ImportanceBefore surgery, the best strategy for managing patients who are taking renin-angiotensin system inhibitors (RASIs) (angiotensin-converting enzyme inhibitors or angiotensin receptor blockers) is unknown. The lack of evidence leads to conflicting guidelines.ObjectiveTo evaluate whether a continuation strategy vs a discontinuation strategy of RASIs before major noncardiac surgery results in decreased complications at 28 days after surgery.Design, setting, and participantsRandomized clinical trial that included patients who were being treated with a RASI for at least 3 months and were scheduled to undergo a major noncardiac surgery between January 2018 and April 2023 at 40 hospitals in France.InterventionPatients were randomized to continue use of RASIs (n = 1107) until the day of surgery or to discontinue use of RASIs 48 hours prior to surgery (ie, they would take the last dose 3 days before surgery) (n = 1115).Main outcomes and measuresThe primary outcome was a composite of all-cause mortality and major postoperative complications within 28 days after surgery. The key secondary outcomes were episodes of hypotension during surgery, acute kidney injury, postoperative organ failure, and length of stay in the hospital and intensive care unit during the 28 days after surgery.ResultsOf the 2222 patients (mean age, 67 years [SD, 10 years]; 65% were male), 46% were being treated with angiotensin-converting enzyme inhibitors at baseline and 54% were being treated with angiotensin receptor blockers. The rate of all-cause mortality and major postoperative complications was 22% (245 of 1115 patients) in the RASI discontinuation group and 22% (247 of 1107 patients) in the RASI continuation group (risk ratio, 1.02 [95% CI, 0.87-1.19]; P = .85). Episodes of hypotension during surgery occurred in 41% of the patients in the RASI discontinuation group and in 54% of the patients in the RASI continuation group (risk ratio, 1.31 [95% CI, 1.19-1.44]). There were no other differences in the trial outcomes.Conclusions and relevanceAmong patients who underwent major noncardiac surgery, a continuation strategy of RASIs before surgery was not associated with a higher rate of postoperative complications than a discontinuation strategy.Trial registrationClinicalTrials.gov Identifier: NCT03374449.

Background: The exact extent of and the mechanism by which trastuzumab causes cardiac side effects are not completely unravelled. We investigated the (cardiotoxic) side effects of trastuzumab in a relatively large homogeneous population. Methods: Healthy male volunteers (n = 54) with a left ventricle ejection fraction (LVEF) >55% were administered 6 mg/kg trastuzumab (n = 46) IV in 90 min in a placebo-controlled, parallel study. Placebo consisted of 0 · 9% NaCl (n = 8). Assessments included body weight, routine and cardiac laboratory markers and serial echocardiographic examinations (8 placebo and 9 trastuzumab treated participants) up to 63 days after dosing. Statistical analysis was done using repeated measurements of variance. Findings: Following trastuzumab infusion, fluid retention was observed: mean body weight increased over the first 4 days post-administration with 0 · 4 kg (95%-confidence interval: −0 · 2, 0 · 9, p = 0 · 2261) compared to placebo, mean haemoglobin concentration decreased with 0 · 3 mM (−0 · 6, −0 · 1; p = 0 · 0043), as did haematocrit (−0 · 013 L/L [−0 · 024, −0 · 002], p = 0 · 0216), and protein (−2 g/L [−4, −0], p = 0 · 0443) and albumin (−2 g/L [−3, −1], p

Nationally reported greenhouse gas inventories are a core component of the Paris Agreements transparency framework. Comparisons with emission estimates derived from atmospheric observations help identify improvements to reduce uncertainties and increase the confidence in reported values. To facilitate comparisons over the contiguous United States, we present a 0.1° × 0.1° gridded inventory of annual 2012-2018 anthropogenic methane emissions, allocated to 26 individual source categories, with scale-dependent error estimates. Our inventory is consistent with the U.S. Environmental Protection Agency (EPA) Inventory of U.S. Greenhouse Gas Emissions and Sinks (GHGI), submitted to the United Nations in 2020. Total emissions and patterns (spatial/temporal) reflect the activity and emission factor data underlying the GHGI, including many updates relative to a previous gridded version of the GHGI that has been extensively compared with observations. These underlying data are not generally available in global gridded inventories, and comparison to EDGAR version 6 shows large spatial differences, particularly for the oil and gas sectors. We also find strong regional variability across all sources in annual 2012-2018 spatial trends, highlighting the importance of understanding regional- and facility-level activities. Our inventory represents the first time series of gridded GHGI methane emissions and enables robust comparisons of emissions and their trends with atmospheric observations.

Abstract Background Despite advances in surgical techniques and care, pancreatoduodenectomy (PD) continues to have high morbidity and mortality rates. Complications such as sepsis, hemorrhage, pulmonary issues, shock, and pancreatic fistula are common postoperative challenges. A key concern in PD outcomes is the high incidence of infectious complications, especially surgical site infections (SSI) and postoperative pancreatic fistula (POPF). Bacteriobilia, or bile contamination with microorganisms, significantly contributes to these infections, increasing the risk of early postoperative complications. The occurrence of SSI in patients who undergo hepatobiliary and pancreatic (HPB) surgeries such as PD is notably higher than that in patients who undergo other surgeries, with rates ranging from 20 to 55%. Recent research by D'Angelica et al. revealed that, compared to cefoxitin, piperacillin/tazobactam considerably lowers the rate of postoperative SSI. However, these findings do not indicate whether extending the duration of antibiotic treatment is beneficial for patients at high risk of bacterial biliary contamination. In scenarios with a high risk of SSI, the specific agents, doses and length of antibiotic therapy remain unexplored. The advantage of prolonged antibiotic prophylaxis following PD has not been established through prospective studies in PD patients following biliary drainage. Methods This is an intergroup FRENCH-ACHBT-SFAR multicenter, open-labelled randomized, controlled, superiority trial comparing 2 broad-spectrum antibiotic (piperacillin/tazobactam) treatment modalities to demonstrate the superiority of 5-day postoperative antibiotic therapy to antibiotic prophylaxis against the occurrence of surgical site infections (SSI) following pancreaticoduodenectomy in patients with preoperative biliary stents. The primary endpoint of this study is the overall SSI rate, defined according to the ACS NSQIP, as a composite of superficial SSI, deep incisional SSI, and organ/space SSI. In addition, we will analyze overall morbidity, antibiotic resistance profiles, the pathogenicity of bacteriological and fungal cocontamination, the impact of complications after bile drainage and neoadjuvant treatment on the bacteriological and fungal profile of biliculture and cost-effectiveness. Conclusion This FRENCH24-ANIS study aims to evaluate 5-day post-operative antibiotic therapy combined with antibiotic prophylaxis on the occurrence of surgical site infections (SSI) following pancreaticoduodenectomy in patients with preoperative biliary stents. Abstract High volume hemofiltration (HVHF) could remove from plasma inflammatory mediators involved in sepsis-associated acute kidney injury (SA-AKI). The IVOIRE trial did not show improvements of outcome and organ dysfunction using HVHF. The aim of this study was to evaluate in vitro the biological effects of plasma of patients treated by HVHF or standard volume hemofiltration (SVHF). We evaluated leukocyte adhesion, apoptosis and functional alterations of endothelial cells (EC) and tubular epithelial cells (TEC). In vitro data were correlated with plasma levels of TNF-α, Fas-Ligand (FasL), CD40-Ligand (CD40L), von Willebrand Factor (vWF) and endothelial-derived microparticles. An experimental model of in vitro hemofiltration using LPS-activated blood was established to assess cytokine mass adsorption during HVHF or SVHF. Plasma concentrations of TNF-ɑ, FasL, CD40L and von Willebrand Factor (vWF) were elevated at the start (d1h0) of both HVHF and SVHF, significantly decreased after 6 h (d1h6), remained stable after 12 h (d1h12) and then newly increased at 48 h (d3h0). Plasma levels of all these molecules were similar between HVHF- and SVHF-treated patients at all time points considered. In addition, the levels of endothelial microparticles remained always elevated, suggesting the presence of a persistent microvascular injury. Plasma from septic patients induced leukocyte adhesion on EC and TEC through up-regulation of adhesion receptors. Moreover, on EC, septic plasma induced a cytotoxic and anti-angiogenic effect. On TEC, septic plasma exerted a direct pro-apoptotic effect via Fas up-regulation and caspase activation, loss of polarity, altered expression of megalin and tight junction molecules with an impaired ability to internalize albumin. The inhibition of plasma-induced cell injury was concomitant to the decrease of TNF-α, Fas-Ligand and CD40-Ligand levels. The protective effect of both HVHF and SVHF was time-limited, since a further increase of circulating mediators and plasma-induced cell injury was observed after 48 h (d3h0). No significant difference of EC/TEC damage were observed using HVHF- or SVHF-treated plasma. The in vitro hemofiltration model confirmed the absence of a significant modulation of cytokine adsorption between HVHF and SVHF. In comparison to SVHF, HVHF did not increase inflammatory cytokine clearance and did not reverse the detrimental effects of septic plasma-induced EC and TEC injury. Further studies using adsorptive membranes are needed to evaluate the potential role of high dose convective therapies in the limitation of the harmful activity of plasma soluble factors involved in SA-AKI. Understanding the impacts that mineral grain size and seawater salinity have on magnesium hydroxide (Mg(OH)2) dissolution and secondary calcium carbonate (CaCO3) precipitation is critical for the success of ocean alkalinity enhancement. We tested Mg(OH)2 dissolution kinetics in seawater using three Mg(OH)2 grain sizes (, 63–180 and >180 µm) at three salinities (∼36, ∼28 and ∼20). While Mg(OH)2 dissolution occurred more quickly the smaller the grain size, salinity did not significantly impact measured rates. Our results also demonstrate that grain size can impact secondary CaCO3 precipitation, suggesting that an optimum grain size exists for ocean alkalinity enhancement (OAE) using solid Mg(OH)2. Of the three grain sizes tested, the medium grain size (63–180 µm) was optimal in terms of delaying secondary CaCO3 precipitation. We hypothesise that in the lowest-grain-size experiments, the higher surface area provided numerous CaCO3 precipitation nuclei, while the slower dissolution of bigger grain sizes maintained a higher alkalinity and pH at the surface of particles, increasing CaCO3 precipitation rates and making them observable much more quickly than for the intermediate grain size. Salinity also played a role in CaCO3 precipitation, where the decrease in magnesium (Mg) allowed secondary precipitation to occur more quickly, similar in effect size to another known inhibitor, i.e. dissolved organic carbon (DOC). In summary, our results suggest that OAE efficiency as influenced by CaCO3 precipitation depends not only on seawater composition but also on the physical properties of the alkaline feedstock used.

Abstract Purpose Shared decision‐making (SDM) is crucial in pancreatic cancer treatment due to its choice‐sensitive nature and limited prognosis. Treatment of pancreatic cancer is organized in a network approach. Several obstacles exist on different levels—patient, healthcare professional, organizational, societal—that impede integration of SDM. This study aims to identify barriers and facilitators to SDM implementation within a comprehensive cancer network. Methods A qualitative research design was applied, involving interviews and focus groups on barriers and facilitators with healthcare professionals involved in the implementation of SDM. In one comprehensive cancer network in the Netherlands, including seven hospitals, a project was initiated with the goal of empowering patients and healthcare professionals in SDM throughout primary, secondary and tertiary healthcare settings. A total of 17 participants were assessed. Directed qualitative content analysis was performed by two researchers. Results Main findings revealed barriers such as time constraints, lack of priority of physicians, little involvement of general practitioners, and insufficient social context of patients in referrals, alongside facilitators including learning communities with practical SDM examples, metro mapping, involvement of case manager in implementation and patient empowerment strategies. Conclusion Addressing cultural, systemic barriers and developing innovative strategies are of importance to enhance SDM in pancreatic cancer treatment in a network approach. This study provides understanding of SDM implementation in complex healthcare settings and offers valuable guidance for future interventions seeking to improve decision‐making processes in pancreatic cancer treatment and beyond.

Abstract The route and speed of migration into Sahul by Homo sapiens remain a major research question in archaeology. Here, we introduce an approach which models the impact of the physical environment on human mobility by combining time-evolving landscapes with Lévy walk foraging patterns, this latter accounting for a combination of short-distance steps and occasional longer moves that hunter-gatherers likely utilised for efficient exploration of new environments. Our results suggest a wave of dispersal radiating across Sahul following riverine corridors and coastlines. Estimated migration speeds, based on archaeological sites and predicted travelled distances, fall within previously reported range from Sahul and other regions. From our mechanistic movement simulations, we then analyse the likelihood of archaeological sites and highlight areas in Australia that hold archaeological potential. Our approach complements existing methods and provides interesting perspectives on the Pleistocene archaeology of Sahul that could be applied to other regions around the world.

Coastal flood damage is primarily the result of extreme sea levels. Climate change is expected to drive an increase in these extremes. While proper estimation of changes in storm surges is essential to estimate changes in extreme sea levels, there remains low confidence in future trends of surge contribution to extreme sea levels. Alerting local populations of imminent extreme sea levels is also critical to protecting coastal populations. Both predicting and projecting extreme sea levels require reliable numerical prediction systems. The SurgeMIP (surge model intercomparison) community has been established to tackle such challenges. Efforts to intercompare storm surge prediction systems and coordinate the community's prediction and projection efforts are introduced. An overview of past and recent advances in storm surge science such as physical processes to consider and the recent development of global forecasting systems are briefly introduced. Selected historical events and drivers behind fast increasing service and knowledge requirements for emergency response to adaptation considerations are also discussed. The community's initial plans and recent progress are introduced. These include the establishment of an intercomparison project, the identification of research and development gaps, and the introduction of efforts to coordinate projections that span multiple climate scenarios.

Anthropogenic methane (CH4) emissions increases from the period 1850–1900 until 2019 are responsible for around 65% as much warming as carbon dioxide (CO2) has caused to date, and large reductions in methane emissions are required to limit global warming to 1.5°C or 2°C. However, methane emissions have been increasing rapidly since ~2006. This study shows that emissions are expected to continue to increase over the remainder of the 2020s if no greater action is taken and that increases in atmospheric methane are thus far outpacing projected growth rates. This increase has important implications for reaching net zero CO2 targets: every 50 Mt CH4 of the sustained large cuts envisioned under low-warming scenarios that are not realized would eliminate about 150 Gt of the remaining CO2 budget. Targeted methane reductions are therefore a critical component alongside decarbonization to minimize global warming. We describe additional linkages between methane mitigation options and CO2, especially via land use, as well as their respective climate impacts and associated metrics. We explain why a net zero target specifically for methane is neither necessary nor plausible. Analyses show where reductions are most feasible at the national and sectoral levels given limited resources, for example, to meet the Global Methane Pledge target, but they also reveal large uncertainties. Despite these uncertainties, many mitigation costs are clearly low relative to real-world financial instruments and very low compared with methane damage estimates, but legally binding regulations and methane pricing are needed to meet climate goals.

Abstract Background With the increase in the number of long-term survivors, interest is shifting from cancer survival to life and quality of life after cancer. These include consequences of long-term side effects of treatment, such as gonadotoxicity. Fertility preservation is becoming increasingly important in cancer management. International recommendations agree on the need to inform patients prior to treatments about the risk of fertility impairment and refer them to specialized centers to discuss fertility preservation. However, the literature reveals suboptimal access to fertility preservation on an international scale, and particularly in France, making information for patients and oncologists a potential lever for action. Our overall goal is to improve access to fertility preservation consultations for women with breast cancer through the development and evaluation of a combined intervention targeting the access and diffusion of information for these patients and brief training for oncologists. Methods Firstly, we will improve existing information tools and create brief training content for oncologists using a qualitative, iterative, user-centred and participatory approach (objective 1). We will then use these tools in a combined intervention to conduct a stepped-wedge cluster randomized trial (objective 2) including 750 women aged 18 to 40 newly treated with chemotherapy for breast cancer at one of the 6 participating centers. As the primary outcome of the trial will be the access to fertility preservation counselling before and after using the combined intervention (brochures and brief training for oncologists), we will compare the rate of fertility preservation consultations between the usual care and intervention phases using linear regression models. Finally, we will analyse our approach using a context-sensitive implementation analysis and provide key elements for transferability to other contexts in France (objective 3). Discussion We expect to observe an increase in access to fertility preservation consultations as a result of the combined intervention. Particular attention will be paid to the effect of this intervention on socially disadvantaged women, who are known to be at greater risk of inappropriate treatment. The user-centred design principles and participatory approaches used to optimize the acceptability, usability and feasibility of the combined intervention will likely enhance its impact, diffusion and sustainability. The integration of AI and robotics in healthcare raises concerns, and additional issues regarding autonomous systems are anticipated. Effective communication is crucial for robots to be seen as “caring”, necessitating advanced mechatronic design and natural language processing (NLP). This paper examines the potential of humanoid robots to autonomously replicate compassionate care. The study employs computational simulations using mathematical and agent-based modeling to analyze human–robot interactions (HRIs) surpassing Tetsuya Tanioka’s TRETON. It incorporates stochastic elements (through neuromorphic computing) and quantum-inspired concepts (through the lens of Martha Rogers’ theory), running simulations over 100 iterations to analyze complex behaviors. Multisensory simulations (visual and audio) demonstrate the significance of “dynamic communication”, (relational) “entanglement”, and (healthcare system and robot’s function) “superpositioning” in HRIs. Quantum and neuromorphic computing may enable humanoid robots to empathetically respond to human emotions, based on Jean Watson’s ten caritas processes for creating transpersonal states. Autonomous AI humanoid robots will redefine the norms of “caring”. Establishing “pluralistic agreements” through open discussions among stakeholders worldwide is necessary to align innovations with the values of compassionate care within a “posthumanist” framework, where the compassionate care provided by Level 4 robots meets human expectations. Achieving compassionate care with autonomous AI humanoid robots involves translating nursing, communication, computer science, and engineering concepts into robotic care representations while considering ethical discourses through collaborative efforts. Nurses should lead the design and implementation of AI and robots guided by “technological knowing” in Rozzano Locsin’s TCCN theory.
