Introduction: Type 2 diabetes mellitus (T2DM) patients are at high risk of dyslipidemia, which in turn is associated with macrovascular diseases, such as heart diseases and stroke, and microvascular diseases, such as neuropathy and nephropathy. There are contradictory findings in the literature regarding the relationship between glycated hemoglobin (HbA1c) and the lipid profile among T2DM patients. This study was performed to investigate the association between HbA1c level and the lipid profile in elderly T2DM patients at a primary care hospital in Jeddah City, Saudi Arabia. Methods: This study is a retrospective cross-sectional study conducted at the Prince Abdul Majeed Healthcare Center (PAMHC) in Jeddah, Saudi Arabia. The sociodemographic and clinical data of the T2DM patients who had visited the PAMHC from 1 January 2020 to 31 December 2021, were collected from the data registry of the PAMHC and analyzed for publication. Results: The study included a total of 988 T2DM patients (53.3% male). Of the participants, 42.9% were aged between 55 and 64 years. Dyslipidemia parameters were presented as high LDL-c (in 60.3% cases), low HDL-c (in 39.8% cases), high triglycerides (in 34.9% cases), and high total cholesterol (in 34.8% cases). The correlation of HbA1c with total cholesterol (TC) and triglycerides (TGs) was positively significant, thereby highlighting the important link between glycemic control and dyslipidemia. A mean increase of 4.88 mg/dL and 3.33 mmHg in TG level and diastolic blood pressure, respectively, was associated with the male gender, in comparison to the female gender. However, the male gender was significantly associated with the reduction in the mean cholesterol level, BMI, HbA1c, HDL-c, and LDL-c by 11.49 mg/dL, 1.39 kg/m2, 0.31%, 7.47 mg/dL, and 5.6 mg/dL, respectively, in comparison to the female gender. Conclusions: The results of this study show that HbA1c was significantly associated with cholesterol and triglyceride levels in the T2DM patients included in the study. Our findings highlight the important relationship between glycemic control and dyslipidemia.

Introduction: Patient safety captures the essence of the primary principle of medical ethics, primum non nocere, first do no harm; this is an important concern in the health care system. Nurses are indispensable members of this system and are the largest group of health care providers involved in the direct delivery of patient care. As an integral part of the health care system, it is important to know nurses’ opinions on patient safety culture. Objectives: First, to evaluate and measure the existing safety culture and safety of patients in medical−surgical wards (MSW) in hospitals located in the Qassim region, in the Kingdom of Saudi Arabia. Second, to survey the opinion of registered nurses and supervisors/managers about safety culture and issues concerned with safety in hospitals in the region. Materials and Methods: A validated cross-sectional survey, namely the Hospital Survey on Patient Safety Culture (HSOPSC), was used. This survey queried 300 nurses in different MSWs in four hospitals in the Qassim Region. Results: Overall, a positive culture of safety exists in MSWs, with 69% of RNs rating their wards as having great/excellent safety culture. Notably, some participants felt it was problematic that blame was assigned to nurses for reported errors. While 55.9% of participants noted that all errors or narrowly avoided errors had been reported, less than half actually reported errors in the last year. Conclusion: The perceived safety culture was largely positive; however, the results also indicated that a culture of safety comes with some risk and blame.

Background: Medication administration errors significantly impact patient safety, potentially leading to severe harm or fatality. Reporting such errors through active systems improves medication administration, thereby enhancing patient safety and the quality of care. However, in the context of Saudi Arabia, little is understood about the causes of medication administration errors and the obstacles hindering their reporting. Objective: This study aimed to explore nurses’ perceptions of the causes of medication administration errors and the barriers to reporting them. Methods: The study employed a qualitative descriptive design, conducting face-to-face semi-structured interviews with 43 nurses from three hospitals in Taif Governorate, Saudi Arabia, between October and November 2023. Purposive sampling was used to recruit participants, and thematic analysis was utilized for data analysis. Results: The following themes emerged regarding the causes of medication administration errors: order deficiencies, high workloads and staff shortages, and malpractice. Regarding the barriers to reporting errors, the emerging themes were fear of punishment and lack of support, lack of knowledge and awareness about reporting, and lack of feedback. Conclusion: This study reveals nurses’ perceptions of the causes of medication administration errors and the barriers to reporting them. Recognizing and addressing these causes and barriers are essential for patient safety and the improvement of the healthcare environment. Efforts should be directed toward implementing interventions that address high workloads, enhance staff education and awareness, and promote a workplace culture conducive to reporting errors without fear of repercussions. Additionally, supportive mechanisms, such as feedback systems and resources for professional development, should be implemented to empower nurses to actively participate in error reporting and contribute to continuous improvement in medication administration practices.

Purpose Insomnia-related affective functional disorder may negatively affect social cognition such as empathy, altruism, and attitude toward providing care. No previous studies have ever investigated the mediating role of attention deficit in the relationship between insomnia and social cognition. Methods A cross-sectional survey was carried out among 664 nurses (Mage = 33.03 years; SD ± 6.93 years) from December 2020 to September 2021. They completed the Scale of Attitude towards the Patient (SAtP), the Athens Insomnia Scale (AIS), a single-item numeric rating scale assessing the increasing severity of attention complaints, and questions relating to socio-demographic information. The analysis was carried out by examining the mediating role of attention deficit in the relationship between insomnia and social cognition. Results The prevalence of insomnia symptoms was high (52% insomnia using the AIS). Insomnia was significantly correlated with attention problems (b = 0.18, standard error (SE) = 0.02, p < 0.001). Attention problems were significantly negatively correlated with nurses’ attitudes towards patients (b = −0.56, SE = 0.08, p < 0.001), respect for autonomy (b = −0.18, SE = 0.03, p < 0.001), holism (b = −0.14, SE = 0.03, p < 0.001), empathy (b = −0.15, SE = 0.03, p < 0.001), and altruism (b = −0.10, SE = 0.02, p < 0.001). Attention problems indirectly mediated the effect of insomnia on attitudes toward patients (99% CI = −0.10 [−0.16 to −0.05]), respect for autonomy (99% CI = −0.03 [−0.05 to −0.02]), holism (99% CI = −0.02 [−0.04 to −0.01]) empathy (99% CI = −0.03 [−0.04 to −0.01]), and altruism (99% CI = −0.02 [−0.03 to −0.01]). Conclusion Nurses with insomnia-related attention problems are likely to have poor explicit social cognition such as attitude toward patients, altruism, empathy, respect for autonomy, and holism.

Background: In clinical practice, patients' sleep is disrupted by medical professionals' care or evaluations both at night and during the day. Multiple factors contribute to this sleep disruption resulting in poor clinical outcomes. Aims: This study aims to determine the quality of sleep perceived by patients admitted to various hospital settings, as well as the factors affecting sleep quality. Settings and Design: Descriptive explorative design involving purposive convenient sampling of 90 hospitalized admitted at King Khalid General Hospital, Al Majmaah, and General Hospital, Zulfi. Methodology: St. Mary's Hospital Sleep Questionnaire used to assess patients' sleep and sleep-disturbing scale. Statistical Analysis Used: The Chi-square test was used to compare the different parameters related to sleep. Results: Subjective conclusions suggested that 38 (26.4%) patients slept lightly, 67 (46.5%) were moderately satisfied with their last night's sleep, and only 9 (6.3%) were mildly dissatisfied. Continuous light, uncomfortable Pillow, strange fragrances (P = 0.004), hearing frequent sound (P = 0.018), telephone (P = 0.046), and machine alarms (P = 0.006) were the elements impacting sleep pattern connected to the patient themselves. Tubings, monitor leads, pain, and fear of death all significantly influenced sleep pattern. Many of the healthcare workers' behaviors such as awakening by doctors and nurses (χ2 = 13.08, P = 0.02), making loud noise (χ2 = 15.85, P = 0.007), assessing vitals (χ2 = 12.90, P = 0.02), hygiene care (χ2 = 5.24, P = 0.14), feeding (χ2 = 9.82, P = 0.09), tubing and drain (χ2 = 17.92, P < 0.002), and suctioning (χ2 = 21.30, P = 0.0001) had significant influence on sleep pattern. Conclusion: Based on the results, nurses can minimize the factors that cause sleep interference in hospitalized patients and avoid the possible negative effects caused by sleep deprivation.

Dans un premier temps, nous conduirons, un diagnostic de la stratégie de communication de l‟entreprise ainsi que ses pratiques de parrainage. Ensuite, une étude qualitative (basée sur des entretiens semi-directifs) permet de mesurer l‟effet de ces actions sur son image de marqueen passant par l‟étude de la congruence du parrain et de l‟entité parrainée.

Introduction and objectives: Phoenix dactilyfera L known as date palm is mainly found in North Africa and Asia, due to that, a wide range of varieties of date fruit exist. It has been consumed for long ago and also used in the traditional medicine for its virtues on human body. The edible part of the fruit contains a high amount of carbohydrates, dietary fiber, certain essential vitamins, minerals and a low amount of lipids and proteins. However, the pits, which are considered as a waste product, has higher amount in lipids and proteins and also some minerals and dietary fiber. Further investigations revealed also the composition in phytochemicals of the fruit. Dates comprise phenols, sterols, anthocyanin, carotenoid, procyanidin, and flavonoid. The richness in the composition has a positive impact on health. Indeed, the fruit present activities like anti-oxidant, free radical scavenging, antimutagenic, antimicrobial, antiproliferative, antiinflammatory, nephroprotective, hepatoprotective, gastroprotective, immunostimulant, anticancer, anticholesteremic. The aim of this work is to figure out the composition and to explore the different activities of dates (fruit and seed) with new methods of extractions which are considered to be "green" and to assess the differences between the varieties coming from different localities. Materials and Methods: From the seeds, oil has been extracted by a supercritical fluid which was CO2 and analyzed by gas chromatography to determine the profile in fatty acids. However, the extraction from the pulp was not selective and it was operated by Pressurized liquid extraction in our case the solvent was ethanol 72% then the extracts were analyzed by HPLC. Results and conclusion: The oil extracted from the seeds presented almost the same profile of fatty acids for all the varieties: myristic acid (C14: 0), palmitic acid (C16: 0), stearic acid (C18: 0), oleic acid (C18:1) and linoleic acid (C18: 2) except for some varieties which presented one fatty acid that couldn't be identified. The extracts obtained from the pulp had a yield of 54%. The HPLC analysis is still in process. Thanks to the tremendous benefits that dates have on human body this fruit need to be more investigated and incorporated in our daily intake of food. [ABSTRACT FROM AUTHOR]

This study aimed to estimate the effective dose and the risk of exposure-induced cancer death (REID), as well as to establish diagnostic reference levels (DRLs) for common CT examinations conducted in Tabriz, Iran. The investigation included adult patients undergoing abdomen-pelvis, brain, neck, sinus, and chest CT scans. Patient data, exposure parameters, and radiation dose metrics, such as volume CT dose index (CTDI vol ) and dose length product (DLP), were collected and analyzed. The results showed significant variations in radiation dose across different centers for the CT scans. The average effective doses for the different CT scans were 5.65, 1.08, 1.40, 0.46, and 3.68 mSv for abdomen-pelvis, brain, neck, sinus, and chest scans, respectively. The REID values ranged from 14 per million (for sinus scans) to 196 per million (for abdomen-pelvis scans). Additionally, the DRL values for CTDIvol were 11.03 (for abdomen-pelvis), 59.52 (for brain), 8.33 (for neck), 17.05 (for sinus), and 7.83 mGy (for chest). Our results showed that most of the investigated CT scans had lower effective doses compared to the literature and the REIDs were estimated to be low. Minimizing radiation risk can be achieved by reducing CT exams and keeping doses as low as reasonably achievable. The local DRLs from this study were comparable to previous reports and can serve as benchmarks for setting national and international DRLs, helping healthcare facilities optimize radiation practices and improve patient safety in diagnostic imaging., Competing Interests: Declarations. Talin-1 is one of the major scaffold proteins in focal adhesions playing a vital role in cell migration, metastasis, and cancer progression. Although studies regarding the importance of Talin-1 in cancer have rapidly developed, its prognostic and diagnostic value still remain unsatisfying in pancreatic cancer (PC). Therefore, the present study aims to investigate the expression, clinical significance, as well as the prognostic and diagnostic value of Talin-1 in different types of PC. Bioinformatic analysis was applied to determine the clinical importance and biological role of Talin-1 expression in PC tumors and the normal adjacent samples. The expression patterns, clinical significance, prognosis, and diagnosis value of Talin-1 were evaluated in tissue microarrays (TMAs) of 190 PC samples including 170 pancreatic ductal adenocarcinoma (PDAC), and 20 pancreatic neuroendocrine tumors (PNET), along with 24 adjacent normal tissues using immunohistochemistry (IHC). The results indicated that the expression of Talin-1 was upregulated in tumor cells compared with adjacent normal tissues. A statistically significant association was observed between the higher cytoplasmic expression of Talin-1 and lower histologic grade ( P <0.001) in PDAC samples. Further, our findings indicated an inverse significant correlation between cytoplasmic expression of Talin-1 and recurrence ( P =0.014) in PNET samples. No significant association was observed between the cytoplasmic expression of Talin-1 and survival outcomes as well as diagnostic accuracy. In conclusion, our observations demonstrated that a higher cytoplasmic level of Talin-1 protein was significantly associated with less aggressive tumor behaviors in PC samples. Introduction Patient satisfaction is a critical indicator of health care quality, yet research exploring patient satisfaction with nursing care in Saudi Arabia (SA) remains limited. This study investigated patients' satisfaction with nursing care quality during their hospitalization in SA. Material and methods A cross-sectional study was conducted using a convenience sample of 746 patients from five hospitals in five different regions of SA. Data were collected using the Newcastle Satisfaction With Nursing Scale (NSMS), which has been shown to be valid and reliable. Results The average overall patient satisfaction with nursing care was 71.16 ± 13.51 out of 95 (75.96%), indicating a high level of satisfaction with the nursing care the patients received. The highest-scoring items of patient satisfaction were the nurses' manners when carrying out their duties, the amount of privacy the nurses provided and their capability to perform their jobs. However, the level of satisfaction regarding the frequency of nurse check-ins, time spent with patients, and support provided to patients' relatives indicated areas for potential improvement. Also, patients with primary education who were divorced or married and those admitted to the gynecology ward were more satisfied with nursing care. Conclusion This study's findings indicate that the patients were highly satisfied with the care their nurses provided, although opportunities for improvement in some areas of nursing care were identified. These findings may encourage nurses to be more attentive in meeting their patients' needs according to best nursing practices. Background The elderly population continues to grow worldwide, including in Saudi Arabia. Caring for older people with Alzheimer's and dementia disease is very challenging and merits specific skills, knowledge, and attitudes among nurses and nursing students. Consequently, nursing students must be prepared with the appropriate knowledge and attitude to care for patients affected by Alzheimer's in their future professions. This study aimed to investigate the knowledge and attitudes of Alzheimer's disease (AD) among bachelor's nursing students in Saudi Arabia. Methods This study used a descriptive cross-sectional design, and data were collected via an online questionnaire comprising two main instruments: the Alzheimer's Disease Knowledge Scale (ADKS) and the Dementia Attitudes Scale (DAS). A total of 477 undergraduate nursing students participated in the study at four universities in four regions of Saudi Arabia. Result The results indicated that Saudi nursing students exhibited insufficient knowledge regarding individuals with AD, reflected by a mean ADKS score of 13.83 out of 30. Yet, they displayed positive attitudes, as indicated by a mean DAS score of 99.29 out of 140. Nursing students in their third year and those who had family members with AD had a higher significant score regarding their knowledge of AD than nursing students who were in their fourth year or those who did not have family members who had AD. Additionally, nursing students aged 20 to 25 years, as well as nursing students in their fourth year, had more positive attitudes toward working with AD patients. Conclusions In conclusion, this study revealed that although many nursing students have a positive attitude toward working with AD patients, they have insufficient knowledge of AD. Therefore, there is an urgent necessity for enhanced educational initiatives, encompassing both greater depth and improved quality, as well as increased clinical training to address this knowledge gap among nursing students in Saudi Arabia., Competing Interests: Human subjects: Consent was obtained or waived by all participants in this study. Agriculture faces many challenges such as climate change, droughts, and salinity, which requires urgent interventions for fast adaptation and diversification of the sector. In this context, introduction of new crops that tolerate stresses and generate high added value such as quinoa would allow simultaneously to address two interlinked challenges: abiotic stresses that limit agricultural production and poverty that impacts negatively the rural people. The quinoa Rehamna project funded by the International Development Research Centre aims to contribute to the development the quinoa value chain toward achieving food and nutritional security of rural poor communities. The obtained results from the farmer's survey show that 14% of surveyed farmers already grow quinoa and 2/3 never heard about quinoa, among them 96% are interested to grow quinoa. The marketing study reveals that the psychological price that satisfies most of consumers is ranging between 2 and 2.6 USD/500 g of processed quinoa seeds. The findings indicate that quinoa has a great potential for both producers and consumers in Morocco and can be a judicious solution toward achieving food and nutritional security., (© 2020. Cephalic malposition is an anatomical variant that is found particularly in Middle Eastern people. Malpositioning of the alar cartilage may cause both functional and aesthetic problems. This study aimed to investigate and compare the functional and aesthetic outcomes of the two techniques used to correct cephalic malposition of the lower lateral cartilage: lateral crural overlay (LCO) and lateral crural transposition. This current randomised double-blinded clinical trial was performed on 40 patients who were referred for rhinoplasty in 2017-2018. They were randomly divided into two groups, the lateral crural transposition group (n = 20), and the LCO group (n = 20). The angle between the lateral crura and the midline, nasal rotation, nasal tip projection, nasal axis deviation, columella labial angle, and nasofacial angle, plus a visual analogue scale (VAS) and the 10-item Standardised Cosmesis and Health Nasal Outcomes Survey (SCHNOS), were assessed for each patient before surgery and after 12 months, and further compared between the two groups. Both techniques successfully improved the lateral crural angle, nasal tip rotation, and columella labial angle (p < 0.05). Despite the satisfactory outcomes of the VAS and SCHNOS based on both the patients' and the surgeons' opinions (p < 0.05), comparison of the indices revealed insignificant differences between the two groups (p> 0.05). Objective: The potential metabolic adverse effects of second-generation antipsychotics (SGA) need to be monitored. The Canadian Alliance for Monitoring Effectiveness and Safety of Antipsychotics (CAMESA) offers guidelines for this purpose. We aimed to evaluate the long-term rates of youths receiving monitoring in mental health clinics and document the factors that may influence them., Method: The charts of 180 patients (13.3 ± 3.1 years, 54.4% males) receiving SGA treatment for the first time between January 2016 and June 2018 were reviewed. Monitoring was divided into baseline and 1- to 6-month and 9- to 24-month periods. Population under study was stratified into children (4 to 12 years) and adolescents (13 to 18 years). Sociodemographic characteristics, psychiatric diagnosis and comorbidities, prescribed SGAs and comedications, anthropometric measures (AM), blood pressure (BP), blood tests (BT), electrocardiogram, and the psychiatrist's years of practice were collected. Cross tables were used to present the monitoring rates. Categories were compared by covariate analysis. Rates of patients monitored across categories were compared using Fisher exact test., Results: Monitoring rates for AM, BT, and BP were 55%, 47.8%, and 46.7% at baseline; 50%, 41.7%, and 45.2% at 1 to 6 months; and 47.2%, 41.5%, and 40.6% at 9 to 24 months, respectively. Higher monitoring rates were significantly associated with adolescent status (baseline, 1 to 6 months), a diagnosis of psychotic and/or affective disorder (baseline, 1 to 6 months, 9 to 24 months), having ≤1 psychiatric comorbidities (1 to 6 months), high SGA dose (baseline, 1 to 6 months), and clinician's experience (baseline, 9 to 24 months). Significantly lower monitoring rates were associated with the psychostimulant/atomoxetine comedication (baseline, 1 to 6 months, 9 to 24 months)., Conclusion: Five years after publication of the CAMESA guidelines, metabolic monitoring is conducted for less than half of patients. In our sample, age, diagnostic category, psychiatric comorbidities, SGA dose, clinician's experience, and comedications influenced the monitoring rates. Major progress still needs to be made before reaching a satisfactory level of monitoring.

Gracilaria algae is red macro algae which has demonstrated considerable anti-inflammatory effects. Our objective was to compare the efficacy of Gracilaria algae topical cream 3% vs Clobetasol cream 0.05% in treatment of plaque-type psoriasis. Thirty adult patients with baseline modified Psoriasis Area and Severity Index (PASI) score ≤12 were randomized to receive either Clobetasol or Gracilaria algae cream on right or left-sided symmetric plaques once daily for 8 weeks and follow-up of 4 weeks. Modified PASI score, patient's satisfaction using VAS and global physician assessment score were assessed to evaluate clinical response. Thirty patients with 94 symmetrical psoriasis plaques were enrolled in this trial. The mean baseline modified PASI score of both sides was similar; however, at the end of trial, modified PASI score was reduced more on the sides treated with Gracilaria algae cream (0.80 ± 0.19% vs 0.63 ± 0.25%, P < .05). No significant difference was found regarding mean physician global assessment score between the two groups (P > .05). Patients' satisfaction was significantly higher in favor of algae cream only at week 8 of the intervention (P < .05). Background: Cutaneous leishmaniasis (CL) is an endemic disease of Iran and despite self-healing nature of this disease and many suggested treatments, scar is usually inevitable results of the affection with this cutaneous infection. CO 2 laser in CL has a specific thermolysis effect on infected tissues and application of trichloroacetic acid (TCA) 50% solution for treatment of leishmaniasis scars showed promising results., Aim: To evaluate CO 2 laser plus TCA 50% versus CO 2 laser alone for treatment of CL., Patients/methods: In the current report, we present 6 cases of noduloulcerative CL that were treated successfully by the combination of the continuous wave CO 2 laser plus topical application of TCA 50%., Results: Five patients improved after 3 weeks of starting treatment, and 1 patient needed to repeat treatment. The scars were flat and good., Conclusions: The CO 2 laser plus topical TCA 50% may be more effective in treatment of the cutaneous leishmaniasis and results in shorter healing time., (© 2019 Wiley Periodicals, Inc.)
