Abstract Background Rifampicin-resistant pulmonary tuberculosis (RR-PTB) presents a significant threat to global public health security. China bears a substantial burden of RR-PTB cases globally, with Guizhou Province experiencing particularly alarming trends, marked by a continual increase in patient numbers. Understanding the population characteristics and treatment modalities for RR-PTB is crucial for mitigating morbidity and mortality associated with this disease. Methods We gathered epidemiological, diagnostic, and treatment data of all RR-PTB cases recorded in Guizhou Province from January 1, 2017 to December 31, 2023. Utilizing composition ratios as the analytical metric, we employed Chi-square tests to examine the spatiotemporal distribution patterns of RR-PTB patients and the evolving trends among different patient classifications over the study period. Results In our study, 3396 cases of RR-PTB were analyzed, with an average age of 45 years. The number of RR-PTB patients rose significantly from 176 in 2017 to 960 in 2023, peaking notably among individuals aged 23–28 and 44–54, with a rising proportion in the 51–80 age group (P

MOX (mixed oxide) fuel plays a positive role in promoting sustainable development in nuclear energy. The quantitative determination of plutonium in mixed MOX fuel is also essential to nuclear security and safeguards. In this regard，neutron multiplicity measurement methods serve as crucial non-destructive testing techniques and play a vital role in quantitatively detecting plutonium in MOX fuel. In order to study the factors that influence the quantification process of plutonium，enhance measurement accuracy，and streamline the process，this study develops a comprehensive and specific simulation system for the multiplicity-based plutonium quantification method in MOX fuel. Based on the AWCC device as the basic model (simulation model with a detection efficiency of 23.89% and a die-away time of 45.42 μs)，the combination of MCNP and MATLAB software is utilized to recombine the detector capture times of fission neutrons and (α，n) neutrons obtained from MCNP software with the particle emission time series obtained by MATLAB sampling，forming a complete pulse time series. After receiving the multiplicity moments of fission neutrons in MOX fuel (vs1=2.157，vs2=3.808，vs3=5.283，vi1=2.855，vi2=6.953，vi3=13.899)，simulated pulse sequence acquisition，multiplicity analysis，and quantitative calculation are performed on four samples with different shapes and sizes. Each sample is measured for 300 s with a pre-delay time of 3 μs，a gate width of 54 μs，a long delay of 2 ms，and repeated three time’s measurements. Additionally，the simulated quantitative calculation results 240Pu are compared with the set values. The results indicate that the mass of 240Pu and the α-value obtained through simulated pulse analysis meet the requirement of less than 5% relative error compared to the actual input values. This work provides technical support for data interpretation in the quantitative measurement of plutonium in MOX fuel.

ObjectiveTo identify cortical and subcortical volume, thickness and cortical area features and the networks they constituted related to anxiety in Parkinson’s disease (PD) using structural magnetic resonance imaging (sMRI), and to integrate multimodal features based on machine learning to identify PD-related anxiety.MethodsA total of 219 patients with PD were retrospectively enrolled in the study. 291 sMRI features including cortical volume, subcortical volume, cortical thickness, and cortical area, as well as 17 clinical features, were extracted. Graph theory analysis was used to explore structural networks. A support vector machine (SVM) combination model, which used both sMRI and clinical features to identify participants with PD-related anxiety, was developed and evaluated. The performance of SVM models were evaluated. The mean impact value (MIV) of the feature importance evaluation algorithm was used to rank the relative importance of sMRI features and clinical features within the model.Results17 significant sMRI variables associated with PD-related anxiety was used to build a brain structural network. And seven sMRI and 5 clinical features with statistically significant differences were incorporated into the SVM model. The comprehensive model achieved higher performance than clinical features or sMRI features did alone, with an accuracy of 0.88, a precision of 0.86, a sensitivity of 0.81, an F1-Score of 0.83, a macro-average of 0.85, a weighted-average of 0.92, an AUC of 0.88, and a result of 10-fold cross-validation of 0.91 in test set. The sMRI feature right medialorbitofrontal thickness had the highest impact on the prediction model.ConclusionWe identified the brain structural features and networks related to anxiety in PD, and developed and internally validated a comprehensive model with multimodal features in identifying.

Abstract Background Mycobacterium leprae (ML) is the pathogen that causes leprosy, which has a long history and still exists today. ML is an intracellular mycobacterium that dominantly induces leprosy by causing permanent damage to the skin, nerves, limbs and eyes as well as deformities and disabilities. Moreover, ML grows slowly and is nonculturable in vitro. Given the prevalence of leprosy, a highly sensitive and rapid method for the early diagnosis of leprosy is urgently needed. Results In this study, we devised a novel tool for the diagnosis of leprosy by combining restriction endonuclease, real-time fluorescence analysis and multiple cross displacement amplification (E-RT-MCDA). To establish the system, primers for the target gene RLEP were designed, and the optimal conditions for E-RT-MCDA at 67 °C for 36 min were determined. Genomic DNA from ML, various pathogens and clinical samples was used to evaluate and optimize the E-RT-MCDA assay. The limit of detection (LoD) was 48.6 fg per vessel for pure ML genomic DNA, and the specificity of detection was as high as 100%. In addition, the detection process could be completed in 36 min by using a real-time monitor. Conclusion The E-RT-MCDA method devised in the current study is a reliable, sensitive and rapid technique for leprosy diagnosis and could be used as a potential tool in clinical settings.

ObjectivesThe present study analyzed the impact of the COVID-19 pandemic on the prevalence and incidence of new leprosy cases, as well as the diversity, distribution, and temporal transmission of Mycobacterium leprae strains at the county level in leprae-endemic provinces in Southwest China.MethodsA total of 219 new leprosy cases during two periods, 2018–2019 and 2020–2021, were compared. We genetically characterized 83 clinical isolates of M. leprae in Guizhou using variable number tandem repeats (VNTRs) and single nucleotide polymorphisms (SNPs). The obtained genetic profiles and cluster consequences of M. leprae were compared between the two periods.ResultsThere was an 18.97% decrease in the number of counties and districts reporting cases. Considering the initial months (January–March) of virus emergence, the number of new cases in 2021 increased by 167% compared to 2020. The number of patients with a delay of >12 months before COVID-19 (63.56%) was significantly higher than that during COVID-19 (48.51%). Eighty-one clinical isolates (97.60%) were positive for all 17 VNTR types, whereas two (2.40%) clinical isolates were positive for 16 VNTR types. The (GTA)9, (TA)18, (TTC)21 and (TA)10 loci showed higher polymorphism than the other loci. The VNTR profile of these clinical isolates generated five clusters, among which the counties where the patients were located were adjacent or relatively close to each other. SNP typing revealed that all clinical isolates possessed the single SNP3K.ConclusionCOVID-19 may have a negative/imbalanced impact on the prevention and control measures of leprosy, which could be a considerable fact for official health departments. Isolates formed clusters among counties in Guizhou, indicating that the transmission chain remained during the epidemic and was less influenced by COVID-19 preventative policies.

ObjectiveTo examine the prevalence of tuberculosis/human immunodeficiency virus (TB/HIV) co-infection in Guizhou province during 2011 – 2020 for providing evidence to prevention and control of the dual infection. Methods The information on all registered TB, HIV/AIDS patients and the patients with dual infection of TB and HIV during the 10-year period were collected from a TB/HIV co-infection screening and management program conducted across the province. Describe statistics was performed on the extracted data and trend chi-square test was used to assess temporal variation in the co-infection prevalence. ResultsOf the 231 444 registered HIV/AIDS patients, 85.89% (n = 198 785) and 70.02% (n = 162 052) had symptom and chest radiograph/sputum smear screening for TB; the prevalence rate of TB was 0.91% among all the registered HIV/AIDS patients and the annual prevalence rate showed a downward trend (P < 0.05). For a total of 401 572 registered TB patients, 89.89% (n = 360 993) had HIV antibody test and HIV antibody positive rate was 0.29% among the TB patients detected. The screening tests resulted in an average annual prevalence rate of 0.88/100 000 for dual infection with TB and HIV among all registered TB and HIV/AIDS patients in the province during the period and the rate showed an upward trend (P < 0.05). The number of co-infected patients was the highest in 2019 (n = 472) and the lowest in 2011 (n = 178). The majority of the co-infected patients were males aged ≥ 15 years and the patients distributed unevenly across the prefectures in the province. Of all the co-infected patients (n = 3 142 ), 2 820 (89.75%) received treatments and the proportion of the patients receiving therapy of anti-TB and HIV, anti-HIV alone, and anti-TB alone were 65.75% (n = 2 066), 7.7% (n = 242), and 16. 29% (n = 512), respectively. Conclusion The screening on dual infection of TB and HIV needs to be improved among TB and HIV/AIDS patients and medication compliance of the co-infected patients should be promoted simultaneously, especially among the population in the regions with a higher risk of the co-infection.

Abstract Background A paucity of studies focused on the genetic association that tuberculosis (TB) patients with non-communicable diseases (NCDs) are more likely to be infected with Mycobacterium tuberculosis (MTB) with more potent virulence on anti-TB drug resistance than those without NCDs. The study aimed to document the predominant genotype, determine the association between MTB genotypes and NCD status and drug resistance. Methods We conducted a molecular study in 105 TB patients based on a cross-sectional study focused on the comorbid relationship between chronic conditions and TB among 1773 subjects from September 1, 2019 to August 30, 2020 in Guizhou, China. The participants were investigated through face-to-face interviews, followed by NCDs screening. The DNA of MTB isolates was extracted prior to genotyping using 24 loci MIRU-VNTR. The subsequent evaluations were performed by phylogenetic trees, combined with tests of statistical power, Chi-square or Fisher and multivariate logistic regression analysis. Results The Beijing family of Lineage 2 (East Asia) was the predominant genotype accounting for 43.8% (46/105), followed by Lineage 4 (Euro-America) strains, including Uganda I (34.3%, 36/105), and the NEW-1 (9.5%, 10/105). The proportion of Beijing strain in patients with and without NCDS was 28.6% (8/28) and 49.4% (38/77), respectively, with a statistical power test value of 24.3%. No significant association was detected between MTB genotype and NCD status. A low clustering rate (2.9%) was identified, consisting of two clusters. The rates of global, mono-, poly- and multi-drug resistance were 16.2% (17/105), 14.3% (15/105), 1.0% (1/105) and 4.8% (5/105), respectively. The drug-resistant rates of rifampicin, isoniazid, and streptomycin, were 6.7% (7/105), 11.4% (12/105) and 5.7% (6/105), respectively. Isoniazid resistance was significantly associated with the Beijing genotype of Lineage 2 (19.6% versus 5.1%). Conclusions The Lineage 2 East Asia/Beijing genotype is the dominant genotype of the local MTB with endogenous infection preponderating. Not enough evidence is detected to support the association between the MTB genotype and diabetes/hypertension. Isoniazid resistance is associated with the Lineage 2 East Asia/Beijing strain.

This study focuses on investigating the surface settlement characteristics induced by the construction of a super-large-diameter shield tunnel in composite strata. By utilizing a combination of field monitoring and numerical simulation analysis, the surface settlement patterns encountered during the construction process in horizontally distributed typical soil–rock composite strata were summarized based on the 16.03 m super-large-diameter shield tunnel project in the southerly extension of He’ping Avenue in Wuhan. In addition, the collected data were used to enhance the Peck empirical formula. The results of the study show the following: (1) Significant non-uniform settlement occurs along the tunneling direction when the shield machine passes through soil–rock composite strata. The range of non-uniform settlement is approximately 3.1 times the tunnel diameter (D) in soil sections and 1.9 times the tunnel diameter (D) in rock sections. (2) The impact of composite strata on the maximum settlement is greater than its effect on the settlement trough width, with a larger impact within the soil sections compared to the rock sections. (3) The Peck correction formula, which takes into account the distance between the monitoring cross-section and the composite interface, provides more accurate predictions than the original Peck empirical formula.

Abstract Pulmonary tuberculosis (PTB) and diabetes mellitus (DM) remain high morbidity and mortality, especially when they are comorbid with each other. Screening for diabetes mellitus in tuberculosis is essential as the incidence and mortality of DM in the population with PTB are higher than in the general people. We aimed to examine the gradient association of tuberculosis on developing DM, the additional yield and the number needed to screen (NNS) to find a new diabetes case. A cross-sectional study was conducted on 801 tuberculosis cases and 972 household contacts in Guizhou, China, from April 2019 to October 2020. After screening for PTB among contacts, all participants were screened for DM and interviewed. Kendall’s tau-b test and proportional odds logistic regression analysis were applied to identify the gradient associations. Among the 1773 subjects, the additional yield of screening was 21.8%. The NNSs of the non-PTB group, the sputum-culture negative and positive groups were 50, 60 and 113, respectively. The gradient incremental establishment of DM and PTB were positively correlated. The general trend on the gradient of DM significantly increased with the gradient increase of PTB. Age 35 years and over, excessive edible oil intake and DM family history were identified as significant predictors of diabetes. Integrated screening for DM targeted to different gradients of PTB combined with associated factors is necessitated to achieve a higher additional yield.

The Common Koel Eudynamys scolopaceus Linnaeus 1758, belongs to the family Cuculidae and order Cuculiformes. This omnivorous bird exhibits obligate nest parasitism. Here, the complete mitogenome of E. scolopaceus was determined and phylogenetically compared with those of other Cuculidae species. The newly sequenced complete mitogenome was a circular DNA molecule with a size of 17,610 bp (OM115963). This mitogenome had a higher A + T content (57.58%) than G + C content (42.42%). Phylogenetic analysis revealed that E. scolopaceus was most closely related to Eudynamys taitensis and the genus Cuculus, providing useful molecular information for further research on the phylogeny of the family Cuculidae.

Abstract The incidence of Tuberculosis (TB) in Guizhou province has ranked to be the top four among the 31 China provinces. The spatial distribution and influencing factors of adverse outcomes of TB in Guizhou are unclear. In our study, the cases information of TB in Guizhou province from 2013 to 2018 was collected, we analyzed the spatial distribution and clusters of five adverse outcomes of TB with ArcMap10.2 software, used logistics regression analysis to assessed risk factors and used Chi-square analysis to analyze variation trend of the five adverse outcomes. A total of 237, 806 cases information of TB were collected. The proportion of adverse outcomes in TB patients was 6.18%, among which adverse reactions accounted for 1.05%, lost to follow-up accounted for 1.44%, treatment failed accounted for 1.15%, died accounted for 2.31%, switch to MDR accounted for 0.24%. Traditional $\ell _{1}$ -regularized compressed sensing magnetic resonance imaging (CS-MRI) model tends to underestimate the fine textures and edges of the MR image, which play important roles in clinical diagnosis. In contrast, the convex nonconvex (CNC) strategy allows the use of nonconvex regularization while maintaining the convexity of the total objective function. Plug-and-play (PnP) algorithm is a powerful framework for sparse regularization problems, which plug any advanced denoiser into traditional proximal algorithms. In this paper, we propose a PnP-ADMM algorithm for CS-MRI reconstruction with CNC sparse regularization. We first obtain the proximal operator for CNC sparse regularization. Then we present PnP-ADMM algorithm by replacing the proximal operator of ADMM with properly pre-trained denoisers. Furthermore, we conduct comparison experiments using various denoisers under different sampling templates for different images. The experimental results verify the effectiveness of the proposed algorithm with both numerical criteria and visual effects.

Objectives: Understanding the nature of Mycobacterium leprae transmission is vital to implement better control strategies for leprosy elimination. The present study expands the knowledge of county-level strain diversity, distribution, and transmission patterns of leprosy in endemic provinces of China. Methods: We genetically characterized 290 clinical isolates of M. leprae from four endemic provinces using variable number tandem repeats (VNTR) and single nucleotide polymorphisms (SNPs). Attained genetic profiles and cluster consequences were contrasted with geographical and migration features of leprosy at county levels. Results: Considering the allelic variability of 17 VNTR loci by the discriminatory index, (GTA)9, (AT)17, (AT)15, (TA)18, (TTC)21, and (TA)10 are reported to be more highly polymorphic than other loci. The VNTR profile generated the low-density clustering pattern in the counties of Sichuan and Yunnan, whereas clusters have been observed from the isolates from Huayuan (N = 6), Yongding (N = 3), Zixing (N = 3), Chenxi (N = 2) and Zhongfang (N = 2) counties of Hunan, and Zhijin (N = 3), Anlong (N = 2), Zhenning (N = 2), and Xixiu (N = 2) counties of Guizhou. In some clusters, people’s social relations have been observed between villages. From the 290 clinical isolates, the most predominantly reported SNP was 3K (278, 95.8%), followed by SNP 1D (10, 3.4%), which are typically observed to be predominant in China. We also detected the novel SNP 3J (2, 0.8%), which has not yet been reported in China. Conclusion: The clustering pattern of M. leprae indicates the transmission of leprosy still persists at county levels, suggesting that there is a need to implement better approaches for tracing the close contacts of leprosy patients.

IntroductionDiabetes mellitus (DM), hypertension and pulmonary tuberculosis (PTB) are catastrophic illnesses that collectively lead to increased mortality and premature death. However, the size of the problem and the appropriate approach to deal with the burden is still unclear. We aimed to evaluate the yield, number needed to screen (NNS) to prevent one death or adverse event for screening DM and hypertension and assess the prevalence and contributors to DM and/or hypertension.MethodsBased on PTB contact tracing, a cross-sectional study was conducted among 801 PTB index cases and 972 household contacts from April 2019 to October 2020 in Guizhou, China. All the participants were screened for DM and hypertension. The yield was calculated as the proportion of newly detected cases among the study subjects, excluding known cases. The NNS was computed by dividing the number needed to treat for risk factors by the prevalence of the unrecognized diseases. The univariate and multivariate logistic regression analyses were applied to determine the independent predictors of DM and/or hypertension.ResultsOf the 1,773 participants, the prevalence of DM and hypertension was 8.7% (70/801) and 15.2% (122/801) in the PTB patients, 3.2% (31/972) and 14.0% (136/972) in the contacts, respectively. The prevalence of DM and/or hypertension was 21.2% (170/801) among the PTB patients and 15.4% (150/972) among their contacts. The screening yields to detect new cases of DM and hypertension among PTB patients were 1.9% and 5.2%, and that in the contacts were 0.8% and 4.8%, respectively. The NNS for DM was 359 for the PTB cases and 977 for the contacts, 299 for PTB cases and 325 for hypertension, respectively. Older age, under or overweight and obesity, family history hypertension and earlier diagnosis of other chronic conditions were the independent predictors for DM and/or hypertension among both PTB cases and their contacts.ConclusionScreening for DM and hypertension should be mandated in PTB patients and their household contacts to disclose undetected cases of these two conditions during TB contact tracing, which might reduce the potential cardiovascular disease deaths.

ABSTRACTReports on antimicrobial resistance (AMR) of Mycobacterium leprae, relationship with bacteriological index (BI), and transmission in China are limited. We investigated the emergence of AMR mutations, the relationship between BI and AMR in complete, moderate and lack of BI decline cases, and molecular epidemiological features of AMR cases by enrolling 290 leprosy cases from four endemic provinces. Seven (2.41%), one (0.34%), five (1.72%), one (0.34%), and one (0.34%) strains had single mutations in folP1, rpoC, gyrA, gyrB, and 23S rRNA, respectively. Double mutations in folP1 and gyrA, rpoB and gyrA, and gyrA and 23S rRNA were observed in one (0.34%) strain each. Mutated strains occurred in three out of 81 (95% CI−0.005-0.079, p = 0.083) cases with complete BI decline, in seven out of 103 (95% CI 0.018-0.117, p = 0.008) cases with moderate BI decline, and in four out of 34 (95% CI 0.003-0.231, p = 0.044) cases with lack of BI decline. Most of these mutated strains were geographically separated and diverged genotypically. AMR mutations may not be the main cause of the lack of BI decline. The low transmission of AMR strains at the county level indicates an ongoing transmission at close contact levels.

In this paper, the chemical vapor deposition (CVD) processing for 4H-SiC epilayer is investigated with particular emphasis on the defects and the noise properties. It is experimentally found that the process parameters of C/Si ratio strongly affect the surface roughness of epilayers and the density of triangular defects (TDs), while no direct correlation between the C/Si ratio and the deep level defect Z1/2 could be confirmed. By adjusting the C/Si ratio, a decrease of several orders of magnitudes in the noise level for the 4H-SiC Schottky barrier diodes (SBDs) could be achieved attributing to the improved epilayer quality with low TD density and low surface roughness. The work should provide a helpful clue for further improving the device performance of both the 4H-SiC SBDs and the Schottky barrier ultraviolet photodetectors fabricated on commercial 4H-SiC wafers.

Sensing coverage, which is one of vital issues in the design of wireless sensor networks (WSNs), can usually interact with other performance metrics such as network connectivity and energy consumption. Whatever the metrics, the fundamental problem is to know at least how many sensor nodes are needed to maintain both sensing coverage and network connectivity. In this paper, we propose a Percolation Model on Novel Gilbert Graph (PM-NGG) to obtain the critical density at which the network can become fully covered and connected considering the similarity between the occurrence of percolation and the formation of a covered and connected network. The PM-NGG is based on directional sensor network where sensors are assigned a determined sensing direction with angular intervals varying from 0 to 2 π . Furthermore, we define the sensing and communication model in directional sensor network in presence of channel randomness including deterministic path attenuation, shadow fading, and multipath fading. Besides, we discuss the coverage and connectivity together as a whole under the proposed model. It is worth mentioning that the theoretical analysis and simulation results of the relationship between critical density and transmitting power give insights into the design of directional sensor network in practice.

Abstract Plasma epidermal growth factor receptor mutation (EGFRm) circulating tumor DNA (ctDNA) dynamics exhibit promise in predicting outcomes in patients with EGFRm-advanced non-small cell lung cancer (NSCLC). However, there remains limited trial-level data on integrating ctDNA monitoring into clinical practice. We performed a prospective, multicenter trial to investigate the relationship between EGFRm ctDNA dynamic changes and clinical outcomes in NSCLC patients with EGFRm. Ninety-eight treatment-naive EGFRm-advanced NSCLC patients were recruited and administered icotinib until disease progression. Plasma samples were collected at baseline and four weeks after icotinib administration. ctDNA was analyzed using a droplet-digital polymerase chain reaction. At baseline, 71.4% of patients had detectable EGFRm ctDNA. Among them, 45.9% of patients’ ctDNA became undetectable within four weeks of treatment. Abstract Aim The purpose of the present study was to evaluate the diagnostic performance of serum miR-4534 combined with Transvaginal Color Doppler Ultrasound (TVCDS) in cervical cancer patients. Methods Blood samples from 126 patients with cervical cancer and 83 patients with benign uterine lesions were retrospectively analyzed. Quantitative real time polymerase chain reaction (qRT-PCR) was applied to examine the relative abundances of serum miR-4534 in cervical cancer based on a case–control study.
