Associations between vitamin D (VD) deficiency and the risk of SARS-CoV-2 infection have been documented in cross-sectional population studies. Intervention studies in patients with moderate to severe COVID-19 have failed to consistently document a beneficial effect.To determine the efficacy and safety of VD-supplementation in the prevention of SARS-CoV-2 infection in highly exposed individuals.A double-blind, parallel, randomized trial was conducted. Frontline healthcare workers from four hospitals in Mexico City, who tested negative for SARS-CoV-2 infection, were enrolled between July 15 and December 30, 2020. Participants were randomly assigned to receive 4,000 IU VD (VDG) or placebo (PG) daily for 30 d. RT-PCR tests were taken at baseline and repeated if COVID-19 manifestations appeared during follow-up. Serum 25-hydroxyvitamin D3 and antibody tests were measured at baseline and at day 45. Per-protocol and intention-to-treat analysis were conducted.Of 321 recruited subjects, 94 VDG and 98 PG completed follow-up. SARS-CoV-2 infection rate was lower in VDG than in PG (6.4 vs. 24.5%, p0.001). The risk of acquiring SARS-CoV-2 infection was lower in the VDG than in the PG (RR: 0.23; 95% CI: 0.09-0.55) and was associated with an increment in serum levels of 25-hydroxyvitamin D3 (RR: 0.87; 95% CI: 0.82-0.93), independently of VD deficiency. No significant adverse events were identified.Our results suggest that VD-supplementation in highly exposed individuals prevents SARS-CoV-2 infection without serious AEs and regardless of VD status.

Knowledge of chronic kidney disease (CKD) with pubertal disorders (PD) in adolescent boys is limited as few studies have explored this disorder. This study aimed to identify the usefulness of assessing hormonal parameters in male adolescents with CKD and their correlation with PD in a 12-month follow-up period.A prospective cohort study was conducted among male adolescents with CKD (stages IV and V). Data regarding the age at puberty onset were collected from the patients' clinical records and through interview. The patients were followed up for 12 months during their pubertal development. At the beginning, routine hormonal profile tests were performed to examine the patients' thyroid profile, prolactin levels, luteinizing hormone, follicle-stimulating hormone, testosterone, leptin, and receptor leptin. The hormonal profiles of patients with and without PD were compared. Comparisons between the groups were performed using the Student t-test and Fisher's exact tests. Logistic regression analysis was also performed.Data of 64 patients (26/64 with PD) were analyzed. The median age was 15 years and the median time for CKD evolution was 11 months. No differences between groups were noted in the general or biochemical characteristics of the patients. The hormonal parameters, prolactin levels were higher and the free leptin and free thyroxine levels were lower in patients with PD. Leptin receptor levels of0.90 ng/mL (risk ratio [RR], 8.6; P = 0.004) and hyperprolactinemia (RR, 21.3; P = 0.049) were the risk factors for PD.Leptin receptor levels of0.90 ng/mL and hyperprolactinemia are associated with the development of PD in male adolescents with CKD.

This study aimed to identify the usefulness of hormonal measurements in adolescents with CKD and their relationships with MD during a 1-year follow-up. A prospective cohort study was designed. Adolescents with CKD stages IV and V were included. Through clinical files and via interview, the ages at puberty onset, menarche and the date of last menstruation were identified. A 1-year follow-up was conducted over a menstrual cycle calendar. At the beginning of follow-up, routine hormonal profiles (thyroid profiles, prolactin, luteinizing hormone (LH), follicle-stimulating hormone (FSH), and estradiol) were assessed. We compared the hormonal profiles of the patients with and without MD (wMD vs. woMD). Comparisons between groups were made by Wilcoxon and Fisher’s tests. Logistic regression analysis was used. Fifty-seven patients, including 30 patients classified as wMD, were analyzed. The median age was 15 years, and the median time of CKD evolution was 18 months. There were no differences in general and biochemical characteristics between patients wMD and woMD. In terms of hormonal measurements, the levels of thyroid-stimulating hormone (TSH) and prolactin were higher in the wMD patients. A prolactin level ≥ 36.8 ng/ml was a risk factor for presenting with MD (RR 34.4, p = 0.002). Hyperprolactinemia is correlated with MD in adolescents with CKD.

Childhood obesity is associated with stress. However, most treatment strategies include only dietary and physical activity approaches. Mindfulness may assist in weight reduction, but its effectiveness is unclear. We assessed the effect of mindfulness on stress, appetite regulators, and weight of children with obesity and anxiety. A clinical study was conducted in a pediatric hospital. Eligible children were 10–14 years old, BMI ≥95th percentile, Spence anxiety score ≥55, and who were not taking any medication or supplementation. Participants were assigned to receive an 8-week conventional nutritional intervention (CNI) or an 8-week mindfulness-based intervention plus CNI (MND-CNI). Anthropometry, body composition, leptin, insulin, ghrelin, cortisol, and Spence scores were measured at baseline and at the end of the intervention. Anthropometry was analyzed again 8 weeks after concluding interventions. Log-transformed and delta values were calculated for analysis. Thirty-three MND-CNI and 12 CNI children finished interventions; 17 MND-CNI children accomplished 16 weeks. At the end of the intervention, significant reductions in anxiety score (−6.21 ± 1.10), BMI (−0.45 ± 1.2 kg/m2), body fat (−1.28 ± 0.25%), ghrelin (−0.71 ± 0.37 pg/mL), and serum cortisol (−1.42 ± 0.94 µg/dL) were observed in MND-CNI children. Changes in anxiety score, ghrelin, and cortisol were different between groups (P 2, P 2, P = 0.001). We concluded that mindfulness is a promising tool as an adjunctive therapy for childhood obesity. However, our findings need confirmation in a larger sample population.

Mexico is a country with a high frequency of type 2 diabetes mellitus (DM2). DM-related complications can increase the number of medical appointments and hospitalizations, impact patient quality of life, and exacerbate hospital care costs. The goals of this study were to analyze epidemiological data of DM2 over the last 10 years and to compare its comorbidities and mortality in individuals listed at the Instituto Mexicano del Seguro Social (IMSS) in 2013 and 2015. The database was compiled for doctors, validated by engineers in computer systems, and analyzed by statisticians. E-10-E-14 registered patients with DM2 (based on International Classification of Diseases (ICD-10)) from 2005, 2013, and 2015 were analyzed through the Family Medicine Units and Non-Communicable Diseases Analysis System (SANENT)® databases. Incidence, comorbidity, and mortality were described. We included 29,525,905 individuals, including 3,395,389 DM2 patients, in 2013, and 31,389,711 individuals, including 3,547,006 DM2 patients, in 2015, in the IMSS. The incidences of DM2 in 2005 and 2015 were 124 and 120 per 100,000 IMSS members, respectively, without significant differences. The diabetes complications caused partial disability in 253 and 341 per 100,000 individuals in 2013 and 2015, respectively. Introduction: A history of pre-maturity may be a risk factor for complications in patients under 24 months of age hospitalized for viral respiratory infections (VRIs). Objective: To identify the impact of a history of pre-maturity on in-hospital complications and mortality in patients under 24 months of age who were hospitalized for VRIs over a period of 5 years. Material and Methods: This was a propensity score-matched study. The database was compiled by physicians, electronically validated by engineers, and analyzed by statisticians. Patients diagnosed with VRIs (based on International Classification of Diseases [ICD-10]) codes B974, J12, J120-J129X, J168, J17, J171, J178, J20, J203-J209, J21, J210, J211, J218, J219, J22X, and J189) from 2013 to 2017 were enrolled in the study. The subjects were classified into two groups according to the absence or presence of a history of pre-maturity (P070, P072, P073). Patients with congenital heart disease (CHD) (Q20-Q26) were excluded. Length of hospital stay, in-hospital complications, surgical procedures, and mortality were analyzed. Statistical Analysis: Patients were matched according to age. For comparisons between groups, Student's t-tests and chi2 tests were applied. A logistic regression model was constructed to identify factors related to in-hospital complications and mortality. Results: In total, 5,880 patients were eligible for inclusion in the analysis. The average patient age was 14.25 weeks. The presence of pre-maturity (coefficient = 1.16), male sex, bronchopulmonary dysplasia (BPD), in-hospital infectious complications (coefficient = 11.31), and invasive medical procedures (coefficient = 18.4) increased the number of days of hospitalization. Invasive medical procedures (OR = 6.13), a history of pre-maturity (OR = 2.54), and male sex (OR = 1.78) increased the risk for in-hospital complications. In-hospital infectious complications (OR = 84.2) and invasive medical procedures (OR = 58.4) were risk factors for mortality. Conclusions: A history of pre-maturity increased the length of hospital stay and the rate of in-hospital complications but did not increase mortality in patients under 24 months of age hospitalized for VRIs.

The Mexican Society of Pediatric Endocrinology presents recommendations for the diagnosis and treatment of precocious puberty (PP), a condition defined as the development of sexual characteristics due to an increase in pituitary gonadotropin secretion before 8 or 9 years of age in girls and boys, respectively.Three systematic reviews were conducted: controlled clinical trials on interventions for PP treatment, diagnostic tests, and observational studies on the long-term effects of PP. The quality evaluation and data extraction from the studies were conducted by two independent reviewers. The Scottish Intercollegiate Guidelines Network and the Oxford Center for Evidence-Based Medicine systems were used for grading the quality of evidence for recommendations on intervention and diagnosis, respectively. Recommendations were submitted to a consensus by a Delphi method and were validated by another 143 certified pediatric endocrinologists through an online questionnaire.The group generated 12 recommendations on the diagnosis of PP, seven on the diagnosis of secondary causes of PP, eight on interventions for inhibition of puberty, five on other interventions for PP treatment, and 14 for the monitoring and follow-up of these patients. The online questionnaires submitted to certified pediatric endocrinologists showed more than 90% of approval for each one of the recommendations.Although a high degree of consensus for the recommendations for diagnosis, treatment, and monitoring of PP among pediatric endocrinologists was achieved, most of these recommendations showed a low level of evidence.La Sociedad Mexicana de Endocrinología Pediátrica presenta recomendaciones para el diagnóstico y el tratamiento de la pubertad precoz (PP), condición definida como el desarrollo de caracteres sexuales por incremento en la secreción hipofisiaria de gonadotropinas antes de los 8 años en las niñas y de los 9 años en los niños.Se realizaron tres revisiones sistemáticas de ensayos clínicos controlados sobre intervenciones para el tratamiento de la PP, pruebas diagnósticas y estudios observacionales sobre efectos a largo plazo de la PP. La evaluación de la calidad de los estudios y la extracción de datos se realizó por pares. La evidencia se graduó con el sistema de la Scottish Intercollegiate Guidelines Network (SIGN) y del Oxford Centre for Evidence-Based Medicine (OCEBM) para las recomendaciones sobre la intervención y el diagnóstico, respectivamente. Las recomendaciones generadas se sometieron a un consenso por el método Delphi y fueron validadas por otros 143 endocrinólogos pediatras certificados mediante un cuestionario en línea.Mediante consenso se generaron 12 recomendaciones para el diagnóstico de PP, siete sobre diagnóstico de causas secundarias de PP, ocho sobre intervenciones para inhibición de la pubertad, cinco sobre otras intervenciones en PP y 14 para la monitorización del tratamiento y el seguimiento de estos pacientes. Se obtuvo más del 90% de aprobación para cada una de las recomendaciones por el grupo de endocrinólogos certificados que respondieron el cuestionario en línea.Si bien se logró un alto grado de consenso para las recomendaciones para el diagnóstico, el tratamiento y la monitorización de la PP entre los endocrinólogos pediatras, el nivel de evidencia para la mayoría de estas recomendaciones resultó bajo.

Objective To compare serum resistin concentrations between prepubertal girls with a BMI > 85th percentile and girls with precocious puberty (CPP) who have and have not undergone GnRH analog treatment. Patients and methods This is a cross-sectional study in girls with a BMI > 85th percentile and a median age of 8 years. We included 31 girls with CPP who did not receive treatment (CPPoT), 23 girls with CPP who were treated with leuprolide (CPPT), 22 prepubertal girls and 24 pubertal girls. Anthropometric data and the fasting plasma concentrations of lipids, glucose, insulin, and resistin were measured. Results The z-BMI scores were similar among the groups (p = 0.344), and body fat percentage (BF%) was similar among CPPT, CPPoT and prepubertal girls (p = 0.151). Resistin and insulin levels were lower in girls with CPP (CPPT and CPPoT) than in prepubertal and pubertal girls (median resistin level: CPPT 11.8 pg/ml vs CPPoT 11 pg/ml vs prepubertal 16 pg/ml vs pubertal 16 pg/ml, p = 0.001; median insulin level: CPPT 10.7 μUI/mL vs CPPoT 10.2 μUI/mL vs prepubertal 14.4 μUI/mL vs pubertal 32 μUI/mL p = 0.02). ANCOVA analysis, after adjustments for pubertal stage, BF% and z-BMI, showed that CPP modifies resistin levels (F = 31.4; p = 0.0001) independently of these parameters (p Conclusions In the group of girls with overweight or obesity, the resistin level was lower in girls with CPP than in prepubertal and pubertal girls. More studies are needed to understand the role of resistin in CPP patients.

To compare cardiometabolic factors and adipokines between patients with recently diagnosed CPP and controls without CPP, paired by BMI Z scores (BMIz) and classified into girls with adequate nutritional status and girls who are overweight or obese.This cross-sectional study was performed from January 2012 to May 2015 at two tertiary care pediatric centers in Mexico City. We included female patients with idiopathic CPP without other chronic pathology and healthy controls. Patients were divided into groups, BMI 85th and BMI ≥ 85th percentile, according to 2000 CDC Growth Charts. Anthropometric data and fasting plasma concentrations of lipids, glucose, insulin, and leptin were assessed.There were 73 patients with CPP and 82 without CPP. Sixty-six patients were matched between the groups; no significant difference was noted between the groups according to zBMI. However, differences in the bone/chronological age relationship, birth weight and proportions in different Tanner stages were observed. Among girls with normal BMI, the percentage of body fat (24.6% vs 18.9%, p 0.001), serum triglycerides (102.9 vs 54.3 mg/dl, p 0.001), leptin (7.46 vs 5.4 ng/ml, p = 0.010) and free leptin (0.44 vs 0.29 ng/ml, p = 0.044) were higher in those with CPP; additionally, girls with CPP presented a higher proportion of hypertriglyceridemia. In the overweight/obese group, adiponectin levels were lower in girls with CPP (6.23 vs 7.28 pg/ml, p = 0.011).Girls with CPP and normal BMI at diagnosis had a worse cardiometabolic profile, as reflected by higher levels of free leptin, and higher proportion of hypertriglyceridemia than girls without CPP.

Resumen Introduccion En pediatria se reportan complicaciones del cateter central de insercion periferica (CCIP) del 30 al 40% de los casos, siendo los factores de riesgo mas frecuentes la edad del paciente y la localizacion del CCIP. El objetivo de este trabajo fue describir las caracteristicas de los CCIP e identificar los factores relacionados con su retiro debido a complicaciones. Metodos Estudio de cohorte prospectiva. Se incluyeron pacientes de 2 meses a 18 anos de edad. Se les dio seguimiento hasta el retiro del CCIP. Se registro la edad, sexo, medicamentos administrados, localizacion del CCIP, calibre, numero de lumenes, dias que estuvo colocado y el motivo del retiro. Se realizo analisis univariado y multivariado. Resultados Se incluyeron 225 pacientes. En 33.02%, el CCIP fue retirado por alguna complicacion. En el analisis bivariado, la edad y la localizacion fueron los factores asociados con las complicaciones. Se realizo un modelo de riesgos proporcionales de Cox y se identifico que solamente el grosor del calibre se asocio con cualquier complicacion (razon de tasas (RT): 0.5, intervalo de confianza (IC)95%: 0.35-0.99), y la edad (RT: 1.02, IC95%: 1-1.04) solamente con el retiro del CCIP por sospecha de flebitis. Conclusiones El 33% de los pacientes presentaron retiro del CIPP por complicaciones; solamente el calibre del CIPP y la edad de los pacientes estuvieron relacionados con este evento.

Patients with central precocious puberty (CPP) may have increased serum leptin levels; however, it is not well known whether this increase differs between patients with and without obesity. Our objectives were to describe the changes in serum leptin in girls with CPP in the first 12 months after diagnosis based on body mass index (BMI) and to explore whether serum leptin level at CPP diagnosis is related to BMI z-score (BMIz) after a 1-year follow-up.A prospective cohort study was performed. We included 42 girls with idiopathic CPP in Tanner stages II and III. Anthropometric measurements were performed, and serum leptin was measured at study initiation and after 12 months. Patients were stratified according to BMI category (30 with a BMI in the94th percentile and 12 with a BMI in the95th percentile). Study variables were compared. Correlations among leptin, BMIz, and body fat were assessed.Leptin increased gradually during the first year of treatment. In girls with a BMI in the94th percentile at diagnosis, body fat percentage increased gradually during the first year of follow-up.Girls with a BMI in the94th percentile have a greater risk of weight increase. Background: A significant body of international evidence suggests that effective primary care systems are associated with better health, lower total health costs, and more equitable access to health care services, which has led the World Health Organization (WHO) to call for strengthening primary care systems globally. Patients with diabetes often seek treatment and have follow-up as outpatients, be it at private or government health facilities. Thus, the objective of this study was to assess the effect of glucose control during outpatient care on first hospitalization risk in type 2 diabetes mellitus (T2DM) patients.Methods: The data used in this retrospective cohort study were obtained from the Non-Communicable Diseases Analysis System (SANENT)® of the Instituto Mexicano del Seguro Social (IMSS). Thus, this work was performed on 3,716,031 DM2 patients, with data regarding a first hospitalization risk estimated using a Cox regression model.Results: The first hospitalization crude risk for diabetes mellitus decreased by 32% in T2DM patients under control through outpatient care (HR: 0.68, 95% CI [0.68,0.69]). Adjusted risk decreased by 23% (HR: 0.77, 95% CI [0.76, 0.77]).Conclusion: The obtained results are consistent with what has been stated in the literature. Outpatient care of diabetic patients at the IMSS should be further strengthened by means of preventive and educational strategies and supported by technology.

It has been described that menstruation is perceived as a natural but cumbersome event, with a negative impact on women's daily life. The objective of this study was to identify the perception of menstruation among female adolescents and the factors that may affect it.Observational, crosssectional, comparative study in postmenarcheal adolescents with and without chronic conditions in two children's hospitals and two primary and secondary schools. Age, education level, type of population, date of menarche, and presence of dysmenorrhea were recorded. A previously validated questionnaire was administered to assess the perception of menstruation.A total of 346 female adolescents were included. The most common perception of menstruation was negative in 65.6 % (n = 227), positive in 16 % (n = 55), discreet in 13 % (n = 45), and indefinite in 5.4 % (n = 19). The presence of a chronic condition demonstrated to be a protective factor against a negative perception of menstruation (odds ratio: 0.4 [95 % confidence interval: 0.20-0.78], p = 0.007).More than half of female adolescents had a negative attitude toward menstruation, but the presence of a chronic condition improved it.Introducción. Se ha descrito que la menstruación se percibe como un evento natural pero molesto, con un impacto negativo en la vida diaria de la mujer. El objetivo del estudio fue identificar la percepción de la menstruación en adolescentes y los factores que podían influir sobre esta. Material y métodos. Estudio transversal observacional comparativo en adolescentes posmenárquicas con y sin enfermedades crónicas, en 2 hospitales pediátricos y 2 escuelas de nivel primario y secundario. Se recabaron los datos de edad, escolaridad, tipo de población, fecha de menarca y presencia de dismenorrea. Se aplicó un cuestionario previamente validado para evaluar la percepción sobre la menstruación. Resultados. Se incluyó un total de 346 adolescentes. Se encontró que la percepción más frecuente hacia la menstruación fue la negativa en un 65,6 % (n = 227), positiva en el 16 % (n = 55), sigilosa en el 13 % (n = 45) e indeterminada en el 5,4 % (n = 19). La presencia de enfermedad crónica demostró ser un factor protector de la percepción negativa de la menstruación (odds ratio 0,4 [intervalo de confianza del 95 %: 0,20-0,78], p = 0,007). Conclusiones. Más de la mitad de las adolescentes presentaron una actitud negativa hacia la menstruación, pero la presencia de una enfermedad crónica la mejoró.

espanolIntroduccion: los pacientes con enfermedad renal cronica (ERC) deben ser considerados como un grupo de alto riesgo cardiovascular, ya que existen multiples factores que incrementan la presencia de dislipidemia. Objetivo: determinar si el cociente androide/ginecoide (CA/G) tiene utilidad como factor cardiometabolico para dislipidemia en pacientes pediatricos con insuficiencia renal cronica. Materiales y metodos: estudio de una cohorte. Se incluyeron pacientes con ERC terminal en dialisis peritoneal y hemodialisis. A cada paciente se le realizo determinacion de la composicion corporal, indice de masa corporal (IMC) y perfil de lipidos. Posteriormente, se realizo somatometria y perfil de lipidos a los 6 y 12 meses de seguimiento. Analisis estadistico: para identificar la diferencia entre las variables somatometricas y bioquimicas iniciales, a los 6 y 12 meses se aplico la prueba de Friedman. El coeficiente de Spearman determino la correlacion de variables corporales y bioquimicas. Resultados: se analizaron 21 pacientes. Las concentraciones sericas de los trigliceridos a 12 meses de seguimiento aumentaron significativamente (6 vs. 12 meses; p = 0,05), sin evidencia de un incremento en el score Z del IMC (p = 0,98) o colesterol total (p = 0,49). La grasa corporal, su porcentaje y score Z del IMC no se correlacionaron con los cambios en los niveles del colesterol y trigliceridos a los 6 y 12 meses; sin embargo, el CA/G presento una asociacion estadisticamente significativa con la modificacion en las concentraciones sericas de los trigliceridos a los 6 (r = 0,65, p = 0,003) y 12 meses de seguimiento (r = 0,54, p = 0,02). Conclusiones: el CA/G mostro asociacion al incremento en la concentracion serica de trigliceridos a 12 meses de seguimiento. EnglishBackground: there are multiple factors that increase the presence of dyslipidemia in chronic kidney disease (CKD). Objective: to determine if the android/gynecoid ratio (A/GR) has utility as a cardiometabolic factor for dyslipidemia in pediatric patients with chronic renal failure. Materials and methods: cohort study. Patients with terminal CKD in peritoneal dialysis and hemodialysis were included. Determinations of body composition, body mass index (BMI), and lipid profile were assessed for each patient. Subsequently, somatometry and lipid profile were performed at 6 and 12 months of follow-up. Statistical analysis: to identify the difference between the initial somatic and biochemical variables, and at 6 and 12 months, the Friedman test was applied. The Spearman coefficient determined the correlation of bodily and biochemical variables. Results: twenty-one patients were analyzed. Triglycerides (TGL) serum at 12 months of follow-up increased significantly (6 vs 12 months, p = 0.05), without evidence of an increase in the Z score of the BMI (p = 0.98) or total cholesterol (p = 0.49). Body fat, fat percentage and Z score BMI did not correlate with changes in cholesterol and triglyceride levels at 6 and 12 months; however, the A/GI presented a statistically significant association with the change in serum concentrations of TGL at 6 (r = 0.65, p = 0.003) and 12 months of follow-up (r = 0.54, p = 0.02). Conclusions: the A/GI showed an association with the increase in the serum concentration of TGL at 12 months of follow-up.

To evaluate the resistin/uric acid index as a prognostic factor associated with body mass index (BMI) z-score change after 1 year of lifestyle interventions for obesity.In this prospective cohort study, we included 102 adolescents with overweight or obesity (BMI ≥85th percentile). Weight and height were measured at the start of the lifestyle change intervention and 12 months later. Serum levels of resistin and uric acid were quantified at the beginning of the intervention. The intervention consisted of nutrition education, exercise, and physical activity promotion.The sample included 54 girls and 48 boys; the median age was 11 years (range 10-16 years). The BMI z-score decreased during follow-up (median BMI z-score at baseline was 1.81 vs 1.70 after 1 year, P.001). The resistin/uric acid index was positively correlated with BMI z-score change (r = 0.27, P.01). In the linear regression analysis, the resistin/uric acid index was significantly associated with BMI z-score modification at the 12-month follow-up (β = 0.17; 95% CI 0.08-0.26; P.01).The resistin/uric acid index can be considered a prognostic factor for identifying adolescents with overweight or obesity with a greater probability of improving their BMI. This index could help establish different interventions for adolescents with overweight and obesity; however, additional studies are needed to confirm the usefulness of this index.

Introduccion La enfermedad renal cronica (ERC) se asocia con alteraciones menstruales, y el manejo del sangrado uterino suele ser complejo por las condiciones de este grupo de pacientes. El objetivo de este trabajo fue describir la respuesta clinica al tratamiento hormonal de las alteraciones menstruales de adolescentes con ERC. Metodos Se presentan los datos de una serie de casos de pacientes adolescentes con ERC que cursaron con alteraciones menstruales y que recibieron tratamiento desde el ano 2008 al 2012. Se identificaron las caracteristicas del trastorno menstrual, del tratamiento hormonal recibido y de la respuesta al mismo. El analisis estadistico fue descriptivo. Resultados Se estudiaron 11 pacientes de sexo femenino con edad promedio de 14.5 anos, que se encontraban en predialisis (n = 1), dialisis peritoneal (n = 7) y hemodialisis (n = 3). Las pacientes presentaron hiperpolimenorrea asociada a la opsomenorrea (n = 3), en su mayoria clasificadas como hemorragia uterina anormal secundaria. El tratamiento, en general, fue con progestagenos de manera inicial (clormadinona con o sin medroxiprogesterona) o bien con anticonceptivos combinados. En la mayoria de las pacientes se obtuvo una respuesta favorable; sin embargo, hubo casos en los que fue necesario modificar la dosis y el tiempo de tratamiento. Conclusiones La mayor parte de las adolescentes con ERC que han sido tratadas por hemorragia uterina anormal en nuestro estudio tuvieron una respuesta favorable al tratamiento hormonal. Background Chronic kidney disease (CKD) is associated with menstrual abnormalities and management of uterine bleeding is often complex because of the conditions in this group of patients. The aim of this study was to describe the clinical response to hormonal treatment of menstrual alterations in adolescents with CKD. Methods We present data of cases of adolescent patients with CKD who had undergone menstrual changes and received treatment during the period 2008 to 2012. The characteristics of the menstrual disorder, hormone treatment received, and response to treatment were evaluated. The statistical analysis aplicated to analyze the results was descriptive. Results We studied 11 patients with a mean age of 14.5 years, who were in predialysis (n = 1), peritoneal dialysis (n = 7), hemodialysis (n = 3). Patients had hyperpolymenorrhea associated with opsomenorrhea (n = 3), mostly classified as secondary abnormal uterine bleeding. Treatment, in general, was with progestins initially (chlormadinone with or without medroxyprogesterone) or combined contraceptives. In the majority of the patients, a favorable response was obtained; however, there were cases where it was necessary to modify the dose and time of treatment. Conclusions The majority of adolescents with CKD who have been treated for abnormal uterine bleeding in our study had a favorable response to hormonal treatment.

Foreign body (FB) ingestion is a common home accident during childhood; a timely management by the specialists may help prevent complications in the short and long term.To describe the characteristics and complications of FB ingestion located in the gastrointestinal tract in the pediatric population.Two phases, retrospective, and prospective study. Patients diagnosed with FB ingestion between January 1971 and December 2016 were included. The general characteristics of patients, FB type, removal method, and complications were registered. A descriptive analysis was performed.Over 45 years, 2637 FBs were removed from the pharynx (n= 118), the esophagus (n= 2410), the stomach (n= 103), and the intestines (n= 6). Male patients predominated (50.9%); 74% were younger than 5 years. Besides, 57% arrived within the first 24 hours; ptyalism, dysphagia, and vomiting were the main symptoms; 16% of patients had no symptoms. It was possible to locate the FB using an X-ray in 93% of cases; the most common FBs were coins (78%); the most frequent location was the upper third of the esophagus (79%); 86% of FBs were removed using a rigid endoscope, and complications were observed in 7.8% of patients.FB ingestion predominated among children younger than 5 years; metal objects were the most common ones. A plain X-ray is the test of choice for diagnosis; removal is usually done with a rigid or flexible endoscope, depending on the endoscopist's experience.Introducción: La ingestión de un cuerpo extraño (CE) es un accidente frecuente en el hogar en la edad pediátrica; su manejo oportuno por especialistas puede evitar complicaciones a corto y largo plazo. Objetivo: Describir características y complicaciones de los CE en el tubo digestivo en población pediátrica. Material y métodos: Estudio transversal, retrospectivo y prospectivo. Se incluyeron pacientes con diagnóstico de ingestión de un CE desde enero de 1971 a diciembre de 2016. Se recabaron características generales de los pacientes, tipo de objeto, métodos de extracción y complicaciones. Se realizó análisis descriptivo. Resultados: Durante 45 años, se extrajeron 2637 CE localizados en faringe (n= 118), esófago (n= 2410), estómago (n= 103) e intestino (n= 6). Predominaron en el sexo masculino (50,9%); 74% fueron en menores de 5 años. El 57% llegó en las primeras 24 horas; sialorrea, disfagia y vómito fueron los principales síntomas y un 16% estaba asintomático. La radiografía permitió localizar el CE en el 93%; el más ingerido fue la moneda (78%); la localización más común fue el tercio superior del esófago (79%); el 86% se extrajo con endoscopio rígido y las complicaciones se presentaron en el 7,8% de los casos. Conclusiones: La ingestión de un CE predomina en menores de 5 años, y los metales son los más frecuentes. La radiografía simple es el estudio de elección, que permite realizar el diagnóstico y la extracción es por vía endoscópica con endoscopio rígido o flexible según la experiencia del endoscopista.

Background Type 2 diabetes mellitus (DM2) is a chronic disease, and for treatment to succeed, it is necessary to harmonize the mental health of the patient with the environment, which impacts quality of life and adherence to medical regimens. The objetive of this study is describe the quality of life of patients with DM2 and the factors relates to its modification. Methods This investigation was a cross-sectional study. Patients over 18 years of age with DM2 were selected. The following variables related to quality of life were studied: age, sex, occupation, marital status, years of DM2 evolution, comorbidities and presence of depression (Beck Depression Inventory). Perceived quality of life was measured with a health-related quality of life (HRQoL) scale, the 36-Item Short-Form Survey (SF-36). Patients were classified according to SF-36 HRQoL score ( 76 points). Results Among the 1394 patients included, the median age was 62 years. Global HRQoL had a median of 50.1 points. Bivariate analysis showed that age, marital status, sex, occupation, comorbidities, duration of DM2 and comorbidities had impacts on HRQoL. The logistic regression model identified age (odds ratio [OR] 1.04) and depression (OR 4.4) as independent factors that influenced overall quality of life. Conclusions Patients with DM2 have poor HRQoL, which is associated with a high frequency of depression. Older age and the presence of depression impair patient HRQoL.
