This article summarized the nursing practice of an elderly patient with dermatomyositis complicated by dysphagia and malnutrition. A comprehensive risk assessment on dysphagia and nutritional status was conducted before treatment. A personalized rehabilitation plan consisting of nutrition support, skin care, medication guidance, prevention and treatment of complications and mental health was formulated to relieve the dysphagia symptoms and improve the recovery of the patient. This paper summarized the nursing management for a COVID-19 patient with spontaneous pneumothorax caused by pulmonary bulla. Routing nursing care such as medication, isolation, nutrition support, life management were conducted. In addition to conventional nursing, targeted interventions included airway management, drainage nursing, exhaust and sputum excretion management were performed to reduce the risk of complications and improve the recovery of the patient. Background: Both immunotherapy (IO) and targeted therapy (TT) are used as adjuvant (adj) treatment for stage III melanoma, however, data describing real‐world outcomes are limited. In addition, a significant proportion of patients relapse, for whom best management is unclear. The aim of our study was to assess the efficacy, and safety of adj anti‐PD1 IO and TT in a real‐world cohort of patients with resected stage III melanoma, and further delineate patterns of recurrence and treatment strategies. Methods: We retrospectively analyzed 130 patients who received adj therapy (100 anti‐PD1 IO and 30 TT). Results: At a median follow‐up of 30 months, median relapse‐free survival (RFS) was 24.6 (95% CI, 17–not reached [NR]) versus 64 (95% CI, 29.5–NR) months for the TT and IO groups, respectively (p = 0.26). Median overall survival (OS) was NR for either subgroup. At data cutoff, 77% and 82% of patients in TT and IO arms were alive. A higher number of grade ≥3 treatment‐related adverse events (AEs) were noted in the IO group (11% vs. 3%), however, a higher proportion of patients permanently discontinued adj therapy in the TT group (43% vs. 11%) due to toxicity. Strategies at relapse and outcomes were variable based on location and timing of recurrence. A significant number of patients who relapsed after adj IO received a second round of IO. Among them, patients who were off adj IO at relapse had superior second median RFS (mRFS2), compared to those who relapsed while on adj IO; mRFS2 was NR versus 5.1 months (95% CI, 2.5–NR), respectively, p = 0.02. Conclusion: In summary, both TT and IO yielded prolonged RFS in a real‐world setting, however, longer follow‐up is needed to determine any potential OS benefit. Adj therapy, particularly TT, may not be as well tolerated as suggested in clinical trials, with lower completion rates (59% vs. 74%) in a real‐life setting. Overall, patients who relapse during adj therapy have poor outcomes, while patients who relapse after discontinuation of adj IO therapy appear to benefit from IO re‐treatment. [ABSTRACT FROM AUTHOR]

Composite material La0.7Sr0.3MnO3/α-Fe2O3 was prepared by high-temperature calcination method and its structure, electromagnetic parameters, and electromagnetic wave absorption performance were studied. Further preliminary exploration was conducted on the electromagnetic wave absorption effect of this powder for fabric coatings. The results showed that the significant absorption performance of La0.7Sr0.3MnO3/α-Fe2O3 is achieved when the powder and paraffin are mixed in a mass ratio of 7:3, and they can absorb over 99.9% of incident electromagnetic waves at a thickness of 5.0 mm and 9.5 mm, respectively. The absorption performance is excellent. In addition, within the range of 1-18 GHz and 1.0-10 mm, the total effective absorption (≤-10 dB) bandwidth of the sample La0.7Sr0.3MnO3/α-Fe2O3 covers 91.8% of the test bandwidth, achieving effective absorption of radar multi frequency bands. The excellent absorption performance of composite materials is attributed to the La0.7Sr0.3MnO3/α-Fe2O3. The combined effect of dielectric loss and magnetic loss of La0.7Sr0.3MnO3/α-Fe2O3 achieves effective absorption of electromagnetic waves. The fabrics prepared by coating method and plain weave methods can also absorb more than 90% of incident electromagnetic waves at 5.84 GHz, providing new ideas for the design and research of composite absorbing material and absorbing fabrics. Objectives: To determine the influence of major head and neck procedures on readmission and complication rates following tracheostomy. Methods: A retrospective cohort study using the 2005 to 2017 National Surgical Quality Improvement Program (NSQIP) database. Current Procedural Terminology codes were used to identify tracheostomy patients and to define the underlying head and neck procedure. Patients under the age of 18 and with unknown pre-operative variables were excluded. Univariate and multivariable analyses were performed. Results: A total of 3240 tracheostomy patients undergoing major head and neck surgery were identified in NSQIP. The 30-day mortality rate was 104 (3.2%) and 258 (9.0%) patients were readmitted. 637 (19.7%) patients had an unplanned return to the operating room. There were 1606 (49.6%) non-tracheostomy specific complications, which included 850 (26.2%) medical and 1142 (35.2%) surgical complications. On multivariable analysis, we found that the underlying procedures did not impact the risk of readmission (P >.05 for all). The underlying procedure was also not associated with unplanned return to the operating room except for thyroidectomies, which had a lower risk than free tissue graft reconstruction (OR = 0.53 (95%CI 0.31, 0.88), P =.018). Conclusion: While almost 1 in every 2 patients had a complication following major head and neck surgery that included creation of a tracheostomy, the rate of readmission is comparatively low and is not associated with the underlying procedure. These findings should reassure head and neck surgeons that properly managed tracheostomies do not constitute a disproportionate risk of readmission. [ABSTRACT FROM AUTHOR]

Background: Spitzoid melanocytic neoplasms are diagnostically challenging; criteria for malignancy continue to evolve. The ability to predict chromosomal abnormalities with immunohistochemistry (IHC) could help select cases requiring chromosomal evaluation. Methods: Fluorescence in situ hybridization (FISH)‐tested spitzoid neoplasms at our institution (2013–2021) were reviewed. p16, BRAF V600E, and preferentially expressed antigen in melanoma (PRAME) IHC results were correlated with FISH. Results: A total of 174 cases (1.9F:1M, median age 28 years; range, 5 months–74 years) were included; final diagnoses: Spitz nevus (11%), atypical Spitz tumor (47%), spitzoid dysplastic nevus (9%), and spitzoid melanoma (32%). Sixty (34%) were FISH positive, most commonly with absolute 6p25 gain (RREB1 > 2). Dermal mitotic count was the only clinicopathologic predictor of FISH. Among IHC‐stained cases, p16 was lost in 55 of 134 cases (41%); loss correlated with FISH positive (p < 0.001, Fisher exact test). BRAF V600E (14/88, 16%) and PRAME (15/56, 27%) expression did not correlate with FISH alone (p = 0.242 and p = 0.359, respectively, Fisher exact test). When examined together, however, p16‐retained/BRAF V600E‐negative lesions had low FISH‐positive rates (5/37, 14%; 4/37, 11% not counting isolated MYB loss); all other marker combinations had high rates (56%–75% of cases; p < 0.001). Conclusions: p16/BRAF V600E IHC predicts FISH results. "Low‐risk" lesions (p16+/BRAF V600E−) uncommonly have meaningful FISH abnormalities (11%). PRAME may have limited utility in this setting.
