Pagers, though reliable and familiar technology, can be suboptimal for facilitating healthcare team communication.1,2 Most paging systems utilize single-function pagers and only allow one-way communication, requiring recipients to disrupt workflow to respond to pages. Paging transmissions can also be intercepted, and the information presented on pager displays can be viewed by anyone in possession of the pager. Smartphones allow for instantaneous two-way and group communication through advanced technological features. Their use is widespread; over 81% of American physicians owned a smartphone in 2011.3 Previous studies demonstrate that healthcare providers rate smartphone-based email positively, and that team smartphones can facilitate communication between nurses and physicians.4,5 However, these studies specifically examined the utility of smartphone-based email and voice calls, and did not include text messaging. Limitations of traditional smartphone-based text messaging include Health Insurance Portability and Accountability Act (HIPAA) noncompliance and dependence on in-hospital cellular reception, which can be unreliable. HIPAA is a 1996 US federal law that established a set of privacy and security rules governing not only what is considered protected health information (PHI), but also minimum standards for the protection of such information. HIPAA compliance is defined as meeting these minimum standards for physical, network, and process security.6,7 Though PHI is often transmitted via paging systems and commercial carrier-based text messaging, these modalities are not secure and are thus not HIPAA-compliant. Text messaging applications that address these security and reliability issues have the potential to greatly enhance in-hospital communication. We hypothesized that a smartphone-based HIPAA-compliant group messaging application could improve in-hospital communication on the inpatient medicine service. To our knowledge, our study is the first to examine a HIPAA-compliant text messaging system, and also the first to compare a combination paging/HIPAA-compliant group messaging (HCGM) system with a paging-only system in assessing healthcare provider perception of communication efficiency.

Background Variations in urothelial carcinoma (UC) response to platinum chemotherapy are common and frequently attributed to genetic and epigenetic variations of somatic DNA. We hypothesized that variations in germline DNA may contribute to UC chemosensitivity. Patients and methods DNA from 210 UC patients treated with platinum-based chemotherapy was genotyped for 80 single nucleotide polymorphisms (SNPs). Logistic regression was used to examine the association between SNPs and response, and a multivariable predictive model was created. Significant SNPs were combined to form a SNP score predicting response. Eleven UC cell lines were genotyped as validation. Results Six SNPs were significantly associated with 101 complete or partial responses (48%). Four SNPs retained independence association and were incorporated into a response prediction model. Each additional risk allele was associated with a nearly 50% decrease in odds of response [odds ratio (OR) = 0.51, 95% confidence interval 0.39–0.65, P = 1.05 × 10-7). The bootstrap-adjusted area under the curves of this model was greater than clinical prognostic factors alone (0.78 versus 0.64). The SNP score showed a positive trend with chemosensitivity in cell lines (P = 0.115). Conclusions Genetic variants associated with response of UC to platinum-based therapy were identified in germline DNA. A model using these genetic variants may predict response to chemotherapy better than clinical factors alone.

Fibrosis is a pathological condition in which tissue structure is disrupted by production of excess extracellular matrix (ECM), and chronic tissue fibrosis is associated with tumor development. Myofibroblasts are the principal mediators of fibrosis, producing abundant ECM as well as inflammatory and angiogenic factors. Myofibroblasts are also abundant in tumor stroma, where they facilitate tumor growth and progression. Matrix metalloproteinases (MMPs), enzymes that degrade and remodel the ECM, are believed to play a critical role in the development of fibrotic tissue, though the mechanism by which this occurs is unclear. Expression of MMP-3 in mammary epithelial cells of transgenic mice stimulates development of fibrosis and subsequent tumor formation. We have recently determined that exposure of mammary epithelial cells to MMP-3 induces a specialized form of epithelial-mesenchymal transition in which the cells acquire myofibroblast-like characteristics and that this process is dependent upon the generation of cellular reactive oxygen species (ROS). New data from culture models in which MMPs are inducibly expressed in human lung cell lines, and transgenic mouse models in which MMPs are inducibly expressed in lung alveolar epithelial cells, suggest that similar processes likely exist in the lung.

BACKGROUND Though current hospital paging systems are neither efficient (callbacks disrupt workflow), nor secure (pagers are not Health Insurance Portability and Accountability Act [HIPAA]-compliant), they are routinely used to communicate patient information. Smartphone-based text messaging is a potentially more convenient and efficient mobile alternative; however, commercial cellular networks are also not secure. OBJECTIVE To determine if augmenting one-way pagers with Medigram, a secure, HIPAA-compliant group messaging (HCGM) application for smartphones, could improve hospital team communication. DESIGN Eight-week prospective, cluster-randomized, controlled trial SETTING Stanford Hospital INTERVENTION Three inpatient medicine teams used the HCGM application in addition to paging, while two inpatient medicine teams used paging only for intra-team communication. [Background]: Germline mutations in the BRCA1 and BRCA2 genes are associated with increased risks of breast and ovarian cancers. Although several common variants have been associated with breast cancer susceptibility in mutation carriers, none have been associated with ovarian cancer susceptibility. A genome-wide association study recently identified an association between the rare allele of the single-nucleotide polymorphism (SNP) rs3814113 (ie, the C allele) at 9p22.2 and decreased risk of ovarian cancer for women in the general population. We evaluated the association of this SNP with ovarian cancer risk among BRCA1 or BRCA2 mutation carriers by use of data from the Consortium of Investigators of Modifiers of BRCA1/2. [Methods]: We genotyped rs3814113 in 10 029 BRCA1 mutation carriers and 5837 BRCA2 mutation carriers. Associations with ovarian and breast cancer were assessed with a retrospective likelihood approach. All statistical tests were two-sided. [Results]: The minor allele of rs3814113 was associated with a reduced risk of ovarian cancer among BRCA1 mutation carriers (per-allele hazard ratio of ovarian cancer = 0.78, 95% confidence interval = 0.72 to 0.85; P = 4.8 × 10-9) and BRCA2 mutation carriers (hazard ratio of ovarian cancer = 0.78, 95% confidence interval = 0.67 to 0.90; P = 5.5 × 10-4). This SNP was not associated with breast cancer risk among either BRCA1 or BRCA2 mutation carriers. BRCA1 mutation carriers with the TT genotype at SNP rs3814113 were predicted to have an ovarian cancer risk to age 80 years of 48%, and those with the CC genotype were predicted to have a risk of 33%. Matrix metalloproteinases (MMPs) are enzymes that digest components of the extracellular matrix (ECM) as well as cell surface receptors for soluble factors and junctional proteins involved in cell-cell and cell-ECM interactions. MMPs are involved in many physiological processes that require tissue remodeling but are also expressed in nearly all tumors, where they stimulate tumor growth, invasion, and metastasis. Previous studies have shown that expression of stromelysin-1/MMP-3 in the mammary glands of transgenic mice causes the production of invasive carcinomas; our recent investigations provide insight into how MMPs can promote the development of such tumors. We show that exposing mouse mammary epithelial cells to MMP-3 stimulates epithelial-mesenchymal transition (EMT), a phenotypic alteration in which epithelial cells acquire invasive mesenchymal characteristics. EMT is a necessary component of embryonic development, but acquisition of EMT characteristics by cancer cells facilitates invasion and metastasis. MMP-3-induced EMT involves induction of Snail, a transcription factor long studied for its role in physiological EMT but which is increasingly recognized as a factor involved in tumor progression and malignancy. In this review, we examine how MMPs and Snail function in tumor progression and how identification of an MMP-Snail signaling axis provides insight into new anticancer therapeutic strategies.

The genetics of lymphoma susceptibility reflect the marked heterogeneity of diseases that comprise this broad phenotype. However, multiple subtypes of lymphoma are observed in some families, suggesting shared pathways of genetic predisposition to these pathologically distinct entities. Using a two-stage GWAS, we tested 530,583 SNPs in 944 cases of lymphoma, including 282 familial cases, and 4,044 public shared controls, followed by genotyping of 50 SNPs in 1,245 cases and 2,596 controls. A novel region on 11q12.1 showed association with combined lymphoma (LYM) subtypes. SNPs in this region included rs12289961 near LPXN, (PLYM = 3.89×10−8, OR = 1.29) and rs948562 (PLYM = 5.85×10−7, OR = 1.29). A SNP in a novel non-HLA region on 6p23 (rs707824, PNHL = 5.72×10−7) was suggestive of an association conferring susceptibility to lymphoma. Four SNPs, all in a previously reported HLA region, 6p21.32, showed genome-wide significant associations with follicular lymphoma. The most significant association with follicular lymphoma was for rs4530903 (PFL = 2.69×10−12, OR = 1.93). Three novel SNPs near the HLA locus, rs9268853, rs2647046, and rs2621416, demonstrated additional variation contributing toward genetic susceptibility to FL associated with this region. Genes implicated by GWAS were also found to be cis-eQTLs in lymphoblastoid cell lines; candidate genes in these regions have been implicated in hematopoiesis and immune function. These results, showing novel susceptibility regions and allelic heterogeneity, point to the existence of pathways of susceptibility to both shared as well as specific subtypes of lymphoid malignancy., Author Summary B-cell lymphomas comprise several diseases representing aberrant proliferations of immune cells at various stages of maturation. It might be expected that dissimilar subtypes of lymphoma will have different etiologic and pathogenic mechanisms, reflecting the distinct histologic and clinical characteristics of these diseases. This study aims to define both shared as well as specific genetic risk factors for lymphoma. Utilizing a genome-wide approach, we discovered novel locations in the genome associated with risk for lymphoid malignancies. Common variants in these regions, on chromosome 11q12.1 and 6p23, were each associated with a modest modification of risk for lymphoma. These regions harbor several genes of biological importance in lymphoid maturation and function. We also further characterized the HLA region at 6p21.32, previously associated with lymphoma risk and thought to be important in immune function. Some of the associated SNP markers were specific for one common subtype of lymphoma, e.g. follicular lymphoma. However, others were associated with combined subsets of disease, suggesting that there are both shared and subtype-specific associations between common genetic variants and human lymphoid cancer. Secondary analyses showed that the two novel regions harbor candidates that are biologically relevant and that regulate cell development and hematopoiesis.

e15022 Background: A marker of response to Bacillus Calmette-Guérin (BCG) intravesical therapy in high-risk non-muscle invasive bladder cancer (NMIBC) could help avoid unsuccessful BCG treatment and potentially enhance care. This study examines germline single nucleotide polymorphisms (SNPs) as markers of BCG-refractory NMIBC. Methods: Saliva or blood was prospectively collected from patients treated with BCG for NMIBC between 1991 and 2010, treated either at MSKCC or referring centers prior to MSKCC registration. BCG-refractory disease was defined as the presence of pathologically documented tumor in the bladder six months after BCG administration. SNPs were selected based on either reported associations with BCG response or putative mechanisms of BCG activity. Eighty candidate SNPs were genotyped (using the Sequenom Mass ARRAY iPLEX platform) in a discovery set (n=158) and those significantly associated were analyzed in a validation set (n=168). Univariate logistic regression was used to test the association between refractory disease, selected clinical variables, and SNPs. Because center of treatment was found to be a confounding variable (p=0.02), primary analysis was limited to cases treated at a single center (MSKCC), consisting of 276 patients (108 from discovery set and 168 from validation set). Results: Median age of diagnosis of the 276 cases was 65.7 years, 94% diagnosed with high-grade superficial bladder cancer, 35% stage Ta, 30% stage T1, and 35% stage Tis. At six month follow-up, 37 (13.4%) had BCG–refractory disease. Nine SNPs showed significant associations with outcome in the discovery cohort (two SNPs failed genotyping). In primary analysis, One SNP, rs11615, an intronic variant within the ERCC1 gene, was associated with BCG-refractory disease. Patients with CC genotype were 3.6 times more likely to be refractory to BCG compared to TT genotype (OR for each C allele was 1.8: 95% CI 1.1-3; p=0.01). No other factors were found to be predictive. Conclusions: A single nucleotide polymorphism of ERCC1 may be associated with BCG-refractory disease. Validation in independent datasets is needed.

Methods: Saliva or blood was collected from pts with NMIBC treated at a single center and diagnosed between 1984 and 2010. SNPs were selected based on reported associations with bladder cancer and BCG response, and genotyped using the Sequenom MassARRAY iPLEX system. No response to BCG was defined as the presence at 6 months of pathologically documented tumor in the bladder. Univariate logistic regression was used to test the association between the outcome of interest (no response to BCG at 6 months) and clinical variables (stage, grade and multifocality), or individual SNPs. Results: The cohort consisted of 158 pts with a median age of 65 years who received intravesical BCG for NMIBC (35.2% stage T1, 32.7% stage Ta and 31.4% stage Tis) with 93% having high-grade disease. At 6 months follow up, 22 (13.9%) patients showed no response to BCG. We successfully genotyped 80 of the 88 selected SNPs. Of these, 2 SNPs were associated with lack of response to BCG; rs798766 is an intronic SNP in TACC3 (OR for no response is 2.4 for each T allele relative to CC genotype, P=0.01), and rs1994251 an intronic SNP in BCL2L1 (OR for no response is 3.2 for each C allele relative to AA, p= 0.0008). Both SNPs remained significantly associated with lack of response after adjusting for predictive clinical variables. Conclusions: Single nucleotide polymorphisms of BCL2L1 and TACC3 may be predictive of BCG refractory bladder cancer. Future validation studies on independent datasets are needed to determine the clinical utility of these findings.

236 Background: Platinum-based therapy improves outcome in neoadjuvant and metastatic UC. Since cell-line and human subject studies in other cancers show germline associations with platinum response, we hypothesized that germline variation also identifies genes that determine UC response to platinum-based therapy. Methods: Saliva or blood was prospectively collected from 651 UC patients (pts) diagnosed between 1984 and 2010. SNPs (n=80) were selected based on previously reported associations with UC and/or platinum response and genotyped using Sequenom MassArray iPLEX system. Samples and SNPs were filtered for genotyping rate ( Results: Sixty-nine pts received neoadjuvant therapy (RES=54%), 126 chemo-naive pts received first-line metastatic treatment (RES=37%) and 4 received systemic treatment after prior chemoradiotherapy (RES=25%). We expected 4 associations due to chance alone at p < 0.05. However, on univariate analysis using a 2 degree of freedom test, 9 SNPs were associated (p < 0.05) with RES. Individual odds ratios (ORs) ranged from 0.32 to 3.29 with p-values ranging from 0.006 to 0.05. In analysis of rs9344 (CCND1) in all treated pts, 75/148 (51%) responded with AG/GG compared to 12/44 (27%) with AA [p=0.005312, OR 2.74 (CI 1.34, 5.92)]. In neoadjuvant pts treated with cisplatin, rs9344 analysis showed 29/48 (60%) responses in AG/GG pts compared to 5/17 (29%) with AA [p=0.026, OR 3.66 (CI 1.16, 13.1)]. Conclusions: We identified associations between 9 SNPs and response of UC to platinum-based therapy that are biologically and clinically relevant. A larger study will be required to independently validate these findings. Additional investigation of associations with time to metastasis and overall survival is ongoing. [Table: see text]
