The disease caused by the new severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), ie, coronavirus disease 2019 (COVID-19), has become a global pandemic since it was first reported in Wuhan, China in December 2019. Its severe clinical manifestations, which often necessitate admission to intensive care units, and high mortality rate represent a therapeutic challenge for the medical community. To date, no drugs have been approved for its treatment, and various therapeutic options are being assayed to address the pathophysiological processes underlying the clinical manifestations experienced by patients. New and old drugs administered as monotherapy or in combination to immunologically compromised patients may favor the development of adverse drug reactions, including drug hypersensitivity reactions, which must be identified and managed accordingly. Given the lack of herd immunity and the high rate of viral contagion, new cases are expected to emerge in the coming months. Thus, the probability of more adverse reactions or even new clinical manifestations may increase in parallel. Allergists must receive updated information on these treatments, as well as on the management of possible drug hypersensitivity reactions.

In this review article, we intend to summarize the diagnostic tools available for the study of drug hypersensitivity reactions to chemotherapy and biologicals, based on the experience and controversies of the main research groups leading the topic. Recent publications with large cohorts of patients reacting to these drugs are allowing for a better understanding of phenotyping, endotyping, and optimally managing these patients. There is a remarkable heterogeneity on the diagnostic and therapeutic approach among the different groups, and yet this has allowed for very interesting discoveries. Clinical history alone is insufficient for a diagnosis, but in vitro and in vivo biomarkers are essential for personalized management plans and precision medicine. Drug provocation testing is an essential tool and criterion standard. Despite heterogeneity, dedicated multidisciplinary drug desensitization program/unit, with dedicated space and personnel for their activities, have shown to be the optimal approach. In this acticle, we provide detailed information on diagnostics tools, with an specific focus on drug provocation testing. Looking forward, collaboration, standardization of techniques, and generalization of solidly established drug allergy teams in large hospitals are fundamental steps for the specialty of allergy in the twenty-first century.

Background Despite the increasing incidence of anaphylaxis, its underlying molecular mechanisms and biomarkers for appropriate diagnosis remain undetermined. The rapid onset and potentially fatal outcome in the absence of managed treatment prevent its study. Up today, there are still no known biomarkers that allow an unequivocal diagnosis. Therefore, the aim of this study was to explore metabolic changes in patients suffering anaphylactic reactions depending on the trigger (food and/or drug) and severity (moderate and severe) in a real-life set-up. Methods Eighteen episodes of anaphylaxis, one per patient, were analysed. Sera were collected during the acute phase (T1), the recovery phase (T2) and around 2-3 months after the anaphylactic reaction (T0: basal state). Reactions were classified following an exhaustive allergological evaluation for severity and trigger. Sera samples were analysed using untargeted metabolomics combining liquid chromatography coupled to mass spectrometry (LC-MS) and proton nuclear magnetic resonance spectroscopy (1 H-NMR). Results 'Food T1 vs T2' and 'moderate T1 vs T2' anaphylaxis comparisons showed clear metabolic patterns during the onset of an anaphylactic reaction, which differed from those induced by drugs, food + drug or severe anaphylaxis. Moreover, the model of food anaphylaxis was able to distinguish the well-characterized IgE (antibiotics) from non-IgE-mediated anaphylaxis (nonsteroidal anti-inflammatory drugs), suggesting a differential metabolic pathway associated with the mechanism of action. Metabolic differences between 'moderate vs severe' at the acute phase T1 and at basal state T0 were studied. Among the altered metabolites, glucose, lipids, cortisol, betaine and oleamide were observed altered. Conclusions The results of this exploratory study provide the first evidence that different anaphylactic triggers or severity induce differential metabolic changes along time or at specific time-point, respectively. Besides, the basal status T0 might identify high-risk patients, thus opening new ways to understand, diagnose and treat anaphylaxis.

[EN] BACKGROUND: Quinolones are the second most frequent cause of hypersensitivity reactions (HSRs) to antibiotics. A marked increase in the number of patients with HSRs to quinolones has been detected. OBJECTIVE: To describe the clinical characteristics of patients with HSRs to quinolones and present methods for their diagnosis. METHODS: Patients attending the allergy unit due to reactions suggestive of HSRs to quinolones were prospectively evaluated between 2005 and 2018. Diagnosis was achieved using clinical history, skin tests (STs), basophil activation tests (BATs), and drug provocation tests (DPTs) if ST and BAT results were negative. RESULTS: We included 128 subjects confirmed as having HSRs to quinolones and 42 found to be tolerant. Anaphylaxis was the most frequent entity in immediate HSRs and was most commonly induced by moxifloxacin. Patients were evaluated a median of 150 days (interquartile range, 60-365 days) after the reaction. Of patients who underwent ST and BAT, 40.7% and 70%, respectively, were positive. DPT with a quinolone was performed in 48 cases, giving results depending on the culprit drug: when moxifloxacin was involved, 62.5% of patients gave a positive DPT result to ciprofloxacin, whereas none reacted to levofloxacin. The risk of HSR was 96 times higher in subjects who reported moxifloxacin-induced anaphylaxis and 18 times higher in those reporting immediate reactions compared with clinical entities induced by quinolones other than moxi-floxacin and nonimmediate reactions. CONCLUSIONS: The diagnosis of HSR to quinolones is complex. The use of clinical history is essential as a first step. BAT shows higher sensitivity than STs. ResumenEl proyecto R-ALERGO, desarrollado entre la Universitat Politècnica de València y el Hospital Universitario La Fe, tiene como objetivo crear una aplicación móvil (App) que permita a los/las usuarios/as seleccionar, dentro de la ciudad de Valencia, las rutas más favorables para las personas alérgicas. En el desarrollo de la aplicación se ha seleccionado un conjunto de nueve variables ambientales presentes en la ciudad, que pueden influir en la aparición de manifestaciones clínicas en las personas alérgicas. Mediante el uso de un método de análisis espacial apoyado en tecnología de redes y sistemas de información geográfica se ha desarrollado la versión 01 de la App que se audita a través del Distintivo AppSaludable. On the urban areas there are sources of information about the exposition to environmental allergens and others potential factors of allergies, such as: weather conditions, vegetation, urban morphology, etc. For the last two years we have been developing, together with the “La Fe” Hospital of Valencia, the Cartography Institute of Valencia (Instituto Cartografico Valenciano), and the Polytechnic University of Valencia (UPV), a mobile phone App to help citizens to avoid high environmental allergens exposure areas while they are walking around the city. The App uses modified grid calculations where impedances have been calculated based on the variables that affect allergens exposure: temperature, humidity and wind direction, pollution level, trees, pollination, season period, pet areas, fountains and water surfaces. People’s allergy information and the factors that have effects on the allergens exposure were agreed by a group of specialized doctors. The design follows the recommendations of the Agency of Health Quality of Andalucia (Agencia de Calidad Sanitaria de Andalucia). R-ALERGO is the first App to avoid the exposure to air allergens in Spain. The App works following 4 steps: 1st Patient profile; 2nd Definition of the origin-destination of the route; 3rd Calculation of the route with the lower exposure to allergens; 4th Specific recommendations for users. The App R-ALERGO is developed following the concept of other applications dedicated to improve life quality of citizens and recovering, by using the idea of Smart Cities, the link between medicine and health, with the purpose of achieving more responsible cities towards their citizens.
