1. Plastic-surgical reconstruction of the lower extremity in senior patients

Abstract: ZusammenfassungDer Anteil an Patienten jenseits der 7. Lebensdekade in der Bevölkerung steigt weltweit, insbesondere in den hochentwickelten Ländern. Der Bedarf an komplexen Rekonstruktionen der unteren Extremitäten nach Trauma, Tumor oder Infektion in dieser Altersgruppe nimmt zu. Die Rekonstruktion von Weichteildefekten an der unteren Extremität sollte nach dem Prinzip der plastisch-rekonstruktiven Stufenleiter resp. des Aufzugs erfolgen, um schmerzfreies und sicheres Stehen und Gehen zu erhalten. Insbesondere beim älteren Patienten sind sorgfältige multidisziplinäre Planung, gezielte präoperative Beurteilung und Optimierung von Komorbiditäten, wie z. B. Diabetes, Mangelernährung oder pathologischen Gefäßveränderungen, sowie ein altersentsprechendes perioperatives Management erforderlich. Durch die Umsetzung dieser Prinzipien können ältere und hochbetagte Patienten ihre Mobilität und ihre Autonomie, die für eine hohe Lebensqualität entscheidend sind, beibehalten.

5. Carotenoids and retinoids in the gonad of brood-stock pikeperch: accumulation during vitellogenesis and influence on egg quality in farmed pikeperch Sander lucioperca

Abstract: Carotenoids are determinants of reproductive fitness and egg quality. Here we studied the accumulation of astaxanthin (AX), canthaxanthin (CA) zeaxanthin (ZX), lutein (LU), retinol (RX) and dehydroretinol (DR) during vitellogenesis comparing previtellogenic and vitellogenic pikeperch (Sander lucioperca) eggs (n = 5 each), as well as selected tissues (liver, fat and muscles) in first süawning females (1176-1450 g). Futhermore, we compared egg batches with high (88-99% hatching rate, n = 5) or low (40-67% hatching rate, n= 5) egg quality. Vitellogenic follicles revealed higher concentrations of DR, RX, ZX and LU compared to previtellogenic follicles. Neither CA nor AX was detectable. In parallel, DR and RX were mobilized in the liver. In adipose and muscle tissue, comparing previtellogenic and vitellogenic females, no significant differences in carotenoid/retinoid content were observed. In high quality egg batches, both DR and RX were increased. LU was lower in high quality than in low quality eggs. In a conclusion, the amount of retinoids seems suboptimal in low quality egg batches and increased DR and RX are desirable in pikeperch. Since hypervitaminosis of retinoids can be problematic though, supplementation of the food with carotenoids, which can serve as precursors for retinoids, has to be carried out carefully.

Abstract: ZusammenfassungDeutsch Brustkrebs und die damit verbundenen chirurgischen Eingriffe haben grosse Auswirkungen auf die Lebensqualität der betroffenen Patientinnen. Die Studienlage zeigt, dass der Verlust der Brust und die damit verbundene Veränderung des Körperbildes für viele Frauen grosse Auswirkungen auf das weibliche Selbstverständnis haben. Das hat einen starken negativen Einfluss auf die Lebensqualität der Betroffenen. Mit moderner klinischer Outcome Forschung unter Einbezug der Behandlungsergebnisse aus der Patientinnenperspektive in Form von Patient-reported Outcome Measures (PROMs) lassen sich die Zufriedenheit und Lebensqualität von Betroffenen messen, analysieren und vergleichen. Diese Übersichtsarbeit durchleuchtet inwiefern die Wiederherstellung der Brust für viele Frauen eine Verbesserung der Lebensqualität bedeutet und wie die unterschiedlichen Rekonstruktionsmethoden in ihrem Einfluss auf die Lebensqualität und Zufriedenheit variieren. Ausserdem wird die Wichtigkeit der standardisierten Erfassung und Analyse von PROMs in Kombination mit objektiven klinischen Daten sowie deren Integration den Behandlungsprozess diskutiert und die Implementierungsmöglichkeiten aufgezeigt. Eine solche systematische Erfassung ermöglicht die Anlage von Datenbanken und Registern, deren Auswertungen Informationen bereitstellen, die für wissenschaftliche, wie klinische Zwecke verwendbar sind. So lassen sich, basierend auf den Forschungsergebnissen, prognostische Modelle kreieren und Behandlungsergebnisse in Vergleichsstudien untersuchen, die klinische Entscheidungsfindungen und Qualitätskontrollen erleichtern.

7. Generation of a Bone Organ by Human Adipose‐Derived Stromal Cells Through Endochondral Ossification

Abstract: Recapitulation of endochondral ossification (ECO) (i.e., generation of marrow‐containing ossicles through a cartilage intermediate) has relevance to develop human organotypic models for bone or hematopoietic cells and to engineer grafts for bone regeneration. Unlike bone marrow‐derived stromal cells (also known as bone marrow‐derived mesenchymal stromal/stem cells), adipose‐derived stromal cells (ASC) have so far failed to form a bone organ by ECO. The goal of the present study was to assess whether priming human ASC to a defined stage of chondrogenesis in vitro allows their autonomous ECO upon ectopic implantation. ASC were cultured either as micromass pellets or into collagen sponges in chondrogenic medium containing transforming growth factor‐β3 and bone morphogenetic protein‐6 for 4 weeks (early hypertrophic templates) or for two additional weeks in medium supplemented with β‐glycerophosphate, l‐thyroxin, and interleukin1‐β to induce hypertrophic maturation (late hypertrophic templates). Constructs were implanted in vivo and analyzed after 8 weeks. In vitro, ASC deposited cartilaginous matrix positive for glycosaminoglycans, type II collagen, and Indian hedgehog. Hypertrophic maturation induced upregulation of type X collagen, bone sialoprotein, and matrix metalloproteinase13 (MMP13). In vivo, both early and late hypertrophic templates underwent cartilage remodeling, as assessed by MMP13‐ and tartrate‐resistant acid phosphatase‐positive staining, and developed bone ossicles, including bone marrow elements, although to variable degrees of efficiency. In situ hybridization for human‐specific sequences and staining with a human specific anti‐CD146 antibody demonstrated the direct contribution of ASC to bone and stromal tissue formation. In conclusion, despite their debated skeletal progenitor nature, human ASC can generate bone organs through ECO when suitably primed in vitro. Significance Recapitulation of endochondral ossification (ECO) (i.e., generation of marrow‐containing ossicles through a cartilage intermediate) has relevance to develop human organotypic models for bone or hematopoietic cells and to engineer grafts for bone regeneration. This study demonstrated that expanded, human adult adipose‐derived stromal cells can generate ectopic bone through ECO, as previously reported for bone marrow stromal cells. This system can be used as a model in a variety of settings for mimicking ECO during development, physiology, or pathology (e.g., to investigate the role of BMPs, their receptors, and signaling pathways). The findings have also translational relevance in the field of bone regeneration, which, despite several advances in the domains of materials and surgical techniques, still faces various limitations before being introduced in the routine clinical practice.

8. Knochenrekonstruktion an der oberen Extremität mittels eines freien vaskularisierten Knochentransplantates aus der medialen Femurkondyle ohne Berücksichtigung des Skaphoids

Abstract: Zusammenfassung Ziel Diese Studie untersucht, ob das vaskularisierte Knochentransplantat aus der medialen Femurkondyle (MFC) auch erfolgreich bei kleinen (< 4 cm) Knochendefekten an der oberen Extremität eingesetzt werden kann, die nicht das Skaphoid betreffen. Patienten und Methoden Achtmal wurde bei 7 Patienten mit einem Alter zwischen 28 und 66 Jahren ein vaskularisiertes Knochentransplantat aus der MFC zur Rekonstruktion von Knochendefekten, die nicht das Kahnbein betrafen, an der oberen Extremität eingesetzt. Knochenheilung, Komplikationen, Hebemorbidität und die Patientenzufriedenheit wurden erfasst. Die Nachuntersuchungszeit betrug 3 bis 40 Monate. Die Indikationen für die Rekonstruktion mittels freier MFC waren 2 trotz mehrerer Voroperationen nicht geheilte distale Radiusfrakturen, ein ossärer Defekt am distalen Radius nach Fraktur mit radioskaphoidaler Arthrose, ein akutes, komplexes Trauma mit Verlust von Anteilen des Karpus und des distalen Radius bei Explosionsverletzung, eine Rekonstruktion des ersten Mittelhandknochens (MHK) nach Resektion eines Riesenzelltumors und 3 Arthrodesen zwischen dem 1. und 2. MHK als Rettungsoperation nach mehrfachen Operationen bei Daumensattelgelenksarthrose. Ergebnisse Vier der 8 Knochentransplantate zeigten eine regelrechte Heilung; 3-mal war sie mit 7, 8 und 9 Monaten verzögert. Einmal zeigte sich nur eine partielle Knochenheilung. Zweimal kam es zu einem revisionsbedürftigen Hämatom an der Entnahmestelle. Die Patientenzufriedenheit war mit Schmerzfreiheit in 7 der 8 Fälle in Anbetracht der Komplexität der Fälle zufriedenstellend. Schlussfolgerung Das freie vaskularisierte Knochentransplantat aus der MFC kann erfolgreich ohne Längenverlust zur Rekonstruktion bei Knochendefekten an der oberen Extremität eingesetzt werden, die nicht das Kahnbein betreffen und kleiner als 4 cm sind.

9. Osteomyelitis and antibiotic treatment in patients with grade IV pressure injury and spinal cord lesion—a retrospective cohort study

Abstract: Retrospective cohort study.To analyze characteristics and treatment of osteomyelitis (OM) in the treatment of grade IV pressure injury (PI) in patients with spinal cord injury/disorder (SCI/D) following the Basel Decubitus Concept.Acute care and rehabilitation clinic specialized in SCI/D.Patients with SCI/D were admitted for grade IV PI treatment between 1st January 2010 and 28th February 2015. Patients, SCI/D, and PI characteristics were collected from chart reviews. Descriptive statistics and differences between groups with and without OM were evaluated.In total, 117 patients (87 male, 30 female) with 130 PI grade IV were included. In 95 patients (81%), OM was diagnosed histologically. In 87 cases, more than one bacterial species was involved. Out of 49 different bacterial species, Enterococcus faecalis and Staphylococus aureus were most frequently observed. Amoxicillin/clavulanic acid and ciprofloxacin were the most frequently used out of 24 different antibiotics. Length of antibiotic treatment varied between8 days and91 days with 31 patients receiving antibiotics for about 8 weeks. Complications occurred in all groups of antibiotic duration. Having a paraplegia, no OM and sacral PI was associated with increased complication rates, but the number of patients did not allow comprehensive risk factor analysis.Because the variety of patients concerning SCI/D, PI, and OM characteristics did not show a conclusive relation between length of antibiotic treatment and complication rates, the development of a subgroup specific treatment concept for PI in patients with SCI/D would be favorable to further optimize antibiotic treatment.

Abstract: The Cherenkov Telescope Array Observatory (CTAO) consists of three types of telescopes: large-sized (LST), mediumsized (MST), and small-sized (SST), distributed in two observing sites (North and South). For the CTA South "Alpha Configuration" the construction and installation of 37 (+5) SST telescopes (a number that could increase up to 70 in future upgrades) are planned. The SSTs are developed by an international consortium of institutes that will provide them as an in-kind contribution to CTAO. The SSTs rely on a Schwarzschild-Couder-like dual-mirror polynomial optical design, with a primary mirror of 4 m diameter, and are equipped with a focal plane camera based on SiPM detectors covering a field of view of ~9°. The current SST concept was validated by developing the prototype dual-mirror ASTRI-Horn Cherenkov telescope and the CHEC-S SiPM focal plane camera. In this contribution, we will present an overview of the SST key technologies, the current status of the SST project, and the planned schedule.

13. [Bone Reconstruction in the Upper Extremity other than the Scaphoid with the free vascularized Medial Femoral Condyle]

Abstract: To evaluate whether a vascularized bone graft from the medial femur condyle (MFC) can successfully be used to reconstruct small bone defects (4 cm) on the upper extremity other than the scaphoid.In 7 patients at the age of 28 to 66 years 8 vascularized bone grafts from the MFC were used to reconstruct bone defects on the upper extremity other than the scaphoid. Bone healing, complications, donor side morbidity, and patient´s satisfaction were evaluated. Follow-up was 3 to 40 months. Indications were: two nonunion of the distal radius in spite of several surgical procedures, one defect of the distal radius following a radius fracture with osteoarthritis of the radiocarpal joint, one acute trauma with partial loss of the carpus and radius due to an explosion injury, reconstruction of the first metacarpal (MC) following resection of a giant cell tumour, and three fusions between the 1There was a regular bony healing in 4 of the 8 cases; delayed bony healing was seen in three cases after 7, 8, and 9 months. In one case there was only a partial bony healing. Two donor side haematomas required surgical revision. Abstract: Highly extended gamma-ray emission around the Geminga pulsar was discovered by Milagro and verified by HAWC. Despite many observations with Imaging Atmospheric Cherenkov Telescopes (IACTs), detection of gamma-ray emission on angular scales exceeding the IACT field-of-view has proven challenging. Recent developments in analysis techniques have enabled the detection of significant emission around Geminga in archival data with H.E.S.S.. In 2019, further data on the Geminga region were obtained with an adapted observation strategy. Following the announcement of the detection of significant TeV emission around Geminga in archival data, in this contribution we present the detection in an independent dataset. Abstract: The realtime follow-up of neutrino events is a promising approach to search for astrophysical neutrino sources. It has so far provided compelling evidence for a neutrino point source: the flaring gamma-ray blazar TXS 0506+056 observed in coincidence with the high-energy neutrino IceCube-170922A detected by IceCube. The detection of very-high-energy gamma rays (VHE, $\mathrm{E} > 100\,\mathrm{GeV}$) from this source helped establish the coincidence and constrained the modeling of the blazar emission at the time of the IceCube event. The four major imaging atmospheric Cherenkov telescope arrays (IACTs) - FACT, H.E.S.S., MAGIC, and VERITAS - operate an active follow-up program of target-of-opportunity observations of neutrino alerts sent by IceCube. This program has two main components. One are the observations of known gamma-ray sources around which a cluster of candidate neutrino events has been identified by IceCube (Gamma-ray Follow-Up, GFU). Second one is the follow-up of single high-energy neutrino candidate events of potential astrophysical origin such as IceCube-170922A. GFU has been recently upgraded by IceCube in collaboration with the IACT groups. Abstract: Primordial Black Holes are expected to be formed in the early Universe by the gravitational collapse of overdense regions, among other mechanisms. They are also expected to loose their mass over time by the Hawking radiation process. As the rates of this radiation increase with temperature, the PBH evaporation should result in a violent explosion. The current upper limits on explosion rates are on the order of 10$^{4}$ − 10$^{5}$pc$^{-3}$yr$^{-1}$. In this contribution we'll present the results of a search for TeV γ-ray burst within timescale of few seconds, using nearly 5000 hours of H.E.S.S. data. The search algorithm and statistical estimation strategy will be presented as well as cosmological implications of this measurement.

Abstract: Extreme high synchrotron peaked blazars (EHBLs) are amongst the most powerful accelerators found in nature. Usually the synchrotron peak frequency of an EHBL is above $10^{17}\,$Hz, i.e., lies in the range of medium to hard X-rays making them ideal sources to study particle acceleration and radiative processes. EHBL objects are commonly observed at energies beyond several TeV, making them also powerful probes of gamma-ray absorption in the intergalactic medium. During the last decade, several attempts have been made to increase the number of EHBL detected at TeV energies and probe their spectral characteristics. Here we report new detections of EHBLs in the TeV energy regime, each at a redshift of less than 0.2, by the High Energy Stereoscopic System (H.E.S.S.). Also, we report on X-ray observations of these EHBLs candidates with Swift XRT. Abstract: While the youngest known supernova remnants (SNRs), such as Cassiopeia A (Cas A), have been proven to be able to accelerate cosmic rays (CRs) only up to ∼ 10$^{14}$ eV at their present evolutionary stages, recent studies have shown that particle energies larger than a few PeV (10$^{15}$ eV) could be reached during the early stages of a core-collapse Supernova (cc-SN), when the high-velocity forward shock expands into the dense circumstellar medium (CSM) shaped by the stellar progenitor wind. Such environments, in particular the type IIn SNe whose progenitors may exhibit mass loss rates as high as 10$^{-2}$ M$_{\bigodot}$ yr$^{-1}$, could thus lead to γ-ray emission from $\pi ^{0}$ decay in hadronic interactions, potentially detectable with current Cherenkov telescopes at very-high energies. Such a detection would provide direct evidence for efficient acceleration of CR protons/nuclei in supernovae, and hence new insights on the long-standing issue of the origin of Galactic Cosmic Rays. In that context, the High Energy Stereoscopic System (H.E.S.S.) has been carrying out a Target of Opportunity program since 2016 to search for such an early very-high-energy γ-ray emission towards nearby core-collapse supernovae and supernova candidates (up to ∼ 10 Mpc), within a few weeks after discovery. After giving an overview of this H.E.S.S. Target of Opportunity program, we present the results obtained from the July 2019 observations towards the transient AT2019krl, originally classified as a type IIn supernova, which occurred in the galaxy M74 at ∼ 9.8 Mpc. Although its nature still remains unclear, the derived H.E.S.S. constraints on this transient are placed in the general context of the expected VHE γ-ray emission from core-collapse supernovae.

Abstract: The majority of the active galactic nuclei (AGN) detected at very-high-energies above 100 GeV belong to the class of blazars with a small angle between the jet-axis and the line-of-sight. Only about 10 percent of the gamma-ray AGN are objects with a larger viewing angle resulting in a smaller Doppler boosting of the emission. Originally, it was believed that gamma-ray emission can only be observed from blazars and those are variable in its brightness. Instead, the last years have shown that non-blazar active galaxies also show a fascinating variability behaviour which provide important new insights into the physical processes responsible for the gamma-ray production and especially for flaring events. Here, we report on the observation of gamma-ray variability of the active galaxy PKS 0625−354 detected with the H.E.S.S. telescopes in November 2018. The classification of PKS 0625−354 is a still matter of debate. The H.E.S.S. measurements were performed as part of a flux observing program and showed in the first night of the observation a detection of the object with > 5σ. A denser observation campaign followed for the next nine nights resulting in a decrease of the gamma-ray flux. Those observations were accompanied with Swift in the X-ray and UV/optical band allowing for the reconstruction of a multi-band broad-band spectral energy distribution. We will discuss the implications of the gamma-ray variability of the object.

Abstract: The presence of dark matter (DM) is suggested by a wealth of astrophysical and cosmological measurements. However, its underlying nature is yet unknown. Among the most promising candidates are weakly interacting massive particles (WIMPs): particles with mass and coupling strength at the electroweak scale and thermally produced in the early universe have a present relic density consistent with that observed today. WIMP self-annihilation would produce Standard Model particles including gamma-rays, which have been long-time recognized as a prime messenger to indirectly detect dark matter signals. The centre of the Milky Way is predicted as the brightest source of DM annihilations. The H.E.S.S. collaboration is currently performing a survey of the inner region of the Milky Way, the Inner Galaxy Survey (IGS), intended to achieve the best sensitivity to faint and diffuse emissions in a region of several degrees around the Galactic Centre. We analyzed 2014-2020 observations taken with the five-telescope array to search for a DM annihilation signal. With the current dataset of about 550 hours, we found no significant excess and therefore derived strong constraints on the velocity-weighted annihilation cross-section. TeV thermal WIMPs can be probed in different annihilation channels.

Abstract: Language constitutes the arena of political debate and is therefore itself the object of political struggle. But what language do populists speak? Existing research on this subject attests to its importance without being able to substantiate this empirically. This study examines the election programmes of the parties and the first ‘general debate’ in the 19th German Bundestag in order to understand what is really specific to populists’ use of language. Starting from a broad understanding of language, the author develops the theoretical concept of party language and applies a modified variant of linguistic multilevel discourse analysis. In the end, he identifies six characteristics of the language of populists.

Previously, the H.E.S.S. Collaboration has reported about very extended $\gamma$-ray emission around Westerlund 1, a massive young stellar cluster in the Milky Way. In this contribution we present an updated analysis that employs a new analysis technique and is based on a much larger data set, allowing us to constrain better the morphology and the energy spectrum of the emission. The analysis technique used is a three-dimensional likelihood analysis, which is especially well suited for largely extended sources. Abstract: 37th International Cosmic Ray Conference, ICRC 2021, Berlin, online, Germany, 12 Jul 2021 - 23 Jul 2021; Proceedings of Science / International School for Advanced Studies (ICRC2021), 805 (2021). doi:10.22323/1.395.0805, Kepler��s supernova remnant (SNR) which is produced by the most recent naked-eye supernova in our Galaxy is one of the best studied SNRs, but its gamma-ray detection has eluded us so far. Observations with modern imaging atmospheric Cherenkov telescopes (IACT) have enlarged the knowledge about nearby SNRs with ages younger than 500 years by establishing Cassiopeia A and Tycho��s SNRs as very high energy (VHE) gamma-ray sources and setting a lower limit on the distance to Kepler��s SNR. This SNR is significantly more distant than the other two and expected to be one of the faintest gamma-ray sources within reach of the IACT arrays of this generation. We report strong evidence for a VHE signal from Kepler��s SNR based on deep observations of the High Energy Stereoscopic System (H.E.S.S.) with an exposure of 152 hours, including 122 hours accumulated in 2017-2020. Abstract: In addition to outcome assessments, cost effectiveness of surgical treatments becomes increasingly important. Both, insurance companies and hospital administrations aim for short and efficient procedures to reduce costs.Microsurgical procedures are often surpassing traditional treatment options in terms of function and aesthetics. However, they are more expensive as they require a high level of surgical expertise, more theatre capacity and longer inpatient treatment. Adequate reimbursement is mandatory, if we want to continuously perform these procedures with the best possible quality and outcome. To cover the case-related expenses of each specialty, multidisciplinary procedures require appropriate distribution of reimbursements to each department.The main diagnosis as well as all complications and relevant comorbidities should be documented to obtain the correct DRG. The additional financial benefit of a microsurgical procedure in a multidisciplinary case can be calculated by specifying the procedural increment in pay. Therefore, a fair distribution of revenues to each participating department should be performed. Different models exist, which lead to a benefit in compensation for all departments. Unfortunately, distribution of resources is still insufficiently managed in many hospitals, which hampers high quality multidisciplinary microsurgical procedures. Still, picking the best possible procedure for our patients, independently of financial incentives, is of utmost importance.Im Zuge des zunehmenden Kostendrucks im Gesundheitssystem werden Therapien neben ihrer Qualität zunehmend auch hinsichtlich ihrer Wirtschaftlichkeit bewertet. Mikrochirurgische Eingriffe haben sich vor allem im Langzeitverlauf in vielen Bereichen als funktionell und ästhetisch überlegen gezeigt. Im Vergleich zu Alternativeingriffen sind sie aber auf Grund ihrer Komplexität sowie langen Operationszeiten und stationären Verweildauern mit hohen Kosten verbunden. Damit diese langfristig in hoher Qualität angeboten werden können, muss entsprechend von Seiten der Kostenträger, insbesondere auch bei interdisziplinären Eingriffen, eine adäquate, kostendeckende Vergütung erfolgen. Um einen korrekten DRG-Erlös zu erhalten sollten daher neben der Hauptdiagnose auch die relevanten Nebendiagnosen und Prozeduren richtig und vollständig dokumentiert werden. Auch der finanzielle Mehrwert eines interdisziplinären Eingriffs lässt sich durch die Erhöhung des Erlöses mit mikrochirurgischem Operationsanteil errechnen.Zwischen den Kliniken eines Krankenhauses sollte daher eine interdisziplinäre Erlösaufteilung stattfinden. Für eine faire und transparente interdisziplinäre Leistungsverrechnung existieren mehrere Modelle, mit denen alle beteiligten Kliniken einen Mehrwert erwirtschaften können. Leider bestehen an vielen Krankenhäusern unzureichende interdisziplinäre Vergütungsmodelle, welche eine qualitativ hochwertige, kostendeckende Patientenversorgung mit mikrochirurgischen Eingriffen erschweren. Ungeachtet des zunehmenden Kostendrucks und nicht medizinischer wirtschaftlicher Aspekte muss unser Handeln als Arzt immer die bestmögliche Patientenversorgung sicherstellen.

Abstract: Language constitutes the arena of political debate and is therefore itself the object of political struggle. But what language do populists speak? Existing research on this subject attests to its importance without being able to substantiate this empirically. This study examines the election programmes of the parties and the first ‘general debate'in the 19th German Bundestag in order to understand what is really specific to populists'use of language. Starting from a broad understanding of language, the author develops the theoretical concept of party language and applies a modified variant of linguistic multilevel discourse analysis. In the end, he identifies six characteristics of the language of populists.

Abstract: The profitability of medical treatment has gained increasing importance in health politics and likewise has become a considerable part of a microsurgeon's daily practice. The resulting cost pressure leads to microsurgeons having to justify their often complex and expensive treatments against hospital providers and health insurances. In this position paper of the German Speaking Group for Microsurgery of Peripheral Nerves and Vessels, we analyze the current status of profitability of microsurgical extremity and breast reconstruction, and its impact on choice of therapy and residency training. We specifically highlight the available literature, that shows often reduced long-term treatment costs after microsurgical reconstruction in comparison to cheaper initial treatments. The statements are based on a consensus workshop on the 40th meeting of the DAM in Lugano, Switzerland.Die Wirtschaftlichkeit von Therapien hat im Gesundheitswesen zunehmenden Stellenwert erlangt und wird dadurch auch für Mikrochirurgen ein relevanterer Aspekt ihrer Arbeit. Vor allem der verbundene Kostendruck, langfristig wirtschaftlich zu agieren, nötigt Mikrochirurgen ihre oftmals komplexen Behandlungen gegenüber Krankenversicherungen und Krankenhausträgern zu rechtfertigen. In diesem Positionspaper der deutschsprachigen Arbeitsgemeinschaft für Mikrochirurgie (DAM) wird der derzeitige Status Quo der Wirtschaftlichkeit in der mikrochirurgischen Extremitäten- und Brustrekonstruktion analysiert, sowie auch die daraus resultierenden Konsequenzen auf Therapiewahl und Weiterbildung. Ebenfalls wurde eine Analyse der Literatur gelegt, welche oftmals gesenkte Folgekosten nach komplexen Rekonstruktionen im Vergleich zu chirurgisch günstigeren Alternativverfahren beschreibt. Die beschriebenen Inhalte wurden auf einem Konsensusworkshop anlässlich der 40. Jahrestagung der DAM 2018 in Lugano erarbeitet.

Abstract: 36th International Cosmic Ray Conference, ICRC2019, Madison, USA, 24 Jul 2019 - 1 Aug 2019; 9 pages (2019)., The search for Dark Matter (DM) has great potential to reveal physics beyond the Standard Model. As such, searches for evidence of DM particles are being carried out using a wide range of techniques, such as direct searches for DM particles, searches for DM produced with colliders, and indirect searches for the Standard Model annihilation products of DM. Dwarf spheroidal galaxies (dSphs) are excellent targets for indirect Dark Matter searches due to their relatively high DM content and negligible expected astrophysical background. A collaboration was formed to maximise the sensitivity of DM searches towards dSphs by combining for the first time dSph data from three imaging air Cherenkov telescope (IACT) arrays: HESS, MAGIC, and VERITAS; the Fermi-LAT satellite, and the water Cherenkov detector HAWC. Due to the diverse nature of the instruments involved, each experiment will analyse their individual datasets from multiple targets and then the results will be combined at the likelihood level. For consistency of the likelihoods across the five experiments, a common approach is used to treat the astrophysical factor (J-Factor) for each target and an agreed set of annihilation channels are considered. We also agree on a common statistical approach and treatment of instrumental systematic uncertainties. The results are presented in terms of constraints on the velocity-weighted cross section for DM self-annihilation as a function of the DM particle mass.

45. Metastasis of Meningioma: A Rare Differential Diagnosis in Subcutaneous Masses of the Scalp

Abstract: Background: Subcutaneous masses of the scalp have a wide range of differential diagnosis. After removal of a meningioma in the patient’s history, scalp metastasis from the previously resected meningioma should be considered. Methods: A 86 year old patient presented with a local swelling on the left temporal forehead and no other clinical symptoms. Eleven years earlier an extra-axial meningioma was resected. The patient was receiving immunosuppressive therapy subsequent to kidney transplantation. After clinical examination and MRI, a lipoma was suspected. The mass was resected under local anesthesia. Results: Histopathology revealed a metastasis of the previously removed meningioma (WHO grade II). No further treatment was recommended. Clinical follow-up was without pathological findings so far. Conclusion: Scalp metastases of meningiomas are a rare finding. However, if patient history reveals removal of a meningioma, scalp metastasis must be a differential diagnosis for subcutaneous masses even years after the initial surgery.

Es sind Fälle wie diese, die für den Arbeitgeber die Frage aufwerfen, inwieweit er für derartiges Verhalten verantwortlich ist.Ziel dieser Arbeit ist es, die Verantwortlichkeit des Arbeitgebers für diskriminierendes Verhalten von Dritten zu strukturieren. Dabei wird insbesondere untersucht, wie sich die Maßnahmepflichten des Arbeitgebers nach dem Allgemeinen Gleichbehandlungsgesetz (AGG) in die bestehende Zivilrechtsdogmatik einfügen.

Abstract: We investigated whether nasal chondrocytes (NC) can be used to generate composite constructs with properties necessary for the repair of osteochondral (OC) lesions, namely maturation, integration and capacity to recover from inflammatory burst. OC grafts were fabricated by combining engineered cartilage tissues (generated by culturing NC or articular chondrocytes - AC - onto Chondro-Gide® matrices) with devitalized spongiosa cylinders (Tutobone®). OC tissues were then exposed to IL-1β for three days and cultured for additional 2 weeks in the absence of IL-1β. Cartilage maturation extent was assessed (immune) histologically, biochemically and by delayed gadolinium-enhanced magnetic resonance imaging of cartilage (dGEMRIC) while cartilage/bone integration was assessed using a peel-off mechanical test. The use of NC as compared to AC allowed for more efficient cartilage matrix accumulation and superior integration of the cartilage/bone layers. dGEMRIC and biochemical analyzes of the OC constructs showed a reduced glycosaminoglycan (GAG) contents upon IL-1β administration. Cartilaginous matrix contents and integration forces returned to baseline up on withdrawal of IL-1β. By having a cartilage layer well developed and strongly integrated to the subchondral layer, OC tissues generated with NC may successfully engraft in an inflammatory post-surgery joint environment.

Abstract: Tissue engineering (TE) is a new interdisciplinary field of applied research combining engineering and biosciences together with clinical application, mainly in surgical specialities, to develop living substitutes for tissues and organs. Tissue engineering approaches can be categorized into substitutive approaches, where the aim is the ex vivo construction of a living tissue or organ similar to a transplant, vs. histioconductive or histioinductive concepts in vivo. The main successful approaches in developing tissue substitutes to date have been progresses in the understanding of cell-cell interactions, the selection of appropriate matrices (cell-matrix interaction) and chemical signalling (cytokines, growth factors) for stimulation of cell proliferation and migration within a tissue-engineered construct. So far virtually all mammalian cells can be cultured under specific culture conditions and in tissue specific matrices. Future progress in cell biology may permit the use of pluripotent stem cells for TE. The blueprint for tissue differentiation is the genome: for this it is reasonable to combine tissue engineering with gene therapy. The key to the progress of tissue engineering is an understanding between basic scientists, biochemical engineers, clinicians, and industry.

Abstract: Processed bovine cancellous bone (PBCB) is an attractive material for tissue engineering of bone. It is biocompatible, osteoconductive, nonimmunogenic, and porous and its biomechanical properties are close to those of native bone. In this study, differentiation of primary rat osteoblasts (rOBs) incubated on PBCB was investigated in vitro. rOBs were isolated and expanded in two-dimensional culture. Expanded rOBs were seeded into PBCB disks and cultured either in basal medium (BM) or differentiation medium (DM) containing ascorbic acid, beta-glycerol phosphate, and dexamethasone. Alkaline phosphatase (ALP) activity and RNA expression of ALP, bone sialoprotein (BSP), collagen type I (COL1), osteocalcin (OC), and osteopontin (OPN) were assessed by chemiluminescence assay and quantitative real-time RT-PCR over 14 days. Histologic analysis was performed on day 14. ALP increased over the observation period independent of stimulation. OPN and BSP expression was significantly higher in the DM group whereas COL1 and OC expression was significantly higher in the BM group. Matrix calcification was detectable only in the DM group by von Kossa stain. The observed expression patterns suggest a physiological response of rOBs to the differentiation stimulus. PBCB is a suitable matrix for in vitro differentiation of osteoblasts. Cell-seeded PBCB is a potential osteogenic construct for in vivo application.

Abstract: We have previously shown a new approach to expand cultured human keratinocytes and reconstitute the epidermis in full-thickness wounds using a new microsperical transport system. This was a new approach to increase the cell yield for seeding without altering the anchoring proteins by enzymatic steps. That time we used Cytodex 3 which failed to be degraded and induced an inflammatory reaction in a t-cell-deficient organism. Therefore, we have investigated another microcarrier consisting of PLGA, which is a well-known carrier material for cell culture and transplantation. After coating the PLGA carrier with gelatine the seeding time of viable cells reached 4 h and the cell gain after 7 days of spinner culture was 16-fold. At 14 days after transplantation, we could detect a new stratified epithelium in our full-thickness wound healing model. Because cytokines play a major role in wound healing, we loaded this carrier material with different concentrations of rhEGF, showing a dose dependent release of the protein in vitro and in vivo. This result might lead to a different approach in the treatment of wounds.
