1. Interaction of South Asian spices with conventional antibiotics: Implications for antimicrobial resistance for Mycobacterium abscessus and cystic fibrosis

Abstract

Background: Antimicrobial resistance (AMR) has rendered certain species of Mycobacterium difficult to treat clinically, particularly, the nontuberculous Mycobacterium, Mycobacterium abscessus. This bacterium is emerging in specific disease populations, including amongst cystic fibrosis (CF) patients, where AMR represent a true treatment dilemma. Therefore, any innovation with traditional antimicrobial compounds in spices, which increases the potency of existing conventional antibiotics should be investigated. Methods: M. abscessus isolates (n = 9 multidrug-resistant clinical isolates from CF patients + 1 Reference Strain) were examined for their direct susceptibility to 27 spices, as well as the interactive effect of this spice combination to their susceptibility to amikacin and linezolid antibiotic, with standard disk diffusion assay. Results: Five isolates of M. abscessus (5/10; 50%) failed to grow on the spice enriched medium, which included four clinical isolates and the National Culture Type Collection (NCTC) Reference Strain. Of the remaining five isolates which grew on the spice medium, no cultural phenotypic differences were observed, compared to unsupplemented controls. In the case of both amikacin and linezolid, the zone of inhibition increased with the inclusion of the spices. Initially, all isolates of M. abscessus were fully resistant to linezolid (mean zone of inhibition = 0 mm), and growth was to the edge of the antibiotic disk, whereas when in the presence of spices, large zones of inhibition were observed (mean zone of inhibition = 33.3 mm). With amikacin, the mean zone of inhibition increased from 23.2 mm to 32.0 mm, in the presence of spices. Conclusion: These data suggest that the spices were interacting synergistically with the antibiotics, thus making the antibiotic more potent against the bacteria tested. This study is significant as it demonstrates a positive interaction between spices and the conventional antimycobacterial antibiotics, amikacin, and linezolid. Given the burden of AMR to M. abscessus, particularly in a patient with chronic disease such as CF, any food-related innovation that can help maximize the potency of existing antimycobacterial antibiotics is to be encouraged and developed. The specific mechanism as to how spices increase the potency of such antibiotics with M. abscessus needs to be elucidated, as well as novel food (spice) delivery modalities developed, including novel medicinal foodstuffs or functional foods, that can harness this beneficial effect in vivo to medicine and society.

With the continuously increasing epidemic of drug-resistant tuberculosis, new drugs are desperately needed. However, even for the treatment of drug-sensitive tuberculosis, new drugs are required to shorten the treatment duration and thereby prevent development of drug resistance. Within the past ten years, major advances in tuberculosis drug research have been made, leading to a considerable number of antimycobacterial compounds which are now in the pipeline. Here we discuss a number of these novel promising tuberculosis drugs, as well as the discovery of two new potential drug targets for the development of novel effective drugs to curb the tuberculosis pandemic, ie, the coronin 1 and protein kinase G pathways. Protein kinase G is secreted by mycobacteria and is responsible for blocking lysosomal delivery within the macrophage. Coronin 1 is responsible for activating the phosphatase, calcineurin, and thereby preventing phagosome-lysosome fusion within the macrophage. Abstract

Background: Antimicrobial resistance (AMR) has rendered certain species of Mycobacterium difficult to treat clinically, particularly, the nontuberculous Mycobacterium, Mycobacterium abscessus. This bacterium is emerging in specific disease populations, including amongst cystic fibrosis (CF) patients, where AMR represent a true treatment dilemma. Therefore, any innovation with traditional antimicrobial compounds in spices, which increases the potency of existing conventional antibiotics should be investigated. Methods: M. abscessus isolates (n = 9 multidrug-resistant clinical isolates from CF patients + 1 Reference Strain) were examined for their direct susceptibility to 27 spices, as well as the interactive effect of this spice combination to their susceptibility to amikacin and linezolid antibiotic, with standard disk diffusion assay. Results: Five isolates of M. abscessus (5/10; 50%) failed to grow on the spice enriched medium, which included four clinical isolates and the National Culture Type Collection (NCTC) Reference Strain. Of the remaining five isolates which grew on the spice medium, no cultural phenotypic differences were observed, compared to unsupplemented controls. In the case of both amikacin and linezolid, the zone of inhibition increased with the inclusion of the spices. Initially, all isolates of M. abscessus were fully resistant to linezolid (mean zone of inhibition = 0 mm), and growth was to the edge of the antibiotic disk, whereas when in the presence of spices, large zones of inhibition were observed (mean zone of inhibition = 33.3 mm). With amikacin, the mean zone of inhibition increased from 23.2 mm to 32.0 mm, in the presence of spices. Conclusion: These data suggest that the spices were interacting synergistically with the antibiotics, thus making the antibiotic more potent against the bacteria tested. This study is significant as it demonstrates a positive interaction between spices and the conventional antimycobacterial antibiotics, amikacin, and linezolid. Given the burden of AMR to M. abscessus, particularly in a patient with chronic disease such as CF, any food-related innovation that can help maximize the potency of existing antimycobacterial antibiotics is to be encouraged and developed. The specific mechanism as to how spices increase the potency of such antibiotics with M. abscessus needs to be elucidated, as well as novel food (spice) delivery modalities developed, including novel medicinal foodstuffs or functional foods, that can harness this beneficial effect in vivo to medicine and society.

Abstract

Stay-green in sorghum [Sorghum bicolor (L.) Moench] is an important component of post-flowering drought tolerance. This research was conducted to describe (i) stay-green as post-flowering green leaf area duration (GLAD) and its components [green leaf area (GLA) at flowering, timing for onset of senescence, and senescence rate] and (ii) the expression of heterosis for stay-green in terms of heterosis for its components. The study was conducted during the 1992 to 1993 and 1993 to 1994 post-rainy season at ICRISAT headquarters near Hyderabad, India. It involved a nine-parent complete diallel, in two experiments differing in soil-water availability after flowering. Weekly estimations of % GLA were made on 36 leaves per plot. Relative (%days) and absolute ([m.sup.2] days) GLAD and their components were derived from a fitted logistic function. The variances in both relative and absolute GLAD were each fully ([R.sup.2] [greater than] 0.96) accounted for by their components. In spite of significant genotype x environment interactions for the component traits, the expression of heterosis for non-senescence as related to the stay-green trait was stable across experiments. The inheritance of the onset of senescence was additive, but a slow senescence rate was dominant over a fast rate. Consequently, a large relative GLAD (slow senescence) was partially dominant over a small relative GLAD. Because of the dominance of a large leaf area at flowering, the partial dominance in relative GLAD translated into overdominance for a large absolute GLAD. These results offer an opportunity for improving drought tolerance of sorghum in environments with post-flowering drought stress.

Abstract

Polymeric materials subjected to large strains undergo an evolution in molecular orientation. The developing orientation and corresponding strengthening are highly dependent on the state of strain. In this paper, we examine and compare the very different stress-strain results of polycarbonate produced from four types of mechanical testing: uniaxial compression, plane strain compression, uniaxial tension, and simple shear. These tests produce different states of orientation within the material and, in the case of simple shear, the principle axes of orientation rotate during the deformation. The ability of the recent constitutive model of Arruda and Boyce (1992) to predict the observed behavior is evaluated. The model has been incorporated into a finite element code in order to properly simulate the material behavior during the inhomogeneous deformations of tension (cold drawing) and simple shear. The material properties of the model are obtained from the uniaxial compression test and the model is then found to be truly predictive of the other states of deformation demonstrating its fully three dimensional capability. The disadvantages of the tensile and simple shear tests for obtaining the data needed to accurately quantify the large strain material behavior of polymers are shown and discussed.

Abstract

Attacks of wheezing induced by upper respiratory viral infections are common in preschool children between the ages of 10 months and 6 years. A short course of oral prednisolone is widely used to treat preschool children with wheezing who present to a hospital, but there is conflicting evidence regarding its efficacy in this age group.We conducted a randomized, double-blind, placebo-controlled trial comparing a 5-day course of oral prednisolone (10 mg once a day for children 10 to 24 months of age and 20 mg once a day for older children) with placebo in 700 children between the ages of 10 months and 60 months. The children presented to three hospitals in England with an attack of wheezing associated with a viral infection; 687 children were included in the intention-to-treat analysis (343 in the prednisolone group and 344 in the placebo group). The primary outcome was the duration of hospitalization. Secondary outcomes were the score on the Preschool Respiratory Assessment Measure, albuterol use, and a 7-day symptom score.There was no significant difference in the duration of hospitalization between the placebo group and the prednisolone group (13.9 hours vs. 11.0 hours; ratio of geometric means, 0.90; 95% confidence interval, 0.77 to 1.05) or in the interval between hospital admission and signoff for discharge by a physician. In addition, there was no significant difference between the two study groups for any of the secondary outcomes or for the number of adverse events.In preschool children presenting to a hospital with mild-to-moderate wheezing associated with a viral infection, oral prednisolone was not superior to placebo. Abstract

The clinical diagnosis of asthma represents several putative wheeze phenotypes, each associated with a unique cluster of risk factors, underlying inflammation, and response to therapy. In school-age children, the 'classical' atopic asthma phenotype predominates. By contrast, asthma in children aged between 1 and 5 years is frequently characterised by transient episodes of wheeze trigged by viral colds, with few or no interval symptoms (preschool viral wheeze). This phenotype has a different cluster of risk factors from atopic asthma and thus might not respond to asthma therapies of proven efficacy in older children. The objective of this review is to look at the current evidence in the management of 'preschool viral wheeze'.

Abstract

The availability of recessive genes for early flowering-day length insensitivity offers an opportunity to synchronize the flowering of late-flowering or day length-sensitive parents with that of early-flowering parents without necessarily affecting the flowering of their hybrids, provided that the earlier parent does not carry the same recessive allele. This study evaluated the hypothesis that incorporating the recessive e1 allele for early flowering-day length insensitivity into a late-flowering, photoperiod-sensitive pollinator in pearl millet [Pennisetum glaucum (L.) R. Br.] would synchronize its flowering and improve seed production with earlier-flowering female parents, without affecting the time to flowering or the performance of the resulting hybrids. An [e.sub.1]/[e.sub.1] isoline of the late-flowering pollinator ICMP 85410, produced by six backcrosses, flowered 16 dl earlier under natural day lengths at Patancheru, India, (17 ° N) and 19 d earlier under extended day lengths (equivalent to 29 ° N) than its near isogenic [E.sub.]/[E.sub.1] counterpart. As a consequence, it successfully produced hybrid seed when sown simultaneously with early, male-sterile line 843A, whereas the late isoline failed under the same conditions. The [E.sub.1]/[e.sub.1] versions of eight near-isogenic hybrids (on a range of eight [E.sub.1]/[E.sub.1] male-steriles) flowered an average of 3 d earlier than their [E.sub.1]/[E.sub.1] counterparts in 2 yr of tests under both natural and extended day length conditions at the same location. This earlier flowering had small effects on hybrid yield components, consistent with known effects of earliness in the crop, but did not affect grain yield. Abstract

Tuberculosis, caused by Mycobacterium tuberculosis, has become an important health and economic burden, with more than four thousand people succumbing to the disease every day. Thus, there is an urgent need to understand the molecular basis of this pathogen's success in causing disease in humans, in order to develop new drugs superior to conventional drugs available at present. One reason why M. tuberculosis is such a dangerous microbe lies within its ability to survive within infected hosts, thereby efficiently circumventing host immune responses. Over the past few years, a number of mechanisms have been unravelled that are utilized by M. tuberculosis to survive within hosts and to avoid immune defence mechanisms. Several of these mechanisms have been described in this communication that may be useful for the development of novel compounds to treat tuberculosis.

Abstract

Obesity negatively impacts skeletal muscle protein metabolism, and also impairs skeletal muscle maintenance and regeneration. We analyzed muscle biopsy samples from humans with increased body mass index (BMI) (i.e. > 30 kg/m2) and controls (i.e., BMI < 25 kg/m2) for expression of syncytin-1, a fusogenic protein regulating skeletal muscle regeneration. When compared to controls, humans with increased BMI and concomitant reduction in muscle protein synthesis had higher expression of syncytin-1 in skeletal muscle (p < 0.05). Across human subjects, muscle protein synthesis correlated inversely (r = −0.51; p = 0.03) with syncytin-1 expression in muscle. Using a C2C12 cell line we found that expression of syncytin-A (i.e, corresponding protein in murine tissue) is increased by insulin, and that this response is impaired in the presence of fatty acids, whose metabolism is altered within the metabolic environment induced by increased BMI. In C2C12 cells, the response of the protein 4E-BP1, which signals increase in protein synthesis in muscle, resembled that of syncytin-A. These findings provide novel insights into the expression of syncytin-1 in skeletal muscle of humans with increased BMI, as well as its basic regulation by insulin and fatty acids in muscle. The findings signify the need for further research into the regulation of syncytin-1 in skeletal muscle of humans with increased BMI, as well as its biological implications for altering muscle protein metabolism and regeneration.

Abstract

Background: Childhood mortality has decreased markedly over the last three decades. A study was undertaken to determine trends in deaths from respiratory illness in children in England and Wales. Methods: Mortality data collected by the Office for National Statistics were analysed. The data included all deaths registered from all causes in children aged between 28 days and 16 years in England and Wales from 1 January 1968 to 31 December 2000. The main outcome measures were overall and age-specific mortality rates due to all respiratory disorders and specific rates for pneumonia, asthma, cystic fibrosis (CF), and bronchiolitis. Results: In children aged 1–16 years the overall mortality rate (per 100 000 children) declined from 49.9 in 1968 to 16.3 in 2000, and rates due to respiratory illness fell from 8.6 to 1.3. The proportion of all deaths caused by respiratory illness in children aged 28 days to 16 years fell from 30.8% in 1968 to 9.9% in 2000. In post-neonatal infants (aged 28–364 days), the "all cause" mortality rate fell from 592.8 in 1968 to 176 in 2000 and the rates due to respiratory illness fell from 280 to 22.8. In 2000, pneumonia, asthma and CF together accounted for 73% of all respiratory deaths in 1–16 year olds. In this age group, mortality rates per 100 000 for pneumonia fell from 4.22 to 0.57, for asthma from 0.83 to 0.25, and for CF from 0.66 to 0.12 between 1968 and 2000. Over the same period mortality rates for pneumonia in post-neonatal infants fell from 165 to 6.78 per 100 000 and for CF from 4.88 to 0.33. Bronchiolitis mortality rates per 100 000 in post-neonatal infants fell from 21.47 in 1979 to 1.82 in 2000. Conclusions: Mortality rates due to all respiratory illnesses in children have fallen markedly in the last three decades. This decline has been more rapid than the overall decline in childhood mortality and respiratory diseases are now responsible for a smaller proportion of deaths in children. These data could provide a foundation for assessing the impact on mortality of future health initiatives such as the introduction of a universal pneumococcal vaccination programme in England and Wales.

Abstract

We treated a selected group of children attending a difficult asthma clinic with intramuscular triamcinolone acetonide. This study retrospectively reviews markers of asthma severity in those who received one or more monthly doses for three periods: 1) 3 months preceding the first injection (pretreatment), 2) from the first injection to 1 month after the last injection (treatment period), and 3) 3 months after the treatment period (follow-up period). Severity markers during the treatment and follow-up periods were compared with the pretreatment period by paired t-test. Five children (5-13 years old) received a single dose, and 8 children (12-15 years old) received multiple doses. Multiple doses of triamcinolone (n = 3-5) were associated with a fall in the number of asthma exacerbations (P < 0.01) and hospital admissions (P < 0.01) in both the treatment and follow-up periods. A single dose reduced exacerbations (P < 0.05, treatment vs. pretreatment) but not hospital admissions. We conclude that intramuscular triamcinolone is a useful short-term therapy in difficult asthma. Whether its efficacy is due to improved compliance, or an improved anti-inflammatory profile compared with oral steroids, remains unclear.
