Summary Aim To describe the ambulatory changes in drug prescriptions 3 months after hospital discharge among elderly patients aged 75 and over, and to identify the reasons for these changes. Methods A prospective cohort study was conducted on subjects, discharged between 09/2016 and 01/2017 from the Bordeaux University Hospital. Prescription forms were collected from patients’ pharmacists. The main outcome was the occurrence of at least one significant change (SC) defined as an initiation, a discontinuation, a switch or change in drug daily dosage as regards the drugs prescribed upon hospital discharge and those prescribed 3 months after. Whenever drug SC occurred, general practitioners were requested to elicit reasons for such changes. Results Among the 126 patients included in our study, 73 underwent a 3-month follow-up period, without death or being re-hospitalised. 87.7% of them had at least one SC 3 months after discharge, with an average of 3.1 ± 2.5 SC per patient. Main changes involved: discontinuation or dose decrease of anxiolytics, hypnotics, antalgics, betablockers and calcium channel blockers; start or dose increase of diuretics, ACE inhibitors and angiotensin receptor blockers. In patients with a 3-month follow-up period, 27.4% underwent at least one ADR-induced SC. Conclusion Most elderly patients experience drug prescription changes after discharge. Some, according to drug iatrogenic, could be avoided through better cooperation between hospital and ambulatory prescribers.

BACKGROUND In Nouvelle-Aquitaine (a French region with a population of almost 6 million), the density of dermatologists is less than 3.8/100,000 inhabitants. This lack of dermatological care is delaying diagnosis and management, especially for skin cancer. The SmartDerm Project is a store-and-forward (SAF) teledermatology platform for primary care in Nouvelle-Aquitaine providing dermatological counselling to general practitioners (GPs). OBJECTIVE The main objective was to determine the concordance between the diagnosis of skin cancer made by dermatologists and the pathologists’ diagnosis. METHODS GPs in 3 pilot departments of Nouvelle-Aquitaine (Lot-Et-Garonne, Deux-Sèvres, Creuse) sent their dermatology requests using their smartphone, via an app called PAACO/Globule; dermatologists at the University Hospital of Bordeaux answered within 48-72 hours. Consecutive cases of skin cancer suspected by the referent dermatologists during the intervention were included, if the result of biopsy interpreted by a certified pathologist was available at the time of the study. RESULTS Among the 1727 requests, 163 (9%) concerned a possible diagnosis of skin cancer and were eligible. For 61 cases, the histopathological findings were not available. Eventually, 93 patients with a total of 102 skin lesions were included. Median age was 75 years (range 26-97 years), with 53% women. The skin lesions had progressed for 8 months on average (range 0.5-36 months). The median response time was 1 day (range 0-61 days); 65 days (range 1-667 days) elapsed on average between the SAF opinion and the histological sample. Histopathology diagnosed 83 malignant lesions (57 basal cell carcinomas, 69%; 18 squamous cell carcinomas, 22%; 6 melanomas, 7%; 1 cutaneous lymphoma, 1%; 1 secondary location of a primary cancer, 1%), 1 precancerous lesion, and 18 benign lesions. The concordance between the opinion of the referent dermatologist and the final pathological finding was 83% for nonmelanocytic lesions and 67% for melanocytic lesions. CONCLUSIONS This study showed the reliability of SAF teledermatology in the diagnosis of skin cancer, comparable to literature data in the absence of dermatoscopy. The median delay of about two months between request and histology was an improvement compared to the delay of usual appointments in the intervention area. The lack of data for 61 patients showed that SAF telemedicine requires better coordination and follow-up, especially for the management of skin cancer. With this reservation in mind, teledermatology offers an alternative answer for the triage of patients with skin cancer residing in areas with low medical density.

Background: Female surgeons are exposed to physical and mental stressors that differ from those of other specialties. We aimed to assess whether female surgeons are more at risk of pregnancy complications than women in other medical specialties. Methods: We used a cross-sectional electronic survey of female physicians working in the French Region Nouvelle-Aquitaine who were pregnant between 2013 and 2018. A pregnancy complication was defined as the occurrence of miscarriage, ectopic pregnancy, fetal growth restriction, prematurity, fetal congenital malformation, stillbirth, or medical termination of the pregnancy. Multivariate logistic regression models were used to evaluate the risk of pregnancy complications for female surgeons relative to women practicing in other medical specialties. Results: Among the 270 women included, 52 (19.3%) experienced pregnancy complications and 28 (10.4%) were surgeons. In the multivariate analysis, female surgeons had a higher risk of pregnancy complications: adjusted odds ratio = 3.53, 95% confidence interval [1.27–9.84]. Conclusion: Further research is necessary to identify the hazards specifically involved in the practice of surgery to be able to propose preventive actions targeted to female surgeons during their pregnancy.

International audience; Objectives: To assess the efficacy of inhaled ciclesonide in reducing the risk of adverse outcomes in COVID-19 outpatients at risk of developing severe illness.Methods: COVERAGE is an open-label, randomized controlled trial. Outpatients with documented COVID-19, risk factors for aggravation, symptoms for ≤7 days, and absence of criteria for hospitalization are randomly allocated to either a control arm or one of several experimental arms, including inhaled ciclesonide. The primary efficacy endpoint is COVID-19 worsening (hospitalization, oxygen therapy at home, or death) by Day 14. Other endpoints are adverse events, maximal follow-up score on the WHO Ordinal Scale for Clinical Improvement, sustained alleviation of symptoms, cure, and RT-PCR and blood parameter evolution at Day 7. The trial's Safety Monitoring Board reviewed the first interim analysis of the ciclesonide arm and recommended halting it for futility. The results of this analysis are reported here.Results: The analysis involved 217 participants (control 107, ciclesonide 110), including 111 women and 106 men. Their median age was 63 years (interquartile range 59-68), and 157 of 217 (72.4%) had at least one comorbidity. The median time since first symptom was 4 days (interquartile range 3-5). During the 28-day follow-up, 2 participants died (control 2/107 [1.9%], ciclesonide 0), 4 received oxygen therapy at home and were not hospitalized (control 2/107 [1.9%], ciclesonide 2/110 [1.8%]), and 24 were hospitalized (control 10/107 [9.3%], ciclesonide 14/110 [12.7%]). In intent-to-treat analysis of observed data, 26 participants reached the composite primary endpoint by Day 14, including 12 of 106 (11.3%, 95% CI: 6.0%-18.9%) in the control arm and 14 of 106 (13.2%; 95% CI: 7.4-21.2%) in the ciclesonide arm. Secondary outcomes were similar for both arms.Discussion: Our findings are consistent with the European Medicines Agency's COVID-19 task force statement that there is currently insufficient evidence that inhaled corticosteroids are beneficial for patients with COVID-19.

Background Sexually transmitted infections are a major public health issue, both in France and worldwide. Primary healthcare professionals play a key role in sexual health and prevention, but few take on this subject. Prevention strategies are diverse, thus risk reduction strategies focussing on behavioural changes are still needed. Purpose We conducted a systematic review to analyse risk reduction interventions focussing on behavioural change in OECD countries in primary healthcare settings to help develop a prevention tool easy to apply in primary care. Methods We searched for English- or French-language controlled trials in PubMed, Cochrane Library, Scopus, PsycINFO, PsycArticle, PBSC, SocINDEX, Google Scholar, and CAIRN, supplemented with the bibliographies of previous systematic reviews. Thirty controlled, randomized, or nonrandomized trials were included in the systematic review. We did not conduct any meta-analysis due to the diversity of populations, outcomes and study designs. Results There are efficient interventions in primary healthcare settings for reducing sexual risk behaviours and/or incident STI. Their efficiency seems to grow with the length and intensity of the intervention. Most interventions target only young, high-risk patients, and their long-term impact is uncertain. Most included studies had an overall risk of bias estimated as high or with some concerns. Conclusions Some tools could be used in primary care, with possible efficiency though results are difficult to generalize, and value should be assessed in daily practice. Future research should also focus on older population given the epidemiological evolutions, but also lower-risk population to target all patients seen in primary care settings.

Objectives To assess the efficacy of several repurposed drugs to prevent hospitalisation or death in patients aged 65 or more with recent symptomatic SARS-CoV-2 infection (COVID-19) and no criteria for hospitalisation. Trial design Phase III, multi-arm (5) and multi-stage (MAMS), randomized, open-label controlled superiority trial. Participants will be randomly allocated 1:1:1:1:1 to the following strategies: Arm 1: Control arm Arms 2 to 5: Experimental treatment arms Planned interim analyses will be conducted at regular intervals. Their results will be reviewed by an Independent Data and Safety Monitoring Board. Experimental arms may be terminated for futility, efficacy or toxicity before the end of the trial. New experimental arms may be added if new evidence suggests that other treatments should be tested. A feasibility and acceptability substudy as well as an immunological substudy will be conducted alongside the trial. Participants Inclusion criteria are: 65-year-old or more; Positive test for SARS-CoV-2 on a nasopharyngeal swab; Symptoms onset within 3 days before diagnosis; No hospitalisation criteria; Signed informed consent; Health insurance. Exclusion criteria are: Inability to make an informed decision to participate (e.g.: dementia, guardianship); Rockwood Clinical Frailty Scale ≥7; Long QT syndrome; QTc interval > 500 ms; Heart rate 5.5 mmol/L or The trial is being conducted in France in the Bordeaux, Corse, Dijon, Nancy, Paris and Toulouse areas as well as in the Grand Duchy of Luxembourg. Participants are recruited either at home, nursing homes, general practices, primary care centres or hospital outpatient consultations. Intervention and comparator The four experimental treatments planned in protocol version 1.2 (April 8th, 2020) are: (1) Hydroxychloroquine 200 mg, 2 tablets BID on day 0, 2 tablets QD from day 1 to 9; (2) Imatinib 400 mg, 1 tablet QD from day 0 to 9; (3) Favipiravir 200 mg, 12 tablets BID on day 0, 6 tablets BID from day 1 to 9; (4) Telmisartan 20 mg, 1 tablet QD from day 0 to 9. The comparator is a complex of vitamins and trace elements (AZINC Forme et Vitalité®), 1 capsule BID for 10 days, for which there is no reason to believe that they are active on the virus. In protocol version 1.2 (April 8th, 2020): People in the control arm will receive a combination of vitamins and trace elements; people in the experimental arms will receive hydroxychloroquine, or favipiravir, or imatinib, or telmisartan. Main outcome The primary outcome is the proportion of participants with an incidence of hospitalisation and/or death between inclusion and day 14 in each arm. Randomisation Participants are randomized in a 1:1:1:1:1 ratio to each arm using a web-based randomisation tool. Participants not treated with an ARB or ACEI prior to enrolment are randomized to receive the comparator or one of the four experimental drugs. Participants already treated with an ARB or ACEI are randomized to receive the comparator or one of the experimental drugs except telmisartan (i.e.: hydroxychloroquine, imatinib, or favipiravir). Randomisation is stratified on ACEI or ARBs treatment at inclusion and on the type of residence (personal home vs. nursing home). Blinding (masking) This is an open-label trial. Participants, caregivers, investigators and statisticians are not blinded to group assignment. Numbers to be randomised (sample size) A total of 1057 participants will be enrolled if all arms are maintained until the final analysis and no additional arm is added. Three successive futility interim analyses are planned, when the number of participants reaches 30, 60 and 102 in the control arm. Two efficacy analyses (interim n°3 and final) will be performed successively. Trial Status This describes the Version 1.2 (April 8th, 2020) of the COVERAGE protocol that was approved by the French regulatory authority and ethics committee. The trial was opened for enrolment on April 15th, 2020 in the Nouvelle Aquitaine region (South-West France). Given the current decline of the COVID-19 pandemic in France and its unforeseeable dynamic in the coming months, new trial sites in 5 other French regions and in Luxembourg are currently being opened. A revised version of the protocol was submitted to the regulatory authority and ethics committee on June 15th, 2020. It contains the following amendments: (i) Inclusion criteria: age ≥65 replaced by age ≥60; time since first symptoms <3 days replaced by time since first symptoms <5 days; (ii) Withdrawal of the hydroxychloroquine arm (due to external data); (iii) increase in the number of trial sites. Trial registration The trial was registered on Clinical Trials.gov on April 22nd, 2020 (Identifier: NCT04356495): and on EudraCT on April 10th, 2020 (Identifier: 2020-001435-27). Introduction La densite en dermatologues en France est inegale avec une moyenne de 5,5/100 000 habitants. Dans trois departements de Nouvelle-Aquitaine (Lot-et-Garonne, Deux-Sevres et Creuse), elle est inferieure a 2,4/100 000 habitants et les delais de consultation superieurs a 100 jours. Le projet SmartDerm est une etude pilote de mise en place de tele-expertise dermatologique sous l’egide de l’ARS, dans le cadre d’une aide regionale aux soins primaires, dans ces departements entre septembre 2018 a decembre 2019. L’objectif principal etait d’evaluer le nombre de consultations presentielles apres avis. Materiel et methodes Il s’agit d’une cohorte observationnelle des patients ayant consulte leur medecin generaliste (MG) necessitant un avis dermatologique par tele-expertise (hors urgence) dans les 3 departements cites. Participants Les MG maitres de stage des universites et/ou ayant deja l’outil numerique developpe par l’ARS Nouvelle-Aquitaine PAACO/Globule. Il etait propose aux dermatologues liberaux et des centres hospitaliers territoriaux, CHU de Poitiers et Limoges inclus, de participer (niveau 1), et en cas de non-reponse dans les 48 h, le centre coordinateur (niveau 2) repondait aux avis. Le recueil des donnees a ete realise par la plateforme smartderm.paaco.fr. Les pathologies dermatologiques etaient classees selon la classification CISP-2. Resultats Cent neuf MG ont participe et ont realise 702 demandes soit 0,4 demandes/mois/MG [0–3,1]. Le delai de reponse etait en moyenne de 1 jour [0–34]. Deux photographies [0–12] ont ete envoyees en moyenne, interpretables pour 96 % (672) des demandes et 37 reponses de niveau 1. Les patients avaient 51 ans [0–102] en moyenne, 52 % de femmes. Les symptomes associes etaient une modification de la lesion (n = 277, 39 %) et un prurit (n = 186, 26,5 %). Une consultation presentielle etait jugee necessaire pour 249 patients (35 %) dans un delai de 1 a 3 mois pour 38 % des patients, principalement pour les pathologies tumorales (n = 129). Il y avait necessite d’une biopsie ou une exerese pour 122 patients (17 %), de cryotherapie pour 45 patients (6 %). Les dermatoses les plus frequemment rencontrees etaient inflammatoires (n = 201, 29 %), puis tumorales (n = 188, 27 %) et infectieuses (n = 144, 21 %). Selon la classification CISP-2, les pathologies les plus frequentes etaient les cancers de la peau (n = 72, 10 %), les tumeurs benignes (n = 72, 10 %), la dermatite atopique (n = 70, 10 %). Discussion Ce projet a permis d’eviter plus de 60 % de consultations chez le dermatologue dans ces zones sous-denses. On souligne l’importance de la dermatite atopique et des cancers cutanes pour le ciblage d’un appui specialise. La participation des dermatologues locaux etant limitee, la coordination et le soutien par le centre hospitalier pivot etaient cruciaux pour cette phase pilote.

Background There is currently an absence of valid and relevant instruments to evaluate how Evidence-based Practice (EBP) training improves, beyond knowledge, physicians’ skills. Our aim was to develop and test a tool to assess physicians’ EBP skills. Methods The tool we developed includes four parts to assess the necessary skills for applying EBP steps: clinical question formulation; literature search; critical appraisal of literature; synthesis and decision making. We evaluated content and face validity, then tested applicability of the tool and whether external observers could reliably use it to assess acquired skills. We estimated Kappa coefficients to measure concordance between raters. Results Twelve general practice (GP) residents and eleven GP teachers from the University of Bordeaux, France, were asked to: formulate four clinical questions (diagnostic, prognosis, treatment, and aetiology) from a proposed clinical vignette, find articles or guidelines to answer four relevant provided questions, analyse an original article answering one of these questions, synthesize knowledge from provided synopses, and decide about the four clinical questions. Concordance between two external raters was excellent for their assessment of participants’ appraisal of the significance of article results (K = 0.83), and good for assessment of the formulation of a diagnostic question (K = 0.76), PubMed/Medline (K = 0.71) or guideline (K = 0.67) search, and of appraisal of methodological validity of articles (K = 0.68). Conclusions Our tool allows an in-depth analysis of EBP skills, thus could supplement existing instruments focused on knowledge or specific EBP step. The actual usefulness of such tools to improve care and population health remains to be evaluated. Resume Contexte et objectif Les statines prescrites en prevention primaire avant 75 ans permettent de reduire les evenements cardiovasculaires de 20 a 30 % et la mortalite de 10 % au prix d’effets indesirables acceptables. Nous avons cherche a savoir si ces resultats persistaient apres 75 ans. Sources documentaires Revue de la litterature des grands essais randomises statines versus placebo et des meta-analyses ayant inclus des patients de 75 ans et plus traites par statines en prevention primaire. Resultats Depuis les annees 1990, une vingtaine d’essais controles randomises etudiant les statines versus placebo en prevention primaire ont ete publies et etudies dans des meta-analyses. Les criteres d’exclusions, en particulier l’âge superieur a 70 ans, sont le plus souvent restrictifs. L’impact sur la mortalite toutes causes dans les quatre principales etudes et les meta-analyses chez les plus de 75 ans n’est pas demontre. D’un autre cote, une recente meta-analyse d’etudes observationnelles incluant des sujets entre 70 et 89 ans traites par statines a retrouve qu’un cholesterol total bas etait associe a une baisse moderee de la mortalite cardiovasculaire, sans diminution de la mortalite toutes causes. De plus, dans un contexte frequent de polymedication dans cette tranche d’âge, les statines peuvent etre responsables de nombreux effets indesirables, d’interactions medicamenteuses et d’une alteration de la qualite de vie. Conclusion Compte tenu de l’absence de preuve formelle d’efficacite en termes de mortalite toutes causes et d’un niveau eleve d’effets secondaires, le rapport benefice/risque d’une prevention primaire par statines n’est pas etabli chez les personnes âgees. Le poids economique des prescriptions de statines et leur retentissement possible sur la qualite de vie justifient une analyse medico-economique de l’arret de ce traitement chez les personnes de 75 et plus.

Statins in primary prevention before 75 years old reduce cardiovascular events from 20 to 30% and mortality from 10% with acceptable side effects. We investigated whether these results persisted for patients aged 75 and older taking statin.Methodic review of large randomized clinical trials and meta-analyzes that included patients 75 years and older treated with statins in primary prevention.Since the 1990s, a score of randomized controlled trials studying statins versus placebo in primary prevention were published and studied in meta-analyses. Exclusion criteria, including persons older than 70 years, are often restrictive. The impact on all-cause mortality in the four main studies and meta-analyses in over 75 years has not been demonstrated. On the other hand, a recent meta-analyses of observational studies including subjects between 70 and 89 years treated with statins found that low total cholesterol was associated with a moderate decrease in cardiovascular mortality, with no decrease in all-cause mortality. Moreover, in a common context of comorbidities in this age group, statins may be responsible for many adverse effects, drug interactions and impaired quality of life.Given the lack of formal evidence of effectiveness in terms of all-cause mortality and a high level of adverse effects, the benefit/risk of primary prevention with statins is not established in the elderly. The economic weight of statin prescriptions and their possible impact on quality of life justify an economic analysis of discontinuing statin therapy for people 75 years and older.

Introduction : dans le cadre d’un projet de création d’une maison de santé pluriprofessionnelle universitaire (MSPU) à Bordeaux centre, mené par le département de médecine générale de l’Université de Bordeaux, il est nécessaire de déterminer les besoins exprimés par les médecins généralistes du secteur ainsi que d’établir la place que cette structure pourrait occuper sur le territoire. Objectif : décrire l’organisation et l’offre de soin des cabinets médicaux, ainsi que les représentations des médecins généralistes autour d’un projet de MSPU sur le secteur de Bordeaux centre. Méthode : il s’agit d’une étude mixte quantitative et qualitative menée auprès des médecins généralistes, dans une zone de 1,5 kilomètres, centrée sur le lieu de la future MSPU (CHU Saint André). Des questionnaires ont été remis aux médecins de la zone. La fin de ces questionnaires les invitait à participer à des entretiens collectifs (focus groups). L’étude à eu lieu entre le 06 décembre 2020 et le 06 janvier 2021. Trois focus groups ont été réalisés. Une analyse statistique des données quantitatives et thématique des verbatims a été réalisée. Résultats : quarante-six médecins généralistes ont répondu au questionnaire et nous avons réalisé trois focus groups. Les principaux besoins exprimés par les participants concernaient un manque de confraternité au sein du secteur ainsi qu’une meilleur gestion des soins non programmés. Ils souffrent également de difficultés à se faire remplacer sur les périodes de congés. Ils expriment également le souhait d’améliorer l’accès aux examens complémentaires tel que l’IRM, et à certaines spécialités notamment la gynécologie, la dermatologie et la psychiatrie. Conclusion : la MSPU devra essayer de répondre aux besoins en santé de la population locale, en particulier aux patients sans médecin traitant, en situation de vulnérabilité, atteint de maladies psychiatriques, d’addiction ou de polypathologie complexe. Elle doit devenir un lieu de formation médicale continue plébiscité par les médecins généralistes du secteur et vecteur de confraternité.

Introduction: in dermatology, consultation deadlines are longer over the years in France. In consequence, a dermatology teleexpertise project (SmartDerm project) has been implemented in departments in medical decline: Lot-et-Garonne, Deux-Sèvres and Creuse. The primary objective of our study was to assess the types of pathologies most frequently met during tele-expertise requests, as part of the SmartDerm project. Methods: we performed a retrospective observational cohort study over a period of twelve months. Firstly, the general practitioner sent his tele-expertise request on the PAACO / Globule server. Then, the dermatologists in his department had 48 hours to respond to the request. In the absence of a response, the request was sent to the dermatologists at the CHU concerned. Results : a total of 574 tele-expertise requests were sent during the twelve-month inclusion period: 278 in Lot-et-Garonne, 230 in Deux-Sèvres and 66 in Creuse. 99 general practitioners participated to the project: 35 in Lot-et-Garonne, 47 in Deux-Sèvres and 17 in Creuse. The most frequently met pathologies were inflammatory (30%) and tumoral (23.9%). For inflammatory dermatoses, 65 cases (37.8%) of eczema-like dermatoses were found. As far as tumor pathologies are concerned, they were distributed as 57 cases (41.6%) of skin cancer and 56 cases (40.9%) of benign tumors. The date of onset of lesions was between ten days and three months for 187 cases (32.6%). The lesions were mainly localized on the head (36.9%) and the trunk (34.3%). Finally, after the teledermatological opinion, a face-to-face consultation was necessary for 197 cases (34.3%). Conclusion: tele-expertise has many advantages: a sorting role, a rapid dermatological advice and a cost reduction for health insurance. Therefore, the SmartDerm project will be extended to other departments., Introduction : les délais de consultation en dermatologie se sont progressivement allongés depuis plusieurs années en France. Face à ce constat un projet de télé-expertise en dermatologie (le projet SmartDerm) a été lancé dans certains départements en déprise médicale : en Lot-et-Garonne, dans les Deux-Sèvres et en Creuse. L’objectif principal de notre étude était d’évaluer les types de pathologies les plus fréquemment rencontrés lors des demandes de télé-expertise, dans le cadre du projet Smartderm. Méthodes : nous avons réalisé une étude observationnelle rétrospective de cohorte sur une durée de douze mois. Dans un premier temps le médecin généraliste envoyait sa demande de télé-expertise sur le serveur PAACO/Globule. La date d’apparition des lésions était comprise entre dix jours et trois mois pour 187 cas (32,6%). Les lésions étaient principalement localisées sur la tête (36,9%) et le tronc (34,3%). Enfin, au décours de l’avis télé-dermatologique, une consultation présentielle était jugée nécessaire pour 197 cas (34,3%). Conclusion : la télé-expertise possède de nombreux avantages : un rôle de tri, un avis dermatologique rapide et une réduction des couts pour l’Assurance maladie. Le projet SmartDerm va donc être étendu à d’autres départements.

Introduction: antibioresistance is one of the biggest threat of the twenty first century. The fecal carriage of extended spectrum beta lactamase producing enterobacteria (ESBL-PE) is a hidden plague and the world prevalence was 14% in 2016, according to Karanika and al. systematic review of the litterature. The goal of our study was to determine in which direction this rate had evolved. Methods: systematic review of the litterature acording to the PRISMA model via the PUBMED and SCOPUS databases and among the grey litterature between july 15th 2015 and may 20th 2020. Selection of the studies containing data regarding the fecal carriage of ESBL-PE among healthy individuals of the community for each World Health Organization defined region. The prevalence of ESBL-PE and the distribution of the different types of bacteria and enzymes were extracted. Results: twelve studies out of 1014 were included. The pool prevalence of ESBL-PE fecal colonization is currently estimated at 22,9% worldwide versus 14% in 2016, showing an 8,9% growth since july 2015. The highest prevalence is in South East Asia (41,6%) and in Africa (38%) and the smallest in the West Pacific region (15,4%) and in Europe (6,1%). No study corresponding to the inclusion criteria was found for the American and East Mediterranean regions. The most frequently isolated bacteria is Escherichia coli and the enzymes are CTX-M 14 and 15. Conclusions: the prevalence of ESBL-PE fecal carriage among healthy individuals in the community, altough uneven, globally keeps on growing. It is becoming worrysome and must be taken in consideration by the industrials, the politicians, the health workers and the population in order to avoid a heath disaster., Introduction : l’antibiorésistance est une des grandes menaces du vingt et unième siècle. Le portage fécal d’entérobactéries productrices de bêta-lactamases à spectre étendu (BLSE-PE) est un fléau invisible et la prévalence mondiale en 2016 de la colonisation chez les individus sains de la communauté était de 14% selon une revue de la littérature de Karanika et al. L’objectif de notre étude était de déterminer dans quel sens ce taux avait évolué. Matériels et Méthodes : revue systématique de la littérature selon le modèle PRISMA via les bases de données Pubmed, SCOPUS et au sein de la littérature grise entre le 11 juillet 2015 et le 20 mai 2020. Sélection des études contenant des données sur le portage fécal de BLSE-PE chez les individus sains de la communauté, pour chaque région définie par l’Organisation Mondiale de la Santé (OMS). La prévalence de BLSE-PE et la distribution des différents types de bactéries et d’enzymes ont été extraites. Résultats : douze études sur 1014 ont été incluses. La prévalence globale de la colonisation fécale de BLSE-PE est actuellement estimée à 22,9% au niveau mondial contre 14% en 2016, soit une croissance depuis juillet 2015 de 8,9%. La prévalence est plus élevée en Asie du Sud Est (41,6%) et en Afrique (38%) et plus faible dans la région pacifique Ouest (15,4%) ainsi qu’en Europe (6,1%). Aucune étude ne correspondant aux critères d’éligibilité n’était disponible pour les zones régionales Américaines et de l’est de la Méditerranée. La bactérie la plus souvent isolée est E. coli et les enzymes CTX-M 14 et 15 sont les plus fréquemment retrouvées. Conclusion : le taux de portage de BLSE-PE chez les individus sains de la communauté bien qu’inégal au niveau mondial, poursuit sa croissance. Il devient inquiétant et doit être pris en considération par les industriels, les politiques, les professionnels de santé et la population afin d’éviter une catastrophe sanitaire.

Introduction: the number of HIV’s new infections is relatively stable with conventional prevention methods. Pre-exposure prophylaxis (PrEP) is available in France since January 2016, and its prescription is renewable by general practitioners (GPs) since March 2017, in addition to the previous ones to match a multimodal prevention strategy. Since July 2018, the infectious diseases day hospital (HDJ) of Saint-André hospital (Bordeaux university hospital (BUH) has systematically offered each patient benefiting from PrEP to be taken care in alternation by their GP and the HDJ. The purpose of this study was to assess the GPs implication and the evolution of their practices as part of this new care. Materiel and method: we carried out a descriptive study between Decembre 11, 2019 and February 14, 2020, using a survey administered by email or telephone to each 130 GPs who took care of at least one patient with PrEP followed by the HDJ of the BUH. Result: among 97 GPs who responded (response rate 74.6%), 52 (53.6%) followed at least one patient alternately with the HDJ, and 22 (22.7%) GPs followed only one patient. A third of GPs didn’t know PrEP during the first alternate follow-up consultation and 76.9% did not refer the patient to this treatment. After the first consultation of the alternate follow-up, 67.3% of GPs thought that their practice in terms of HIV prevention had evolved and 82.7% that their knowledge of PrPE had improved. 23% were ready to initiate PrEP and provide full follow-up, 40% wished to extend alternate follow-up whilte agreeing to strat it, and 37% dit not feel ready. Conclusion: majority of GPs renewing PrEP noted an evolution in their practice and were ready to initiate it, althought most of them preferred to continue an alternate follow-up with an infectious disease specialist. Education is necessary to improve knowloedge of GPs who are already involved in PrEP and to modify the perceptions of those who are still reluctant to it in order to facilitate the access to this sort of prevention., Introduction : le nombre de nouvelles infections VIH était stable à près de 6500 par an avec les méthodes de prévention classiques. La prophylaxie pré-exposition (PrEP), accessible en France depuis janvier 2016 et renouvelable par les médecins généralistes (MG) depuis mars 2017, vient se rajouter aux précédentes dans une stratégie de prévention diversifiée. Depuis juillet 2018, l’hôpital de jour de maladies infectieuses (HDJ) de l’hôpital Saint-André (CHU de Bordeaux) propose systématiquement à chaque patient sous PrEP un suivi alterné par son médecin traitant. L’objectif de notre travail était d’étudier l’implication et l’évolution des pratiques des MG dans le suivi alterné ville/hôpital de leurs patients sous PrEP. Matériel et méthode : nous avons mené du 11 décembre 2019 au 14 février 2020 une étude descriptive à l’aide d’un questionnaire administré par mail ou par téléphone à l’ensemble des 130 MG ayant au moins un patient avec une PrEP suivi à l’hôpital de jour de l’hôpital Saint-André. Résultats : parmi les 97 MG ayant répondu (taux de réponse 74,6%), 52 (53,6%) suivaient en alternance avec l’hôpital au moins un patient, dont 22 un seul. Un tiers des MG ne connaissait pas La PrEP lors de la première consultation de suivi alterné et 76,9% n’étaient pas à l’origine de l’orientation du patient vers ce traitement. Après la première consultation du suivi alterné, 67,3% des MG pensaient que leur pratique en termes de prévention du VIH avait évoluée et 82,7% que leurs connaissances sur la PrEP s’étaient améliorées. Ils étaient 23% à être prêts à initier la PrEP et assurer un suivi complet, 40% à souhaiter prolonger le suivi alterné tout en acceptant de l’initier, et 37% à ne pas se sentir prêts. Conclusion : la majorité des médecins renouvelant la PrEP notait une évolution de leur pratique et était prête à l’initier, même si la plupart restait favorable à un suivi alterné avec un spécialiste infectiologue.

Introduction: from October 2018 until June 2019, the Bordeaux University Hospital has tested the SmartDerm teledermatology consultation system in the department of Lot-et-Garonne in order to mitigate medical desertification. The objective of this study was to analyze the use of SmartDerm by the general practitioners after 35 weeks of use. The primary endpoint was the average number of cases referred by general practitioner (GP) per week. Methods: the user data of the teleexpertise system were analyzed and a satisfaction questionnaire was sent to GPs and dermatologists of Lot-et-Garonne. Results: 26 GPs and one intern trained and equipped with the PAACO application sent at least one request for teleexpertise during this period. They sent 171 requests for teleexpertise, which corresponds to an average of one request per GP every 5 weeks and a half. This corresponds to 6 requests on average per doctor over the period. The average duration of a request for teleexpertise is 10 minutes. The median is 2 photos per request. 93% of the answers of teleexpertise are obtained by the applicant in less than 3 days. 57% of patients avoided face-to-face dermatological consultation. 18 of the 26 general practitioners (8 did not respond) and the intern were very satisfied with SmartDerm and considered that it has improved patient care. The experience of each user was evaluated by the System Usability Scale score; the average was 77 out of 100. It was positively related in a statistically significant way only at an input duration of less than 10 minutes. Conclusion: the number of requests for teleexpertise is lower than the average of 1 request every 2 weeks estimated during a preliminary study among training supervisors of New Aquitaine. The low level of involvement of level 1 dermatologists, the impossibility of coding teleexpertise acts to be paid for, and the time spent using the application may explain a lower than expected use of this teleexpertise system., Introduction : d’octobre 2018 à juin 2019, le CHU de Bordeaux a testé un dispositif de téléexpertise dermatologique SmartDerm dans le département du Lot-et-Garonne afin de pallier la déprise médicale. L’objectif de cette étude était d’analyser l’utilisation de SmartDerm par les médecins généralistes après 35 semaines d’utilisation. Le critère de jugement principal était le nombre moyen de cas adressés par médecin généraliste par semaine. Méthode : les données utilisateur de la plateforme de téléexpertise ont été analysées et un questionnaire de satisfaction a été adressé aux médecins généralistes et aux dermatologues du Lot-et-Garonne. Résultats : 26 médecins généralistes et un interne formés et équipés de l’application PAACO ont effectué au moins une demande de téléexpertise durant cette période. Ils ont envoyé 171 demandes de téléexpertise, soit en moyenne une demande par médecin généraliste toutes les 5 semaines et demi. Cela correspond à 6 demandes en moyenne par médecin sur la période. La durée moyenne de réalisation d’une demande de téléexpertise est de 10 minutes. La médiane est de 2 photos par demande. 93% des réponses de téléexpertise sont obtenues par le requérant en moins de 3 jours. 57% des patients ont évité une consultation dermatologique en présentielle. 18 des 26 médecins généralistes utilisateurs (8 n’ont pas répondu) ainsi que l’interne ont été très satisfaits de SmartDerm et ont considéré que ce dispositif a permis d’améliorer la prise en charge du patient. L’expérience de chaque utilisateur était évaluée par le score System Usability Scale, la moyenne était de 77 sur 100. Elle était liée positivement de manière statistiquement significative uniquement à une durée de saisie inférieure à 10 minutes. Conclusion : le nombre de demandes de téléexpertise est inférieur à la moyenne de 1 demande toutes les 2 semaines estimée lors d’une étude préalable auprès des maîtres de stage de Nouvelle-Aquitaine. La faible participation des dermatologues de niveau 1, l’impossibilité de réaliser une codification des actes de téléexpertise pour être rémunéré, et le temps passé à l’utilisation de l’application peuvent expliquer un usage plus faible que prévu de ce dispositif de téléexpertise.

Introduction : les femmes ayant des relations sexuelles avec des femmes (FSF) sont à risque d’infection par le Human Papillomavirus (HPV). Elles font pourtant moins fréquemment réaliser le dépistage du cancer du col de l’utérus par le frottis cervico-utérin (FCU). L’objectif de cette étude était de déterminer quels étaient les facteurs liés à la réalisation du FCU chez les FSF en France. Méthode : une enquête transversale anonyme auto-administrée basée sur le volontariat a été diffusée sur internet et les réseaux sociaux par des associations et médias LGBT ainsi que par effet boule de neige sous forme d’un questionnaire ciblant les FSF, les interrogeant sur leur historique sexuel, les expériences passées du FCU et des médecins généralistes et gynécologues, et leur parcours de santé. Les critères d’inclusion étaient d’être une femme majeure ou mineure émancipée cisgenre résidant en France et ayant eu au moins un rapport sexuel avec une femme au cours des douze derniers mois. Pour répondre à la question de recherche nous avons sélectionné les répondantes de 25 à 65 ans pour l’analyse, âge de recommandation du dépistage en France. Le critère principal de jugement était la date de réalisation du FCU et avait 3 modalités : moins de 3 ans, plus de 3 ans ou jamais. Les variables pour lesquelles p < 0.20 ont été sélectionnées pour la réalisation d’un modèle d’analyse multivariée. Résultats : notre échantillon était constitué de 711 FSF âgées de 25 à 65 ans de haut niveau d’éducation. La réalisation du FCU était liée à la perception de leur nécessité personnelle de réaliser le FCU et à la méconnaissance de leur risque personnel d’infection à HPV. Elle était également liée à l’activité sexuelle avec des hommes : les FSF ayant eu des rapports avec un homme au cours des douze derniers mois étaient 5.5 fois plus susceptibles d’être à jour de leur FCU par rapport aux FSF ayant eu exclusivement des relations sexuelles avec des femmes au cours de leur vie. Les pratiques de dépistage par le FCU différaient selon certains éléments du parcours de santé tels qu’avoir un antécédent de grossesse, d’utilisation de contraception ou encore de consultation d’un gynécologue. Enfin, la recommandation du FCU par un médecin triplait les chances d’être à jour. A l’inverse, pour 12% des FSF un médecin avait dit que la réalisation du FCU était inutile en raison de leur orientation sexuelle ce qui multipliait par 3.7 leur risque d’être en retard dans leur dépistage et par 6 leur risque de n’avoir jamais réalisé de FCU. Conclusion : les FSF doivent bénéficier de recommandations positives de réalisation du FCU de la part des médecins. Elles doivent être informées du risque de transmission du HPV entre femmes. Il est nécessaire d’inclure les spécificités de santé des FSF dans la formation médicale, et de les visibiliser dans des campagnes de prévention de la transmission des IST.

Introduction: sexually transmitted infections (STIs) are a major public health issue, both in France and worldwide. Primary healthcare professionals play a key role for sexual health and prevention, but few take on this subject. In order to develop a tool to make sexual prevention easier in primary care, we conducted a systematic review to analyze sexual risk behaviors assessment strategies and risk reduction interventions in OECD countries in primary healthcare settings. Methods: we searched for controlled trials (randomized or not), diagnostic accuracy studies and studies developing an assessment tool on the following databases: PubMed, Cochrane Library, Scopus, PsycINFO, PsycArticle, PBSC, SocINDEX, Google Scholar and CAIRN. Due to the diversity of interventions no meta-analysis was done. Results: 37 studies were included for analysis. We brought four kind of risk assessment tools to light: questionnaires, scores, event history calendars and risk stratification algorithms. There are efficient interventions for reducing sexual risk behaviors and/or incident STIs. Their efficiency seems to grow with the length and intensity of the intervention. Discussion: diverse tools can be used in primary care, with probable efficiency though some results are difficult to generalize. Very few assessment tools are studied and validated. Future research should also focus on older population given the epidemiological evolutions., Introduction : les infections sexuellement transmissibles (IST) représentent un enjeu majeur de santé publique, en France et à l’échelle mondiale. Les professionnels de soins primaires ont un rôle clé à jouer dans ce domaine mais peu se saisissent du sujet en consultation. Dans la perspective d’élaborer un outil facilitant la prévention en santé sexuelle en médecine générale, nous avons conduit une revue systématique de la littérature pour analyser les stratégies de repérage et interventions de réduction des comportements à risque d’IST proposées en soins primaires dans les pays de l’OCDE. Méthode : nous avons recherché des essais contrôlés (randomisés ou non), des essais diagnostiques et des études d’élaboration d’outils de repérage sur les bases de données PubMed, Cochrane Library, Scopus, PsycINFO, PsycArticle, PBSC, SocINDEX, Google Scholar et CAIRN. Les participants étaient de tous âges, sans restriction de genre ou d’orientation sexuelle, en soins primaires. Il n’a pas été réalisé de méta-analyse et les résultats sont présentés individuellement. Résultats : au total, 37 études ont été incluses dans l’analyse. Quatres familles d’outils de repérage ont été mises en évidence : questionnaires systématiques, scores, calendriers autobiographiques et algorithmes de stratification du risque. Il existe des interventions efficaces pour réduire le taux d’IST incidentes et/ou les comportements à risque d’IST, l’efficacité semble augmenter avec la durée de l’intervention. Discussion : il existe des outils divers utilisables en soins primaires, dont l’efficacité est probable bien que la possibilité de généraliser certains résultats pose question. Il n’existe que peu d’outils de repérage validés. Il faudrait également développer les recherches futures sur des populations moins jeunes au vu des évolutions épidémiologiques actuelles.

The problem of social inequalities in health is today one of the major public health axes for the development of new health and care policies. The repercussions of social inequalities and precariousness on health can no longer be demonstrated. The objective of this work is to study how these social inequalities are now addressed in major epidemiological studies. An article selection based on a literature review shows two categories of social health factors ; the so-called « structural » factors (employment, income, education) and « intermediaries »(social ties, psycho-social factors). The study of the ISS according to structural factors (level of education, employment and income) is done by geographical unit, the data come from regional or national statistical institutes. However, individual indicators (social interactions, housing characteristics, social cohesion, psychosocial factors) are most often favored by group studies. The importance of these immaterial variables, defined empirically, explains the discomfort to make consensus on the characterization of the ISS. The interest of the score EPICES resort by its propensity to make the link between these different components, in a standardized and validated way., La problématique des inégalités sociales de santé est aujourd’hui l’un des axes majeurs en santé publique pour le développement de nouvelles politiques de santé et de soins. La répercussion des inégalités sociales et de la précarité sur la santé n’est plus à démontrer. L’objectif de ce travail de thèse est d’étudier comment ces inégalités sociales sont aujourd’hui abordées dans les grandes enquêtes épidémiologiques. Une sélection d’articles sur la base d’une revue de littérature montre deux catégories de facteurs sociaux de santé, les facteurs dit « structurels » (emploi, revenu, éducation) et « intermédiaires » (liens sociaux, facteurs psycho-sociaux). L’étude des ISS selon des facteurs structurels se fait par unité géographique, les études par groupes, dont les données sont extraites des instituts statistiques régionaux ou nationaux. Cependant, des indicateurs individuels (interactions sociales, caractéristiques du logement, cohésion sociale, facteurs psycho-sociaux) sont à privilégier pour une optique de prévention et de repérage en soins primaires. L’importance de ces variables immatérielles, définies le plus souvent de manière empirique, explique la gêne à faire consensus quant à la caractérisation des ISS. Le score EPICES, validé, individuel et personnalisé, a toute sa place à prendre dans la pratique clinique du médecin de premier recours.

Introduction: General practitioners (GPs) are confronted with increasing rates of antibiotic-resistance in bacteria causing urinary tract infections (UTIs) to antibiotics most commonly prescribed in UTI treatment. As a result, GPs must adapt their prescribed treatments in response to the changing bacterial ecology. Objective: To evaluate whether antibiotics prescribed by GPs for the empirical treatment of UTIs showed a rate of bacterial resistance of less than 20% in uncomplicated cystitis and less than 10% in complicated cystitis, pyelonephritis, and men urinary tract infections. Patient and methods: Prospective, multicenter study of ambulatory medicine in the Aquitain region of France. This study included 392 patients with suspected UTIs, seen by 38 participating GPs between January and July 2014. Following the initial consultation, a urine culture was ordered for each patient, regardless of the chosen treatment strategy. A clinical and epidemiological survey specified the empirical course of antibiotic therapy. Results: In the uncomplicated cysticis, the majority of antibiotics showed a resistance rate of less than 20%. Fosfomycin was prescribed in first intention (61.7% of prescriptions, resistance rate 10.5%). Fluoroquinolones were most commonly prescribed as an empiric first-choice antibiotic for complicated cystitis (46.5%, resistance 10.9%), pyelonephritis (73.7%, resistance 5.3%), and UTIs in male patients (69.4%, resistance 12.5%). In complicated cystitis and male UTIs, the resistance rate was frequently more than 10%. Conclusions: Due to the bacterial resistance of pathogens found in complicated cystitis and male IUTs, it is necessary to wait for the results of the urine culture before treating patients. For this reason, it is important to postpone the beginning of antibiotic treatment whenever is possible.; Introduction : Les médecins généralistes (MG) sont confrontés à une augmentation des résistances bactériennes aux antibiotiques les plus utilisés dans les infections urinaires (IU) et doivent en conséquence adapter leurs prescriptions à l’évolution de l’écologie bactérienne. Objectif : Évaluer si les antibiotiques prescrits en première intention par les MG dans les IU présentaient un seuil de résistance inférieur à 20% pour les cystites aiguës simples et à 10% pour les cystites à risque de complication, les pyélonéphrites aiguës et les IU masculines. Méthodes : Étude prospective multicentrique, en médecine ambulatoire en Aquitaine. De janvier à juillet 2014, 38 MG maîtres de stage ont inclus 392 patients présentant une suspicion d’infection urinaire. Un examen cytobactériologique des urines (ECBU) était systématiquement prescrit à l‘issue de la consultation, quelle que soit la stratégie thérapeutique choisie. Un questionnaire clinique et épidémiologique précisait l’antibiothérapie prescrite en première intention. Résultats : Dans les cystites aiguës simples, la majorité des antibiotiques avait un taux de résistance inférieur à 20%. La fosfomycine trométamol était prescrite en première intention (61,7% des prescriptions, taux de résistance 10,5%). Les fluoroquinolones étaient les plus prescrites pour les cystites aiguës à risque de complication (46,5%, résistance 10,9%), les pyélonéphrites (73,7%, résistance 5,3%), ou les IU masculines (69,4%, résistance 12,5%). Dans les cystites aiguës compliquées et les IU masculines, le taux de résistance des antibiotiques dépassait fréquemment le seuil de 10%. Conclusion : Le profil de résistance des germes dans les cystites aiguës compliquées et les IU masculines confirme la nécessité devant une suspicion d’IU d’attendre le résultat de l’ECBU avant de traiter. D’où l’importance de différer la prise d’antibiotique lorsque cela est possible.

Introduction : Resistance of bacterias to antibiotics represents a major public health issue and requires a rational use of antibiotics to fight this real danger. The aim of our study was to clarify the clinical determinants of the antibiotic prescription in acute otitis media in general practice and to analyse the quality of the antibiotic prescription (molecule choice, posology, length of treatment). Patients and methods : we started from a prospective investigation on the upcoming event AOM seen in consultation. Two physician groups are studied and compared : on one hand, the general practitioners from Aquitaine watched by their resident (group 1), and on the other hand, the substitute general practitioners group (group 2). A form was sent to both groups. Results : Forty eight general practitioners in group 1 and 95 in group 2 are included. Fever, intensity of pain and bulging tympanic membrane were the main factors influencing antibiotic prescription. Severe earache, fever above 39°C, and otorrhoea appeared as factors of systematic antibiotic prescription. This study shows that 60% and 68% of congestive eardrums at otoscopy, respectively in groups 1 and 2 received antibiotherapy. Amoxicillin is the most prescribed molecule in both groups. Respectively 45 and 22% of the prescriptions, in groups 1 and 2, are under recommended posology. Conclusion : The antibiotic prescription in AOM, influenced by some clinical elements of seriousness, is too often not justified. A better education of general practitioners and patients on the benignity of AOM could decrease antibiotic prescriptions., Introduction : La résistance des bactéries aux antibiotiques pose un problème de santé publique majeur et nécessite une utilisation rationnelle des antibiotiques pour lutter contre ce fléau. L’objectif de notre étude était de préciser les déterminants cliniques de la prescription antibiotique dans l’otite moyenne aiguë (OMA) en médecine générale et d’analyser la qualité de la prescription antibiotique (choix de la molécule, posologie, durée du traitement). Matériels et méthodes : il s’agissait d’une enquête prospective sur le prochain cas d’OMA vu en consultation. Deux groupes de médecins ont été étudiés et comparés : le groupe des maîtres de stages aquitains (MDS), observés par leur interne d’une part, et un groupe de médecins généralistes remplaçants d’autres part (MR). L’étude consistait à répondre à un questionnaire envoyé à ces 2 groupes de médecins. Résultats : Quarante-huit MDS et 95 MR ont été inclus. La fièvre, l’intensité de la douleur et l’aspect bombé du tympan étaient les principaux facteurs influençant la prescription antibiotique. L’otalgie sévère, une température supérieure à 39°C, ainsi que la présence d’une otorrhée sont apparus comme des facteurs de prescription systématique d’antibiotiques. Cette étude montre que 60 et 68 % des tympans congestifs à l’otoscopie, respectivement des groupes MDS et MR, ont reçus une antibiothérapie. L’amoxicilline est la molécule la plus prescrite dans les 2 groupes. Respectivement 45 et 22 % des prescriptions, dans les groupes MDS et MR, sont à doses infra-posologiques. Conclusion : La prescription antibiotique dans l’OMA, influencée par certains éléments cliniques de gravité, est trop souvent non justifiée. Une meilleure sensibilisation des professionnels et des patients sur la bénignité de cette pathologie permettrait de diminuer ces prescriptions.

Objectives: The urinary tract infection (UTI) is the 2nd motive for prescription of antibiotics in primary care. There are clinical scores and the use of the urinary dispticks is recommended to limit the prescription. The objective of our study was to determine factor influencing the general practitioner (GP) in its prescription of empiric antibiotic therapy in case of suspicion of low urinary tract infection to woman. Methods: From AquiBMR, prospective study led from February till July, 2014 with 38 GP, we realized a study concerning the suspicions of low UTI at the woman. We made a multivariated analysis with the software R on the different factors influencing the prescription of antibiotic after adjustment on the possible confounders. Results: Among 306 women suspected to present an UTI, 238 (77,8%) received an antibiotic. The clinical signs associated to this prescription were burning sensations (OR 3.89, p 0.00002) and urgency (OR 2.66, p 0.01). The signs associated negatively were the age upper to 75 years (OR 0.3, p 0.02) a vaginal discharge (OR 0.29, p 0.05) or others symptoms (0.2, p 0.03). An urinary disptick was ealized at 237 women (77,4 %). 84,1% of prescription if positive and 44,8 % if negative. The fact of realizing a urinary dipstick was not significantly associated with a lesser prescription (OR 1.66, p 0.18) after adjustment on the clinical signs. Conclusions: The GP prescribes a treatment antibiotic more easily as the patient is young and present typical and noisy symptoms. The realization of an urinary disptick is not a significant protective factor of the prescription in multivariated analysis.; Objectif : Les infections urinaires (IU) sont le 2e motif de prescription d’antibiotiques en soins primaires. Il existe des scores cliniques et l’utilisation de la bandelette urinaire est préconisée pour en limiter la prescription. L’objectif de notre étude était de déterminer les facteurs influençant le médecin généraliste (MG) dans sa prescription d’antibiothérapie probabiliste en cas de suspicion d’IU basse chez la femme. Matériel et méthodes : À partir d’AquiBMR, étude prospective menée de février à juillet 2014 auprès de 38 MG, nous avons réalisé une étude portant sur les suspicions d’IU basse chez la femme. Nous avons effectué une analyse multivariée avec le logiciel R sur les différents facteurs influençant la prescription d’antibiotique après ajustement sur les éventuels facteurs de confusion. Résultats : Parmi les 306 femmes suspectées de présenter une IU, 238 (77,8%) ont reçu d’emblée un traitement antibiotique. Les signes cliniques associés à cette prescription étaient des brûlures mictionnelles (OR 3.89, p 0.00002) et une impériosité mictionnelle (OR 2.66, p 0.01). Les signes associes négativement étaient l’âge supérieur à 75 ans (OR 0.3, p 0.02) un prurit (OR 0.29, p 0.05) ou des symptômes non listes (OR 0.2, p 0.03). La BU a été réalisée chez 237 patientes (77,4%). Elle entraînait 84,1 % de prescription d’emblée si positive et 44,8% si négative. Le fait de réaliser une BU n’était pas associé significativement à une moindre prescription (OR 1.66, p 0.18) après ajustement sur les signes cliniques. Conclusion : Les MG prescrivent un traitement antibiotique d’emblée d’autant plus facilement que la patiente est jeune et présente des symptômes typiques et bruyants. La réalisation de la BU n’est pas un facteur protecteur significatif de la prescription en analyse multivariée.
