Background: The clinical presentation of COVID‐19 ranges from a mild, self‐limiting disease, to multiple organ failure and death. Most severe COVID‐19 cases present low lymphocytes counts and high leukocytes counts, and accumulated evidence suggests that in a subgroup of patients presenting severe COVID‐19, there may be a hyperinflammatory response driving a severe hypercytokinaemia which may be, at least in part, signalling the presence of an underlying endothelial dysfunction. In this context, available data suggest a prognostic role of neutrophil‐lymphocyte ratio (NLR) in various inflammatory diseases and oncological processes. Following this rationale, we hypothesized that NLR, as a marker of endothelial dysfunction, may be useful in identifying patients with a poor prognosis in hospitalized COVID‐19 cases. Design: A retrospective observational study performed at Hospital Universitario HM Puerta del Sur, Madrid, Spain, which included 119 patients with COVID‐19 from 1 March to 31 March 2020. Patients were categorized according to WHO R&D Expert Group. Results: Forty‐five (12.1%) patients experienced severe acute respiratory failure requiring respiratory support. Forty‐seven (12.6%) patients died. Those with worse outcomes were older (P =.002) and presented significantly higher NLR at admission (P =.001), greater increase in Peak NLR (P <.001) and higher increasing speed of NLR (P =.003) compared with follow‐up patients. In a multivariable logistic regression, age, cardiovascular disease and C‐reactive protein at admission and Peak NLR were significantly associated with death. Conclusions: NLR is an easily measurable, available, cost‐effective and reliable parameter, which continuous monitoring could be useful for the diagnosis and treatment of COVID‐19. The superadobe is a construction technique that consists on building using bags filled with soil and adding barbwire between the courses. It is a simple, durable, sustainable, and affordable technique that has increased in popularity over the last years. Moreover, a remarkable effort was performed in the past years in order to analyse the structural behaviour and characterize the mechanical properties of these constructions. However, since it is a relatively recent technique, no previous studies of complete structures have been performed using numerical strategies and there is still a lack of knowledge regarding its structural response. Consequently, the present thesis attempts to analyse these type of structures using the finite element method, assessing its suitability and applicability to the superadobe. For this purpose, several models of both walls and arches are studied, subjected to different load dispositions and considering three construction types, plain, adding barbwire and stabilizing the infill. Initially two possible constitutive models are considered, Mohr Coulomb and Total strain crack based, and they are evaluated and compared in order to select the most suitable for all the cases studied. Afterwards, all the analyses are performed and calibrated to match the results obtained from the experimental campaigns developed in the University of Bath by Pelly (2009), Vagdama (2010) and Croft (2011). Finally, a sensitivity analysis is also carried out to examine the influence of certain parameters, which had to be adjusted during the analyses, into the structural response of the models. El superadobe es una solución constructiva que consiste en la utilización de bolsas rellenas de tierra apisonadas a las que se les añade alambre de espino entre ellas para aportar cohesión y Resistencia a tracción al conjunto. Es una técnica simple, duradera, sostenible y asequible cuya popularidad ha ido creciendo durante los últimos años. Además, recientemente se ha realizado un notable esfuerzo con el fin de analizar el comportamiento estructural y determinar las propiedades mecánicas del superadobe. No obstante, al tratarse de una técnica relativamente nueva, no se han desarrollado aún estudios de estructuras completas utilizando estrategias numéricas, y existe todavía una carencia de información en lo que respecta a la respuesta estructural de estas construcciones. Por consiguiente, la presente tesis tiene como objetivo el análisis de estas estructuras utilizando el método de los elementos finitos, evaluando su compatibilidad y aplicabilidad al superadobe. Con este fin, se estudian varios modelos, tanto de muros como de arcos, sujetos a diferentes disposiciones de carga y considerando tres tipos de construcción, simple, añadiendo alambre de espino y estabilizando el relleno de las bolsas. Inicialmente se consideran dos posibles modelos constitutivos, Mohr Coulomb y Total strain crack based, se evalúan y comparan a fin de seleccionar el que más se adecúa a todos los casos estudiados. Posteriormente, se llevarán a cabo y calibrarán todos los análisis para hacer coincidir los resultados con los obtenidos en las campañas experimentales desarrolladas en la Universidad de Bath por Pelly (2009), Vagdama (2010) y Croft (2011). Finalmente, también se realizará un estudio de sensibilidad para examinar la influencia de ciertos parámetros, que han debido ser ajustados durante los análisis, en la respuesta estructural de los modelos.

El núcleo de este trabajo consiste en un programa de actuación sobre el duelo infantil en un centro escolar a través de distintas respuestas educativas para atender a las necesidades tanto la familia como el alumno en duelo, teniendo en cuenta a toda la comunidad educativa Para la elaboración de dicho programa de actuación previamente se realizó una revisión bibliográfica. Dicha fundamentación teórica incluye una conceptualización sobre la cultura de la muerte, las características del duelo, sus consecuencias en la etapa de educación primaria y la pedagogía de la muerte en el aula. A continuación, se procede al desarrollo del programa de actuación, en el cual se establecerán lo objetivos y los pasos a seguir para dar respuesta a las necesidades del alumno y la familia. También se plantearán diferentes actividades para introducir la pedagogía de la muerte de manera lúdica en las aulas.

[ES] A pesar del placer asociado al consumo de golosinas, los problemas de caries, sobrepeso y aumento de glucosa en sangre asociadas a las mismas implican un control en su ingesta. En este sentido, el reemplazamiento de los azúcares convencionales por edulcorantes saludables, supondría un gran avance en el sector de la confitería. Concretamente, en este trabajo se han reformulado golosinas tipo espuma dulce utilizando diferentes proporciones de isomaltulosa y oligofructosa y/o extractos acuosos de estevia. A su vez, se han reemplazado los colorantes y aromas comerciales por jugo de fresa natural con el fin de obtener un producto más saludable. Se han realizado determinaciones de ºBrix pH, humedad, actividad de agua, y capacidad antioxidante, así como de propiedades de color y de textura a lo largo del tiempo. Además, se ha llevado a cabo un análisis sensorial, para conocer la aceptación que tendría el producto. Los resultados ponen de manifiesto que elevadas proporciones de isomaltulosa endurecen considerablemente el producto y por tanto no serían adecuadas para la reformulación de espumas dulces. En cuanto a la estevia, no mejora la capacidad antioxidante de las muestras y supone un rechazo en la evaluación sensorial del colectivo infantil. Las espumas dulces elaboradas con la mayor proporción de oligofructosa, presentan una intención de compra elevada, aunque menor que las espumas tradicionales. Sin embargo, sería necesario mejorar el aspecto y el aroma de las muestra formuladas con jugo de fruta de fresa., [EN] Despite the pleasure linked to the consumption of candies, it is necessary a control in their consumption due to the problems of tooth decay, overweight and increase in glucose-blood associated with them. In this regard, replacement of conventional sugars by healthy sweeteners would mean a great advance in the confectionery sector. Concretely, in this study candies type marshmallows have been reformulated by using different proportions of isomaltulose and oligofructose and/or aqueous extracts of stevia. Besides, commercial colourings and flavourings have been substituted by natural strawberry juice in order to obtain a healthier product. Analysis of ºBrix, pH, moisture content, water activity and antioxidant capacity along with measures of colour and mechanical properties have been carried out throughout time. Furthermore, a sensorial analysis has been performed to know the acceptability that this product could have. Results give evidences that high proportions of isomaltulose harden considerably the product and therefore they would not be suitable to reformulate marshmallows. Regarding stevia, it does not improve antioxidant capacity and means a rejection in the sensorial analysis of children. Marshmallows prepared with the highest proportion of oligofructose show a great purchase intention although lower than for traditional marshmallows. Concretament, en aquest treball s'han reformulat llepolies tipus espuma dolça utilitzant diferents proporcions d'isomaltulosa i oligofructosa i/o extractes aquosos d'estevia. Al seu torn, s'han reemplaçat els colorants i aromes comercials per suc de maduixa natural a fi d'obtindre un producte més saludable. S'han realitzat determinacions de ºBrix pH, humitat, activitat d'aigua, i capacitat antioxidant, així com de propietats de color i de textura al llarg del temps. A més, s'ha dut a terme una anàlisi sensorial, per a conèixer l'acceptació que tindria el producte. Els resultats posen de manifest que elevades proporcions d'isomaltulosa endureixen considerablement el producte i per tant no serien adequades per a la reformulació de espumes dolces. Quant a l'estevia, no millora la capacitat antioxidant de les mostres i suposa un rebuig en l'avaluació sensorial del col·lectiu infantil. Les espumes dolces elaborades amb la major proporció d'oligofructosa, presenten una intenció de compra elevada, encara que menor que les bromeres tradicionals. No obstant això, seria necessari millorar l'aspecte i l'aroma de les mostra formulades amb suc de fruita de maduixa.

16. Usefulness of the Hemogram in COVID-19

SARS-CoV2 infection has devastating consequences on healthcare systems and has caused 3 million deaths by April 2021. Identifying patients at risk of death is a priority. Moderate–severe COVID-19 cases seem to associate a cytokine release that follows endothelial injury, triggering a hyperinflammatory and procoagulant state in which leukocytes and platelets are protagonists. Our group has published some reports about the usefulness of the hemogram in COVID-19. Hemogram-derived ratios, mainly the neutrophil-to-lymphocyte ratio (NLR) and the novelty neutrophil-to-platelet ratio (NPR), obtained on admission and their rate of change during hospitalization, can easily detect patients with high risk of mortality. Hemogram is a tool available to all hospitals and analyzing the hemogram-derived ratios would provide much more information than could be extracted by evaluating the counts in isolation. We now know that in COVID-19 it is essential to start early anti-inflammatory treatment when patient deteriorates and the hemogram could be a good indicator of this situation. More comprehensive studies are needed to determine how useful these hemogram-derived ratios and prognostic scores are. In the next chapter we will present information related to this aspect as well as our group’s research on the usefulness of the hemogram in COVID-19.
