Ultrasonography is a useful technique to detect soft tissue changes of rheumatoid arthritisnot only synovitis, but also tenosynovitis, bursitis, and enthesitis-even at a subclinical stage. However, radiologists tend to focus on synovitis in daily practice, and unusual peri- or extraarticular manifestations of rheumatoid arthritis are difficult to detect at the initial presentation. This pictorial essay describes a broad spectrum of ultrasonographic findings in tendons, bursae, ligaments, subcutaneous tissues, bones, and nerves to assist in the accurate diagnosis of rheumatoid arthritis.

Purpose: In this study, we aim to study the functionality of multiple effects of peptide collagen and naturally derived compounds through in vitro experiments and human application tests. Method: Cytotoxicity of inner beauty film (IBF), pro-collagen generation ability, elastase and collagenase inhibition, DPPH radical scavenging and tyrosinase inhibition ability were measured, and skin improvement, through in vitro and human application test. Results: Cytotoxicity was not confirmed up to 1000 μg/mL. Elastase inhibition was 7.2% at a concentration of 125 μg/mL, and collagenase showed an inhibition rate of 16.3% at 31.3 μg/mL. The pro-collagen synthesis showed expression of 34%, compared with the negative control when the concentration was 62.5 μg/mL. The antioxidant activity of 250 μg/mL and whitening effect of 62.5 μg/mL are showed in the Results section, and there was no observed deformation or color change even after confirming the stability for 6 months. Even in the human clinical study, the wrinkle improvement before and after using the IBF was 32.2% at maximum, skin melanin and brown spot improvement was 32.5%, pore improvement was 31.5%, and skin density improvement was 12.8%, as shown in the Results section. Conclusion: Therefore, the IBF containing the complex of peptide collagen and naturally derived compounds has a positive effect on wrinkle improvement, skin melanin and brown spot improvement, skin pore improvement, and skin density improvement, as well as exhibiting antioxidant and whitening effects. As a result, IBF can be developed as a key material for cosmetics and foods.

Purpose: This study aimed to determine the optimal blending ratio of extracts of Hippophae rhamnoides (H. rhamnoides ) fruit, Rubus fruticosus (R. fruticosus ) leaf, and Perillae folium (P. folium) leaf as a potential functional material for antiwrinkle, antioxidant, anti-inflammatory, and whitening properties. Methods: We used the design of experiment (DOE) efficacy method to identify the optimal blend ratio for a natural extract complex. In addition, we tested the efficacy of the extract complex and the individual extracts by experiments, including elastase inhibition assay, NO inhibition assay, 2,2-diphenyl-1-picrylhydrazyl (DPPH) radical scavenging activity assay, tyrosinase inhibition assay, and ROS inhibition assay. Results: DOE methods using natural extracts showed that the most efficacious ratio of H. rhamnoides fruit: R. fruticosus leaf: P. folium leaf was 0.25:0.7:0.05. At this ratio, the elastase inhibitory effect, an indicator of wrinkle improvement, was 28% compared to the control. The antioxidant activity was 71.8% higher than the control. Additionally, NO formation was significantly suppressed by the extract complex. Tyrosinase inhibitory activity, a whitening index, showed 24.4% improvement over the control. Finally, hydrogen peroxide-induced cytotoxicity was significantly inhibited following treatment with the extract complex. Conclusion: The natural extract complex of H. rhamnoides , R. Fruticosus , and P. folium was found to have beneficial wrinkle improvement, antioxidant, anti-inflammatory, and whitening effects, and has potential as a functional cosmetic material.

Introduction We evaluated trends in hospitalization incidence and mortality due to hemorrhagic and ischemic stroke in young adults, according to gender and developed regions in Brazil. Methods Between 2008–2018, we performed a population-based time-series study using official hospitalization and death data due to stroke, in individuals aged 10–49 years, from Southeast and South, Brazil. Data were based on reports from the Unified Health System of Hospital Information System and Mortality Information System. Stroke was defined by the International Classification of Diseases, 10th revision (I60–I63). A Prais-Winsten regression model was performed and the Annual Percentage Change was calculated. Results In total, 78,123 hospitalizations of individuals aged 10–49 years were recorded, of which 59,448 (76%) resulted from hemorrhagic stroke (HS). The hospitalizations for HS was significantly decreased (- 4.37%) among men and women in both regions. The hospitalizations for ischemic stroke (IS) was flat, except between 2011 and 2018, when IS hospitalization rates increased. In the analysis by states, HS hospitalizations declined across all states, except for Espírito Santo, where it remained unchanged (p > 0.05). IS flat hospitalizations were observed in all states, except Espírito Santo, where it increased by 24.93%. In terms of mortality, 28,625 deaths were recorded, of which 26,548 (92.7%) resulted from HS. HS mortality decreased significantly by -3.48%and IS mortality by -3.84%. Decreases also occurred in all Southeast and South states (p < 0.05). IS remained unchanged across all states, except Minas Gerais, where it decreased by -14.95%. Conclusions We identified a decline in the hospitalizations and mortality of HS and a flat trend for IS in developed regions of Brazil. The recent period (2011–2018) demonstrated increasing rates in the hospitalizations of IS in both regions and genders. The mortality rates for HS and IS decreased between 2008–2018 in Southeast and South Brazil for both genders.

An optimized, ceramide-based, nanostructured lipid carrier (NLC) formulation was developed for isoliquiritigenin (ILTG), and its potential as a transdermal delivery system was evaluated. ILTG-loaded NLCs were prepared by blending solid (ceramide, cholesterol) and liquid lipids (caprylic/capric triglyceride) in various proportions using a hot homogenization and ultrasonication method. The physicochemical characteristics were investigated by DLS, ZP, EE%, TEM, DSC and XRD analyses and in vitro skin permeation studies. The results showed that the particle size of the formulation was 150.19–251.69 nm with a ZP>−20mV. The EE% was 56.45–89.97%. The NLC structure was influenced by lipid ratio, and increasing the caprylic/capric triglyceride ratio caused a less ordered structure, as confirmed by DSC. The XRD analysis indicated that ILTG was not in the crystalline state in all formulations. The skin permeation study showed that the ILTG-NLCs promoted ILTG permeation. In conclusion, ceramide-based NLCs could be a promising vehicle for the ILTG transdermal delivery of ILTG.

With the growth of aging population in Korea, a better care of chronic and other degenerative illnesses is urgently needed. Evi-dences suggest that this can be achieved through incorporating a wide range of care options, expanding beyond medical interven-tions. The aim of this study is to analyze the distribution of publically funded research to understand if the Korean research and de-velopment funding system matches various approaches and purposes to successfully tackle the chronic care needs of an aging so-ciety. We complied the list of funded projects to be analyzed by searching the National Technical Information Service database with key words such as aging society/senescence, chronic diseases, disability, and health promotion. Most projects were based on the biomedical approach with the purpose of establishing the etiology and clinical (treatment) interventions. Health promotion proj-ects showed a distinctive distribution with more percentage of projects based on psycho-behavioral approaches while research on chronic diseases predominantly biomedical. It would be necessary to diversify publically-funded research projects to develop effec-tive and efficient care technologies for the future.Keywords: National Technical Information Service; Health research and development; Chronic disease; Disability; Aging; Health pro-motion

Purpose Based on our previous experiences of endoscopic or robotic neck surgery utilizing the retroauricular (RA) or modified facelift (MFL) approach, we realized the necessity of verifying the feasibility of endoscopic supraomohyoid neck dissection (SOND). Therefore this study aims to evaluate the potential role of endoscopic SOND. Materials and methods Six patients who underwent elective endoscopic SOND of the ipsilateral neck for biopsy proven head and neck cancer from January 2011 to February 2012 were analyzed. Results All endoscopic operations via RA or MFL were successfully performed, without any significant intraoperative complications or conversion to open surgery. Based on patient-reported outcome questionnaires, all patients were satisfied with the cosmetic surgical outcomes. Conclusion Endoscopic selective neck dissection via a RA or a MFL approach is technically feasible and safe with satisfactory cosmetic results for patients with clinically node negative early-stage head and neck cancer.

Purpose To evaluate the clinical outcome of frontalis sling operation using preserved fascia lata with modified direct tarsal fixation in congenital ptosis patients. Methods Forty-seven congenital ptosis patients (60 eyes) who underwent a frontalis sling operation using preserved fascia lata with modified direct tarsal fixation method between March 2001 and December 2008 with a mean follow-up time of 52 months (range, 26 to 122 months) were included in this study. The medical records were reviewed retrospectively. Results A retrospective chart review was conducted in patients who were diagnosed with congenital ptosis and underwent frontalis suspension surgery using preserved fascia lata with modified direct tarsal fixation from 2001 through 2008 at Dong-A University Hospital. The patients were 34 males and 14 females. The age of the patients ranged from 1 to 18 years with an average age of 4.51 years. At a mean follow-up of 60 months, good final results were achieved in 46 eyes (76.6%), fair in 8 eyes (13.3%), and poor in 6 eyes (10%). The poor results consisted of undercorrection of 1 eye and recurrence in 5 eyes. The accumulative survival rate was 87.2%, with all recurrences occurring within 12 months postoperatively. Conclusions Frontalis sling operation by preserved fascia lata with modified direct tarsal fixation appears to be an effective treatment for severe congenital ptosis, showing good long term results.

Purpose: To compare the effects of the lateral tarsal strip procedure alone and the lateral tarsal strip procedure combined with silicone tube intubation in the treatment of functional lacrimal duct obstruction. Methods: The present study investigated 27 eyes of 20 patients underwent lateral tarsal strip procedure (LTS) alone and 29 eyes of 18 patients underwent combined lateral tarsal strip procedure and silicone tube intubation and who were diagnosed with functional lacrimal duct obstruction. The authors retrospectively reviewed patients' medical records for information including age, gender, history, follow-up period, tear meniscus height (TMH), and symptoms. Results: The average age of the LTS alone group was 60.3 years, and that of the combined groups was 65.9 years. The TMHs in the LTS only and combined operation groups were 0.62 0.23 mm and 0.62 0.19 mm before the operation and 0.43 0.20 mm and 0.26 0.09 mm after the operation, respectively. The postoperative improvement in epiphora was 74.1% after the LTS alone and 86.2% after combined LTS with silicone tube intubation. The postoperative improvement in epiphora and the operative technique of the lateral tarsal strip-canthus sparing or cantholysis were not relevant. Conclusions: Lateral tarsal strip combined with silicone tube intubation appeared useful in the treatment of epiphora patients without anatomic lacrimal duct obstruction. Determining which operative technique of lateral tarsal strip procedure should be chosen is difficult, and the preoperative syringing and snap back test may be helpful.
