Abstract

Abstract Analysis of malaria imported into eight European countries from the Indian sub-continent (ISC) (India, Pakistan, Bangladesh and Sri Lanka) led to a consensus statement on the use of chemoprophylaxis within TropNetEurop. The proportion of cases from the ISC in 2004 ranged from 1.4%–4.6% of total imported cases. Plasmodium falciparum cases reported from the eight countries was only 23 (13% of all cases from the region). Total malaria reports between 1999–2004 fell from 317 to 180. The risk of malaria in UK residents visiting the region was > 1 case per 1,000 years exposed. The group recommended non-selective prescribing of chemoprophylaxis for visitors to India, Pakistan, Bangladesh and Sri Lanka should be dropped.

Abstract

Abstract Background Plasmodium vivax is the second most common species among malaria patients diagnosed in Europe, but epidemiological and clinical data on imported P. vivax malaria are limited. The TropNetEurop surveillance network has monitored the importation of vivax malaria into Europe since 1999. Objectives To present epidemiological and clinical data on imported P. vivax malaria collected at European level. Material and methods Data of primary cases of P. vivax malaria reported between January 1999 and September 2003 were analysed, focusing on disease frequency, patient characteristics, place of infection, course of disease, treatment and differences between network-member countries. Results Within the surveillance period 4,801 cases of imported malaria were reported. 618 (12.9%) were attributed to P. vivax. European travellers and immigrants were the largest patient groups, but their proportion varied among the reporting countries. The main regions of infection in descending order were the Indian subcontinent, Indonesia, South America and Western and Eastern Africa, as a group accounting for more than 60% of the cases. Regular use of malaria chemoprophylaxis was reported by 118 patients. With 86 (inter-quartile range 41–158) versus 31 days (inter-quartile range 4–133) the median symptom onset was significantly delayed in patients with chemoprophylaxis (p < 0.0001). Common complaints were fever, headache, fatigue, and musculo-skeletal symptoms. All patients survived and severe clinical complications were rare. Hospitalization was provided for 60% and primaquine treatment administered to 83.8% of the patients, but frequencies varied strongly among reporting countries. Conclusions TropNetEurop data can contribute to the harmonization of European treatment policies.

Abstract

The use of cognitive reappraisal (CR) has been linked with improved emotional wellbeing in populations with a history of traumatic stress. Whilst research suggests that the extent to which individuals master CR (CR ability) moderates the relationship between depression and stress, studies have not attempted to improve CR ability in individuals experiencing stress due to everyday events or test for potential health benefits of this intervention. Past experimental studies using CR have largely employed group designs in which the effects of intervention are averaged across participants, leading to potentially valuable information being disguised. To this end, this study employed a single case experimental design to investigate the impact of repeated use of CR on affect, perceived stress, and depression in a female adult sample with high stress. The study also included an aspect of helping behaviour in the intervention to investigate whether there are additive benefits to using CR for self and to help others compared to using CR for oneself only. Twelve adult females were recruited from the community (university staff and students) to take part in the study lasting 21 days. Daily measures were collected over the course of the study and pre-post study measures were taken at baseline, CR intervention, and follow-up stages. At the beginning of the intervention phase, participants were randomly allocated to one of two groups. Group 1 completed 10 days of the daily CR Task whereby daily stressors were described in writing, reappraised and then described again giving the event a newly acquired meaning. Group 2 completed five days of the CR Task followed by five days of using CR to help reappraise written accounts of daily stressful events written by others (CR Helping Task). At the group level, using randomisation tests, no significant improvements in emotional affect and daily stress were found in response to the intervention. At the individual level, using the reliable change index, depressive symptoms decreased reliably in three out of five participants for whom a decrease was possible and for whom CR Ability increased with the intervention. Perceived stress decreased reliably in five out of 10 participants for whom CR Ability increased. Finally, changes in depressive symptoms did not differ between groups, but, contrary to expectations, perceived stress decreased reliably in a larger number of participants in group 1 compared to group 2. The CR intervention showed promise as a feasible short-term stand-alone intervention and demonstrated the utility of targeting specific aspects within psychological care to clarify mechanisms of change and theory. Further research is needed to explore how to optimise the intervention, particularly in terms of length and the design of the CR Helping Task.

CO-010 Introducción: El número de células tumorales circulantes (CTCs) predice el riesgo de transformación en mieloma múltiple (MM) asintomático, así como la supervivencia en MM activo. Datos recientes sugieren que, conforme el tumor progresa y el microambiente se vuelve más hipóxico, las células plasmáticas (CP) clonales pasan a la circulación sistémica favoreciendo la constante invasión de nuevas regiones en la médula ósea (MO). Habría que señalar que la frecuencia de las CTCs es típicamente baja, por lo que sería concebible que dicha diseminación del MM dependiera de unas pocas células con unas características únicas que inducirían su salida de la médula y la extensión de la enfermedad a través de la sangre periférica (SP). Esta hipótesis no ha sido demostrada hasta ahora debido a que el perfil transcriptómico de la CTC en el MM no ha sido investigado. 27. Real-world effectiveness and safety of ixazomib-lenalidomide-dexamethasone in relapsed/refractory multiple myeloma

Abstract

Real-world data on regimens for relapsed/refractory multiple myeloma (RRMM) represent an important component of therapeutic decision-making. This multi-centric, retrospective, observational study conducted by the treating physicians evaluated the effectiveness and safety of ixazomib-lenalidomide-dexamethasone (IRd) in 155 patients who received ixazomib via early access programs in Greece, the UK, and the Czech Republic. Median age was 68 years; 17% had an Eastern Cooperative Oncology Group performance status ≥ 2; median number of prior therapies was 1 (range 1–7); 91%, 47%, and 17% had received prior bortezomib, thalidomide, and lenalidomide, respectively. Median duration of exposure to ixazomib was 9.6 months. Overall response rate was 74%, including 35% very good partial response or better (16% complete response). Median progression-free survival (PFS) was 27.6 months (27.6 and 19.9 months in patients with 1 or > 1 prior lines, respectively). IRd treatment for ≥ 6 months was associated with longer PFS (hazard ratio 0.06). Fourteen patients (9%) discontinued IRd due to adverse events/toxicity in the absence of disease progression. Peripheral neuropathy was reported in 35% of patients (3% grades 3–4). These findings support the results of the phase III TOURMALINE-MM1 trial in a broader real-world RRMM population. © 2020, Springer-Verlag GmbH Germany, part of Springer Nature.
