Exercise-induced bronchoconstriction, otherwise known as exercise-induced bronchoconstriction with asthma or without asthma, is an acute airway narrowing that occurs as a result of exercise and can occur in patients with asthma. A panel of members from the American Academy of Allergy, Asthma & Immunology Sports, Exercise, & Fitness Committee reviewed the diagnosis and management of exercise-induced bronchoconstriction in athletes of all skill levels including recreational athletes, high school and college athletes, and professional athletes. A special emphasis was placed on the recommendations and regulations set forth by professional athletic organizations after a detailed review of their collective bargaining agreements, substance abuse policies, antidoping program manuals, and the World Anti-Doping Agency antidoping code. The recommendations in this review are based on currently available evidence in addition to providing guidance for athletes of all skill levels as well as their treating physicians to better understand which pharmaceutical and nonpharmaceutical management options are appropriate as well as which medications are permitted or prohibited, and the proper documentation required to remain compliant.

Background Asthma exacerbation risk increases with worsening asthma control. Prevailing numerical control tools evaluate only current symptom impairment despite the importance of also assessing risk based on exacerbation history. An easy-to-use questionnaire addressing impairment and risk domains of control is needed. Objective To validate a composite asthma control tool that includes impairment and risk assessments (Asthma Impairment and Risk Questionnaire [AIRQ]). Methods Four-hundred forty-two patients aged ≥12 years with physician-diagnosed asthma who were followed in specialty practices completed 15 impairment and risk questions with dichotomized yes/no responses. Patients spanned all Global Initiative for Asthma severities and were classified as well-controlled, not well-controlled, or very poorly controlled according to a standard of Asthma Control Test (ACT) score plus prior-year exacerbations. Logistic regression analyses identified questions with the greatest predictive validity to discriminate among patients and determine cut points for these 3 classifications. Results The final AIRQ comprises 10 equally weighted yes/no impairment and risk questions. The final 10-item models yielded receiver operating characteristic curves of 0.94 to identify well-controlled versus not well-/very poorly controlled and 0.93 to identify well-/not well-controlled versus very poorly controlled asthma, as reflected by the ACT plus prior-year exacerbations standard. Cut points of 0-1, 2-4, and 5-10 best represented well-, not well-, and very poorly controlled asthma. Conclusions AIRQ is a rigorously validated composite measure designed to identify adults and adolescents with varying degrees of asthma control. Ongoing investigations will determine test-retest reliability, responsiveness to change, and predictive ability for future exacerbations.

ratory disease but often is the first manifestation of allergic disease. Most patients with atopic dermatitis will develop allergic rhinitis or asthma. The evaluation and management of atopic dermatitis are, therefore, an integral part of an allergist/immunologist’s training and practice. It is also important for the primary care physician to understand the basis for effective evaluation and management of patients with this condition, since atopic dermatitis affects more than 10% of children and can have a significant impact on the patient’s quality of life. As discussed in this document, it is also important for the primary care physician to know when to appropriately consult a specialist in atopic dermatitis.

This document contains complete guidelines for diagnosis and management of rhinitis developed by the Joint Task Force on Practice Parameters in Allergy, Asthma and Immunology, representing the American Academy of Allergy, Asthma and Immunology, the American College of Allergy, Asthma and Immunology and the Joint Council on Allergy, Asthma and Immunology. The guidelines are comprehensive and begin with statements on clinical characteristics and diagnosis of different forms of rhinitis (allergic, non-allergic, occupational rhinitis, hormonal rhinitis [pregnancy and hypothyroidism], drug-induced rhinitis, rhinitis from food ingestion), and other conditions that may be confused with rhinitis. Recommendations on patient evaluation discuss appropriate use of history, physical examination, and diagnostic testing, as well as unproven or inappropriate techniques that should not be used. Parameters on management include use of environmental control measures, pharmacologic therapy including recently introduced therapies and allergen immunotherapy. Because of the risks to patients and society from sedation and performance impairment caused by first generation antihistamines, second generation antihistamines that reduce or eliminate these side effects should usually be considered before first generation antihistamines for the treatment of allergic rhi-nitis. The document emphasizes the importance of rhinitis management for co-morbid conditions (asthma, sinusitis, otitis media). Guidelines are also presented on special considerations in patients subsets (children, the elderly, pregnancy, athletes and patients with rhinitis medicamentosa); and when consultation with an allergist-immunologist should be considered.

INTRODUCTION Atopic dermatitis is an important manifestation of the atopic diathesis.1,2 It not only frequently accompanies allergic respiratory disease but often precedes it as the initial clinical manifestation of allergic disease. The evaluation and management of patients with atopic dermatitis is therefore an integral part of an allergist’s practice and training. Since atopic dermatitis affects more than 10% of children, it is important for the primary care physician/provider to be familiar with the evaluation and management of this common skin condition, and know when to consult a qualified subspecialist, particularly when the diagnosis may not be established (Table 1). Although the exact role of IgE antibodies in the pathogenesis of atopic dermatitis is not clear, most individuals with atopic dermatitis have elevated serum IgE levels and there is evidence that the eosinophil plays a role in disease pathogenesis, based on elevated tissue and serum levels of eosinophil derived cationic proteins.3,4 Research into the pathogenesis of allergic disease continues to suggest a complex inflammatory process involving mast cells, lymphocytes, and infiltrating leukocytes which are orchestrated by a cytokine profile that has been identified with the T Helper Type 2 (TH2) lymphocyte.5 Recent advances in our understanding of the immunopathogenesis of atopic dermatitis are leading to the development of novel forms of therapy that may be helpful in selected patients. The response to topical corticosteroids as the mainstay of treatment for atopic dermatitis is likely to be the result of corticosteroid reduction of cellular immune activation. CLINICAL CRITERIA FOR DIAGNOSIS Intense pruritus and cutaneous reactivity associated with a lowered “itch threshold” are hallmarks of atopic dermatitis.6,7 Several skin lesions are commonly seen in atopic dermatitis. Acute lesions are characterized by intensely pruritic, erythematous papules and vesicles over erythematous skin. These are frequently associated with extensive excoriations and erosions which are accompanied by a serous exudate. Subacute lesions are characterized by erythema, excoriation, and scaling. Chronic lesions are characterized by thickened plaques of skin, accentuated skin markings (lichenification), and fibrotic papules (prurigo nodularis). In patients with chronic atopic dermatitis, all three skin reaction patterns may coexist in the same individual. Although atopic dermatitis may present at any age, it often begins between 2 and 6 months of age. The infantile form of atopic dermatitis involves the extensor surfaces of extremities, face, trunk and neck areas early, whereas the flexural aspects of the antecubital fossa and the popiliteal fossa become involved in chronic childhood and adult atopic dermatitis. Frequently, atopic dermatitis subsides in severity as the child matures, leaving an adult with skin that is prone to itching and inflammation when exposed to exogenous irritants.8 Last, chronic hand eczema may be the primary manifestation of many adults with atopic dermatitis. The first practice parameter on exercise-induced bronchoconstriction (EIB) was published in 2010. This updated practice parameter was prepared 5 years later. In the ensuing years, there has been increased understanding of the pathogenesis of EIB and improved diagnosis of this disorder by using objective testing. At the time of this publication, observations included the following: dry powder mannitol for inhalation as a bronchial provocation test is FDA approved however not currently available in the United States; if baseline pulmonary function test results are normal to near normal (before and after bronchodilator) in a person with suspected EIB, then further testing should be performed by using standardized exercise challenge or eucapnic voluntary hyperpnea (EVH); and the efficacy of nonpharmaceutical interventions (omega-3 fatty acids) has been challenged. The workgroup preparing this practice parameter updated contemporary practice guidelines based on a current systematic literature review. The group obtained supplementary literature and consensus expert opinions when the published literature was insufficient. A search of the medical literature on PubMed was conducted, and search terms included pathogenesis, diagnosis, differential diagnosis, and therapy (both pharmaceutical and nonpharmaceutical) of exercise-induced bronchoconstriction or exercise-induced asthma (which is no longer a preferred term); asthma; and exercise and asthma. References assessed as relevant to the topic were evaluated to search for additional relevant references. Published clinical studies were appraised by category of evidence and used to document the strength of the recommendation. The parameter was then evaluated by Joint Task Force reviewers and then by reviewers assigned by the parent organizations, as well as the general membership. Based on this process, the parameter can be characterized as an evidence- and consensus-based document.

Rodents (Rodentia) are the largest order of mammals in the world, with estimates ranging from 1,500 to 2,000 different documented species. The order includes commonly known mammals, such as rats, mice, guinea pigs, hamsters, and gerbils. The order Rodentia also includes beavers, muskrats, porcupines, woodchucks, chipmunks, squirrels, prairie dogs, marmots, chinchillas, voles, lemmings, and many others, although significant human exposure to these rodents is uncommon. The presence of 2 sets of incisors (top and bottom), used for chewing, is the common characteristic of rodent species. Incidentally, rabbits, hares, and a few other species make up the Lagomorpha order and therefore are not classified as rodents. Figure 1 shows an abbreviated taxonomy of the order Rodentia.2 Figure 1 Taxonomy of the order Rodentia. Adapted from Taxonomy of Common Rodent and Rodent-like pets.2 The common house mouse (Mus musculus) is a small, primarily nocturnal mammal. Although usually considered to be pests, mice also are popular pets. Mice are known to invade and establish residence in homes and other buildings, including schools, to obtain food and can at times be harmful, causing structural damage. Certain rodents, such as the deer mouse, can also spread diseases such as hantavirus through their feces and possibly also through their urine and saliva.3,4 Mice also are commonly used in research laboratories, where laboratory animal handlers come into contact with their allergens. Mice tend to have litter sizes of 4 to 8 pups and can have 6 to 8 litters per year, depending on the availability of food. Their life span in the wild is less than 1 year because of predation, although under laboratory conditions they can live as long as 2 years.5 Mice have the ability to jump 12 inches up and down 8 feet to the floor. They can run up almost any vertical surface, including wood, brick, metal pipes, wire mesh, and cables, and can run along suspended electric wires and ropes. They can squeeze through a 1/4-inchdiameter hole, travel upside down, cling from 1/4-inch hardware mesh, swim well, and survive at 24 °F for many generations.6 Rats are long-tailed mammals also of the order Rodentia. The most important rats from a human perspective are the black or roof rat, Rattus rattus, and the brown or Norway rat, Rattus norvegicus. The Norway rat is found in every state of the United States, whereas the roof rat tends to be found in coastal states. Rats usually can be distinguished from mice by their larger size. Rats also can serve as vectors for certain pathogens, such as Lassa fever. They are popular as pets but also are commonly used in research laboratories, where laboratory animal handlers come into contact with their allergens.6 Rats tend to be nocturnal and search for food and water between dusk and dawn; however, they will come out in daylight if their habitat is overcrowded or food is lacking. They require daily water; prefer traveling along edges, pipes, and rafters; and can even travel along overhead utility lines. They prefer to not cross open spaces to avoid predation, and although they have poor visual acuity, they have acute senses of smell, taste, hearing, and touch using their whiskers. Rats tend to have litter sizes of 5 to 12, and they can have up to 9 litters per year, depending on food supplies. Their life span generally is less than 1 year. Rats can pass through openings as small as 3/4 inch, theycan climb up vertical surfaces and even inside vertical pipes, and they can crawl horizontally on any type of pipe or conduit. They can jump vertically 24 inches and fall more than 50 feet and survive. They are able to swim under water for up to 30 seconds, tread water for up to 3 days, and swim up to ½ mile in open water. This permits them to enter buildings through drains and toilets.7 Although guinea pigs (Cavia porcellus), hamsters (Mesocritecus auratus), and gerbils (Gerbillus jerboa) also are rodents, they usually are kept as pets in a confined area and usually are not considered to be pests. Patients who keep such pets clearly can develop allergic symptoms after sensitization; however, other than in the case of laboratory animal workers, exposure to such rodents can usually be contained, and complete abatement, should it become necessary, is easily accomplished with removal of the offending animal.

Mild local reactions and fever after vaccinations are common and do not contraindicate future doses. Anaphylactic reactions to vaccines are rare and should be evaluated with skin tests to the vaccine and its components. If the skin test results are negative, subsequent doses can be administered in the usual manner but under observation. If the skin test results are positive and the patient requires subsequent doses, the vaccine can be administered in graded doses under observation. Some nonanaphylactic reactions to vaccines might also require evaluation, but only a few are contraindications to future doses. Pregnant women and persons who are immune compromised should generally not receive live vaccines. Purported long-term sequelae of vaccination, such as autism, are not supported by epidemiologic studies. Patients with egg allergy of any severity should receive annual influenza vaccinations because studies have demonstrated a very low rate of reactions. Studies to date have evaluated the injectable trivalent influenza vaccine (TIV), and thus TIV, rather than the live attenuated influenza vaccine (LAIV), should be used for recipients with egg allergy. All influenza vaccines available in the United States contain low amounts of ovalbumin. Neither skin testing with the vaccine nor dividing the dose is required; however, the vaccine should be administered in a setting in which anaphylaxis can be recognized and treated. EXECUTIVE SUMMARY Mild local reactions and constitutional symptoms, such as fever, after vaccinations are common and do not contraindicate future doses. Rarely, delayed-type hypersensitivity to a vaccine constituent can cause an injection-site nodule, but this is not a contraindication to subsequent vaccination. Anaphylactic reactions to vaccines are estimated to occur at a rate of approximately 1 per million doses. There are approximately 220million doses of vaccines distributed in the United States each year. All serious events occurring after vaccine administration should be reported to the Vaccine Adverse Event Reporting System (VAERS), even if it is not certain that the vaccine was the causal agent. Measuring levels of IgG antibodies to the immunizing agents in a vaccine suspected of causing a serious adverse reaction to determine whether they are at protective levels can help determine whether subsequent doses are required. All suspected anaphylactic reactions to vaccines should ideally be evaluated in an attempt to determine the culprit allergen. IgE-mediated reactions to vaccines are more often caused by additive or residual vaccine components, such as gelatin, rather than the microbial immunizing agent itself. Patients who have had an apparent anaphylactic reaction after immunization should undergo immediate-type allergy skin testing to help confirm that the reaction was IgE mediated and to determine the responsible component of the vaccine. If the intradermal skin test result is negative, the chance that the patient has IgE antibodies to any vaccine constituent is negligible, and the vaccine can be administered in the usual manner. Nonetheless, it is prudent in a patient with a history suggestive of an anaphylactic reaction to administer the vaccine under observation with epinephrine and other treatment available. In a patient with a history and skin test results consistent with an IgE-mediated reaction to a vaccine who requires additional doses of the suspect vaccine or other vaccines with common ingredients, consideration can be given to administering the vaccine in graded doses under observation. Some nonanaphylactic reactions to vaccines might also require evaluation, but only a few are absolute contraindications to future doses. Pregnant women should not be vaccinated with live vaccines. However, pregnant women should be given inactivated influenza vaccine, as well as tetanus and hepatitis B vaccine, if otherwise indicated. In general, live vaccines should not be given to persons who are immune compromised because of a risk of generalized infection with the immunizing agent. Specific vaccines or vaccination in general have been purported to have long-term consequences, including atopy, autism, and multiple sclerosis. Epidemiologic studies have not supported such associations. Patients with egg allergy should receive influenza vaccinations (TIV) because the risks of vaccinating are outweighed by the risks of not vaccinating. Persons with a history of suspected egg allergy should be evaluated by an allergist to determine the status of their egg allergy, but this should not delay their influenza vaccination. A growing number of studies suggest that influenza vaccines can be safely administered even to patients with a history of anaphylaxis to egg ingestion. Skin testing (prick, intradermal, or both) with the influenza vaccine itself in subjects with egg allergy (but without a history of reacting to the vaccine itself) does not reliably identify patients who are at increased risk of reacting to the vaccine and is not recommended. Influenza vaccine can be administered as a single dose to patients with egg allergy. Patients with egg allergy should receive influenza vaccines in a setting in which clinicians experienced in recognizing and treating anaphylaxis and equipment to manage anaphylaxis are immediately available and should be observed for 30minutes after vaccination. Patients with egg allergy with a history of only hives after egg ingestion can receive influenza vaccine in a primary care provider’s office provided the appropriate personnel and equipment are available, whereas those with a history of more severe reactions to egg ingestion should receive their vaccine in an allergist’s office. All influenza vaccines available in the United States contain low amounts of ovalbumin. The NAEPP defines patient education as one of the 4 key components for long term asthma control. Self management education is basic to the treatment of asthma. Self management programs must include the patient, family, and caregivers. Programs must be appropriate for age, socioeconomic backgound, culture, and environment. Education is an active process and involves knowledge plus opportunities to practice skills. Camp Programs for children were the “seed” for self management training and have proven effectiveness. Self management programs have been designed for medical centers: (ER, hospitals, clinic), private offices, as well as schools, camps, and communities. All program analysis must include long term outcomes. Self Management programs can be cost effective. Early disease recognition and treatment ,including self-management skills, will improve outcome-improve quality of life.

By means of monoclonal mouse anti-human IgE antibody, the microtiter solid-phase radioimmunoassay (MSPRIA) was modified to measure honeybee venom (HBV) IgE. HBV IgG was measured by MSPRIA(G) using affinity column purified goat anti-human IgG antibody and correlated with results obtained with staphylococcal protein A-SPRIA. Nonlinear specific IgE dilution curves were associated with high HBV IgG content. By serial transfer and assay of the supernatant, the interference of IgE by specific IgG could be reduced. The degree of interference was correlated with the specific IgG content. The principle of serial supernatant transfer and assay was applied to the standardization of reference HBV IgE serum, and the quantitation of HBV IgE contents in patients and controls. HBV IgE levels in patients allergic to honeybee stings correlated with the skin-test endpoint titrations. While 75% of the allergic patients were skin-test positive, 83% of them were IgE positive by the MSPRIA and 67% were IgE positive by the RAST.

Two cases of coexistent cystic fibrosis and infantile thoracic neuroblastoma are presented. In one patient, neuroblastoma was congenital, and diagnosis of cystic fibrosis was made at 3 months of age; in the other, the diagnosis of cystic fibrosis was made at 7 months of age, preceding that of neuroblastoma by 4 months. In both infants, surgical resection of the tumors have been successful. Recent advances in the genetic aspects of neuroblastoma, including translocation and activation of the oncogene N-myc, are discussed. Current recombinant DNA technology, which can identify translocation of N-myc and allow localization of the cystic fibrosis gene if the translocation occurs near the cystic fibrosis allele, is being applied to these cases.

Allergic reactions to anti-Pseudomonal penicillin derivatives are an increasing problem in therapy of cystic fibrosis lung disease. We evaluated 15 patients, ages 12 to 37 years, with documented allergic reactions to carbenicillin, ticarcillin, or piperacillin. Intradermal skin test reactions were positive for benzylpenicillin in seven patients, penicilloyl-polylysine in one, and ticarcillin or piperacillin in eight, for a total of 11 of 11 tested. Results of radioallergosorbent testing to penicilloyl conjugates were positive in eight of 14 patients and equivocal in four others. Overall, skin tests or RAST results were positive in 13 of 15 patients. All patients were desensitized with a semisynthetic penicillin by continuous serial intravenous infusion of 10-fold dose increments, beginning with 10(-6) of the therapeutic dose. Desensitization was successful in 25 of 26 instances. After intravenously administered therapy, maintenance of desensitization with dicloxacillin orally was unsuccessful in four of six patients. We conclude that (1) allergy to semisynthetic penicillins in cystic fibrosis usually is IgE mediated; (2) such allergy can be evaluated by skin testing; (3) it can be safely and in most cases successfully treated by intravenous desensitization; and (4) allergic patients should be desensitized on each subsequent admission for intravenously administered therapy.

CYSTIC FIBROSIS (CF) is an autosomal recessive genetic disorder with a frequency of about 1/1,600 among newborn infants of European descent. In spite of extensive investigation its primary molecular abnormality remains unknown. It has been observed that most CF patients lack an α2 macroglobulin–protease complex1 and that binding of trypsin and other proteases to α2 macroglobulin (α2M) is lower in plasma of CF patients than in that of normal controls2. We therefore decided to study the interaction of trypsin with plasma proteins of CF patients. Here we report that trypsin-binding IgG (TbIgG) is present in 80% of CF patients and in 30% of their mothers, but not in 97% of normal controls or in other relatives of CF patients. Experimental evidence supports the antibody nature of TbIgG, which is presumably produced against the porcine trypsin ingested by CF patients as pancreatic extracts for substitutive enzyme therapy.

Samples of plasma or serum from 53 cystic fibrosis (CF) patients, 90 relatives of CF patients , and 159 controls have been incubated with porcine or bovine 125I-trypsin, electrophoresed on polyacrylamide gel, and autoradiographed. In these individuals, the main binding protein for 125I-trypsin has been shown to be alpha 2-macroglobulin (alpha 2M). Using this method of analysis, no difference in electrophoretic migration of 125I-trypsin-alpha 2M complexes has been observed between CF and control individuals. However, trypsin binding to IgG has been observed in 80% of CF patients, 30% of their mothers, 3% of controls, and in two patients affected with pancreatitis. These trypsin binding immunoglobulins are called TbIg, and specifically, Tb1gG when referring to the G class. Experimental evidence indicates that binding of trypsin to IgG occurs through the Fab portion of the molecule. Tb1gG must be antibodies most probably induced by the exogenous trypsin ingested daily by most CF patients (and by patients affected with chronic pancreatitis). Antibodies against porcine pancreatic elastase have been observed using the same analysis, but not as frequently as Tb1g.

Lewiston, N., Conley, J. and Blessing-Moore, J. (Children's Hospital at Stanford, Palo Alto, California) Measurement of hypothetical burnout in cystic fibrosis caregivers. Acta Paediatr Scand, 70: 935, 1981.-Three hypothetical phases of staff burnout were measured in a population of cystic fibrosis caregivers and a control population from other areas of specialty pediatrics. The cystic fibrosis caregivers displayed a higher level of emotional exhaustion, lower level of patient depersonalization, and an equal sense of personal accomplishment from the job compared to the controls. We conclude that hypothetical staff burnout can be measured and postulate that these values can be used to quantitate the outcome of anti-burnout programs.

Minimal inhibitory concentrations of carbenicillin, chloramphenicol, tetracycline, gentamicin, tobramycin, and trimethoprim–sulfamethoxazole were determined for rough, smooth, and mucoid Pseudomonas sp. isolates from sputa collected from 40 cystic fibrosis patients. Ninety-four percent of the minimal inhibitory concentrations obtained by using mixed inocula of colonies of different morphological types fell within one serial dilution of the most resistant minimal inhibitory concentration obtained when each colony type was tested separately.
