Abstract

The 2022 report of the Intergovernmental Panel on Climate Change (IPCC) paints a dark picture of the future of life on earth, characterised by ecosystem collapse, species extinction, and climate hazards such as heatwaves and floods (1). These are all linked to physical and mental health problems, with direct and indirect consequences of increased morbidity and mortality. To avoid these catastrophic health effects across all regions of the globe, there is broad agreement—as 231 health journals argued together in 2021—that the rise in global temperature must be limited to less than 1.5oC compared with pre-industrial levels. While the Paris Agreement of 2015 outlines a global action framework that incorporates providing climate finance to developing countries, this support has yet to materialise (2). COP27 is the fifth Conference of the Parties (COP) to be organised in Africa since its inception in 1995. Ahead of this meeting, we—as health journal editors from across the continent—call for urgent action to ensure it is the COP that finally delivers climate justice for Africa and vulnerable countries. This is essential not just for the health of those countries, but for the health of the whole world. Africa has suffered disproportionately although it has done little to cause the crisis The climate crisis has had an impact on the environmental and social determinants of health across Africa, leading to devastating health effects (3). Impacts on health can result directly from environmental shocks and indirectly through socially mediated effects (4). Climate change-related risks in Africa include flooding, drought, heatwaves, reduced food production, and reduced labour productivity (5). Droughts in sub-Saharan Africa have tripled between 1970-79 and 2010-2019 (6). In 2018, devastating cyclones impacted three million people in Malawi, Mozambique and Zimbabwe (6). In west and central Africa, severe flooding resulted in mortality and forced migration from loss of shelter, cultivated land, and livestock (7). Changes in vector ecology brought about by floods and damage to environmental hygiene has led to increases in diseases across sub-Saharan Africa, with rises in malaria, dengue fever, Lassa fever, Rift Valley fever, Lyme disease, Ebola virus, West Nile virus and other infections (8, 9). Rising sea levels reduce water quality, leading to water-borne diseases, including diarrhoeal diseases, a leading cause of mortality in Africa (8). Extreme weather damages water and food supply, increasing food insecurity and malnutrition, which causes 1.7 million deaths annually in Africa (10). According to the Food and Agriculture Organization of the United Nations, malnutrition has increased by almost 50% since 2012, owing to the central role agriculture plays in African economies (11). Environmental shocks and their knock-on effects also cause severe harm to mental health (12). In all, it is estimated that the climate crisis has destroyed a fifth of the gross domestic product (GDP) of the countries most vulnerable to climate shocks (13). The damage to Africa should be of supreme concern to all nations. This is partly for moral reasons. It is highly unjust that the most impacted nations have contributed the least to global cumulative emissions, which are driving the climate crisis and its increasingly severe effects. North America and Europe have contributed 62% of carbon dioxide emissions since the Industrial Revolution, whereas Africa has contributed only 3% (14). The fight against the climate crisis needs all hands on deck Yet it is not just for moral reasons that all nations should be concerned for Africa. The acute and chronic impacts of the climate crisis create problems like poverty, infectious disease, forced migration, and conflict that spread through globalised systems (6, 15). These knock-on impacts affect all nations. COVID-19 served as a wake-up call to these global dynamics and it is no coincidence that health professionals have been active in identifying and responding to the consequences of growing systemic risks to health. But the lessons of the COVID-19 pandemic should not be limited to pandemic risk (16, 17). Instead, it is imperative that the suffering of frontline nations, including those in Africa, be the core consideration at COP27: in an interconnected world, leaving countries to the mercy of environmental shocks creates instability that has severe consequences for all nations. The primary focus of climate summits remains to rapidly reduce emissions so that global temperature rises are kept to below 1.5 °C. This will limit the harm. But, for Africa and other vulnerable regions, this harm is already severe. Achieving the promised target of providing $100bn of climate finance a year is now globally critical if we are to forestall the systemic risks of leaving societies in crisis. This can be done by ensuring these resources focus on increasing resilience to the existing and inevitable future impacts of the climate crisis, as well as on supporting vulnerable nations to reduce their greenhouse gas emissions: a parity of esteem between adaptation and mitigation. These resources should come through grants not loans, and be urgently scaled up before the current review period of 2025. They must put health system resilience at the forefront, as the compounding crises caused by the climate crisis often manifest in acute health problems. Financing adaptation will be more cost-effective than relying on disaster relief. Some progress has been made on adaptation in Africa and around the world, including early warning systems and infrastructure to defend against extremes. But frontline nations are not compensated for impacts from a crisis they did not cause. This is not only unfair, but also drives the spiral of global destabilisation, as nations pour money into responding to disasters, but can no longer afford to pay for greater resilience or to reduce the root problem through emissions reductions. A financing facility for loss and damage must now be introduced, providing additional resources beyond those given for mitigation and adaptation. This must go beyond the failures of COP26 where the suggestion of such a facility was downgraded to “a dialogue” (18). The climate crisis is a product of global inaction, and comes at great cost not only to disproportionately impacted African countries, but to the whole world. Africa is united with other frontline regions in urging wealthy nations to finally step up, if for no other reason than that the crises in Africa will sooner rather than later spread and engulf all corners of the globe, by which time it may be too late to effectively respond. If so far they have failed to be persuaded by moral arguments, then hopefully their self-interest will now prevail. This Comment is being published simultaneously in multiple journals. Abstract

This study aimed to describe real-life microbiological testing of adults hospitalised with community-acquired pneumonia (CAP) and to assess concordance with the 2007 Infectious Diseases Society of America (IDSA)/American Thoracic Society (ATS) and 2011 European Respiratory Society (ERS) CAP guidelines. This was a cohort study based on the Global Initiative for Methicillin-resistant Staphylococcus aureus Pneumonia (GLIMP) database, which contains point-prevalence data on adults hospitalised with CAP across 54 countries during 2015. In total, 3702 patients were included. Testing was performed in 3217 patients, and included blood culture (71.1%), sputum culture (61.8%), Legionella urinary antigen test (30.1%), pneumococcal urinary antigen test (30.0%), viral testing (14.9%), acute-phase serology (8.8%), bronchoalveolar lavage culture (8.4%) and pleural fluid culture (3.2%). A pathogen was detected in 1173 (36.5%) patients. Testing attitudes varied significantly according to geography and disease severity. Abstract

I. Objective: to highlight the clinico-MRI prole of lumbar disc herniation on sciatic lumboradiculgia in bi- river hospital settings (cases of Kinshasa and Brazzaville). This was aII. Materials and methods: retrospective, documentary and multicenter study of 302 patients who performed lumbar MRI examinations in 18 months in hospitals in Kinshasa and Brazzaville. The mean age of patients was 53.4 years. The most represented age groupIII. Results: was 50 to 59 years old. The female sex was predominant (55.4%) with a ratio of 0.79. The majority of patients (68.2%) came from Kinshasa. Lumboradiculalgia was the most predominant indication with (49.3%), followed by low back pain (48%). The majority of MRI examinations (91.4%) were performed without injection of contrast product. T1 and T2 weightings were performed in 100% of patients. Diseased disc degeneration was found at (41.7%) and lumbar disc herniation at (27.2%). The MRI examination was normal in (47%) of the patients. Based on potentially clinically positive relevance, only 48.7% of our MRI diagnoses were clinically positive. Age (p=0.000), lumboradiculalgia (p=0.000) and specialist physician prescription (0.039) had a statistically signicant relationship with clinically positive diagnosis. HDL is a pathological reality in the hospitals ofIV. Conclusion: Kinshasa and Brazzaville. The MRI prole found mainly corroborates the observations of the literature. Lumbar disc herniation and degenerative disc disease remain the most frequently encountered pathologies on magnetic resonance imaging in hospitals in Kinshasa and Brazzaville. They are at the origin of the lumbar spinal syndrome and affect the young population. The posterolateral disc herniation remains the most frequently encountered subtype with predominance of damage to the oors: L4-L5 and L5-S1.

Abstract

The 2022 report of the Intergovernmental Panel on Climate Change (IPCC) paints a dark picture of the future of life on earth, characterised by ecosystem collapse, species extinction, and climate hazards such as heatwaves and floods (1). These are all linked to physical and mental health problems, with direct and indirect consequences of increased morbidity and mortality.COP27 is the fifth Conference of the Parties (COP) to be organised in Africa since its inception in 1995. Ahead of this meeting, we—as health journal editors from across the continent—call for urgent action to ensure it is the COP that finally delivers climate justice for Africa and vulnerable countries. This is essential not just for the health of those countries, but for the health of the whole world.

Abstract

Pulmonary rehabilitation (PR) is an integral part of the management of patients with chronic respiratory diseases. However, there is limited information available on the effectiveness and practice of PR in Africa. This study was conducted to examine the prevalence, structure, and organization of PR in Africa, as well as its substance and claimed efficacy. We conducted a multimethod study involving systematic review of PR studies (obtained from PubMed, Google Scholar, and Cochrane databases) and a web-based survey of African healthcare professionals engaged in PR (using a standardized questionnaire). The review included papers on at least one component of PR in Africa and excluded those on PR from other continents or assessing pulmonary disorders in general without PR, cardio-rehabilitation, or physiotherapy practice in general in Africa. The Cochrane risk of bias and the Newcastle Ottawa scale instruments were used to assess the quality of included studies. We narratively synthesised data across the studies to produce a holistic picture. Of the 14 studies included for qualitative synthesis, seven were randomized controlled trials on the effectiveness of PR treatments with a total number of 333 participants. Of the 39 surveys mailed to health professionals working in Africa, only 14 (35.8%) were returned. We found aerobic exercise and breathing exercises were the most used technique and that quality of life, exercise capacity, and lung function improved significantly after PR treatments. There were differences in the duration, frequency, and length of the programs across the continent. Half of the respondents indicated that their institutions had one or more PR programs for inpatient, outpatient, maintenance, and/or home-based programs. Additionally, aerobic activities, upper and lower extremity strength training were the most frequently used exercise modalities in PR programs, followed by breathing exercises. Pulmonary rehabilitation is understudied in Africa, but it has been linked to improved lung function, exercise capacity, and quality of life. Abstract

Introduction:l´objectif principal de cette étude, consiste à ressortir les aspects tomodensitométriques thoraciques (TDM) observés lors de la première vague de la pandémie à SARS-CoV-2 chez les patients hospitalisés pour pneumonie à COVID-19 aux Cliniques Universitaires de Kinshasa (CUK). Méthodes:étude descriptive portant sur les examens TDM thoraciques des 26 patients hospitalisés pour pneumonie à COVID-19 aux CUK sur une période de 9 mois, allant de 17 mars au 17 novembre 2020. Un appareil TDM 16 barrettes de marque Hitachi a été utilisé chez tous nos patients. Après analyse, les lésions trouvées ont été réparties en lésions évocatrices et non évocatrices de l´infection à SARS-CoV-2. Résultats:l´âge médian des patients était de 53,02 ans. Le sexe masculin était le plus touché avec 76,9%. La détresse respiratoire était le symptôme clinique le plus rencontré avec 61,5%. Comme comorbidité, l´hypertension artérielle (HTA) et l´insuffisance rénale étaient les plus retrouvées avec 30% et 6%. Les opacités en Verre dépoli bilaterales avec prédominance périphérique étaient prédominantes (69,2%), suivies des condensations (57,7%) et des crazy paving (19,2%). Le stade sévère était le plus fréquemment rencontré (34,61%). L´embolie pulmonaire distale et proximale était la complication la plus fréquente (11,5%). Parmi les pathologies associées la pleurésie et l´HTA pulmonaire étaient les plus rencontrées (30,8%). La majorité de nos patients, ont présentés des lésions parenchymateuses pulmonaires correspondant au stade TDM précoce de la maladie (50%). Conclusion:les lésions TDM évocatrices de COVID-19, observées aux CUK lors de la première vague sont dominées par les plages d´opacités en verre dépoli, suivies des condensations parenchymateuses non systématisées et des crazy paving. Les lésions atypiques moins observées, étaient constituées des condensations pseudo-nodulaires, d´atteinte unilatérale, d´atteinte péribronchovasculaire et d´infection sur le poumon remanié. Le stade TDM sévère était la plus fréquente. L´embolie pulmonaire proximale et distale était la complication la plus rencontrée. Les aspects trouvés corroborent majoritairement les observations de la littérature.

Abstract

Background: For countries with limited resources such as the Democratic Republic of the Congo (DRC), the diagnosis of Multidrug-resistant tuberculosis (MDR-TB) is still insufficient. The MDR-TB identification is done primarily among at-risk groups. The knowledge of the true extent of the MDR-TB remains a major challenge. This study tries to determine the proportion of MDR-TB in each group of presumptive MDR-TB patients and to identify some associated factors. Methods: This is an analysis of the DRC surveillance between 2007 and 2016. The proportions were expressed in Percentage. The logistic regression permits to identify the associated factors with the RR-/MDR-TB with adjusted Odds-ratio and 95% CI. Significance defined as p ≤ 0.05. Results: Overall, 83% (5407/6512) of the MDR-TB presumptive cases had each a TB test. 86.5% (4676/5407) had each a culture and drug sensitive testing (DST) on solid medium, and 24.3% (1312/5407) had performed an Xpert MTB/RIF test. The proportion of those with at least one first-line drug resistance was 59.3% [95% CI 57.2 - 61.4] among which 50.1%, [95% CI 47.9 - 52.3] for the isoniazid, 45.6% [95% CI 43.4 - 47.8] for the rifampicin, 49.9% [95% CI 47.8 - 52.1] for ethambutol and 35.8% [95% CI 33.7 - 37.9] for streptomycin. The confirmation of MDR-TB was 42.8% [95% CI 38.4 - 47.8]. Combining both tests, the proportion of RR-/MDR-TB was 49.6% [95% CI 47.9 - 51.4] for all presumptives. This proportion was 60.0% for failures, 40.7% for relapses and 34.7% for defaulters. Associated factors with the diagnosis of MDR-TB were: aged less than 35 years; prior treatment failure; defaulters; the delay between the collection of sputum and the test completion. Conclusion: The proportion of RR-/MDR-TB among the presumptives has been higher than those estimated generally. The National tuberculosis programme (NTP) should improve patient follow-up to reduce TB treatment failures and defaulting. Moreover, while increasing the use of molecular tests, they should reduce sample delivery times when they use culture and DST concomitantly.

Abstract

The Democratic Republic of Congo is one of the countries with a high preva-lence of multidrug-resistant tuberculosis (MDR-TB) in Africa and worldwide. This study aims to describe the trends and level of screening since 2013. This is a retrospective descriptive study based on quarterly reports from the provinces. The number of cases, past histories, age, sex and HIV Co-infection was identi-fied. It emerges that the number of cases increases significantly but does not reach the forecasts yet (912 cases of MDR/RR and XDR diagnosed in 2017 out of 3948 cases expected with a gap of 77%). Patients with a history of TB pre-dominate (63.5% in 2017); but the proportion of patients with no history of TB increases (36.5% in 2017). This suggests high rates among contacts. Women (37.8%) and children under 15 (2.7%) are less represented. TB/HIV Co-infection is 12% as for sensitive cases. Efforts still need to be made to improve screening.

Abstract

Background: Aspiration community-acquired pneumonia (ACAP) and community-acquired pneumonia (CAP) in patients with aspiration risk factors (AspRFs) are infections associated with anaerobes, but limited evidence suggests their pathogenic role. Research Question: What are the aspiration risk factors, microbiology patterns, and empiric anti-anaerobic use in patients hospitalized with CAP? Study Design and Methods: This is a secondary analysis of GLIMP, an international, multicenter, point-prevalence study of adults hospitalized with CAP. Patients were stratified into three groups: (1) ACAP, (2) CAP/AspRF+ (CAP with AspRF), and (3) CAP/AspRF- (CAP without AspRF). Data on demographics, comorbidities, microbiological results, and anti-anaerobic antibiotics were analyzed in all groups. Patients were further stratified in severe and nonsevere CAP groups. Results: We enrolled 2,606 patients with CAP, of which 193 (7.4%) had ACAP. Risk factors independently associated with ACAP were male, bedridden, underweight, a nursing home resident, and having a history of stroke, dementia, mental illness, and enteral tube feeding. Among non-ACAP patients, 1,709 (70.8%) had CAP/AspRF+ and 704 (29.2%) had CAP/AspRF-. Microbiology patterns including anaerobes were similar between CAP/AspRF-, CAP/AspRF+ and ACAP (0.0% vs 1.03% vs 1.64%). Patients with severe ACAP had higher rates of total gram-negative bacteria (64.3% vs 44.3% vs 33.3%, P =.021) and lower rates of total gram-positive bacteria (7.1% vs 38.1% vs 50.0%, P 50% in all groups) independent of AspRFs or ACAP received specific or broad-spectrum anti-anaerobic coverage antibiotics. Interpretation: Hospitalized patients with ACAP or CAP/AspRF+ had similar anaerobic flora compared with patients without aspiration risk factors. Gram-negative bacteria were more prevalent in patients with severe ACAP. Despite having similar microbiological flora between groups, a large proportion of CAP patients received anti-anaerobic antibiotic coverage.

Abstract

An accurate knowledge of the epidemiology of community-acquired pneumonia (CAP) is key for selecting appropriate antimicrobial treatments. Very few etiological studies assessed the appropriateness of empiric guideline recommendations at a multinational level. This study aims at the following: (i) describing the bacterial etiologic distribution of CAP and (ii) assessing the appropriateness of the empirical treatment recommendations by clinical practice guidelines (CPGs) for CAP in light of the bacterial pathogens diagnosed as causative agents of CAP. Secondary analysis of the GLIMP, a point-prevalence international study which enrolled adults hospitalized with CAP in 2015. The analysis was limited to immunocompetent patients tested for bacterial CAP agents within 24 h of admission. The CAP CPGs evaluated included the following: the 2007 and 2019 American Thoracic Society/Infectious Diseases Society of America (ATS/IDSA), the European Respiratory Society (ERS), and selected country-specific CPGs. Among 2564 patients enrolled, 35.3% had an identifiable pathogen. Streptococcus pneumoniae (8.2%) was the most frequently identified pathogen, followed by Pseudomonas aeruginosa (4.1%) and Klebsiella pneumoniae (3.4%). CPGs appropriately recommend covering more than 90% of all the potential pathogens causing CAP, with the exception of patients enrolled from Germany, Pakistan, and Croatia. Abstract

Background: The diagnosis of hypertension in children and adolescents is not always easy. The blood pressure-to-height ratios (BPHR) have been proposed as a screening tool for diagnosing hypertension. Objective: To evaluate the diagnostic value of BPHR for detecting hypertension in young adults. Methods: A cross-sectional study was conducted among 12621 healthy young adults aged of 18-25 years in Kinshasa, from July 2018 to February 2019. Systolic blood pressure-to-height ratio (SBPHR) was calculated as systolic blood pressure (SBP) in mmHg devised by height in centimeter. Diastolic blood pressure-to-height ratio (DBPHR) was calculated as diastolic blood pressure (DBP) in mmHg devised by height in centimeter. The receiver operating characteristics (ROC) curve analysis had allowed determining the cut-off points of SBPHR and DBPHR to detect hypertension. Results: The prevalence of hypertension, prehypertension, obesity and overweight were 10.5%, 9.2%, 3.0% and 22.4% respectively. The median values of SBPHR were 0.75 (0.69-0.79) mmHg/cm and 0.77 (0.67-0.80) mmHg/cm respectively in men and in women. The median values of DBPHR were 0.48 (0.44-0.53) mmHg/cm and 0.51 (0.49-0.56) mmHg/cm respectively in men and in women. Optimal cutoff points of SBPHR to identify hypertensionwere0.81mmHg/cm and 0.89 mmHg/cm respectively in men and in women. The threshold of DBPHR to detect hypertension was 0.54 mmHg/cm in both men and women. SBPHR and DBPHR had high sensitivity and specificity for identifying hypertension. Conclusion: SBPHR and DBPHR can be used to detect hypertension in Bantu young adults in Kinshasa.

Abstract

Background: To date, world widely, only a couple of papers have reported the association between pulmonary tuberculosis, Coronavirus disease 2019 (COVID-19) and pregnancy, and none of these reports was from sub-Saharan Africa where tuberculosis is endemic. Objective: the main objective of this study is to describe the co-infection Pulmonary Tuberculosis and Severe COVID-19 in pregnant young Woman at the University Hospital of Kinshasa. Method: The report case is of a pregnant woman aged 19 (Pare 2, Gesture 2, Abortion 0) with no known significant medical history, at 32 weeks of gestation based last menstrual period She has benefited from clinic examination, biological examinations (Ziehl’s on sputum), a chest CT scan and a morphological ultrasound. Result: On admission, COVID-19 was the only working diagnosis. However, the persistent coughing prompted clinicians to request a Ziehl-Neelsen staining of sputum that revealed the diagnosis of pulmonary TB. The reverse-transcription polymerase chain reaction (RT-PCR)-confirmed COVID-19 infection and HIV serology negative. A contrast-enhanced chest computed tomography (CT) showed airspace disease involving the right upper lobar, right medial basal segment and left upper lobe in the background of diffuse micro-nodular opacities favored to represent military pulmonary tuberculosis. There were associated cystic bronchiectasis in bilateral upper lobe and bilateral small amount of pleural effusion. Aforementioned findings were favored to represent a secondary or reactivation tuberculosis. The obstetrical ultrasound showed a single live intrauterine pregnancy in breech presentation, estimated at 34 weeks 3 days of gestation without usual features including detectable congenital malformation. Conclusion: The outcome of a pregnant woman with simultaneous COVID-19 and pulmonary tuberculosis is improved when the diagnosis is made early and management is promptly initiated. This attitude also improves the fetal prognosis. In the context of the COVID-19, the association of COVID-19 and pulmonary tuberculosis, especially in immunocompromised patients should be considered.

Abstract

Objective: To evaluate the effects of “strength-velocity” training on the evolution of some cardiorespiratory parameters in postpubertal judoists of the Congolese national elite. Methods: The survey, exploratory, experimental and longitudinal, was about 14 boys, belonging to seven categories of weight of the International Federation of Judo. These judoists, at the level belonged to stages 3, 4 and 5 of the classification to Tanner, of pubertal maturation. Their median age was 16 years (range: 15 - 17 years). All topics have been submitted to “strength-velocity” training, focused around exercises bound to the development of the power and automatisms, from plyometric and isometric training. Three assessments have been realized in the beginning, to mid-training (3 months) and at the end of the program (6 months). The measured values were cardiorespiratory parameters. Results: The heart rate to the doorstep was decreased significantly (p = 0.048), so VO2 max (p = 0.046), FVC (p = 0.0244) and ratio VEMS/FVC (p = 0.046) as function of pubertal stage. In contrast, increase was not significant for VEMS (p = 0.205) and absolute VO2 to doorstep (p = 0.097). No significant effect of pubertal state on respiratory frequency was found. On the other hand, strength-velocity training improved cardiorespiratory data. To the recovery of the “strength-velocity test”, the decrease of the VO2 took place more quickly that the one of the VE, the VCO2 and the HR, variations linked to the category of topic weight. Conclusion: Among high level Congolese postpubertal judoists, the cardiorespiratory modifications induce faculty to the realization of a good performance and pursue objectives of health. However, it is necessary to be heedful as for situations to put in place in sittings at the time of such of “strength-velocity” training.

Abstract

This study aimed at assessing the serological and virological status of Rift Valley fever virus (RVFV) in cattle from four climatically diverse zones of the Democratic Republic of the Congo (DRC). A total of 1675 sera samples collected between 2014 and 2015 from cattle without clinical manifestation of RVF infection were tested using competitive and capture enzyme ELISA to detect both IgG and IgM. RT-PCR was used for the detection of nucleic acid of RVFV. Out of the 1675 cattle sera tested, 203 were found to be IgG-positive, giving an overall true seroprevalence of 12.37% (95% CI 10.86–14.05). This seroprevalence varied between the four zones with a seroprevalence of 16.16% (95% CI 12.86–20.12), 14.70% (95% CI 11.72–18.29), 10.82% (95% CI 7.19–14.19), and 7.34% (95% CI 5.13–10.41) recorded in cattle sampled in the mountainous, humid savannah, dry savannah, and forest zones, respectively (p 3 years, respectively, although the difference was not statistically significant (p > 0.05, χ2 = 2.95). Similarly, no statistically significant difference (p > 0.05, χ2 = 0.04) was found between the sexes of the animals. Among the IgG-positive samples screened for anti-RVFV IgM, only 1.47% (3/203) was IgM-positive. One of the IgM-positive samples was positive by RT-PCR. These findings reveal country-wide distribution of RVF in the DRC for the first time.

Abstract

BACKGROUND: Antibiotic resistance is a major global health problem and pathogens such as meticillin-resistant Staphylococcus aureus (MRSA) have become of particular concern in the management of lower respiratory tract infections. However, few data are available on the worldwide prevalence and risk factors for MRSA pneumonia. We aimed to determine the point prevalence of MRSA pneumonia and identify specific MRSA risk factors in community-dwelling patients hospitalised with pneumonia.METHODS: We did an international, multicentre study of community-dwelling, adult patients admitted to hospital with pneumonia who had microbiological tests taken within 24 h of presentation. We recruited investigators from 222 hospitals in 54 countries to gather point-prevalence data for all patients admitted with these characteristics during 4 days randomly selected during the months of March, April, May, and June in 2015. We assessed prevalence of MRSA pneumonia and associated risk factors through logistic regression analysis.FINDINGS: 3702 patients hospitalised with pneumonia were enrolled, with 3193 patients receiving microbiological tests within 24 h of admission, forming the patient population. 1173 (37%) had at least one pathogen isolated (culture-positive population). The overall prevalence of confirmed MRSA pneumonia was 3·0% (n=95), with differing prevalence between continents and countries. Three risk factors were independently associated with MRSA pneumonia: previous MRSA infection or colonisation (odds ratio 6·21, 95% CI 3·25-11·85), recurrent skin infections (2·87, 1·10-7·45), and severe pneumonia disease (2·39, 1·55-3·68).INTERPRETATION: This multicountry study shows low prevalence of MRSA pneumonia and specific MRSA risk factors among community-dwelling patients hospitalised with pneumonia.FUNDING: None.
