In this context, a study was undertaken to understand the perceptions of patients and their caregivers, healthcare providers, health system decision makers, and vendors associated with a remote monitoring and tracking solution aimed at enhancing the outcomes and experiences of chronic kidney disease (CKD) patients receiving PD at home. Methods: A qualitative design using semi-structured interviews with 25 stakeholders was used in this study. Narrative data were analyzed by a thematic analysis approach. Results: The following two themes emerged from the data: (1) leveraging data to monitor and intervene to keep patients safe and (2) increasing efficiencies and having control over supplies. Discussion: Our study findings elucidated the ability of patients (and in some cases, caregivers) to monitor and trend their data and order and track directly on-line their dialysis supplies were key to their active participation in managing their CKD and keeping them safe at home. Their active participation and functionality of the virtual care solution also led to enhanced efficiencies (eg, process faster, easier, convenient, time savings) for both patients and healthcare providers. Conclusion: The virtual care solution showed promising signs of a patient-centric approach and may serve as a blueprint for other virtual care solutions for chronic disease management. Abstract

The SuperCDMS SNOLAB dark matter search experiment aims to be sensitive to energy depositions down to O(1 eV). This imposes requirements on the resolution, signal efficiency, and noise rejection of the trigger system. To accomplish this, the SuperCDMS level-1 trigger system is implemented in an FPGA on a custom PCB. A time-domain optimal filter algorithm realized as a finite impulse response filter provides a baseline resolution of 0.38 times the standard deviation of the noise, $\sigma_n$, and a 99.9% trigger efficiency for signal amplitudes of 1.1 $\sigma_n$ in typical noise conditions. Embedded in a modular architecture, flexible trigger logic enables reliable triggering and vetoing in a dead-time-free manner for a variety of purposes and run conditions. 3. The impacts of state intervention on corporation policy with particular reference to housing and town planning 1890-1939

Abstract

At the end of the nineteenth century, the most pressing urban problem facing the authorities was that of public health. In 1890 the government introduced legislation to encourage local authorities to clear insanitary housing and re-house the displaced tenants. Many city councils responded, but in the vast majority of cases, these early experiments foundered in the face of stiff opposition from ratepayers' associations. In Aberdeen, a limited number of workmen's dwellings were built, but the Council soon abandoned the policy and left house building to private enterprise. Increased government intervention in all aspects of public life during the First World War brought home to local authorities that the only way forward was to enter into partnership with central government. For the next twenty years successive governments sought, by subsidies and grants, to encourage councils to build working class housing. Some legislation was successful, some was less so, but as the nineteen-thirties progressed, the most congested areas were cleared and the inhabitants rehoused. In 1939, the Department of Health for Scotland was confident that Scotland's housing problems would be solved by 1942. Unfortunately, war again intervened and building was halted for six years; when the war ended in 1945, the problem was as bad as ever. In the course of fifty years, municipal government had metamorphosed from a purely local form of administration accountable only to its ratepayers into a junior branch of central government. Change was inevitable, for no local authority had the economic muscle to replace its housing stock or plan for future development without state assistance. At the same time, democracy itself had suffered, the day of independent representation in council chambers had all but ended, with councillors assuming party labels and following orders from party headquarters in London.

Item does not contain fulltext, BACKGROUND: The bacille Calmette-Guérin (BCG) vaccine has immunomodulatory "off-target" effects that have been hypothesized to protect against coronavirus disease 2019 (Covid-19). METHODS: In this international, double-blind, placebo-controlled trial, we randomly assigned health care workers to receive the BCG-Denmark vaccine or saline placebo and followed them for 12 months. Symptomatic Covid-19 and severe Covid-19, the primary outcomes, were assessed at 6 months; the primary analyses involved the modified intention-to-treat population, which was restricted to participants with a negative test for severe acute respiratory syndrome coronavirus 2 at baseline. RESULTS: A total of 3988 participants underwent randomization; recruitment ceased before the planned sample size was reached owing to the availability of Covid-19 vaccines. The modified intention-to-treat population included 84.9% of the participants who underwent randomization: 1703 in the BCG group and 1683 in the placebo group. The estimated risk of symptomatic Covid-19 by 6 months was 14.7% in the BCG group and 12.3% in the placebo group (risk difference, 2.4 percentage points; 95% confidence interval [CI], -0.7 to 5.5; P = 0.13). The risk of severe Covid-19 by 6 months was 7.6% in the BCG group and 6.5% in the placebo group (risk difference, 1.1 percentage points; 95% CI, -1.2 to 3.5; P = 0.34); the majority of participants who met the trial definition of severe Covid-19 were not hospitalized but were unable to work for at least 3 consecutive days. In supplementary and sensitivity analyses that used less conservative censoring rules, the risk differences were similar but the confidence intervals were narrower. There were five hospitalizations due to Covid-19 in each group (including one death in the placebo group). The hazard ratio for any Covid-19 episode in the BCG group as compared with the placebo group was 1.23 (95% CI, 0.96 to 1.59). No safety concerns were identified. CONCLUSIONS: Vaccination with BC

Abstract

Objective: Hepatocellular carcinoma (HCC) is increasingly associated with non-alcoholic steatohepatitis (NASH). HCC immunotherapy offers great promise; however, recent data suggests NASH-HCC may be less sensitive to conventional immune checkpoint inhibition (ICI). We hypothesised that targeting neutrophils using a CXCR2 small molecule inhibitor may sensitise NASH-HCC to ICI therapy. Design Neutrophil infiltration was characterised in human HCC and mouse models of HCC. Late-stage intervention with anti-PD1 and/or a CXCR2 inhibitor was performed in murine models of NASH-HCC. The tumour immune microenvironment was characterised by imaging mass cytometry, RNA-seq and flow cytometry. Results: neutrophils expressing CXCR2, a receptor crucial to neutrophil recruitment in acute-injury, are highly represented in human NASH-HCC. In models of NASH-HCC lacking response to ICI, the combination of a CXCR2 antagonist with anti-PD1 suppressed tumour burden and extended survival. Combination therapy increased intratumoural XCR1(+) dendritic cell activation and CD8(+) T cell numbers which are associated with anti-tumoural immunity, this was confirmed by loss of therapeutic effect on genetic impairment of myeloid cell recruitment, neutralisation of the XCR1-ligand XCL1 or depletion of CD8(+) T cells. Therapeutic benefit was accompanied by an unexpected increase in tumour-associated neutrophils (TANs) which switched from a protumour to anti-tumour progenitor-like neutrophil phenotype. Reprogrammed TANs were found in direct contact with CD8(+) T cells in clusters that were enriched for the cytotoxic anti-tumoural protease granzyme B. Neutrophil reprogramming was not observed in the circulation indicative of the combination therapy selectively influencing TANs. 11. Changes in microbial communities during seawater pre-treatment within a desalination plant

Abstract

We analysed prokaryotic and eukaryotic communities across the seawater pre-treatment system of Penneshaw (Kangaroo Island, South Australia) desalination plant, using 16S and 18S rRNA gene sequencing. The richness of operational taxonomic units increased downstream of the pre-treatmentsystem (reverse osmosis feedwater) compared to raw seawater for Archaea, while it decreased for bacteria and protists. Overall, the reverse osmosis feedwater was found to be enriched in ammonia-oxidising bacteria and Archaea compared to raw seawater, and also contained greater proportions of taxa typically observed in aquatic biofilms and/or within other water treatment systems. Although the microbial load was reduced by the pre-treatment system, the increase in proportion of biofilm-associated microbes suggests the presence of active microbial communities within multimedia filters and other parts of the pre-treatment system that might increase biofouling risks.

Abstract

INTRODUCTION: BCG vaccination modulates immune responses to unrelated pathogens. This off-target effect could reduce the impact of emerging pathogens. As a readily available, inexpensive intervention that has a well-established safety profile, BCG is a good candidate for protecting healthcare workers (HCWs) and other vulnerable groups against COVID-19. METHODS AND ANALYSIS: This international multicentre phase III randomised controlled trial aims to determine if BCG vaccination reduces the incidence of symptomatic and severe COVID-19 at 6 months (co-primary outcomes) compared with no BCG vaccination. We plan to randomise 10 078 HCWs from Australia, The Netherlands, Spain, the UK and Brazil in a 1:1 ratio to BCG vaccination or no BCG (control group). The participants will be followed for 1 year with questionnaires and collection of blood samples. For any episode of illness, clinical details will be collected daily, and the participant will be tested for SARS-CoV-2 infection. The secondary objectives are to determine if BCG vaccination reduces the rate, incidence, and severity of any febrile or respiratory illness (including SARS-CoV-2), as well as work absenteeism. The safety of BCG vaccination in HCWs will also be evaluated. Immunological analyses will assess changes in the immune system following vaccination, and identify factors associated with susceptibility to or protection against SARS-CoV-2 and other infections. ETHICS AND DISSEMINATION: Ethical and governance approval will be obtained from participating sites. Results will be published in peer-reviewed open-access journals. The final cleaned and locked database will be deposited in a data sharing repository archiving system. TRIAL REGISTRATION: ClinicalTrials.gov NCT04327206.

14. Real-time data reorganizer for the D0 central fiber tracker trigger system at Fermilab

Abstract

A custom digital data mixer system has been designed to reorganize, in real time, the data produced by the Fermilab D0 Scintillating Fiber Detector. The data are used for the Level 1 and Level 2 trigger decisions. The mixer system receives the data from the front-end digitization electronics over 320 low voltage differential signaling (LVDS) links running at 371 MHz. The input data are de-serialized down to 53 MHz by the LVDS receivers, clock/frame re-synchronized and multiplexed in field programmable gate arrays (FPGAs). The data are then re-serialized at 371 MHz by LVDS transmitters over 320 LVDS output links and sent to the electronics responsible for Level 1 and Level 2 trigger decisions. The data mixer system processes 311 gigabits per second of data with an input to output delay of 200 ns. 22. Design and test of a prototype DART system

Abstract

A 1.2-meter prototype Dual Anamorphic Reflector Telescope (DART) system has been build and tested. The key design feature of the telescope is a pair of membrane mirrors stretched to single curvature parabolic cylindrical sections. The parabolic figure of the mirrors is controlled by a pair of edge rails at two opposing ends of the membrane. The flexible edge rails are adjusted to parabolic to very high accuracy and can potentially be easily refigured on-orbit. The prototype telescope is lightweight and has demonstrated excellent optical performance for the farIR. The design is readily scalable to larger apertures and for operation at shorter wavelengths. Design and test results are discussed.

23. To what extent can behaviour change techniques be identified within an adaptable implementation package for primary care? A prospective directed content analysis

Abstract

BACKGROUND: Interpreting evaluations of complex interventions can be difficult without sufficient description of key intervention content. We aimed to develop an implementation package for primary care which could be delivered using typically available resources and could be adapted to target determinants of behaviour for each of four quality indicators: diabetes control, blood pressure control, anticoagulation for atrial fibrillation and risky prescribing. We describe the development and prospective verification of behaviour change techniques (BCTs) embedded within the adaptable implementation packages. METHODS: We used an over-lapping multi-staged process. We identified evidence-based, candidate delivery mechanisms-mainly audit and feedback, educational outreach and computerised prompts and reminders. We drew upon interviews with primary care professionals using the Theoretical Domains Framework to explore likely determinants of adherence to quality indicators. We linked determinants to candidate BCTs. With input from stakeholder panels, we prioritised likely determinants and intervention content prior to piloting the implementation packages. Our content analysis assessed the extent to which embedded BCTs could be identified within the packages and compared them across the delivery mechanisms and four quality indicators. RESULTS: Each implementation package included at least 27 out of 30 potentially applicable BCTs representing 15 of 16 BCT categories. Whilst 23 BCTs were shared across all four implementation packages (e.g. BCTs relating to feedback and comparing behaviour), some BCTs were unique to certain delivery mechanisms (e.g. 'graded tasks' and 'problem solving' for educational outreach). BCTs addressing the determinants 'environmental context' and 'social and professional roles' (e.g. 'restructuring the social and 'physical environment' and 'adding objects to the environment') were indicator specific. We found it challenging to operationalise BCTs targeting 'environmental context', 'social influences' and 'social and professional roles' within our chosen delivery mechanisms. CONCLUSION: We have demonstrated a transparent process for selecting, operationalising and verifying the BCT content in implementation packages adapted to target four quality indicators in primary care. There was considerable overlap in BCTs identified across the four indicators suggesting core BCTs can be embedded and verified within delivery mechanisms commonly available to primary care. Whilst feedback reports can include a wide range of BCTs, computerised prompts can deliver BCTs at the time of decision making, and educational outreach can allow for flexibility and individual tailoring in delivery.

The inflammatory response is normally limited by mechanisms regulating its resolution. In the absence of resolution, inflammatory pathologies can emerge, resulting in substantial morbidity and mortality. We have been studying the D6 chemokine scavenging receptor, which played an indispensable role in the resolution phase of inflammatory responses and does so by facilitating removal of inflammatory CC chemokines. In D6-deficient mice, otherwise innocuous cutaneous inflammatory stimuli induce a grossly exaggerated inflammatory response that bears many similarities to human psoriasis. In the present study, we have used transcriptomic approaches to define the molecular make up of this response. The data presented highlight potential roles for a number of cytokines in initiating and maintaining the psoriasis-like pathology. Most compellingly, we provide data indicating a key role for the type I interferon pathway in the emergence of this pathology. Neutralizing antibodies to type I interferons are able to ameliorate the psoriasis-like pathology, confirming a role in its development. Comparison of transcriptional data generated from this mouse model with equivalent data obtained from human psoriasis further demonstrates the strong similarities between the experimental and clinical systems. As such, the transcriptional data obtained in this preclinical model provide insights into the cytokine network active in exaggerated inflammatory responses and offer an excellent tool to evaluate the efficacy of compounds designed to therapeutically interfere with inflammatory processes.

Abstract

Mucin deposition in the skin (scleromyxedema) is a rare disorder with a historically confusing nomenclature. The etiology and pathogenesis of the disease is poorly understood. The precise extent of potential systemic involvement is uncertain but the prevailing hypothesis is that scleromyxedema is a disorder of skin fibroblasts without internal organ involvement. Two patients with scleromyxedema skin lesions are described in whom an erosive arthropathy developed in joints contiguous to clinically involved skin. Because of the absence of other causes of destructive arthritis, it is postulated that the associated skin disease resulted in infiltration of adjacent synovium with subsequent erosions.
