The novel coronavirus epidemic is transforming the world\ud in which we live. This pandemic will bring sweeping changes everywhere, not least in the field of primary care medicine. Like one of our colleagues said: ‘after this crisis, perhaps even our calendar needs to be redefined. From now on, “BC” might stand for “Before Coronavirus”’. This quote puts into perspective just how significant the current times are for our profession. In this editorial, we will discuss challenges and tasks the COVID-19 crisis presents for family medicine.

Huisarts Wet 2017;60(4):152-4.Achtergrond Om de diagnose ‘longembolie’ uit te sluiten adviseert de NHG-Standaard een diagnostisch predictiemodel te gebruiken, zoals de Wells-beslisregel, in combinatie met een D-dimeerbepaling. Veel huisartsen gebruiken echter geregeld hun eigen impliciete kansschatting. Wij onderzochten of longembolie bij eerstelijnspatiënten met de klinische blik even veilig en efficiëntkan worden uitgesloten als met de Wells-beslisregel.Methode In een cohort van 598 patiënten met mogelijk een longembolie gaf de huisarts de eigen klinische inschatting aan op een schaal van 0-100%, met daarnaast de kans volgens de Wells-beslisregel. Alle patiënten werden vervolgens verwezen naar het ziekenhuis voor een referentietest. Wij deelden de patiënten in in laag- en hoogrisicogroepen op basis van beide strategieën. Als afkapwaarden kozen we een score < 20% voor de klinische blik respectievelijkeen score ≤ 4 voor de Wells-beslisregel, beide in combinatie met een negatieve D-dimeerbepaling. Wij vergeleken de efficientie en de veiligheid van beide strategieen.Resultaten Van de 598 deelnemers hadden er 73 (12%) een veneuzetrombo-embolie. Op basis van klinische blik werden 152 patientenals laag risico geclassificeerd (efficientie 25%; 95%-BI 22-29). Van deze 152 bleken er twee alsnog een longembolie te hebben(veiligheid 1,3%; 95%-BI 0,2-4,7). Met de Wells-beslisregel warden 272 patienten als laag risico geclassificeerd (efficientie 45%; 95%-BI 41-50), van wie er vier alsnog een longembolie hadden (veiligheid 1,5%; 95%-BI 0,4-3,7).Conclusie Om een longembolie veilig uit te sluiten, is de Wellsbeslisregeleven veilig als de klinische blik van de huisarts, en aanzienlijk efficiënter.

Hendriksen JM, Geersing GJ, Lucassen WA, Erkens PM, Stoffers HE, vanWeert HC, Buller HR, Hoes AW, Moons KG. Een bruikbare klinische beslisregelvoor longembolie. Huisarts Wet 2016;59(5):200-3. Om overdiagnostiek te voorkomen, is in de huisartsenpraktijk behoefte aan een bruikbare, efficiente en veilige beslisregel om longembolie bij een patient met klachten uit te sluiten. Wij valideerden in de eerste lijn een aantal klinische beslisregels die gebruikt worden in de tweede en derde lijn. In een systematisch literatuuronderzoek zochten wij naar klinische beslisregels voor longembolie. De beslisregels die bruikbaar leken voor de huisarts, valideerden we in een cohort van 598 deelnemers aan het AMUSE-2-onderzoek, allen met vermoede longembolie, uit 300 Nederlandse huisartsenpraktijken in 2007-2010. Van iedere beslisregel bepaalden we de efficientie, dat wil zeggen het percentage laagrisicopatienten dat de beslisregel identificeerde, en de veiligheid, dat wil zeggen het percentage patienten met een longembolie dat de beslisregel ten onrechte als laagrisicopatient categoriseerde. In de literatuur vonden wij tien klinische beslisregels voor longembolie, waarvan er vijf gevalideerd konden worden in onze dataset: de originele, gemodificeerde en gesimplificeerde Wells-beslisregel, de gereviseerde Geneva-score en de gesimplificeerde gereviseerde Geneva-score. Alle beslisregels sloten, in combinatie met een D-dimeersneltest, longembolie uit bij 43-48% van de patienten, het percentage foutnegatieven lag tussen 1,2% (95%-BI 0,2-3,3) voor de gesimplificeerde Wells-beslisregel en 3,1% (95%-BI 1,4-5,9) voor de gesimplificeerde gereviseerde Geneva-score. De vijf klinische beslisregels die wij valideerden voor de huisartsenpraktijk waren alle ongeveer even efficient. Het veiligst zijn de drie Wells-beslisregels. Met deze beslisregels, in combinatie met een D-dimeersneltest, kan de huisarts bij ongeveer 4 op de 10 patienten longembolie uitsluiten met een percentage foutnegatieven van nog geen 2%, hetgeen naar omstandigheden acceptabel is.

De huisarts kan zelf diepe veneuze trombose veilig uitsluiten. Huisarts Wet 2009;52(10):497-502. Achtergrond Tot 90% van de patienten die de huisarts instuurt voor compressie-echografie onder verdenking van diepe veneuze trombose (DVT) blijkt die aandoening niet te hebben. In het AMUSE-1-onderzoek (Amsterdam-Maastricht-Utrecht Study on thrombo-Embolism) is onderzocht of een eenvoudige klinische beslisregel in combinatie met een D-dimeertest in staat is om een trombosebeen al in de eerste lijn op veilige en efficiente wijze uit te sluiten. Methode Driehonderd huisartsen deden mee aan het onderzoek. Patienten bij wie de beslisregel op een lage klinische verdenking wees, stuurden zij niet in voor echografie en behandelden zij niet met antistolling. Patienten die positief scoorden op de beslisregel verwezen zij voor compressie-echografie. Alle patienten volgden wij drie maanden lang om eventuele trombo-embolische complicaties te kunnen vaststellen. Resultaten Uiteindelijk leverde de beslisregel bij 1002 patienten een geldige score op. Bij 500 patienten wees de beslisregel op een lage klinische verdenking (score ≤ 3). Zeven van hen (1,4%, 95%-BI 0,6-2,9%) kregen een trombo-embolische complicatie tijdens de drie maanden follow-up. De overige 502 patienten hadden een score ≥ 4 en werden verwezen voor echografie. Het echogram wees bij 125 (25%) patienten op een DVT. Van de 374 patienten met een normale echo kregen er 4 (1,1%, 95% BI 0,3-2,7%) een trombo-embolische complicatie tijdens de follow-upperiode. Bij 3 patienten werd geen echo verricht. Discussie Ons onderzoek bevestigt dat de diagnostische strategie die de NHG-Standaard Diepe veneuze trombose aanbeveelt, in de dagelijkse praktijk verantwoord is. Een werkwijze waarbij de huisarts een eenvoudige beslisregel combineert met een D-dimeertest is veilig om DVT uit te sluiten in de eigen praktijk of bij de patient thuis. Met deze strategie kan de helft van de verwijzingen voor compressie-echografie worden voorkomen.

The incidence and prevalence of dyspnea increases with age. Frequently, for the general practitioner with his limited diagnostic facilities, it is impossible to separate dyspnea from cardiac causes and non-cardiac causes. Without cardiac imaging it is also impossible to separate systolic dysfunction from diastolic dysfunction. After a thorough physical examination, initial screening of systolic and diastolic heart failure can be done by measurement of plasma NT-pro BNP or plasma BNP. Additionally a Chest X-Ray or ECG can be performed. To improve diagnostic performance an open access echocardiographic service can be initiated. Recent studies showed, that open access echocardiography can easily detect systolic and diastolic dysfunction in the community and can separate cardiac from non-cardiac dyspnea.

Hart- en vaatziekten zijn de meest voorkomende doodsoorzaak in de westerse wereld. De belangrijkste oorzaak van deze aandoeningen is atherosclerose (slagaderverkalking). Atherosclerose kan zich manifesteren in de hersenen als een TIA of CVA, in het hart als angina pectoris of als een hartinfarct, in de nieren door nierfalen of in de benen als perifeer arterieel vaatlijden (PAV). Dit artikel gaat over perifeer arterieel vaatlijden, een doorbloedingsprobleem in de benen als gevolg van slagaderverkalking. U krijgt een overzicht van de huidige kennis: het ontstaan van atherosclerose en PAV, de risicofactoren, epidemiologie en prognose, preventie en behandeling. De enkel-armindexmeting (doppleronderzoek) komt uitgebreid aan bod, evenals de valkuilen bij deze meetmethode.

Contains fulltext : 71514.pdf (Publisher’s version ) (Closed access) OBJECTIVE: Assessment of patients' responsiveness to a decision support tool for primary prevention of cardiovascular diseases (CVDs). The booklet focuses on barriers at patient level. METHODS: Process evaluation of an intervention in primary care. Patients at high or potentially high-cardiovascular risk were asked by their GP to prepare themselves for a second consultation in order to participate in decisions on risk management. OUTCOMES: Patients' actually having read the booklet and returning for the second consultation; comprehension and perceived relevance of the information; perceived reassurance. RESULTS: 17 GPs, in the intervention arm of a cluster RCT, issued 276 decision support tools during the first consultation and were instructed to ask them to return for a second consultation to discuss their CVD risk. Patients had a mean age of 54 years, 47% were male and 19% actually had a high cardiovascular risk. Data on 239 patients, a mixture of returnees and non-returnees, showed that they all read the booklet; comprehension was fair to good; 85% perceived the information as relevant; 68% of the patients felt reassured by the information. Satisfaction with the first consultation was higher in the non-returnees. CONCLUSIONS: Cardiovascular prevention spread over two consultations with use of a decision support tool for patients is not easily applicable for GPs. However, based on the findings of good patients' responsiveness, we recommend further development and implementation of decision support tools in primary care. PRACTICE IMPLICATIONS: Decision support for primary CV-prevention is welcomed by patients but needs further adjustment of both the GP and the organization of CV-prevention in primary care. Sharing information between professional and patient on a personal CV-risk management plan is difficult, more training is needed.

Jelle Stoffers is als vertegenwoordiger van Nederland vast bezoeker van deze congressen. Hij zal daarvan steeds verslag doen in het NHG-nieuws. Onderstaand geeft hij een terugblik op de congressen van afgelopen jaar.

We reviewed the literature for the association between superficial vein thrombosis (SVT) and venous thromboembolism (VTE) and for evidence of treatment of SVT with low-molecular-weight heparin (LMWH). There is some evidence for an association between SVT and VTE. This association seems stronger for proximal SVT as compared with distal SVT. In general practice, the absolute risk of VTE with or after SVT is low, approximately 3%. There is evidence that treatment of SVT with LMWH may have a beneficial effect on its course. NSAIDs have a similar effect. There is indirect evidence that LMWH is effective in the prevention of VTE, if the treatment is continued for more than 4 weeks. In case of a proximal SVT, we suggest an ultrasound examination be carried out and LMWH treatment given if the diagnosis is confirmed; in other cases, NSAIDs can be considered.

To investigate the opinion of general practitioners on reflective testing, i.e. the practice of additional tests being performed and comments added to the results by laboratory staff when appropriate.Descriptive.In the eastern South Limburg region 155 general practitioners received a list of 10 fictitious patient cases, each involving the possible addition of a specific test. The general practitioners could choose what they preferred the laboratory to do: add tests, phone the general practitioner, add a written comment or do nothing. In addition the general practitioners were asked to judge the effect of additional tests and comments on patient management with respect to diagnosis, treatment and referral, using 200 laboratory reports from their own patients.The response to the fictitious cases was 45%. Most general practitioners favoured the laboratory taking the initiative by adding on tests and commenting on the results in the given clinical scenarios. 78% of the questionnaires accompanying the lab reports were returned by 87% of the general practitioners. In nearly all cases (99%) the service was marked as useful. In more than half of the cases (53%) reflective testing affected the measures taken by the general practitioners.Reflective testing was in general welcomed by the general practitioners. In the majority of cases this led to an improvement in the diagnosis or adjustment of treatment.

In the present paper, we explored the role of patients in the feasibility of cardiovascular preventive care in general practice. Patient-related barriers were examined by means of a qualitative study. Fifteen GPs audio-taped one or two consultations on primary cardiovascular preventive care. The tapes were used to guide the subsequent semi-structured in-depth interviews with patients. Twenty-two patients were interviewed. Patients' understanding of prevention of cardiovascular disease (CVD) was often insufficient. The risk table and the multi-factorial approach were difficult to understand. Risk perception was often unrealistic and dichotomous, and mainly based on personal experiences. There was a demand for more information and cholesterol tests. At the patient level, many barriers impede effective prevention of cardiovascular diseases. In particular, the highly individualized high-risk approach needs to be explained to patients. Educational patient materials, intended to support both the GP and the patient, should take into account the ideas, fears and expectations of patients.

Het najaarscongres 2004 van het European General Practice Research Network (EGPRN) werd gehouden in Gozo (Malta), met als thema ‘Research using electronic patient records’. De 55 deelnemers (onder wie vier Nederlanders) gingen gedurende twee dagen met elkaar in discussie over 41 presentaties. Pre-conference workshops over het gebruik van de ICPC, respectievelijk wetenschappelijke gegevensverzameling met behulp van het elektronisch medisch dossier (EMD) gingen aan het eigenlijke congres vooraf.
