Bisphosphonates are well-known drugs as inhibitors of bone resorption acting on inducing programmed cell death of osteoclasts. However, many in vitro studies report that optimal concentration of the bisphosphonate affects not only osteoclasts but also osteoblasts, that is, it induces the anabolic effects of osteoblasts. Recently reported 15-deoxy-Δ12,14-prostaglandin J2 (15d-PGJ2) is an endogenous ligand of peroxisome proliferator-activated receptor-gamma, with an inhibitory activity on bone loss. Researchers have also suggested that 15d-PGJ2 has the ability to reduce bone destruction and as the possibility of regeneration of bone. The purpose of this study is to demonstrate the anabolic effect of two anti-resorptive materials, alendronate and 15d-PGJ2, in a critical sized segmental defect model of rat fibula. The regenerated bone on the operative site was assessed through gross, radiographic (plain X-ray, and micro-computed tomography), histomorphologic evaluation, and statistical analysis. Consequently, the locally applied alendronate prevented resorption of grafted materials, and had a positive effect on bone regeneration with positive micro-architectural modification of the surrounding bone, although this study did not verify a significant capacity of bone regeneration of 15d-PGJ2 and instead only shed a light on its possibility.

J Korean Assoc Oral Maxillofac Surg 2012;38:50-4) According to Luhr's classification, a fracture in the mandible with a width of less than 15-20 mm is considered to be an atrophic mandibular fracture and its incidence is very rare. Because of the reduced cross-sectional area and smaller contact area of the fractured ends as well as the poorly vascularized bony structure and delayed bone healing, an atrophic mandibular fracture is a great challenge for oral and maxillofacial surgeons. Surgeons tend to perform closed reduction, because open reduction is considered a non-life-saving surgery among elderly patients. Thus, most of them have limited experience in surgical management. According to recent reports, open reduction yields a good result, and the Association for Osteosynthesis (AO) group has recommended open reduction. This is a case report of our two experiences of open reduction and rigid fixation of atrophic mandibular fractures by the AO principle. Articles were also reviewed here.

Glandular odontogenic cyst (GOC, 선양치성낭)은 1987년 Gardener가 보고한 이래 현재 문헌으로 보고된 증례가 111 case에 불과한 serre잔사로부터 유래한 희귀한 구강 내 낭 이다. 이것은 중년, 하악에 호발하는 경향을 가지며, 임상 적 특징으로 피질골의 천공, 치근흡수의 국소적으로 공격 적인 양상을 특징으로 하며 환자는 보통 무통성의 종창을 우연히 발견하게 된다. 크기는 0.5-12 cm으로 다양한 경 향을 나타내며. 재발률은 적출술, 소파술의 보존적 처치 시 64.3%, 부분절제술, 변연절제술 등의 광범위 절제술 시 0% 로 조사되어 치성각화낭, 치아모세포종과 같은 임상적 예 후를 보인다. 따라서 재발에 대한 주기적인 경과 관찰이 필수적이다. 방사선학적으로는 다방성의 방사선투과성을 특징으로 한다. 조직병리학적으로는 특징적으로 상피세 포구, 상피세포층 내의 점액세포, 위도관 구조의 세포배열 을 특징으로 한다. 본 교실에서는 하악 우측에 발생된 GOC을 치험하여 그 증례를 공유하기 위하여 문헌 고찰과 함께 보고한다.

Traumatic ulcerative granuloma with stromal eosinophilia (TUGSE)는 흔치 않은 구강 내 궤양성 병소로 만성적이며, 양성의 자기제한적인 병소이다. 하지만 관찰되는 임상양 상은 융기되고 내측으로 함몰된 변연, 공격적인 외양과 크 기로 인하여 편평세포암종으로 흔히 오인된다. 원인 요소 는 만성적인 자극으로 인한 점막파괴로 생각하며, 조직병 리 소견으로는 호중구와 다른 염증세포가 상피층을 넘어 하방의 근육층까지 침범된 특징적 양상을 보이고, 면역 검사에서는 CD30+T-cell이 특징적으로 나타난다. 치료 를 위하여 병소의 절제와 원인 자극요소의 제거, 주기적인 관찰이 필요하다. 하지만 암종으로 오인되는 특징적인 임상양상으로 인해 광범위한 수술과 재건을 계획하기도 하여 술전 절개생검술과 임상가의 주의가 필수이다. 저자 등은 본원에 내원한 TUGSE 환자를 성공적으로 치료한 경 험을 하였기에이를 보고하고자한다.

본 59세 남자 환자는 2009년 6월 하악 우측 치은의 불편 감을 주소로 거주지 인근 대학병원에 내원하여 시행한 절 개 조직 생검 결과 암종으로 진단되어 술 전 방사선 치료 시행 후 수술을 하기 위해 2009년 9월 9일 본과로 협진 의 뢰되었다. 고혈압, 당뇨, 진폐증 등의 전신 질환 병력을 가 지고 있었으며 2009년 7월 27일부터 2009년 9월 2일 까지 상, 하 경부 부위에 총 5040 cGy의 방사선을 조사받은 병력 이 있었다. 2009년 10월 12일 병소부의 광범위 절제술 및 부분적 하 악골 절제술, 우측 선택적 경부 곽청술(level I, II, III)을 시 행한 후 경조직 결손부는 재건 금속판(reconstruction plate) 을 이용하여, 연조직 결손부는 대흉근 피판을 이용하여 재 건하였다. 술후 상처 관리(wound management)하던 중 치유 상태가 좋지 않고 구강 내에서 대흉근 피판 주위로 점막이 용해(lysis)되면서 구강 내 상처 열개(dehiscence)가 생겼다. 이후 열개 부위로 지속적으로 음식물이 끼어 들어가고 타 액이 흘러 들어가게 되면서 우측 경부쪽으로 연조직이 괴 사되어 최종적으로 구강-경부 누공이 형성되었다. 적극적 인 항생제 치료에도 위의 상태가 지속되면서 패혈증 증상 이 심화되어 2009년 11월 4일 재수술을 통하여 괴사 조직 남 웅 120-752 서울특별시서대문구성산로 250 연세대학교치과대학구강악안면외과학교실 Woong Nam Department of Oral and Maxillofacial Surgery, College of Dentistry, Yonsei University, 250 Seongsanno, Seodaemoon-Gu, Seoul, 120-752, Korea Tel: +82-2-2228-2971 FAX: +82-2-2227-8022 E-mail: omsnam@yuhs.ac 구강암 수술 후 발생한 구강 경부 누공(orocutaneous fistula) 치료 시

The end-stage dilated cardiomyopathy is usually treated with cardiac transplantation although some limited success have also been obtained in selected patients using dynamic cardiomyoplasty or medical assist devices. Recently, a new surgical alternatives, called partial left ventriculectomy (PLV) was introduced by Randas J. V Batista in 1995. A 40-year-old man who had end-stage dilated cardiomyopathy refractory to optimal doses of medicines underwent partial left ventriculectomy (Batista’s operation), which reduces ventricular volume to improve left ventricular function. The left ventricular ejection fraction increased from 20 % to 58 % at 4 month after operation. (Korean Circulation J 1998;28(7):1211-1215) KEY WORD:Partial left ventriculectomy.

We report a first case of meningitis due to listeria monocytogenes after cardiac transplantation in Korea. This patient is a 40-year-old man with Dilated cardiomyopathy, he presented with intermittent dyspnea and abdominal distension for about 1 year. After cardiac transplantation, he was treated with azathioprine, cyclosporine and prednisolone for graft rejection. He was presented with intermittent fever, headache and lethargy for about 10days after cardiac transplantation. Listeria monocytogenes was isolated from blood culture and CSF culture. He was treated with intravenous penicillin G for 10days successfully and changed with ampicillin for 10days, took oral ampicillin for 10days without any complication after discharge.
