OBJECTIVE: The aim of this study was to investigate and evaluate the risk of dyspepsia and anorexia in patients with type 2 diabetes mellitus (T2DM) induced by glucagon-like peptide 1 receptor agonist (GLP-1 RA) hypoglycemic drugs. MATERIALS AND METHODS: We searched papers in PubMed, Web of Science, Cochrane Library, Google Scholar, CNKI, Wanfang, Embase and VIP databases, and the retrieval time limit was set from the establishment of the database to May 2023. Randomized Controlled Trials (RCTs) were collected in which the subjects were T2DM patients, the intervention was GLP-1RA compared with placebo or traditional hypoglycemic drugs, and the outcome indicators included dyspepsia and anorexia. A meta-analysis and a network meta-analysis were performed. RESULTS: The results of the traditional meta-analysis showed that the risk of dyspepsia and anorexia of total GLP-1 RA was 3.01 and 2.56 times that of placebo, respectively. All types of GLP-1RA were compared with placebo and the results also showed a trend towards increased risk of digestive system adverse events (DSAEs). Among all interventions included, liraglutide was the one with the highest risk of dyspepsia in patients with T2DM, and dulaglutide was the one with the highest risk of anorexia. CONCLUSIONS: The results of the two meta-analyses are consistent, and both clearly show that GLP-1RA can increase the risk of dyspepsia and anorexia in T2DM patients. [ABSTRACT FROM AUTHOR]

OBJECTIVE: The purpose of this study was to retrospectively assess the efficacy and safety of percutaneous nephrolithotomy (PCNL) for upper urinary stones using upper pole access (UPA) and other (low or middle) pole access (OPA). MATERIALS AND METHODS: A comprehensive literature review of articles investigating the clinical efficacy and safety of UPA and OPA was performed. The relevant literature was obtained from PubMed, EMBASE, Science Direct, Google Scholar and the Cochrane Library. The primary outcomes, including the stone-free rate, were evaluated using Review Manager 5.4 software. The secondary outcomes (peri- and postoperative complications and operative date) were also compared and analyzed. RESULTS: Ten comparative studies involving 5,290 patients were included in the analysis. The pooled data showed that the UPA group had a stone-free rate (SFR) similar to that of the OPA group [odds ratio (OR) 1.38, 95% confidence interval (CI): 0.94 to 2.03; p=0.22] but a higher incidence of blood transfusion [OR: 1.50; 95% CI: (1.03, 2.19), p=0.04]. There was no statistically significant difference in operative time [mean difference (MD): -7.27; 95% CI: (-25.18, 10.65), p=0.43] or hospital stay [MD: -0.13; 95% CI: (-0.64, 0.37), p=0.60] between the two groups. In addition, the results support that UPA causes fewer complications than OPA. CONCLUSIONS: Our findings suggest that UPA and OPA are both effective treatments for the management of upper urinary stones. Compared to OPA, UPA is associated with less need for blood transfusion and fewer complications. Nevertheless, the findings should be further confirmed by well-designed prospective randomized controlled trials (RCTs) with large samples and strict standards. [ABSTRACT FROM AUTHOR]

Abstract Background Compelling evidence suggests that calcium/phosphorus homeostasis-related parameters may be linked to diabetes mellitus and cardiovascular events. However, few studies have investigated the association of fibroblast growth factor 23 (FGF23), α-klotho and FGF23/α-klotho ratio with atherosclerosis in patients with type 2 diabetes mellitus (T2DM). Objective This study was designed to evaluate whether FGF23, α-klotho and FGF23/α-klotho ratio are associated with T2DM and further to explore the relationships between these three factors and atherosclerosis in Chinese patients with T2DM. Methods Serum FGF23 and α-klotho levels were measured via an enzyme-linked immunosorbent assay (ELISA) kit, and the carotid intima-media thickness (CIMT) was assessed via high-resolution color Doppler ultrasonography. The associations of serum FGF23, α-klotho and FGF23/α-klotho ratio with atherosclerosis in T2DM patients were evaluated using multivariable logistic regression models. Results This cross-sectional study involved 403 subjects (207 with T2DM and 196 without T2DM), 41.7% of the patients had atherosclerosis, and 67.2% of the carotid intima were thickened to a thickness greater than 0.9 mm. The generation of air quality concentration data is imperative for the health and environment of highly urbanized regions. Through remote sensing, air pollutant concentrations can be obtained over large areas for a long time. In this study, particulate matter (PM2.5 and PM10) concentrations were estimated using satellite-derived Moderate Resolution Imaging Spectroradiometer (MODIS) Multi-Angle Implementation of Atmospheric Correction (MAIAC) Aerosol Optical Depth (AOD) values observed in the National Capital Region (NCR), Philippines. Models were generated using multiple linear regression (MLR) and gradient boosting regression to determine the best models for the whole data from 2017 to 2020, dry season, and wet season with a 70–30 split for the train-test sets. Initial models resulted with the best coefficient of determination R2 values of 2.6% and 1.2% using MLR and 2.0% using gradient boosting regression. The results for PM2.5 and PM10 showed the lowest Root Mean Square Error (RMSE) values of 8.79 μg/m3 and 18.99 μg/m3 using MLR and 8.08 μg/m3 and 16.85 μg/m3 using gradient boosting, respectively. The preliminary results indicate the relatively poor performance of models in estimating particulate matter using satellite-derived AOD images. Improvements in the models will include the integration of more in-situ data from air quality monitoring stations and the addition of additional variables and features such as meteorological parameters and geographical layers. Highly urbanized regions such as the Metro Manila area in the Philippines contribute to the deterioration of air quality through overpopulation, excessive vehicle emissions, and industrialization. However, the limited number of ground monitoring stations hinders the detailed estimation of the region's overall air quality. Satellite-derived air pollutant concentrations have been used in several research studies as a substitute or supplementary to ground-based data due to their extensive spatial and temporal coverage. Using the aerosol optical depth (AOD) from the MODIS Multi-Angle Implementation of Atmospheric Correction (MAIAC) algorithm and ground measurements of coarse particulate matter (PM10), this study explores the comparison between satellite-derived and ground-based air pollutant concentrations measured from 2017 to 2020 through trend analysis of monthly average values per city. With 16 stations located in different cities, the monthly average values of AOD vs PM10 showed inconsistent results due to significant gaps in the ground data. Through optimized hotspot analysis, it was found that 7.24% of the Metro Manila region are considered hotspots using the MAIAC AOD values from 2017 to 2019 (pre-pandemic). From 2018 to 2020 (pandemic), 23.86% of Metro Manila are counted as hotspots. The AOD derived from satellite imagery and hotspot analysis can be used for future studies that focus on the development of models to predict ground pollutant values and the designation of non-attainment areas. ABSTRACTRiemerella anatipestifer is an important bacterial pathogen in poultry. Pathogenic bacteria recruit host complement factors to resist the bactericidal effect of serum complement. Vitronectin (Vn) is a complementary regulatory protein that inhibits the formation of the membrane attack complex (MAC). Microbes use outer membrane proteins (OMPs) to hijack Vn for complement evasion. However, the mechanism by which R. anatipestifer achieves evasion is unclear. This study aimed to characterise OMPs of R. anatipestifer which interact with duck Vn (dVn) during complement evasion. Far-western assays and comparison of wild-type and mutant strains that were treated with dVn and duck serum demonstrated particularly strong binding of OMP76 to dVn. These data were confirmed with Escherichia coli strains expressing and not expressing OMP76. Combining tertiary structure analysis and homology modelling, truncated and knocked-out fragments of OMP76 showed that a cluster of critical amino acids in an extracellular loop of OMP76 mediate the interaction with dVn. Moreover, binding of dVn to R. anatipestifer inhibited MAC deposition on the bacterial surface thereby enhancing survival in duck serum. Virulence of the mutant strain ΔOMP76 was attenuated significantly relative to the wild-type strain. Furthermore, adhesion and invasion abilities of ΔOMP76 decreased, and histopathological changes showed that ΔOMP76 was less virulent in ducklings. Thus, OMP76 is a key virulence factor of R. anatipestifer. The identification of OMP76-mediated evasion of complement by recruitment of dVn contributes significantly to the understanding of the molecular mechanism by which R. anatipestifer escapes host innate immunity and provides a new target for the development of subunit vaccines.
