Background: Janus kinase (JAK) inhibition is a new mode of action in atopic dermatitis (AD); clarity about drug class safety considerations in the context of AD is important. Baricitinib, an oral, reversible, selective inhibitor of JAK1/JAK2, is in late‐stage development for adult patients with moderate‐to‐severe AD. Objective: To report pooled safety data for baricitinib in patients with moderate‐to‐severe AD in the clinical development program including long‐term extension (LTE) studies. Methods: This analysis included patient‐level safety data from six double‐blinded, randomized, placebo‐controlled studies (one phase 2 and five phase 3), one double‐blinded, randomized, LTE study and one open‐label LTE study, reported in three data sets: placebo‐controlled, 2‐mg – 4‐mg extended and All‐bari AD. Safety outcomes include treatment‐emergent adverse events, adverse events of special interest and abnormal laboratory changes. Proportions of patients with events and incidence rates were calculated. Results: Data were collected for 2531 patients who were given baricitinib for 2247 patient‐years (median duration 310 days). The frequency of serious infections, opportunistic infections and conjunctival disorders was low and similar between treatment groups in the placebo‐controlled period. The most common serious infections were eczema herpeticum [n = 11, incidence rates (IR) = 0.5], cellulitis (n = 6, IR = 0.3) and pneumonia (n = 3, IR = 0.1). There were four opportunistic infections (IR = 0.2). No malignancies, gastrointestinal perforations, positively adjudicated cardiovascular events or tuberculosis were reported in the placebo‐controlled period in baricitinib‐treated patients. Frequency of herpes simplex was higher in the 4‐mg group (6.1%) vs. the 2‐mg (3.6%) and placebo group (2.7%); IRs in the extended data set (2‐mg IR = 9.6; 4‐mg IR = 14.5) were lower vs. the placebo‐controlled data set (2‐mg IR = 12.4; 4‐mg IR = 21.3). In the All‐bari AD data set, there were two positively adjudicated major adverse cardiovascular events (2‐mg group): two venous thrombosis events (4‐mg group) and one death. Conclusion: This integrated safety analysis in patients with moderate‐to‐severe AD confirms the established safety profile of baricitinib. [ABSTRACT FROM AUTHOR]

Education - Abstract
Abstract: The complexity of the challenges for nursing education makes the nursing study program think hard to produce various innovations in curriculum implementation during the COVID-19 pandemic. This study aims to explore the strategy of the nursing undergraduate study program in implementing a nursing curriculum that is oriented to stakeholder demands during the COVID-19 pandemic. This study uses a qualitative approach, using interpretive phenomenological methods. Purposive and snowball sampling was used in this study to help the researchers identify the key informant, with the criteria: ten nursing lecturers who are active in giving lectures in the COVID-19 pandemic, twenty stakeholders (among them: students, nursing clinical instructors, clinical professional nurses, users of nursing graduates) and willing to be an informant. Using semi-structured interview techniques, the data obtained is then analyzed using the interactive analysis model of Miles, Huberman & Saldana. The results found five themes, including (1) Increase collaboration with instructors from the clinic; (2) Strengthening the skills of teachers with the blended learning method; (3) Streamline the learning management system; (4) Develop community-based nursing learning during pandemic; (5) Open-effective communication with stakeholders. The effectiveness of the curriculum implementation strategy during the COVID-19 pandemic requires pro-activity from the entire academic community of the nursing study program, to produce quality nursing graduates and meet the high demands of stakeholders during the multi-crisis of the COVID-19 pandemic. The success of the nursing education curriculum system during the COVID-19 pandemic is strongly influenced by the active and positive contributions and collaborations of nursing stakeholders. Keywords: COVID-19 pandemic; curriculum strategies; nursing education; resilience; stakeholders challenges.

BACKGROUND: Few data are available regarding the use of antithrombotic strategies in coronary artery disease patients with atrial fibrillation (AF) in everyday practice. We sought to describe the prevalence of AF and its antithrombotic management in a contemporary population of patients with stable coronary artery disease.METHODS AND FINDINGS: CLARIFY is an international, prospective, longitudinal registry of outpatients with stable coronary artery disease, defined as prior (≥12 months) myocardial infarction, revascularization procedure, coronary stenosis >50%, or chest pain associated with evidence of myocardial ischemia. Overall, 33,428 patients were screened, of whom 32,954 had data available for analysis at baseline; of these 2,229 (6.7%) had a history of AF. Median (interquartile range) CHA2DS2-VASc score was 4 (3, 5). Oral anticoagulation alone was used in 25.7%, antiplatelet therapy alone in 52.8% (single 41.8%, dual 11.0%), and both in 21.5%. OAC use was independently associated with permanent AF (pCONCLUSIONS: In this contemporary cohort of patients with stable coronary artery disease and AF, most of whom are theoretical candidates for anticoagulation, oral anticoagulants were used in only 47.2%. Half of the patients received antiplatelet therapy alone and one-fifth received both antiplatelets and oral anticoagulants. Efforts are needed to improve adherence to guidelines in these patients.TRIAL REGISTRATION: ISRCTN registry of clinical trials: ISRCTN43070564.

Patient safety culture is essential in creating a safe and healthy hospital environment. This study aims to portray nurses' perceptions of patient safety culture during the Pandemic in COVID-19 Referral Hospitals. This paper was a descriptive study. The population was nurses working in inpatient, Covid-19 isolation, intensive, and emergency rooms. In addition, the sample was 268 nurses with a stratified random sampling. Data collection used a questionnaire using Google Forms. It consisted of the demographic characteristics of respondents (name, gender, age, marital status, working time, education, nurse position, levels of nursing, and patient safety training) also nurses' perception of patient safety culture. The Hospital Survey on Patient Safety Culture (HSOPC) was an instrument to assess patient safety culture. Data analysis used IBM SPSS Statistics version 22.0 to describe the mean, median, minimum, and maximum values and presentation. Results showed that the mean patient safety culture among nurses in COVID-19 referral hospitals was 147.09 (62.55%). In addition, the lowest dimension of patient safety culture was feedback and communication (31.75%), while the highest was an organizational improvement (73.16%). Thus, the patient safety culture in the COVID-19 referral hospital still needed improvement. A good patient safety culture can increase patient safety and the quality of health services. Therefore, hospital management should optimize all dimensions of patient safety culture to ensure patient safety. Future research could explore predisposing factors of patient safety culture

MEMS ,LTCC ,SIP ,RF MEMS ,system in package - Abstract
RF-PLATFORM is combining a wide field of R&D activities with a consolidated service action. The partners in the consortium investigate different technologies, develop applications and perform multi project wafer production runs consisting of numerous structures in each technology. The network is open for interested parties, who can join a user club and participate in the MPW’s. Ceramic multilayers are known as very universal PCB's and packages with advanced properties, 3-dimensional functionalities, good RF performance or integration of passives. One of the main technological challenges in RF-Platform is the development of systems-in-package for a couple of applications in combination with RF-MEMS switches. Since LTCC as a highly reliable ceramic interconnection technology enables an increased circuit density and an advanced versatility of functions, it acts as the carrier for the electrical wiring and packaging, contains buried components and structures, waveguides and antenna structures and is last but not least the package for the system with the opportunity of hermetic encapsulation. Partner EADS has developed an 35 GHz LTCC patch antenna module for communication links, steerable by RF-MEMS switches being integrated directly in the system. Additional applications are a 24 GHz sensing system and a radar sensor module in the 79 GHz range. The paper will give an overview about investigations, simulations and measurements performed with the test vehicles and intermediate structures being manufactured in different MPW runs.

Background: Out-of-hospital cardiac arrest can occur anywhere, including in rural areas. Nurses are demanded to have skills for critical care to save lives. Objective: This study aimed to explore lived experience of nurses in caring for persons with out-of-hospital cardiac arrest in the rural areas of East Java, Indonesia. Methods: This study employed a phenomenological research design. Five nurses were selected from five village health posts using a purposive sampling. A semi-structure interview was used to collect data. Colaizzi’s method was used for data analysis. Results: Four themes emerged, including (1) being fast and responsive, (2) needing a family trust, (3) feeling worried, and (4) lacking personnel and infrastructure. Conclusion: The role of nurses in saving lives of persons with out-of-hospital cardiac arrest is very important. It is suggested for the nurses to always improve the quality of knowledge management of persons with critical conditions, provide understanding of basic life support in community, increase the trust of family in nurses, and improve the collaboration between nurses and community. The government also need to provide better emergency equipment and add health personnel in community.

50. Influence of deposition and etching on high aspect ratio patterning

- Abstract
The tailoring of a dry etching process in order to fulfill all requirements in a high yield, low cost production becomes increasingly ambitious. Stringent specifications on plasma induced damage, etching selectivity, linewidth control, and etching uniformity necessitates time consuming and expensive plasma process development. Moreover, uniform etching suffers from unexpected problems depending on aspect ratio and pattern density. The effect of decreasing etchrate with increasing aspect ratio, commonly labeled as microloading, is detected in a wide variety of materials and etching conditions. This includes halogen- as well as oxygen- and hydrogen-based chemistry in low and high pressure plasmas. In different reactor types ranging from RIE diodes, high density plasma sources, magnetically enhanced triodes, and microwave driven systems microloading is observed. A number of approaches explaining microloading consider ion angular distributions, ion and neutral shadowing, charging of micros tructures, and Knudsen transport of neutrals. However, predicting profiles in a specific plasma process requires additional investigations for the understanding of microloading. This contribution focusses on aspect ratio dependent etch rates and profiles in SiO2 as a balance of etching and deposition. Patterning of contact holes down to 0.2mu m with aspect ratios of more than 10:1, the influence of polymer deposition and etching is studied. TEM analysis of patterns gives an insight into process mechanisms identifying polymer deposition in low and high aspect ratio features.
