Abstract

Introduction Aging is a natural, cumulative, and irreversible process that culminates in various chronic conditions, such as cognitive decline and decrease in handgrip strength (HGS). Both conditions share not only [...], Objective. The aim of this study was to analyze the bidirectional association between handgrip strength (HGS) and cognitive performance in different cognitive functions in a European population and to evaluate the predictive validity of HGS for the risk of future cognitive impairment in aging individuals. Methods. This was a prospective cohort study conducted using data on individuals over 50 years of age from the Survey of Health, Aging and Retirement in Europe (SHARE). HGS measures and scores in numeracy, recall, and verbal fluency were repeated and analyzed biannually for 4 years and were used in generalized estimating equations to test the bidirectional association, categorized by sex. Results. Of the 8236 individuals included, 55.73% were women with a mean age of 67.55 (standard deviation [SD] = 8.4) years and 44.27% were men with a mean age of 68.42 (SD =7.7) years. HGS predicted cognitive decline in both sexes, except for numeracy in men, even after adjustments. The strongest association with HGS in women was in verbal fluency ([beta] =.094; 95% CI =0.039 to 0.151), whereas the strongest association with HGS in men was in delayed verbal recall ([beta] =.095; 95% CI =0.039 to 0.151). Conversely, the greatest cognitive predictor of HGS decline was verbal fluency in men ([beta] = .796; 95% CI =0.464 to 1.128), and in women ([beta] = .801; 95% CI= 0.567 to 1.109). Conclusion. There is a significant and bidirectional association between HGS and different cognitive functions in a European multicentric population. This bidirectional association differed between sexes. Impact. Both men and women who presented with cognitive decline also showed early changes in their HGS measures, and vice versa, but there still were differences between the sexes. These findings reinforce that HGS may be a simple and inexpensive method to identify early signs of cognitive decline, and that studies and rehabilitation strategies should be more sex specific.

Abstract

OBJECTIVE: To develop a checklist that guides the requirements necessary for public digital platforms in providing AT, based on the WHO-proposed "The WHO-GATE 5P framework" and the Global Report on Assistive Technology. METHODS: A qualitative descriptive research approach was employed consisting of 4 stages: 1) Construction of the checking tool, 2) Discussion of the checklist by a group of three researchers until a final proposal was reached, 3) Analysis of the checklist by two external researchers with experience in AT research and services, 4) Analysis of the suggestions and formatting of the final version of the checklist. Three of the authors/researchers conducted a documental analysis of the "WHO-GATE 5P framework" and Global Report on Assistive Technology, and two experts in research and AT services, performed the checklist analysis independently. RESULTS: The final version of the checklist with 25 items was created and made available in English and Portuguese version. Conclusion: This checklist based on the fundamentals of the assistive technology model centred on people, the "WHO-GATE 5P framework" and the Global Report on Assistive Technology provides direction for digital AT service delivery, delivery practices and data science in digital services aimed at addressing the needs of AT consumers, while supporting advancements in the provision of digital AT services. Abstract

The Validation Method (VM) is a specific communication method for older people with dementia. The aim of this study was to analyze the effect of an introductory training course on the Bases of the Validation Method (BVM) on job satisfaction and motivation in healthcare professionals working in nursing homes (NHs) and its influence on self-efficacy, perceived personal competence, organizational commitment, attitudes toward dementia and burnout. A quasi-experimental pilot study was conducted during the COVID-19 pandemic (September 2020–February 2021). Participants were divided into two groups: experimental group (who did 16 hours of training on BVM), and a control group (no intervention). Assessments were made at baseline, one week and 4 months after starting the intervention through validated questionnaires to assess job satisfaction, work motivation, self-efficacy, perceived competence, organizational commitment, attitudes toward people with dementia and burnout. A paired Student's t-test was used to check intragroup differences, and unpaired Student's test and multilevel mixed-effects linear regression model were used to assess the impact of the intervention; Hedges'g was used to determine the effect size. The sample consisted of 31 NH professionals, assigned to an experimental group (n = 11) or a control group (n = 20). The BVM training had a positive moderate effect size on job satisfaction, work motivation, self-efficacy, perceived competence and organizational commitment (Hedges'g > 0.30). In contrast, there was a smaller effect on attitudes toward people with dementia (Hedges'g = –0.21), and a moderate negative effect on burnout (Hedges'g = –0.38). BVM training had a positive moderate effect on all variables 4 months after finishing training, except for attitudes toward dementia, for which the effect size was smaller. [ABSTRACT FROM AUTHOR]

31. Effectiveness of interventions for reducing sedentary behaviour in older adults living in long-term care facilities: a protocol for a systematic review

Abstract

Background. Sedentary behaviour (SB) is an important risk factor for several health-related outcomes. The prevalence of SB is alarmingly high in older adults, who spend on average 9.4 h being sedentary each day, making them the most sedentary of all age groups. Objectives. The primary objective of this review is to assess the impact of interventions aimed at reducing SB in older adults (aged 60 years and older) living in long-term care facilities (LTCFs). The research question for this systematic review is as follows: in older people living in LTCFs, do interventions aimed at reducing SB, compared to usual care, result in a decrease in SB daily time or a reduction in the length of prolonged and uninterrupted sitting bouts? Data sources. Only peer-reviewed articles will be included in this systematic review, articles will be identified using the PICO method in seven different databases. Participants and interventions. Any primary intervention study (including randomized controlled trials, non-randomized controlled trials, and cohort studies) with the aim to reduce SB daily time or shorten the length of prolonged and uninterrupted sitting bouts in older adults living in LTCFs will be included. After searching databases, abstracts of the studies will be screened, and, after retrieving full text articles, data extraction will be conducted by two independent reviewers. Study appraisal and synthesis methods. The review will adhere to PRISMA reporting guidelines. Risk of bias (RoB) will be assessed using ROBINS-I or the RoB 2.0 tool and will be discussed with a third reviewer. The data will be grouped according to study design, with separate analysis for randomised and non-randomised designs. Results. The primary outcomes will be SB or time spent sedentary, assessed before and after the intervention. For the outcomes with the same measurement units, the pooled mean differences will be calculated. Standardised mean differences will be calculated for the outcomes with different measurement units. The data not suitable in numbers will be synthesised narratively. The strength of evidence of the outcomes will be assessed using GRADE assessment. If the data are suitable for quantitative analysis, we plan to use the Revman software to conduct a meta-analysis. Conclusions and implications of key findings. This protocol can serve as a valuable resource for other researchers interested in conducting similar systematic reviews or meta-analyses in the field of SB and older adult health.

Abstract

The aim of this study was to determine whether the pandemic has reinforced the choice of pursuing health-related bachelor’s degrees, and to identify underlying factors that could contribute to that impact. This is a cross-sectional study using an online survey of 2,344 students of nursing, physiotherapy, medicine, psychology and podiatry who started health-related bachelor’s degrees after the COVID-19 outbreak in Spanish higher education institutions. The pandemic influenced the choice of these studies by increasing the desire to help others (33.2%), by increasing citizenship values (28.4%), and by increasing the desire to contribute to improving the situation of the country (27.5%). Women had a significantly greater influence on the increase in social values related to the practice of the profession produced by the pandemic, whereas men and the bachelor’s degree in podiatry were more influenced by salary prospects. An increased desire to help others was significantly higher among women and nursing and medical students. Podiatry and psychology were the degrees were most influenced by the pandemic, as more students decided to pursue them, something they had previously doubted, while in nursing, psychology, and medicine the pandemic reinforced their interest in pursuing the degree the most. Students personally affected by COVID-19 reported being more influenced in reconsidering their professional path and in reinforcing their desire to pursue the health-related studies.

Abstract

Background: Total knee arthroplasty (TKA) is a common surgical procedure for patients with advanced osteoarthritis. !is study aimed to assess the e'ects of using versus not using a fast-track protocol, including a new mobilization device called Flexet. Methods: !is is a retrospective comparative study. Two groups were formed with a total of 507 TKA patients. 283 were treated in 2010 with a standard program (S group) and 224 with a fast-track protocol (FT group) in 2016. !e variables studied were active knee #exion and extension, length of stay, and time to autonomous gait. Results: Study groups were comparable. !e mean time from surgery to autonomous gait was shorter for the FT group (4.43 hours, SD = 2.11) than for the S group (59.95 hours, SD = 16.59) (p < 0.001). Mean stay for the FT group was 2.36 nights (SD = 1.81) and 6.20 nights (SD = 1.52) for the S group (p < 0.001).
