The glenohumeral internal rotation deficit (GIRD), primarily caused by the tightness of the posterior capsule, is a major risk factor for shoulder injuries in overhead throwing athletes. Quantitative evaluation of posterior capsular thickness and tightness can help determine the relationship between the posterior inferior capsule and GIRD. One previous study has assessed posterior capsule tightness using shear wave elastography (SWE), in college baseball players; however, it did not address the cutoff value of capsular elasticity that could be considered as abnormal capsular tightness. We aimed to re-evaluate effectiveness of SWE in quantifying posterior shoulder capsule tightness in college baseball players and determine the cutoff value of abnormal capsular elasticity that can predict impending throwing-related shoulder injuries associated with GIRD.Twenty-four college baseball players were enrolled in this study. External and internal rotation of the shoulder joint was assessed. The participants were classified into the GIRD group if their throwing shoulder showed20° of internal rotation loss compared with their nonthrowing shoulder. In a longitudinal ultrasonographic scan of the posterior inferior capsule, shear wave (SW) velocity and thickness were measured at the point nearest to the labrum on both shoulders.Of the 24 subjects, 6 had a GIRD. The mean value of the SW velocity in the throwing shoulder was greater in the GIRD group than in the non-GIRD group (P = .006). The SW velocity difference between the throwing and nonthrowing shoulder was also greater in the GIRD group than in the non-GIRD group (P.001). There was no significant difference in the thickness of the posterior inferior capsule between both groups. In correlation analysis, the difference in the SW velocity was more strongly correlated with the GIRD than with the SW velocity in the throwing shoulder. When we assume that a 20° GIRD is indicative of a shoulder at risk, the cutoff SW velocity in the throwing shoulder is 4.81 m/s and the SW velocity difference is 0.77 m/s.The SW velocity is closely associated with posterior shoulder capsular tightness and may be of quantitative value in baseball players.

BACKGROUND: This study evaluated the shoulder girdle muscle characteristics of elite archery athletes and provides suggestions for archery training programs.METHODS: This study enrolled 15 cases of high level archery athletes (7 males, 8 females) and 30 cases of (15 males, 15 females) sex, age-matched, healthy, non-athletic individuals. We measured peak torques of flexion (FL), extension (EX), abduction (ABD), adduction (ADD), external rotation (ER) and internal rotation (IR) of both shoulders at an angular velocity of 30°/sec, 60°/sec, and 180°/sec. The peak torques and peak torque ratios of FL/EX, ABD/ADD, and ER/IR of the two groups were compared.RESULTS: The archer group had a greater peak torque of IR and ADD, but only in the left shoulder (p < 0.05). In the same group, both shoulders had greater peak torque of EX and lower peak torque of FL. The peak torque ratios of FL/EX of both shoulders were significantly lower in the archer group at all three angular velocities (p < 0.05). The peak torque ratios of ABD/ADD were significantly greater in only the left shoulder of the archer group (p < 0.05).CONCLUSIONS: The prominent characteristics of the shoulder girdle muscles of an elite archer are stronger adductor muscles of the bow shoulder and stronger extensors of both shoulders, as compared to healthy, non-athletic individuals. These muscle groups of the shoulder probably contribute a major role in maintaining the accuracy and stability during archery shooting. Hence, a training program that selectively enhances the adductor and extensor muscles could prove helpful in enhancing the archery skills of the athlete.

We expect this work will help construct an intelligent crime-prevention system to protect the weak in our society.

Purpose To determine the prevalence and characteristics of neuropathic pain (NP) in patients with lumbar spinal stenosis (LSS) according to subgroup analysis of symptoms. Materials and Methods We prospectively enrolled subjects with LSS (n=86) who were scheduled to undergo spinal surgery. The patients were divided into two groups according to a chief complaint of radicular pain or neurogenic claudication. We measured patient's pain score using the visual analog scale (VAS), Oswestry Disability Index (ODI) and Leads Assessment of Neuropathic Symptoms and Signs (LANSS). According to LANSS value, the prevalence of NP component pain in patients with LSS was assessed. Statistical analysis was performed to find the relationship between LANSS scores and the other scores. Results From our sample of 86 patients, 31 (36.0%) had a NP component, with 24 (63.4%) in the radicular pain group having NP. However, only seven patients (15.6%) in the neurogenic claudication group had NP. For accurate analysis and decision-making of the most common indicators of the stock market by proposing a number of indicators of economic transformation techniques were applied to the convergence model combining estimation and forecasts problem confirmed its effectiveness. Experimental result, gave the model estimation method to apply a transform to show the valid combinations proposed model state estimation result was confirmed in a very similar exercise aspect of the physical problem and the KOSPI index prediction.

Resection margin involvement after curative intent resection for gastric cancer results in a poor prognosis and deprives the patient of the chance for a cure. Reoperation to achieve an R0 status should guarantee tolerable morbidity and achievement of negative margins. We performed laparoscopic distal gastrectomy with extracorporeal Billroth II reconstruction in a 56-year-old woman with gastric cancer following neoadjuvant chemotherapy. Scattered cancer cells were observed in the proximal and distal resection margins on immunohistochemical staining for cytokeratin. Two weeks postoperatively, remnant total gastrectomy and supra-ampullary duodenectomy were performed. Before reoperation, percutaneous transhepatic gallbladder drainage and angiocatheter placement outside the ampulla of Vater (AoV) via the cystic duct were performed to avoid pancreaticoduodenectomy and to obtain the maximal distal margin. Duodenal transection was performed 1 cm above the AoV. The resected duodenum was 4 cm in length. The patient had no postoperative complications and received adjuvant chemotherapy 1 month after the reoperation.

Is generated in real time in the real world, a large amount of time series data from a wide range of business areas. But it is not easy to determine the optimal model for the description and understanding of the time series data is represented as a dynamic feature. In this study, through the HMM suitable for estimating the short and long-term forecasting model of time-series data to estimate a model that can explain the characteristics of these time series data, it was estimated to predict future patterns of movement. The actual stock market through various materials, information criterion and optimal model estimation for the length of the most efficient data was found to accurately estimate the state of the model. Similar movement patterns predictive than the long-term prediction is more similar to the short-term prediction of the experimental result were found to be.

Forex trading participants, due to the intensified economic internationalization exchange risk avoidance measures are needed. In this research, Model suitable for estimation of time-series data, such as stock prices and exchange rates, through the concealment of HMM and estimate the short-term exchange rate forecasting model is applied to the prediction of the future. Estimated by applying the optimal model if the real exchange rate data for a certain period of the future will be able to predict the movement aspect of it. Alleged concealment of HMM. For the estimation of the model to accurately estimate the number of states of the model via Bayesian Information Criterion was confirmed as a model predictive aspect of physical exercise aspect and predict the movement of the two curves were similar. Capsaicin is a major pungent ingredient found in hot pepper, has long been reported to control of obesity and anti-carcinogenic activities. Capsaicin induced apoptosis in a various cancer cells, however the precise molecular mechanisms have been poorly understood. In present study, the effect of capsaicin in cell viability the U87MG human glioma cells and its molecular mechanisms of cell death were investigated. Capsaicin induced reduction of cell viability in doseand time-dependent manners. Apoptosis was determined based on the increase of positive TUNEL stained cells. The mechanisms of apoptosis were related with mitochondrial pathway (Bcl-2/Bax), activation of MAPK pathway. These data suggest that capsaicin could be a novel chemotherapeutic agent of human malignant gliomas.

Among various scenarios that are applicable to designed system, development and test were conducted based on scenario that provides shopping information and discount information to customers through SMS as they approach to vicinity of large discount store and allow them to download discount coupons through Call Back. The suggested system will be applied to various service event and can be used as a customized real time marketing method according to user`s personal activity area.

Most real word systems such as world economy, stock market, and medical applications, contain a series of dynamic and complex phenomena. One of common methods to understand these systems is to build a model and analyze the behavior of the system. In this paper, we investigated methods for best clustering over time series data. As a first step for clustering, BIC (Bayesian Information Criterion) approximation is used to determine the number of clusters. A search technique to improve clustering efficiency is also suggested by analyzing the relationship between data size and BIC values. For clustering, two methods, model-based and similarity based methods, are analyzed and compared. A number of experiments have been performed to check its validity using real data(stock price). BIC approximation measure has been confirmed that it suggests best number of clusters through experiments provided that the number of data is relatively large. It is also confirmed that the model-based clustering produces more reliable clustering than similarity based ones.

Influence of substrate temperature and atmosphere on the electrical and optical properties of Ga-doped ZnO thin films deposited by rf magnetron sputtering were investigated. The electrical resistivity of Ga-doped ZnO (GZO) films decreases first and then increases as the substrate temperature increases from room temperature to 400 °C. A minimum resistivity of 3.3 x 10 -4 Ω cm is obtained at 300 °C. The resistivity nearly does not change with the O 2 /Ar flow ratio, R for R 0.25. Changes in resistivity with the substrate temperature and R are related to the crystallinity of GZO films. The crystallinity is enhanced as R increases, but if the oxygen partial pressure is higher than a certain level (R = 0.25 ± 0.10) gallium oxides precipitate at grain boundaries, which decrease both carrier concentration and mobility. Optical transmittance increases as R increases for R < 0.75. This change in transmittance with R is related to changes in oxygen vacancy concentration and surface roughness with R.

Choroid plexus cysts (CPCs) are the most common neuroepithelial cysts, occurring in more than 50% of some autopsy series. They are typically small and asymptomatic and are discovered incidentally in older patients, usually in the trigone of the lateral ventricle. Symptomatic CPCs (usually exceptionally large, 2-8 cm) are rare. The authors report a case of large symptomatic choroid plexus cyst, located in the trigone of the right lateral ventricle in a 26-yr-old man who presented with headache and vomiting. The patient underwent endoscopic removal through a burr hole placed 3 cm from the midline and just behind the hair line. The histological examination of the cyst wall was consistent with choroid epithelium. Despite of postoperative intraventricular hemorrhage and catheter infection, he discharged home without neurologic deficits. The endoscopic fenestration rather than excision should be considered as the first surgical procedure because the goal of treatment is shrinkage of the cyst until normal cerebrospinal fluid flow is restored.

We report the first observation of hyperfine structures (HFSs) and isotope shifts (ISs) of Sm I in the range of 540–560 nm using an atomic beam resonance–fluorescence technique. The least square fitting was used to determine hyperfine constants in 4f 5 5d6s 2 and 4f 6 6s6p of atomic samarium (Sm I). The isotope shifts of 18 transition lines were also determined and analyzed by the King plot.

We experimentally demonstrated that optical vortices with opposite charges could be generated on a probe beam transmitted through a medium composed of rubidium atoms with V-type energy levels when the refractive index of the medium was modified by a strong pump laser beam with an L-shape intensity distribution.

We performed the experiment of self-induced transparency in samarium atomic vapor under the condition of high temperature and high density. The sudden onset of the transmission, the maximum pulse delay and the pulse broadening were observed with π pulse. With this result we estimate the dipole moment of the transition line for 152Sm. The peak amplification, the pulse narrowing and the pulse breakup were observed when the pulse area was above 3π. These observations are strong evidences of self-induced transparency. Our result is the observation of self-induced transparency under the highest-temperature, highest-density condition and the first observation of this phenomenon in samarium vapor to our knowledge.

Background: Graft extrusion after meniscus allograft transplantation (MAT) may be affected by horn fixation, which differs between medial and lateral MAT. Few studies have compared graft extrusion, especially sagittal extrusion, after medial and lateral MAT. Hypothesis: In patients undergoing medial and lateral MAT, graft extrusion is likely similar and not correlated with postoperative Lysholm scores. Study Design: Cohort study; Level of evidence, 2. Methods: Meniscus graft extrusion in the coronal and sagittal planes was compared in 51 knees undergoing medial MAT and 84 undergoing lateral MAT. Distances from the anterior and posterior articular cartilage margins to the anterior (anterior cartilage meniscus distance [ACMD]) and posterior (posterior cartilage meniscus distance [PCMD]) horns, respectively, were assessed on immediate postoperative magnetic resonance imaging and compared in patients undergoing medial and lateral MAT. Correlations between coronal and sagittal graft extrusion and between extrusion and the Lysholm score were compared in the 2 groups. Results: In the coronal plane, mean absolute (4.3 vs 2.7 mm, respectively; P < .001) and relative (39% vs 21%, respectively; P < .001) graft extrusions were significantly greater for medial than lateral MAT. In the sagittal plane, mean absolute and relative ACMD and PCMD values were significantly greater for medial than lateral MAT ( P < .001 each). For both medial and lateral MAT, mean absolute and relative ACMDs were significantly larger than PCMDs ( P < .001 each). Graft extrusion >3 mm in the coronal plane was significantly more frequent in the medial (78%) than in the lateral (35%) MAT group. In the sagittal plane, the frequencies of ACMDs (72% vs 39%, respectively) and PCMDs (23% vs 4%, respectively) >3 mm were also significantly greater in the medial than in the lateral MAT group. Coronal and sagittal extrusions were not correlated with postoperative Lysholm scores for both medial and lateral MAT. Conclusion: The amount and incidence of graft extrusion were greater after medial than lateral MAT in both the coronal and sagittal planes. In the sagittal plane, graft extrusion was greater and more frequent on the anterior than the posterior horn in both medial and lateral MAT. However, graft extrusion was not correlated with early clinical outcomes after both medial and lateral MAT.

Purpose This study assessed the incidence of graft extrusion on the sagittal plane on magnetic resonance imaging (MRI) and evaluated the correlation between the sagittal position of the allograft and coronal graft extrusion. Methods The study involved 99 patients who underwent lateral meniscus allograft transplantation (LMAT) for knees that had undergone total meniscectomy and 50 sex- and age-matched control patients who underwent MRI for evaluation of knee pain and had no intra-articular lesions. Graft extrusion and sagittal graft position parameters, including the distance from the articular cartilage center to the anterior meniscus (CAMD), the distance from the articular cartilage center to the posterior meniscus, the distance from the anterior articular cartilage margin to the anterior horn (ACMD), or the distance from the posterior articular cartilage margin to the posterior horn, were assessed on immediate postoperative MRI studies (2 days after surgery) and compared between the LMAT and control groups. In the LMAT group, correlations between graft extrusion and MRI parameters were analyzed, and multiple linear regression analysis was performed to identify predictors of graft extrusion. Results The mean CAMD and mean ACMD were significantly greater and the mean distance from the articular cartilage center to the posterior meniscus and the mean distance from the posterior articular cartilage margin to the posterior horn were significantly smaller in the LMAT group than in the normal control group ( P r = 0.294, P = .015) and ACMD ( r = 0.244, P = .041) correlated with relative extrusion, and CAMD ( r = 0.288, P = .013) correlated with absolute extrusion. CAMD was the only predictor independently associated with both absolute (β = 0.248, P = .013) and relative (β = 0.244, P = .015) extrusion. Conclusions Transplanted lateral meniscal allografts were located more anteriorly on the sagittal plane than normal lateral menisci. More anterior allograft placement correlated with a greater degree of graft extrusion on the coronal plane. Level of Evidence Level III, retrospective case-control study.

Few reports to date have discussed acute deltoid muscle injury, including partial or complete deltoid muscle detachment from its origin. The present report describes a different pathomechanism and clinical manifestations of deltoid muscle injury. A 52-year-old man experienced acute severe pain in the right shoulder as a result of abrupt elevation of the arm. The deltoid muscle had apparently been scratched by an osteophyte on the greater tuberosity. After arthroscopic excision of the osteophyte, the symptoms resolved completely.

The aim of this study was to investigate the mechanism underlying the development of gap differences in total knee arthroplasty using the navigation-assisted gap technique and to assess whether these gap differences have statistical significance. Ninety-two patients (105 knees) implanted with cruciate-retaining prostheses using the navigation-assisted gap balancing technique were prospectively analysed. Medial extension and flexion gaps and lateral extension and flexion gaps were measured at full extension and at 90° of flexion. Repeated measures analysis of variance was used to compare the mean values of these four gaps. The correlation coefficient between each pair of gaps was assessed using Pearson’s correlation analysis. We hereby report a case of acute colitis resulting from coffee enema, which was presented with severe abdominal pain and hematochezia.

Flow boiling in parallel microchannels has received attention as an effective cooling method for high-power-density microprocessor. Despite a number of experimental studies, the bubble dynamics coupled with boiling heat transfer in microchannels is still not well understood due to the technological difficulties in obtaining detailed measurements of microscale two-phase flows. In this study, complete numerical simulation is performed to further clarify the physics of flow boiling in microchannels. The level set method for tracking the liquid-vapor interface is modified to include the effects of phase change and contact angle. The method is further extended to treat the no-slip and contact angle conditions on the immersed solid. Also, the reverse flow observed during flow boiling in parallel microchannels has been investigated. Based on the numerical results, the effects of channel shape and inlet area restriction on the bubble growth, reverse flow and heat transfer are quantified.Copyright © 2008 by ASME

In our experiment conical emission (CE) has no wavelength shift and the same polarization as the original beam. Varying laser power, detuning, and atomic density, the images of CE were captured and analyzed. Typical far-field images are presented. Under the blue detuning condition, manifest CE patterns were observed, and under red detuning, original beam blurs due to not only to self-defocusing but also the conical pattern were observed. By comparing laser power and atomic density dependence of cone angle and theoretical curve, we could estimate spectroscopic data such as nonlinear refractive index. From the density dependence data, we get the fact that cone angle is proportional to atomic density which supports the self-phase-modulation (SPM) model. In conclusion, we observed and analyzed CE from near-resonant Sm vapor. With this simple setup, we could measure spectroscopic data.

41. Neuropathic Pain Components in Patients with Lumbar Spinal Stenosis.

- Abstract
Purpose: To determine the prevalence and characteristics of neuropathic pain (NP) in patients with lumbar spinal stenosis (LSS) according to subgroup analysis of symptoms. Materials and Methods: We prospectively enrolled subjects with LSS (n=86) who were scheduled to undergo spinal surgery. The patients were divided into two groups according to a chief complaint of radicular pain or neurogenic claudication. We measured patient's pain score using the visual analog scale (VAS), Oswestry Disability Index (ODI) and Leads Assessment of Neuropathic Symptoms and Signs (LANSS). According to LANSS value, the prevalence of NP component pain in patients with LSS was assessed. Statistical analysis was performed to find the relationship between LANSS scores and the other scores. Results: From our sample of 86 patients, 31 (36.0%) had a NP component, with 24 (63.4%) in the radicular pain group having NP. However, only seven patients (15.6%) in the neurogenic claudication group had NP. The LANSS pain score was not significantly correlated with VAS scores for back pain, but did correlate with VAS scores for leg pain (R=0.73, p<0.001) and with ODI back pain scores (R=0.54, p<0.01). Conclusion: One-third of the patients with LSS had a NP component. The presence of radicular pain correlated strongly with NP. The severity of leg pain and ODI score were also closely related to a NP component. This data may prove useful to understanding the pain characteristics of LSS and in better designing clinical trials for NP treatment in patients with LSS. [ABSTRACT FROM AUTHOR]

We studied the conical emission in samarium (Sm) vapor under the near-resonant condition. The incident dye laser was tuned to the transition line, 4f66s2 7F→4f6(7F)6s6p(1P0)(λ=562.601 nm), of the Sm atom. Using a high-temperature oven, we could obtain the atomic density of 8×1014 atoms/cm3, large enough to observe the conical emission. We observed one to three rings around the original laser beam, and the number of the rings depended on the laser intensity, the laser wavelength, and the atomic density. These results are attributed to the self-phase modulation of the laser beam interacting with the near-resonant atomic medium. We give a simple expression that describes the dependence of the locations of the rings on the laser intensity. We compare the experimental results with this expression quantitatively and then estimate the nonlinear susceptibility of Sm vapor.

We present simple methods to produce optical vortices on the axis of beam propagation with nonspiral phase plates. We show that a phase plate that provides linear phase retardation on one half of a laser beam produces optical vortices, which is demonstrated experimentally by use of a thickness-varying glass platelet. We also demonstrate and explain that mixed dislocations of a bent edge dislocation transform into a pair of vortices with opposing topological charges.

Abstract Background Acute retinal necrosis (ARN) has characterized by panuveitis, vitritis, severe vaso-occlusive vasculitis, and diffuse necrotizing retinitis. There are no case reports on atypical ARN combined with Terson’s syndrome. Herein, we report a case of ARN with atypical clinical features combined with Terson’s syndrome that we successfully treated by intravitreal ganciclovir injection. Case presentation A 64-year-old man visited our eye clinic with a complaint of decreased visual acuity in his right eye. At the initial visit, his best corrected visual acuity was 20/125 in the right eye. Slit-lamp examination demonstrated mild hyperemia, keratic precipitates, and anterior chamber inflammatory reaction. Fundus examination revealed multiple diffuse white-yellowish infiltrations in the peripheral retina combined with dot hemorrhages. Ultra-wide-field fluorescence angiography showed obstructive arteritis with peripheral non-perfusion and leakage from the retinal vessels. As a result of the PCR analysis, varicella zoster virus DNA was identified in the aqueous humor. Under the diagnosis with VZV-mediated ARN, we started with intravenous acyclovir and oral prednisolone. After 3 days of the above treatment, the anterior chamber inflammation and vitreous opacity were increased. On fundus examination, multiple whitish infiltrations were increased. In addition, newly developed vitreous and peripapillary hemorrhages were detected. On the T2 brain magnetic resonance imaging (MRI) demonstrated a sub-acute or old hemorrhagic infarction in the right occipital lobe, and contrast-enhancing lesions in the right basal ganglia. The spinal tapping was performed in the department of neurology in our hospital at the time when the patient complained of headache, and intracranial pressure was 31 mmHg. Under the diagnosis of ARN with Terson’s syndrome, we started intravitreal ganciclovir (2 mg/0.5 ml) injections. After 5 intravitreal ganciclovir injections over a period of 8 months, the diffuse whitish infiltrating retinal lesions combined with dot hemorrhage were decreased. The vitreous and peripapillary hemorrhage was significantly reduced. There was no recurrence in the patient’s right eye, in which his visual acuity had improved to 20/60. Conclusions In the event of a poor response to traditional treatment such as intravenous acyclovir, intravitreal ganciclovir may have a role as an adjunctive therapy in patients of VZV associated ARN combined with Terson’s syndrome.

Background/Aims: Inflammatory bowel disease (IBD), including Crohn's disease (CD) and ulcerative colitis (UC), has been reported to have various ophthalmologic manifestations. The aim of this study was to evaluate the prevalence of ophthalmologic manifestations associated with IBD in Korea.Methods: Sixty-one patients were examined between May 2013 and October 2014. We performed complete ophthalmologic examinations.Results: Findings included 36 patients with CD and 25 with UC. The mean age of the patients was 34±16 years and disease duration was 45.3±23.9 months. Ophthalmologic manifestations were positive in 44 cases. Primary complication was diagnosed in 5 cases, as follows; iritis in 2 cases, episcleritis in one case, iritis with optic neuritis in 1 case, and serous retinal detachment in 1 case, without secondary complications. The most common coincidental complication was dry eye syndrome (DES), in 35 patients (57.4%). The prevalence of DES in the control group was 21.3%. The proportion of DES in patients with IBD was significantly higher than in the control group (P=0.002).Conclusions: Ophthalmologic manifestations were high (72.1%) in IBD patients. Clinically significant primary ocular inflammation occurred in 8.2% of patients. The most common complication was DES. There was a higher rate of DES in patients with IBD compared to the control group. Evaluation of the eye should be a routine component in patients with IBD.

Purpose Acute care surgery (ACS) models have evolved worldwide over the last decade. However, South Korea has an established trauma system and does not consider the ACS model. This study compares the management and outcome of emergency cholecystectomy in the ACS model to those of traditional on-call attending surgeon model for emergency surgery. Methods Retrospectively collected data for patients who underwent emergency cholecystectomy from May 2013 to January 2015 was analyzed to compare data from a traditional on-call system (OCS) and ACS. Results One hundred and twenty-four patients were enrolled in the study (62 patients ACS vs. 62 patients OCS). Hospital stay (days) (ACS=4.29±2.49 vs. OCS=4.82±4.48, p=0.46) and stay in emergency room (minutes) (ACS=213.10±113.99 vs. OCS=241.10±150.73, p=0.20) did not differ significantly between groups. OBJECTIVE:To evaluate the seizure characteristics and outcome after immunotherapy in adult patients with autoimmune encephalitis (AE) and new-onset seizure. METHODS:Adult (age ≥18 years) patients with AE and new-onset seizure who underwent immunotherapy and were followed-up for at least 6 months were included. Seizure frequency was evaluated at 2-4 weeks and 6 months after the onset of the initial immunotherapy and was categorized as "seizure remission", "> 50% seizure reduction", or "no change" based on the degree of its decrease. RESULTS:Forty-one AE patients who presented with new-onset seizure were analysed. At 2-4 weeks after the initial immunotherapy, 51.2% of the patients were seizure free, and 24.4% had significant seizure reduction. At 6 months, seizure remission was observed in 73.2% of the patients, although four patients died during hospitalization. Rituximab was used as a second-line immunotherapy in 12 patients who continued to have seizures despite the initial immunotherapy, and additional seizure remission was achieved in 66.6% of them. In particular, those who exhibited partial response to the initial immunotherapy had a better seizure outcome after rituximab, with low adverse events. CONCLUSION:AE frequently presented as seizure, but only 18.9% of the living patients suffered from seizure at 6 months after immunotherapy. Aggressive immunotherapy can improve seizure outcome in patients with AE.

50. Improved Yield and Functional Parameters of Rat Pancreas Islets Isolated under Intramuscular Anesthesia

Intraperitoneal (IP) anesthesia is commonly used for laboratory animal experiments including rat islet isolation. However, the direct effects of anesthetics on pancreatic islets have been neglected. This study compared the islet function and recovery yield from rats that were anesthetized using IP and intramuscular (IM) injection. In addition, the lag time required to lose deep pain was measured in the following anesthetics combinations. Lewis rats were anesthetized using ketamine and xylazine (K/X) or zoletil and xylazine (Z/X). A glucose challenge test was performed on each group of prepared islets. The effect of the anesthetic agents (e.g., ketamine, zoletil, xylazine alone, and the combination of K/X and Z/X) on cell lines (rat insulinoma; RIN-5F) was investigated by determining their effect on the cell viability, the amount of insulin, and insulin mRNA expression levels of RIN-5F. The time needed for deep anesthesia in IM anesthesia was significantly shortened in comparison to IP [K/X (IM: 313 ± 66 s, IP: 371 ± 84 s) and Z/X (IM: 206 ± 76 s, IP: 245 ± 92 s)]. In addition, number of isolated islet yield by IM anesthesia was significantly improved [K/X (IM: 1530 ± 242, IP: 1245 ± 149) and Z/X (IM: 1136 ± 226, IP: 511 ± 154)]. The functions of fresh islets, indicated by the stimulation index, acquired under IM anesthesia was better preserved than that of IP. The viability and the insulin secretion of RIN-5F were decreased at 24 and 48 h. Insulin gene expression levels were decreased at 24 h as well. Anesthetics may be absorbed through the pancreas surface to the islets and have a direct effect, resulting in islet exposure and deterioration during isolation. In conclusion, for rodent islet isolation, IM anesthesia is simpler and safer in comparison to IP anesthesia.
