In the design of long-term care systems, preferences can serve as an essential indication to better tailor services to the needs, wishes and expectations of its consumers. The aim of this systematic review was to summarize and synthesize available evidence on long-term care preferences that have been elicited by quantitative stated-preference methods. The databases PubMed and Web of Science were searched for the period 2000 to 2020 with an extensive set of search terms. Two independent researchers judged the eligibility of studies. The final number of included studies was 66, conducted in 19 different countries. Studies were systematized according to their content focus as well as the survey method used. Irrespective of the heterogeneity of studies with respect to research focus, study population, sample size and study design, some consistent findings emerged. When presented with a set of long-term care options, the majority of study participants preferred to “age in place” and make use of informal or home-based care. With increasing severity of physical and cognitive impairments, preferences shifted toward the exclusive use of formal care. Next to the severity of care needs, the influence on preferences of a range of other independent variables such as income, family status and education were tested; however, none showed consistent effects across all studies. The inclusion of choice-based elicitation techniques provides an impression of how studies operationalized long-term care and measured preferences. Future research should investigate how preferences might change over time and generations as well as people’s willingness and realistic capabilities of providing care.

Zusammenfassung Hintergrund Um eine spezialisierte Versorgung von Menschen mit seltenen Erkrankungen (sE) zu gewährleisten, wurden zahlreiche Zentren für Seltene Erkrankungen (ZSE) gegründet. Für die ambulante Behandlung von Betroffenen in Krankenhäusern steht dabei eine Vielzahl von Versorgungs- und Vergütungsformen zur Verfügung. Studien zu einzelnen sE ergaben bereits Hinweise auf Defizite in Bezug auf eine kostendeckende Vergütung der ZSE. Ziel der Arbeit Untersuchung der aktuellen Versorgungs- und Vergütungsstrukturen in den ZSE und die Entwicklung von Ansätzen für zukünftige nachhaltige Vergütungsstrukturen. Material und Methoden Mittels Fragebogenerhebung wurden zunächst ZSE in Deutschland zu ihrer Versorgungs- und Vergütungsform befragt. Im Rahmen zweier Fokusgruppen- und eines Experteninterviews mit Vertreter:innen der ZSE, der Kostenträger, der Gesundheitspolitik sowie Patient:innen wurden im Anschluss die aktuellen Versorgungs- und Vergütungsformen, Möglichkeiten der zukünftigen Gestaltung der Versorgung von Menschen mit sE sowie Ansätze für eine leistungsorientierte Vergütung diskutiert. Das Material wurde inhaltsanalytisch nach Kuckartz ausgewertet. Ergebnisse und Diskussion 39 Zentren beteiligten sich an der Fragebogenerhebung. 38 % dieser Zentren werden über eine Pauschale für Hochschulambulanzen (HSA) vergütet, deren Höhe stark variiert. 41 % weisen eine Mischvergütung aus HSA-Pauschale und weiteren Vergütungsformen auf. In den Interviews wurde eine Unterdeckung der Kosten in den ZSE mit Auswirkungen auf die Patientenversorgung benannt und zur Sicherstellung einer nachhaltigen Versorgung Handlungsbedarf zur Weiterentwicklung der Vergütungsstrukturen festgestellt. Eine „Sonderpauschale für sE“, die den besonderen zeitlichen Bedarf in der Versorgung von Menschen mit sE abbildet, wurde als möglicher nachhaltiger Vergütungsansatz präferiert.

OBJECTIVES To investigate real-world haematological toxicity, overall survival (OS) and the treatment characteristics of docetaxel and cabazitaxel chemotherapy in metastatic castration-resistant prostate cancer (mCRPC). PATIENTS AND METHODS This retrospective claims data study followed patients with mCRPC receiving cabazitaxel or docetaxel from their first chemotherapy infusion. Haematological toxicities were measured using treatment codes and inpatient diagnoses. OS was estimated using the Kaplan-Meier method. A multivariable Cox regression analysis was used to identify OS predictors. RESULTS Data from 539 patients administered docetaxel and 240 administered cabazitaxel were analysed. Regarding adverse events, within 8 months of treatment initiation, some kind of treatment for haematological toxicity was documented in 31% of patients given docetaxel and in 61% of patients given cabazitaxel. In the same period, hospitalization associated with haematological toxicity was documented in 11% of the patients in the docetaxel cohort and in 15% of the patients in the cabazitaxel cohort. In the docetaxel cohort, 9.9% of patients required reverse isolation and 13% were diagnosed with sepsis during hospitalization. In the cabazitaxel cohort, the cumulative incidence was 7.9% and 15%, respectively. The median OS was reached at 21.9 months in the docetaxel cohort and, because of a later line of therapy, at 11.3 months in the cabazitaxel cohort. A multivariate Cox regression revealed that indicators of locally advanced and metastatic disease, severe comorbidities, and prior hormonal/cytotoxic therapies were independent predictors of early death. CONCLUSION Cabazitaxel patients face an increased risk of haematological toxicities during treatment. Together with their short survival time, this calls for a strict indication when using cabazitaxel in patients with mCRPC.

Die demografische Entwicklung fuhrt zu einer Alterung der deutschen Bevolkerung. Hierdurch ergeben sich vielfaltige sozialpolitische Herausforderungen. Prognosen gehen von langfristigen Beitragsanpassungen in der Sozialversicherung aus. Der Einfluss der Alterung auf die Gesundheitsausgaben wird kontrovers diskutiert. Experten sind sich einig, dass die hohen Kosten am Lebensende einen wesentlichen Anteil der Ausgaben ausmachen. Fur Prognosen der zukunftigen Ausgaben sind valide Informationen zu den Kosten von Uberlebenden und Verstorbenen notwendig. Umfangreiche Daten sind fur Deutschland kaum verfugbar. Ziel der Arbeit war die detaillierte Analyse der jahrlichen Ausgaben der gesetzlichen Krankenversicherung (GKV). Datenbasis der Kostenkalkulation sind Abrechnungsdaten der Allgemeinen Ortskrankenkasse Niedersachsen (AOKN). Alle im Jahr 2017 durchgangig versicherte uberlebende und verstorbene Personen mit durchgangiger Versicherung vor dem Tod wurden eingeschlossen. Durchschnittskosten wurden standardisiert. Daten von 2,46 Mio. uberlebenden sowie 34.307 verstorbenen Versicherten wurden ausgewertet. Die jahresbezogenen Kosten in der Population der Uberlebenden (2756 €) lagen deutlich unter den Kosten der Verstorbenen (21.830 €). In der Gruppe der Uberlebenden nehmen die mittleren Kosten mit dem Alter zu und in der Gruppe der Verstorbenen sind die Kosten im jungeren Alter am hochsten. Bei detaillierter Analyse der Kosten am Lebensende zeigt sich ein exponentieller Anstieg der Kosten in den letzten 3 Lebensjahren mit den hochsten Kosten im Todesquartal (10.577 €). Die Daten geben einen detaillierten Einblick in die Struktur der GKV-Gesundheitsausgaben und konnen als Grundlage fur Prognosemodelle zur Ausgabenentwicklung dienen.

With an incidence of 58,000 cases per year, colorectal cancer (CRC) is the third most common type of cancer in Germany. Although guaiac-based fecal occult blood tests (gFOBT) and colonoscopy are accepted strategies for CRC screening offered for individuals aged 50 or 55 onwards, utilization rates remain low.This study examines various determinants for participation in CRC screening using claims data provided by the AOK Niedersachsen and covering the years 2014 to 2016. Using multivariate logistic regression models, we analyzed sociodemographic factors (sex, age, nationality, type of employment) associated with screening behavior, including individuals who underwent colonoscopy or gFOBT. The effect of school education and professional qualification was estimated using subgroups of employees with social insurance.The analysis consisted of 620,977 insured individuals in the study population for screening colonoscopy, while the gFOBT study population contained 845,191 individuals. With increasing age, individuals were less likely to participate in CRC screening. Participation rates for screening were higher for women than men in younger age groups. However, men in higher age groups showed increased participation rates in gFOBT screening. When compared with German citizens, Turkish citizens use the colonoscopy less often and the gFOBT more often. In contrast to employees with social insurance, unemployed individuals accept the prevention services less frequently, whereas pensioners and voluntarily insured individuals exhibit increased participation rates. In terms of education and professional qualification, we estimated a significantly lower participation rate exclusively for the least educated individuals. The results help to better understand patterns of utilization and can contribute to the development of information programs for specific groups.Das Kolorektale Karzinom stellt mit einer Inzidenz von ca. 58.000 Fällen pro Jahr die dritthäufigste Krebserkrankung in Deutschland dar. Obwohl GKV-Versicherten mit dem Stuhlbluttest (gFOBT) und der Koloskopie ab dem 50. bzw. 55. Lebensjahr effektive Präventionsmaßnahmen zur Verfügung stehen, sind die Inanspruchnahmeraten seit Jahren gering.Anhand von Routinedaten der AOK Niedersachsen wurde für den Zeitraum 2014 bis 2016 untersucht, welche Faktoren die Inanspruchnahme von Darmkrebspräventionsleistungen determinieren. Dafür wurde der Einfluss soziodemographischer Variablen (Alter, Geschlecht, Staatsangehörigkeit, Versichertenart) mithilfe multivariater, logistischer Regressionsmodelle geschätzt. Der Einfluss der Schul- und Berufsausbildung wurde mithilfe von Subgruppen sozialversicherungspflichtig Beschäftigter abgebildet.Die Studienpopulationen der Koloskopie und des gFOBT umfassen 620.977 bzw. 845.191 Versicherte. Mit steigendem Alter sinken bei Männern und Frauen die Teilnahmeraten von Darmkrebspräventionsleistungen. Frauen weisen dabei im Geschlechtervergleich in den jüngeren Altersgruppen höhere Inanspruchnahmeraten auf. Beim gFOBT hingegen zeigen Männer eine mit dem Alter steigende Inanspruchnahmerate. Türkische Staatsangehörige zeigen im Vergleich zu Bundesbürgern eine geringere Inanspruchnahmerate der Koloskopie, allerdings eine höhere für den gFOBT. Im Vergleich zu sozialversicherungspflichtig Beschäftigten nehmen Arbeitslose die Präventionsangebote seltener, freiwillig Versicherte und Rentenbezieher häufiger in Anspruch. In Bezug auf die Schul- und Berufsausbildung zeigen sich nur bei sehr gering Gebildeten signifikant geringere Teilnahmeraten. Die Ergebnisse können dazu beitragen relevante Personengruppen gezielter auf die Darmkrebsprävention anzusprechen.

Zusammenfassung Hintergrund Die nichtalkoholische Fettlebererkrankung (NAFLD) zählt zu den häufigsten Lebererkrankungen in Deutschland. Langfristig besteht das Risiko einer Leberzirrhose und weiterer Folgeerkrankungen. Epidemiologische Studien zur NAFLD in Deutschland liegen kaum vor. Ziel der vorliegenden Arbeit ist eine dezidierte Analyse der administrativen Inzidenz und Prävalenz (der diagnostizierten Erkrankungen) im Zeitraum von 2008 bis 2018. Methodik Die Grundlage der Analysen bilden GKV-Routinedaten. Es wurden Personen mit durchgängiger Versicherung im Analysejahr sowie im 3-jährigen Vorbeobachtungszeitraum eingeschlossen (1,7–2 Mio. Versicherte pro Analysejahr). Erkrankte Personen wurden über relevante ICD-10-Codes (K76.0 und K75.8) identifiziert. Ergebnisse Im Jahr 2018 wurde bei 4,66 % der Versicherten eine NAFLD-Diagnose gestellt, eine erstmalige Diagnose lag bei 0,87 % vor. Diagnosen einer Fettleberentzündung (NASH) waren mit 0,09 % selten. Im Zeitverlauf zeigt sich eine steigende NAFLD-Prävalenz, wobei sich die Zahl der jährlich erstmals diagnostizierten Patienten kaum verändert hat. Bei Vorliegen von Erkrankungen des metabolischen Syndroms war die Wahrscheinlichkeit einer NAFLD signifikant erhöht. Schlussfolgerungen Es zeigt sich, dass eine NAFLD im ärztlichen Alltag häufig diagnostiziert wird, auch wenn Daten aus populationsbasierten Untersuchungen eine noch höhere Prävalenz vermuten lassen.

Zusammenfassung Hintergrund Um den strukturellen und finanziellen Bedurfnissen der steigenden Zahl an Pflegebedurftigen gerecht zu werden, haben seit der Einfuhrung der Pflegeversicherung im Jahr 1995 diverse Reformen zur Weiterentwicklung des Pflegesystems beigetragen. Ziel dieser Studie ist es, eine Einschatzung der Reformansatze, Herausforderungen und Verbesserungsvorschlage aus Sicht von Pflegeberater*innen aufzuzeigen. Methode Grundlage der Analyse sind 14 semi-strukturierte, leitfadengestutzte Einzelinterviews mit Pflegeberater*innen. Die Interviews wurden wortlich transkribiert und nach der Methode der qualitativen Inhaltsanalyse ausgewertet. Ergebnisse Bei den gesetzlichen Anderungen nahmen die befragten Pflegeberater*innen spurbare positive Auswirkungen in Bezug auf den Leistungsumfang wahr, jedoch mit dem Verweis begrenzter Umsetzungsmoglichkeiten aufgrund fehlender Pflegeinfrastruktur. Herausforderungen wurden insbesondere in den haufigen Gesetzesanderungen, unklaren Zustandigkeiten und landerspezifischen Regelungen gesehen. Diskussion und Schlussfolgerung Insgesamt zeigten die Interviews komplexe Abhangigkeiten im deutschen Pflegesystem auf und die Notwendigkeit eines offentlichen Diskurses zum Thema Eigenverantwortung, Gerechtigkeit und einer flachendeckenden Versorgung. Der Aufbau einer zentralen Informationsplattform wurde vorgeschlagen, um die Transparenz der Angebote auf kommunaler Ebene zu erhohen.

To ensure specialized care of patients with rare diseases, numerous centres for rare diseases were funded over the past few years. The reimbursement of patients' ambulatory care in hospitals, however, is characterized by a plurality of forms of care and payment. There is some evidence of deficits in the reimbursement of care of patients suffering from a rare disease from studies on individual rare diseases.To investigate current forms of care provision and reimbursement of centres for rare diseases and to develop future approaches for sustainable compensation.Initially, centres for rare diseases in Germany were asked to provide information about their forms of care and reimbursement using questionnaires. Subsequently, two focus group interviews and one expert interview with representatives from centres for rare diseases, health insurance, health politics and patients were conducted to discuss current and future meritocratic forms of care provision and reimbursement. The data were evaluated using content analysis.Thirty-nine centres for rare diseases participated in the questionnaire survey. Of those, 38% receive a flat fee/allowance for university outpatient departments, the amount of which varies notably, and 41% obtain a mixed payment comprising an allowance for university outpatient departments and other forms of reimbursement. An under-recovery of costs in centres for rare diseases and its impact on patient care were mentioned in the interviews. In this context, a need to further develop forms of care provision and reimbursement has been identified. As the German population is continually aging and the majority of older adults still wish to ‘age in place’, the need for informal care provided by family and friends will correspondingly continue to increase. In addition, while the need for formal (professional) care services is also likely to increase, the supply already does not meet the demand in Germany today. The aim of our study is the elicitation of people’s willingness to provide informal care by means of a discrete choice experiment. The self-complete postal survey was disseminated to a random sample of the German general population in Lower Saxony. Data cleansing resulted in a final sample size of 280 participants. A conditional logit and a latent class model were estimated. All attributes were judged as highly relevant by the respondents. The results revealed that an increase in the care hours per day had the greatest negative impact overall on the willingness to provide informal care in our sample. The marginal willingness-to-accept for 1 h of informal care was €14.54 when having to provide informal care for 8 h in reference to 2 h per day. This value is considerably higher than the national minimum wage of €9.82. A three-class latent class model revealed preference heterogeneity. While a monetary compensation is often discussed to increase the willingness and availability of informal care in a country, our results show that this statement could not be generalized within our entire sample.

Zusammenfassung Hintergrund Seit vielen Jahren erweitern Biologika die Therapieoptionen bei rheumatoider Arthritis (RA). Der Einsatz dieser biologischen „disease-modifying antirheumatic drugs“ (bDMARDs) ist gemäß deutscher und europäischer Behandlungsleitlinien jedoch erst bei Ausschöpfung der Methotrexat (MTX)-Erstlinientherapie von mindestens 20 mg/Woche indiziert. Ziel der Studie ist es, die Leitliniengerechtigkeit der MTX-Verordnung im ambulanten Sektor vor der Biologikatherapie zu überprüfen. Methodik Es wurden Routinedaten der AOK-Niedersachsen der Jahre 2013 bis 2016 für alle Versicherten zur Verfügung gestellt, die im Studienzeitraum eine RA-Diagnose sowie eine bDMARD-Verordnung aufweisen. Innerhalb eines patientenindividuellen Beobachtungszeitraums von 180 Tagen vor der ersten bDMARD-Verordnung wurde die maximal verordnete MTX-Dosierung untersucht. Ergebnisse Die Studienpopulation umfasst 405 Patienten (90 inzident, 315 prävalent). With the introduction of biologics the treatment landscape for patients with rheumatoid arthritis (RA) has rapidly expanded; however, according to German and European treatment guidelines the use of biologic disease-modifying antirheumatic drugs (bDMARD) is only indicated after insufficient response under methotrexate (MTX) doses of at least 20 mg/week (first-line treatment). The aim of the study was to analyze the guideline compliance of MTX prescription in the outpatient sector prior to treatment with biologics.Claims data from the AOK Lower Saxony from 2013 to 2016 were provided for all insured patients with a diagnosis of RA and bDMARD prescription during the study period. Within a patient-specific observational period of 180 days prior to the first bDMARD prescription, the maximum prescribed MTX dosage was examined.Data from 90 incident and 315 prevalent RA patients were analyzed. A maximum MTX prescription of 20 mg/week was observed in 60.0% of incident patients and in 67.0% of prevalent patients. Men had a higher mean MTX maximum dose (17.1 ± 4.8 mg) than women (14.9 ± 5.0 mg; p 0.0001). Of the study population 29.6% received oral only prescriptions during the observational period. In 12.4% of patients a switch to parenteral administration was made.Targeted use of the full spectrum of therapies provided prior to initiation of bDMARD treatment may contribute to cost-effective RA care. Die Studie zeigt Indizien für mögliche Defizite in der ambulanten MTX-Verordnungspraxis auf und kann für eine effiziente Therapie sensibilisieren.

Zum Auftakt der 11. Jahrestagung der Deutschen Gesellschaft für Gesundheitsökonomie am 18. März 2019 in Augsburg hatte der Ausschuss „Krankenversicherung“ der dggö vier Experten eingeladen, um das Motto „Der Innovationsfonds an der Schwelle zur Regelversorgung (?)“ zu diskutieren. In diesem Artikel werden die Ergebnisse aus den Vorträgen der vier Experten und die anschließende Podiumsdiskussion zusammengefasst.

Although isocyanates are increasingly used in manufacturing and workplace exposure to isocyanates is widely recognized as one of the most frequent causes for occupational lung and skin diseases, little is known about the economic burden on the affected individual and the society. This study provides an overview on costs of occupational diseases related to isocyanates. We performed a systematic literature search of studies in the electronic databases of the German Institute of Medical Documentation and Information, and the Canadian Centre for Occupational Health and Safety. We extracted the key characteristics of the studies and performed a study quality assessment. We identified eight studies on the costs of illness, of which five focused on occupational lung diseases and three on occupational skin diseases. Further, eight studies calculated loss of income/compensation payments. Out of the 16 identified articles, only two reported costs directly attributable to isocyanate-induced diseases (asthma). Studies were hardly comparable because they differed substantially in their methodological approaches. Moreover, the quality assessment of the studies revealed substantial limitations. While a wide range of isocyanate-related costs was identified, consequences of isocyanate-related occupational diseases were considerable in terms of societal costs and loss of income. In most studies, indirect costs were the main cost driver. There is a need for high-quality cost of illness studies on isocyanate-induced diseases stratified by degree of severity and sex. Such studies provide valuable information to develop preventive strategies and set priorities for measures to lower the burden of professional health risks.

Zusammenfassung Hintergrund Mit Einführung der Biologika hat sich die Therapie der rheumatoiden Arthritis (RA) hinsichtlich der Wirksamkeit und Kosten grundlegend gewandelt. Ziel dieser Studie ist es, die der S1-Leitlinie der DGRh entsprechende Versorgung von RA-Patienten mit krankheitsmodifizierenden Medikamenten (disease-modifying anti-rheumatic drugs; DMARDs) anhand von Routinedaten der gesetzlichen Krankenversicherung zu analysieren. Methodik Die Abrechnungsdaten der Techniker Krankenkasse wurden retrospektiv für die Jahre 2011 – 2014 analysiert. Im Rahmen von Subgruppenanalysen wurden Verschreibungs- und Versorgungsunterschiede einzelner in der Leitlinie empfohlener Wirkstoffe wie konventionelle DMARDs und Biologika analysiert. Ergebnisse Die Studienpopulation umfasste 55 538 RA-Patienten (29,7 % inzidente, 70,3 % prävalente; 22,3 % M05: Seropositive chronische Polyarthritis, 77,7 % M06: Sonstige chronische Polyarthritis). Lediglich 21 616 Versicherte (38,9 %) bekamen mindestens einmal innerhalb eines Jahres nach der Erstdiagnose ein in der Leitlinie empfohlenes konventionelles DMARD oder Biologikum verordnet. Bei den inzidenten Patienten lag die Versorgungsquote mit krankheitsmodifizierenden Medikamenten unterhalb der prävalenten Patienten (31,5 vs. 42,1 %). 60,9 % der M05-Patienten und nur 29,7 % der M06-Patienten erhielten einen Einzelwirkstoff nach Index-Diagnose. Wenn ein Basistherapeutikum verordnet wurde, dann wurde dies durchschnittlich innerhalb eines Quartals nach Erstdiagnose verschrieben. Die führende Rolle in der Verschreibung von Basistherapeutika für die Indexmedikation übernimmt der Rheumatologe. Dennoch zeigte sich, dass nur 68,3 % der Patienten mindestens einmal innerhalb eines Jahres nach der Erstdiagnose einen rheumatologisch tätigen Facharzt konsultierten. Schlussfolgerung Mittels einer großen Stichprobe konnten Unterschiede in der leitliniengerechten Verordnung von krankheitsmodifizierenden Arzneimitteln für verschiedene Subgruppen sowie eine Unterversorgung bei nicht vom internistischen Rheumatologen behandelten Patienten aufgezeigt werden.

BackgroundElectrically assisted bicycles (e-bikes) have become increasingly popular and may facilitate active commuting and recreational cycling.ObjectiveTo evaluate the physical activity levels and usage characteristics of e-bikers and conventional cyclists under real-world conditions.MethodsWe conducted a prospective observational study in Germany to examine the effects of e-biking compared with conventional cycling on reaching the World Health Organization (WHO) target for physical activity—at least 150 min of moderate-to-vigorous physical activity (MVPA) per week. Study participants (1250 e-bikers and 629 conventional bike users) were equipped with activity trackers to assess the time, distance and heart rate during cycling over four consecutive weeks. Questionnaires were used to assess any traffic accidents incurred over 12 months.ResultsThe proportion of participants reaching 150 min of MVPA per week was higher for conventional bike users than for e-bike users (35.0% vs 22.4%, pConclusionE-bikes are associated with a lower probability of reaching WHO targets for MVPA due to reduced duration and a reduced cardiovascular effort during riding. However, e-bikes might facilitate active transportation, particularly in older individuals or those with pre-existing conditions.

Introduction In Germany, pediatric surgery is organized in a decentralized manner. A nationwide clinical registry does not exist. The aim of this research is to analyze the current status of pulmonary surgery in the country in respect of children and adolescents. Materials and Methods Claims data have been extracted from two health insurance databases representing 34% of the German population. Operative diagnoses and procedures, as well as patients' characteristics, were recorded from 2016 to 2019. Results The incidence of the four index diagnoses investigated matched the current literature in three of four entities. The main indications Conclusion Indications for pulmonary surgery in children arise from rare diagnoses. In total, 66% of lung resections are performed in patients 14 years. The majority of lung resections are performed as VATS. Patients ≤14 years are predominantly operated on by pediatric surgeons. The accessibility of pediatric pulmonary surgery is acceptable, but there is a low caseload per center.

Non-alcoholic fatty liver disease (NAFLD) is one of the most common chronic liver diseases in Germany. In the long term, there is an increased risk of developing liver cirrhosis and subsequent diseases. Epidemiologic studies on NAFLD prevalence in Germany are scarce. The aim of the study was to assess administrative incidence and prevalence or, more specifically, the number of patients diagnosed with NAFLD in the period from 2008 to 2018. Analyses are based on administrative data of a large statutory health insurance fund. All individuals who were insured in the year of analysis and in the three-year pre-observation period were included (between 1.7-2 million insured per analysis year). NAFLD-patients were identified using relevant ICD-10 codes (K76.0 und K75.8). In 2018, 4.66 % of insured persons had a NAFLD diagnosis, 0.87 % were diagnosed first-time. Diagnoses of nonalcoholic steatohepatitis (NASH) were comparatively rare (0.09 %). Data show an uptake of NAFLD diagnoses over time. The number of incident cases per year has hardly changed. Patients with diseases of the metabolic syndrome had an increased chance of being diagnosed with NAFLD. It becomes evident that NAFLD is frequently diagnosed in everyday medical practice, although data from population-based studies suggest an even higher prevalence. Die nichtalkoholische Fettlebererkrankung (NAFLD) zählt zu den häufigsten Lebererkrankungen in Deutschland. Langfristig besteht das Risiko einer Leberzirrhose und weiterer Folgeerkrankungen. Epidemiologische Studien zur NAFLD in Deutschland liegen kaum vor. Ziel der vorliegenden Arbeit ist eine dezidierte Analyse der administrativen Inzidenz und Prävalenz (der diagnostizierten Erkrankungen) im Zeitraum von 2008 bis 2018. Die Grundlage der Analysen bilden GKV-Routinedaten. Es wurden Personen mit durchgängiger Versicherung im Analysejahr sowie im 3-jährigen Vorbeobachtungszeitraum eingeschlossen (1,7–2 Mio. Versicherte pro Analysejahr). Erkrankte Personen wurden über relevante ICD-10-Codes (K76.0 und K75.8) identifiziert. Im Jahr 2018 wurde bei 4,66 % der Versicherten eine NAFLD-Diagnose gestellt, eine erstmalige Diagnose lag bei 0,87 % vor. Diagnosen einer Fettleberentzündung (NASH) waren mit 0,09 % selten. Im Zeitverlauf zeigt sich eine steigende NAFLD-Prävalenz, wobei sich die Zahl der jährlich erstmals diagnostizierten Patienten kaum verändert hat. Bei Vorliegen von Erkrankungen des metabolischen Syndroms war die Wahrscheinlichkeit einer NAFLD signifikant erhöht. Es zeigt sich, dass eine NAFLD im ärztlichen Alltag häufig diagnostiziert wird, auch wenn Daten aus populationsbasierten Untersuchungen eine noch höhere Prävalenz vermuten lassen.

Germany faces various socio-political challenges due to its ongoing ageing population. Significant increases in social security contributions are widely expected. The impact of ageing on healthcare expenditure is a controversial issue. Experts agree that costs for end-of-life care account for a significant part of total healthcare expenditures. For a meaningful forecast, detailed information on healthcare costs differentiated by survivors and decedents is necessary. Extensive data are hardly available for Germany. The aim of the analysis was therefore to describe healthcare costs in the statutory health insurance.The basis for the calculation is billing data from the statutory health insurance "AOK Niedersachsen" (Lower Saxony). Persons who survived or died in 2017 were included in the analysis. Average costs were standardised.The data of 2.46 million survivors and 34,307 decedents were analysed. The average annual healthcare costs were 2756 € for survivors and 21,830 € for decedents in the last year of life. The average healthcare costs for survivors increase with age whereas costs for decedents are highest in younger age groups and decline with increasing age. A detailed analysis of end-of-life costs shows an exponential increase of costs in the last three years of life with the highest costs in the quarter before death (10,577 €).The analysis gives a detailed overview on the structure of healthcare expenditure in the statutory health insurance and can serve as a basis for future forecasts regarding healthcare expenditure.HINTERGRUND: Die demografische Entwicklung führt zu einer Alterung der deutschen Bevölkerung. Hierdurch ergeben sich vielfältige sozialpolitische Herausforderungen. Prognosen gehen von langfristigen Beitragsanpassungen in der Sozialversicherung aus. Der Einfluss der Alterung auf die Gesundheitsausgaben wird kontrovers diskutiert. Experten sind sich einig, dass die hohen Kosten am Lebensende einen wesentlichen Anteil der Ausgaben ausmachen. Für Prognosen der zukünftigen Ausgaben sind valide Informationen zu den Kosten von Überlebenden und Verstorbenen notwendig. Umfangreiche Daten sind für Deutschland kaum verfügbar. Ziel der Arbeit war die detaillierte Analyse der jährlichen Ausgaben der gesetzlichen Krankenversicherung (GKV).Datenbasis der Kostenkalkulation sind Abrechnungsdaten der Allgemeinen Ortskrankenkasse Niedersachsen (AOKN). Alle im Jahr 2017 durchgängig versicherte überlebende und verstorbene Personen mit durchgängiger Versicherung vor dem Tod wurden eingeschlossen. Durchschnittskosten wurden standardisiert.Daten von 2,46 Mio. überlebenden sowie 34.307 verstorbenen Versicherten wurden ausgewertet. Die jahresbezogenen Kosten in der Population der Überlebenden (2756 €) lagen deutlich unter den Kosten der Verstorbenen (21.830 €). In der Gruppe der Überlebenden nehmen die mittleren Kosten mit dem Alter zu und in der Gruppe der Verstorbenen sind die Kosten im jüngeren Alter am höchsten. Bei detaillierter Analyse der Kosten am Lebensende zeigt sich ein exponentieller Anstieg der Kosten in den letzten 3 Lebensjahren mit den höchsten Kosten im Todesquartal (10.577 €).Die Daten geben einen detaillierten Einblick in die Struktur der GKV-Gesundheitsausgaben und können als Grundlage für Prognosemodelle zur Ausgabenentwicklung dienen.

Background Families of children with rare diseases (i.e., not more than 5 out of 10,000 people are affected) are often highly burdened with fears, insecurities and concerns regarding the affected child and its siblings. Although families caring for children with rare diseases are known to be at risk for mental disorders, the evaluation of special programs under high methodological standards has not been conducted so far. Moreover, the implementation of interventions for this group into regular care has not yet been accomplished in Germany. The efficacy and cost-effectiveness of a family-based intervention will be assessed. Methods/design The study is a 2x2 factorial randomized controlled multicenter trial conducted at 17 study centers throughout Germany. Participants are families with children and adolescents affected by a rare disease aged 0 to 21 years. Families in the face-to-face intervention CARE-FAM, online intervention WEP-CARE or the combination of both will be treated over a period of roughly 6 months. Topics discussed in the interventions include coping, family relations, and social support. Families in the control condition will receive treatment as usual. The primary efficacy outcome is parental mental health, measured by the Structured Clinical Interview for DSM-IV (SCID-I) by blinded external raters. Further outcomes will be assessed from the parents’ as well as the children’s perspective. Participants are investigated at baseline, 6, 12 and 18 months after randomization. In addition to the assessment of various psychosocial outcomes, a comprehensive health-economic evaluation will be performed. Discussion This paper describes the implementation and evaluation of two family-based intervention programs for Children Affected by Rare Disease and their Family’s Network (CARE-FAM-NET) in German standard care. A methodologically challenging study design is used to reflect the complexity of the actual medical care situation. This trial could be an important contribution to the improvement of care for this highly burdened group. Trial registration German Clinical Trials Register: DRKS00015859 (registered 18 December 2018) and ClinicalTrials.gov: NCT04339465 (registered 8 April 2020). Protocol Version: 15 August 2020 (Version 6.1). Trial status: Recruitment started on 1 January 2019 and will be completed on 31 March 2021.

Zusammenfassung Hintergrund und Ziel der Studie Bisher gibt es in Deutschland nur wenige Untersuchungen zu den soziodemographischen Einflussgrößen auf die Diagnosestellung einer ADHS-Erkrankung, obwohl die Kenntnis dieser Merkmale bedeutend für das diagnostische Prozedere und die Ausgestaltung von Präventionsprogrammen sein kann. Unser Ziel war es daher, die Evidenz zu den Einflussgrößen einer ADHS-Diagnose zu erweitern und Hinweise dafür zu geben, welchen Einfluss die familiäre Vorbelastung mit psychiatrischen Diagnosen und soziodemographischen Faktoren haben. Material und Methoden Durch ein 3:1 Kontrollgruppendesign mit Daten der Techniker Krankenkasse wurden psychosoziale, regionale und demographische Einflussfaktoren deskriptiv und anhand einer logistischen Regression untersucht. Die unabhängigen Variablen bestanden aus Alter, Geschlecht, Region und Komorbiditäten der Patienten, dem sozioökonomischen Berufsstatus und dem Bildungsniveau der Mitglieder, den psychiatrischen Diagnosen der Eltern sowie dem Alter der Eltern bei der Geburt der Kinder (Variablen des Familienhintergrunds). Ergebnisse Es wurden 9.881 ADHS-Patienten mit einem Durchschnittsalter von 14,9 Jahren [SD: 12,6] identifiziert. Anhand dieser umfassenden Stichprobe konnte bestätigt werden, dass das ADHS-Risiko bei männlichen Patienten höher liegt als bei weiblichen Patientinnen (OR: 2,3 [95%-KI: 2,2–2,5]). Ein mittlerer oder höherer Bildungsabschluss eines Mitglieds war mit niedrigerem Risiko einer ADHS-Diagnose im Vergleich zur Referenzkategorie niedriger Bildungsstatus verbunden (OR: 0,78 [95%-KI: 0,70–0,89] bzw. OR: 0,55 [95%-KI: 0,47–0,63]). Zudem erhöhte sich das ADHS-Risiko mit höherem Alter der Eltern sowie bei Vorliegen von psychiatrischen Diagnosen dieser (OR: 1,02 [95%-KI: 1,01–1,03] bzw. OR: 2,09 [95%-KI: 1,95–2,25]). Für den sozioökonomischen Beschäftigungsstatus konnte kein eindeutiger Zusammenhang nachgewiesen werden. Diskussion Das Wissen um soziodemographische Einflussfaktoren kann die Diagnosestellung unterstützen und Hinweise zur Entwicklung individueller Präventions- und Therapiekonzepte geben.

ZusammenfassungDie Verknüpfung verschiedener Datenquellen, genannt Datenlinkage oder auch Record Linkage, zur Beantwortung von wissenschaftlichen Fragestellungen findet in den letzten Jahren in Deutschland vermehrt Anwendung. Jedoch mangelt es bisher an publizierten Erfahrungen. Neue Projekte erarbeiten sich in der Regel autark voneinander das notwendige Handwerkszeug. Daher hat sich eine Gruppe von Forschern zusammengefunden, um ihre Erfahrungen zum Datenlinkage in Deutschland als mögliche Hilfestellung bzw. Anregung für Projekte, Gutachter sowie Datenschützer und Ethikkommissionen zusammenzustellen. Ziel dieser ersten Bestandsaufnahme zum Datenlinkage ist es deshalb, eine Unterstützung für zukünftige Projekte zu liefern, die Daten aus Deutschland auf individueller Ebene verknüpfen möchten. Neben den (datenschutz-)rechtlichen Rahmenbedingungen werden dabei auch praxisorientiert die Arten des Datenlinkage, deren Anwendungsfelder und Ansätze zur Vermeidung von Fehlern anhand von Beispielen dargestellt.

Introduction Benzodiazepines and related drugs (BZDR) should be avoided in patients with cognitive impairment. We evaluated the relationship between a BZDR treatment and the health status of patients with Alzheimer’s disease (AD). Methods Cross-sectional study in 395 AD patients using bivariate and multiple logistic analyses to assess correlations between the prescription of BZDR and patients’ characteristics (cognitive and functional capacity, health-related quality of life (HrQoL), neuropsychiatric symptoms). Results BZDR were used in 12.4% (n=49) of all participants. In bivariate analyses, the prescription was associated with a lower HrQoL, a higher need of care, and the presence of anxiety. Multivariate models revealed a higher risk of BZDR treatment in patients with depression (OR 3.85, 95% CI: 1.45 – 10.27). Community-dwelling participants and those treated by neurologists/psychiatrists had a lower risk of receiving BZDR (OR 0.33, 95% CI: 0.12 – 0.89 and OR 0.16, 95% CI: 0.07 – 0.36). Discussion The inappropriate use of BZDR conflicts with national and international guidelines. We suggest evaluating indications and treatment duration and improving the knowledge of alternative therapies in healthcare institutions.

A restrictive use of antipsychotic drugs in patients with Alzheimer's disease (AD) is recommended due to an increased risk of cerebrovascular events and mortality. We hypothesise that the prescription of antipsychotics is associated with the patients' socio-demographic and clinical status (e.g., dementia severity).The prescription of antipsychotics was cross-sectionally evaluated in 272 community-dwelling and 123 institutionalised patients with AD across all severity stages of dementia. The patients' clinical characteristics covered the cognitive status, neuropsychiatric symptoms, daily activities, and quality of life (HrQoL). To determine associations with the use of antipsychotics bivariate and logistic regression analyses were conducted.Totally, 25% of the patients were treated with antipsychotics. significantly less frequently than nursing home inhabitants (15.1% vs. 45.5%). Severely demented patients (MMSE 0-9) received antipsychotics most often (51.5%). Additionally, multiple regression analyses revealed a higher chance of prescription for participants with depressive symptoms (OR 2.3, 95% CI: 1.019-5.160) and those treated by neuropsychiatric specialists (OR 3.4, 95% CI: 1.408-8.328).Further longitudinal studies are required to assess the appropriateness of indications for antipsychotics and the reasons for a higher use in nursing home inhabitants and patients with severe dementia and depression.

Ein zentrales Informationsportal uber seltene Erkrankungen wurde etabliert, uber das u. a. Betroffene und Angehorige auf hochwertige Informationen zugreifen konnen. Hierfur wurden Qualitatskriterien entwickelt, die speziell auf Seiten uber seltene Erkrankungen ausgerichtet sind. Gleichzeitig soll das Informationsangebot die Bedarfe von Betroffenen berucksichtigen. Zunachst werden die Bedarfe von Patienten und Angehorigen an ein onlinebasiertes Informationsangebot eruiert. Darauf aufbauend wird gepruft, inwiefern die gemas den Qualitatskriterien verfugbare Informationsbasis diese Bedarfe decken kann. Ggf. sind Masnahmen zu entwickeln, die ein ebenso qualitats- wie bedarfsorientiertes Informationsmanagement sicherstellen. Zunachst finden qualitative Interviews mit Patienten und Angehorigen statt, die inhaltsanalytisch ausgewertet werden. Anschliesend erfolgt eine quantitative Auswertung von identifizierten Informationsseiten zu seltenen Erkrankungen. Untersucht wird, wie viele Seiten die Qualitatskriterien nicht erfullen, von welchen Anbietern diese stammen und welche Kriterien nicht erfullt sind. Es folgt ein Abgleich der quantitativen und qualitativen Ergebnisse. Bei der Suche nach Informationen im Internet stellen Webseiten von Selbsthilfegruppen eine bedeutsame Quelle dar. Diese werden als sehr vertrauenswurdig wahrgenommen und bieten im spateren Erkrankungsverlauf tiefgehende Information zu relevanten Informationsbereichen. Haufig entsprechen diese jedoch nicht den ZIPSE-Qualitatsanforderungen. Daher wird eine transparente Darstellung der Qualitat der ZIPSE-Informationsseiten gewahlt. Nicht qualitatsgesicherte Seiten konnen nun aktiv angefordert werden, sind jedoch deutlich gekennzeichnet.

Background:Inflammatory rheumatic disorders are very complex and require high medical resources. However, there is a shortage of care for these patients, which results in suboptimal reach of therapy objectives. Nevertheless, these very objectives need to be pursued quickly to prevent permanent joint damage. In order to ensure adequate care, multidisciplinary teams which include clinical nurse specialists are required. These clinical nurse specialists play an important role in improving standard-of-care in addition to the rheumatologist. The current standard of care ensures that essential medical provision remains intact, however, psychological, social, rehabilitative and educational needs are often skipped due to time constraints. While studies from e.g. the UK and Denmark have already supported the non-inferiority of nurse-led care (NLC)1, no such studies have yet been published in Germany.Objectives:To demonstrate the non-inferiority of NLC to the current standard-of-care, rheumatologist-led care (RLC), for patients with seropositive rheumatoid arthritis (RA) with induction, escalation or change of therapy regarding disease activity as well as different patient reported outcomes (PROs).Methods:This trial was conducted as a prospective multi-centered RCT with a non-inferiority design over the course of 12 months. Based on power calculations, 236 adults with RA were included in the study and randomized to either NLC or RLC. The primary outcome measure is disease activity (DAS28), assessed at baseline (T0), 6 weeks (T1), 3,6, 9, and 12 months (T3, T6, T9, T12). Secondary measures are health related quality of life (RAID), functionality (FFbH) and depression (PHQ9).Results:There are no significant differences between intervention group (IG) (n=117) and control group (CG) (n=119) at baseline. The mean age of the IG is 58.80 years (SD=12.09) and of the CG 58.34 years (SD=11.72). 72.4% of the IG and 78.1% of the CG are female. The mean duration of symptoms was 147 months (SD=144.63) for the IG and 116 months (108.89) for the CG. The mean DAS28 for the IG is 4.36 (SD=1.24) and 4.51 (SD=1.24) for the CG.A mixed one-way repeated measures ANOVA showed that the DAS28 improves significantly over time, Huyn-FeldtF(4.42, 751.72) = 105.701,p< .001, partialη2= 0.383, but the interaction of the DAS28 and the randomization is not significant, Huyn-FeldtF(4.42, 751.72) = 1.464,p= 0.260, partialη2= 0.009. No main effect for randomization was found, meaning that the IG and CG did not differ significantly,F(1, 170) = 1.005,p= 0.317, partialη2= 0.006.The Mann-Whitney-Test showed that the change of the secondary outcomes does not depend on the randomization FFbHU= 4978.50,Z= -.755,p=.450. RAIDU= 5121.00,Z= -.539,p=.590. PHQ9U= 4800.50,Z= -1.281,p=.200. Since the implementation of long-term care insurance in 1995, various reforms have contributed to the development of the German nursing care system in order to meet the structural and financial needs of the growing number of people in need of care. The aim of this paper is to present an assessment of care consultants on past legislation, challenges encountered during their work and suggestions to improve the current care system.14 semi-structured, face-to-face interviews were conducted with care consultants. All interviews were transcribed verbatim and a qualitative content analysis was performed.Care consultants perceived tangible improvements with regard to the scope of benefits as a direct result of recent legislation changes. The use of these benefits, however, is limited due to an insufficient growth of care infrastructure. Particularly frequent legislation changes, unclear responsibilities and dissimilar regulations between federal states were seen as challenges.Complex dependencies within the German nursing care system were identified and the necessity for a public discourse on themes such as self-responsibility, equity and a comprehensive (nationwide) care supply was underlined. A central information platform was proposed in order to concretely improve information about services available locally.

Individual data linkage of different data sources for research purposes is being increasingly used in Germany in recent years. However, generally accepted methodological guidance is missing. The aim of this article is to define such methodological standards for research projects. Another aim is to provide readers with a checklist for critical appraisal of research proposals and articles. Since 2016, an expert panel of members of different German scientific societies have worked together and developed 7 guidelines with a total of 27 practical recommendations. These recommendations include (1) research aims, questions, data sources and resources, (2) infrastructure and data flow, (3) data privacy, (4) ethics, (5) key variables and type of linkage, (6) data validation/quality assurance and (7) long-term use for future research questions. The authors provide a rationale for each recommendation. Future revisions will include any new developments in science and data privacy.Das personenbezogene Verknüpfen verschiedener Datenquellen (Datenlinkage) für Forschungszwecke findet in den letzten Jahren in Deutschland zunehmend Anwendung. Jedoch fehlen hierfür konsentierte methodische Standards. Ziel dieses Beitrages ist es, solche Standards für Forschungsvorhaben zu definieren. Eine weitere Intention ist es, dem Lesenden eine Checkliste zur Bewertung geplanter Forschungsvorhaben und Artikel bereitzustellen. Zu diesem Zweck hat eine aus Mitgliedern verschiedener Fachgesellschaften zusammengesetzte Expertengruppe seit 2016 insgesamt 7 Leitlinien mit 27 konkreten Empfehlungen erstellt. Die Gute Praxis Datenlinkage beinhaltet die folgenden Leitlinien: (1) Forschungsziele, Fragestellung, Datenquellen und Ressourcen, (2) Dateninfrastruktur und Datenfluss, (3) Datenschutz, (4) Ethik, (5) Schlüsselvariablen und Linkageverfahren, (6) Datenprüfung/Qualitätssicherung sowie (7) Langfristige Datennutzung für noch festzulegende Fragestellungen. Jede Leitlinie wird ausführlich diskutiert. Zukünftige Aktualisierungen werden wissenschaftliche und datenschutzrechtliche Entwicklungen berücksichtigen.

Background Rheumatoid arthritis (RA), psoriasis arthritis (PsA) and spondyloarthritis (SpA) are the most common chronic autoimmune rheumatic diseases. For all 3, the so-called “window of opportunity” [1-3] has been identified as the decisive factor affecting the outcome. Objectives The aim of the prospective study is to improve early diagnosis of RA, PsA and SpA and thus positively impact the quality of care for patients with the help of coordinating centers. The primary endpoint is the number of cases with an actual indication for immediate referral to a specialist in proportion to all reported cases. Methods The project establishes a network of the University Medical Center Mainz, the Medical School Hannover, the Center for Rheumatology Lower Saxony e.V., the University Medical Center Saarland, the ACURA Center of Rheumatology Rhineland-Palatinate, the Leibniz University Hannover and the University Medical Center Freiburg, local rheumatology specialists, the associations of statutory health insurance physicians and general practitioners and the 3 regional associations of the Deutsche Rheuma-Liga e.V. Covering 3 federal states, the network reaches approximately 13 million inhabitants. Primary care providers are given access to screening questionnaires to document potential early cases of RA, PsA, and SpA, based on characteristic symptoms. These are evaluated by multidisciplinary teams at the regional coordinating centers. If they fulfill the criteria for referral, patients get an appointment at a cooperating rheumatology specialist within weeks. If a rheumatic disease is diagnosed, physicians and patients receive questionnaires on sociodemographic data, quality of life, functionality, medication, diagnosis and psychological aspects. After 12 months, the questionnaires are completed a second time. The collected data is compared with a matched reference group of the German Rheumatism Research Center. It is planned to screen up to 8.700 people within the 30-month survey phase that started in October 2017. Results Preliminary data will be presented at the conference. Currently, 2.400 patients have been screened, of which 400 patients were included in the study and 15 already underwent their follow-up screening. Conclusion Previous studies have shown positive results of coordinated cooperation among the relevant care providers. This study estimates the healthcare costs associated with breast cancer (BC) for different treatment phases (initial, intermediate, terminal) in Germany from the payer’s perspective. The analysis uses claims data from the AOK Bayern covering 2011–2014 for continuously insured BC patients identified through inpatient and outpatient diagnoses. We calculate the healthcare costs attributable to BC using a control group design comparing the target population to a 1:2 matched control group adjusted for age, gender, and comorbidities. For incident and prevalent BC cases, we calculate age-standardized phase-specific incremental costs stratified by cost domain. The initial, intermediate, and terminal phases comprise 3841, 28,315, and 1767 BC cases, respectively. BC-related incremental costs follow a u-shaped curve, with costs highest near diagnosis and death, and lower in between. With average costs of €33,237 per incident and €28,211 per prevalent case in the remaining 11 months before death, the highest BC-related incremental healthcare costs can be found in the terminal phase. In the initial phase, there were mean incremental costs of €21,455 the first 11 months after diagnosis. In the intermediate phase, incremental costs totaled €2851 per incident and €2387 per prevalent case per year. Healthcare costs decreased with age in most phases. The cost drivers depend on the treatment phase, with cytostatic drugs and inpatient treatment showing the highest economic impact in most phases. The study concludes that BC care costs impose a relevant economic burden on statutory health insurance and vary substantially depending on the treatment phase.

Health technology assessments (HTAs) are an interdisciplinary method to support sustainable, evidence-based healthcare decisions. They systematically assess medical products, procedures, and technologies with respect to medical, economic, legal, social, and ethical aspects. This review analyzes the current use of HTAs in radiology in Germany and discusses challenges associated with HTAs. In particular, incentive structures of various players in the healthcare field involved in HTA implementation are considered for both the inpatient and outpatient sectors. Taking into account that the Joint Federal Committee (G-BA) has different authority between sectors ("ban reservation" for inpatients and "authorization right" for outpatients), we focus on the repercussions on reimbursement for new diagnosis or treatment methods by statutory health insurance companies. The G-BA's authority implicitly creates a paradox in terms of incentives to implement and finance HTAs: in the outpatient sector HTAs are considered necessary to evaluate new medical services while players may not have sufficient incentive to implement and finance HTAs in the inpatient sector. Characteristics of HTAs differ widely with respect to the items to be assessed. Therefore, an HTA for drug effectiveness is not easily transferable to radiological procedures. Within radiology, each method must be assessed individually (e. g. according to tumor stage). Despite these challenges, systematic compilation and critical assessment (regarding both cost and medical effectiveness) of available evidence should be a basic component of evidence-based radiology. Health-Technology-Assessments (HTA) sind eine interdisziplinär anzuwendende Methode zur Unterstützung nachhaltiger, evidenzbasierter Entscheidungen im Gesundheitswesen. Sie untersuchen in einem systematischen Prozess medizinische Produkte, Verfahren und Technologien unter medizinischen, ökonomischen, juristischen, sozialen und ethischen Aspekten hinsichtlich ihres Einsatzes in der medizinischen Versorgung. In dieser Übersichtsarbeit wird die aktuelle Verbreitung von HTA in der Radiologie in Deutschland betrachtet. Es werden Anreizstrukturen für die Durchführung von HTA diskutiert und die sich daraus ableitenden Herausforderungen für die Akteure im Gesundheitswesen im stationären und ambulanten Sektor erörtert. Dies erfolgt vor dem Hintergrund des zwischen den Sektoren bestehenden Unterschieds im Entscheidungsprozess darüber, welche Leistungen durch die gesetzlichen Krankenkassen erstattet werden. Es wird insbesondere betrachtet, welche Auswirkungen sich aus dem Verbotsvorbehalt (stationär) bzw. Erlaubnisvorbehalt (ambulant) des Gemeinsamen Bundesausschusses (G-BA) bezüglich der Erstattung neuer Diagnose- und Behandlungsmethoden ergeben. Aus den Befugnissen des G-BA ergibt sich hinsichtlich der Anreize zur Durchführung und Finanzierung systematischer Nachweise von Effektivität und Effizienz medizinischer Leistungen implizit ein Paradox: Im ambulanten Sektor sind derartige Nachweise ein notwendiges Instrument, während im stationären Sektor keine ausreichenden Anreize für die Akteure bestehen, in die Durchführung solcher Bewertungen der gleichen Leistung zu investieren. Es zeigt sich, dass der Bewertungsgegenstand gravierende Unterschiede innerhalb des HTA begründet. Bewertungsprozesse, die zur Überprüfung der Effektivität von Arzneimitteln etabliert sind, können nicht einfach auf radiologische Verfahren übertragen werden. Bei der Bewertung einzelner radiologischer Verfahren muss extrem differenziert, z. B. nach Tumorstadien, vorgegangen werden. Trotz dieser Herausforderungen sollte eine systematische Übersicht und kritische Bewertung verfügbarer Evidenz sowohl zur medizinischen Wirksamkeit als auch der Kosteneffizienz ein elementarer Baustein in der evidenzbasierten Radiologie (EbR) sein.
