Abstract

Modern approaches to investigate complex brain dynamics suggest to represent the brain as a functional network of brain regions defined by a brain atlas, while edges represent the structural or functional connectivity among them. This approach is also utilized for mathematical modeling of the resting-state brain dynamics, where the applied brain parcellation plays an essential role in deriving the model network and governing the modeling results. There is however no consensus and empirical evidence on how a given brain atlas affects the model outcome, and the choice of parcellation is still rather arbitrary. Accordingly, we explore the impact of brain parcellation on inter-subject and inter-parcellation variability of model fitting to empirical data. Our objective is to provide a comprehensive empirical evidence of potential influences of parcellation choice on resting-state whole-brain dynamical modeling. We show that brain atlases strongly influence the quality of model validation and propose several variables calculated from empirical data to account for the observed variability. A few classes of such data variables can be distinguished depending on their inter-subject and inter-parcellation explanatory power.

Abstract

Sleep complaints are the most prevalent syndromes in older adults, particularly in women. Moreover, they are frequently accompanied with a high level of depression and stress. Although several diffusion tensor imaging (DTI) studies reported associations between sleep quality and brain white matter (WM) microstructure, it is still unclear whether gender impacts the effect of sleep quality on structural alterations, and whether these alterations mediate the effects of sleep quality on emotional regulation. We included 389 older participants (176 females, age = 65.5 ± 5.5 years) from the 1000BRAINS project. Neuropsychological examinations covered the assessments of sleep quality, depressive symptomatology, current stress level, visual working memory, and selective attention ability. Based on the DTI dataset, the diffusion parameter maps, including fractional anisotropy (FA), mean diffusivity (MD), axial diffusivity (AD), and radial diffusivity (RD), were calculated and normalized to a population-specific FA template. According to the global Pittsburgh Sleep Quality Index (PSQI), 119 poor sleepers (PSQI: 10∼17) and 120 good sleepers (PSQI: 3∼6) were identified. We conducted a two by two (good sleepers/poor sleepers) × (males/females) analysis of variance by using tract-based spatial statistics (TBSS) and JHU-ICBM WM atlas-based comparisons. Moreover, we performed a voxel-wise correlation analysis of brain WM microstructure with the neuropsychological tests. Finally, we applied a mediation analysis to explore if the brain WM microstructure mediates the relationship between sleep quality and emotional regulation. No significant differences in brain WM microstructure were detected on the main effect of sleep quality. However, the MD, AD, and RD of pontine crossing tract and bilateral inferior cerebellar peduncle were significant lower in the males than females. Voxel-wise correlation analysis revealed that FA and RD values in the corpus callosum were positively related with depressive symptomatology and negatively related with current stress levels. Additionally, we found a significantly positive association between higher FA values in visual-related WM tracts and better outcomes in a visual pattern recognition test. Furthermore, a mediation analysis suggested that diffusion metrics within the corpus callosum partially mediated the associations between poor sleep quality/high stress and depressive symptomatology.

Abstract

Parameters of water diffusion in white matter derived from diffusion-weighted imaging (DWI), such as fractional anisotropy (FA), mean, axial, and radial diffusivity (MD, AD, and RD), and more recently, peak width of skeletonized mean diffusivity (PSMD), have been proposed as potential markers of normal and pathological brain ageing. However, their relative evolution over the entire adult lifespan in healthy individuals remains partly unknown during early and late adulthood, and particularly for the PSMD index. Here, we gathered and analyzed cross-sectional diffusion tensor imaging (DTI) data from 10 population-based cohort studies in order to establish the time course of white matter water diffusion phenotypes from post-adolescence to late adulthood. DTI data were obtained from a total of 20,005 individuals aged 18.1 to 92.6 years and analyzed with the same pipeline for computing skeletonized DTI metrics from DTI maps. For each individual, MD, AD, RD, and FA mean values were computed over their FA volume skeleton, PSMD being calculated as the 90% peak width of the MD values distribution across the FA skeleton. Mean values of each DTI metric were found to strongly vary across cohorts, most likely due to major differences in DWI acquisition protocols as well as pre-processing and DTI model fitting. However, age effects on each DTI metric were found to be highly consistent across cohorts. RD, MD, and AD variations with age exhibited the same U-shape pattern, first slowly decreasing during post-adolescence until the age of 30, 40, and 50 years, respectively, then progressively increasing until late life. FA showed a reverse profile, initially increasing then continuously decreasing, slowly until the 70s, then sharply declining thereafter. By contrast, PSMD constantly increased, first slowly until the 60s, then more sharply. These results demonstrate that, in the general population, age affects PSMD in a manner different from that of other DTI metrics. The constant increase in PSMD throughout the entire adult life, including during post-adolescence, indicates that PSMD could be an early marker of the ageing process.

4. Heavy rockbursts due to longwall mining near protective pillars: A case study

Abstract

Rockburst represents a very dangerous phenomenon in deep underground mining in unfavourable conditions such as great depth, high horizontal stress, proximity of important tectonic structures, and unmined pillars. The case study describes a recorded heavy rockburst in the Czech part of the Upper Silesian Coal Basin, which occurred during longwall mining near the protective pillar. The artificial dividing of geological blocks and creation of mining protective pillars (shaft pillars, crosscut pillars etc.) is a dangerous task in light of rockbursts occurring mainly due to overstressing of remaining pillars. A simple model of this situation is presented. Natural and mining conditions are analysed and presented in detail as well as registered seismicity during longwall mining in the area. Recorded rockbursts in the area of interest are described and their causes discussed. Many rockbursts near protective pillars were recorded in this mining region. Methodical instructions for rockburst prevention in proximity of protective pillars as well as for gates driving were devised based on the evaluation of rockburst causes. Abstract

Background: Progressive cognitive decline following multimodal neurooncological treatment is a common observation in patients suffering from malignant glioma. Alterations of the default-mode network (DMN) represent a possible source of impaired neurocognitive functioning and were analyzed in these patients. Methods: Eighty patients (median age, 51 years) with glioma (WHO grade IV glioblastoma, n = 57; WHO grade III anaplastic astrocytoma, n = 13; WHO grade III anaplastic oligodendroglioma, n = 10) and ECOG performance score 0–1 underwent resting-state functional MRI (rs-fMRI) and neuropsychological testing at a median interval of 13 months (range, 1–114 months) after initiation of therapy. For evaluation of structural and metabolic changes after treatment, anatomical MRI and amino acid PET using O-(2-[18F]fluoroethyl)-L-tyrosine (FET) were simultaneously acquired to rs-fMRI on a hybrid MR/PET scanner. A cohort of 80 healthy subjects matched for gender, age, and educational status served as controls. Results: The connectivity pattern within the DMN (12 nodes) of the glioma patients differed significantly from that of the healthy subjects but did not depend on age, tumor grade, time since treatment initiation, presence of residual/recurrent tumor, number of chemotherapy cycles received, or anticonvulsive medication. Small changes in the connectivity pattern were observed in patients who had more than one series of radiotherapy. In contrast, structural tissue changes located at or near the tumor site (including resection cavities, white matter lesions, edema, and tumor tissue) had a strong negative impact on the functional connectivity of the adjacent DMN nodes, resulting in a marked dependence of the connectivity pattern on tumor location. In the majority of neurocognitive domains, glioma patients performed significantly worse than healthy subjects. Correlation analysis revealed that reduced connectivity in the left temporal and parietal DMN nodes was associated with low performance in language processing and verbal working memory. Furthermore, connectivity of the left parietal DMN node also correlated with processing speed, executive function, and verbal as well as visual working memory. Overall DMN connectivity loss and cognitive decline were less pronounced in patients with higher education. Conclusion: Personalized treatment strategies for malignant glioma patients should consider the left parietal and temporal DMN nodes as vulnerable regions concerning neurocognitive outcome.

7. Intensity standardisation of 7T MR images for intensity-based segmentation of the human hypothalamus.

Abstract

The high spatial resolution of 7T MRI enables us to identify subtle volume changes in brain structures, providing potential biomarkers of mental disorders. Most volumetric approaches require that similar intensity values represent similar tissue types across different persons. By applying colour-coding to T1-weighted MP2RAGE images, we found that the high measurement accuracy achieved by high-resolution imaging may be compromised by inter-individual variations in the image intensity. To address this issue, we analysed the performance of five intensity standardisation techniques in high-resolution T1-weighted MP2RAGE images. Twenty images with extreme intensities in the GM and WM were standardised to a representative reference image. We performed a multi-level evaluation with a focus on the hypothalamic region-analysing the intensity histograms as well as the actual MR images, and requiring that the correlation between the whole-brain tissue volumes and subject age be preserved during standardisation. The results were compared with T1 maps. Linear standardisation using subcortical ROIs of GM and WM provided good results for all evaluation criteria: it improved the histogram alignment within the ROIs and the average image intensity within the ROIs and the whole-brain GM and WM areas. This method reduced the inter-individual intensity variation of the hypothalamic boundary by more than half, outperforming all other methods, and kept the original correlation between the GM volume and subject age intact. Mixed results were obtained for the other four methods, which sometimes came at the expense of unwarranted changes in the age-related pattern of the GM volume. The mapping of the T1 relaxation time with the MP2RAGE sequence is advertised as being especially robust to bias field inhomogeneity. We found little evidence that substantiated the T1 map's theoretical superiority over the T1-weighted images regarding the inter-individual image intensity homogeneity.

Abstract

Background: Hypertrophic pyloric stenosis in otherwise healthy neonates frequently requires urgent surgical procedure but anaesthesia care may result in respiratory complications, such as hypoxaemia, pulmonary aspiration of gastric contents, and postoperative apnoea. The primary aim was to study whether or not the incidence of difficult airway management and of hypoxaemia in neonates undergoing pyloric stenosis repair was higher than that in neonates undergoing other surgeries. Methods: Data on neonates and infants undergoing anaesthesia and surgery for pyloric stenosis were extracted from the NEonate and Children audiT of Anesthesia pRactice In Europe (NECTARINE) database, for secondary analysis. Results: We identified 310 infants who had anaesthesia for surgery for pyloric stenosis. Difficult airway management (more than two attempts at laryngoscopy) was higher in children with pyloric stenosis when compared with the entire NECTARINE cohort (7.9% [95% confidence interval {CI}, 5.22–11.53] vs 4.4% [95% CI, 1.99–6.58]; relative risk [RR]=1.81 [95% CI, 1.21–2.69]; P=0.004), whereas transient hypoxaemia with oxygen saturation

10. Interrelating differences in structural and functional connectivity in the older adult's brain

Abstract

In the normal aging process, the functional connectome restructures and shows a shift from more segregated to more integrated brain networks, which manifests itself in highly different cognitive performances in older adults. Underpinnings of this reorganization are not fully understood, but may be related to age-related differences in structural connectivity, the underlying scaffold for information exchange between regions. The structure-function relationship might be a promising factor to understand the neurobiological sources of interindividual cognitive variability, but remain unclear in older adults. Here, we used diffusion weighted and resting-state functional magnetic resonance imaging as well as cognitive performance data of 573 older subjects from the 1000BRAINS cohort (55-85 years, 287 males) and performed a partial least square regression on 400 regional functional and structural connectivity (FC and SC, respectively) estimates comprising seven resting-state networks. Our aim was to identify FC and SC patterns that are, together with cognitive performance, characteristic of the older adults aging process. Results revealed three different aging profiles prevalent in older adults. FC was found to behave differently depending on the severity of age-related SC deteriorations. A functionally highly interconnected system is associated with a structural connectome that shows only minor age-related decreases. Because this connectivity profile was associated with the most severe age-related cognitive decline, a more interconnected FC system in older adults points to a process of dedifferentiation. Thus, functional network integration appears to increase primarily when SC begins to decline, but this does not appear to mitigate the decline in cognitive performance.

Abstract

Mining of hard coal creates large underground cavities, which significantly affect the subsurface and land surface. Observations of land surface behaviour after the closure of mines demonstrate that these threats do not disappear. During mine water rebound in the post‑mining phase of underground coalmines, the water flows into the open mine workings and other altered or fissured areas. This paper provides early observations from the PostMinQuake project, which is designed to identify mechanisms, relevant parameters and dependencies of mining and geological parameters causing post-mining seismicity in several European coal regions. It also presents the correlation between seismicity and water table level in the studied basins that has been observed during the post‑mining phase and shows the necessity of implementing new procedures to monitor these seismic events.

Abstract

Background: Neonates and infants are susceptible to hypoxaemia in the perioperative period. The aim of this study was to analyse interventions related to anaesthesia tracheal intubations in this European cohort and identify their clinical consequences. Methods: We performed a secondary analysis of tracheal intubations of the European multicentre observational trial (NEonate and Children audiT of Anaesthesia pRactice IN Europe [NECTARINE]) in neonates and small infants with difficult tracheal intubation. The primary endpoint was the incidence of difficult intubation and the related complications. The secondary endpoints were the risk factors for severe hypoxaemia attributed to difficult airway management, and 30 and 90 day outcomes. Results: Tracheal intubation was planned in 4683 procedures. Difficult tracheal intubation, defined as two failed attempts of direct laryngoscopy, occurred in 266 children (271 procedures) with an incidence (95% confidence interval [CI]) of 5.8% (95% CI, 5.1e6.5). Abstract

BACKGROUND Little is known about current clinical practice concerning peri-operative red blood cell transfusion in neonates and small infants. Guidelines suggest transfusions based on haemoglobin thresholds ranging from 8.5 to 12���g���dl-1, distinguishing between children from birth to day 7 (week 1), from day 8 to day 14 (week 2) or from day 15 (���week 3) onwards. OBJECTIVE To observe peri-operative red blood cell transfusion practice according to guidelines in relation to patient outcome. DESIGN A multicentre observational study. SETTING The NEonate-Children sTudy of Anaesthesia pRactice IN Europe (NECTARINE) trial recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. PATIENTS The data included 5609 patients undergoing 6542 procedures. Inclusion criteria was a peri-operative red blood cell transfusion. MAIN OUTCOME MEASURES The primary endpoint was the haemoglobin level triggering a transfusion for neonates in week 1, week 2 and week 3. Secondary endpoints were transfusion volumes, 'delta haemoglobin' (preprocedure - transfusion-triggering) and 30-day and 90-day morbidity and mortality. RESULTS Peri-operative red blood cell transfusions were recorded during 447 procedures (6.9%). The median haemoglobin levels triggering a transfusion were 9.6 [IQR 8.7 to 10.9] g���dl-1 for neonates in week 1, 9.6 [7.7 to 10.4] g���dl-1 in week 2 and 8.0 [7.3 to 9.0] g���dl-1 in week 3. The median transfusion volume was 17.1 [11.1 to 26.4] ml���kg-1 with a median delta haemoglobin of 1.8 [0.0 to 3.6] g���dl-1. Thirty-day morbidity was 47.8% with an overall mortality of 11.3%. CONCLUSIONS Results indicate lower transfusion-triggering haemoglobin thresholds in clinical practice than suggested by current guidelines. The high morbidity and mortality of this NECTARINE sub-cohort calls for investigative action and evidence-based guidelines addressing peri-operative red blood cell transfusions strategies. 22. Volumetric changes in the focal areas of seismic events corresponding to destress blasting

Abstract

The typical development of total volumetric change in the focal areas of seismic events, corresponding to destress blasting, is characterized as an explosive phase followed by an implosive phase and with alternating additional phases following on from that. In a few cases, a non-typical development of volumetric change was identified, where the first phase was implosive and the second phase, explosive. This development is mainly typical for induced seismic events recorded during mining, not for destress blasting. Seismic events were recorded during longwall mining in the Czech part of the Upper Silesian Coal Basin, where the destress blasting technique is used as a rockburst prevention active measure. Kinematic source processes in the focal areas of selected seismic events were analyzed by the seismic moment tensor inversion method, as well as by studying geomechanical rock mass conditions at the localities of the seismic events. The main goal of the analysis was to attempt to identify the reasons for non-typical development of volumetric changes in these cases. Volumetric changes were analyzed for seismic events with energy greater than 104 J, recorded in the period of time from 1993 to 2009 (1109 events). 80% (891) of the recorded seismic events were induced seismic events that were registered during longwall mining and 20% (218) corresponded to destress blasting events. Research shows that the main reason for the non-typical development of volumetric changes in the focal areas of seismic events is an association with destress blasting in the rock mass, which is very close to rock mass overstressing. The detonation of explosives in boreholes, which would dominate the first phase of volumetric changes, probably obscured stress release in the rock mass, as manifested in the first implosion phase of the volumetric changes in this case. Abstract

Background: Neonates and infants requiring anaesthesia are at risk of physiological instability and complications, but triggers for peri-anaesthetic interventions and associations with subsequent outcome are unknown. Methods: This prospective, observational study recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. The primary aim was to identify thresholds of pre-determined physiological variables that triggered a medical intervention. The secondary aims were to evaluate morbidities, mortality at 30 and 90 days, or both, and associations with critical events. Results: Infants (n=5609) born at mean (standard deviation [SD]) 36.2 (4.4) weeks postmenstrual age (35.7% preterm) underwent 6542 procedures within 63 (48) days of birth. Abstract

Rockburst represents a very dangerous phenomenon in deep underground mining in unfavourable conditions such as great depth, high horizontal stress, proximity of important tectonic structures, and unmined pillars. The case study describes a recorded heavy rockburst in the Czech part of the Upper Silesian Coal Basin, which occurred during longwall mining near the protective pillar. The artificial dividing of geological blocks and creation of mining protective pillars (shaft pillars, crosscut pillars etc.) is a dangerous task in light of rockbursts occurring mainly due to overstressing of remaining pillars. A simple model of this situation is presented. Natural and mining conditions are analysed and presented in detail as well as registered seismicity during longwall mining in the area. Recorded rockbursts in the area of interest are described and their causes discussed. Many rockbursts near protective pillars were recorded in this mining region. Methodical instructions for rockburst prevention in proximity of protective pillars as well as for gates driving were devised based on the evaluation of rockburst causes. Abstract

In the Upper Silesian Coal Basin (USCB), coal seams are exploited under progressively more difficult geological and mining conditions (greater depth, higher horizontal stress, more frequent occurrence of competent rock layers, etc.). Mining depth, dislocations and mining remnants in coal seams are the most important factors responsible for the occurrence of rockburst hazards. Longwall mining next to the mining edges of neighbouring coal seams is particularly disadvantageous. The levels of rockburst hazards are minimised via the use of rockburst prevention methods. One active prevention method is torpedo blasting in roof rocks. Torpedo blastings are performed in order to decrease local stress concentrations in rock masses and to fracture the roof rocks to prevent or minimise the impact of high-energy tremors on excavations. The estimation of the effectiveness of torpedo blasting is particularly important when mining is under difficult geological and mining conditions. Torpedo blasting is the main form of active rockburst prevention in the assigned colliery in the Polish part of the USCB. The effectiveness of blasting can be estimated using the seismic effect method, in which the seismic monitoring data and the mass of explosives are taken into consideration. The seismic effect method was developed in the Czech Republic and is always being used in collieries in the Czech part of the coal basin. Now, this method has been widely adopted for our selected colliery in the Polish part of the coal basin. The effectiveness of torpedo blastings in the faces and galleries of the assigned longwall in coal seam 506 has been estimated. The results show that the effectiveness of torpedo blastings for this longwall was significant in light of the seismic effect method, which corresponds to the in situ observations. The seismic effect method is regularly applied to estimating the blasting effectiveness in the selected colliery.

Abstract

A first attempt of using the Room and Pillar mining method in the Czech Republic was implemented in the southern part of the Upper Silesian Coal Basin. This stratigraphical unit is well known for its complex geological structure with more than 40 main coal seams vertically distributed in the area of about 122 km 2 of active mines and a complex slide/overlap fault system. The fact that the extraction of coal by this method is realised at a depth around 900 m below the surface has also made it one of the deepest Room and Pillar mining sites in the world. These facts brought lead to several questions. The main question connected to sustaining the safety of the workers before and after the advance of the mining works has resulted in this study of the prediction of unnatural seismological activity based on data that were collected during and after the excavation of the coal seam. Also, we ask whether the methods can be used for the prediction of inhomogeneous stress distributions in this kind of openings. Seismological monitoring, which is the main goal of this paper, was supplemented by stress and deformation measurements in coal pillars and surrounding rocks. More stations were added to the network of seismic stations of the coal mine at different levels surrounding the Room and Pillar site. We give the first results of the evaluation of seismic activity during the driving of the roadway in the first mining panel. We describe the site and mining conditions, details about seismological network, and a comparison of seismic activity with the gate driving advance and creation of coal pillars. This is compared to stress changes in the coal pillars.

Abstract

The hypothalamus, a small diencephalic gray matter structure, is part of the limbic system. Volumetric changes of this structure occur in psychiatric diseases, therefore there is increasing interest in precise volumetry. Based on our detailed volumetry algorithm for 7 Tesla magnetic resonance imaging (MRI), we developed a method for 3 Tesla MRI, adopting anatomical landmarks and work in triplanar view. We overlaid T1-weighted MR images with gray matter-tissue probability maps to combine anatomical information with tissue class segmentation. Then, we outlined regions of interest (ROIs) that covered potential hypothalamus voxels. Within these ROIs, seed growing technique helped define the hypothalamic volume using gray matter probabilities from the tissue probability maps. This yielded a semi-automated method with short processing times of 20-40 min per hypothalamus. In the MRIs of ten subjects, reliabilities were determined as intraclass correlations (ICC) and volume overlaps in percent. Three raters achieved very good intra-rater reliabilities (ICC 0.82-0.97) and good inter-rater reliabilities (ICC 0.78 and 0.82). Overlaps of intra- and inter-rater runs were very good (≥ 89.7%). We present a fast, semi-automated method for in vivo hypothalamus volumetry in 3 Tesla MRI.

Abstract

Advances in imaging technology have enabled automated approaches for quantitative image analysis. In this study, a high content object based image analysis method was developed for quantification of β-amyloid (Aβ) plaques in postmortem brains of Alzheimer's disease (AD) subjects and in transgenic mice over overexpressing Aβ. Digital images acquired from immunohistochemically stained sections of the superior frontal gyrus were analyzed for Aβ plaque burden using a Definiens object-based segmentation approach. Blinded evaluation of Aβ stained sections from AD and aged matched human subjects accurately identified AD cases with one exception. Brains from transgenic mice overexpressing Aβ (PS1APP mice) were also evaluated by our Definiens object based image analysis approach. We observed an age-dependent increase in the amount of Aβ plaque load that we quantified in both the hippocampus and cortex. From the contralateral hemisphere, we measured the amount of Aβ in brain homogenates biochemically and observed a significant correlation between our biochemical measurements and those that we measured by our object based Definiens system in the hippocampus. Assessment of Aβ plaque load in PS1APP mice using a manual segmentation technique (Image-Pro Plus) confirmed the results of our object-based image analysis approach. Image acquisition and analysis of 32 stained human slides and 100 mouse slides were executed in 8 h and 22 h, respectively supporting the relatively high throughput features of the Definiens platform. The data show that digital imaging combined with object based image analysis is a reliable and efficient approach to quantifying Aβ plaques in human and mouse brain.

Abstract

The purpose of this study was to establish and validate fully automatic measurement of cell proliferation on scanned images of rat cecum and colon. Tissue slides were taken from a 4-week mechanistic study and processed for BrdU immunohistochemistry. Four sections of the cecum and colon per slide were scanned with the Zeiss MIRAX SCAN and transferred to the Definiens eCognition Analyst LS5.0 system for evaluation. Two rule sets for automatic counting of BrdU-positive and negative nuclei from mucosal cells on the image tiles were created by Definiens, one for cecum, one for colon. For validation, manual counting of 16 randomly selected tiles from five different slides of colon and cecum was performed. Negative and positive cell nuclei were counted in each image tile by four different people. Comparison of results from manual counting with the automatic counting showed that the sum as well as single tile data and labeling index (LI) from automatic counting were within the range of manual counting results ±10%. Automatic counting included only cell nuclei within the mucosa whereas muscularis and lymphoid tissue as well as wrinkles from tissue preparation were excluded. In addition, two data sets from automatic counting of the same image tile were compared: (1) data where image tiles with incorrect detection of mucosa were excluded from further calculation of LI and area, and (2) data where no visual check was performed and all measurements were included. Results were very similar for both data sets. The necessity of the manual correction may therefore be doubted.

Abstract

Diffusion tensor imaging revealed that trait anxiety predicts the microstructural properties of a prespecified fiber tract between the amygdala and the perigenual anterior cingulate cortex. Besides this particular pathway, it is likely that other pathways are also affected. We investigated white matter differences in persons featuring an anxious or a nonanxious personality, taking into account all potential pathway connections between amygdala and anxiety-related regions of the prefrontal cortex (PFC). Diffusion-weighted images, measures of trait anxiety and of reappraisal use (an effective emotion-regulation style), were collected in 48 females. With probabilistic tractography, pathways between the amygdala and the dorsolateral PFC, dorsomedial PFC, ventromedial PFC, and orbitofrontal cortex (OFC) were delineated. The resulting network showed a direct ventral connection between amygdala and PFC and a second limbic connection following the fornix and the anterior limb of the internal capsule. Reappraisal use predicted the microstructure of pathways to all calculated PFC regions in the left hemisphere, indicating stronger pathways for persons with high reappraisal use. Trait anxiety predicted the microstructure in pathways to the ventromedial PFC and OFC, indexing weaker connections in trait-anxious persons. These effects appeared in the right hemisphere, supporting lateralization and top-down inhibition theories of emotion processing. Whereas a specific microstructure is associated with an anxious personality, a different structure subserves emotion regulation. Both are part of a broad fiber tract network between amygdala and PFC.

Abstract

Diffusion MRI (dMRI) measurements are used for inferring the microstructural properties of white matter and to reconstruct fiber pathways. Very often voxels contain complex fiber configurations comprising multiple bundles, rendering the simple diffusion tensor model unsuitable. Multi-compartment models deliver a convenient parameterization of the underlying complex fiber architecture, but pose challenges for fitting and model selection. Spherical deconvolution, in contrast, very economically produces a fiber orientation density function (fODF) without any explicit model assumptions. Since, however, the fODF is represented by spherical harmonics, a direct interpretation of the model parameters is impossible. Based on the fact that the fODF can often be interpreted as superposition of multiple peaks, each associated to one relatively coherent fiber population (bundle), we offer a solution that seeks to combine the advantages of both approaches: first the fiber configuration is modeled as fODF represented by spherical harmonics and then each of the peaks is parameterized separately in order to characterize the underlying bundle. In this work, the fODF peaks are approximated by Bingham distributions, capturing first and second-order statistics of the fiber orientations, from which we derive metrics for the parametric quantification of fiber bundles. We propose meaningful relationships between these measures and the underlying microstructural properties. We focus on metrics derived directly from properties of the Bingham distribution, such as peak length, peak direction, peak spread, integral over the peak, as well as a metric derived from the comparison of the largest peaks, which probes the complexity of the underlying microstructure. We compare these metrics to the conventionally used fractional anisotropy (FA) and show how they may help to increase the specificity of the characterization of microstructural properties. While metrics relying on the first moments of the Bingham distributions provide relatively robust results, second-order metrics representing the peak spread are only meaningful, if the SNR is very high and no fiber crossings are present in the voxel.

This article describes a method for the automatic detection of the proximal femur in radiographs using a template-based mutual information registration method. It will be part of a planned, larger system for automated estimation of osteoporosis in the femoral neck. Our multi-step optimization process achieves a successful registration rate of 70% to 95%.

Abstract

Based on an examination of current methods used to define and assess a defendant's competency to stand trial, the authors propose an assessment and research instrument, referred to as the Interdisciplinary Fitness Interview (IFI). The IFI is a structured interview and rating scale designed to take into account both legal and mental health issues, and calls for an interdisciplinary approach to the assessment of competency. The purpose of the present study was to provide preliminary reliability and validity data on the use of the IFI in one jurisdiction. The results are discussed in terms of policy implications and the development of methods for evaluating competency with brief screening interviews in less restrictive settings.

Abstract

Diffusion MRI is a non-invasive method that potentially gives insight into the brain's white matter structure regarding the pathway of connections and properties of the axons. Here, we propose a novel global tractography method named Plausibility Tracking that provides the most plausible pathway, modeled as a smooth spline curve, between two locations in the brain. Compared to other tractography methods, plausibility tracking combines the more complete connectivity pattern of probabilistic tractography with smooth tracks that are globally optimized using the fiber orientation density function and hence is relatively robust against local noise and error propagation. It has been tested on phantom and biological data and compared to other methods of tractography. Plausibility tracking provides reliable local directions all along the fiber pathways which makes it especially interesting for tract-based analysis in combination with direction dependent indices of diffusion MRI. In order to demonstrate this potential of plausibility tracking, we propose a framework for the assessment and comparison of diffusion derived tissue properties. This framework comprises atlas-guided parameterization of tract representation and advanced bundle-specific indices describing fiber density, fiber spread and white matter complexity. We explore the new method using real data and show that it allows for a more specific interpretation of the white matter's microstructure compared to rotationally invariant indices derived from the diffusion tensor.

Highlights • Following therapy, integrity of the DMN is affected in malignant glioma patients. • Treatment-related brain tissue changes reduce the connectivity of adjacent DMN nodes • Connectivites of the left parietal and temporal DMN nodes relate to cognitive outcome • For therapy planning, these DMN nodes should be regarded as vulnerable regions. • DMN connectivity loss and cognitive decline are less pronounced in higher education, Background Progressive cognitive decline following multimodal neurooncological treatment is a common observation in patients suffering from malignant glioma. Alterations of the default-mode network (DMN) represent a possible source of impaired neurocognitive functioning and were analyzed in these patients. Methods Eighty patients (median age, 51 years) with glioma (WHO grade IV glioblastoma, n = 57; WHO grade III anaplastic astrocytoma, n = 13; WHO grade III anaplastic oligodendroglioma, n = 10) and ECOG performance score 0–1 underwent resting-state functional MRI (rs-fMRI) and neuropsychological testing at a median interval of 13 months (range, 1–114 months) after initiation of therapy. For evaluation of structural and metabolic changes after treatment, anatomical MRI and amino acid PET using O-(2-[18F]fluoroethyl)-L-tyrosine (FET) were simultaneously acquired to rs-fMRI on a hybrid MR/PET scanner. A cohort of 80 healthy subjects matched for gender, age, and educational status served as controls. Results The connectivity pattern within the DMN (12 nodes) of the glioma patients differed significantly from that of the healthy subjects but did not depend on age, tumor grade, time since treatment initiation, presence of residual/recurrent tumor, number of chemotherapy cycles received, or anticonvulsive medication. Small changes in the connectivity pattern were observed in patients who had more than one series of radiotherapy. In contrast, structural tissue changes located at or near the tumor site (including resection cavities, white matter lesions, edema, and tumor tissue) had a strong negative impact on the functional connectivity of the adjacent DMN nodes, resulting in a marked dependence of the connectivity pattern on tumor location. In the majority of neurocognitive domains, glioma patients performed significantly worse than healthy subjects. Correlation analysis revealed that reduced connectivity in the left temporal and parietal DMN nodes was associated with low performance in language processing and verbal working memory. Furthermore, connectivity of the left parietal DMN node also correlated with processing speed, executive function, and verbal as well as visual working memory. Overall DMN connectivity loss and cognitive decline were less pronounced in patients with higher education. Conclusion Personalized treatment strategies for malignant glioma patients should consider the left parietal and temporal DMN nodes as vulnerable regions concerning neurocognitive outcome.
