21. Low prevalence of tissue detection of cefepime and daptomycin used as empirical treatment during revision for periprosthetic joint infections: results of a prospective multicenter study.

Abstract

Data demonstrating that antibiotics administered intraoperatively in patients with surgical revision for periprosthetic joint infection achieve concentrations exceeding minimal inhibitory concentrations of the identified bacteria at the surgical site when the new implant is inserted are lacking. We prospectively included patients with periprosthetic joint infection operated with one- or two-stage replacement during which cefepime (2g)-daptomycin (10mg/kg) combination was administered intravenously as intraoperative empirical antibiotic treatment. Three biopsies (two bones and one synovial membrane) were taken from each patient just before the insertion of the new implant. Eighteen adults of median age 68 years were included. Knee was involved in 10 patients (55.6%) and surgery consisted in one-/two-stage replacement in 11/7 patients. A tourniquet was used during the intervention in the 10 patients with knee prosthesis. Among 54 tissue samples, cefepime and daptomycin were detected respectively in 35 (64.8%) and 21 (38.9%) cases (P=0.01). A total of 17 bacteria dominated by staphylococci (n=14) were identified in 10 patients; tissue inhibitory quotient calculated in 51 samples was >1 in 22 cases (43.1%) for cefepime and in 16 cases (31.4%) for daptomycin. The proportion of tissue samples with detectable antibiotic was significantly higher in hip versus knee prosthesis (P=0.03). The present study suggests that intraoperative empirical administration of cefepime-daptomycin combination during septic prosthetic joint replacement results in a high proportion of tissue samples in which at least one of the two antibiotics was not detected or at a low concentration despite satisfactory concomitant blood serum concentrations. 37. Group A2 Analogs of Innovator Teicoplanin: Variability of their Ratio during Treatment Time Course in Human Blood; Consequences for the Generics

Abstract

Absence of therapeutic equivalence (TE) of generic antibiotics versus the innovator has been reported, even in case of pharmaceutical equivalence (PE).The pharmaceutically active principle of teicoplanin is mainly represented by the A2 group analogs (A2-1 to A2-2) which exist in given ratios in the innovator. We studied the evolution of these ratios in humans volunteers receiving 2 dosage regimen (6mg/kg IV every 12 hours, 3 times, and then 6 mg/kg per day for ten days - group1-, and 12 mg/kg IV every 12h, 3 times, followed by 15 mg/kg every other day till D11 - group 2-), by a specific HPLC methodology, after single dose (D1) and at steady state at day 11(D11). The less lipophilic analogs A2-1, A2-2, and A2-3 percentage decreases respectively were 18%, 11%, and 21% (group 1) and 34%, 14%, and 15% (group 2) between D1 and D11 for trough concentrations. At the same time, the more lipophilic A2-4 and A2-5 analogs ratio increases respectively were 15% and 19% (group 1) and 13% and 30% (group 2). These variations raise the problem of generics having the same global qualitative composition of analogs (PE) but with differing initial ratios. The bactericidal activity of innovator teicoplanin is optimal at its own ratios of the different analogs that act in a synergistic manner. Regarding generics, it would lead to final ratio in blood and tissues that are at risk of a sub optimal bactericidal activity. Thus, concluding in bioequivalence and therapeutic equivalence of these generics with the innovator remains highly questionable.

Abstract

The bioavailability of the recently developed 1 g dispersible tablet form of amoxicillin (B) and the 1 g dispersible tablet in suspension form (C) were compared to that of the 1 g standard reference formulation (A). Twelve healthy volunteers were involved in this single-dose, open, randomized, three-way cross-over study. The mean peak serum levels were 14.1 +/- 4.1 micrograms/ml after A, 15.1 +/- 3.1 micrograms/ml after B and 15.1 +/- 5.4 micrograms/ml after C. The area under the drug concentration versus time curves were 47.6 +/- 12.0 micrograms.h/ml after A, 52.8 +/- 10.2 micrograms.h/ml after B and 51.1 +/- 13.8 micrograms.h/ml after C. On the basis of these two pharmacokinetic parameters, the three formulations were found to be bioequivalent. In addition, the predicted serum concentrations during multiple dosing (3 times a day), derived from the corresponding mean concentrations after a single 1 g dose of C showed that 8 hourly administration would yield therapeutic serum concentrations for infections such as uncomplicated community-acquired pneumonia due to susceptible or less susceptible strains in otherwise healthy subjects.

46. Staphylococcus aureusméticillino-résistants : concentrations critiques et pharmacodynamie de la vancomycine et des nouvelles céphalosporines

Abstract

Les données récentes de pharmacocinétique/pharmacodynamie (PK/PD) des antibiotiques ont considérablement influencé la révision des concentrations critiques cliniques des anciennes molécules et l’établissement de celles des molécules nouvellement disponibles. L’exemple de la vancomycine, dont le paramètre clef de l’efficacité bactérioclinique est le rapport aire sous la courbe (24h)/concentration minimale inhibitrice (ASC/CMI), est éloquent. La valeur cible de ce paramètre est 400 (prédictive de l’efficacité) et 600 (préventive du passage de staphylocoques hVISA à VISA). Or les concentrations sériques résiduelles, ou le plateau lors de perfusions continues, nécessaires à l’obtention d’une ASC suffisante lorsque les CMI sont supérieures ou égales 1mg/L sont souvent rapportées comme synonymes de néphrotoxicité avérée ou potentielle. Le problème de l’abaissement de la concentration critique clinique de la vancomycine est donc posé. Les céphalosporines dites de « 5egénération », ceftaroline et ceftobiprole, très souvent actives sur les Staphylococus aureusrésistants à la méticilline (SARM), représentent une alternative potentiellement intéressante, pour peu que les posologies soient en adéquation avec les prérequis PK/PD, les probabilités d’atteindre ces prérequis (probabilité d’atteindre la cible) en fonction des CMI et plus particulièrement du break-pointclinique. C’est clairement le cas du ceftobiprole, ce devrait l’être avec la ceftaroline à la faveur d’une augmentation prochaine des posologies de 600mg×2 en perfusion d’1 heure à 600mg×3 en perfusion de 2heures.
