Background: Extracorporeal membrane oxygenation (ECMO) has been widely used in patients with COVID-19, but uncertainty remains about the determinants of in-hospital mortality and data on post-discharge outcomes are scarce. The aims of this study were to investigate the variables associated with in-hospital outcomes in patients who received ECMO during the first wave of COVID-19 and to describe the status of patients 6 months after ECMO initiation. Methods: EuroECMO-COVID is a prospective, multicentre, observational study developed by the European Extracorporeal Life Support Organization. This study was based on data from patients aged 16 years or older who received ECMO support for refractory COVID-19 during the first wave of the pandemic—from March 1 to Sept 13, 2020—at 133 centres in 21 countries. In-hospital mortality and mortality 6 months after ECMO initiation were the primary outcomes. Mixed-Cox proportional hazards models were used to investigate associations between patient and management-related variables (eg, patient demographics, comorbidities, pre-ECMO status, and ECMO characteristics and complications) and in-hospital deaths. Survival status at 6 months was established through patient contact or institutional charts review. This study is registered with ClinicalTrials.gov, NCT04366921, and is ongoing. Findings: Between March 1 and Sept 13, 2020, 1215 patients (942 [78%] men and 267 [22%] women; median age 53 years [IQR 46–60]) were included in the study. Median ECMO duration was 15 days (IQR 8–27). 602 (50%) of 1215 patients died in hospital, and 852 (74%) patients had at least one complication. Multiorgan failure was the leading cause of death (192 [36%] of 528 patients who died with available data). In mixed-Cox analyses, age of 60 years or older, use of inotropes and vasopressors before ECMO initiation, chronic renal failure, and time from intubation to ECMO initiation of 4 days or more were associated with higher in-hospital mortality. 613 patients

At 6 months, respiratory rehabilitation was required in 88 (17%) of 522 patients with available data, and the most common residual symptoms included dyspnoea (185 [35%] of 523 patients) and cardiac (52 [10%] of 514 patients) or neurocognitive (66 [13%] of 512 patients) symptoms.INTERPRETATION: Patient's age, timing of cannulation (FUNDING: None.

- Abstract
Background:High-quality evidence for post-cardiotomy extracorporeal life support (PC-ECLS) management is lacking. This study investigated real-world PC-ECLS clinical practices. Methods: This cross-sectional, multi-institutional, international pilot survey explored center organization, anticoagulation management, left ventricular unloading, distal limb perfusion, PC-ECLS monitoring, and transfusion practices. Twenty-nine questions were distributed among 34 hospitals participating in the Post-cardiotomy Extra-Corporeal Life Support Study. Results: Of the 32 centers [16 low-volume (50%); 16 high-volume (50%)] that responded, 16 (50%) had dedicated ECLS specialists. Twenty-six centers (81.3%) reported using additional mechanical circulatory supports. Anticoagulation practices were highly heterogeneous: 24 hospitals (75%) reported using patients bleeding status as a guide, without a specific threshold in 54.2% of cases. Transfusion targets ranged from 7 to 10 g/dL. Most centers used cardiac venting on a case-by-case basis (78.1%) and regular distal limb perfusion (84.4%). Nineteen (54.9%) centers reported dedicated monitoring protocols, including daily echocardiography (87.5%), Swan-Ganz catheterization (40.6%), cerebral near-infrared spectroscopy (53.1%), and multimodal assessment of limb ischemia. Inspection of the circuit (71.9%), oxygenator pressure drop (68.8%), plasma free hemoglobin (75%), d-dimer (59.4%), lactate dehydrogenase (56.3%), and fibrinogen (46.9%) are used to diagnose hemolysis and thrombosis. Conclusions: This study shows remarkable heterogeneity in clinical practices for PC-ECLS management. More standardized protocols and better implementation of the available evidence are recommended.

- Abstract
Introduction: Patients requiring postcardiotomy veno-arterial extracorporeal membrane oxygenation (V-A-ECMO) have a high risk of early mortality. In this analysis, we evaluated whether any interinstitutional difference exists in the results of postcardiotomy V-A-ECMO.Methods: Studies on postcardiotomy V-A-ECMO were identified through a systematic review for individual patient data (IPD) meta-analysis. Analysis of interinstitutional results was performed using direct standardization, estimation of observed/expected in-hospital mortality ratio and propensity score matching.Results: Systematic review of the literature yielded 31 studies. Data from 10 studies on 1269 patients treated at 25 hospitals were available for the present analysis. In-hospital mortality was 66.7%. The relative risk of in-hospital mortality was significantly higher in six hospitals. Observed versus expected in-hospital mortality ratio showed that four hospitals were outliers with significantly increased mortality rates, and one hospital had significantly lower in-hospital mortality rate. Participating hospitals were classified as underperforming and overperforming hospitals if their observed/expected in-hospital mortality was higher or lower than 1.0, respectively. Among 395 propensity score matched pairs, the overperforming hospitals had significantly lower in-hospital mortality (60.3% vs 71.4%, p= 0.001) than underperforming hospitals. Low annual volume of postcardiotomy V-A-ECMO tended to be predictive of poor outcome only when adjusted for patients’ risk profile.Conclusions: In-hospital mortality after postcardiotomy V-A-ECMO differed significantly between participating hospitals. These findings suggest that in many centers there is room for improvement of the results of postcardiotomy V-A-ECMO.

Background: High‐quality evidence for post‐cardiotomy extracorporeal life support (PC‐ECLS) management is lacking. This study investigated real‐world PC‐ECLS clinical practices. Methods: This cross‐sectional, multi‐institutional, international pilot survey explored center organization, anticoagulation management, left ventricular unloading, distal limb perfusion, PC‐ECLS monitoring, and transfusion practices. Twenty‐nine questions were distributed among 34 hospitals participating in the Post‐cardiotomy Extra‐Corporeal Life Support Study. Results: Of the 32 centers [16 low‐volume (50%); 16 high‐volume (50%)] that responded, 16 (50%) had dedicated ECLS specialists. Twenty‐six centers (81.3%) reported using additional mechanical circulatory supports. Anticoagulation practices were highly heterogeneous: 24 hospitals (75%) reported using patients bleeding status as a guide, without a specific threshold in 54.2% of cases. Transfusion targets ranged from 7 to 10 g/dL. Most centers used cardiac venting on a case‐by‐case basis (78.1%) and regular distal limb perfusion (84.4%). Nineteen (54.9%) centers reported dedicated monitoring protocols, including daily echocardiography (87.5%), Swan‐Ganz catheterization (40.6%), cerebral near‐infrared spectroscopy (53.1%), and multimodal assessment of limb ischemia. Inspection of the circuit (71.9%), oxygenator pressure drop (68.8%), plasma free hemoglobin (75%), d‐dimer (59.4%), lactate dehydrogenase (56.3%), and fibrinogen (46.9%) are used to diagnose hemolysis and thrombosis. Conclusions: This study shows remarkable heterogeneity in clinical practices for PC‐ECLS management. More standardized protocols and better implementation of the available evidence are recommended. Background: The age limit for the use of extracorporeal membrane oxygenation (ECMO) support for post-cardiotomy cardiac failure is not defined. The aim of the study was to evaluate the outcomes of octogenarians supported with ECMO due to cardiogenic shock. Methods: A retrospective review of consecutive elderly patients supported with ECMO during a 13-year period in a tertiary care center. Patient’s demographic variables, comorbidities, perioperative data and outcomes were collected from patient medical records. Data of octogenarian patients were compared with the septuagenarian group. The main outcomes of the study was in hospital mortality, 6-month survival and 1-year survival after hospital discharge and discharge options. Multivariate logistic regression analysis was performed to identify the factors associated with hospital survival. Results: Eleven patients (18.3%) in the elderly group were octogenarians (aged 80 years or above), and forty-nine (81.7%) were septuagenarians (aged 70–79 years). There were no differences except age in demographic and preoperative variables between groups. Pre ECMO SAVE, SOFA, SAPS—II and inotropic scores were significantly higher in septuagenarians than octogenarians. There was no statistically significant difference in hospital mortality, 6-month survival, 1 year survival or discharge options between groups. Conclusions: ECMO could be successfully used in selected octogenarian patients undergoing cardiac surgery to support a failing heart. An early decision to initiate ECMO therapy in elderly post-cardiotomy shock patients is associated with favorable outcomes.

Background: Recently extracorporeal membrane oxygenation is becoming the commonly used mechanical assist device for the treatment of severe cardiogenic shock in postcardiotomy patients. Evaluation of risk factors of negative outcome would be beneficial in decision-making in the elderly patient population. Methods: This was a retrospective single-centre analysis of elderly patients who underwent extracorporeal membrane oxygenation treatment for refractory cardiogenic shock in a tertiary care centre. Demographic data, comorbidities and perioperative parameters were collected to evaluate their impact on the outcome of extracorporeal membrane oxygenation treatment in this patient group. Logistic regression analysis of the variables was performed to identify factors predicting an adverse outcome. Results: Forty consecutive elderly patients underwent extracorporeal membrane oxygenation treatment during the study period. The mean age was 76.7 ± 3.8 years, 27 (68%) were male. The mean Survival after Veno–Arterial extracorporeal membrane oxygenation score before initiating extracorporeal membrane oxygenation support was − 11 ± 6. Intra-aortic counterpulsation was used as the first-line mechanical support in 31 (77%) patients. The mean duration of extracorporeal membrane oxygenation support was 172 ± 128 hours. Twenty-four patients (56%) were successfully weaned from extracorporeal membrane oxygenation, and 8 (20%) survived to hospital discharge. - Abstract
Background. The efficacy of antidepressant treatment is fair, but the efficacy is considerably lower in patients failing two or more trials underscoring the need for new treatment options. Our study evaluated the augmenting antidepressant effect of 8-weeks transcranial pulsed electromagnetic field (T-PEMF) therapy in patients with treatment-resistant depression. Methods. A multicenter 8-week single-arm cohort study conducted by the Danish University Antidepressant Group. Results. In total, 58 participants (20 men and 38 women) with a moderate to severe depression as part of a depressive disorder according to ICD-10 who fulfilled criteria for treatment resistance were included, with 19 participants being nonresponders to electroconvulsive therapy during the current depressive episode. Fifty-two participants completed the study period. Scores on the Hamilton Depression Scale 17-items version (HAM-D17) decreased significantly from baseline (mean = 20.6, SD 4.0) to endpoint (mean = 12.6, SD 7.1; N = 58). At endpoint, utilizing a Last Observation Carried Forward analysis, 49 and 28% of those participants with, respectively, a nonchronic current episode (≤2 years; N = 33) and a chronic current episode (>2 years; N = 25) were responders, that is, achieved a reduction of 50% or more on the HAM-D17 scale. At endpoint, respectively, 30 and 16% obtained remission, defined as HAM-D17 ≤ 7. On the Hamilton Scale 6-item version (HAM-D6), respectively, 51 and 16% obtained remission, defined as HAM-D6 ≤ 4. Conclusions. The findings indicate a potential beneficial role of T-PEMF therapy as an augmentation treatment to ongoing pharmacotherapy in treatment-resistant depression.

This paper reports on a qualitative research undertaken with Lithuanian secondary school and gymnasium music teachers. The target of the research is to reveal what teacher's competences are necessary in the process of fostering pupils' musical creativity through MCT. The research data were collected through in depth interview with 10 music teachers. Research findings show that one of the essential components of competences for fostering pupils' musical creativity are subject (music) and didactic competences of music teachers. The creative music teacher is able to organize the developmental process in such a way that each pupil would acquire knowledge and skills necessary for musical creativity, would be capable of creating an original composition, and experiencing joy of discovery and creation. To ascertain the opinion of psychiatrists of the factors those determine antidepressants selection.An original questionnaire of 30 questions, which deals with reliance of antidepressants selection according to the subtype of depression, was represented for a quarter of all Lithuanian psychiatrists.Respondents for depression with obsession 36% chose paroxetine. It is interesting that despite the controversial opinion about the TCAprescribing according to their side effects profile and safety to use, our respondent chose amitriptyline for the melancholic depression with suicidal thoughts (50.2%) and for the anesthetic depression (28%). In some cases there is no unanimous opinion among the psychiatrists--data scattering was received in selection, the respondents chose different antidepressants from different groups in similar frequency. For the treatment of the adynamic depression--7.6%--amitriptyline, 12.1%--citalopram, 10.6%--reboxetine, 10.6%--venlafaxine, for the anxious depression--15.2%--amitriptyline, about 20%--citalopram, 15.2%--mirtazapin, for the anesthetic depression 14.3%--escitalopram, 9%--sertraline, 8.3%--venlafaxine. There is no clear tendency or prevailing antidepressant.Psychopathological peculiarity of depression can be one of the most important criteria in antidepressant selection. However, in many cases, the subtype of depression is ascertained empirically and based solely on the personal experience and clinical practice of the psychiatrist. There are no clear diagnostic criteria or practical guidelines for the reliable verification of the psychopathological subtype of depression, which would allow for the selection of a more adequate and prompt treatment for the patient.

Objective: To study the differences in levels of alexithymia, depression, and anxiety between a sample of adolescents diagnosed with ICD-10 persistent somatoform pain disorder (defined by the DSM-IV as a pain disorder associated with psychological factors) and healthy adolescent control subjects. Method: Using the Toronto Alexithymia Scale and the Hospital Anxiety and Depression Scale, we investigated the point prevalence of alexithymia, anxiety, and depression among adolescents aged 12 to 17 years, with somatoform disorder, who were hospitalized in Kaunas Medical University Hospital, Lithuania ( n =120), and a healthy control group ( n = 60) of adolescents aged 12 to 17 years, who were randomly selected from 6 schools in Kaunas, Lithuania. Results: The rate of alexithymia in adolescents with somatoform disorder was 59%, which was significantly higher than that in healthy control subjects (1%, P < 0.001). Similarly, the rate of anxiety was significantly higher in the patient group (62%), compared with control subjects (15%, P < 0.001). The rate of depression was low in both groups and did not differ significantly between groups. Conclusions: Adolescents with somatoform disorder have higher levels of alexithymia and anxiety than healthy adolescent control subjects, but adolescents with somatoform disorder and adolescent control subjects do not have significantly different levels of depression.

The aim of this study--to investigate the effect of Levothyroxine on thyroid nodules avoiding the growth of nodules. We have compared results with studies, had evaluated this treatment method and discuss about guidelines of endocrinologist's-practitioner's consensus on this subject.Sixty-two patients (all female) were evaluated. The mean age of the group was 47.89 +/- 13.73 year. Plasma thyroid-stimulating hormone (TSH) concentration was measured in all patients before therapy initiation and after 6 months, some of them were observed during 24 months (20 (32.3%)) and were examined every 6 months. Nodules evaluation was made by ultrasound. Therapy with Levothyroxine was prescribed for 37 (59.68%) patients. The data was compared with 25 (40.32%) cases of control group not treated with Levothyroxine.There was reached mild TSH suppression in 11 (29.7%) cases, moderate--in 4 (10.8%), strong in 2 (5.4%) prescribing 54.39 +/- 26.71 micrograms of Levothyroxine. Thirteen women's TSH significantly decreased, while was1 mIU/L. Seven (18.9%) patients failed suppression.The treatment with little-moderate doses decreased TSH significantly and TSH suppression mild to strong was achieved in a half of treated investigative; furthermore, the size of thyroid nodules have changed statistically significant after 6 months of therapy and positive effect was reached for 89.2% of treated patients.

Study object:To evaluate the opinion of nurses and nursing students about patients belonging to sexual minorities.Methods:The study was performed using of anonymous questionnaire at the Hospital and Nursing department of Kaunas University of Medicine. 32 general practice nurses from departments of Ophthalmology, Nephrology, Thoracic Surgery and 22 nursing students took part in the survey.Results:It was found, that the attitude of nurses towards sexual minority was more negative (55 percent). The attitude of students was more neutral (53 percent), however there was no significant statistical difference (p=0,32). Evaluating the influence of age to the attitude, we determined that all nurses with negative attitude towards sexual minorities were more than 40 years old. When analyzing the influence of work experience to the attitude, it was found, that most of nurses (37 percent) showing negative attitude had work experience more than 15 years. Most nurses (37 percent) and nursing students (50 percent) were thinking that homosexuality is the result of nurture. When determining the influence of education level to the attitude towards sexual minorities, it was found, that nurses with negative attitude, had college and university education and most of students with negative attitude (92 percent) had secondary education.

Mental and neurological disorders due to cerebrovascular changes are quite frequent, but are very rarely analyzed in connection with morphological changes in blood vessels.Objective:To analyze correlation between clinical symptoms and histological changes of human a.basilaris.Methods:We retrospectively studied relation between histological changes of a.basilaris and psychoneurological state in 10 cases of 40-70 years old patients, who died by accident.Results:Analyzing retrospectively case histories we found such symptoms in psychical neurological state: basilar syndrome, emotional lability, rapid changes of mood, asthenia. 6 patients were diagnosed basilar syndrome by neurologists, 4 patients were diagnosed symptoms of F 06 group (according to ICD-10). Histological changes in a.basilaris were reduced amount of elastic fibre in blood-vessel media, increased quantity of collagen fibre and widening of intima.Conclusions:Most frequent symptoms correlating with a.basilaris structural changes were basilar syndrome and organic mood, anxiety disorders.

Objective:because of the increasing rates of suicides, especially among young people, suicidal behavior becomes more and more actual problem in many countries. Lithuania is known as a country where suicides rate is the highest in Europe.The goal is to find out and compare gender differences in teenagers suicidal behavior relations with depression, personal peculiarities.Methods:two groups of teenagers from 14 to 17 were researched: cases (who tried to commit a suicide, N=109) and control group (without suicidal anamnesis, N=218). Aiming to establish teenagers' diagnoses in cases group, clinical psychiatric research was performed, in accordance with ICD- 10 classification of mental and behavioural disorders diagnostic reference requirements, diagnoses distribution was compared in relation to sex.
