Insect meal inclusion in aquaculture feed has received increased interest as a sustainable alternative to fishmeal and recent evidence has shown additional effects on modulating the immune response. However, lack of effects of insect meal on fish immunity in a few studies have put these beneficial effects into question. The objective of this meta-analysis was to summarize the effects of fishmeal replacement with insect meal on the innate immune response of several fish species based on differential gene expression via qPCR. A systematic literature search was conducted using online databases including Web of Science and ScienceDirect that found 197 studies as of August 2023, however only 20 studies met the criteria of high-quality studies focused on the immune response of fish and were included in this meta-analysis. The most studied insect meal was from black soldier fly, followed by yellow mealworm, and the most commonly analyzed tissue was the liver, followed by the intestine. The effect of fishmeal replacement with insect meal on fish immune responses were examined using a mixed model, with study as the random effect. This meta-analysis found a non-linear (quadratic) relationship (P = 0.017) between immune gene expression fold change and the level of fishmeal replacement with insect meal. Based on the fitted quadratic curve, a 40 % replacement of fishmeal with insect meal resulted in the highest upregulated immune response in fish. Fish taxonomic family was also found to have an effect (P = 0.004) on immune gene expression, where the taxonomic family Moronidae was more affected, with an average fold change of 2.783 (± 0.325). Other variables examined, including the insect type, tissue analyzed, dietary crude protein level, did not affect the immune response (P > 0.05). This meta-analysis also found that dietary lipid had a significant correlation with immune gene expression, and should be taken into consideration in future studies and meta-analyses. These findings are impactful since they provide evidence for the optimal dietary replacement level of insect meals required to significantly affect the innate immune response across several fish species.

BackgroundHealth systems had to rapidly implement infection control strategies to sustain their workforces during the COVID-19 pandemic. Various outbreak response tools, such as digital contact tracing (DCT), have been developed to monitor exposures and symptoms of health care workers (HCWs). Limited research evidence exists on the experiences with these technologies and the impacts of DCT innovations from the perspective of stakeholders in health care environments. ObjectiveThis study aims to identify the factors influencing the adoption of DCT, highlight variations in perspectives across 3 key stakeholder groups concerning the impact of DCT, and provide benchmarking evidence for future pandemic preparedness. MethodsGuided by the Reach, Effectiveness, Adoption, Implementation, and Maintenance (RE-AIM) framework, we conducted an exploratory qualitative study to investigate the implementation and impact of DCT at the Children’s Hospital of Eastern Ontario between December 2022 and April 2023. We conducted 21 semistructured interviews with key stakeholders, including health care administrators (6/21, 29%), occupational health and safety specialists (8/21, 38%), and HCWs (7/21, 33%). Stakeholders were asked about the factors influencing engagement with the DCT tool, organizational-level uptake, the implementation process, long-term use and sustainability of DCT, and unintended consequences. Verbatim transcripts were subject to thematic analysis using NVivo (QSR International). ResultsThe implementation of DCT was viable and well received. End users indicated that their engagement with the DCT tool was facilitated by its perceived ease of use and the ability to gain awareness of probable COVID-19 exposures; however, risk assessment consequences and access concerns were reported as barriers (reach). Participants commonly agreed that the DCT technology had a positive influence on the hospital’s capacity to meet the demands of COVID-19 (effectiveness). Implementors and occupational specialists referred to negative staffing impacts and the loss of nuanced information as unintended consequences (effectiveness). Safety-focused communication strategies and having a DCT tool that was human-centered were crucial factors driving staff adoption of the technology. Conversely, adoption was challenged by the misaligned delivery of the DCT tool with HCWs’ standard practices, alongside the evolving perceived threat of COVID-19. Stakeholders collectively agreed on the viability of DCT and its applicability to infectious disease practices (maintenance). ConclusionsHospital stakeholders were highly satisfied with DCT technology and it was perceived as feasible, efficient, and having a positive impact on organizational safety. Challenges related to the alignment and delivery of DCT, alongside the evolving perspectives on COVID-19, posed obstacles to continued adoption by HCWs. Our findings contribute to evidence-based practices and present benchmarks that can inform preparedness for future pandemics and infectious disease outbreaks and help other organizations implement similar technologies.

Feather pecking (FP) is a significant issue in laying hens, which impacts societal acceptance of poultry farming, farm productivity, and bird welfare. Environmental enrichment—modifications of the environment to stimulate biological functioning and psychological well-being of animals—is one management strategy farmers can use to mitigate FP. However, inconsistent results of environmental enrichment are reported across studies questioning its value. A meta-analysis was used to determine the effect of environmental enrichment on FP and feather damage (FD) in laying hens. A systematic review of published literature from 4 databases resulted in 23 publications that met inclusion criteria. Feather pecking and FD outcomes were standardized between studies using different scoring systems. Driving variables included the presence of enrichment, production period when the enrichment started, housing type, beak trimming, bird strain, and age of the birds when FP and FD was measured. We investigated the uptake and perceptions of virtual care solutions by rural Canadian primary and specialist providers during the early phase (May-June 2020) of the COVID-19 pandemic. A web-based, cross-sectional survey of rural primary and specialty care providers examined types of virtual care platforms used (eg, phone, video), appointment length, experience and satisfaction with the solution used, plans for future use of virtual care, and patients’ use of virtual care services. Targeted participants were actively-practicing providers in rural Western Canada who were emailed an invitation for the study and its survey link. Fifty-nine providers (26% response rate) completed the survey. During the pandemic, 78% of providers reported using virtual care for more than 60% of their appointments, while only 3% did so frequently pre-pandemic. Most providers used phone consultations, despite believing that video provided a better virtual visit. Key barriers included workflow interruptions, unique concerns about quality of care, remuneration and sustainability, or poor internet access and bandwidth for both providers and patients. The key opportunity noted was improved access to care. While most virtual care visits were not conducted using video technologies, overall virtual care resulted in high provider satisfaction, while not increasing workload. Virtual care will continue to play an important role in future rural care practice; however, sustainability will require both provider-level and system-level changes.

Poultry egg and meat production continue to be optimized for productivity and efficiency. However, genetic selection focusing on production efficiency has overlooked other aspects critical to bird wellbeing, such as skeletal development. As a result, modern birds are more prone to leg weakness, osteoporosis, and, subsequently, fractures. Dietary omega-3 polyunsaturated fatty acid (n-3 PUFA) enrichment has been proposed to benefit bone development, quality, and strength. However, there is a lack of conclusive and quantitative results across studies. Therefore, a meta-analysis approach was used to evaluate published studies to determine the effects of dietary n-3 PUFA enrichment on bone quality in laying- and meat-type birds. Publications were retrieved from multiple sources (databases and hand searching), and ten studies were selected for inclusion in the final dataset. A model to predict tibial bone ash content (BAC) was developed in Proc MIXED of SAS, treating the study as a random effect. The dietary concentration of n-3 PUFA, n-3 PUFA:n-6 PUFA ratio, calcium (Ca), phosphorus and feeding duration (days) were used as independent variables to predict BAC. The final model included the dietary n-6:n-3 FA ratio and the calcium concentration in the diet. The final model was selected based on the corrected Akaike Information Criteria, the root mean square prediction error (0.999) and its components, and the concordance correlation coefficient (CCC) (0.99). In laying-type birds, BAC was reduced by n-3 PUFA (p = 0.001) but was increased by Ca (p = 0.014). In contrast, in broiler chickens, BAC was increased by n-3 PUFA (p = 0.001) and decreased by Ca (p = 0.014). The influence of n-3 PUFA:n-6 PUFA ratio on tibia BAC in laying-type birds was not statistically significant (p = 0.505), whereas in meat-type birds, the influence of PUFA ratio was significant (p < 0.05). These results may indicate a low biological significance in laying-type birds but not in meat-type birds.

ABSTRACT: Temperature stress (TS) is a significant issue in poultry production, which has implications for animal health and welfare, productivity, and industry profitability. Temperature stress, including both hot (heat stress) and cold conditions (cold stress), is associated with increased incidence of meat quality defects such as pale, soft, and exudative (PSE) and dark, firm, and dry (DFD) meat costing poultry industries millions of dollars annually. A meta-analysis was conducted to determine the effect of ambient TS on meat quality parameters of poultry. Forty-eight publications which met specific criteria for inclusion were identified through a systematic literature review. Temperature stress was defined by extracting 2 descriptors for each treatment mean from the chosen studies: (1) temperature imposed for the experimental treatments (°C) and duration of temperature exposure. Treatment duration was categorized for analysis into acute (≤24 h) or chronic (>24 h) treatments. Meat quality parameters considered were color (L*-a*-b* scheme), pH (initial and ultimate), drip loss, cooking loss, and shear force. Linear mixed model analysis, including study as a random effect, was used to determine the effect of treatment temperature and duration on meat quality. Model evaluation was conducted by performing a k-fold cross-validation to estimate test error, and via assessment of the root mean square prediction error (RMSPE), and concordance correlation coefficient (CCC). Across both acute and chronic durations, treatment temperature was found to have a significant effect on all studied meat quality parameters. As treatment temperature increased, meat demonstrated characteristics of PSE meat and, as temperature decreased, meat demonstrated characteristics of DFD meat. The interaction between treatment temperature and duration was significant for most traits, however, the relative impact of treatment duration on the studied traits was inconsistent. Acute TS had a larger effect than chronic TS on ultimate pH, and chronic stress had a more considerable impact on color traits (L* and a*). This meta-analysis quantifies the effect of ambient TS on poultry meat quality. However, quantitative effects were generally small, and therefore may or may not be of practical significance from a processing perspective.

Bile acids (BA) are produced in the liver and conjugated with glycine or taurine before being released into the small intestine to aid with lipid digestion. However, excessive BA losses through feces can occur due to several dietary factors that in turn require greater production of BA by the liver due to a reduction in BA recycling. Consequently, net utilization of taurine and/or glycine is increased. To quantify this impact, we conducted a meta-analysis to investigate the effect of soluble fiber, diet composition, and species on fecal excretion of BA. After a systematic review of the literature, twelve studies met all inclusion criteria. Dietary carbohydrate, protein, fat, cellulose, cholesterol, soluble fiber and animal species were tested as independent variables. Mixed models were developed treating study as a random effect, and fixed effect variables were retained at P < 0.05 significance and where collinearity was absent between multiple X variables. A total of ten studies comprised of four species [(rat = 5), hamster (n = 1), guinea pig (n = 3) and dog (n = 1)], and 30 observations were evaluated in the final models after outlier removal. Model evaluation was based on the corrected Akaike Information Criteria, the concordance correlation coefficient and the root mean square prediction error. Three base models were developed, examining carbohydrate, protein and fat impacts separately. The best fitting models included the fixed effect of species and the interaction between soluble fiber (yes/no) and dietary carbohydrate, protein or fat (%, as-fed). Increased concentrations of dietary protein and fat resulted in greater fecal excretion of BA (P < 0.05). Conversely, increasing levels of dietary carbohydrate led to lower excretions of BA (P < 0.05). Increased dietary soluble fiber containing ingredients resulted in greater excretion of BA in all models (P < 0.05). Rats had greater excretion of BA compared to hamsters and guinea pigs (P < 0.05) in all models, and also compared to dogs (P < 0.05) in the carbohydrate model. The findings from this meta-analysis indicate that not only soluble fiber, but also increasing levels of dietary fat and protein may result in greater fecal excretion of BA, potentially altering taurine and/or glycine metabolism and affecting the need for diet delivery of these AA.

Lactobacillus species play a critical role in the bidirectional communication between the gut and the brain. Consequently, they have the potential to aid in the treatment of psychological disorders. The impact of Lactobacillus supplementation on the stress responses triggering psychological disorders has not been systematically reviewed. Therefore, the aim of this meta-analysis is to summarize the body of research assessing the effects of Lactobacillus-based probiotics in rodents that underwent an experimental stress treatment or not. The duration of immobility in a Forced Swim Test (FST) was the outcome used to measure changes induced by various treatments. Four online databases were systematically searched for relevant studies published in English. Fourteen studies meeting the criteria were included in the meta-analysis. The effects of probiotic supplementation and stress treatment on the duration of immobility in the FST were analyzed using a generalized linear mixed model. Publication bias was evaluated by funnel plots. Our analysis shows that Lactobacillus-based probiotic supplements significantly reduce immobility in the FST (P < 0.001) in stressed rodents. However, probiotics did not affect the rodents that did not undergo the stress treatment (P = 0.168). These findings provide a better understanding of the potential of Lactobacillus-based probiotics for the management of stress-induced behavior.

Smart engineered and naturally derived nanovesicles, capable of targeting specific tissues and cells and delivering bioactive molecules and drugs into them, are becoming important drug delivery systems. Liposomes stand out among different types of self-assembled nanovesicles, because of their amphiphilicity and non-toxic nature. By modifying their surfaces, liposomes can become stimulus-responsive, releasing their cargo on demand. Recently, the recognized role of exosomes in cell-cell communication and their ability to diffuse through tissues to find target cells have led to an increase in their usage as smart delivery systems. Moreover, engineering “smarter” delivery systems can be done by creating hybrid exosome-liposome nanocarriers via membrane fusion. These systems can be loaded in naturally derived hydrogels to achieve sustained and controlled drug delivery. Here, the focus is on evaluating the smart behavior of liposomes and exosomes, the fabrication of hybrid exosome-liposome nanovesicles, and the controlled delivery and routes of administration of a hydrogel matrix for drug delivery systems.

In the aftermath of COVID-19 school closures, parents, educational technology, and other home factors have played an increasing role in student support and success. As part of a larger evaluation study on the equity and impacts of the nation's largest community broadband initiative, this paper investigates the relationship between access, beliefs, and parents' use of educational technology over two years. Surveying a random sample of 400+ households in addition to in-depth longitudinal interviews with eleven families, results show most parents actively engaged and positively valued several aspects of technology applied to support their child's learning. Moreover, initial results indicate provision of broadband access also had a statistically significant relationship with parent uses of technology.

Patients with systemic sclerosis (SSc) are routinely investigated with high-resolution computed tomography (HRCT) chest for early detection and accurate characterization of complicating interstitial lung diseases. Though the primary aim of HRCT is to delineate the burden of pulmonary involvement and to characterize the nature of fibrosis to potentially help guide management, it provides an opportunity to evaluate extrapulmonary manifestations, particularly the dilated pulmonary artery, esophageal dilatation, and pericardial abnormalities which have their own clinical significance. The aim of this article is to discuss the significance of various pulmonary and extrapulmonary abnormalities that may be identified on HRCT chest of SSc patients.

All rumen models cover the main drivers of variation in rumen function, which are feed intake, the differences between feedstuffs and feeds in their intrinsic rumen degradation characteristics, and fractional outflow rate of fluid and particulate matter. Dynamic modeling approaches are best suited to the prediction of more nuanced responses in rumen metabolism, and represent the dynamics of the interaction between substrates and micro-organisms and inter-microbial interactions. The concepts of dynamics are discussed for the case of rumen starch digestion as influenced by starch intake rate and frequency of feed intake, and for the case of fermentation of fiber in the large intestine. Adding representations of new functional classes of micro-organisms (i.e. with new characteristics from the perspective of whole rumen function) in rumen models only delivers new insights if complemented by the dynamics of their interactions with other functional classes. Rumen fermentation conditions have to be represented due to their profound impact on the dynamics of substrate degradation and microbial metabolism. Although the importance of rumen acidity is generally acknowledged, more emphasis is needed on predicting its variation as well as variation in the processes that underlie rumen fluid dynamics. The rumen wall has an important role in adapting to rapid changes in the rumen environment, clearing of volatile fatty acids (VFA), and maintaining rumen pH within limits. Dynamics of rumen wall epithelia and its role in VFA absorption needs to be better represented in models which aim to predict rumen responses across nutritional or physiological states. For a detailed prediction of rumen N balance there is merit in a dynamic modeling approach compared to the static approaches adopted in current protein evaluation systems. Improvement is needed on previous attempts to predict rumen VFA profiles, and this should be pursued by introducing factors that relate more to microbial metabolism. For rumen model construction, data on rumen microbiomes are preferably coupled with knowledge consolidated in rumen models instead of relying on correlations with rather general aspects of treatment or animal. This helps to prevent disregard of basic principles and underlying mechanisms of whole rumen function.

Foundational undergraduate courses in chemistry and mathematics are gateways to a number of lucrative majors and pre-professional programs. Historically, however, students often struggle with these disciplines and can benefit from alternate approaches to examining their learning strategies. This article describes the use of exam wrappers--reflective activities performed by students after taking exams--to improve students' knowledge and regulation of metacognition--the process of thinking about thinking. Quantitative and qualitative analyses of student learning are described, along with suggestions for other practitioners and researchers to consider in coaching metacognitive strategies.

Background: Alberta Health Services (AHS) measures hand hygiene compliance through direct observations performed by trained site-based reviewers (SBRs) and facilitated by the Infection Prevention and Control (IPC) program. Within AHS there are >100 acute care facilities, ranging in bed size from four beds to more than 1,000, with catchment populations ranging from one million. A time-in-motion study using trained AHS IPC staff was proposed to validate the completeness and accuracy of data being collected by the SBRs. Methods: The AHS IPC staff performed direct observations at pre-selected facilities across all five zones and four different unit types (emergency, medical, surgical, and intensive care) for four 30-minute periods during weekdays between June and September 2023. An iPad app was used to capture results from all four moments of hand hygiene. The reviewer indicated the day and time of the review and captured as many representative hand hygiene moments and healthcare providers as possible. The distributions of the four moments of hand hygiene, healthcare provider group and overall compliance were compared at the unit type and facility level (tertiary, large urban, regional, pediatric, and small sites) between this time-in-motion study and SBR data collected June-September 2023. Results: The study collected 175 reviews and 4,683 observations from 14 facilities and 48 units. Between June and September 2023, SBRs collected 2,625 reviews and 61,506 observations from these same facility and unit types. Across all facility and unit types, the distribution of the four moments was similar between the study and SBRs. Similar proportions of healthcare providers were also observed. However, the overall hand hygiene compliance collected in the study was approximately 10% lower across all unit types as compared to that collected by the SBRs (study: 63%-84%; SBRs: 75%-92%). Conclusions: In public health surveillance, completeness and accuracy are two characteristics of high-quality data. A time-in-motion study identified that the hand hygiene observations collected by SBRs were complete, as the range of healthcare providers observed, and the distribution of their moments, mirrored that collected in the study. However, the SBRs reported higher compliance than the study participants and the true hand hygiene compliance is likely lower than what is currently being reported. Since this difference was seen consistently across all unit and facility types, trending data over time should still identify areas in need of improvement and may help to suggest causes of the over-reporting.

Background: Bloodstream infections (BSIs) are an important cause of morbidity and mortality in severely ill patients, contributing to increased length of hospital stay and higher cost of care. Alberta Health Services Infection Prevention and Control (IPC) conducts inpatient surveillance of new episodes of BSIs with methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-resistant enterococci (VRE) or carbapenemase-producing organisms (CPO) in 112 acute care facilities. A case-finding process was undertaken to verify the accuracy of BSI data entry. Methods: All positive MRSA, VRE or CPO blood cultures in 2021 were linked to the Inpatient Discharge Abstract Database (DAD) and the National Ambulatory Care Reporting System (NACRS) to identify new cases during acute care admissions. The results were then compared to surveillance records captured by infection control professionals (ICPs). Cases with unmatched culture date and/or encounter date and cases not identified by ICPs were screened by the study team with final decision made by ICPs. Results were analyzed by ARO and by % increase in number of surveillance records. Results: The laboratory linkage identified 286 new cases. Comparing to surveillance records (n = 248) captured by ICPs, 137 (57.3%) had matching collection dates and encounter dates, 85 (35.6%) had close matches on collection dates and encounter dates, 17 (7.1%) records had either matching collection dates or encounter dates, and 1 (0.4%) record did not have any matches on dates. There were 46 records identified in the laboratory data that were not in the surveillance system and 8 records that were in the surveillance system but not matched to the laboratory data. After review, 22 Surveillance records had data entry errors (1 CPO BSI, 20 MRSA BSI, and 1 VRE BSI), and there were 14 BSI records found to be missing (13 MRSA BSI, 1 VRE BSI). This represents a 6% increase in MRSA BSI and a 3% increase in VRE BSI identified in 2021 and no increase in CPO BSI. Conclusions: A laboratory validation to determine if BSIs with an ARO were missed during routine IPC surveillance identified a small proportion of missed bloodstream infections. The most common reason for the miss was admission through the emergency department with multiple blood cultures collected during a single admission. These results will be shared with the Infection Control program to facilitate correct BSI capture.
