A case of a 73-year-old patient with critical aortic stenosis, porcelain aorta and occluded femoral arteries is presented. We performed apico-aortic valved conduit (A-AVC) without cardiopulmonary bypass (CPB). Hegar dilator inserted through the apex into the left ventricle allowed anastomosis of dacron tube to the apex. Valved conduit was anastomosed to the descending aorta. Both tubes were connected. Before the operation, maximal gradient through the valve was 95, after operation dropped to 33 mmHg. This method of apical anastomosis allowed to perform A-AVC without CPB in a patient with extremely high peri-operative risk while using CPB.

At present, according to the increase of average age in modern societies, the patients who are over 70 years old and require coronary arteries reconstruction constitute a constantly growing group. In the Cardiosurgical Clinic of the Medical University of Lublin 30 coronary arteries bypass grafting operations in over 70-year-old patients were performed in 2000. The operations were performed using extra-body circulation. In the early postoperation period 3 patients died (10%), 2 patients (6%) required applying mechanical support of the left ventricle (IABP). 2 patients (6%) needed reoperation because of excessive drainage in the immediate postoperational period. CABG in patients over 70 years of age are connected with higher "perioperational" risk, prolonged recovering period and higher mortality. According to the results of the survey, the overwhelming majority of patients at that age gets improvement in their health and in the quality of life.

The paper discusses the problem of treating Asherman's syndrome, which due to common procedures in gynaecology and obstetrics becomes more and more frequent occurrence in gynaecological practice. The genesis of this syndrome is often a result of a too intensive excochleation of the uterus, accompanying infection and lack of endometrial reaction to oestrogens. The authors discussed 14 cases of patients in whom, after excochleation of the uterus, there appeared intrauterine adhesions followed by amenorrhoea. The therapy consisted in removing the adhesions by means of a hysteroscope and inserting an intrauterine device the shape of which was selected depending on the character and localization of adhesions and applying oestrogens locally into the uterus cavity. In order to have the menstrual cycle reconstructed, the patients were administered orally hormones during 4 successive artificially reconstructed cycles. Among the patients undergoing therapy, 86% of them began to menstruate again and 57% of them got pregnant. The use of hysteroscopy reduced maximally the risk of breaking the continuity of the uterus during the resection of large adhesions. Inserting a proper device prevented the recurrence of adhesions and local administering oestrogens into the uterine cavity provided good conditions for the reconstruction of the endometrium.

Diazepam (0.4-0.8 mg/kg i.p.) reduced the spontaneous activity in the electromyogram (EMG) recorded from the gastrocnemius-soleus muscle of spastic mutant Han--Wistar rats in a dose-dependent manner. The depressant effect of diazepam (0.8 mg/kg) on the EMG activity was antagonized by Ro 15-1788 (5 mg/kg i.p.) and by picrotoxin (2 mg/kg i.p.), but not by bicuculline (2 mg/kg i.p.). These results suggest that diazepam depresses EMG activity of mutant rats by mechanisms related to the interaction of the drug with GABA-independent benzodiazepine receptors.

A rare case is reported of coexistence of intrauterine and ectopic pregnancy with delivery of a healthy newborn. Diagnostic difficulties are discussed and the management of such multiple pregnancy is described.
