1. Scaling Technology Acceptance Analysis with Large Language Model (LLM) Annotation Systems

Abstract

Technology acceptance models effectively predict how users will adopt new technology products. Traditional surveys, often expensive and cumbersome, are commonly used for this assessment. As an alternative to surveys, we explore the use of large language models for annotating online user-generated content, like digital reviews and comments. Our research involved designing an LLM annotation system that transform reviews into structured data based on the Unified Theory of Acceptance and Use of Technology model. We conducted two studies to validate the consistency and accuracy of the annotations. Results showed moderate-to-strong consistency of LLM annotation systems, improving further by lowering the model temperature. LLM annotations achieved close agreement with human expert annotations and outperformed the agreement between experts for UTAUT variables. These results suggest that LLMs can be an effective tool for analyzing user sentiment, offering a practical alternative to traditional survey methods and enabling deeper insights into technology design and adoption., Comment: This is a preprint of a paper accepted for the 32nd International Conference on Information Systems Development (ISD 2024), Gdansk, Poland

Abstract

Poland is currently undergoing substantial transformation in its energy sector, and gaining public support is pivotal for the success of its energy policies. We conducted a study with 338 Polish participants to investigate societal attitudes towards various energy sources, including nuclear energy and renewables. Applying a novel network approach, we identified a multitude of factors influencing energy acceptance. Political ideology is the central factor in shaping public acceptance, however we also found that environmental attitudes, risk perception, safety concerns, and economic variables play substantial roles. Considering the long-term commitment associated with nuclear energy and its role in Poland's energy transformation, our findings provide a foundation for improving energy policy in Poland. Our research underscores the importance of policies that resonate with the diverse values, beliefs, and preferences of the population. While the risk-risk trade-off and technology-focused strategies are effective to a degree, we advocate for a more comprehensive approach. 5. A novel radiomics approach for predicting TACE outcomes in hepatocellular carcinoma patients using deep learning for multi-organ segmentation

Abstract

Abstract Transarterial chemoembolization (TACE) represent the standard of therapy for non-operative hepatocellular carcinoma (HCC), while prediction of long term treatment outcomes is a complex and multifactorial task. In this study, we present a novel machine learning approach utilizing radiomics features from multiple organ volumes of interest (VOIs) to predict TACE outcomes for 252 HCC patients. Unlike conventional radiomics models requiring laborious manual segmentation limited to tumoral regions, our approach captures information comprehensively across various VOIs using a fully automated, pretrained deep learning model applied to pre-TACE CT images. Evaluation of radiomics random survival forest models against clinical ones using Cox proportional hazard demonstrated comparable performance in predicting overall survival. However, radiomics outperformed clinical models in predicting progression-free survival. Explainable analysis highlighted the significance of non-tumoral VOI features, with their cumulative importance superior to features from the largest liver tumor. The proposed approach overcomes the limitations of manual VOI segmentation, requires no radiologist input and highlight the clinical relevance of features beyond tumor regions. Our findings suggest the potential of this radiomics models in predicting TACE outcomes, with possible implications for other clinical scenarios.

Abstract

Abstract Despite substantial progress in understanding the complex pathophysiology, hypertension remains a serious public health challenge affecting over 1.2 billion adults aged 30–79 years worldwide. Appropriate knowledge of the different pharmaceutical classes of antihypertensive agents and an understanding of the characteristics of individual molecules are essential to optimize clinical outcomes in patients with hypertension. We conducted a computer‐assisted web interviewing (CAWI) quantitative survey in Italy, Poland, and Turkey to investigate physicians’ prescriptions, knowledge, and perceptions of antihypertensive drugs with a focus on β‐blockers, to assess antihypertensive usage patterns and the reasons underlying prescription choices. The survey findings show that β‐blockers retain a pivotal role in the management of hypertension and are prescribed more often for patients with cardiovascular comorbidities than for patients with diabetic comorbidities. In all three countries, nebivolol is the only β‐blocker among the ones analyzed which is consistently prescribed to 20% or more of patients and is overall the most prescribed one for the population with comorbid diabetes. In terms of specific β‐blockers’ features, this study revealed knowledge gaps that underline the need for educational activities focused on the differences among β‐blockers, which are important in choosing the most suitable agent for individualized antihypertensive therapy.

Abstract

Introduction Numerous studies, but not all, have suggested a positive effect of allopurinol on the cardiovascular system. The randomised, double-blind, placebo-controlled study evaluating the effect of allopurinol on the risk of cardiovascular events in patients with high and very high cardiovascular risk, including the presence of long-COVID-19 syndrome (ALL-VASCOR) study aims to evaluate the efficacy of allopurinol therapy for improving cardiovascular outcomes in patients at high and very high cardiovascular risk excluding ischaemic heart disease. This is particularly important due to the high cost of cardiovascular disease treatment and its status as one of the leading causes of mortality.Methods and analysis The ALL-VASCOR study is a randomised, double-blind, placebo-controlled, multicentre trial that examines the effect of allopurinol therapy (200–500 mg of allopurinol daily) versus an equivalent dose of placebo on the risk of cardiovascular events in 1116 patients aged 40–70 with serum uric acid levels above 5 mg/dL at high and very high risk of cardiovascular disease. The ALL-VASCOR study will also assess the occurrence of long-COVID-19 syndrome. The study will measure primary and secondary as well as additional endpoints and the planned intervention will end on 31 July 2028 unless advised otherwise by the Safe Monitoring Board or other applicable authorities. Participant recruitment is planned to begin in March 2024 in Poland.Ethics and dissemination The study was ethically approved by the Bioethics Committee of Poznan University of Medical Sciences (No 03/23, 12 January 2023). 10. Influence of Non-Invasive Zirconium Oxide Surface Treatment on Phase Changes

Abstract

The aim of the research was to find a zirconia treatment method that would reduce or minimize the transformation from the tetragonal phase to the monoclinic phase. Background: Yttria-stabilized zirconia is increasingly chosen for the base of permanent prosthetic restorations. To achieve a good bond between the prosthetic cup and the veneer material, the material must be treated to achieve surface development. This is a mechanical process, during which an unfavorable transformation from the tetragonal into the monoclinic phase takes place, which leads to the weakening of the internal structure of zirconium dioxide, and later damages the prosthetic restoration. Methods: The tested material consisted of cylindrical samples of 3Y-TZP CeramillZi zirconium oxide, which were sintered after cutting out from the block. After sintering, the samples were subjected to the following types of processing: laser structuring, chemical etching and plasma etching. After the surface treatments, the samples were subjected to diffraction tests to determine the phase composition. Next, the wettability was tested to determine the surface free energy. Results: On the basis of the conducted tests, it was noticed that the applied treatments caused a phase transformation from the tetragonal to the monoclinic phase. After the process of chemical etching, the range of the monoclinic phase for the sample was 5%; after plasma etching, it was 8%, and after laser structuring, it was 2%. In addition, post-surface free energy studies have shown that zirconia is wetted better with an apolar than a polar liquid. Conclusions: The obtained results indicate that the transformation was minimized with the treatments we applied; that is why they are called non-invasive methods. According to the literature data, depending on the parameters of the sandblasting process, the percentage of the monoclinic phase in the treated surfaces ranges from 22% to 52%, which confirms the above-mentioned conclusion.

15. Skylark Wiki: Accessing research data of the MRC National Survey of Health and Development

Abstract

Background & Introduction The Medical Research Council National Survey of Health and Development (NSHD) is the oldest and longest-running of the British birth cohort studies. In 2018 we launched the Skylark Wiki, a central metadata repository and the first port of call for any data sharing queries. In 2023 we utilised the HDRUK funding initiative to work with our advisory group and research community to improve the transparency of our data access process and make the content more accessible. Methods We conducted a focus group session with the NSHD Participant Advisory Panel to understand their views on the data sharing process and whether the information we provided them, sufficiently explained how their data are shared. We also sent out a questionnaire to all Skylark Users to identify areas of improvement. Lastly, we undertook an audit of the website’s content and used Google Analytics to see how Users navigated across the site. Combining all this information we identified areas requiring improvements, from the point of view of our stakeholders, and comparing these to the requirements indicated in the HDRUK Transparency Standards we mapped out updates to the Skylark Wiki. The work was carried out by an external agency (Aubergine262 Ltd). Results To increase transparency and searchability, we improved the flow diagram of the data sharing process to include links to the relevant web pages. We also created guidance to explain why the information in the data request application form is being asked. As feedback from Skylark Users suggested difficulties in finding the required information, the navigation has been updated. We have also changed the layout of the front page to include navigation boxes that takes users straight to the content. The NSHD Advisory Panel reinforced the notion of trust that study participants place in the study and its approach to data sharing. They confirmed the information provided on data sharing was adequate and appreciated that this information was placed on the main study website (https://nshd.mrc.ac.uk/) for their benefit. Initial feedback from Users indicates that it is easier to find information on the website. We will keep monitoring access to the website, the number of queries, and data access requests to evaluate long-term benefits. We have changed the sitemap and added in large link boxes, to make it easier to find the different sections. We hope this will improve the searchability of the site so users can find information they required. We hope this will therefore lead to fewer emails and correspondence between the data sharing team and users. We have also updated the flow diagram of the data sharing process to include links to the web pages which are related to that section. We hope this will help the users find the information they require. Conclusions The new Skylark data-sharing website supports easier navigation providing a better User experience. In return, this should lead to fewer queries from Users. The website will be easier to maintain having changed the content management system from a Wiki to WordPress. WordPress requires less technical knowledge, allowing more staff to contribute to regular maintenance. Abstract

IntroductionEndocrine hypertension (EHT) due to pheochromocytoma/paraganglioma (PPGL), Cushing’s syndrome (CS), or primary aldosteronism (PA) is linked to a variety of metabolic alterations and comorbidities. Accordingly, patients with EHT and primary hypertension (PHT) are characterized by distinct metabolic profiles. However, it remains unclear whether the metabolomic differences relate solely to the disease-defining hormonal parameters. Therefore, our objective was to study the association of disease defining hormonal excess and concomitant adrenal steroids with metabolomic alterations in patients with EHT.MethodsRetrospective European multicenter study of 263 patients (mean age 49 years, 50% females; 58 PHT, 69 PPGL, 37 CS, 99 PA) in whom targeted metabolomic and adrenal steroid profiling was available. The association of 13 adrenal steroids with differences in 79 metabolites between PPGL, CS, PA and PHT was examined after correction for age, sex, BMI, and presence of diabetes mellitus.ResultsAfter adjustment for BMI and diabetes mellitus significant association between adrenal steroids and metabolites – 18 in PPGL, 15 in CS, and 23 in PA – were revealed. In PPGL, the majority of metabolite associations were linked to catecholamine excess, whereas in PA, only one metabolite was associated with aldosterone. In contrast, cortisone (16 metabolites), cortisol (6 metabolites), and DHEA (8 metabolites) had the highest number of associated metabolites in PA. In CS, 18-hydroxycortisol significantly influenced 5 metabolites, cortisol affected 4, and cortisone, 11-deoxycortisol, and DHEA each were linked to 3 metabolites.DiscussionsOur study indicates cortisol, cortisone, and catecholamine excess are significantly associated with metabolomic variances in EHT versus PHT patients. Notably, catecholamine excess is key to PPGL’s metabolomic changes, whereas in PA, other non-defining adrenal steroids mainly account for metabolomic differences. In CS, cortisol, alongside other non-defining adrenal hormones, contributes to these differences, suggesting that metabolic disorders and cardiovascular morbidity in these conditions could also be affected by various adrenal steroids.

Abstract

This study explores the intersection of waste management and sustainable fuel production, focusing on the pyrolysis of plastic waste, specifically polystyrene. We examine the physicochemical parameters of the resulting waste plastic pyrolytic oils (WPPOs), blended with kerosene to form a potential alternative fuel for gas turbines. Our findings reveal that all WPPO blends lead to increased emissions, with NOX rising by an average of 61% and CO by 25%. Increasing the proportion of WPPO also resulted in a higher exhaust gas temperature, with an average rise of 12.2%. However, the thrust-specific fuel consumption (TSFC) decreased by an average of 13.8%, impacting the overall efficiency of waste-derived fuels. This study underscores the need for integrated waste-to-energy systems, bridging the gap between waste management and resource utilization.

Abstract

Poland is currently undergoing substantial transformation in its energy sector, and gaining public support is pivotal for the success of its energy policies. We conducted a study with 338 Polish participants to investigate societal attitudes towards various energy sources, including nuclear energy and renewables. Applying a novel network approach, we identified a multitude of factors influencing energy acceptance. Political ideology is the central factor in shaping public acceptance, however we also found that environmental attitudes, risk perception, safety concerns, and economic variables play substantial roles. Considering the long-term commitment associated with nuclear energy and its role in Poland's energy transformation, our findings provide a foundation for improving energy policy in Poland. Our research underscores the importance of policies that resonate with the diverse values, beliefs, and preferences of the population. While the risk-risk trade-off and technology-focused strategies are effective to a degree, we advocate for a more comprehensive approach. The framing strategy, which tailors messages to distinct societal values, shows particular promise.

Abstract

Purpose To describe the history of the Excellence Centre (EC) programme of the European Society of Hypertension (ESH) since the beginning in 2006, its achievements, and its future developments. Materials and Methods We list the number of ECs per country, the research projects performed so far, and the organisational steps needed to reshape the EC programme for the future. Results In August 2023, the ESH EC programme includes 118 registered ECs in 21 European and 7 non-European countries. Updates about the formal steps for application, re-application, transfer of EC and retirement of EC heads are given. Conclusions The EC programme of the ESH has been a success from the beginning. Further refinements will make it fit for the next decades.

Abstract

This paper presents a comparative analysis of ausferritic ductile cast iron matrix obtained through heat treatment and in its raw state. Ausferrite without heat treatment was achieved by modifying the chemical composition, while nodular graphite was produced using Inmold technology. The presence of compacted graphite in the as-cast ausferritic cast iron was attributed to elements that impede the crystallization of nodular graphite. This study demonstrates that an ausferritic matrix in ductile cast iron can be achieved by incorporating molybdenum in conjunction with nickel or copper. Thermal and derivative analysis (TDA) revealed a minor thermal effect during the transformation of austenite into bainitic ferrite in as-cast ausferritic cast iron. Furthermore, the transformation of austenite in cast iron containing nickel was observed to occur at a temperature of approximately 60 °C higher than in cast iron with copper. The structure of bainitic ferrite platelets in as-cast ausferritic ductile cast iron resembled that of Austempered Ductile Iron (ADI). It was revealed that the amount of austenite in as-cast ausferritic ductile cast iron is more than double that in ADI. The carbon content of austenite was estimated theoretically, revealing that alloying additives in the as-cast ausferritic ductile cast iron reduce the solubility of carbon in austenite, thereby significantly influencing the properties of the cast iron.

The text discusses the issue of long-term discrimination against women in history, especially in the professional sphere. The authors point out that although women played important roles in ancient societies as mothers, priestesses and wives of rulers, they never had access to the highest positions or decision-making roles.The text emphasizes that beliefs play a key role in shaping society and influence our choices. However, good or bad choices are often imposed by society, which operates on the basis of specific cultural patterns.The text distinguishes three types of activities affecting discrimination:The authors note that the dominance of certain beliefs over others leads to long-term discrimination. In the context of professional discrimination against women in Poland, the text points to the repetition of cultural patterns and disregard of women's interests by men, often under the pretext of tradition and a hypocritical interpretation of the Catholic faith.Finally, the authors emphasize that assessing the actions of the government and non-governmental organizations is difficult because it is influenced by powerful corporations and their economic interpretations, which are often placed above ethical and moral issues. Abstract

Summary: Background: Pheochromocytomas and paragangliomas have up to a 20% rate of metastatic disease that cannot be reliably predicted. This study prospectively assessed whether the dopamine metabolite, methoxytyramine, might predict metastatic disease, whether predictions might be improved using machine learning models that incorporate other features, and how machine learning-based predictions compare with predictions made by specialists in the field. Methods: In this machine learning modelling study, we used cross-sectional cohort data from the PMT trial, based in Germany, Poland, and the Netherlands, to prospectively examine the utility of methoxytyramine to predict metastatic disease in 267 patients with pheochromocytoma or paraganglioma and positive biochemical test results at initial screening. Another retrospective dataset of 493 patients with these tumors enrolled under clinical protocols at National Institutes of Health (00-CH-0093) and the Netherlands (PRESCRIPT trial) was used to train and validate machine learning models according to selections of additional features. The best performing machine learning models were then externally validated using data for all patients in the PMT trial. For comparison, 12 specialists provided predictions of metastatic disease using data from the training and external validation datasets. Findings: Prospective predictions indicated that plasma methoxytyramine could identify metastatic disease at sensitivities of 52% and specificities of 85%. The best performing machine learning model was based on an ensemble tree classifier algorithm that used nine features: plasma methoxytyramine, metanephrine, normetanephrine, age, sex, previous history of pheochromocytoma or paraganglioma, location and size of primary tumours, and presence of multifocal disease. Abstract

IntroductionDigital behavioural weight loss interventions have the potential to improve public health; however, these interventions are often not adequately tailored to the needs of the participants. This is the protocol for a trial that aims to determine the effectiveness and cost-effectiveness of the Choosing Health programme as a means to promote weight loss and weight loss maintenance among overweight/obese adults.Methods and analysisThe proposed study is a two-group randomised controlled trial with a nested interrupted time series (ITS) within-person design. Participants (n=285) will be randomly assigned to either the Choosing Health digital intervention or a control group. For intervention participants, ecological momentary assessment will be used to identify behavioural determinants for each individual in order to tailor evidence-based behaviour change techniques and intervention content.Control group participants will receive non-tailored weight loss advice via e-book and generic emails. The primary outcome is the mean difference in weight loss between groups at 6 months controlled for baseline. Secondary outcomes include blood pressure and percentage of body fat; self-reported measures of physical activity, sitting time, quality of life, cost and theory-derived correlates of weight loss. Secondary outcomes will be measured at baseline, 3, 6 and 12 months. The primary outcome for ITS will be daily weight loss plan adherence. Data will be analysed using regression and time series analyses.Ethics and disseminationEthics approval was granted by Faculty of Psychology, SWPS University of Social Sciences and Humanities, Wroclaw, Poland, approval number 03/P/12/2019. Abstract

Background: We aimed to determine the influence of coronavirus disease 2019 (COVID-19) pandemic on blood pressure (BP) control assessed by ambulatory blood pressure monitoring (ABPM). Methods: Office BP and ABPM data from two visits conducted within a 9-15 months interval were collected from patients treated for hypertension. In the prepandemic group, both visits took place before, while in the pandemic group, Visit-1 was done before and Visit-2 during the pandemic period. Results: Of 1811 collected patients 191 were excluded because they did not meet the required ABPM time frames. Thus, the study comprised 704 patients from the pandemic and 916 from the prepandemic group. Groups did not differ in sex, age, duration of hypertension, frequency of first line antihypertensive drug use and mean 24 h BP on Visit-1. The prevalence of sustained uncontrolled hypertension was similar in both groups. On Visit-2 mean 24 h BP, daytime and nighttime systolic BP and diastolic BP were higher in the pandemic compared to the prepandemic group (P < 0.034). The prevalence of sustained uncontrolled hypertension on Visit-2 was higher in the pandemic than in the prepandemic group [0.29 (95% confidence interval (95% CI): 0.26-0.33) vs. 0.25 (95% CI: 0.22-0.28), P < 0.037]. In multivariable adjusted analyses a significant difference in BP visit-to-visit change was observed, with a more profound decline in BP between visits in the prepandemic group. Conclusions: This study using ABPM indicates a negative impact of the COVID-19 pandemic on BP control. It emphasizes the need of developing strategies to maintain BP control during a pandemic such as the one induced by COVID-19.
