Abstract

Summary: Background: While vaccines have established utility against COVID-19, phase 3 efficacy studies have generally not comprehensively evaluated protection provided by previous infection or hybrid immunity (previous infection plus vaccination). Individual patient data from US government-supported harmonized vaccine trials provide an unprecedented sample population to address this issue. We characterized the protective efficacy of previous SARS-CoV-2 infection and hybrid immunity against COVID-19 early in the pandemic over three-to six-month follow-up and compared with vaccine-associated protection. Methods: In this post-hoc cross-protocol analysis of the Moderna, AstraZeneca, Janssen, and Novavax COVID-19 vaccine clinical trials, we allocated participants into four groups based on previous-infection status at enrolment and treatment: no previous infection/placebo; previous infection/placebo; no previous infection/vaccine; and previous infection/vaccine. The main outcome was RT-PCR-confirmed COVID-19 >7–15 days (per original protocols) after final study injection. We calculated crude and adjusted efficacy measures. Findings: Previous infection/placebo participants had a 92% decreased risk of future COVID-19 compared to no previous infection/placebo participants (overall hazard ratio [HR] ratio: 0.08; 95% CI: 0.05–0.13). Among single-dose Janssen participants, hybrid immunity conferred greater protection than vaccine alone (HR: 0.03; 95% CI: 0.01–0.10). Too few infections were observed to draw statistical inferences comparing hybrid immunity to vaccine alone for other trials. Vaccination, previous infection, and hybrid immunity all provided near-complete protection against severe disease. Interpretation: Previous infection, any hybrid immunity, and two-dose vaccination all provided substantial protection against symptomatic and severe COVID-19 through the early Delta period. Thus, as a surrogate for natural infection, vaccination remains the safest approach to protection. Funding: National Institutes of Health.

Abstract

Tumorigenic cell behaviors can be suppressed or enhanced by their physicochemical environment. As a first step toward developing materials that allow tumorigenic behaviors to be observed and manipulated, we cultured related MCF10 breast cell lines on fibers composed of the Drosophila protein Ultrabithorax (Ubx). These cell lines, originally derived from fibrocystic breast tissue, represent a continuum of tumorigenic behavior. Immortal but nontumorigenic MCF10A cells, as well as semitumorigenic MCF10AT cells, attached and spread on Ubx fibers. MCF10CA-1a cells, the most highly transformed line, secreted high concentrations of matrix metalloproteinases when cultured on Ubx materials, resulting in differences in cell attachment and cytoskeletal structure, and enabling invasive behavior. Because the mechanical and functional properties of Ubx fibers can be genetically manipulated, these materials provide a valuable tool for cancer research, allowing creation of diverse microenvironments that allow assessment of invasive, metastatic behavior.

The recombinant protein Ultrabithorax (Ubx), a Drosophila melanogaster Hox transcription factor, self-assembles into biocompatible materials in vitro that are remarkably extensible and strong. Here, we demonstrate that the strength of Ubx materials is due to intermolecular dityrosine bonds. Ubx materials auto-fluoresce blue, a characteristic of dityrosine, and bind dityrosine-specific antibodies. Monitoring the fluorescence of reduced Ubx fibers upon oxygen exposure reveals biphasic bond formation kinetics. Two dityrosine bonds in Ubx were identified by site-directed mutagenesis followed by measurements of fiber fluorescent intensity. One bond is located between the N-terminus and the homeodomain (Y4/Y296 or Y12/Y293), and another bond is formed by Y167 and Y240. Fiber fluorescence closely correlates with fiber strength, demonstrating that these bonds are intermolecular. To our knowledge, this is the first identification of specific residues that participate in dityrosine bonds in protein-based materials. The percentage of Ubx molecules harboring both bonds can be decreased or increased by mutagenesis, providing an additional mechanism to control the mechanical properties of Ubx materials. Although the in vivo function of the Drosophila melanogaster Hox protein Ultrabithorax (Ubx) is to regulate transcription, in vitro Ubx hierarchically self-assembles to form nanoscale to macroscale materials. The morphology, mechanical properties, and functionality (via protein chimeras) of Ubx materials are all easily engineered. Ubx materials are also compatible with cells in culture. These properties make Ubx attractive as a potential tissue engineering scaffold, but to be used as such they must be biocompatible and nonimmunogenic. In this study, we assess whether Ubx materials are suitable for in vivo applications. When implanted into mice, Ubx fibers attracted few immune cells to the implant area. Sera from mice implanted with Ubx contain little to no antibodies capable of recognizing Ubx. Furthermore, Ubx fibers cultured with macrophages in vitro did not lyse or activate the macrophages, as measured by TNF-α and NO secretion. Finally, Ubx fibers do not cause hemolysis when incubated with human red blood cells. The minimal effects observed are comparable with those induced by biomaterials used successfully in vivo. We conclude Ubx materials are biocompatible and nonimmunogenic.

Abstract

A key advantage of protein-based materials is the potential to directly incorporate novel functions via gene fusion to produce a single chimeric polypeptide capable of both self-assembly and the desired activity. However, facile production of functionalized protein materials is frequently hampered by the need to trigger materials assembly using conditions that will not irreversibly damage the functional protein. In contrast, the recombinant Drosophila melanogaster transcription factor Ultrabithorax (Ubx) rapidly self-assembles under mild, aqueous conditions to form highly extensible materials with a variety of morphologies. Here, it is demonstrated that materials composed of Ubx chimeras with Enhanced Green Fluorescent Protein (EGFP), mCherry, luciferase, or myoglobin display the functions of the appended proteins, indicating that these activities are neither impaired by the assembly process nor by confinement within the materials. Finally, methods are established that combine EGFP-Ubx and mCherry-Ubx monomers to self-assemble materials patterned on the microscale to macroscale. The self-adhesive properties of Ubx materials also permit manual construction and patterning of more complex forms. The ability to easily functionalize and pattern protein-based materials greatly expands their potential utility in a wide variety of applications.

Abstract

In Australia, newly initiated, publicly subsidised Home-Care Packages' designed to assist older people (65years of age) living in their own home must now be offered on a consumer-directed care' (CDC) basis by service providers. However, CDC models have largely developed in the absence of evidence on users' views and preferences. The aim of this study was to determine what features (attributes) of consumer-directed, home-based support services are important to older people and their informal carers to inform the design of a discrete choice experiment (DCE). Semi-structured, face-to-face interviews were conducted in December 2012-November 2013 with 17 older people receiving home-based support services and 10 informal carers from 5 providers located in South Australia and New South Wales. Salient service characteristics important to participants were determined using thematic and constant comparative analysis and formulated into attributes and attribute levels for presentation within a DCE. Initially, eight broad themes were identified: information and knowledge, choice and control, self-managed continuum, effective co-ordination, effective communication, responsiveness and flexibility, continuity and planning. Attributes were formulated for the DCE by combining overlapping themes such as effective communication and co-ordination, and the self-managed continuum and planning into single attributes. Six salient service features that characterise consumer preferences for the provision of home-based support service models were identified: choice of provider, choice of support worker, flexibility in care activities provided, contact with the service co-ordinator, managing the budget and saving unspent funds. Best practice indicates that qualitative research with individuals who represent the population of interest should guide attribute selection for a DCE and this is the first study to employ such methods in aged care service provision. Further development of services could incorporate methods of consumer engagement such as DCEs which facilitate the identification and quantification of users' views and preferences on alternative models of delivery. Abstract

Protein sequences can be easily customized using standard molecular biology techniques. Thus, the use of protein monomers for materials assembly provides an opportunity for facile tuning of structural, mechanical, and functional properties. We discovered that the Drosophila transcription factor Ultrabithorax (Ubx) self-assembles into ordered, robust, nanoscale and macroscale materials that are biocompatible and non-immunogenic. Further, Ubx fibers are remarkably strong and extensible, properties which can be genetically tuned by altering the number of intermolecular dityrosine covalent bonds. In addition, a surprising array of full-length proteins can be successfully incorporated into Ubx materials by fusing the gene for the additional protein to the ubx gene, so that they are transcribed and translated as a single polypeptide. We have successfully attached proteins that vary greatly in size, quaternary structure, charge, and solubility, without impacting Ubx self-assembly. The fused proteins retain their functions because Ubx rapidly forms materials in mild aqueous buffers, in contrast to most materials-forming proteins. As proof-of-principle, we created VEGF-Ubx fibers that stimulated and sustained VEGF signaling in cultured cells. In vivo, these fibers both induced and patterned vascular formation. Finally, Ubx materials, like Ubx monomers, can bind DNA in a sequence-specific manner. DNA sequences with multiple binding sites were retained on the fibers for over one week. In summary, these versatile protein materials offer multiple avenues for long-term functionalization, without creating toxic by-products that are a common problem when chemically crosslinking materials.

Abstract

Incorporation of nanoparticles during the hierarchical self-assembly of protein-based materials can impart function to the resulting composite materials. Herein we demonstrate that the structure and nanoparticle distribution of composite fibers are sensitive to the method of nanoparticle addition and the physicochemical properties of both the nanoparticle and the protein. Our model system consists of a recombinant enhanced green fluorescent protein-Ultrabithorax (EGFP-Ubx) fusion protein and luminescent CdSe-ZnS core-shell quantum dots (QDs), allowing us to optically assess the distribution of both the protein and nanoparticle components within the composite material. Although QDs favorably interact with EGFP-Ubx monomers, the relatively rough surface morphology of composite fibers suggests EGFP-Ubx-QD conjugates impact self-assembly. Indeed, QDs templated onto EGFP-Ubx film post-self-assembly can be subsequently drawn into smooth composite fibers. Additionally, the QD surface charge impacts QD distribution within the composite material, indicating that surface charge plays an important role in self-assembly. QDs with either positively or negatively charged coatings significantly enhance fiber extensibility. Conversely, QDs coated with hydrophobic moieties and suspended in toluene produce composite fibers with a heterogeneous distribution of QDs and severely altered fiber morphology, indicating that toluene severely disrupts Ubx self-assembly. Understanding factors that impact the protein-nanoparticle interaction enables manipulation of the structure and mechanical properties of composite materials. Since proteins interact with nanoparticle surface coatings, these results should be applicable to other types of nanoparticles with similar chemical groups on the surface.

Abstract

This paper describes a partnership between researchers and policy actors that was developed within a short timeframe to produce a rapid appraisal case study of a government policy initiative – South Australia’s Social Inclusion Initiative – for the Social Exclusion Knowledge Network of the international Commission on Social Determinants of Health. The paper does not focus on the case study findings or content, but rather on the researcher–policy actor partnership that developed in the process of producing the case study and its report. The paper is set against the broader literature on researcher–policy collaboration and is written to share lessons that may help others quickly establish or improve researcher–policy partnerships. It sets out six key elements for success in a framework for partnership that can meet policy rather than academic timeframes and which can effectively co-produce knowledge that meets both research and policy objectives.

Abstract

Despite decades of concern about reducing health inequity, the Commission on the Social Determinants of Health (CSDH) painted a picture of persistent and, in some cases, increasing health inequity. It also made a call for increased evaluation of interventions that might reduce inequities. This paper describes such an intervention-the Social Inclusion Initiative (SII) of the South Australian Government-that was documented for the Social Exclusion Knowledge Network of the CSDH. This initiative is designed to increase social inclusion by addressing key determinants of health inequity-in the study period these were education, homelessness and drug use. Our paper examines evidence from a rapid appraisal to determine whether a social inclusion initiative is a useful aspect of government action to reduce health inequity. It describes achievements in each specific area and the ways they can be expected to affect health equity. Our study highlighted four factors central to the successes achieved by the SII. These were the independent authority and influence of the leadership of the SII, the whole of government approach supported by an overarching strategic plan which sets clear goals for government and the clear and unambiguous support from the highest level of government. We conclude that a social inclusion approach can be valuable in the quest to reduce inequities and that further research on innovative social policy approaches is required to examine their likely impact on health equity.

Abstract

Chronic oxidative injury by allylamine (AAM) induces proliferative vascular smooth muscle cell (vSMC) phenotypes in the rat aorta similar to those seen in rodent and human atherosclerotic lesions. The proliferative advantage of AAM vSMC compared to control cells is maintained with serial passage of the cells and the advantage is nullified when AAM cells are seeded on a collagen substrate. In this study, we evaluate the potential role of cyclin dependent kinase inhibitors, p27 and p21, and mitogen activated protein (MAP) kinases, ERK1/2, in mediating the proliferative advantage of AAM stressed vSMC over control cells on plastic or collagen substrates. p27 levels in randomly cycling cells were comparable in both cell types irrespective of the substrate. Abstract

Incorporation of nanoparticles during the hierarchicalself-assemblyof protein-based materials can impart function to the resulting compositematerials. Herein we demonstrate that the structure and nanoparticledistribution of composite fibers are sensitive to the method of nanoparticleaddition and the physicochemical properties of both the nanoparticleand the protein. Our model system consists of a recombinant enhancedgreen fluorescent protein–Ultrabithorax (EGFP-Ubx) fusion proteinand luminescent CdSe–ZnS core–shell quantum dots (QDs),allowing us to optically assess the distribution of both the proteinand nanoparticle components within the composite material. AlthoughQDs favorably interact with EGFP-Ubx monomers, the relatively roughsurface morphology of composite fibers suggests EGFP-Ubx-QD conjugatesimpact self-assembly. Indeed, QDs templated onto EGFP-Ubx film post-self-assemblycan be subsequently drawn into smooth composite fibers. Additionally,the QD surface charge impacts QD distribution within the compositematerial, indicating that surface charge plays an important role inself-assembly. QDs with either positively or negatively charged coatingssignificantly enhance fiber extensibility. Conversely, QDs coatedwith hydrophobic moieties and suspended in toluene produce compositefibers with a heterogeneous distribution of QDs and severely alteredfiber morphology, indicating that toluene severely disrupts Ubx self-assembly.Understanding factors that impact the protein–nanoparticleinteraction enables manipulation of the structure and mechanical propertiesof composite materials. Abstract

A presacral “mass” was seen on the lateral plain film of the abdomen in 5 of 9 recent cases of meconium plug syndrome. Contrast-medium studies revealed the intraluminal meconium plug was forming the “mass” outlined anteriorly by rectal gas. This condition should be considered in the differential diagnosis of a presacral mass density in the neonate.

Abstract

An American physician-traveler to East Africa presented with manifestations of cerebral malaria and was treated with intravenous quinidine for chloroquine-resistant falciparum malaria. He later relapsed with Plasmodium ovale infection, despite previous primaquine therapy. Treatment of chloroquine-resistant malaria is discussed. Abstract

Proteins that naturally self-assemble to form materials are often challenging to produce as recombinant proteins, and require harsh conditions to instigate assembly in vitro, limiting their ability to be engineered for specific applications. Surprisingly, the Drosophila melanogaster Hox protein Ultrabithorax (Ubx) readily self-assembles in vitro in mild buffers, even though this protein does not form large aggregates as part of its native function. Ubx hierarchically associates to form materials that are ordered on the nanoscale to macroscale. The mechanical properties of Ubx fibers are tunable, and fibers can be constructed with a similar breaking stress and strain as natural elastin. Because Ubx self-assembles within hours in a mild buffer, other proteins can be incorporated into Ubx materials via gene fusion without compromising the ability of Ubx to assemble, or the functionality of the appended protein. A variety of proteins have successfully been incorporated into Ubx materials, ranging in structure, size, and charge. These functions can be easily patterned within the materials. This strategy has been used to control the behavior of cells in culture.
