1. Local alterations of left arcuate fasciculus and transcallosal white matter microstructure in schizophrenia patients with medication-resistant auditory verbal hallucinations: A pilot study

Abstract

Auditory verbal hallucinations (AVH) in schizophrenia (SZ) have been associated with abnormalities of the left arcuate fasciculus and transcallosal white matter projections linking homologous language areas of both hemispheres. While most studies have used a whole-tract approach, here we focused on analyzing local alterations of the above-mentioned pathways in SZ patients suffering medication-resistant AVH.Fractional anisotropy (FA) was estimated along the left arcuate fasciculus and interhemispheric projections of the rostral and caudal corpus callosum. Then, potential associations between white matter tracts and SZ symptoms were explored by correlating local site-by-site FA values and AVH severity estimated via the Auditory Hallucinations Rating Scale (AHRS). Compared to a sample of healthy controls, SZ patients displayed lower FA values in the rostral portion of the left arcuate fasciculus, near the frontal operculum, and in the left and right lateral regions of the rostral portion of the transcallosal pathways. In contrast, SZ patients showed higher FA values than healthy controls in the medial portion of the latter transcallosal pathway and in the midsagittal section of the interhemispheric auditory pathway. Finally, significant correlations were found between local FA values in the left arcuate fasciculus and the severity of the AVH's attentional salience. Contributing to the study of associations between local white matter alterations of language networks and SZ symptoms, our findings highlight local alterations of white matter integrity in these pathways linking language areas in SZ patients with AVH. Abstract

Introduction: Bipolar disorders (BD) are characterized by mood symptoms that can worsen medication side effects. We aimed to study the association between residual mood signs and self‐reported side effects in the euthymic phase of BD. Methods: We assessed residual mood signs using the Montgomery–Asberg Depression Rating scale (MADRS) and Young Mania Rating scale (YMRS) and self‐reported side effects using the Patient‐Rated Inventory of Side Effects (PRISE‐M) for 880 males and 1369 females with BD. We conducted a network analysis to test the associations between 52 items of the three scales for males and females separately. We then identified clusters of nodes that fit the networks well. Results: We report only positive associations between residual mood signs and side effects. An elevated mood (YMRS) in females and increased energy (YMRS) in males were central nodes, strongly influencing the development of additional mood symptoms and side effects. Furthermore, we identified three clusters of nodes in both sexes: (1) a "mood cluster", including most YMRS and MADRS items and the PRISE‐M items evaluating sedation, sleep, and restlessness, (2) a cluster of nonsexual side effects (mostly PRISE‐M items), and (3) a cluster of sexual side effects. In both sexes, we identified bridge nodes that may favor the communication between mood and side effects, namely palpitations (PRISE‐M) and agitation (PRISE‐M). Conclusions: The results justify the particular attention of practitioners to monitor elevated moods or increased energy to try to reduce self‐reported side effects and other residual mood symptoms in the euthymic phase of BD. Our findings suggest that clinicians could consider patient‐reported loss of energy, difficulty in falling asleep, and restlessness as mood symptoms rather than medications' side effects. Abstract

Introduction: Metacognition is the ability to reflect on one's own cognitive processes, monitor and regulate them to enhance mental performance. Social cognition involves the capacity to perceive and respond to social cues from others. The study of metacognition and social cognition is an expanding research field in psychiatry. Both domains are related to neurocognition, symptoms and psychosocial functioning in schizophrenia. Understanding the relationship between social cognition and metacognition may be pivotal for enhancing the treatment of cognitive symptoms in schizophrenia. Methods: We conducted a PRISMA systematic review and meta-analysis on quantitative studies comparing metacognition to social cognitive outcomes in adult outpatients with a schizophrenia spectrum disorder. Reports were retrieved from the Medline, ScienceDirect and PsycINFO databases up to July 13th, 2023. Risk of bias was assessed with the Cochrane tool. Results: Our review included 1,036 participants across 17 reports, with 12 reports included in the meta-analysis. We found a significant positive correlation (r = 0.28, 95% CI: [0.14, 0.41]) between social cognition and metacognition. Subgroup analyses indicated that metacognition was specifically associated with theory of mind, attribution, and emotion processing. Different patterns of correlations were observed according to the assessment of metacognition and its subdimensions. Conclusion: Despite discrepancies among the included studies, no publication bias was detected. The results suggest that metacognition and social cognition are distinct but related constructs. Abstract

Catatonia is a severe and potentially life-threatening neuropsychiatric condition. Electroconvulsive therapy (ECT) is the gold standard second-line intervention for catatonia after benzodiazepine failure. However, the access to ECT can be particularly challenging, especially during periods of increased strain on medical facilities, such as the COVID-19 pandemic. Several case reports have suggested the potential efficacy of transcranial direct current stimulation (tDCS) in addressing catatonia. In our case, we present the successful application of intensive tDCS, delivering five sessions per day, each lasting 20 min, with an intensity of 2 mA. The tDCS montage involved placing the anode on the left dorsolateral prefrontal cortex (DLPFC) and the cathode on the left temporoparietal junction (TPJ). This approach was well-tolerated and resulted in a significant improvement in a 70-year-old patient with catatonia, for whom ECT was deemed necessary. Abstract

Background. Physical pain is a common issue in people with bipolar disorder (BD). It worsens mental health and quality of life, negatively impacts treatment response, and increases the risk of suicide. Lithium, which is prescribed in BD as a mood stabilizer, has shown promising effects on pain. Methods. This naturalistic study included 760 subjects with BD (FACE-BD cohort) divided in two groups: with and without self-reported pain (evaluated with the EQ-5D-5L questionnaire). In this sample, 176 subjects were treated with lithium salts. The objectives of the study were to determine whether patients receiving lithium reported less pain, and whether this effect was associated with the recommended mood-stabilizing blood concentration of lithium. Results. Subjects with lithium intake were less likely to report pain (odds ratio [OR] = 0.59, 95% confidence interval [CI], 0.35-0.95; p = 0.036) after controlling for sociodemographic variables, BD type, lifetime history of psychiatric disorders, suicide attempt, personality traits, current depression and anxiety levels, sleep quality, and psychomotor activity. Subjects taking lithium were even less likely to report pain when lithium concentration in blood was =0.5 mmol/l (OR = 0.45, 95% CI, 0.24-0.79; p = 0.008). Conclusions. This is the first naturalistic study to show lithium's promising effect on pain in subjects suffering from BD after controlling for many confounding variables. This analgesic effect seems independent of BD severity and comorbid conditions. Randomized controlled trials are needed to confirm the analgesic effect of lithium salts and to determine whether lithium decreases pain in other vulnerable populations. [ABSTRACT FROM AUTHOR]

Abstract

International audience; Cerebellum plays a role in the regulation of cognitive processes. Cerebellar alterations could explain cognitive impairments in schizophrenia. We describe the case of a 50 years old patient with schizophrenia whom underwent cerebellar transcranial direct current stimulation (tDCS). In order to study the effect of cerebellar stimulation on cognitive functions, the patient underwent a neuropsychological assessment and an eyeblink conditioning (EBC) protocol. Although the effect of brain stimulation cannot be only assessed in a single-case study, our results suggest that cerebellar stimulation may have an effect on a broad range of cognitive functions typically impaired in patients with schizophrenia, including verbal episodic, short term, and working memory. In addition to neuropsychological tests, we evaluated the cerebellar function by performing EBC before and after tDCS. Our data suggest that tDCS can improve EBC. Further clinical trials are required for better understanding of how cerebellar stimulation can modulate cognitive processes in patients with schizophrenia and healthy controls.

Abstract

International audience; Background: The potential of non-invasive repetitive transcranial magnetic stimulation (rTMS) to improve auditory verbal hallucinations (AVH) in schizophrenia patients has been increasingly explored over the past decade. Despite highly promising results, high inter-individual variability of clinical response and ineffective outcomes in a significant number of patients underscored the need to identify factors associated with the clinical response to rTMS. It should help improve the efficacy of rTMS in patients with medication-resistant AVH, and allow a better understanding of its neural impact. Here, we describe an exploratory study protocol which aims to identify structural and functional brain biomarkers associated with clinical response after an rTMS treatment for medication-resistant AVH in schizophrenia.; Contexte: Le potentiel de la stimulation magnétique transcrânienne répétitive non invasive (SMTr) pour améliorer les hallucinations verbales auditives (AVH) chez les patients schizophrènes a été de plus en plus exploré au cours de la dernière décennie. Malgré des résultats très prometteurs, la forte variabilité interindividuelle de la réponse clinique et les résultats inefficaces chez un nombre important de patients ont souligné la nécessité d'identifier les facteurs associés à la réponse clinique à la SMTr. Il devrait aider à améliorer l'efficacité de la SMTr chez les patients atteints d'AVH résistant aux médicaments et permettre une meilleure compréhension de son impact neuronal. Ici, nous décrivons un protocole d'étude exploratoire qui vise à identifier les biomarqueurs cérébraux structurels et fonctionnels associés à la réponse clinique après un traitement rTMS pour AVH résistant aux médicaments dans la schizophrénie.
