Abstract

Background: Effective communication skill of physicians is an important component of high‐quality healthcare delivery and safe patient care. Communication is embedded in the social and cultural contexts where it takes place. An understanding of medical students' attitudes and learning communication skills would help to design and deliver culturally appropriate medical education. The Communication Skills Attitude Scale (CSAS) is a widely used and validated tool to measure the attitude of medical students toward learning communication skills in different populations, settings, and countries. However, there is no culturally adapted and validated scale in Bangla in the Bangladesh context. This study aims to culturally adapt the CSAS into Bangla, and validate it in a cohort of medical students in Bangladesh. Methods: This study used a cross‐sectional survey design to collect data from purposively selected 566 undergraduate medical students from the Rajshahi division. The survey was conducted from January to December 2023. Descriptive statistics like frequency distribution and measures of central tendency were used to measure perception regarding communication skills. The sample adequacy was measured through the Kaiser–Meyer–Olkin test. The internal consistency of the items was identified using Cronbach's alpha (α) coefficients. Result: The results of the study show that the Bangla version of the scale is feasible, valid, and internally consistent in the context of a developing country, Bangladesh. The overall internal consistency of the Bangla version is good since the value of Cronbach's alpha (α) is 0.882. For PAS, the internal consistency is 0.933. While, for NAS, the value is 0.719. The item‐wise average scores in the PAS indicate that female medical students are more willing to learn communication skills compared with male students (α = 0.933). While, the scores in the NAS indicate that the male students tend to have more negative attitude toward learning communication skills compared with female students (α = 0.719). Conclusion: The CSAS‐Bangla is a valid and reliable tool for assessing communication skill attitudes among Bangla speaking medical students. Abstract

Aim: Technology including artificial intelligence (AI) may play a key role to strengthen primary health care services in resource‐poor settings. This systematic review aims to explore the evidence on the use of AI and digital health in improving primary health care service delivery. Methods: Three electronic databases were searched using a comprehensive search strategy without providing any restriction in June 2023. Retrieved articles were screened independently using the "Rayyan" software. Data extraction and quality assessment were conducted independently by two review authors. A narrative synthesis of the included interventions was conducted. Results: A total of 4596 articles were screened, and finally, 48 articles were included from 21 different countries published between 2013 and 2021. The main focus of the included studies was noncommunicable diseases (n = 15), maternal and child health care (n = 11), primary care (n = 8), infectious diseases including tuberculosis, leprosy, and HIV (n = 7), and mental health (n = 6). Included studies considered interventions using AI, and digital health of which mobile‐phone‐based interventions were prominent. m‐health interventions were well adopted and easy to use and improved the record‐keeping, service deliver, and patient satisfaction. Conclusion: AI and the application of digital technologies improve primary health care service delivery in resource‐poor settings in various ways. However, in most of the cases, the application of AI and digital health is implemented through m‐health. 23. Trust in the Bangladeshi health system during the COVID-19 pandemic: a mixed-methods exploration

Abstract

Lack of trust in public health institutions hinders pandemic management. Our aim was to determine levels of impersonal and interpersonal trust in the context of coronavirus disease 2019 (COVID-19) pandemic response in Bangladesh. As this mixed-methods research was conducted during the pandemic, data was gathered via an online survey involving 508 respondents, along with seven online focus group discussions with 50 purposively selected participants. Survey respondents have less trust in the health system than in the service providers. The lowest level of trust was observed in the Fairness content area of trust. While some non-clinical participants blamed doctors for shying away from caregiving during the pandemic, most praised them for providing care, risking their lives, and even sacrificing their lives in the process. Several participants also cited a lack of fairness in pandemic management, such as visible attempts by the decision-makers to protect the business interests without consideration for the safety of the poor. However, both clinicians and non-clinicians concurred on the need for the service providers to improve communication related to COVID-19 management. Health sector stewards in Bangladesh should take a science-based, equity-focused pandemic response to gain both impersonal and interpersonal trust and build a resilient health system in the long run.

Abstract

BackgroundLack of trust hinders care seeking, and limits community support for contact tracing, care seeking, information and communication uptake, multisectoral or multi-stakeholder engagement, and community participation. We aimed at exploring how trust might be breached and what implications this may have in Covid-19 pandemic response by the Bangladesh health systems.MethodsWe conducted this qualitative research during the pandemic, through seven online focus group discussions, with purposively selected mixed-gender groups of clinicians and non-clinicians (n=50). Data were analyzed through conventional content analysis method.ResultsThe common thread throughout the findings was the pervasive mistrust of the people in Bangladeshi health systems in its management of Covid-19 pandemic. In addition to the existing health systems weaknesses, few others became evident throughout the progression of the pandemic, namely, the lack of coordination challenges during the preparatory phase as well as the advanced stages of the pandemic. This; compounded by the health systems and political leadership failures, lead to opportunistic corruption and lack of regulations; leading to low quality, discriminatory, or no service at all. These have trust implications, manifested in health seeking from unqualified providers, nonadherence to health advices, tension between the service seekers and providers, disapproval of the governance mechanism, misuse of already scarce resources, disinterest in community participation, and eventually loss of life and economy.ConclusionsHealth sector stewards should learn the lessons from other countries, ensure multisectoral engagement involving the community and political forces, and empower the public health experts to organize and consolidate a concerted health systems effort in gaining trust in the short run, and building a resilient and responsive health system in the long.Key MessagesImplications for policy makersThe preexisting health systems weaknesses, widely discussed in many literatures on Bangladeshi health systems, need to be addressed first, in consultation with health policy and systems experts.In order to improve the coordination and science-based professional response to Covid-19 pandemic, the relevant experts, instead of administrators or bureaucrats, should be immediately engaged and deployed.In order to facilitate adaptive leadership, health system should ensure transparency in every aspects of its functions, curb corruption and discrimination, regulate private sector for cost and quality of services, and ensure equity and fairness.Politicians in power should engage with other social, cultural and religious forces and formally engage with other political parties in facing the Covid-19 crisis, with a view to fostering multisectoral collaboration and community engagement.The health system actors should ensure a free flow of correct information following evidence based, scientifically oriented social and behavior change communication (SBCC) strategies.Implications for publicBangladeshi health system is grappling with the Covid-19 pandemic. The authors believe that a better response was possible. In this research, people themselves expressed their perceptions and views regarding the alleged mishandling of the situation by health systems stewards. Careful addressing of the issues explored in this article may lead to a better pandemic response in the short run, and develop a resilient health system in the long.

Abstract

In a recent Viewpoint in the Lancet , some of us shared our experience of censorship in donor-funded evaluation research and warned about a potential trend in which donors and their implementing partners use ethical and methodological arguments to undermine research.1 Reactions to the Viewpoint—and lively debate at the 2018 Global Symposium on Health Systems Research —suggest that similar experiences are common in implementation and policy research commissioned by international donors to study and evaluate large-scale, donor-funded health interventions and programmes, which are primarily implemented in low resource settings. ‘We all have the same stories’, was one of the first comments on the Viewpoint, followed by many private messages divulging instances of personal and institutional pressure, intimidation and censorship following attempts to disseminate unwanted findings. Such pressure comes from major donors and from international non-governmental organisations (NGOs) obliged to have an external assessment but who then maintain a high degree of confidentiality and control. That such experiences are widespread reflects the deeply political nature of the field of ‘global health’ and the interconnections between priority setting, policy making and project implementation, which sit within a broader set of deeply entrenched power structures.2 3 Researchers in this field routinely find themselves working within—and studying—complex power relations and so experience challenges in negotiating their own position between interests of commissioning agencies and funders, implementers and country governments, as well as those of their own research institutions and their partnerships with other researchers spanning high-income, middle-income and low-income countries.4–7 They often receive research funding from major donor agencies like the UK Department of International Development (DFID), the US Agency for International Development (USAID), the Agence Francaise de Developpement (AFD), UNITAID and the Bill and Melinda Gates Foundation,8 who commission evaluations for their own funded projects, even though they have …
