Slavic populations, such as those in Poland, are considered to have a low prevalence of giant cell arteritis (GCA), although epidemiological data are sparse. The study aimed to compare the reported frequency of GCA in various regions of Poland and analyze the differences between them. We conducted a multicenter, retrospective study of all GCA patients included in the POLVAS registry—the first large multicenter database of patients with vasculitis in Poland. The data from the POLVAS registry were compared with the reported prevalence provided by national insurers from the corresponding regions. A 10-fold increase in the diagnostic rates of GCA was observed in Poland between 2008 and 2019, reaching 8.38 per 100,000 population > 50 years old. It may be attributed to increased interest accompanied by improved diagnostic modalities with the introduction of ultrasound-based, fast-track diagnostic pathways in some centers. However, regional inequities are present, resulting in 10-fold differences (from 2.57 to 24.92) in reported prevalence between different regions. Corticosteroid (CS) monotherapy was the main stem of treatment. Further cooperation and education are needed to minimize regional inequities. This observational study suggests some potential for further increase of the recognizability of GCA and wider use of other than CS monotherapy treatment regimens. We hope that the Polish experience might be interesting and serve as some guidance for the populations where GCA is underdiagnosed.

Objective. To characterize the eosinophilic granulomatosis with polyangiitis (EGPA) population from the POLVAS registry depending on ANCA status and diagnosis onset, including their comparison with the granulomatosis with polyangiitis (GPA) subset with elevated blood eosinophilia (min. 400/μl) (GPA HE) to develop a differentiating strategy. Methods. A retrospective analysis of the POLVAS registry. Results. The EGPA group comprised 111 patients. The ANCA-positive subset (n = 45 [40.54%]) did not differ from the ANCA-negative one in clinics. Nevertheless, cardiovascular manifestations were more common in ANCA-negative patients than in those with anti-myeloperoxidase (MPO) antibodies (46.97% vs. 26.92%, p = 0.045). ObjectivesThe aim of the study was to assess the influence of different factors, including treatment, on the risk of ILD in the course of RA.MethodsA total of 109 RA patients were included in the analysis. High-resolution computed tomography (HRCT) of chest was obtained in each patient. Patients were classified as having ILD (ILD group) or not (N-ILD group). The ILD was graded using the semi-quantitative Warrick scale of fibrosis. Warrick extent score (WES) and Warrick severity score (WSS) were calculated separately for each patient, then combined to obtain a global score (WGS).ResultsIn univariate analysis the presence of ILD was associated positively with age (P = 5x10-6) and negatively with MTX treatment (P = 0.0013), mean MTX dose per year of treatment (P = 0.003) and number of DMARDs used (P = 0.046). On multivariate analysis only age and treatment with MTX were independently associated with the presence of ILD. WGS was significantly lower in patients treated with MTX in a dose of ≥15 mg/week (MTX≥15 group) as compared to patients treated with lower doses of MTX (0ConsclusionsWe found a beneficial effect of MTX on RA-ILD. Importantly, this effect seems to be dose dependent.

Cyklofosfamid należy do leków alkilujących i znajduje istotnezastosowanie w reumatologii. Jedną z chorób, w których się gopodaje, jest zapalenie wielomięśniowe. Podczas stosowania lekutrzeba się liczyć z możliwymi działaniami niepożądanymi.W artykule opisano przypadek 61-letniej chorej na zapalenie wielomięśniowe,leczonej cyklofosfamidem, z dobrym efektem terapeutycznymi początkowo z dobrą tolerancją leku. Po podaniu ósmegodożylnego wlewu cyklofosfamidu u pacjentki wystąpiło krwawieniez dolnego odcinka przewodu pokarmowego. Kolonoskopia wykazałaobecność licznych owrzodzeń w dystalnym odcinku jelita grubego,a badanie histopatologiczne – niespecyficzne zapalenie błonyśluzowej jelita. Wysunięto podejrzenie krwotocznego zapalenia jelitagrubego po podaniu cyklofosfamidu i odstąpiono od dalszego stosowanialeku. Dolegliwości u chorej ustąpiły w ciągu 10 dni pozastosowaniu hydrokortyzonu we wlewkach doodbytniczych i nienawracały w trakcie dalszej 2-letniej obserwacji.Cyklofosfamid może wywoływać istotne działania niepożądane,a poza częstymi i dobrze znanymi należy brać pod uwagę możliwośćwystąpienia rzadkich powikłań, w tym krwotocznego zapaleniajelita grubego.

W przebiegu układowych chorób tkanki łącznej występują różnepowikłania położnicze, m.in. nawracające poronienia, wewnątrzmaciczneopóźnienie wzrostu płodu lub stan przedrzucawkowy. Autorzyopisali przypadek 36-letniej chorej na toczeń rumie niowatyukładowy i wtórny zespół antyfosfolipidowy, u której w 17. tygodniuciąży doszło do rozwoju zespołu HELLP, na który składa się hemoliza,podwyższona aktywność enzymów wątrobowych i małopłytkowość(hemolysis, elevated liver enzymes, low platelet count –HELLP). Leczenie glikokortykosteroidami, heparyną małocząsteczkową,immunoglobulinami dożylnymi i lekami hipotensyjnymipozwoliło uzyskać poprawę stanu klinicznego chorej i normalizacjęwyników badań laboratoryjnych. Pomimo zastosowanego leczenia,w 18. tygodniu ciąży doszło do wewnątrzmacicznego zgonu płodu.Celem publikacji jest zwrócenie uwagi na możliwość wczesnegowystępowania zespołu HELLP u chorych na układowe choroby tkankiłącznej.
