Anorectal malignant melanoma is a rare culprit of malignancies in the anorectal region. With a presentation that mimics the vastly more common colorectal tumours, clinical misdiagnosis and diagnostic delays often occur, contributing to a dismal prognosis. The authors report a case of metastatic anorectal malignant melanoma presenting as seizures. Though our standard diagnostic pathway for suspected anorectal malignancies was followed, and despite the patient having computerized tomography (CT) of the head earlier, this presentation nonetheless led to a prolongation of time needed to reach histological diagnosis and delay in commencing definitive treatment. It also highlights the paucity of research into the pathophysiology and management of this infrequent but aggressive disease, and the need for raising awareness about this condition to the medical community so that it is considered as a plausible differential diagnosis from the outset and diagnostic pathways adjusted accordingly.

OBJECTIVE: This study evaluated the surface energy and wettability of composite resins polymerized by different light-curing units to ascertain the good wetting of tooth surfaces to achieve adhesion. MATERIAL AND METHOD: Filtek Z350 (3M ESPE), Admira (VOCO) and Grandio (VOCO) resins were selected for the testing procedures. The resins were light cured using LED and Halogen devices. Contact angles were measured goniometrically (Ramé-Hart F100) using water and glycerol as test liquids. Surface energy values were calculated with a software program (DROPimage Standard) that uses the harmonic mean method applied to the acid-base theory. The data were analyzed statistically by ANOVA and Tukey's test with a significance of 0.05. RESULTS AND CONCLUSIONS: No statistically significant differences were found between the values of surface energy. The measured wettability differed statistically in most combinations as a function of the type of composite resin, type of light-curing unit, and the test liquid.

Objective: To assess the influence of different surface treatments on the bond strength between the resinous cement Panavia F (Kuraray Co. Ltd., Osaka, Japan) and the structure of In-Ceram YZ (Vita Zahnfabrik, Bad Säckingen, Germany). Methods: Fifteen ceramic blocks were assessed: Group 1, finishing with abrasive paper; Group 2, finishing, airborne Al2O3 particle abrasion and silanization; Group 3, finishing, airborne particle abrasion, silicatization and silanization. After treatment, the blocks received cementation of resin composite cylinders with Panavia F (Kuraray Co. Ltd., Osaka, Japan) and were submitted to the shear bond strength test in a universal testing machine. Results: The results were statistically analyzed (ANOVA and multiple comparison Student-Newman-Keuls test): Group 1 (9.66 ± 1.67 MPa) < Group 2 (16.61 ± 3.38 MPa) = Group 3 (19.23 ± 5.69 MPa), with p = 0.007. Conclusion: The structures of the In-Ceram YZ system (Vita Zahnfabrik, Bad Säckingen, Germany) associated with Panavia F (Kuraray Co. Ltd., Osaka, Japan) require previous etching to achieve greater bond strength between the ceramic and cement, and this treatment may be performed with airborne particle abrasion I or traditional silicatization, both followed by silanization.

FUNDAMENTO: O transplante cardíaco enfrenta o grave problema da escassez de doadores. Estima-se que entre 20% e 40% dos pacientes falecem na fila de espera. Para esses pacientes, a utilização de dispositivos de assistência circulatória é, muitas vezes, a única possibilidade de sobrevivência durante a espera do doador. No Brasil, não existe nenhum programa regular de utilização desses dispositivos como ponte para transplante. OBJETIVO: Avaliar o desempenho hemodinâmico e a resposta inflamatória durante a utilização do DAV-InCor como ponte para transplante. MÉTODOS: Entre outubro de 2003 e abril de 2006, 11 pacientes, indicados em caráter de prioridade para o transplante cardíaco, evoluíram em choque cardiogênico refratário. O implante do DAV-InCor foi realizado em sete pacientes. O diagnóstico etiológico foi cardiopatia chagásica em cinco pacientes e cardiomiopatia dilatada idiopática em dois. RESULTADOS: A assistência mecânica ao ventrículo esquerdo foi mantida nos sete pacientes por períodos entre 14 e 42 dias (média 26,2). O desempenho hemodinâmico foi adequado, com a normalização do índice cardíaco, dos níveis de saturação venosa de O2 e do lactato. O transplante foi realizado em dois pacientes, os outros cinco faleceram por infecção sistêmica ou falência de múltiplos órgãos. CONCLUSÃO: O desempenho do DAV-Incor, no comportamento hemodinâmico dos pacientes estudados, foi adequado para a manutenção de uma condição circulatória satisfatória durante o período estudado. Houve melhora dos parâmetros de perfusão tecidual e manutenção de sinais de resposta inflamatória sistêmica. Houve alta incidência de complicações; contudo, não foram demonstradas complicações relacionadas ao dispositivo que comprometam a segurança da utilização do mesmo.BACKGROUND: Cardiac transplantation faces the serious problem of lack of donors and it is estimated that 20 to 40% of the patients die while waiting for heart transplantation. For these patients, the use of mechanical circulatory assist devices is the only choice of survival while waiting for a donor. In Brazil, the experience with mechanical circulatory support is limited and there is no regular program regarding the use of these devices as a bridge to heart transplantation. OBJECTIVE: To evaluate the hemodynamic performance and the systemic inflammatory response during the clinical use of the InCor-type ventricular assist device (VAD-InCor) as a bridge to heart transplantation. METHODS: Between October 2003 and April 2006, 11 patients in the waiting list for heart transplantation presented hemodynamic deterioration due to refractory cardiogenic shock. Seven of these patients were submitted to VAD-InCor implantation for left ventricular assistance. The etiologic diagnosis was Chagas' disease in 5 patients and idiopathic dilated cardiomyopathy in 2. RESULTS: The duration of left ventricular assistance ranged from 14 to 42 days (mean 26.2 days). During this period, the hemodynamic performance of the DAV-InCor was adequate to support a normal hemodynamic state. There was normalization of central venous oxygen saturation and serum lactate. Two patients were submitted to heart transplantation, while the other 5 patients died under assistance due to infection and multiple organ failure. CONCLUSION: The performance of the VAD-InCor, in the hemodynamic behavior of the studied patients, was adequate for the maintenance of a satisfactory circulatory state during the studied period. There was improvement in the tissue perfusion parameters and maintenance of systemic inflammatory response signs. There was a high incidence of complications; however, complications related to the device, which could compromise the safety of its use, were not demonstrated.

OBJECTIVE: Earlier studies on some dental materials measured roughness and/or contact angles or fluoride release separately. In the present study, five dental polymers were investigated to ascertain their contact angles, wettability, roughness, and fluoride release in dry or wet conditions. METHODS: Samples for 5 materials were prepared and stored dry or wet in deionized water pH 6.8. Samples were submitted to finishing/polishing procedures, and the measurements in Goniometer, roughness (µm) and fluoride analysis RESULTS AND CONCLUSIONS: Except for the Ariston pHc, all the materials displayed high contact angles when measured with water, showing hydrophobic characteristics. Roughness changed the contact angles, especially those of Ariston (α < 0.05). Fluoride did not modify the contact angles, but increased the roughness of the finished material.

INTRODUÇÃO: O choque cardiogênico é responsável por elevados índices de mortalidade na fila de espera para o transplante cardíaco. Na cardiomiopatia chagásica, a alta incidência de disfunção biventricular pode contribuir com a gravidade desta complicação. MÉTODO: Foram estudados 141 pacientes indicados em caráter de prioridade para o transplante. Destes pacientes, 46 eram portadores de cardiomiopatia chagásica e 95 de outras cardiomiopatias. O choque cardiogênico foi tratado farmacologicamente e com o implante ocasional do balão intra-aórtico. Em cinco pacientes chagásicos, foi realizado o implante de dispositivo paracorpóreo de assistência ventricular esquerda. RESULTADOS: Num período médio de 2,8 meses, 58 (41,1%) dos 141 pacientes foram transplantados, 73 (51,7%) faleceram e 10 foram retirados da fila. A mortalidade entre os pacientes chagásicos e não chagásicos foi de 45,6% e 54,7%, respectivamente. No entanto, a expectativa média de vida, sem a realização do transplante cardíaco, dos pacientes chagásicos foi de apenas 1,5 meses, sendo observado risco relativo de mortalidade de 1,6 para estes pacientes em relação aos não chagásicos (pINTRODUCTION: Heart failure is responsible for high mortality rates of patients on heart transplantation waiting lists. In Chagas cardiomyopathy, the presence of biventricular dysfunction increases the severity of this situation. METHOD: One hundred and forty-one patients suffering from cardiogenic shock, listed as high priority for heart transplantation, were studied. Forty-six patients presented with Chagas cardiomyopathy and 95 with other cardiomyopathies. Heart failure was treated using intravenous inotropic drugs and intra-aortic balloon pump implantation. Five patients with Chagas disease underwent paracorporeal left ventricular assist device implantation. RESULTS: During a mean follow-up of 2.8 months, 58 (41.1%) of the 141 patients were transplanted, while 73 (53.7%) died and 10 were removed from the waiting list. The mortality rates in chagasic and non-chagasic patients were 45.6% and 54.7%, respectively. A doença coronária aterosclerótica apresenta situações em que as técnicas cirúrgicas habituais ou as realizadas em laboratórios de hemodinâmica não são passíveis de utilização. São pacientes portadores de comprometimento grave e difuso das artérias coronárias, mas ainda com preservação da viabilidade miocárdica e que evoluem com importante sintomatologia anginosa, apesar da terapêutica clínica adequada. Técnicas de endarterectomia são preconizadas em algumas situações, porém com resultados trans e pós-operatórios muitas vezes desfavoráveis. Em outras ocasiões pode, inclusive, ser necessária a indicação de transplante cardíaco. Para este grupo de pacientes uma nova abordagem vem sendo desenvolvida a partir dos estudos do Dr. Mirhoseini, com a confecção de vários túneis transmiocárdicos com o uso de raios laser de CO2 de alta potência (850 watts). Os autores relatam sua experiência clínica inicial no Hospital Israelita Albert Einstein, em São Paulo, com a utilização da Revascularização Transmiocárdica a laser (Heart Laser) no período de abril de 1995 a fevereiro de 1996. Foram operados 11 pacientes com essa técnica (em um deles foi utilizada revascularização associada), sendo a média de idades de 68 anos. Sete deles eram casos de reoperações. Desses pacientes, 9 encontravam-se em grupo funcional IV. Houve 2 óbitos no período pós-operatório, ambos em pacientes com idade superior a 80 anos. Sete pacientes foram avaliados com 3 meses de pós-operatório, mostrando melhora clínica e maior tolerância a esforços. Em 4 pacientes foi realizado mapeamento cardíaco com isótopos e, em 1, houve melhora da perfusão. Os resultados, embora promissores, ainda requerem uma avaliação mais prolongada em um maior grupo de pacientes, para que sua utilização seja melhor estabelecida.Patients with ischemic heart disease and disabling angina, for whom CABG or PTCA is impossible, present a difficult chalenge. The Transmyocardial Laser Revascularization (TMLR) provides direct perfusion of ischemic myocardium vialaser by creating transmural channels. Since April 1995 we have treated 11 patients, 9 males, mean age 68 years, with TMLR. Preoperatively 9 were in angina class IV, despite adequate and even maximum clinical medical treatment. The patients were screened preoperatively by SESTAMIBI perfusion scan and pharmacological echocardiogram to identify the location and extention of their reversible ischemia. Operative exposure was obtained via a left anterior thoracotomy. Employing a 850W CO2 laser an average of 30 was created. Bleeding from the channels was controlled by direct finger pressure and rarely by epicardial suture. The early mortality was 2 out of 11 patients. There was no late mortality. All patients revealed improvement in their clinical status, and the mean angina class was 1,8 postoperatively; In 4 patients SESTAMIBI scan was performed at the third postoperative month, and 1 showed improvement in the myocardium perfusion. Those early results indicate that TMLR is a single operative technique that may improve myocardial perfusion and provide angina relief for patients not amenable to standard methods of revascularization. However a larger number of patients and a longer time of follow-up will be needed to have definitive conclusions.

Os dispositivos de Assistência Ventricular (D A V) têm sido utilizados para permitir a manutenção de condições cardiocirculatórias em dois tipos de situações: 1-) Como ponte para transplante cardíaco em paciente candidato e que, na espera de doador apropriado, tem suas condições cardiocirculatórias muito deterioradas. 2-) Como suporte circulatório em situações agudas em que se prevê uma possível recuperação funcional do coração, como em miocardites agudas, miocardiopatia puerperal, falência miocárdica pós cardiotomia, rejeição aguda pós transplante cardíaco etc. Entre os vários dispositivos utilizados, o modelo descrito por Pierce-Donachy, desenvolvido pela Thoratec Laboratories Corporation, consiste em aparelho de fluxo pulsátil, pneumático, externo, podendo ser usado como suporte uni ou biventricular. O ventrículo artificial é acionado por um módulo que permite vários ajustes manuais ou mesmo automáticos para melhor adequação às diferentes condições clínicas do paciente. O treinamento para utilização do dispositivo implica na integração de vários setores, como cirurgiões, intensivistas, enfermeiras, biomédicos e eletrotécnicos, necessitando laboratório especializado, com o uso de animais de maior porte, de custo elevado e manuseio complicado, dificultando a repetição freqüente dos experimentos. Os autores desenvolveram um modelo experimental, constituído por um coração bovino conectado pelo átrio esquerdo e pela aorta a um circuito de circulação extracorpórea. Através de técnica cirúrgica padronizada, são instaladas as cânulas apropriadas; as mesmas são conectadas ao ventrículo artificial e este ao módulo de acionamento. Com este modelo podese, de maneira simples, didática e reprodutível, efetuar o treinamento de aspectos técnicos de implante, manuseio de módulo de acionamento e simulação de situações clínicas e emergenciaisThe Ventricular Assist Devices (V A D) have been used to maintain the cardiocirculatory conditions in patients waiting for a heart transplantation (bridge to transplant) or in patients with an acute myocardial failure during a period enough to allow a myocardial recovery. The Pierce-Donachy System (Thoratec Laboratories Corporation) is one of those devices, a pulsatile flow model, pneumatic, external, used as uni or biventricular support. The training program for different groups of surgeons, phisicians, nurses and thecnicians is difficult in many situations, mainly when it is necessary to repeat those experiences several times to permit to those groups to be more familiar with the equipment. The authors developed an experimental model (utilizing a bovine heart and an arterial pump for extracorporeal circulation) to allow a simplified way of training, analising surgical aspects, use of the drive console and simulating clinical and emergencies situations
