1. The genomic landscape of Ménière's disease: a path to endolymphatic hydrops

Abstract: Abstract Background Ménière's disease (MD) is a disorder of the inner ear that causes episodic bouts of severe dizziness, roaring tinnitus, and fluctuating hearing loss. To date, no targeted therapy exists. As such, we have undertaken a large whole genome sequencing study on carefully phenotyped unilateral MD patients with the goal of gene/pathway discovery and a move towards targeted intervention. This study was a retrospective review of patients with a history of Ménière's disease. Genomic DNA, acquired from saliva samples, was purified and subjected to whole genome sequencing. Results Stringent variant calling, performed on 511 samples passing quality checks, followed by gene-based filtering by recurrence and proximity in molecular interaction networks, led to 481 high priority MD genes. These high priority genes, including MPHOSPH8, MYO18A, TRIOBP, OTOGL, TNC, and MYO6, were previously implicated in hearing loss, balance, and cochlear function, and were significantly enriched in common variant studies of hearing loss. Validation in an independent MD cohort confirmed 82 recurrent genes. Pathway analysis pointed to cell–cell adhesion, extracellular matrix, and cellular energy maintenance as key mediators of MD. Furthermore, the MD-prioritized genes were highly expressed in human inner ear hair cells and dark/vestibular cells, and were differentially expressed in a mouse model of hearing loss. Conclusion By enabling the development of model systems that may lead to targeted therapies and MD screening panels, the genes and variants identified in this study will inform diagnosis and treatment of MD.

Abstract: Abstract RIPK1 is a master regulator of inflammatory signaling and cell death and increased RIPK1 activity is observed in human diseases, including Alzheimer’s disease (AD) and amyotrophic lateral sclerosis (ALS). RIPK1 inhibition has been shown to protect against cell death in a range of preclinical cellular and animal models of diseases. SAR443060 (previously DNL747) is a selective, orally bioavailable, central nervous system (CNS)–penetrant, small‐molecule, reversible inhibitor of RIPK1. In three early‐stage clinical trials in healthy subjects and patients with AD or ALS (NCT03757325 and NCT03757351), SAR443060 distributed into the cerebrospinal fluid (CSF) after oral administration and demonstrated robust peripheral target engagement as measured by a reduction in phosphorylation of RIPK1 at serine 166 (pRIPK1) in human peripheral blood mononuclear cells compared to baseline. RIPK1 inhibition was generally safe and well‐tolerated in healthy volunteers and patients with AD or ALS. Taken together, the distribution into the CSF after oral administration, the peripheral proof‐of‐mechanism, and the safety profile of RIPK1 inhibition to date, suggest that therapeutic modulation of RIPK1 in the CNS is possible, conferring potential therapeutic promise for AD and ALS, as well as other neurodegenerative conditions. However, SAR443060 development was discontinued due to long‐term nonclinical toxicology findings, although these nonclinical toxicology signals were not observed in the short duration dosing in any of the three early‐stage clinical trials. The dose‐limiting toxicities observed for SAR443060 preclinically have not been reported for other RIPK1‐inhibitors, suggesting that these toxicities are compound‐specific (related to SAR443060) rather than RIPK1 pathway‐specific.

3. Utility of droplet digital polymerase chain reaction for TERT and BRAF mutational profiling of thyroid nodules

Abstract: Abstract Background Mutations involving BRAF and TERT are important predictors of disease severity in thyroid cancer, but molecular testing is limited by cost and lack of adequate tissue sample. This study aimed to assess the utility of BRAFV600E and TERT testing using droplet digital PCR (ddPCR) as a diagnostic and prognostic tool for thyroid fine needle aspirate biopsy (FNAB). Methods Patients with thyroid nodules were prospectively enrolled from March 2015 to September 2018. Pre-operative FNAB was collected for standard cytology and molecular testing. BRAFV600E and TERT levels were analyzed by ddPCR. Cytology (Bethesda system) and ddPCR results were correlated to surgical pathology. Results A total of 222 patients were enrolled, of which 124 received thyroid surgery. Pre-operative cytology alone with Bethesda ≥5 was 100% specific and 70% sensitive for malignancy on final surgical pathology. BRAFV600E positivity or TERT overexpression was 100% specific and 60.0% sensitive. Combining cytology (Bethesda ≥5) with BRAFV600E and TERT testing increased the sensitivity of a malignant diagnosis to 80.0%. High TERT levels and/or BRAFV600E was associated with aggressive or advanced stage pathology. Conclusions Combining cytology with ddPCR analysis of BRAFV600E and TERT can improve the diagnostic accuracy of thyroid FNAB, and help predict aggressive pathology.

Abstract: Abstract Background The tongue is an essential organ for human interaction, communication and survival. To date, there is a paucity of objective functional, patient reported, or quality of life outcomes of patients undergoing a total glossectomy with preservation of the larynx (TGLP). Objective To examine prospectively collected objective, self‐reported functional and quality of life (QOL) data in patients undergoing TGLP and free flap reconstruction. Methods Sixteen TGLP patients were identified in the prospective head and neck cancer and functional outcomes database between January of 2009 and December 2017. Data collection included patient age, sex, performance status, TNM staging, diagnosis and adjuvant treatment. Swallowing and speech functions were measured and prospectively recorded pre‐ and postoperatively. Patient reported outcomes were measured with the Speech Handicap Index (SHI) and the M.D. Anderson Dysphagia Inventory (MDADI). 5. Telephone consultations with otolaryngology – head and neck surgery reduced emergency visits and specialty consultations in northern Alberta

Abstract: Abstract Background RAAPID (Referral, Access, Advice, Placement, Information, and Destination) is a 24-h call center in Alberta, Canada, facilitating urgent telephone consultations between physicians and specialists. We evaluated the extent to which RAAPID calls to Otolaryngology-Head and Neck Surgery (OHNS) reduced visits to the emergency department and specialty clinics. Methods This was a cross-sectional study evaluating all telephone consultations to OHNS from physicians in northern Alberta between 2013 and 2014 (T1) (where consultations by residents occurred) and 2015 to 2017 (T2) (where consultations were done by consultants during office hours and residents during after hours). Outcomes of the calls included medical advice, specialty clinic referrals, and emergency department (ED) referrals. Differences in the reduction of ED visits and costs, overall as well as in T1 and T2, were assessed using multivariate logistic regression. Results Overall, 62.3% (1064/1709) of telephone consultations reduced ED visits consisting of advice being provided (n = 884; 83.1%) and referral to specialty clinics (n = 180; 16.9%). The adjusted odds ratio of calls reducing emergency visits in T2 as compared to T1 was 2.47 (95% CI 1.99 to 3.08). The adjusted odds ratio of reducing ED visits during office hours compared to after-hours 2.54 (95% CI 1.77–3.64). The estimated direct costs avoided from ED visits in T1 and T2 were $42,224.22 and $114,393.86, respectively. Conclusion RAAPID telephone consultations to OHNS were effective in reducing ED visits and healthcare costs. This model should be considered in other areas to improve efficiencies within the health system.

6. Long-term survival and swallowing outcomes in advanced stage oropharyngeal squamous cell carcinomas

Abstract: Background: There is a paucity of studies reporting long-term survival outcomes for HPV/p16 positive oropharyngeal squamous cell carcinoma (OPSCC). This study aims to compare long-term outcomes of advanced stage p16 positive and negative OPSCCs, treated by surgical and non-surgical modalities. Methods: OPSCC patients from 1998 to 2012 were identified through a prospectively collected cancer registry. P16 immunohistochemistry was used as a surrogate marker for HPV-OPSCC. Overall survival (OS) and aspiration free survival (AFS) comparisons were made between patients treated with chemoradiation (CRT) versus primary surgery and radiation/chemoradiation (S+RT/CRT) at 5, 10 and 15 years post-treatment. Results: A total of 319 patients were included. P16 positive patients and non-smokers had significantly higher long-term (5, 10 and 15-year) OS. Smokers and p16 negative patients treated with S+RT/CRT had improved long-term OS compared to patients who received CRT. Smokers and p16 negative patients had lower long-term AFS. Multivariate analysis showed improved OS was associated with p16 positivity (HR 0.42, 0.28–0.61) and surgery (HR 0.47, 0.32–0.69), whereas lower OS was associated with ECOG ≥ 2 (HR 2.46, 1.61–3.77), smoking (HR 2.37, 1.41–3.99) and higher stage (HR 1.68, 1.05–2.68). Conclusions: In smokers and p16-negative OPSCC patients, primary surgery may be associated with improved long-term survival and dysphagia-related outcomes.

7. Donor site morbidity following radial forearm free flap reconstruction with split thickness skin grafts using negative pressure wound therapy

Abstract: Abstract Background Donor site complications secondary to radial forearm free flap (RFFF) reconstruction can limit recovery. Optimizing hand and wrist function in the post-operative period may allow more efficient self-care and return to activities of daily living. Negative pressure wound dressings (NPD) may increase blood flow and perfusion as compared to static pressure dressings (SPD) designed to minimize shear forces during the healing period. This study aims to compare subjective and objective hand and wrist functional outcomes following RFFF reconstruction with split thickness skin grafts (STSG) in patients treated with NPD and SPD. Methods Adult patients undergoing RFFF with STSG were identified preoperatively and randomized to receive NPD or SPD following their RFFF reconstruction. NPD involved a single-use, portable device capable of applying 80 mmHg of negative pressure to the forearm donor site. SPD involved a volar splint. Dressings were left in place for seven days with subjective and objective function assessed at seven days, one month and three months postoperatively. The primary outcome was self-reported hand function as measured with the function subscale of the Michigan Hand Questionnaire (MHQ). Secondary outcomes included hand and wrist strength, range of motion, sensation, scar aesthetics, and skin graft complications. Results Twenty-four patients undergoing RFFF were randomized to NPD or SPD. Patients treated with NPD had improved MHQ self-reported functional scores as compared to those treated with SPD at seven days postoperatively (P = 0.016). Flexion at seven days was improved in NPD group (P = 0.031); however, all other strength and range of motion outcomes were similar between groups. There were no differences in rates of graft complications, scar aesthetics, or sensation. Conclusions In the immediate post-operative period, NPD was associated with improved patient-reported hand and wrist function. Wound care to optimize hand and wrist function could allow for improved patient outcomes in the immediate postoperative period.

Abstract: Abstract Background To determine the impact of Human Papillomavirus (HPV) status on speech, swallowing, and quality of life (QOL) outcomes after surgical treatment of oropharyngeal cancer (OPSCC). Methods A retrospective review of a prospectively collected database of all patients with OPSCC diagnosed and treated from 1998 to 2009. Speech, swallowing, and quality of life data were gathered at 3 different evaluation points. HPV status was determined using p16 positivity as a surrogate marker. Univariate and multivariate statistical analyses were performed to identify whether p16 status is a significant predictor of functional outcome and QOL. Results One hundred twelve patients with OPSCC and known p16 status were treated with primary surgery between 1998 and 2009, with mean age of 56 years. Out of those patients 63 (56%) were p16 positive. Speech intelligibility remained high at 1-year post operation (95.4%). Only 11.5% of the patients required a feeding tube at 1 year after surgery to maintain their daily caloric requirements and the risk of aspiration after surgery was not significant (p = 0.097). There was no statistically or clinically significant difference in speech, swallowing ability, swallowing safety and QOL outcomes between p16-positive and negative OPSCC. Conclusions Surgically treated OPSCC patients demonstrate excellent swallowing function and can achieve excellent speech perception. P16 status may not be predictive of functional outcomes or QOL in surgically treated OPSCC.

11. Analytic and clinical validity of thyroid nodule mutational profiling using droplet digital polymerase chain reaction

Abstract: Abstract Background Recent guidelines for the management of thyroid nodules incorporate mutation testing as an adjunct for surgical decision-making, however current tests are costly with limited accuracy. Droplet digital PCR (ddPCR) is an ultrasensitive method of nucleic acid detection that is particularly useful for identifying gene mutations. This study aimed to assess the analytic and clinical validity of RAS and BRAF ddPCR mutational testing as a diagnostic tool for thyroid fine needle aspirate biopsy (FNAB). Methods Patients with thyroid nodules meeting indication for FNAB were prospectively enrolled from March 2015 to September 2017. In addition to clinical protocol, an additional FNAB was obtained for ddPCR. Optimized ddPCR probes were used to detect mutations including HRASG12 V, HRASQ61K, HRASQ61R, NRASQ61R, NRASQ61K and BRAFV600E. The diagnostic performance of BRAF and RAS mutations was assessed individually or in combination with Bethesda classification against final surgical pathology. Results A total of 208 patients underwent FNAB and mutational testing with the following Bethesda cytologic classification: 26.9% non-diagnostic, 55.2% benign, 5.3% FLUS/AUS, 2.9% FN/SPN, 2.4% SFM and 7.2% malignant. Adequate RNA was obtained from 91.3% (190) FNABs from which mutations were identified in 21.1% of HRAS, 11.5% of NRAS and 7.4% of BRAF. Malignant cytology or BRAFV600E was 100% specific for malignancy. Combining cytology with ddPCR BRAF600E mutations testing increased the sensitivity of Bethesda classification from 41.7 to 75%. Combined BRAFV600E and Bethesda results had a positive predictive value (PPV) of 100% and negative predictive value (NPV) of 89.7% for thyroid malignancy in our cohort. Conclusions DdPCR offers a novel and ultrasensitive method of detecting RAS and BRAF mutations from thyroid FNABs. BRAFV600E mutation testing by ddPCR may serve as a useful adjunct to increase sensitivity and specificity of thyroid FNAB.

12. Intraoperative Brief Electrical Stimulation of the Spinal Accessory Nerve (BEST SPIN) for prevention of shoulder dysfunction after oncologic neck dissection: a double-blinded, randomized controlled trial

Abstract: Abstract Background Shoulder dysfunction is common after neck dissection for head and neck cancer (HNC). Brief electrical stimulation (BES) is a novel technique that has been shown to enhance neuronal regeneration after nerve injury by modulating the brain-derived neurotrophic growth factor (BDNF) pathways. The objective of this study was to evaluate the effect of BES on postoperative shoulder function following oncologic neck dissection. Methods Adult participants with a new diagnosis of HNC undergoing Level IIb +/− V neck dissection were recruited. Those in the treatment group received intraoperative BES applied to the spinal accessory nerve (SAN) after completion of neck dissection for 60 min of continuous 20 Hz stimulation at 3-5 V of 0.1 msec balanced biphasic pulses, while those in the control group received no stimulation (NS). The primary outcome measured was the Constant-Murley Shoulder (CMS) Score, comparing changes from baseline to 12 months post-neck dissection. Secondary outcomes included the change in the Neck Dissection Impairment Index (ΔNDII) score and the change in compound muscle action potential amplitude (ΔCMAP) over the same period. Results Fifty-four patients were randomized to the treatment or control group with a 1:1 allocation scheme. No differences in demographics, tumor characteristics, or neck dissection types were found between groups. Significantly lower ΔCMS scores were observed in the BES group at 12 months, indicating better preservation of shoulder function (p = 0.007). Only four in the BES group compared to 17 patients in the NS groups saw decreases greater than the minimally important clinical difference (MICD) of the CMS (p = 0.023). However, NDII scores (p = 0.089) and CMAP amplitudes (p = 0.067) between the groups did not reach statistical significance at 12 months. BES participants with Level IIb + V neck dissections had significantly better ΔCMS and ΔCMAP scores at 12 months (p = 0.048 and p = 0.025, respectively). Conclusions Application of BES to the SAN may help reduce impaired shoulder function in patients undergoing oncologic neck dissection, and may be considered a viable adjunct to functional rehabilitation therapies. Methods: A total of 26 residents participated in a session led by a pediatric faculty member and a Neonatal Intensive Care Unit transport nurse using a high-fidelity pediatric simulator. Abstract: Estudo epidemiolÃ³gico realizado com o contributo da American Cancer Society com o objectivo de avaliar o risco de cancro do pulmÃ£o em fumadores de cigarros com filtro com conteÃºdo mÃ©dio (standard) de alcatrÃ£o, comparativamente a fumadores de cigarros com baixo e muito baixo teor em alcatrÃ£o e, ainda, comparativamente a fumadores de cigarros sem filtro com elevado conteÃºdo em alcatrÃ£o. Participaram 364 239 homens e 576 535 mulheres com mais de 29 anos e que nunca tinham fumado, eram ex-fumadores ou fumadores activos fiÃ©is Ã mesma marca de tabaco (mÃnimo de 10 anos). A anÃ¡lise incidiu sobre a mortalidade por cancro do pulmÃ£o. Durante os 6 anos de acompanhamento (1982-1988), morreram por neoplasia 2622 homens e 1406 mulheres. O estudo concluiu que: â¢ Independentemente do teor em alcatrÃ£o, todos os fumadores activos tinham maior risco de cancro do pulmÃ£o do que os ex-fumadores e os nÃ£o fumadores. â¢ O risco de cancro do pulmÃ£o foi semelhante nos fumadores de cigarros com conteÃºdo mÃ©dio, baixo ou muito baixo em alcatrÃ£o. â¢ Os fumadores de tabaco sem filtro e alto teor em alcatrÃ£o apresentaram um risco ainda mais elevado de cancro do pulmÃ£o. â¢ O risco de neoplasia do pulmÃ£o nos ex-fumadores antes dos 35 anos foi prÃ³ximo do risco dos que nunca fumaram. COMENTÃRIO: A maioria dos estudos de caso-controlo examinaram os riscos de cancro de pulmÃ£o entre tabaco com alto teor em alcatrÃ£o e tabaco com teor mÃ©dio (convencional). A presente anÃ¡lise Ã© mais realista, dada a reduÃ§Ã£o da percentagem de fumadores de cigarros sem filtro/alto teor em alcatrÃ£o, pelo menos na Europa Ocidental. Mas, apesar da crÃtica, os resultados obtidos nÃ£o sÃ£o inesperados e sÃ£o atÃ© compatÃveis com o descrito âfenÃ³meno do acto compensatÃ³rio de fumarâ, ou seja, o fumador tem tendÃªncia a regular os nÃveis de nicotina desejados. Assim, ao fumar tabaco com menor concentraÃ§Ã£o, promovemse inalaÃ§Ãµes compensatÃ³rias de forma que a quantidade de nicotina se mantenha a mesma. Abstract: RESUMEN En el año 2005, poco tiempo después de ratificar el Convenio Marco para el Control del Tabaco (CMCT), Uruguay comenzó una ambiciosa campaña de control de tabaco que ubicó a este país a la vanguardia mundial de la lucha antitabaco. La gran diversidad de medidas implementadas en un lapso relativamente corto y la rigurosidad con que se aplicaron y observaron las políticas tienen escasos precedentes, incluso en los países más desarrollados. En este trabajo, se presenta una síntesis y actualización de varios trabajos de los autores en los que se evalúan algunos aspectos de la campaña a diez años de la ratificación del CMCT. En particular, estas investigaciones resaltan el impacto agregado que tuvo la campaña en la caída de la prevalencia en la población adulta, en los jóvenes y en las decisiones de abandono del consumo de tabaco en las embarazadas (1, 2), así como el impacto relativo de las políticas impositivas en relación a medidas de control de tabaco distintas de los precios. Los análisis se basan en estudios observacionales (a nivel de la población o el individuo) que utilizan grupos de control externos (Argentina y Chile) e internos (población no fumadora). Se plantean, también, los desafíos pendientes en el diseño de políticas antitabaco.

19. Impacts of the Statewide COVID-19 Lockdown Interventions on Excess Mortality, Unemployment, and Employment Growth

Abstract: The aim of the study is to determine relationships between lockdowns and excess mortality, unemployment, and employment growth.Each US states' mortality data for 2020 were compared with the prior 3 years to determine excess mortality. Data were compared using measures of lockdowns, or state openness scores and adjusted for age, sex, race/ethnicity, and cardiovascular disease. Comparisons were made with unemployment rates and employment growth rates.The 2020 excess mortality ranged from -9% to 46%. The average openness score was not significant ( P = 0.20). However, openness was strongly associated with both unemployment ( P = 0.01) and employment growth ( P = 0.0008).There was no statistical relationship between excess mortality and openness scores, while there were strong relationships with employment measures. These results suggest that lockdowns are not sufficiently beneficial for future use in this pandemic and raise concerns for use in future pandemics.

20. New Approach to the SIR Inversion Problem: From the 1905-1906 Plague Outbreak in the Isle of Bombay To the 2021-2022 Omicron Surge in New York City

Abstract: We describe a novel approach to recovering the underlying parameters of the SIR dynamic epidemic model from observed data on case incidence or deaths. We formulate a discrete-time approximation to the original continuous-time model and search for the parameter vector that minimizes the standard least squares criterion function. We show that the gradient vector and matrix of second-order derivatives of the criterion function with respect to the parameters adhere to their own systems of difference equations and thus can be exactly calculated iteratively. Applying our new approach, we estimate four-parameter SIR models on two datasets: (1) daily reported cases of COVID-19 during the SARS-CoV-2 Omicron/BA.1 surge of December 2021 - March 2022 in New York City; and (2) weekly deaths from a plague outbreak on the Isle of Bombay during December 1905 - July 1906, originally studied by Kermack and McKendrick in their now-classic 1927 paper. The estimated parameters from the COVID-19 data suggest a duration of persistent infectivity beyond that reported in small-scale clinical studies of mostly symptomatic subjects. The estimated parameters from the plague data suggest that the Bombay outbreak was in fact driven by pneumonic rather than bubonic plague.

21. Doctoral writing workshops: A pre-registered, randomized controlled trial

Abstract: Doctoral students were randomly assigned to a five-week (30-hour) faculty-led writing workshop intervention, either preceded by a five-week (waiting list) control phase or followed by a five-week maintenance phase. In the workshop, students wrote together, received instruction in genres of academic writing (literature reviews, scientific articles, funding proposals, and presentations), and exchanged feedback on drafts. As a result of the workshop students enjoyed writing more, found writing easier, and gained confidence in themselves as academic writers. They felt able to write productively in shorter blocks of time, and they engaged in more short-term, medium-term, and long-term planning of their research. The intervention also caused participants to pause more frequently for reflection or positive thinking and to generate more new writing. Effects were maintained in a peer-led writing maintenance group for at least five weeks after the intervention ended. This is the first randomized controlled trial of a doctoral-level writing intervention to date and has the potential to support doctoral training in academic and scientific writing across the Social Sciences, Education, and the Humanities.

Abstract: Renaissance Engineering 1 is a first-year engineering course that is the “flagship course” of Lassonde School of Engineering, where students are introduced to essential concepts and practices in ethics, communication, and creative problem solving. It is a large course that impacts over 600 students per year. Since Fall 2020, partly as a response to the pandemic, we fundamentally transformed the content and delivery of the course. This year, we have continued this transformative journey with an emphasis on reinventing the assessment approach. The limitations of normative grading are wellknown in the education field. Specifically, to our situation, the appropriateness of this practice in professional education where the goal is to ensure every student acquires the necessary competence, is suspect. Specification grading bridges normative and competencybased grading paradigms and has been shown to be effective in the engineering education setting. In all assignments, including the final case study, students are asked to satisfy a number of requirements distributed across four levels of competencies: Level 1: Foundational requirements for being a well-adjusted citizen, Level 2: Foundational requirements for being a contributing engineer, Level 3: Advanced requirements for being a well adjusted citizen, and Level 4: Advanced requirements for being a contributing engineer. Students are assigned grades from D to A based on their requirement satisfaction. Students have a limited number of chances to revise and resubmit their work if they have failed to satisfy all requirements in order to demonstrate competency. If they fail to meet multiple level 1 requirements after resubmission, they will fail the course. During the Fall-2021 term, we faced a number of unexpected challenges and surprises. Compared to previous years, this cohort - having experienced tremendous difficulties through the pandemic - were more tentative and insecure and took to a new grading scheme with notable trepidation initially. Surprisingly, many students had notable difficulty following clear written instructions, which is likely another pandemic-induced abnormality. Nevertheless, the majority of the students became comfortable with the scheme by the end of the term and achieved satisfactory learning outcomes. Significantly, while the majority of the students (~58%) achieved A or B grades, a significant minority (~18%) of students had failed the course. The course is offered to a new cohort of students in Winter 2022. Following a system thinking approach, we adjusted the grading scheme implementation based on our experience and learnings from the Fall-2021 term through winter term that led us to new and consistent findings. However, the benefits of specification grading in ensuring students meet critical competencies is particularly relevant for a professional education program such as engineering. Indeed, the bimodal grade distribution calls into question the status quo of normbased grading and calls for further research on assessment schema in engineering.

Abstract: First-year university is an ideal time for students to begin the development of critical thinking and self-directed learning skills. Structuring critical reflection with experiential learning activities can provide opportunities for students to develop and learn how to apply these skills in the future. Previous research has identified considerations for implementing critical reflection into the first-year engineering curricula in ways that students will meaningfully engage. The primary goal of this work was to integrate critical reflection learning outcomes, activities, and assessments into the first-year engineering curriculum connected to experiential learning activities. Critical reflection activities were scaffolded to a critical reflection framework, adapted to the first-year engineering context. Three critical reflection assessments were mapped to experiential learning activities to further the development of these skills in parallel, such as teamwork, problem solving, and communication. One assignment involved a peer review process, where students had the opportunity to learn from each other’s experiences, and give constructive feedback through learning of students’ shared university experiences. The benefits of developing student reflection skills are obvious, and the improvements witnessed are encouraging. However, there remain many challenges, particularly with respect to assessment methodologies, and student motivation. Meaningful integration of critical reflection remains an iterative learning experience for the instructors.

Abstract: This paper explores undergraduate mentors’ perspectives on, and participation in, “Bringing STEM to Life: Work Integrated Learning in Physics” (BSTL), a work-integrated, equity-oriented STEM outreach program administered by the kindergarten to industry (k2i) academy at York University’s Lassonde School of Engineering. To that end, this study brings Feminist Science and Technology Studies and critical pedagogy to bear on a three-phase methodological approach to generating and analyzing qualitative data pertaining to the mentorship component of BSTL. Preliminary findings suggest that (1) undergraduate mentors bring complex STEM motivations, shaped by intersecting marginalized identities, to bear on their mentorship duties; (2) mentors possess nuanced yet occasionally contradictory understandings of STEM, equity, and society; and (3) mentors’ experiences in the BSTL program are variable but positive. These findings suggest that outreach programs can expand their capacity to generate equitable outcomes by actively supporting the creation of STEM counterspaces, foregrounding equity training, and exposing mentors to critical theoretical perspectives on STEM, equity, and society.

30. Geospatial Analysis of a COVID-19 Outbreak at the University of Wisconsin - Madison: Potential Role of a Cluster of Local Bars

Abstract: We combined smartphone mobility data with census track-based reports of positive case counts to study a coronavirus disease 2019 (COVID-19) outbreak at the University of Wisconsin–Madison campus, where nearly 3000 students had become infected by the end of September 2020. We identified a cluster of twenty bars located at the epicentre of the outbreak, in close proximity to campus residence halls. Smartphones originating from the two hardest-hit residence halls (Sellery-Witte), where about one in five students were infected, were 2.95 times more likely to visit the 20-bar cluster than smartphones originating in two more distant, less affected residence halls (Ogg-Smith). By contrast, smartphones from Sellery-Witte were only 1.55 times more likely than those from Ogg-Smith to visit a group of 68 restaurants in the same area [rate ratio 1.91, 95% confidence interval (CI) 1.29–2.85, P < 0.001]. We also determined the per-capita rates of visitation to the 20-bar cluster and to the 68-restaurant comparison group by smartphones originating in each of 21 census tracts in the university area. In a multivariate instrumental variables regression, the visitation rate to the bar cluster was a significant determinant of the per-capita incidence of positive severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) tests in each census tract (elasticity 0.88, 95% CI 0.08–1.68, P = 0.032), while the restaurant visitation rate showed no such relationship. The potential super-spreader effects of clusters or networks of places, rather than individual sites, require further attention.

31. The repeated setbacks of HIV vaccine development laid the groundwork for SARS-CoV-2 vaccines

Abstract: The decades-long effort to produce a workable HIV vaccine has hardly been a waste of public and private resources. To the contrary, the scientific know-how acquired along the way has served as the critical foundation for the development of vaccines against the novel, pandemic SARS-CoV-2 virus. We retell the real-world story of HIV vaccine research - with all its false leads and missteps - in a way that sheds light on the current state of the art of antiviral vaccines. We find that HIV-related RD had more than a general spillover effect. In fact, the repeated failures of phase 2 and 3 clinical trials of HIV vaccine candidates have served as a critical stimulus to the development of successful vaccine technologies today. We rebut the counterargument that HIV vaccine development has been no more than a blind alley, and that recently developed vaccines against COVID-19 are really descendants of successful vaccines against Ebola, MERS, and SARS. These successful vaccines likewise owe much to the vicissitudes of HIV vaccine development. We then discuss how the failures of HIV vaccine development have taught us how adapt SARS-CoV-2 vaccines to immune escape from emerging variants. Finally, we inquire whether recent advances in the development of vaccines against SARS-CoV-2 might in turn further the development of an HIV vaccine - what we describe as a reverse spillover effect.

32. Mobility was a Significant Determinant of Reported COVID-19 Incidence During the Omicron Surge in the Most Populous U.S. Counties

Abstract: BackgroundSignificant immune escape by the Omicron variant, along with the emergence of widespread worry fatigue, have called into question the robustness of the previously observed relation between population mobility and COVID-19 incidence.MethodsWe employed principal component analysis to construct a one-dimensional summary indicator of six Google mobility categories. We related this mobility indicator to case incidence among 111 of the most populous U.S. counties during the Omicron surge from December 2021 through February 2022.ResultsReported COVID-19 incidence peaked earlier and declined more rapidly among those counties exhibiting more extensive decline in mobility between December 20 and January 3. Based upon a fixed-effects, longitudinal cohort model, we estimated that every 1-percent decline in mobility between December 20 and January 3 was associated with a 0.63 percent decline in peak incidence during the week ending January 17 (95% confidence interval, 0.40-0.86 percent). Based upon a cross-sectional analysis including mean household size and vaccination participation as covariates, we estimated that the same 1-percent decline in mobility was associated with a 0.36 percent decline in cumulative reported COVID-19 incidence from January 10 through February 28 (95% CI, 0.18-0.54 percent).ConclusionOmicron did not simply sweep through the U.S. population until it ran out of susceptible individuals to infect. To the contrary, a significant fraction managed to avoid infection by engaging in risk-mitigating behaviors. More broadly, the behavioral response to perceived risk should be viewed as an intrinsic component of the natural course of epidemics in humans.

Abstract: BackgroundSome reports have suggested that as many as one-half of all hospital inpatients identified as COVID-19-positive during the Omicron BA.1 variant-driven wave were incidental cases admitted primarily for reasons other than their viral infections. To date, however, there are no prospective longitudinal studies of a representative panel of hospitals based on pre-established criteria for determining whether a patient was in fact admitted as a result of the disease.Materials and MethodsTo fill this gap, we developed a formula to estimate the fraction of incidental COVID-19 hospitalizations that relies upon measurable, population-based parameters. We applied our approach to a longitudinal panel of 164 counties throughout the United States, covering a 4-week interval ending in the first week of January 2022.ResultsWithin this panel, we estimated that COVID-19 incidence was rising exponentially at a rate of 9.34% per day (95% CI, 8.93-9.87). Assuming that only one-quarter of all Omicron BA.1 infections had been reported by public authorities, we further estimated the aggregate prevalence of active SARS-CoV-2 infection during the first week of January to be 3.45%. During the same week, among 250 high-COVID-volume hospitals within our 164-county panel, an estimated 1 in 4 inpatients was COVID-positive. Based upon these estimates, we computed that 10.6% of such COVID-19-positive hospitalized patients were incidental infections. Across individual counties, the median fraction of incidental COVID-19 hospitalizations was 9.5%, with an interquartile range of 6.7 to 12.7%.ConclusionIncidental COVID-19 infections appear to have been a nontrivial fraction of all COVID-19-positive hospitalized patients during the Omicron BA.1 wave. In the aggregate, however, the burden of patients admitted for complications of their viral infections was far greater.

Using a proposed conceptual framework and inspirationfrom Mezirow’s Transformative Learning Theory, this paper analyzes four engineering students in varying disciplines across the three contexts that a co-op experience provides: pre co-op school setting, co-op work placement and the post co-op school setting. Using reflection and by adapting to their settings, these students transformed their communication, organization/ time management and initiative/ responsibility skills. Analyzing these skills and experiences against the framework, we can see how the details and specifics of the transformation varies for each student; however, each student needs to experience each stage for successful transformation. Each of the stages then feeds into and helps further the skills as it changes.

Abstract: Background Xerostomia is a debilitating side effect of radiotherapy for head and neck cancer. Combining surgical submandibular-gland transfer (SMGT) with intensity-modulated radiotherapy (IMRT) may provide greater protection of salivary function. Methods This was a single-institution, prospective phase II feasibility trial. Patients with head and neck cancer or unknown primary with neck node metastases received primary surgery with SMGT and postoperative radiotherapy with tomotherapy (60 Gy in 30 fractions). Toxicity and quality of life (QOL) were assessed before surgery, before RT, and after RT. Results Forty patients received SMGT and IMRT. Only 1 patient experienced grade 3 salivary gland toxicity. At 12 months post-RT, the rate of absent or only mild xerostomia was 89%, and salivary flow rates were approximately 75% of pre-RT levels. Conclusions The combination of IMRT with SMGT is feasible and with improved dose constraints may maximally spare the parotid and submandibular glands, leading to decreased xerostomia and improved patient QOL.

Abstract: BACKGROUND: The efficacy and safety of benralizumab, an interleukin-5 receptor alpha-directed cytolytic monoclonal antibody, for the prevention of exacerbations in patients with moderate to very severe chronic obstructive pulmonary disease (COPD) are not known.METHODS: In the GALATHEA and TERRANOVA trials, we enrolled patients with COPD (at a ratio of approximately 2:1 on the basis of eosinophil count [≥220 per cubic millimeter vs. RESULTS: In GALATHEA, the estimates of the annualized exacerbation rate were 1.19 per year (95% confidence interval [CI], 1.04 to 1.36) in the 30-mg benralizumab group, 1.03 per year (95% CI, 0.90 to 1.19) in the 100-mg benralizumab group, and 1.24 per year (95% CI, 1.08 to 1.42) in the placebo group; the rate ratio as compared with placebo was 0.96 for 30 mg of benralizumab (P = 0.65) and 0.83 for 100 mg of benralizumab (P = 0.05). In TERRANOVA, the estimates of the annualized exacerbation rate for 10 mg, 30 mg, and 100 mg of benralizumab and for placebo were 0.99 per year (95% CI, 0.87 to 1.13), 1.21 per year (95% CI, 1.08 to 1.37), 1.09 per year (95% CI, 0.96 to 1.23), and 1.17 per year (95% CI, 1.04 to 1.32), respectively; the corresponding rate ratios were 0.85 (P = 0.06), 1.04 (P = 0.66), and 0.93 (P = 0.40). At 56 weeks, none of the annualized COPD exacerbation rate ratios for any dose of benralizumab as compared with placebo reached significance in either trial. Types and frequencies of adverse events were similar with benralizumab and placebo.CONCLUSIONS: Add-on benralizumab was not associated with a lower annualized rate of COPD exacerbations than placebo among patients with moderate to very severe COPD, a history of frequent moderate or severe exacerbations, and blood eosinophil counts of 220 per cubic millimeter or greater (Funded by AstraZeneca [GALATHEA and TERRANOVA] and Kyowa Hakko Kirin [GALATHEA]; GALATHEA and TERRANOVA ClinicalTrials.gov numbers, NCT02138916 and NCT02155660.).

Abstract: We study the motivations of traders in the interbank market around the 2007-09 subprime crisis. We develop a new methodology that reveals the underlying urgency to borrow overnight funds, which we call Trading Urgency. We find that the dispersion of beliefs (market Sidedness) and Trading Urgency reveal information from the interbank market—these statistics both lead sovereign CDS spreads and react to non-standard central bank interventions introduced during the crisis. Our results map the linkages between the interbank market and sovereigns as well as shed light on the channels that give rise to the sovereign-bank nexus.
