The motor function of the gastrointestinal tract relies on the enteric nervous system, which includes neurons spanning from the esophagus to the internal anal sphincter. Disorders of gastrointestinal motility arise as a result of disease within the affected portion of the enteric nervous system and may be caused by a wide array of underlying diseases. The etiology of motility disorders may be primary or due to secondary causes related to infection or inflammation, congenital abnormalities, metabolic disturbances, systemic illness, or medication-related side effects. The symptoms of gastrointestinal dysmotility tend to be nonspecific and may cause diagnostic difficulty. Therefore, evaluation of motility disorders requires a combination of clinical, radiologic, and endoscopic or manometric testing. Radiologic studies including fluoroscopy, CT, MRI, and nuclear scintigraphy allow exclusion of alternative pathologic conditions and serve as adjuncts to endoscopy and manometry to determine the appropriate diagnosis. Additionally, radiologist understanding of clinical evaluation of motility disorders is necessary for guiding referring clinicians and appropriately imaging patients. New developments and advances in imaging techniques have allowed improved assessment and diagnosis of motility disorders, which will continue to improve patient treatment options.

Background We aimed to determine if patient symptoms and computed tomography enterography (CTE) and magnetic resonance enterography (MRE) imaging findings can be used to predict near-term risk of surgery in patients with small bowel Crohn’s disease (CD). Methods CD patients with small bowel strictures undergoing serial CTE or MRE were retrospectively identified. Strictures were defined by luminal narrowing, bowel wall thickening, and unequivocal proximal small bowel dilation. Harvey-Bradshaw index (HBI) was recorded. Stricture observations and measurements were performed on baseline CTE or MRE and compared to with prior and subsequent scans. Patients were divided into those who underwent surgery within 2 years and those who did not. LASSO (least absolute shrinkage and selection operator) regression models were trained and validated using 5-fold cross-validation. Results Eighty-five patients (43.7 ± 15.3 years of age at baseline scan, majority male [57.6%]) had 137 small bowel strictures. Surgery was performed in 26 patients within 2 years from baseline CTE or MRE. In univariate analysis of patients with prior exams, development of stricture on the baseline exam was associated with near-term surgery (P = .006). A mathematical model using baseline features predicting surgery within 2 years included an HBI of 5 to 7 (odds ratio [OR], 1.7 × 105; P = .057), an HBI of 8 to 16 (OR, 3.1 × 105; P = .054), anastomotic stricture (OR, 0.002; P = .091), bowel wall thickness (OR, 4.7; P = .064), penetrating behavior (OR, 3.1 × 103; P = .096), and newly developed stricture (OR: 7.2 × 107; P = .062). This model demonstrated sensitivity of 67% and specificity of 73% (area under the curve, 0.62). Conclusions CTE or MRE imaging findings in combination with HBI can potentially predict which patients will require surgery within 2 years.

Purpose Conventional model observers (MO) in CT are often limited to a uniform background or varying background that is random and can be modelled in an analytical form. It is unclear if these conventional MOs can be readily generalized to predict human observer performance in clinical CT tasks that involve realistic anatomical background. Deep-learning-based model observers (DL-MO) have recently been developed, but have not been validated for challenging low contrast diagnostic tasks in abdominal CT. We consequently sought to validate a DL-MO for a low-contrast hepatic metastases localization task. Methods We adapted our recently-developed DL-MO framework for the liver metastases localization task. Our previously-validated projection-domain lesion- / noise-insertion techniques were used to synthesize realistic positive and low-dose abdominal CT exams, using the archived patient projection data. Ten experimental conditions were generated, which involved different lesion sizes / contrasts, radiation dose levels, and image reconstruction types. Each condition included 100 trials generated from a patient cohort of 7 cases. Each trial was presented as liver image patches (160×160×5 voxels). The DL-MO performance was calculated for each condition and was compared with human observer performance, which was obtained by 3 sub-specialized radiologists in an observer study. The performance of DL-MO and radiologists was gauged by the area under localization receiver-operating-characteristic curves. The generalization performance of the DL-MO was estimated with the repeated 2-fold cross-validation method over the same set of trials used in human observer study. A multi-slice Channelized Hoteling Observers (CHO) was compared with the DL-MO across the same experimental conditions. Results The performance of DL-MO was highly correlated to that of radiologists (Pearson's correlation coefficient: 0.987; 95% C.I.: 0.942, 0.997). The performance level of DL-MO was comparable to that of the grouped radiologists, i.e. the mean performance difference was -3.3%. The CHO performance was poorer than the grouped radiologist performance, before internal noise could be added. The correlation between CHO and radiologists was weaker (Pearson's correlation coefficient: 0.812, and 95% C.I.: [0.378, 0.955]), and the corresponding performance bias (-29.5%) was statistically significant. Most patients experience good functional outcomes following ileal pouch-anal anastomosis (IPAA) for ulcerative colitis.We aimed to determine if asymptomatic patients with an IPAA had findings consistent with normal defecation on standard objective anorectal tests.Patients 18-65 years old with IPAA and self-reported healthy pouch function were recruited. Patients with chronic pouchitis, Crohn's disease, anastomotic stricture, or indication for IPAA other than ulcerative or indeterminate colitis were excluded. Patients underwent an interview with an abbreviated Rome Questionnaire followed by high-resolution ano-pouch manometry, balloon expulsion test, pouch barostat, and magnetic resonance (MR) defecography.Twenty patients completed all testing. Six patients were excluded from the final analysis due to symptoms suggestive of pouch evacuation disorder on the abbreviated Rome Questionnaire (n = 2), structural abnormality on MR imaging (n = 3), or both (n = 1). Of the remaining 14 patients, mean anal resting pressure during high-resolution manometry was 72 ± 16 mmHg, mean anal squeeze pressure was 247 ± 69 mmHg, and mean pouch-anal gradient during the simulated evacuation was -27 ± 37 mmHg. The meantime to balloon expulsion was 54 seconds. During dynamic MR defecography, the mean descent of ano-pouch junction was 2.6 cm, and mean pouch evacuation was 44.5% and 74.2% pre- and posttoilet phase, respectively.A substantial proportion of patients with IPAA and self-reported healthy pouch function have anatomic and/or functional abnormalities of the pouch. In asymptomatic IPAA patients with an anatomically normal pouch, we have proposed normal parameters for high-resolution ano-pouch manometry, time to balloon expulsion, pouch barostat, and MR defecography.

To describe the prevalence, clinical and radiological findings of biliary prolapse in pathologically proven mucinous cystic neoplasm of the liver (MCN-L).Thirty-four patients, all female with median age 50 years (range, 14-82), with histologically confirmed MCN-L were enrolled. Median tumor size was 9 cm (range, 2-21 cm). Fifty-seven examinations (17 ultrasound, 25 CT, and 15 MR) among 34 MCN-Ls were reviewed. Two radiologists retrospectively assessed images for tumor location, size and other morphological features of the tumor, presence of biliary prolapse and upstream bile duct dilatation. Ultrasound, CT, and MR were assessed separately. Clinical features were evaluated. Clinical and radiological characteristics of MCN-L with and without biliary prolapse were compared.15% (5/34) of MCN-Ls showed biliary prolapse confirmed at pathology. None of MCN-Ls were associated with invasive carcinoma. Patients with biliary prolapse were significantly younger than those without (median 27 years [22-56] vs. median 51 years [14-82], p = 0.03). MCN-Ls with biliary prolapse were significantly smaller than those without (median 6.4 cm [2.2-7.5] vs. median 9.6 cm [3.1-21], p = 0.01). The upstream bile duct was dilated more frequently in MCN-Ls with biliary prolapse (100% vs. 38%, p = 0.02). Jaundice was significantly more common in MCN-Ls with biliary prolapse (80 vs 3%, p = 0.0005). Other clinical or radiological features were not significantly different between two groups.Biliary prolapse was found in 15% of MCN-Ls. MCN-Ls with biliary prolapse were significantly smaller and were more commonly associated with upstream bile duct dilation and jaundice than those without biliary prolapse.

Eosinophilic gastrointestinal disorders (EGIDs) are inflammatory conditions of the gastrointestinal tract that are characterized by tissue eosinophilia and end-organ dysfunction or damage. Primary EGIDs are associated with atopy and other allergic conditions, whereas secondary EGIDs are associated with underlying systemic diseases or hypereosinophilic syndrome. Within the spectrum of EGIDs, eosinophilic esophagitis is the most prevalent. Eosinophilic gastroenteritis and eosinophilic colitis are relatively uncommon. Eosinophilic infiltration of the liver, biliary tree, and/or pancreas also can occur and mimic other inflammatory and malignant conditions. Although endoscopic evaluation is the method of choice for eosinophilic esophagitis, radiologic evaluation of the esophagus plays an important role in the assessment of disease severity. CT and MR enterography are the modalities of choice for demonstrating specific forms of eosinophilic gastroenteritis. CT and MRI are important in the detection of abdominal visceral involvement in EGIDs. Diagnosis is often challenging and relies on symptoms, imaging findings, histologic confirmation of tissue eosinophilia, and correlation with peripheral eosinophilia. Imaging is crucial for identifying characteristic organ-specific findings, although imaging findings are not specific. When promptly treated, EGIDs usually have a benign clinical course. However, a delayed diagnosis and associated surgical interventions have been associated with morbidity. Therefore, a radiologist's knowledge of the imaging findings of EGIDs in the appropriate clinical settings may aid in early diagnosis and thereby improve patient care. An overview of the clinical features and imaging findings of EGIDs and the eosinophilic disorders of associated abdominal viscera is provided.

MRI is routinely used for rectal cancer staging to evaluate tumor extent and to inform decision-making regarding surgical planning and the need for neoadjuvant and adjuvant therapy. Extramural venous invasion (EMVI), which is intravenous tumor extension beyond the rectal wall on histopathology, is a predictor for worse prognosis. T2-weighted images (T2WI) demonstrate EMVI as a nodular-, bead-, or worm-shaped structure of intermediate T2 signal with irregular margins that arises from the primary tumor. Correlative diffusion-weighted images demonstrate intermediate to high signal corresponding to EMVI, and contrast enhanced T1-weighted images demonstrate tumor signal intensity in or around vessels. Diffusion-weighted and post contrast images may increase diagnostic performance but decrease inter-observer agreement. CT may also demonstrate obvious EMVI and is potentially useful in patients with a contraindication for MRI. This article aims to review the spectrum of imaging findings of EMVI of rectal cancer on MRI and CT, to summarize the diagnostic accuracy and inter-observer agreement of imaging modalities for its presence, to review other rectal neoplasms that may cause EMVI, and to discuss the clinical significance and role of MRI-detected EMVI in staging and restaging clinical scenarios.

Use of gadoxetate disodium, a hepatobiliary gadolinium-based agent, in patients with chronic parenchymal liver disease offers the advantage of improved sensitivity for detecting hepatocellular carcinoma (HCC). Imaging features of liver observations on gadoxetate-enhanced MRI may also serve as biomarkers of recurrence-free and overall survival following definitive treatment of HCC. A number of technical and interpretative pitfalls specific to gadoxetate exist, however, and needs to be recognized when protocoling and interpreting MRI exams with this agent. This article reviews the advantages and pitfalls of gadoxetate use in patients at risk for HCC, and the potential impact on Liver Imaging Reporting and Data System (LI-RADS) imaging feature assessment and categorization. Background Due to their easy accessibility, CT scans have been increasingly used for investigation of gastrointestinal (GI) bleeding. Purpose To estimate the performance of a dual-phase, dual-energy (DE) GI bleed CT protocol in patients with overt GI bleeding in clinical practice and examine the added value of portal phase and DE images. Materials and Methods Consecutive patients with GI bleeding underwent a two-phase DE GI bleed CT protocol. Two gastroenterologists established the reference standard. Performance was estimated using clinical CT reports. Three GI radiologists rated confidence in GI bleeding in a subset of 62 examinations, evaluating first mixed kV arterial images, then after examining additional portal venous phase images, and finally after additional DE images (virtual non-contrast and virtual monoenergetic 50 keV images). Results 52 of 176 patients (29.5%) had GI bleeding by the reference standard. The overall sensitivity, specificity, and positive and negative predictive values of the CT GI bleed protocol for detecting GI bleeding were 65.4%, 89.5%, 72.3%, and 86.0%, respectively. In patients with GI bleeding, diagnostic confidence of readers increased after adding portal phase images to arterial phase images ( p = 0.002), without additional benefit from dual energy images. In patients without GI bleeding, confidence in luminal extravasation appropriately decreased after adding portal phase, and subsequently DE images ( p = 0.006, p = 0.018). Conclusion A two-phase DE GI bleed CT protocol had high specificity and negative predictive value in clinical practice. Portal venous phase images improved diagnostic confidence in comparison to arterial phase images alone. Dual-energy images further improved radiologist confidence in the absence of bleeding.

To evaluate the biliary tree and hepatic parenchymal findings on magnetic resonance imaging (MRI) and magnetic resonance cholangiopancreatography (MRCP) in small-duct primary sclerosing cholangitis (SD-PSC). Thirty-nine patients with biopsy-proven primary sclerosing cholangitis (PSC) without any bile duct abnormality on MRCP (n = 15) or ERCP (n = 24) at the time of diagnosis were identified. Follow-up MRCP was available in 36/39 patients (12/15 Baseline MRCP group and 24 Baseline ERCP group). Two radiologists in consensus assessed the MRI/MRCP findings. The baseline MRI/MRCP of 15 SD-PSC patients was compared with MRI/MRCP of 15 normal healthy potential liver donors (Control group). Comparisons were made between SD-PSC patients and the Control group, and between baseline and follow-up MRI/MRCP findings in the SD-PSC patients. In the 15 Baseline MRCP SD-PSC subjects, the biliary tree was normal with a trend of larger bile ducts compared to the Control group. Periductal enhancement (arterial phase: 70%, 7/10; delayed phase: 90%, 9/10), heterogeneous parenchymal signal on T2-weighted (53%, 8/15) and post contrast-enhanced images (70%, 7/10), and enlarged periportal lymph nodes (73%, 11/15) were frequently present in patients with SD-PSC. Eight (33%) of 24 SD-PSC patients who had normal MRCP at baseline MRCP or initial follow-up MRCP after normal baseline ERCP showed large-duct PSC (LD-PSC) features on follow-up and the 10-year cumulative incidence for progression to LD-PSC rate was 8.5%. SD-PSC patients have a normal biliary tree but frequently have peribiliary enhancement, abnormal parenchymal signal intensity, and periportal lymphadenopathy. One-third shows progression to LD-PSC on follow-up.

Purpose To evaluate the diagnostic performance and interrater reliability of the Liver Imaging Reporting and Data System (LI-RADS) version 2014 in differentiating hepatocellular carcinoma (HCC) from non-HCC malignancy in a population of patients at risk for HCC. Materials and Methods This retrospective HIPAA-compliant institutional review board-approved study was exempt from informed consent. A total of 178 pathology-proven malignant liver masses were identified in 178 patients at risk for HCC but without established extrahepatic malignancy from August 2012 through August 2015. Two readers blinded to pathology findings and clinical follow-up data independently evaluated a liver protocol magnetic resonance or computed tomography study for each lesion and assigned LI-RADS categories, scoring all major and most ancillary features. Statistical analyses included the independent samples t test, x2 test, Fisher exact test, and Cohen k. Results This study included 136 HCCs and 42 non-HCC malignancies. Specificity and positive predictive value of an HCC imaging diagnosis (LR-5 or LR-5V) were 69.0% and 90.5%, respectively, for reader 1 (R1) and 88.3% and 95.5%, respectively, for reader 2 (R2). Tumor in vein was a common finding in patients with non-HCC malignancies (R1, 10 of 42 [23.8%]; R2, five of 42 [11.9%]). Exclusion of the LR-5V pathway improved specificity and positive predictive value for HCC to 83.3% and 92.9%, respectively, for R1 (six fewer false-positive findings) and 92.3% and 96.4%, respectively, for R2 (one fewer false-positive finding). Among masses with arterial phase hyperenhancement, the rim pattern was more common among non-HCC malignancies than among HCCs for both readers (R1: 24 of 36 [66.7%] vs 13 of 124, [10.5%], P < .001; R2: 27 of 35 [77.1%] vs 21 of 123 [17.1%], P < .001) (k = 0.76). Exclusion of rim arterial phase hyperenhancement as a means of satisfying LR-5 criteria also improved specificity and positive predictive value for HCC (R1, two fewer false-positive findings). Conclusion Modification of the algorithmic role of tumor in vein and rim arterial phase hyperenhancement improves the diagnostic performance of LI-RADS version 2014 in differentiating HCC from non-HCC malignancy. © RSNA, 2017 Online supplemental material is available for this article.

The purpose of this study was to determine the frequency with which biphenotypic primary liver carcinoma (also called hepatocholangiocarcinoma) may be misclassified as hepatocellular carcinoma (HCC) when only Liver Imaging Reporting and Data System (LI-RADS) major features are used and after consideration of ancillary features.A review of all pathologically proven biphenotypic primary liver carcinomas diagnosed at one institution from 2006 to 2014 was performed. Two subspecialized abdominal imagers independently reviewed cases using LI-RADS version 2014 and assigned major features, ancillary features, and additional findings. The number of lesions meeting imaging criteria for HCC was determined after assessment of major features alone and after the addition of ancillary features.Sixty-one patients (30 men, 31 women; mean age, 62 years; range, 22-89 years) with biphenotypic primary liver carcinomas who underwent pretreatment multiphasic contrast-enhanced MRI (48 patients) or CT (13 patients) were included. According to LI-RADS major features alone, 33 (54.1%) lesions met criteria for HCC and therefore might have been misclassified. Thirteen had arterial phase hyperenhancement, washout, and a capsule. Twenty had arterial phase hyperenhancement with either washout (15 lesions) or a capsule (five lesions). After evaluation of ancillary features, 29 of these potential mimics exhibited at least one ancillary feature favoring non-HCC malignancy, possibly leading to appropriate reclassification. Of the four carcinomas that met criteria for HCC by major features and did not have ancillary features favoring non-HCC malignancy, two (3.3% of all tumors) fell within the Milan criteria.Most biphenotypic primary liver carcinomas have features of non-HCC malignancy and can be correctly categorized as such. Addition of ancillary features to major features may improve diagnostic accuracy over systems in which only major features are used.

The Liver Imaging Reporting and Data System (LI-RADS) standardizes the interpretation and reporting of imaging examinations in patients at risk for hepatocellular carcinoma (HCC). For focal liver observations it assigns categories (LR-1 to 5, LR-M, LR-TIV), which reflect the relative probability of benignity or malignancy of the respective observation. The categories assigned are based on major and ancillary image features, which have been developed by the American College of Radiology (ACR) and validated in many studies. This review summarizes the relevant CT and MRI features and presents an image-guided approach for readers not familiar with LI-RADS on how to use the system. The widespread adoption of LI-RADS for reporting would help reduce inter-reader variability and improve communication among radiologists, hepatologists, hepatic surgeons and oncologists, thus leading to improved patient management.

Purpose To determine in a large multicenter multireader setting the interreader reliability of Liver Imaging Reporting and Data System (LI-RADS) version 2014 categories, the major imaging features seen with computed tomography (CT) and magnetic resonance (MR) imaging, and the potential effect of reader demographics on agreement with a preselected nonconsecutive image set. Materials and Methods Institutional review board approval was obtained, and patient consent was waived for this retrospective study. Ten image sets, comprising 38-40 unique studies (equal number of CT and MR imaging studies, uniformly distributed LI-RADS categories), were randomly allocated to readers. Images were acquired in unenhanced and standard contrast material-enhanced phases, with observation diameter and growth data provided. Readers completed a demographic survey, assigned LI-RADS version 2014 categories, and assessed major features. Intraclass correlation coefficient (ICC) assessed with mixed-model regression analyses was the metric for interreader reliability of assigning categories and major features. Results A total of 113 readers evaluated 380 image sets. ICC of final LI-RADS category assignment was 0.67 (95% confidence interval [CI]: 0.61, 0.71) for CT and 0.73 (95% CI: 0.68, 0.77) for MR imaging. ICC was 0.87 (95% CI: 0.84, 0.90) for arterial phase hyperenhancement, 0.85 (95% CI: 0.81, 0.88) for washout appearance, and 0.84 (95% CI: 0.80, 0.87) for capsule appearance. ICC was not significantly affected by liver expertise, LI-RADS familiarity, or years of postresidency practice (ICC range, 0.69-0.70; ICC difference, 0.003-0.01 [95% CI: -0.003 to -0.01, 0.004-0.02]. ICC was borderline higher for private practice readers than for academic readers (ICC difference, 0.009; 95% CI: 0.000, 0.021). Conclusion ICC is good for final LI-RADS categorization and high for major feature characterization, with minimal reader demographic effect. Of note, our results using selected image sets from nonconsecutive examinations are not necessarily comparable with those of prior studies that used consecutive examination series. © RSNA, 2017.

A very large range of disorders, from benign, self-limited conditions, to processes requiring emergency surgery, can present with acute abdominal and pelvic pain. The radiologist plays a substantial role in the routine diagnosis and management of patients with these disorders. Similarly, the radiologist plays a major role in the routine evaluation of patients with abdominal and pelvic trauma, whether blunt or penetrating. Computed tomography (CT) remains the mainstay of the evaluation of such patients, although protocols have evolved along with technological advancements. Ultrasound (US) has a continued important role, particularly for evaluation of right upper quadrant pain, for evaluation of suspected gynecologic disorders, and for evaluation of pediatric and pregnant patients with an acute abdomen. Magnetic resonance imaging (MRI) has a growing role for initial evaluation as well as follow-up of selected patients with a variety of acute abdominal and pelvic conditions (usually non-traumatic). Although it is not possible to cover every aspect of imaging of acute non-traumatic and traumatic conditions of the abdomen and pelvis in a single chapter, an overview is presented, with key concepts and teaching points.

Acute disorders of the abdominal aorta are potentially lethal conditions that require prompt evaluation and treatment. Computed tomography (CT) is the primary imaging method for evaluating these conditions because of its availability and speed. Volumetric CT acquisition with multiplanar reconstruction and three-dimensional analysis is now the standard technique for evaluating the aorta. MR imaging may be useful for select applications in stable patients in whom rupture has been excluded. Imaging is indispensable for diagnosis and treatment planning, because management has shifted toward endoluminal repair. Acute abdominal aortic conditions most commonly are complications of aneurysms and atherosclerosis.

Imaging plays a key role in the diagnosis and management of ruptured abdominal aortic aneurysms (RAAA) as well as acute conditions that may result in aortic rupture, including aortic intramural hematoma (IMH) and penetrating atherosclerotic ulcer (PAU). Today, the most commonly used diagnostic tools for ruptured AAA include computed tomography (CT) and focused assessment with sonography for trauma (FAST). CT scans tailored to evaluate the aorta should include a precontrast scan and a high-quality CT angiogram to evaluate the aortic lumen and to permit multiplanar reconstructions (MPR) to potentially provide measurements for endoluminal therapy. Radiologists and clinicians evaluating these scans need to understand the findings of aortic rupture and related conditions, including the high-attenuation crescent, tangential calcium, and draped aorta signs. FAST is tailored to evaluate free fluid (usually blood) in the peritoneal, pericardial, and pleural space; its critical relevance here is identifying peritoneal blood in RAAA patient, at bedside in the emergency room (American Institute of Ultrasound in Medicine and American College of Emergency Physicians. J Ultrasound Med 33(11):2047–2056, 2014).

The purpose of this study was to determine the prevalence of nonpolypoid adenomas and the sensitivity of CT colonography (CTC) in their detection by use of the restricted criteria of height-to-width ratio50% and height elevation≤3 mm.In the National CT Colonography Trial (American College of Radiology Imaging Network protocol 6664), a cohort of 2531 participants without symptoms underwent CTC and screening colonoscopy. The CTC examinations were interpreted with both 2D and 3D techniques. Nonpolypoid adenomatous polyps identified with CTC or colonoscopy were retrospectively reviewed to determine which polyps met the restricted criteria. The prevalence of nonpolypoid adenomas and the prospective sensitivity of CTC were determined. Descriptive statistics were used to report the prevalence, size, and histologic features. The sensitivities (with 95% CIs) for nonpolypoid and polypoid lesions were compared by two-sided Z test for independent binomial proportions.The retrospective review confirmed 21 nonpolypoid adenomas, yielding a prevalence of 0.83% (21 of 2531 participants). Eight (38.1%) were advanced adenomas, many (50% [4/8]) only because of large size (≥10 mm). The overall per polyp sensitivity of CTC (combined 2D and 3D interpretation) for detecting nonpolypoid adenomas≥5 mm (n=21) was 0.76; ≥6 mm (n=16), 0.75; and ≥10 mm (n=5), 0.80. These values were not statistically different from the sensitivity of detecting polypoid adenomas (p0.37).In this large screening population, nonpolypoid adenomas had a very low prevalence (1%), and advanced pathologic features were uncommon in polyps10 mm in diameter. Most nonpolypoid adenomas are technically visible at CTC. The prospective sensitivity is similar to that for polypoid adenomas when the interpretation combines both 2D and 3D review.

BACKGROUND Imaging diagnosis of hepatocellular carcinoma (HCC) presents an important pathway for transplant exception points and priority for cirrhotic patients. The purpose of this retrospective study is to evaluate the validity of the new Organ Procurement and Transplant Network (OPTN) classification system on patients undergoing transplantation for HCC. METHODS One hundred twenty-nine patients underwent transplantation for HCC from April 14, 2006 to April 18, 2011; a total of 263 lesions were reported as suspicious for HCC on pretransplantation magnetic resonance imaging. Magnetic resonance imaging examinations were reviewed independently by two experienced radiologists, blinded to final pathology. Reviewers identified major imaging features and an OPTN classification was assigned to each lesion. Final proof of diagnosis was pathology on explant or necrosis along with imaging findings of ablation after transarterial chemoembolization. RESULTS Application of OPTN imaging criteria in our population resulted in high specificity for the diagnosis of HCC. Sensitivity in diagnosis of small lesions (≥1 and

Every year more than one million new patients are diagnosed with colon cancer worldwide. Although multiple prospective randomized trials and observational studies have demonstrated that mortality from colon cancer can be reduced with screening and removal of adenomatous polyps, compliance with screening guidelines remains low. Recent CT colonography (CTC) trials have shown that CTC is capable of demonstrating adenomatous polyps ≥10 mm (and in most cases ≥6 mm) with sensitivities comparable to those for optical colonoscopy. Based on these results, at least two expert panels have recommended CTC as an option for colorectal cancer screening. Despite these endorsements, the Centers for Medicare and Medicaid Services (CMS) in the United States recently decided to deny coverage of CTC for colorectal cancer screening. This article addresses the reservations raised by CMS and provides a perspective on whether CTC is ready for routine use as a colorectal cancer screening test.

The purpose of this study was to evaluate the clinical significance of endoleaks detected on combined arterial and delayed contrast-enhanced follow-up CT examinations of patients who have undergone endovascular aneurysm repair of abdominal aortic aneurysm.One hundred forty-four patients underwent periodic follow-up CT examinations 12-72 months after abdominal aortic aneurysm repair. The CT protocol consisted of an unenhanced scan and contrast-enhanced scans in the arterial and 90-second delayed phases. The endoleaks detected on dual-phase CT scans were evaluated in association with the outcome (therapeutic intervention or endoleak resolution).The 144 patients underwent 728 CT examinations with a mean follow-up period of 35.5 +/- 14.5 months. Fifty endoleaks were detected in 50 (34.7%) of the patients. Eight endoleaks were detected in the arterial phase only, eight in the delayed phase only, and 34 in both phases. Intervention was performed to manage 16 endoleaks detected in both phases. CT showed that three endoleaks were stable (two in the arterial phase only and one in both phases) and that 31 had resolved completely (six in the arterial phase only, eight in the delayed phase only, and 17 in both phases). This finding represents a higher frequency of resolution of endoleaks detected in one phase only than in both phases (Fisher's exact test, p = 0.006).Endoleaks detected only in the delayed phase of CT had resolved spontaneously without intervention. Therefore, we can consider eliminating the delayed phase of acquisition to minimize radiation exposure.

An 87-year-old man underwent attempted laparoscopic cholecystectomy. The procedure was characterized by significant inflammation and bleeding requiring conversion to an open procedure. Postoperatively, the patient had continued bile drainage from his surgical drain. He was referred to our institution and found to have complete transection of his common bile duct. Incidentally, he was noted on imaging studies to have absence of his right liver with associated left liver hypertrophy. This was characterized by complete absence of the right portal vein and right bile duct. Review of his preoperative imaging confirmed this finding of right liver agenesis and very unusual hepatic vein anatomy. This represents the first reported case of bile duct injury in the setting of right liver agenesis. We review the details of the case and the natural history of agenesis of a hemiliver.

The objective of the PREDICT (patients with renal impairment and diabetes undergoing computed tomography) study was to compare the incidence of contrast-induced nephropathy (CIN) after administration of low-osmolar (iopamidol 370, 796 mOsm/kg) or isoosmolar (iodixanol 320, 290 mOsm/kg) contrast medium in patients with diabetes and chronic kidney disease undergoing CT.Two hundred sixty-three patients with moderate to severe chronic kidney disease (estimated glomerular filtration rate [GFR] = 20-59 mL/min/1.73 m(2)) and diabetes mellitus were randomized to receive at least 65 mL of iopamidol 370 or iodixanol 320 for a CT procedure. Serum creatinine levels were measured at baseline and 48-72 hours after contrast administration. CIN was defined as an increase in the serum creatinine level after contrast administration ofor= 25% from the baseline level. The incidence of CIN in the total study population and the incidence of CIN in patients at increased risk for CIN were compared using Fisher's exact test.Two hundred forty-eight patients were included in the CIN analysis: 125 receiving iopamidol 370 and 123 receiving iodixanol 320. Study population demographics were comparable, as was baseline renal function (estimated GFR = 47.6 mL/min/1.73 m(2) for the iopamidol 370 group vs 49.9 mL/min/1.73 m(2) for the iodixanol 320 group; p = 0.16). Increases in the serum creatinine value ofor= 25% occurred in seven patients (5.6%) receiving iopamidol 370 and in six patients (4.9%) receiving iodixanol 320 (95% CI, -4.8% to 6.3%; p = 1.0). The mean serum creatinine change from the baseline level was 0.04 mg/dL in both groups (analysis of covariance, p = 0.80). In patients with a baseline serum creatinine value ofor= 2.0 mg/dL, baseline estimated GFR ofor= 40 mL/min/1.73 m(2), or those receiving140 mL of contrast medium, the incidence of CIN was low and comparable between the two study groups (p = 1.0 in all instances).The incidence of CIN in patients with diabetes and chronic kidney disease receiving IV contrast medium was not significantly different after CT using iopamidol 370 or iodixanol 320.

Cancer patients undergo frequent imaging examinations. Computed tomography (CT) examinations for tumor staging and assessment of treatment response generally require administration of intravascular contrast medium. Iodinated contrast agents for CT are associated with the risk of contrast-induced nephrotoxicity (CIN), particularly in patients with impaired renal function and diabetes. In many cancer patients the risk of complications from intravascular contrast medium administration is compounded by advanced age, dehydration and coadministration of nephrotoxic chemotherapeutic drugs. In this article I review the definition, clinical manifestations, possible mechanisms and risk factors for CIN, and provide recommendations for prevention of this potentially life-threatening complication.

Because of its fast scan speed, multi-detector row CT (MDCT) imposes a challenge to optimization of contrast enhancement and scan timing. However, when this challenge is overcome with the use of precisely designed protocols, MDCT provides us with an opportunity to use contrast medium more efficiently and flexibly than with single-detector row CT (SDCT). To fully achieve the benefits of MDCT, it is crucial for radiologists and technologists to 1) understand the factors that determine contrast enhancement and scan timing for MDCT, and 2) identify the required modifications in protocol design to optimize contrast enhancement for MDCT. Clinical considerations for CTA and hepatic imaging are described in this article.

Computed tomography colonography (CTC) (also known as ‘virtual colonoscopy’) is a noninvasive method of imaging the colon using helical CT. Although CTC has been shown to be useful for certain clinical indications, it has not yet been endorsed as a colorectal cancer screening test. The purpose of this article is to review the current status of CTC for colorectal cancer screening. CTC is an accurate method to detect colonic polyps and to select patients who would benefit from colonoscopy. The major advantages of CTC over conventional colonography include its relatively low risk and greater tolerance by patients. In this article, the CTC procedure and results of clinical trials are reviewed, as well as potential pitfalls related to CTC performance and interpretation. Finally, radiation dose, the discovery of incidental extracolonic findings with CTC, bowel preparation methods, and computer-aided diagnosis are addressed.

The spleen has the same relationship to the circulatory system that the lymph nodes have to the lymphatic system. A wide range of diseases can affect the spleen. Pathologic conditions of the spleen can be classified into the following categories: congenital diseases (accessory spleen, polysplenia, and asplenia); trauma; inflammation (abscess, candidiasis, histoplasmosis, and sarcoidosis); vascular disorders (infarction, diseases affecting the splenic vasculature, and arteriovenous malformation); hematologic disorders (sickle cell disease and extramedullary hematopoiesis); benign tumors (cysts, hemangioma, diffuse hemangiomatosis of the spleen, and hamartoma); malignant tumors (sarcoma, lymphoma, and metastases); and other disease processes that affect the spleen diffusely (portal hypertension, Gaucher disease, and sickle cell disease) or focally (Gamna-Gandy nodules). New magnetic resonance (MR) imaging techniques have increased the role of MR imaging in detection and characterization of splenic diseases. MR imaging is an excellent tool for diagnosis and evaluation of focal lesions and pathologic conditions of the spleen.

Clinical assessment of women with urethral symptoms is difficult, necessitating further evaluation with imaging. Urethrography provides limited information on luminal abnormalities of the urethra. Recent advances in ultrasound (US) and magnetic resonance (MR) imaging have dramatically improved evaluation of the female urethra, clarifying findings at physical examination and providing accurate road maps for surgeons. High-resolution transvaginal US, transperineal US, and transurethral US are reliable techniques for diagnosis and characterization of urethral abnormalities. High-resolution multiplanar MR imaging with phased-array pelvic and endovaginal coils demonstrates the urethral anatomy in greater detail. In women with urethral diverticula, US and MR imaging demonstrate the number of diverticula and the location, size, configuration, and possible contents of the sac. Most important, the position of the neck of the diverticulum may be identified for the surgeon. Imaging features do not allow differentiation between histologic subtypes of urethral carcinoma; the diagnosis is established with histopathologic examination. Periurethral cysts do not communicate with the urethra and therefore can often be differentiated from urethral diverticula at endocavitary MR imaging. High-resolution multiplanar US and MR imaging allow comprehensive evaluation of abnormalities of the female urethra.

Tumors of the renal medulla cover a wide spectrum, with characteristic histomorphology and variable biologic profiles. Renal medullary tumors can be categorized into benign and malignant neoplasms based on histologic features and clinico-biologic behavior. They can be further classified into pediatric and adult tumors based on the patient age group. When small, renal medullary tumors may be differentiated from the more common renal adenocarcinomas by their central location and certain demographic characteristics. Although most large malignant medullary tumors demonstrate imaging findings that are indistinguishable from those of other renal malignancies, some tumors demonstrate imaging findings that may suggest a specific diagnosis.

OBJECTIVE To compare the findings of renal ultrasonography (US) in the evaluation of patients with and with no haematuria. The increased use of cross-sectional imaging and US has led to a dramatic improvement in the diagnosis of renal masses, such that computed tomography and/or US have been integrated into the diagnostic evaluation of haematuria, and many more incidental renal lesions are now detected. Thus it is possible that the lesions identified during evaluation for haematuria are incidental, i.e. identified serendipitously, and unrelated to the haematuria. PATIENTS AND METHODS We retrospectively compared the US findings obtained from 301 patients referred for new-onset haematuria to those obtained from 600 patients being evaluated for other than urological reasons. All imaging and patient charts were reviewed to verify the clinical and radiological data. RESULTS Haematuria was associated with all renal abnormalities, with an odds ratio (OR, 95% confidence interval) of 4.7 (3.6-7.3). Importantly, haematuria was associated with a renal mass, with an OR of 6.7 (2.8-16.3). Subset analysis revealed that patients with macroscopic and microscopic haematuria had significantly more renal abnormalities (OR 4.7, 2.7-8.2, and 5.3, 3.2-8.8, respectively) and renal masses (OR 7.3, 2.7-20.3, and 6.5, 2.3-18.6, respectively) than controls. CONCLUSIONS Both macroscopic and microscopic haematuria are associated with a greater risk of identifying renal lesions. This supports the conclusion that the renal lesions identified with modern imaging techniques during the evaluation of both microscopic and macroscopic haematuria are not serendipitous.

Rationale and Objectives. The feasibility of using gadolinium contrast medium for computed tomography angiography (CTA) in multi-detector row computed tomography and the effect of contrast medium dilution was investigated. Materials and Methods. Three pigs were each scanned in multiple sessions with injections of non-dilute and dilute contrast medium at a dose of 0.3 mmol/kg body weight. Non-spiral dynamic scanning at a fixed mid-abdominal aortic level and thoracoabdominal CTA were performed. Results. The magnitude of peak aortic enhancement was not significantly different between dilute and non-dilute contrast medium injections (P = .88), but the former showed earlier enhancement (mean of 2.3 seconds sooner, P < .01) than the latter. CT angiography with gadolinium contrast medium showed much lower enhancement than iodine contrast medium, but small vessels were readily identifiable. Conclusion. Gadolinium contrast medium combined with multi-detector row computed tomography may provide clinically useful CTA. Dilution of contrast medium shortens the enhancement time but has little effect on the magnitude.

Aortic aneurysm rupture, aortic dissection, penetrating atherosclerotic ulcer, acute aortic occlusion, traumatic aortic injury, and aortic fistula represent acute abdominal aortic conditions. Because of its speed and proximity to the emergency department, helical CT is the imaging test of choice for these conditions. MR imaging also plays an important role in the imaging of aortic dissection and penetrating atherosclerotic ulcer. Ultrasound is helpful when CT is not readily available and the patient is unable or too unstable to undergo MR imaging. Because CT is the dominant imaging method for diagnosing acute abdominal aortic conditions, this article focuses on CT techniques and manifestations of this group of acute abdominal disorders.

To evaluate a histogram analysis method for differentiating adrenal adenoma from metastasis at computed tomography (CT).In a retrospective review of 2 years of clinical CT records, 223 adrenal adenomas in 193 patients (115 with contrast material-enhanced CT, 43 with unenhanced and enhanced CT, and 35 with unenhanced CT) and 31 metastases (25 patients with enhanced CT) were found. In 158 patients with adenomas at enhanced CT, diagnosis was based on stable mass size for more than 1 year (n = 135) and characteristic signal intensity decrease at chemical shift magnetic resonance imaging (n = 23). In 35 patients with adenomas at unenhanced CT, mean attenuation was 10 HU or less. Diagnosis of all metastases was based on rapid growth of a mass or new mass in less than 6 months in patients with cancer. Adrenal metastases with extensive necrosis were excluded. Histogram analysis was performed in a circular region of interest (ROI) for mean attenuation, number of pixels, and range of pixel attenuation for all pixels and for the subset of pixels with less than 0 HU ("negative" pixels). Correlation between mean attenuation and percentage negative pixels was calculated.Negative pixels were present in all 74 unenhanced adenomas with mean attenuation of 10 HU or less and in 14 of 16 unenhanced adenomas with mean attenuation above 10 HU. Of 184 enhanced adenomas, only 20 had mean attenuation of 10 HU or less, but 97 contained negative pixels (77 of these 97 masses had mean attenuation above 10 HU). Increase in percentage negative pixels was highly correlated with decrease in mean attenuation of both unenhanced and enhanced adenomas. None of the adrenal metastases had mean attenuation of 10 HU or less or contained negative pixels.The histogram method is far more sensitive than the 10-HU threshold method for diagnosis of adrenal adenomas at enhanced CT, with specificity maintained at 100%.

Rationale and Objectives The authors investigated the feasibility of using computed tomography (CT) with CO2 gas as a negative contrast agent for detecting pulmonary emboli in a porcine model. Materials and Methods Seven pigs with or without pulmonary emboli underwent thoracic imaging with multi–detector row spiral CT. To identify optimal injection and scanning protocols, the first four pigs were scanned repeatedly in the supine and prone positions with different scan delays (10, 15, and 20 seconds) and different volumes of CO2 (60, 120, 180, and 240 mL), which were hand infused (each infusion took 10–15 seconds). The last five pigs with emboli were scanned with iodinated contrast medium and then rescanned with 120 or 180 mL of CO2. The CO2 volumes and scan delays were qualitatively assessed. The supine and prone CT scans and the number and location of thrombi depicted in the CO2- and contrast material–enhanced CT scans were compared. Results Because the pulmonary artery in pigs is in the posterior anatomy, the prone position was more effective than the supine position with CO2 enhancement. An infusion of 120 mL of CO2 was sufficient to enhance the entire pulmonary artery, and scanning timed to coincide with the completion of infusion was the most effective. Both the CO2- and con-trast-enhanced CT scans demonstrated all thrombi. Thrombi were more apparent on the CO2-enhanced CT scans than on the contrast-enhanced scans because of the high contrast interface between soft tissue and gas. However, two of the seven pigs with thrombi experienced abrupt cardiac arrest after CO2-enhanced scanning and could not be resuscitated. The cause of these events was not determined in the current study. Conclusion The CT depiction of pulmonary emboli is feasible with CO2 gas as a negative contrast agent and may even be superior to that with iodinated contrast media. Further studies are required to evaluate the safety of this method and to develop an improved delivery of CO2 gas for this application.

To prospectively evaluate multiobserver diagnostic performance and reader agreement for colorectal polyp detection in a well-characterized cohort of patients with increased number of polyps, compared with an average-risk patient, with colonoscopy as the reference standard.A cohort of 70 patients suspected of having polyps was examined with spiral computed tomographic (CT) colonography, with colonoscopy performed the same day. After air insufflation per rectum, supine and prone images were obtained with single-detector row CT (5-mm collimation, 8-mm table increment, 2-mm reconstruction interval). Images were analyzed independently by four experienced abdominal radiologists using two-dimensional multiplanar reformation followed by selective use of three-dimensional endoscopic volume-rendered images. Data were analyzed both per polyp and per patient.Analysis per polyp demonstrated a pooled sensitivity of 0.68 for lesions 10 mm or larger (n = 40), with 75% agreement among the four readers. Analysis per patient demonstrated improved detection and agreement, with a pooled sensitivity of 0.88 for patients with polyps or cancers 10 mm or larger (n = 28), with 94% agreement. When sensitivity and receiver operating characteristic analyses were analyzed per polyp size threshold, results among readers converged and peaked at polyp diameters of approximately 10 mm.In this patient cohort, diagnostic performance and interobserver agreement with single-detector row CT colonography was sufficient for detection of patients with lesions 10 mm or larger, with more variable results for smaller polyps.

Purpose: We evaluated the accuracy of noncontrast spiral computerized tomography (CT) for determining urinary stone size compared with plain x-ray.Materials and Methods: We retrospectively analyzed noncontrast helical CT and plain x-ray of the kidneys, ureters and bladder images of 39 patients with urolithiasis who underwent each study from July 1997 to February 1999. Stone size on x-ray was measured in the craniocaudal and transverse dimensions by a single radiologist (I.C.B.). The size of the same stone was then determined on blinded noncontrast spiral CT images. Stones that were less than 3 mm. were excluded from study. On CT estimated craniocaudal dimension was based on collimation thickness, the reconstruction interval and the number of images on which the stone was visualized.Results: We evaluated 58 stones 3 mm. or larger in the greatest dimension on plain x-ray of the kidneys, ureters and bladder, and noncontrast spiral CT, including 15 in the distal ureter, 7 in the mid or proximal ureter and 36 ...

Objective: To determine the effectiveness of the CT histogram method to characterize indeterminate adrenal nodules above 10 Hounsfield units (HU) on noncontrast CT. Materials and methods: Retrospective review of clinical CT data from January 2005 through 2008 identified 194 indeterminate adrenal nodules (>10 HU on noncontrast CT) in 175 patients. 20 nodules in 18 patients were excluded due to large standard deviation (SD > 30) of HU values. Of the remaining 174 nodules, 131 were classified as benign lipid-poor nodules based on size stability for ≥1 year (104), in- and opposed-phase MRI (17), adrenal washout CT (3), or biopsy (7). 43 were classified as malignant by size increase over a short time (30), avid FDG uptake on PET/CT (15), or biopsy (5). Histogram analysis was performed by drawing a circular region of interest on all adrenal nodules. Mean attenuation, total number of pixels, number of negative pixels, and percentage of negative pixels were recorded for each nodule. Results: At the threshold value of >10% negative pixels, 59/131 benign nodules were correctly characterized, but 1/43 malignant nodules was falsely characterized as benign (sensitivity 45%, specificity 98%, positive predictive value 98%). With a slightly higher threshold value of >15% negative pixels, there were no false benign judgments. 36 nodules had more than 15% negative pixels, all of which were benign (sensitivity 27%, specificity 100%, positive predictive value 100%). In the subgroup of benign nodules measuring 11–20 HU, 80% and 54% were identified with threshold values of >10% and >15% negative pixels, respectively. Conclusion: The CT histogram method with a threshold value of >10% negative pixels can identify many benign adrenal nodules with attenuation values >10 HU on unenhanced CT with extremely high specificity. A threshold of >15% negative pixels can achieve 100% specificity. This method is highly robust provided very “noisy” CT examinations (SD > 30) are eliminated.
