Abstract

ABSTRACTBackground The breadth of protection of National Immunisation Programmes (NIPs) across Europe varies, however, this has not been assessed within published literature. Therefore, a framework was developed to assess the comprehensiveness of pediatric NIPs in Europe. This study aimed to validate and further develop criteria used to cluster countries into three tiers.Research design and methods Independent Europe-based experts (n = 23) in the field of pediatric vaccination were invited to participate in a double-blinded modified Delphi panel, with two online survey rounds and a virtual consensus meeting. Consensus was defined as ≥ 80% of experts rating their agreement/disagreement on a 9-point Likert scale.Results The number of preventable diseases covered by an NIP, simplification of the vaccination calendar, strengthened protection by increasing serotype, degree of funding and epidemiological factors were considered key concepts for consideration of the comprehensiveness of pediatric NIPs in Europe. Experts highlighted that the framework should be extended to include adolescent vaccines and populations up to 18 years of age. Consensus regarding further amendments to the framework was also reached.Conclusions This Delphi panel validated a framework to assess the comprehensiveness of European NIPs. The framework can be used to facilitate discussions to help countries improve and expand the breadth of protection provided by their NIP.

Abstract

Aim: The six-minute walk test (6MWT) is a common measure of functional capacity in patients with heart failure (HF). Primary clinical study end points in cardiomyopathy (CM) trials, including transthyretin-mediated amyloidosis with CM (ATTR-CM), are often limited to hospitalization and mortality. Objective: To investigate the relationship between the 6MWT and hospitalization or mortality in CM, including ATTR-CM. Method: A PRISMA-guided systematic literature review was conducted using search terms for CM, 6MWT, hospitalization and mortality. Results: Forty-one studies were identified that reported 6MWT data and hospitalization or mortality data for patients with CM. The data suggest that a greater 6MWT distance is associated with a reduced risk of hospitalization or mortality in CM. Conclusion: The 6MWT is an accepted alternative end point in CM trials, including ATTR-CM.

Abstract

OBJECTIVES:. To compare the performance of Sequential Organ Failure Assessment, systemic inflammatory response syndrome, Red Flag Sepsis, and National Institute of Clinical Excellence sepsis risk stratification tools in the identification of patients at greatest risk of mortality from sepsis in nonintensive care environments. DESIGN:. Secondary analysis of three annual 24-hour point-prevalence study periods. SETTING:. The general wards and emergency departments of 14 acute hospitals across Wales. Studies were conducted on the third Wednesday of October in 2017, 2018, and 2019. PATIENTS:. We screened all patients presenting to the emergency department and on the general wards. MEASUREMENTS AND MAIN RESULTS:. We recruited 1,271 patients, of which 724 (56.9%) had systemic inflammatory response syndrome greater than or equal to 2, 679 (53.4%) had Sequential Organ Failure Assessment greater than or equal to 2, and 977 (76.9%) had Red Flag Sepsis. When stratified according to National Institute of Clinical Excellence guidelines, 450 patients (35.4%) were in the “High risk” category in comparison with 665 (52.3%) in “Moderate to High risk” and 156 (12.3%) in “Low risk” category. In a planned sensitivity analysis, we found that none of the tools accurately predicted mortality at 90 days, and Sequential Organ Failure Assessment and National Institute of Clinical Excellence tools showed only moderate discriminatory power for mortality at 7 and 14 days. Furthermore, we could not find any significant correlation with any of the tools at any of the mortality time points. CONCLUSIONS:. Our data suggest that the sepsis risk stratification tools currently utilized in emergency departments and on the general wards do not predict mortality adequately. This is illustrated by the disparity in mortality risk of the populations captured by each instrument, as well as the weak concordance between them. We propose that future studies on the development of sepsis identification tools should focus on identifying predicator values of both the short- and long-term outcomes of sepsis.

Abstract

PurposeAs remote working becomes increasingly popular, remote working could unlock new ways of working through digitisation. However, the construction sector has been slow to adopt digitisation in its processes, making digitisation difficult to assess whether this affordance may be well received and the current capabilities of digitisation to achieve this effectively. The purpose of this paper is to investigate the interest in remote working amongst construction sector personnel and to examine the factors affecting remote working through digitisation affordances.Design/methodology/approachBased on a case study of one of the largest contractor firms in the United Kingdom, an online questionnaire survey was used to collect responses from 125 construction professionals. Statistical package for the social sciences (SPSS) was used to do basic statistical analysis on the results.FindingsThe findings show that there is a general appetite for remote working on a flexible basis where a mix of “on-site” and “off-site” arrangement was deemed practical. This could potentially unlock significant time and cost savings as well as productivity gains. The main factors affecting remote working were the availability of interconnected systems allowing efficient communication and digital infrastructure that enable automated processes.Research limitations/implicationsThe research is limited to a large contractor company and may not be appropriate for small and medium-sized companies. The findings may benefit organisations to evaluate the practical needs of ensuring effective remote working in the construction industry and unlocking efficiencies.Originality/valueThe paper adds value to understanding the affordances and constraints of digitisation for remote working from the perspective of construction professionals.

The study, conducted at ten Veterans Affairs (VA) sites, compares two team-based approaches to improve pain management and reduce potential harms of opioid therapy. The panel shares ten best practices for sustaining a successful engagement partnership.

Abstract

The differentiation of therapeutic monoclonal antibodies in an increasingly competitive landscape requires optimization of clinical efficacy combined with increased patient convenience. We describe here the generation of MEDI5117, a human anti-interleukin (IL)-6 antibody generated by variable domain engineering, to achieve subpicomolar affinity for IL-6, combined with Fc (fragment crystallizable) engineering to enhance pharmacokinetic half-life. MEDI5117 was shown to be highly potent in disease-relevant cellular assays. The pharmacokinetics of MEDI5117 were evaluated and compared to those of its progenitor, CAT6001, in a single-dose study in cynomolgus monkeys. The antibodies were administered, either subcutaneously or intravenously, as a single dose of 5 mg/kg. The half-life of MEDI5117 was extended by approximately 3-fold, and clearance was reduced by approximately 4-fold when compared to CAT6001. MEDI5117 therefore represents a potential 'next-generation' antibody; future studies are planned to determine the potential for affinity-driven efficacy and/or less frequent administration.

Abstract

BACKGROUND AND PURPOSE Interleukin-15 (IL-15) is important in the activation and proliferation of lymphocytic cell populations and is implicated in inflammatory disease. We report the characterization of a novel monoclonal antibody DISC0280 which is specific for human IL-15. EXPERIMENTAL APPROACH DISC0280 was characterized in a direct binding assay of IL-15 with IL-15 receptor α (IL-15Rα) and by its ability to alter IL-15 mediated proliferation of a range of cell lines (cytotoxic T lymphocyte line-2, M-07e, KIT225). A pharmacodynamic model injecting male C57/BL6 mice with IL-15 or IL-15/IL-15Rα, with or without DISC0280, and assessing changes in lymphocytic cell populations and serum cytokines was utilized. KEY RESULTS DISC0280 inhibited the binding of IL-15 to IL-15Rα and also potently inhibits IL-15 dependent proliferation of cells expressing IL-15Rα, shared interleukin 2/ interleukin 15 receptor β chain (IL-15Rβ) and common gamma chain (γc). DISC0280 also inhibited the IL-15 dependent proliferation of M-07e cells that only express IL-15Rβ/γc subunits. Human IL-15 injected into mice caused an increase in NK1.1+ and CD3+ cells in the spleen and peripheral blood and these effects were unexpectedly potentiated by giving DISC0280 with human IL-15. This increase in cells caused by DISC0280/IL-15 co-administration was greater than that observed when IL-15 was administered complexed with soluble IL-15Rα. CONCLUSIONS AND IMPLICATIONS The ability of DISC0280 to bind to the IL-15Rα-binding site on IL-15 allows trans-presentation of IL-15 by DISC0280 in vivo, similar to the trans-presentation by soluble IL-15Rα. DISC0280 may be therefore suitable as a clinical substitute for IL-15.

N,N′-Diphenylamidines, PhNC(R)NHPh (R=H, Me, Et, Ph), and 1,3-diaryltriazenes, ArNNNHAr (Ar=p-C6H4X, X=H, Cl, Me, OMe) react with ruthenium(II) complexes, [RuCl2(C7H8)]n, [RuCl2(CO)2]n and [RuCl2(Me2SO)4] in the presence of a base (NEt3 or Na2CO3) to afford a selection of bis(chelate) products [Ru{PhNC(R)NPh}2L2] and [Ru(ArNNNAr)2L2] (L=CO, Me2SO; L2=C7H8). In contrast one particular combination — [RuCl2(Me2SO)4]/PhNC(H)NHPh/NEt3 — in refluxing dimethylformamide yields a novel amidine fragmentation product [RuCl(NH2Ph){PhNC(H)NPh}(Me2SO)2] which has been characterised by X-ray diffraction methods.

Abstract

The aim of the study was to determine the differences in hand-grip strength and strength-related anthropometric parameters between the highest-performance international paddlers (HILP) and other international paddlers (ILP) in canoe slalom. In male competitors (n=48; 23 canoeists and 25 kayakers) who took part in the 2018 European championship event in canoe slalom in Prague, Czech Republic representing 15 European countries were measured hand-grip strength and strength-related anthropometric parameters two days prior the event. Differences between the group of HILP (medallists from Olympic Games, world championship and European championship in the previous three years and European championship finalists at the same time) and ILP were found. HILP have significantly higher forearm, arm and chest girths than ILP. Moderate or high practical differences and margin statistic differences were found also for hand-grip strength, body weight, body mass index and age. The consistently high-performing paddlers are heavier, have more muscular upper body and are stronger than the other group, while keeping their lower body lean. Therefore, regular upper-body strength training to all canoe slalom paddlers is important but caution is required because body weight over 80 kg may be a limiting factor. [ABSTRACT FROM AUTHOR]
