Introduction Idiopathic rhinitis is a nonallergic and noninfectious rhinitis characterized mainly by nasal obstruction and rhinorrhea, resulting from an autonomic imbalance. Botulinum toxin type A (BTX-A) demonstrated its action in reducing rhinorrhea and nasal obstruction when injected into the nasal turbinates or septum. Objective To analyze the effects of intranasal BTX-A injection to control the symptoms of idiopathic rhinitis and its possible adverse effects. Method Patients with idiopathic rhinitis were divided into two groups. Group A had 15 participants (8 female and 6 male), of ages from 47 to 84 years (mean 66.57 years), and these received 60 U of Dysport (Ipsen Ltd, Maidenhead, Berkshire, UK) in each inferior nasal turbinate; group B had 12 participants (1 male and 11 female), of ages from 50 to 76 years (mean 60 years), and they received 1 ml of 0.9% saline. The individuals were reevaluated in the 1st, 2nd, 4th, 8th, and 12th weeks after injection by a questionnaire, accompanied by nasal inspiratory peak flow and acoustic rhinometry. Results Group A showed significant improvement, mainly regarding the symptoms of sneezing/itching and nasal obstruction, over time and when compared to group B. Acoustic rhinometry confirmed the improvement in nasal obstruction. There was no relationship between the nasal peak flow data and the nasal obstruction score. No major adverse effects have been reported. Conclusion The injection of botulinum toxin in the inferior nasal turbinates of patients with idiopathic rhinitis reduces the symptoms of sneezing, itching, nasal obstruction, and runny nose without significant adverse effects, suggesting that it is an option in the treatment of these patients.

Allergic rhinitis affects 10-30% of the population, negatively impacting one's quality of life and productivity. It has been associated with sinusitis, otitis media, sleep disorders, and asthma. Rupatadine is a second generation antihistamine with increased affinity to histamine receptor H1; it is also a potent PAF (platelet-activating factor) antagonist. It starts acting quite quickly, offers long lasting effect, and reduces the chronic effects of rhinitis. AIM: this study aims to assess the efficacy and safety of rupatadine in the treatment of persistent allergic rhinitis. MATERIALS AND METHOD: this is a multi-centric open prospective study. This study included 241 patients from 13 centers in Brazil and was held between October of 2004 and August of 2005. Signs and symptoms of rhinitis and tolerance to medication were analyzed after one and two weeks of treatment. RESULTS: reduction on general scores from 8.65 to 3.21 on week 2 (pA rinite alérgica acomete 10 a 30% da população, interferindo na qualidade de vida e na capacidade produtiva. Está associada à sinusite, otite, roncopatias e asma. A Rupatadina é um anti-histamínico de segunda geração, com elevada afinidade ao receptor histamínico H1 e potente inibição do fator ativador plaquetário (PAF). Tem rápido início de ação, longa duração e reduz os efeitos crônicos da rinite. OBJETIVO: Avaliar a eficácia e segurança da rupatadina no tratamento da rinite alérgica persistente. By means of scientific evidence reviews, the Brazilian Academy of Rhinology provides its practical and updated guidelines on the most utilized topical nasal medication, except for the drugs that have topical antibiotics in their formulas.

FUNDAMENTOS: Alergia ao látex é comum em trabalhadores da saúde, resultando em absenteísmo e afastamento das atividades profissionais. Há no Brasil poucos estudos publicados sobre o assunto. Enquanto na população geral a prevalência de alergia ao látex é menor que 2%, nos profissionais de saúde pode chegar a 30%. OBJETIVO: Estudar a prevalência de alergia ao látex em profissionais de saúde do Hospital do Servidor Público Estadual de São Paulo. MÉTODOS: Foi aplicado questionário aberto a profissionais que têm contato com luvas de borracha em todos os serviços do hospital. Foi colhido sangue daqueles que referiram sintomas e dosado anticorpo IgE específico por meio do ImmunoCAP. Para análise estatística utilizaram-se porcentagens, tabelas e teste qui-quadrado. RESULTADOS: Foram distribuídos 2.349 questionários, e respondidos 1.045, dos quais 129 referiram correlação entre uso de látex e sintomas sugestivos de alergia, e 54 permitiram a coleta de sangue para determinação de IgE específica. Neste grupo houve dois casos em que foi demonstrado anticorpo específico, estabelecendo-se prevalência de 3,7% de alergia ao látex. Fatores como gênero, profissão, freqüência de contato com luvas, setor de trabalho e antecedentes de atopia foram relacionados com maior prevalência de alergia ao látex. CONCLUSÃO: A prevalência de alergia ao látex encontrada foi de 3,7%.BACKGROUND: Latex allergy is common among healthcare workers and leads to absence from work and withdraw from professional activities. There are few studies published in Brazil on this subject. In the general population the prevalence of allergy to latex is less than 2%, whereas in healthcare workers it may reach 30%. OBJECTIVE: To study the prevalence of latex allergy in healthcare workers at the Hospital do Servidor Público Estadual de São Paulo. METHODS: A self-administered questionnaire was distributed to healthcare professionals from any sector of the hospital who have contact with rubber gloves. Blood was drawn from those who referred symptoms, for analyses of serum antilatex IgE antibody by Immunocap assay. Percentages, tables and the chi-square test were used for statistical analysis. RESULTS: A total of 2349 questionnaires were distributed, and 1045 were answered. In that, 129 reported association between latex and allergy symptoms and 54 out of them agreed to blood collection for specific IgE determination. Specific antibody was demonstrated in tow cases, determining a prevalence of 3.7% of latex allergy. Gender, occupation, frequency of use of gloves, work sector and past history of atopy were related to greater prevalence of latex allergy. CONCLUSION: The prevalence of latex allergy found was 3.7%.

Abstract Introduction: Inflammatory conditions of the nose and paranasal sinuses are very prevalent in the general population, resulting in marked loss of quality of life in affected patients, as well as significant work, leisure, and social activity losses. These patients require specific and specialized treatment. A wide range of oral medications are available. Objective: The present document is aimed to clarify, for professionals treating patients with inflammatory sinonasal diseases, both specialists and general practitioners, specific oral therapies in noninfectious nasal inflammatory conditions. Methods: The methodology used to create this article included the search for the key words: oral corticosteroids, antihistamines, antileukotrienes, rhinitis, rhinosinusitis in the MEDLINE and EMBASE databases in the last 5 years. Since no relevant article was found for the text on the subject of interest in the last 5 years, the search was extended for another 5 years, and so on, according to the authors’ needs. Results: Relevant literature was found regarding the use of antihistamines, antileukotrienes and oral corticosteroids in these conditions. The Brazilian Academy of Rhinology emphasizes, after extensive discussion by the collegiate, key points in the treatment with these drugs. Conclusion: There is support in the literature for the use of these drugs; however, final considerations about the role of each of them have been made.

Introduction Severe uncontrolled chronic rhinosinusitis with nasal polyps has a negative impact on an individual’s quality of life. Therefore, new biologics have emerged for use in specific phenotypes of chronic rhinosinusitis, changing the paradigms of its treatment. Objective To review the current status of biologic treatment indications in chronic rhinosinusitis. Methods The Brazilian Academy of Rhinology brought together different specialists to suggest a course of action, considering its particularities and aspects related to the national reality. Results Of particular interest for decision making will be the identification of subgroups of patients refractory to pre-existing treatment options and the construction of a strategy that improves their quality of life, with the best cost-benefit ratio. Conclusion The use of biologics is a valid option for treatment in more severe cases. This strategy must be better understood and improved in the future, with more studies and greater clinical experience. Resumo Introdução A rinossinusite crônica com pólipos nasais grave não controlada impacta negativamente na qualidade de vida do indivíduo. Para esses casos, novos imunobiológicos têm surgido, para uso em fenótipos específicos da rinossinusite crônica, e mudaram os paradigmas de seu tratamento. Objetivo Revisar o estado atual das indicações de imunobiológicos em rinossinusite crônica. Método A Academia Brasileira de Rinologia reuniu diferentes especialistas para sugerir uma conduta que considerasse suas particularidades e seus aspectos relacionados à realidade nacional. Resultados De particular interesse para a tomada de decisão serão a identificação dos subgrupos de pacientes refratários às opções de tratamento pré-existentes e a construção de uma estratégia que realmente melhore a qualidade de vida deles, dentro da melhor relação custo-benefício. Conclusão O uso de imunobiológicos é uma opção válida para tratamento em casos mais graves. Essa estratégia deve ser mais bem compreendida e aprimorada no futuro, com mais estudos e maior experiência clínica.

17. COMPARAÇÃO DOS INDICADORES DO ESTILO DE VIDA DE ESCOLARES DO ENSINO FUNDAMENTAL E MÉDIO DE ILHABELA

O objetivo do estudo foi comparar os indicadores do estilo de vida de escolares do ensino fundamental e médio de Ilhabela. A amostra foi composta por 400 escolares (11-18 anos) que faziam parte do Projeto Misto-Longitudinal de Crescimento, Desenvolvimento e Aptidão Física de Ilhabela. Foram aferidos o peso, a estatura e com posterior calculo do índice de massa corporal (IMC) para classificação dos escolares em eutrófico, excesso de peso ou obesidade de acordo com as curvas padrão da Organização Mundial da Saúde. Além disso, foram analisados indicadores de estilo de vida (transporte para escola, tempo de tela, TV no quarto, tempo de sono e dieta), atividade física (AF) e tempo sentado (TS) obtidos através de questionários. Foi utilizado o teste t de student e o teste do qui-quadrado na comparação dos resultados entre ensino fundamental e médio. A média de idade dos grupos ensino fundamental e médio foi 12,6 e 16,0 anos, respectivamente. Diferenças significantes entre os de ensino médio e fundamental foram observadas com relação à utilização de transporte ativo para a escola (20% x 48%, respectivamente), não atingir as recomendações de sono (70,3% x 47,8%, respectivamente), menores valores para alimentação saudável (3,61 x 3,86, respectivamente), e maior tempo gasto em AF leve foi maior no ensino médio (372,4 min x 281,1 min, respectivamente). Não houve diferenças entre os grupos quanto ao tempo gasto em AF moderada e vigorosa e o tempo sentado. Conclui-se que os alunos do ensino fundamental e médio apresentam comportamentos diferentes no tipo de transporte usado para escola, no tempo de sono e no gasto de AF com intensidade leve, assim como na alimentação. Estratégias de intervenção e mudanças no estilo de vida devem ser diferentes entre os escolares do ensino médio e fundamental.

These chemicals elements produce damage to the respiratory epithelium and exacerbate pre-existent diseases. In our study we showed these differences in two experimental studies. Study I (dose-response curve - DRCS): Forty frog-palates were exposed to the following dilutions: frog ringer, intact DEPA diluted in frog-ringer at 3 mg/L, 6 mg/L and 12 mg/L. Study II (DEPF) (DEPA fractions diluted at 12 mg/L): Fifty palates - Frog ringer, intact DEPA, DEPA treated with hexane, nitric acid and methanol. Introduction The guidelines on allergic rhinitis aim to update knowledge about the disease and care for affected patients. The initiative called "Allergic Rhinitis and its Impact on Asthma", initially published in 2001 and updated in 2008 and 2010, has been very successful in disseminating information and evidence, as well as providing a classification of severity and proposing a systemized treatment protocol. In order to include the participation of other medical professionals in the treatment of allergic rhinitis, it is important to develop algorithms that accurately indicate what should and can be done regionally. Objective To update the III Brazilian Consensus on Rhinitis - 2012, with the creation of an algorithm for allergic rhinitis management. Methods We invited 24 experts nominated by the Brazilian Association of Allergy and Immunology, Brazilian Association of Otorhinolaryngology and Head and Neck Surgery and Brazilian Society of Pediatrics to update the 2012 document. Results The update of the last Brazilian Consensus on Rhinitis incorporated and adapted the relevant information published in all "Allergic Rhinitis and its Impact on Asthma" Initiative documents to the Brazilian scenario, bringing new concepts such as local allergic rhinitis, new drugs and treatment evaluation methods. Conclusion A flowchart for allergic rhinitis treatment has been proposed. Resumo Introdução As diretrizes sobre rinite alergica visam atualizar os conhecimentos sobre a doença e os cuidados para com esses pacientes. A iniciativa designada "Rinite Alergica e seu Impacto na Asma", cujo relatorio inicial foi publicado em 2001 e atualizada em 2008 e 2010, tem sido muito bem sucedida na disseminaçao de informaçoes e evidencias, bem como na formulaçao da classificaçao de gravidade e proposta de sistematizaçao do tratamento. Entretanto, visando a participaçao de outros profissionais medicos no atendimento da rinite alergica, e importante o desenvolvimento de algoritmos que indiquem com precisao o que deve e pode ser feito regionalmente. Objetivo Atualizar o III Consenso Brasileiro sobre Rinites-2012, com elaboraçao de algoritmo para conduta da rinite alergica. Introduction: Inflammatory conditions of the nose and paranasal sinuses are very prevalent in the general population, resulting in marked loss of quality of life in affected patients, as well as significant work, leisure, and social activity losses. These patients require specific and specialized treatment. A wide range of oral medications are available. Objective: The present document is aimed to clarify, for professionals treating patients with inflammatory sinonasal diseases, both specialists and general practitioners, specific oral therapies in noninfectious nasal inflammatory conditions. Methods: The methodology used to create this article included the search for the key words: oral corticosteroids, antihistamines, antileukotrienes, rhinitis, rhinosinusitis in the MEDLINE and EMBASE databases in the last 5 years. Since no relevant article was found for the text on the subject of interest in the last 5 years, the search was extended for another 5 years, and so on, according to the authors' needs. Results: Relevant literature was found regarding the use of antihistamines, antileukotrienes and oral corticosteroids in these conditions. The Brazilian Academy of Rhinology emphasizes, after extensive discussion by the collegiate, key points in the treatment with these drugs. Conclusion: There is support in the literature for the use of these drugs however, final considerations about the role of each of them have been made. Introduction: Insulin has been used for the treatment of diabetes mellitus (DM) since 1920. At first, the animal insulins were available for human use having frequent allergic reactions due to the differences in amino acid sequences with human insulin. Currently, with the use of human insulin, the prevalence of these reactions is around 2% and less than a third of these events were associated with insulin itself. These cases are associated with their additives or preservatives. The most commonly hypersensitivity reaction found is type I. In these cases, the use of a rapid-acting insulin is the best therapeutic alternative. We report the case of a multi-sensibilizated patient to insulin presenting tolerance to the ultralong-acting: Degludec insulin. Case report: Female patient, 74, referring use of NPH (Neutral Protamine Hagedorn) human insulin for 5 years, with papule at the site of application of the insulin minutes after application. She visited the Allergy and Immunology Clinic of the State Public Servant Hospital of Sao Paulo referring anaphylaxis few minutes after application of human insulin (NPH). Also reported extensive local reaction due the use of detemir, and local reaction to lispro insulin. We conducted immediate skin test reading with insulins: Regular, Lispro, NPH, Detemir and Degludeca at a concentration of 1:1; addition of the additive (Protamine 10 mg/ml) and latex. Presented positive test for all insulins, except Degludeca and presented negative test for protamine and latex. Conclusion: we found no common additive to all insulins, which is not present in degludeca insulin (Tresiba ®), enhancing insulin allergy hypothesis. Patient currently tolerates the use of long-acting insulin (Degludeca) and has adequate control of their blood sugar. In this case the patient had no insulin reaction prolonged action and this may be a new therapeutic option to be tested in patients with allergic reactions to insulin.

Objective The aim of this paper is to study the respiratory muscle strength by evaluating the maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP) and lung volume before and 3 and 6 months after adenotonsillectomy. This is an interventional, before and after trial. It was set at the Department of Otolaryngology, University of Sao Paulo, School of Medicine. We included 29 children (6–13 years old), both genders, consecutively recruited from the waiting list for adenotonsillectomy. Children were submitted to maximal inspiratory pressures (MIP), maximal expiratory pressure (MEP) evaluation using an analog manovacuometer, lung volume, using incentive expirotometer and thoracic and abdominal perimeter using a centimeter tape. MATERIAL E MÉTODOS: Para este estudo prospectivo, foram avaliados 170 pacientes com faixa etária entre 6 e 55 anos de idade. Todos os pacientes passaram por avaliação otorrinolaringológica e fonoaudiológica. Foram colhidos os dados referentes às funções de respiração, mastigação, deglutição e fala e dados da consulta médica. Os dados foram comparados e analisados estatisticamente. RESULTADOS: A diferença dos escores de sinais e sintomas entre GR e GC mostrou-se estatisticamente significante. Quando comparada a presença de alteração nas funções estudadas entre GR e GC, foi observada diferença estatisticamente significante no modo respiratório e nos padrões de mastigação e deglutição. A correlação existente entre o escore de obstrução nasal e a presença de alteração funcional foi significante na análise do modo respiratório e do padrão de mastigação. CONCLUSÃO: O paciente com rinite alérgica apresenta alterações funcionais do sistema estomatognático e o aumento do escore de obstrução nasal pode ser considerado um indicativo destas alterações.Mouth breathing can cause structural and functional alterations to the stomatognathic system. Trata-se de um aspecto crítico do processo de reparação de lesões em todos os órgãos, representando um evento dinâmico de produção e degradação de matriz, em reação a inflamação, levando à reconstrução normal do tecido ou à formação de um tecido patológico. OBJETIVO E MÉTODO: Comparar os dados existentes em literatura entre o remodelamento de vias aéreas inferiores e superiores. RESULTADO: Asma é uma doença inflamatória crônica associada a remodelamento de vias aéreas. Na rinite alérgica, outra doença inflamatória crônica, o remodelamento é ainda pouco entendido. Apesar de a inflamação ser similar na rinite alérgica e asma, a extensão patológica do remodelamento nasal, assim como sua repercussão clínica, pode ser diferente dos brônquios. CONCLUSÃO: O remodelamento nas vias aéreas superiores ocorre em menor intensidade que nas vias inferiores, mas é aparente que a estrutura da mucosa nasal de pacientes com rinite não é normal.

Disfunções no transporte mucociliar trazem desde queda na qualidade de vida dos pacientes, como nas rinites e rinossinusites crônicas, até graves conseqüências com risco de seqüelas irreversíveis e mesmo letais, como nos casos de fibrose cística e das discinesias ciliares primárias. Desta forma, torna-se fundamental o conhecimento do funcionamento normal do aparelho mucociliar e de como alterações em seus componentes (cílio, muco-fluido periciliar e interação dinâmica entre ambos) afetam o transporte das secreções respiratórias. OBJETIVOS: Este artigo visa a revisar e discutir as diferentes técnicas de avaliação do transporte mucociliar descrevendo suas peculiaridades e aplicabilidades clínicas e experimentais. CONCLUSÕES: Os métodos citados nesta revisão nos fornecem informações importantes sobre os diferentes aspectos do transporte mucociliar. Alguns apresentam uma maior facilidade de realização e resultados reprodutíveis, já outros apenas mostraram-se com aplicabilidade em protocolos de pesquisa em virtude de dificuldades técnicas e limitações financeiras. Há que se considerar a inexistência de métodos que avaliem ambulatorialmente a freqüência de batimento ciliar (FBC) "in vivo" e "in situ", o que se tornaria uma ferramenta importante, tanto no âmbito científico, quanto na prática clínica, auxiliando no diagnóstico das discinesias ciliares e evitando a realização de procedimentos mais invasivos para a sua confirmação diagnóstica.Mucociliary transport dysfunctions can impair the quality of life of patients suffering from chronic rhinossinusitis and lead to severe consequences such as alterations in respiratory physiology or even death as in cases of cystic fibrosis and primary ciliary dyskinesia. CONCLUSIONS: The methods listed in this revision provide us with valuable information about different aspects of the mucociliary transport. Some of the methods listed are more suitable for clinical practice and present reproducible results. Others, show only applicability in experimental settings due to technical difficulties or financial limitations. However, it is important to emphasize that up to now there is no method that can evaluate ciliary beating frequency (CBF) in vivo and in situ. Such a method would become a valuable tool in the scientific scenario and in the clinical practice, supporting the diagnosis of ciliary dyskinesias and avoiding the use of invasive procedures to corroborate the clinical suspicion

34. Evaluation of Pain in Three Hysterosalpingography Techniques: Metal Cannula With and Without Paracervical Blockage and Balloon Catheter

The objective of this study was to assess discomfort or pain in patients undergoing hysterosalpingography using three different techniques.Eighty-nine patients were randomly assigned to one of three groups. In group 1, 30 patients underwent the technique performed with a metal cannula without anesthetic; in group 2, 29 patients underwent the technique performed with a metal cannula with previous paracervical anesthetic block; in group 3, 30 patients submitted to the procedure performed with a flexible balloon catheter. Each patient completed two questionnaires, Q1 and Q2: Q1 assessed their personal, clinical, and surgical history, and Q2 provided the visual analog scale (VAS) for pain assessment. The investigator also evaluated discomfort at each potentially painful stage of the procedure based on the patient's verbal expression, physical manifestations, or both.In terms of global discomfort experienced during hysterosalpingography, group 1 had the highest mean VAS score, whereas scores did not significantly differ between groups 2 and 3. During cervical grasping, group 2 experienced less discomfort than group 1. A comparison of pain associated with cervical grasping and balloon inflation (group 3 only) revealed that the highest levels of discomfort occurred during cervical grasping without anesthesia (group 1), followed by balloon inflation inside the cervix (group 3), and, last, cervical grasping after paracervical block (group 2).Performing hysterosalpingography with a flexible balloon catheter or metal cannula with previous paracervical block produced similar levels of pain; however, both techniques appear to be more comfortable than the traditional technique (i.e., metal cannula without anesthesia).

Crianças com aumento do volume de tonsilas palatina e faríngea freqüentemente apresentam anormalidades respiratórias tais como ronco, respiração oral e apnéia do sono. Sabe-se que a obstrução de vias aéreas superiores e conseqüentemente a respiração oral podem resultar em problemas pulmonares. OBJETIVO: Avaliar a pressão inspiratória em crianças com obstrução de vias aéreas superiores devido ao aumento do volume de tonsilas. FORMA DE ESTUDO: clínico com coorte transversal. MATERIAL E MÉTODO: Nós avaliamos 37 crianças (4-13 anos, ambos os sexos) com aumento do volume de tonsilas que seriam submetidas à cirurgia de Adenoamigdalectomia na Divisão de Otorrinolaringologia da Universidade de São Paulo no mesmo período. O grupo controle foi composto de 28 crianças sem aumento de volume tonsilar que foram submetidas aos mesmos testes. A pressão Inspiratória foi obtida pelo uso do manovacuômetro. RESULTADOS: Observamos uma menor pressão inspiratória no grupo com aumento do volume de tonsilas. A média do grupo com aumento do volume das tonsilas foi 14,607 cm/H2O e do grupo normal foi de 27,580 cm/H2O (P< 0,001). CONCLUSÃO: O aumento de volume de tonsilas palatina e faríngea foi associado a uma menor pressão inspiratória, resultando em um aumento do esforço respiratório e do trabalho dos músculos envolvidos.Children with enlarged tonsils and adenoids usually present breathing abnormalities such as snoring, mouth breathing and sleep apnea. It is known that upper airway obstruction and consequent mouth breathing may result in pulmonary diseases. AIM: The goal of this preliminary study was to evaluate the inspiratory pressure in children with upper airway obstruction due to enlarged tonsils. STUDY DESIGN: clinical with transversal cohort. MATERIAL AND METHOD: We evaluated 37 children (4 -13 years old, female/male) with enlarged tonsils who would be submitted to a T&A surgery in the Department of Otolaryngology, Medical School, University of Sao Paulo, from October 2002 to March 2003. The control group comprised 28 children without tonsillar disease submitted to the same tests. Inspiratory pressure was obtained using a manometer and vacuum meter. RESULTS: We could observe lower inspiratory pressures in children with upper airway obstruction. The mean of inspiratory pressure in the upper airway obstruction group was 14.607cm/H2O and in the control group was of 27.580cm/H2O. CONCLUSIONS: Enlarged tonsils and adenoids were associated with poor inspiratory pressure, resulting in increased breathing effort and work of the involved muscles.

A rede mundial de computadores (Internet) é, atualmente, fonte de informação sobre saúde para leigos e profissionais da área médica. A Rinite Alérgica é uma doença muito prevalente que chega a atingir mais de 10% da população geral, causando queda da qualidade de vida. OBJETIVO: Avaliar os princípios éticos de sites brasileiros que divulgam informações a respeito do tema "rinite alérgica". FORMA DE ESTUDO: revisional. MATERIAL E MÉTODO: Foi feita avaliação de 173 sites brasileiros encontrados através de quatro mecanismos de busca (Google, Yahoo, Altavista e Radar Uol). Os sites foram avaliados de acordo com o Manual de Princípios Éticos para Sites de Medicina e Saúde do CREMESP (Conselho Regional de Medicina do Estado de São Paulo), de acordo com os itens transparência, honestidade, qualidade, consentimento livre e esclarecido, privacidade, ética médica, responsabilidade e procedência. RESULTADOS: Entre os sites analisados, 149 (86,1%) não estavam de acordo como Manual de Princípios Éticos para Sites de Medicina e Saúde do CREMESP. As proporções de irregularidades entre os itens avaliados foram: qualidade (84,4%), privacidade (46,2%) honestidade (18,5%), consentimento livre e esclarecido (15,6%), responsabilidade (13,9%), transparência (12,1%), ética médica (2,3%). Havia informações inexatas em 24,3% dos sites analisados. CONCLUSÕES: A maioria dos sites estudados contendo informações sobre rinite alérgica fere os princípios éticos para sites do CREMESP. Tanto a qualidade em geral de grande parte dos sites brasileiros que abordam o tema "rinite alérgica", quanto à qualidade das informações por eles divulgadas, são insuficientes para satisfazer a médicos e pacientes.Nowadays, the World Wide Web (Internet) is an information source for non-experts and physicians. MATERIAL AND METHOD: We performed the evaluation of 173 Brazilian web sites, which were obtained from four search engines (Google, Yahoo, AltaVista and Radar Uol). The web sites were evaluated according to the Manual of Ethical Principles, Regional Council of Medicine of the state of Sao Paulo (CREMESP), regarding transparency, honesty, quality, privacy, medical ethics, informed consent, responsibility and origin. RESULTS: Among the analyzed web sites, 149 (86.1%) were not in accordance with the Manual of Ethical Principles of Regional Council of Medicine of the state of Sao Paulo (CREMESP). According to the analyzed items, the irregularities that were found were quality (84.4%), privacy (46.2%), honesty (18.5%), informed consent (15.6%), responsibility and origin (13.9%), transparency (12.1%), medical ethics (2.3%). There was inaccurate information in 24.3% of the analyzed sites. CONCLUSIONS: The majority of the websites regarding allergic rhinitis are not in accordance with the ethical principles of CREMESP. In general, the quality of a great part of the Brazilian web sites that address "allergic rhinitis", and the quality of the information disseminated by them, are insufficient to satisfy doctors and patients.

Summary Children with enlarged tonsils and adenoids usually present breathing abnormalities such as snoring, mouth breathing and sleep apnea. It is known that upper airway obstruction and consequent mouth breathing may result in pulmonary diseases. Aim : The goal of this preliminary study was to evaluate the inspiratory pressure in children with upper airway obstruction due to enlarged tonsils. Study design: clinical with transversal cohort. Material and Method : We evaluated 37 children (4 -13 years old, female/male) with enlarged tonsils who would be submitted to a T&A surgery in the Department of Otolaryngology, Medical School, University of Sao Paulo, from October 2002 to March 2003. The control group comprised 28 children without tonsillar disease submitted to the same tests. Inspiratory pressure was obtained using a manometer and vacuum meter. Results : We could observe lower inspiratory pressures in children with upper airway obstruction. The mean of inspiratory pressure in the upper airway obstruction group was 14.607cm/H2O and in the control group was of 27.580cm/H2O. Conclusions : Enlarged tonsils and adenoids were associated with poor inspiratory pressure, resulting in increased breathing effort and work of the involved muscles.

Summary Nowadays, the World Wide Web (Internet) is an information source for non-experts and physicians. Allergic rhinitis affects 10-30% of the population, negatively impacting one's quality of life and productivity. It has been associated with sinusitis, otitis media, sleep disorders, and asthma. Rupatadine is a second generation antihistamine with increased affinity to histamine receptor H1; it is also a potent PAF (platelet-activating factor) antagonist. It starts acting quite quickly, offers long lasting effect, and reduces the chronic effects of rhinitis.this study aims to assess the efficacy and safety of rupatadine in the treatment of persistent allergic rhinitis.this is a multi-centric open prospective study. This study included 241 patients from 13 centers in Brazil and was held between October of 2004 and August of 2005. Signs and symptoms of rhinitis and tolerance to medication were analyzed after one and two weeks of treatment.reduction on general scores from 8.65 to 3.21 on week 2 (p0.001). All signs and symptoms improved significantly in the first day of treatment (p0.001), except for nasal congestion and secretion, which improved from the second day of treatment (p0.001). Adverse events occurred in 19.9% of the cases, 27.7% on week 1.rupatadine effectively controls persistent allergic rhinitis; it is safe and presents low incidence of side effects.

CONTEXT AND OBJECTIVE: Knowledge of the profile of allergen sensitization among children is important for planning preventive measures. The objective of this study was to assess the prevalence and profile of sensitization to inhaled allergens and food among children and adolescents in an outpatient population in the city of Palmas. DESIGN AND SETTING: Cross-sectional study at outpatient clinics in Palmas, Tocantins, Brazil. METHODS: Ninety-four patients aged 1-15 years who were attending two pediatric outpatient clinics were selected between September and November 2008. All of the subjects underwent clinical interviews and skin prick tests. RESULTS: A positive skin prick test was observed in 76.6% of the participants (72.3% for inhalants and 28.9% for food allergens). The most frequent allergens were Dermatophagoides pteronyssinus (34%), cat epithelium (28.7%), dog epithelium (21.3%), Dermatophagoides farinae (19.1%), Blomia tropicalis (18.1%), cow's milk (9.6%) and grasses (9.6%). A positive skin prick test correlated with a history of atopic disease (odds ratio, OR = 5.833; P = 0.002), a family history of atopic disease (OR = 8.400; P < 0.001), maternal asthma (OR = 8.077; P = 0.048), pet exposure (OR = 3.600; P = 0.012) and cesarean delivery (OR = 3.367; P = 0.019). CONCLUSION: Dermatophagoides pteronyssinus was the most frequent aeroallergen and cow’s milk was the most prevalent food allergen. There was a positive correlation between a positive skin prick test and several factors, such as a family history of atopic disease, maternal asthma, pet exposure and cesarean delivery.

Background Acoustic rhinometry is more accurate in the anterior portion of the nasal cavities, from the nostril to the nasal valve (NV), the most important site of nasal obstruction. Literature presents different opinions regarding the anatomical correlation of the two notches at the beginning of the rhinogram curve in normal adult white individuals. The aim of this study was to present new data for a better understanding of the anatomic correlation of these two notches. Methods This prospective study included 32 nasal cavities under the following conditions: (1) basal condition, (2) with external nasal dilator strip (ENDS), (3) after decongestion (0.05% oxymetazoline chloride applied as an aerosol spray), and (4) after decongestion with ENDS. Results No statistically significant variation was observed in the cross-sectional area of the first notch (CSA1) after decongestion. However, there was a statistically significant increase in the CSA2 after ENDS. Conclusion Analysis of the results suggested that the first notch of the rhinogram refers to the nostril and the second notch refers to the NV as a whole. Allergies in Latin America is the first cross-national survey that describes the symptoms, impact, and treatment of nasal allergies (NAs) in individuals >or=4 years old in Latin America (LA). In total, 22,012 households across the Latin American countries of Argentina, Brazil, Chile, Colombia, Ecuador, Mexico, Peru, and Venezuela were screened for children, adolescents, and adults with a diagnosis of NA and either symptoms or treatment in the past 12 months. A total of 1088 adults and 457 children and adolescents were included and the sample was probability based to ensure valid statistical inference to the population. Approximately 7% of the LA population was diagnosed with NAs with two of three respondents stating that their allergies were seasonal or intermittent in nature. A general practice physician or otolaryngologist diagnosed the majority of individuals surveyed. Nasal congestion was the most common and bothersome symptom of NAs. Sufferers indicated that their symptoms affected productivity and sleep and had a negative impact on quality of life. Two-thirds of patients reported taking some type of medication for their NAs, with a roughly equal percentage of patients reporting taking over-the-counter versus prescription medications. Changing medications was most commonly done in those reporting inadequate efficacy. The most common reasons cited for dissatisfaction with current medications were related to inadequate effectiveness, effectiveness wearing off with chronic use, failure to provide 24-hour relief, and bothersome side effects (e.g., unpleasant taste and retrograde drainage into the esophagus). Findings from this cross-national survey on NAs have confirmed a high prevalence of physician-diagnosed NAs and a considerable negative impact on daily quality of life and work productivity as well as substantial disease management challenges in LA. Through identification of disease impact on the LA population and further defining treatment gaps, clinicians in LA may better understand and treat NAs, thus leading to improvements in overall patient satisfaction and quality of life.

Na literatura existem terminologias diferentes para se referir às mesmas estruturas anatômicas e, ainda, o mesmo termo se referindo a estruturas anatômicas diferentes. OBJETIVO: Realizamos este trabalho com o objetivo de revisarmos o funcionamento da VN e definirmos com mais clareza estruturas anatômicas da porção anterior das cavidades nasais, principalmente a região da VN. CONCLUSÃO: Existe controvérsia na literatura quanto à nomenclatura das estruturas da VN. Neste trabalho definimos VN como uma estrutura tridimensional compreendida anteriormente pelo ostium internum e posteriormente pelo isthmus nasi. There are different terminologies for the same anatomic structures in the literature. FUNDAMENTOS: Alergia ao látex é comum em trabalhadores da saúde, resultando em absenteísmo e afastamento das atividades profissionais. Há no Brasil poucos estudos publicados sobre o assunto. Enquanto na população geral a prevalência de alergia ao látex é menor que 2%, nos profissionais de saúde pode chegar a 30%. OBJETIVO: Estudar a prevalência de alergia ao látex em profissionais de saúde do Hospital do Servidor Público Estadual de São Paulo. MÉTODOS: Foi aplicado questionário aberto a profissionais que têm contato com luvas de borracha em todos os serviços do hospital. Foi colhido sangue daqueles que referiram sintomas e dosado anticorpo IgE específico por meio do ImmunoCAP. Para análise estatística utilizaram-se porcentagens, tabelas e teste qui-quadrado. RESULTADOS: Foram distribuídos 2.349 questionários, e respondidos 1.045, dos quais 129 referiram correlação entre uso de látex e sintomas sugestivos de alergia, e 54 permitiram a coleta de sangue para determinação de IgE específica. Neste grupo houve dois casos em que foi demonstrado anticorpo específico, estabelecendo-se prevalência de 3,7% de alergia ao látex. Fatores como gênero, profissão, freqüência de contato com luvas, setor de trabalho e antecedentes de atopia foram relacionados com maior prevalência de alergia ao látex. CONCLUSÃO: A prevalência de alergia ao látex encontrada foi de 3,7%. BACKGROUND: Latex allergy is common among healthcare workers and leads to absence from work and withdraw from professional activities. There are few studies published in Brazil on this subject. In the general population the prevalence of allergy to latex is less than 2%, whereas in healthcare workers it may reach 30%. OBJECTIVE: To study the prevalence of latex allergy in healthcare workers at the Hospital do Servidor Público Estadual de São Paulo. METHODS: A self-administered questionnaire was distributed to healthcare professionals from any sector of the hospital who have contact with rubber gloves. Blood was drawn from those who referred symptoms, for analyses of serum antilatex IgE antibody by Immunocap assay. Percentages, tables and the chi-square test were used for statistical analysis. RESULTS: A total of 2349 questionnaires were distributed, and 1045 were answered. In that, 129 reported association between latex and allergy symptoms and 54 out of them agreed to blood collection for specific IgE determination. Specific antibody was demonstrated in tow cases, determining a prevalence of 3.7% of latex allergy. Gender, occupation, frequency of use of gloves, work sector and past history of atopy were related to greater prevalence of latex allergy. CONCLUSION: The prevalence of latex allergy found was 3.7%.
