To estimate the effect of neighborhood-level modification on the efficacy of the MyPEEPS Mobile intervention on the reduction of condomless anal sex acts among same-sex attracted adolescent men. A series of generalized linear mixed model was used to examine if the effect of the MyPEEPS Mobile intervention on condomless anal sex acts was moderated by neighborhood-level factors using data from the 2019 American Community Survey US Census Bureau. The magnitudes of intervention were significantly smaller at both 6- and 9-month follow-up among adolescents living in neighborhood with high proportions of Hispanic or Latino residents (IRR6M = 1.02, 95 % CI: 1.01, 1.02; IRR9M = 1.03, 95 % CI: 1.01, 1.05) and high proportions of families with income below the poverty level (IRR6M = 1.07, 95 % CI: 1.01, 1.12; IRR9M = 1.05, 95 % CI: 1.01, 1.10), which indicated that living in communities with a higher concentration of residents living under poverty or of Hispanic/and Latino ethnicity significantly modified the effective of program intervention on condomless sex among adolescent MSM. Understanding how neighborhood characteristics modify the effect of HIV prevention interventions may be useful in better targeting delivery and tailoring content of interventions based on neighborhood level characteristics such as the ones identified in this study.

ObjectivesWe examined whether molecular cluster membership was associated with public health identification of HIV transmission potential among named partners in Chicago.DesignHistorical cohort study.MethodsWe matched and analyzed HIV surveillance and partner services data from HIV diagnoses (2012-2016) prior to implementation of cluster detection and response interventions. We constructed molecular clusters using HIV-TRACE at a pairwise genetic distance threshold of 0.5% and identified clusters exhibiting recent and rapid growth according to the Centers for Disease Control and Prevention definition (three new cases diagnosed in past year). Factors associated with identification of partners with HIV transmission potential were examined using multivariable Poisson regression.ResultsThere were 5208 newly diagnosed index clients over this time period. Average age of index clients in clusters was 28; 47% were Black, 29% Latinx/Hispanic, 6% female and 89% MSM. Of the 537 named partners, 191 (35.6%) were linked to index cases in a cluster and of those 16% were either new diagnoses or viremic. There was no statistically significant difference in the probability of identifying partners with HIV transmission potential among index clients in a rapidly growing cluster versus those not in a cluster [adjusted relative risk 1.82, (0.81-4.06)].ConclusionPartner services that were initiated from index clients in a molecular cluster yielded similar new HIV case finding or identification of those with viremia as did interviews with index clients not in clusters. It remains unclear whether these findings are due to temporal disconnects between diagnoses and cluster identification, unobserved cluster members, or challenges with partner services implementation.

A linking procedure establishes a "bridge" between the scores from different patient-reported outcome (PRO) instruments that measure similar constructs. After developing a linking relationship however, it is critical to evaluate whether this relationship can be generalized to different groups. Our study aims to validate a published crosswalk for score conversion between the Brief Symptom Inventory Depression subscale and the Patient-Reported Outcomes Measurement Information System Depression 8a using an independent sample. Data were from a sample of young men who have sex with men (MSM), which differs in terms of participant age, race, and ethnicity from the sample used to develop the existing crosswalk. The validity of the newly derived crosswalk was evaluated in terms of the correlation, mean difference and standard deviation between the observed and the linked scores. The two crosswalks were further compared to evaluate if the difference was within an acceptable range. More than half of the item parameters obtained from the two samples were found to overlap in their confidence intervals. Differences between each pair of scores in the two crosswalks was within three T-score points, well within the range of each crosswalk score's standard error. This study concludes that an existing crosswalk is replicable on a sample that differs from that used for crosswalk development, but future research should continue to examine the generalizability of the linked parameters and evaluate the reproducibility of this crosswalk to other populations.

In 2003, Michigan implemented a reentry service to assist HIV-infected people incarcerated in state prisons in linking to HIV medical care immediately upon their release. We examined whether formerly incarcerated people were linked to care successfully, remained in care, and were in good health 3 years after their date of release. In all, 190 people used the service over the 5 years following its inception. Only a minority of those who were alive and not reincarcerated at the time of the evaluation engaged consistently with medical care. Unsurprisingly given low rates of engagement in care, 3 years after their release only 27% had achieved viral suppression. Concerted efforts to support formerly incarcerated HIV-infected individuals' engagement in care over the long term are urgently needed.

Psychometric data linking of psychological and behavioral questionnaires can facilitate the harmonization of data across HIV and substance use cohorts. Using data from the Collaborating Consortium of Cohorts Producing NIDA Opportunities (C3PNO), we demonstrate how to capitalize on previous linking work with a common linked depression metric across multiple questionnaires. Cohorts were young men who have sex with men (MSM), substance-using MSM, HIV/HCV cocaine users, and HIV-positive patients. We tested for differential item functioning (DIF) by comparing C3PNO cohort data with general population data. We also fit a mixed-effects model for depression, entering HIV-status and recent opioid/heroin use as fixed effects and cohort as a random intercept. Our results suggest a minimal level of DIF between the C3PNO cohorts and general population samples. After linking, descriptive statistics show a wide range of depression score means across cohorts. Our model confirmed an expected positive relationship between substance use and depression, though contrary to expectations, no significant association with HIV status. The study reveals the likely role of cohort differences, associated patient characteristics, study designs, and administration settings.

Background: Youth living with HIV (YLH) are disproportionately impacted by HIV with poor outcomes along the entire HIV care continuum. In a 2020–2022 pilot study, iCARE Nigeria, successfully tested a combination intervention incorporating mobile health technology and peer navigation to: 1) improve testing and linkage to HIV care for young men, especially young men who have sex with men (YMSM) and 2) improve medication adherence and treatment outcomes for YLH. The intervention was scaled up to 5 sites in 3 Nigerian cities. Implementation research was used to understand site perspectives on feasibility, readiness and potential facilitators and barriers soon after scale-up commencement. Methods: An explanatory mixed-methods implementation study was conducted, including quantitative surveys on adoption and reach among peer navigators (PNs) and other study staff (55 testing, 172 treatment), and interviews and focus group discussions with PNs and other study staff in both intervention groups (n = 31). Data were analyzed using descriptive statistics (quantitative) and directed content analysis using the Consolidated Framework for Implementation Research and RE-AIM (qualitative). Results: Early into scale-up, PNs and other study staff in the testing and treatment interventions reported high readiness, adoption, feasibility, and appropriateness. Facilitating factors and strategies across both interventions, included supportive institutional culture, ongoing supportive supervision, provision of a manual and training, relevant PN working experiences, communication methods designed to ensure anonymity of targeted youth (testing) or confidentiality (treatment), and access to cellular data and internet. Facilitators specific to each intervention were also identified including PN knowledge of the MSM community, using multiple social media platforms for outreach (testing) and problem-solving by PN and staff to respond to client needs (treatment). Barriers in both interventions included client financial and transportation challenges, and societal stigma. Intervention-specific barriers included legal limitations for MSM and few YMSM friendly clinics (testing), limited client financial resources and cell-phone access (treatment). Conclusions: Implementers of the initial scale-up of both components of the iCARE Nigeria intervention reported high readiness and adoption, supported by implementation strategies and facilitating factors including intervention design. These results are important for informing future work to scale-out iCARE and similar interventions to new settings. Comprehensive Human Immunodeficiency Virus (HIV) services are seldom found within a single organisation. This motivates organisations to form partnerships to achieve optimal outcomes. However, the complexity of inter-organisational relationships can hinder the process of achieving an effective and coordinated response to the HIV epidemic. Understanding the evolution of these inter-organisational relationships can shed light on ways to improve the delivery of these service provisions. This study examines the organisational networks of the HIV care landscape in the Chicago area across 3 years (2015–17). The results of longitudinal analysis indicate that although most organisations still perceive this landscape to be moderately competitive, they have developed more formalised ties and increased their collaborations across 3 years. Further, with passage of time, this organisational network has become denser with no organisational isolation, indicating connection between all organisations in this landscape. Reducing HIV incidence among adolescents represents an urgent global priority. Structural change approaches to HIV prevention may reduce youth risk by addressing the economic, social, cultural, and political factors that elevate it. We assessed whether achievement of structural changes made by eight Connect-to-Protect (C2P) coalitions were associated with improvements in youth's views of their community over the first 4 years of coalitions' mobilization. We recruited annual cross-sectional samples of targeted youth from each C2P community. We sampled youth in neighborhood venues. We interviewed a total of 2461 youth over 4 years. Males (66 %) and youth of color comprised the majority (52 % Hispanic/Latinos; 41 % African Americans) of those interviewed. By year 4, youth reported greater satisfaction with their community as a youth-supportive setting. They reported their needs were better met by available community resources compared with year 1. However, these findings were moderated by risk population such that those from communities where C2P focused on young men who have sex with men (YMSM) reported no changes over time whereas those from communities focused on other at-risk youth reported significant improvements over time in satisfaction and resource needs being met. Internalized HIV stigma increased over time among those from communities serving other at-risk youth and was unchanged among those from YMSM communities. The very different results we observe over time between communities focused on YMSM versus other at-risk youth may suggest it is unreasonable to assume identical chains of structural causality across youth populations who have such different historical relationships to HIV and who encounter very different kinds of entrenched discrimination within their communities.

This study compared the perceptions of the mission identity and mission-driven campus activities of faculty (n = 305) and staff (n = 584) at a large urban Catholic university. Moreover, it compared employees who were self identified as Catholic (n = 375), Christian (n = 204), other faiths (n = 161), or no religious preference (n = 159). Multivariate analysis (controlling for social desirability responding) indicated a main effect for religious preference. Roman Catholic employees, more than employees from the other three categories, believed that the institution's mission-driven activities, reflecting the characteristics of the patron saint of the university and offering faith-formation opportunities, are important programs and initiatives. Implications suggest that Catholic universities may be settings for Catholic and non-Catholic faculty and staff, enabling support of the institution's mission, varied programs, and identity.
