Ensemble, ils échangent sur différentes facettes de la mort, de la vie, mais… surtout de la mort. Ismaël est très fier. À l'école des sorciers, il a reçu le tout premier prix de magie. Il marche avec assurance dans la forêt en pensant à tous les tours qu'il pourrait faire et à comment il pourrait se défendre si le loup, le serpent et le sanglier l'embêtent, comme ils ont l'habitude de le faire, sur le chemin. Mais étrangement, il ne les croise pas. En arrivant chez sa mamie, il trouve la porte entrouverte et découvre les trois intrus. Mais au même moment, sa grand-mère, une grande sorcière réputée, revient à la maison. Comment les trois intrus réussiront-ils à sortir sans être vus? Pourront-ils compter sur l'aide d'Ismaël?

BACKGROUND: Patients with peripheral artery disease have an increased risk of cardiovascular morbidity and mortality. Antiplatelet agents are widely used to reduce these complications.METHODS: This was a multicentre, double-blind, randomised placebo-controlled trial for which patients were recruited at 602 hospitals, clinics, or community practices from 33 countries across six continents. Eligible patients had a history of peripheral artery disease of the lower extremities (previous peripheral bypass surgery or angioplasty, limb or foot amputation, intermittent claudication with objective evidence of peripheral artery disease), of the carotid arteries (previous carotid artery revascularisation or asymptomatic carotid artery stenosis of at least 50%), or coronary artery disease with an ankle-brachial index of less than 0·90. After a 30-day run-in period, patients were randomly assigned (1:1:1) to receive oral rivaroxaban (2·5 mg twice a day) plus aspirin (100 mg once a day), rivaroxaban twice a day (5 mg with aspirin placebo once a day), or to aspirin once a day (100 mg and rivaroxaban placebo twice a day). Randomisation was computer generated. Each treatment group was double dummy, and the patient, investigators, and central study staff were masked to treatment allocation. The primary outcome was cardiovascular death, myocardial infarction or stroke; the primary peripheral artery disease outcome was major adverse limb events including major amputation. This trial is registered with ClinicalTrials.gov, number NCT01776424, and is closed to new participants.FINDINGS: Between March 12, 2013, and May 10, 2016, we enrolled 7470 patients with peripheral artery disease from 558 centres. The combination of rivaroxaban plus aspirin compared with aspirin alone reduced the composite endpoint of cardiovascular death, myocardial infarction, or stroke (126 [5%] of 2492 vs 174 [7%] of 2504; hazard ratio [HR] 0·72, 95% CI 0·57-0·90, p=0·0047), and major adverse limb events including major amputation (32 [1%] vs 60 [2%]; HR 0·54 95% CI 0·35-0·82, p=0·0037). Rivaroxaban 5 mg twice a day compared with aspirin alone did not significantly reduce the composite endpoint (149 [6%] of 2474 vs 174 [7%] of 2504; HR 0·86, 95% CI 0·69-1·08, p=0·19), but reduced major adverse limb events including major amputation (40 [2%] vs 60 [2%]; HR 0·67, 95% CI 0·45-1·00, p=0·05). The median duration of treatment was 21 months. The use of the rivaroxaban plus aspirin combination increased major bleeding compared with the aspirin alone group (77 [3%] of 2492 vs 48 [2%] of 2504; HR 1·61, 95% CI 1·12-2·31, p=0·0089), which was mainly gastrointestinal. Similarly, major bleeding occurred in 79 (3%) of 2474 patients with rivaroxaban 5 mg, and in 48 (2%) of 2504 in the aspirin alone group (HR 1·68, 95% CI 1·17-2·40; p=0·0043).INTERPRETATION: Low-dose rivaroxaban taken twice a day plus aspirin once a day reduced major adverse cardiovascular and limb events when compared with aspirin alone. Although major bleeding was increased, fatal or critical organ bleeding was not. This combination therapy represents an important advance in the management of patients with peripheral artery disease. Rivaroxaban alone did not significantly reduce major adverse cardiovascular events compared with asprin alone, but reduced major adverse limb events and increased major bleeding.FUNDING: Bayer AG.

BACKGROUND: Coronary artery disease is a major cause of morbidity and mortality worldwide, and is a consequence of acute thrombotic events involving activation of platelets and coagulation proteins. Factor Xa inhibitors and aspirin each reduce thrombotic events but have not yet been tested in combination or against each other in patients with stable coronary artery disease.METHODS: In this multicentre, double-blind, randomised, placebo-controlled, outpatient trial, patients with stable coronary artery disease or peripheral artery disease were recruited at 602 hospitals, clinics, or community centres in 33 countries. This paper reports on patients with coronary artery disease. Eligible patients with coronary artery disease had to have had a myocardial infarction in the past 20 years, multi-vessel coronary artery disease, history of stable or unstable angina, previous multi-vessel percutaneous coronary intervention, or previous multi-vessel coronary artery bypass graft surgery. After a 30-day run in period, patients were randomly assigned (1:1:1) to receive rivaroxaban (2·5 mg orally twice a day) plus aspirin (100 mg once a day), rivaroxaban alone (5 mg orally twice a day), or aspirin alone (100 mg orally once a day). Randomisation was computer generated. Each treatment group was double dummy, and the patients, investigators, and central study staff were masked to treatment allocation. The primary outcome of the COMPASS trial was the occurrence of myocardial infarction, stroke, or cardiovascular death. This trial is registered with ClinicalTrials.gov, number NCT01776424, and is closed to new participants.FINDINGS: Between March 12, 2013, and May 10, 2016, 27 395 patients were enrolled to the COMPASS trial, of whom 24 824 patients had stable coronary artery disease from 558 centres. The combination of rivaroxaban plus aspirin reduced the primary outcome more than aspirin alone (347 [4%] of 8313 vs 460 [6%] of 8261; hazard ratio [HR] 0·74, 95% CI 0·65-0·86, pINTERPRETATION: In patients with stable coronary artery disease, addition of rivaroxaban to aspirin lowered major vascular events, but increased major bleeding. There was no significant increase in intracranial bleeding or other critical organ bleeding. There was also a significant net benefit in favour of rivaroxaban plus aspirin and deaths were reduced by 23%. Thus, addition of rivaroxaban to aspirin has the potential to substantially reduce morbidity and mortality from coronary artery disease worldwide.FUNDING: Bayer AG.

Objectifs: L’essai a ete conduit sur le site experimental de l’IRAD station polyvalente de Garoua a Sanguere Paul au Nord Cameroun, ll a ete mise en place en juillet 2009. Il visait a etudier la courbe de reponse du jatropha a la fumure organique et/ou a la fumure minerale (N.P.K.) par rapport au temoin sans fertilisation. Methodologie et Resultats: Le dispositif experimental est du type blocks de Fischer randomises a un facteur de 3 traitements (0 engrais, 200 kg/ha de N.P.K.et 5T de fumier) avec 8 repetitions. Differents parametres ont ete etudies notamment le pourcentage de survie des plants apres 5 ans, les parametres de morphologique (hauteur, diametre des plants, nombre de ramifications) et les parametres de rendement dont notamment le rendement des graines par plant puis une l’evaluation separee du rendement par hectare pour chaque traitement. Les analyses des resultats effectuees par le STATBOX et le test de Student-Newman Keul’s Multiple (P=0.05) ont permis de montrer que la fertilisation a eu un effet significatif sur la hauteur des plants les deux 1eres annees et pas d’effet sur les trois dernieres annees. Par contre elle a eu un effet significatif sur les cinq annees d’observations sur le diametre, le nombre de ramifications, le rendement en graine par plant et a l’hectare. Toutefois l’engrais mineral s’est avere meilleur les deux 1eres annees sur la hauteur (83,32 cm en 2010 et 150 cm en 2014), et pas de difference significative les trois annees suivantes. Pourtant la fertilisation a eu un effet significatif pendant les cinq annees d’observation sur le diametre au collet, le nombre de ramification, le rendement par plant et par hectare en passant de 41,55 mm en 2010 a 101 mm en 2014 ; 4,77 en 2010 a 32,25 en 2014 ; 26,66g en 2010 a 648,82g en 2014 par plant et 66,16 kg/ha en 2010 a 1622 kg/ha en 2014 respectivement par rapport au temoin. Conclusion et application des resultats: Ces resultats montrent que le Jatropha curcas considere comme une plante de zone marginale qui ne necessite pas la fertilisation, repond positivement a une fertilisation aussi bien organique que minerale. Cette fertilisation ameliore nettement aussi bien les parametres morphologiques que par le rendement. Results showed also that fertilization has significant effects during the five years of observation on plants diameter, ramification number and yield parameter per hectare by going to 41.55 mm in 2010 to 101 mm in 2014; 4.77 in 2010 to 32.25 in 2014; 26.66g in 2010 to 648.82g in 2014 per plant and 66.16 kg/ha in 2010 to 1622 kg/ha in 2014 respectively compare to control plants. Conclusion and results application: By these results, Jatropha curcas cannot be considered as marginal zone plant, which does not need fertilization. In fact, as it is presented by the results it responds perfectly as any other plant to mineral or organic fertilization. This fertilization improves morphological parameters and yield component. Background— The origin of 40% of syncope cases remains unknown even after a complete diagnostic workup. Previous studies have suggested that ATP testing has value in selecting successful therapy. This patient-blinded, multicenter, randomized superiority trial tested whether, in patients with syncope of unknown origin, selecting cardiac pacing in those with a positive ATP test leads to fewer recurrences than those who do not receive pacing. Methods and Results— From 2000 to 2005, 80 consenting patients (mean age, 75.9±7.7 years; 81% women; 56% without diagnosed structural heart disease) with syncope of unknown origin and atrioventricular or sinoatrial block lasting >10 seconds (average, 17.9±6.8 seconds) under ATP administration (20-mg IV bolus) were recruited from 10 hospitals, implanted with programmable pacemakers, and randomized to either active pacing (dual-chamber pacing at 70 bpm) or backup pacing (atrial pacing at 30 bpm). Patients were followed up regularly for up to 5 years for any syncope recurrence, the primary outcome. Mean follow-up was 16 months. Syncope recurred in 8 of 39 patients (21%) randomized to active pacing and in 27 of 41 (66%) randomized to backup pacing (control), yielding a hazard ratio of 0.25 (95% confidence interval, 0.12–0.56). After recurrence, the 27 recurrent control patients were reprogrammed to active pacing, and only 1 reported subsequent syncope. Conclusion— This study suggests that, in elderly patients with syncope of unknown origin and positive ATP tests, active dual-chamber pacing reduces syncope recurrence risk by 75% (95% confidence interval, 44–88). Methods and Results From 2000 to 2005, 80 consenting patients (mean age 75.9±7.7 years; 81% women; 56% without diagnosed structural heart disease) with syncope of unknown origin and atrioventricular or sinoatrial block lasting longer than 10 seconds (average 17.9±6.8 seconds) under ATP administration (20 mg IV bolus) were recruited from 10 hospitals, implanted with programmable pacemakers and randomized to either active pacing (dual-chamber pacing at 70 beats per minute (bpm) or backup pacing (atrial pacing at 30 bpm). Patients were followed regularly for up to five years for any syncope recurrence, the primary outcome. Mean follow-up was 16 months. Syncope recurred in eight (21%) of 39 patients randomized to active pacing and in 27 (66%) of 41 randomized to backup pacing (control), yielding a hazard ratio of 0.25 (95% CI: 0.12–0.56). After recurrence, the 27 recurrent control patients were reprogrammed to active pacing and only one reported subsequent syncope. Conclusions This study suggests that, in elderly patients with syncope of unknown origin and positive ATP tests, active dual-chamber pacing reduces syncope recurrence risk by 75% (95% confidence interval, 44–88).

In order to assess the feasibility of microsatellite primers as markers for genomic instability, we conducted a study of DNA stability in cauliflower callus. A protocol is described for the rapid screening of a large number of putative variant calli and plants. Genomic DNA is isolated and screened by microsatellite primers. We believe that inter-simple sequence repeat PCRs can conveniently detect and measure common genetic events underlying plant genomic instability. These include deletions, amplifications, translocations, insertions, recombination or chemical alterations. Our results indicate that instability occurred in an early step in the process of callogenesis. The technique is fast, reproducible, and is a new application for ISSR markers.

17. The value of ν for fission spectrum induced and spontaneous fission of 238U

The effective value of nu for U/sup 238/ fission induced by a fission spectrum of fast neutrons was measured by the scattering technique and, much more accurately, by the coincidence technique. The result for the ratio of nu /sub 238// nu /sub 235/ (thermal) agreed with earlier values of comparable precision reported in the literature, and, when averaged with them, yielded a best value for this ratio of 1.160 plus or minus 0.020. If nu /sub 235/ (thermal) is 2.47 plus or minus 0.0), the best value of nu /sub 238/ (fission spectrum) is 2.86 plus or minus 0.05. A probable explanation was suggested for the apparently high value of nu /sub 238/ (3.5 plus or minus 0.2) reported in an earlier experiment. The value of nu /sub 238/ for spontaneous fission of U/sup 238/ was also obtained. The result 2.10 plus or minus 0.08 (based on nu /sub 235/ (thermal) = 2.47) agreed with earlier values in the literature derived from separate measurements of neutron emission and spontaneous fission rates in U/sup 238/.
