Surgical correction of abdominal muscle diastasis may decrease intra-abdominal volume and increase intra-abdominal pressure. The induced changes may ultimately lead to respiratory compromise. In abdominoplasty, one of the most frequently performed esthetic procedures, those changes are believed to be transient and clinically insignificant. We describe a case where acute change in respiratory physiology after abdominoplasty led to severe respiratory failure with significantly decreased pulmonary compliance in a young and otherwise healthy patient. In this case mechanical ventilation failed to improve compliance, and reversal of abdominoplasty was required to restitute pulmonary function.

Familial hypocalciuric hypercalcemia (FHH) is considered a relatively benign condition characterized by mild elevations in serum calcium and relatively low urinary calcium excretion. It results from an elevated set point in serum calcium arising from variants in the calcium-sensing receptor (CaSR) gene but also AP2S1 and GNA11 genes, which encode for adaptor-related protein complex 2 and G11 proteins, respectively. The manifestations of FHH can vary and sometimes overlap with primary hyperparathyroidism making the diagnosis challenging. Case Presentations. We report a mother and daughter with a novel heterozygous variant in the CaSR gene resulting in a serine to leucine substitution at position 147 (S147L) of the CaSR. Both patients had mild hypercalcemia, relatively low urinary calcium excretion, elevated calcitriol, and low-to-normal intact PTH. The proband (daughter) presented with symptoms associated with hypercalcemia and was incidentally found to have a bony lesion suspicious for osteitis fibrosa cystica, and she was also diagnosed with sarcoidosis. Subtotal parathyroidectomy revealed normal-weight parathyroid glands comprised of 50–80% parathyroid epithelial cells, which has been documented as within the spectrum of normal. Her mother had no symptoms, and no intervention was pursued. Conclusion. We report a novel variant in the CaSR associated with FHH in two patients with similar biochemical features yet differing clinical manifestations. While the relationship of the bony findings and parathyroid histology with this variant remains unclear, these cases enrich our knowledge of CaSR physiology and provide further examples of how varied the manifestations of FHH can be.

Objectives To describe COVID-19 vaccine coverage (i.e., the estimated percentage of people who have received a vaccine) and determinants of vaccine receipt among individuals with a recent experience of homelessness in Ontario, Canada. Approach We conducted a retrospective, population-based cohort study of 23,247 individuals (≥18 years) with a recent experience of homelessness as recorded in routinely collected healthcare databases. Participants were followed from December 14, 2020 to September 30, 2021 for the receipt of a COVID-19 vaccine. Using modified Poisson regression, we identified sociodemographic, healthcare usage, and clinical factors associated with the receipt of one or more doses of a COVID-19 vaccine. Results By September 30, 14,271 (61.4%) of participants with a recent experience of homelessness had received a first dose of a COVID-19 vaccine and 11,082 (47.7%) had received two doses. Over the same period, 86.6% and 81.6% of the total adult population of Ontario had received a first dose and second dose, respectively. In multivariable analysis, factors associated with increased COVID-19 uptake included ≥1 visit to a general practitioner (adjusted Risk Ratio [aRR]:1.37[95% CI 1.31-1.42]), older age (vs. 18-29 years: 50-59 years, aRR:1.18[1.14-1.22]; 60+ years, aRR:1.27[1.22-1.31]), receipt of an influenza vaccine (aRR:1.25[1.23-1.28]), receipt of ≥1 SARS-CoV-2 test (aRR:1.23[1.20-1.26]) and the presence of chronic health conditions (vs. 0 conditions: 1 condition, aRR:1.05[1.03, 1.08]; 2+ conditions, aRR:1.11[1.08-1.14]). In contrast, living in a smaller metropolitan region (aRR:0.92[0.90-0.94]) or rural location (aRR:0.93[0.90-0.97]) compared to a large metropolitan region was associated with lower uptake. Conclusions As of September 30, 2021, COVID-19 vaccine coverage among individuals with a recent experience of homelessness in Ontario was substantially lower than the general adult population of Ontario for a first and second dose. Findings underscore the importance of leveraging organizations that are accessed and trusted by people who experience homelessness for targeted vaccine delivery.

Introduction Due to the ever-growing volume and complexity of clinical data, it has become a tedious task to extract information from data for secondary uses such as decision support, quality assurance, and outcome analysis. Recently, there have been great advances in Natural Language Processing (NLP) approaches that automate knowledge extraction from clinical reports in order to save costs and improve efficiency. Objectives/Approach​ Our goal is the development of an NLP tool designed to automatically extract and encode clinical information from laboratory reports. This study describes and evaluates our NLP tool on provincial repositories of laboratory tests and results called Ontario Laboratory Information System (OLIS). OLIS is an electronic system that covers >200 labs and stores patients’ current and past test results as patients move through different areas of the healthcare system. Our NLP tool is a modular system of pipelined components including Named Entity Recognition module for extracting mentions of virus and test mentions and inference to combine extracted entities into a meaningful outcome. Results ​Initial analyses were conducted on a segment of OLIS related to laboratory tests for respiratory viruses. This data included over a million observations corresponding to ~100 Logical Observation Identifiers Names and Codes (LOINC), with >40,000 unique strings. The clinical text was cleaned, tokenized, and parsed using an in-house text algorithm that was continually refined with manual review from clinical experts. This data was then encoded as virus and test types to be used as a ground truth. The NLP tool was built on ground truth data and achieved an accuracy greater than 95%. Conclusion/Implications Approaches like these can be applied to many areas of health research that make use of clinical reports. Our methods, when optimized and validated, can be deployed into clinical systems to provide on-the-spot analysis of various laboratory reports.

Introduction Respiratory syncytial virus (RSV) is the leading cause of hospitalization among infants globally. Several RSV vaccine candidates are currently in trial and the severity of RSV-related illness can be reduced with prophylaxis. To optimize delivery of these programs and reduce inequities, a comprehensive understanding of risk factors for severe RSV-related illness is required. Objectives and Approach The objective of this study was to quantify the risks of severe, early life RSV-related illness in terms of medical conditions, birth characteristics and novel socio-economic factors. We used linked population-based health and socio-demographic administrative data for all children born in Ontario (Apr 1st, 2012-March 31st, 2018), including laboratory viral testing data for a subset of children, to identify all RSV-related hospitalizations occurring in Ontario before a child’s third birthday or end of follow-up (March 31st, 2019). We calculated the relative risk of RSV-related admission, adjusted for several medical complexity and transmission factors. Critically, we leveraged these routinely collected health administrative data to determine the admission risks associated with various novel measures indicative of maternal social vulnerability, such as homelessness and involvement with the criminal justice system. Results 11,279 RSV-related hospitalizations were identified among 789,484 children; 57% of admissions occurred before 6 months of age. T-cell receptor excision circle levels are a surrogate marker of T-cell production and immune system function. We sought to determine whether non-pathological levels of infant T-cell receptor excision circles were associated with adverse events following immunization. A self-controlled case series design was applied on a sample of 231,693 children who completed newborn screening for severe combined immunodeficiency in Ontario, Canada between August 2013 and December 2015. Exposures included routinely administered pediatric vaccines up to 15 months of age. Main outcomes were combined health services utilization for recognized adverse events following immunization. 1,406,981 vaccination events were included in the final dataset. 103,007 children received the Pneu-C-13 or Men-C-C vaccine and 97,998 received the MMR vaccine at 12 months of age. 67,725 children received the varicella immunization at 15 months. Our analysis identified no association between newborn T-cell receptor excision circle levels and subsequent health services utilization events following DTa-IPV-Hib, Pneu-C-13, and Men-C-C vaccinations at 2-month (RI 0.94[95%CI 0.87-1.02]), 4-month (RI 0.82[95%CI 0.75-0.9]), 6-month (RI 0.63[95%CI 0.57-0.7]) and 12-month (RI 0.49[95%CI 0.44-0.55]). We also found no trends in health services utilization following MMR (RI 1.43[95%1.34-1.52]) or varicella (RI 1.14[95%CI 1.05-1.23]) vaccination. Our findings provide further support for the safety of pediatric vaccinations.

While combination antiretroviral therapy (cART) has significantly improved survival times for persons diagnosed with HIV, estimation of life expectancy (LE) for this cohort remains a challenge, as mortality rates are a function of both time since diagnosis and age, and mortality rates for the oldest age groups may not be available.A validated case-finding algorithm for HIV was used to update the cohort of HIV-positive adults who had entered care in Ontario, Canada as of 2012. The Chiang II abridged life table algorithm was modified to use mortality rates stratified by time since entering the cohort and to include various methods for extrapolation of the excess HIV mortality rates to older age groups.As of 2012, there were approximately 15,000 adults in care for HIV in Ontario. The crude all-cause mortality rate declined from 2.6% (95%CI 2.3, 2.9) per year in 2000 to 1.3% (1.2, 1.5) in 2012. Mortality rates were elevated for the first year of care compared to subsequent years (rate ratio of 2.6 (95% CI 2.3, 3.1)). LE for a 20-year old living in Ontario was 62 years (expected age at death is 82), while LE for a 20-year old with HIV was estimated to be reduced to 47 years, for a loss of 15 years of life. Ignoring the higher mortality rates among new cases introduced a modest bias of 1.5 additional years of life lost. In comparison, using 55+ as the open-ended age group was a major source of bias, adding 11 years to the calculated LE.Use of age limits less than the expected age at death for the open-ended age group significantly overstates the estimated LE and is not recommended. The Chiang II method easily accommodated input of stratified mortality rates and extrapolation of excess mortality rates.

Hepatitis C treatment is rapidly evolving with significant improvements in patient outcomes. With an estimated prevalence of over 3 million persons living with chronic hepatitis C in the United States, it is anticipated that there will be an increase in the number of persons seeking care and treatment for chronic hepatitis C infection. Current systems of care may be overburdened with people seeking care for chronic hepatitis C virus (HCV). Interprofessional models of care have been shown to be feasible and effective in treating different populations affected by chronic HCV. Use of interprofessional teams, integrated models of care, and greater use of nonphysician providers offer a potential solution for expanding capacity to comprehensive HCV treatment and care in the United States.
