24. Management of complicated Bouveret’s syndrome.

Abstract

Bouveret’s syndrome is an uncommon cause of gastric outlet obstruction caused by the impaction of large gallstones in the duodenal lumen. The gallstones pass into the duodenal lumen through a cholecystogastric or a cholecystoduodenal fistula. Endoscopic retrieval with or without lithotripsy is the first line of management, often with variable success. We present a case of a woman in her 70s who presented with signs of gastric outlet obstruction and was diagnosed with Bouveret’s syndrome with a 5 cm diameter gallstone in the third part of her duodenum. Following several unsuccessful attempts of endoscopic extraction, she underwent successful jejunal enterotomy with fragmentation and extraction of the calculus using an Allis tissue holding forceps. 40. Evolution and Characterization of Arc Plasma Under Atmospheric Pressure.

Abstract

This work elueidates the evolution of Are plasma and its eharaeterization in terms of different plasma parameters under atmospherie pressure. The I-V traits of Langmuir probes are plotted from the experiment for a single probe approaehin Are plasma. It is diseovered that the siliea seed alters the eleetron temperature of the plasma substantially, while the temperature of the gas in the surrounding remains nearly unehanged. The single probe traits were applied to find the eleetron temperature, floating potential, Debye length and eleetron density. It is pereeived that eleetron density inereases substantially, while eleetron temperature is redueed after seeding the Are plasma. A large fall in eleetron temperature an unusual growth in eleetron density whieh is diseovered after mixing the plasma with siliea aggregate in eomparison to those before mixing. Abstract

Nowadays, modern technologies are growing rapidly by advancing their application day by day. Moreover, the Internet of Things (IoT) is well utilized in many applications for sensing and information gathering purposes. Considering this, medical applications have used the advances of IoT facilities in sensing the disease rate in humans. After sensing, the sensed data is stored in the cloud system, so securing the cloud system is the more required task to maintain the privacy of the patient's data. But, in front of malicious activities, securing the data is not easy. So, the present research has proposed a novel African buffalo‐based International Data Encryption (ABIDE) public cloud system to secure the medical database during the data transmission. The developed technique is utilized in a public cloud system. The main objective of this research is to secure medical data during transmission. Moreover, the implementation of the proposed ABIDE algorithm is done in the MATLAB environment. Consequently, the performance of the developed technique is compared with existing methods in terms of resource usage, encryption time, decryption time and so forth. Therefore, the proposed model has attained the finest confidential rate of 98.98%, throughput 97.86% and less processing time of 4 ms. [ABSTRACT FROM AUTHOR]
