Abstract

Electronic health records (EHR) is an inherently multimodal register of the patient's health status characterized by static data and multivariate time series (MTS). While MTS are a valuable tool for clinical prediction, their fusion with other data modalities can possibly result in more thorough insights and more accurate results. Deep neural networks (DNNs) have emerged as fundamental tools for identifying and defining underlying patterns in the healthcare domain. However, fundamental improvements in interpretability are needed for DNN models to be widely used in the clinical setting. In this study, we present an approach built on a collection of interpretable multimodal data-driven models that may anticipate and understand the emergence of antimicrobial multidrug resistance (AMR) germs in the intensive care unit (ICU) of the University Hospital of Fuenlabrada (Madrid, Spain). The profile and initial health status of the patient are modeled using static variables, while the evolution of the patient's health status during the ICU stay is modeled using several MTS, including mechanical ventilation and antibiotics intake. The multimodal DNNs models proposed in this paper include interpretable principles in addition to being effective at predicting AMR and providing an explainable prediction support system for AMR in the ICU. Furthermore, our proposed methodology based on multimodal models and interpretability schemes can be leveraged in additional clinical problems dealing with EHR data, broadening the impact and applicability of our results.

Abstract

Introduction: Bayesian approaches to patient phenotyping in clinical observational studies have been limited by the computational challenges associated with applying the Markov Chain Monte Carlo (MCMC) approach to real-world data. Approximate Bayesian inference via optimization of the variational evidence lower bound, variational Bayes (VB), has been successfully demonstrated for other applications. Methods: We investigate the performance and characteristics of currently available VB and MCMC software to explore the practicability of available approaches and provide guidance for clinical practitioners. Two case studies are used to fully explore the methods covering a variety of real-world data. First, we use the publicly available Pima Indian diabetes data to comprehensively compare VB implementations of logistic regression. Second, a large real-world data set, Optum EHR with approximately one million diabetes patients extended the analysis to large, highly unbalanced data containing discrete and continuous variables. A Bayesian patient phenotyping composite model incorporating latent class analysis (LCA) and regression was implemented with the second case study. Results: We find that several data characteristics common in clinical data, such as sparsity, significantly affect the posterior accuracy of automatic VB methods compared with conditionally conjugate mean-field methods. We find that for both models, automatic VB approaches require more effort and technical knowledge to set up for accurate posterior estimation and are very sensitive to stopping time compared with closed-form VB methods. Discussion: Our results indicate that the patient phenotyping composite Bayes model is more easily usable for real-world studies if Monte Carlo is replaced with VB. It can potentially become a uniquely useful tool for decision support, especially for rare diseases where gold-standard biomarker data are sparse but prior knowledge can be used to assist model diagnosis and may suggest when biomarker tests are warranted. Abstract

Sparsity and entropy are pillar notions of modern theories in signal processing and information theory. However, there is no clear consensus among scientists on the characterization of these notions. Previous efforts have contributed to understand individually sparsity or entropy from specific research interests. This paper proposes a mathematical formalism, a joint axiomatic characterization, which contributes to comprehend (the beauty of) sparsity and entropy. The paper gathers and introduces inherent and first principles criteria as axioms and attributes that jointly characterize sparsity and entropy. The proposed set of axioms is constructive and allows to derive simple or \emph{core functions} and further generalizations. Core sparsity generalizes the Hoyer measure, Gini index and $pq$-means. Core entropy generalizes the R\'{e}nyi entropy and Tsallis entropy, both of which generalize Shannon entropy. Finally, core functions are successfully applied to compressed sensing and to minimum entropy given sample moments. Abstract

Introduction: Drug-induced subacute cutaneous lupus erythematosus (DI-SCLE) has been associated with drugs with different mechanisms of action, including anti-hypertensives, tumour necrosis factor-α inhibitors and even some chemotherapy medicines. In the last years, a few reports have been described in patients treated with cyclin-dependent kinase (CDK) 4/6 inhibitors, palbociclib and abemaciclib. Case report: Here, we describe a case of DI-SCLE in association with ribociclib and exemestane in a woman diagnosed with metastatic breast cancer. Management and outcome: Topical mometasone was prescribed for two weeks with complete resolution of lesions, also abemaciclib was substituted for ribociclib, and the patient had stable disease with no relapse of DI-SCLE. Discussion: To our knowledge, this is the first report of ribociclib-induced SCLE but based on the DI-SCLE reported cases associated others CDK4/6 inhibitors, the role of this family of drugs in dermatopathology must be further investigated.
