This paper explores the archaeology of one of the more recently excavated 'convict hut' sites, the structures associated with early convict occupation (c.1788–1818) of the colonised agricultural landscape of what is now Parramatta in New South Wales. The paper primarily examines what substantive conclusions can be drawn from what could be considered repetitious archaeological sites: one hut in a long line of huts. The work focuses on the temporal and spatial constraints of evidence from the Club Parramatta site, building on the legacy of excavations that have occurred over the last 40 years. The results are framed through a conceptual lens of assemblages of practice and make use of comparative artefact analysis of three huts. We argue that New Materialism is particularly helpful for avoiding dualistic interpretations, such as convict/free, and instead allows for more nuanced and active understandings of people in the past. Background: Strategies are needed to improve referral into, and uptake of, osteoarthritis (OA) management programs. This survey investigated and compared patients' and medical professionals' views around hip and knee OA management and factors impacting implementation of an osteoarthritis management program. Methods: As part of a mixed‐methods program of research, patients with hip or knee OA and medical professionals routinely involved in the management of OA, were invited to complete a comprehensive online survey. All data were analysed descriptively or using chi squared tests. Survey findings for factors perceived to impact implementation of an OA management programme were triangulated with previously reported qualitative data. Results: Fifty‐three patients (38 females, 15 males) and 32 medical professionals (orthopaedic surgeons, sports physicians, rehabilitation physicians, rheumatologists and general practitioners) completed the survey. Twenty‐eight patients (53%) had prior participation in the OA management programme (GLA:D®) and 19 medical professionals (59%) had previously referred patients to the programme. Of the participants with prior exposure, 21 patients (75%) and 15 medical professionals (79%) agreed the programme was beneficial. A higher proportion of medical professionals, compared to patients, believed weight loss (100% vs. 67%), injection therapy (50% vs. 21%), hip replacement (100% vs. 62%) and knee replacement (97% vs. 62%) were effective treatments, with no differences for all other treatments. The barriers and enablers identified for referral into, and participation in, an OA management programme mostly aligned to factors identified in previous related qualitative research. Divergent factors in the survey included patients concerns about doing exercise‐therapy with 81% (higher than expected) not reporting any concerns about exercising, and 19% (lower than expected) concerned about their OA joint, such as making their pain worse. Conclusions: This study has extended our understanding of barriers and enablers for referral into, and participation in, an OA management programme with a lower than expected number of patients being concerned about exercising due to their OA joint. Patients and medical professionals had positive views relating to the quality of the programme delivery, patient satisfaction and programme effectiveness. Medical professionals were more likely than patients to consider weight loss, injections and joint replacement as effective treatment options. [ABSTRACT FROM AUTHOR]

Mobilisation is recommended following cardiac surgery, but much less is known about its effects on physiological parameters. This study aimed to determine the effect of mobilisation on cardiorespiratory physiological parameters, and to establish the relationship between physiological parameters and physical function following cardiac surgery. Participants requiring physiotherapy following cardiac surgery were recruited. Mobilisation involved ground-based walking, while physical function was assessed using the short physical performance battery (SPPB) test. During both procedures, physiological measures of heart rate (HR), respiratory rate (RR) and percutaneous oxygen saturation (SpO2) were periodically recorded. Participant perception of breathlessness and perceived exertion were recorded pre and post mobilisation and the SPPB test. Forty-five participants (mean [SD] age 69[7] years) completed the study. HR (mean difference 10[95% CI 8 to 12] bpm) and RR (13[10 to 17] br/min) significantly increased following mobilisation, while SpO2 significantly decreased (baseline median [IQR] 95% [95%–97%] vs nadir 94% [91% to 96%], p < 0.002). There was a significant increase in dyspnoea and perceived exertion (both p < 0.001). There was a moderate relationship between total SPPB score and SpO2 during mobilisation (rs=0.45). Mobilisation following cardiac surgery increases exertion and dyspnoea and alters physiological parameters. Greater physical function is linked to maintaining a higher oxygen saturation during mobilisation. [ABSTRACT FROM AUTHOR]

The article reviews the Patient-Specific Functional Scale (PSFS) which is used to measure the change over time of a limited range of musculoskeletal conditions. PSFS identifies and rates the five different activities patients are unable to perform or have difficulty with. Studies that implement PSFS as an outcome measure or report the measurement properties of PSFS suggest that it was reliable, valid and responsive to people with knee, low back and neck dysfunction.
