehre psychodynamischer Interventionen diskutiert.

Traumafolgestörungen werden oftmals als stressassoziierte seelische Erkrankungen verstanden. Neben den psychischen Symptomen bietet die Stressforschung wichtige Einblicke in biopsychologische Veränderungen der Traumafolgestörungen, die über das hinausgehen, was von Patientinnen und Patienten berichtet werden kann. Der Beitrag stellt Stressmodelle und zentrale Forschungsbefunde vor und diskutiert Fragen der Reversibilität veränderter Stressachsenregulation.

This study examines emotional experience in major depressive disorder (MDD) with and without comorbid borderline personality disorder (BPD). It investigates if depression severity or personality functioning mediates group differences and which aspects of emotional experience change during psychotherapy. The emotional experience of MDD-BPD patients (n = 44) was compared to MDD-only patients (n = 35) before and after multimodal short-term psychotherapy. Emotions were classified based on valence and an active/passive polarity. MDD-BPD patients exhibited more active-negative emotions. This group difference was mediated by the level of personality functioning, but not by depression severity. Although passive-negative emotions decreased and positive emotions increased during therapy, there was no significant change in active-negative emotions. The two patient groups did not significantly differ in the change of emotional experience. Lower levels of personality functioning in depressed patients with BPD are associated with a broader spectrum of negative emotions, specifically more active-negative emotions.

Operationalized Psychodynamic Diagnosis - Structure Questionnaire (OPD-SQ) in Youth: First Results on Reliability and Validity The OPD structure axis questionnaire (OPD-SQ) demonstrated good empirical findings and usability in therapeutic work. Therefore, the aim of this study was to verify the OPD-SQ for an adolescent sample in order to further implement the questionnaire as an economic instrument in clinical childhood and adolescence psychiatry routine. The overall sample consisted of 180 adolescent inpatients (72.8 % female) and 152 pupils (57.9 % female). In order to test reliability, Cronbach's alpha and discriminatory power were assessed. To test validity, YSR11-18 data was collected for the overall sample, and further data (AIDA, LoPF Q12-18, SKID-II, WISC-IV) was collected for the inpatient-sample. Cronbach's alpha was satisfactory for overall OPD-SQ, and all dimensions and sub-dimensions. The differences in mean values between inpatients and pupils were as expected. When controlled for overall symptom severity, gender differences lost significance. Adolescents with one or more SCID-II diagnoses had significantly lower psychic structure. Both construct-related questionnaires (AIDA, LoPF Q12-18) correlated high with OPD-SQ, no correlations were found for psychic structure and intelligence. OPD-SQ proved to be well applicable for the use on adolescents and can be implemented as an economic instrument for the measure of psychic structure in clinical adolescent psychiatry routine.
